We have ascertained the extent to which risk factors for HIV infection may escape detection by standard history-taking procedures in an antenatal clinic. This study was based on 1264 women from a multi-ethnic population in an inner London health district (City and Hackney). All had agreed to undergo attributable HIV testing and a detailed personal interview. Thirty-nine per cent (494 of 1264 women) reported risk factors contributed personally or by a partner. Most of these risk factors had not been earlier disclosed by routine history taking. In most cases the risk was residence and risk activity in a World Health Organization (WHO) Pattern 2 country. [HIV spread WHO categories: Pattern 1—principally homosexual/bisexual males and i.v. drug use (areas=North America, Western Europe, Australasia, parts of South America) with male to female ratio 10/1; Pattern 2—Heterosexual (areas=Sub Saharan Africa, Caribbean and part South America) with male to female 1/1.] Thirty-one subjects (2.4%) were aware that their partners had participated in bisexual activity. Only six subjects perceived themselves at risk through their own or partner's drug injecting activity. The frequency of risk factors was substantially greater than that ascertained by the routine history. The findings highlight the potential risk of heterosexual spread resulting from travel to or residence in high prevalence territories. The contribution by male partners is significant and is particularly difficult to detect during a routine interview. These data support the recommendation that voluntary HIV serum testing should be universal rather than a selective offer based on risk factors determined at a routine history.

PURPOSE While higher educational level (HEL) is associated with generally better health status, and educational attainment is a well-known social determinant of health, HEL has been associated with an increased breast cancer risk (BCR). Likely this paradox is the result of other mediating factors correlated with education such as parity. In Puerto Rico, there have been important changes in reproductive breast cancer risk factorsin particular, the fertility rate has dropped markedly in the last decades. We explored the possible mediation of parity and other risk factors on the association between education attainment and BCR among Caribbean Hispanic women with considerable variation in education. METHOD The Atabey study was a population-based case-control study (315 cases and 348 controls) in the San Juan Puerto Rico metropolitan area. Unconditional logistic regression models were used to assess the association between education (>12 yrs. vs. ≤12 yrs. as reference) and BCR adjusting independently for age and each of the following: BMI, benign breast disease, family history of breast cancer, age at menarche, age at first birth, age at last birth, number of births, breastfeeding, menopausal status, and age at menopause. Mediation analysis was used to explore if including parity (0, 1, ≥2) in the model changes the association between education attainment and BCR. RESULTS A little more than half of both cases (67.62%) and controls (56.03%) had completed more than high school. The association between education level and BCR remained statistically significant after adjusting by age and each risk factor separately (e.g. OR adjusted by age alone: 1.84 [95% CI: 1.33, 2.56] OR adjusted by age and age at first birth: 1.50 [95% CI: 1.03, 2.19]OR adjusted by age and parity: 1.56 [95% CI: 1.10, 2.21]). Parity appears to mediate this association (e.g., OR graduate studies vs <12 yrs. as reference: 3.74 [95% CI: 2.02, 6.94] direct effect: 1.31 [95% CI: 1.13, 1.51] indirect effect: 1.04 [95% CI: 0.99, 1.11]. CONCLUSION These findings suggest that parity could mediate the association between education and BCR. Further analyses are needed to understand the causal web behind the association between education and breast cancer among Caribbean Hispanic women. Abstract Adult T cell Leukemia/Lymphoma (ATLL) is a rare manifestation of Human T-Lymphotropic Virus type 1 (HTLV-1) which is endemic in Japan, the Caribbean, and regions of Africa and Latin America. Endemic regions have also been identified in the US, primarily where immigrants of endemic countries reside. The NYC metropolitan area has the greatest number of Caribbean-born immigrants to the US which has formed the basis for this retrospective review. A diagnosis of ATLL was made according to the following criteria: a clinical history consistent with ATLL; a positive HTLV-1 antibody by ELISA and Western Blot, or evidence of HTLV-1 proviral integration by PCR; and histological findings compatible with ATLL. A total of 89 patients were identified at 3 institutions in NYC from 8/92 to 5/07. There were 37 men and 52 women with a median age of 50y (range 22 to 82y). All but 6 patients had immigrated to the US from the Caribbean, Latin America or Africa, and the majority were from Jamaica (25.8%) and the Dominican Republic (19.1%). The acute subtype predominated (68.5%), followed by the lymphomatous (20.2%), chronic (6.8%) and smoldering (4.5%) subtypes. The most common presenting symptoms were lymphadenopathy (80.9%) and skin rash (39.3%) and the median ECOG performance status was 3. WBC counts ranged from 3.0 to 334.0 x 109/l (median of 12.5 X 109/l). Hypercalcemia was seen in 71.9% of patients (median calcium level of 13.5 mg/dl, range 9.8 to 27.0 mg/dl). Twenty-eight patients (31.5%) had CNS involvement during their course. The median International Prognostic Index (IPI) was 4. Most patients received a combination-alkylator based chemotherapy regimen in the frontline setting (ex: CHOP) (61.3%), with an overall response rate (ORR) to the frontline treatment of 58.2%. Twenty patients (20.2%) received AZT and IFN at sometime during their course with an ORR of 25%. Twelve patients (13.5%) received a biologically based therapy at sometime during their course, with only two patients achieving a partial response (to alemtuzamab and denileukin diftitox). Despite initial responses to therapy, the median overall survival for all subtypes was 6 mos (range 0.5 to 78.5 mos). Median survival for specific subtypes was noted to be: 4 mos for the acute subtype (range 0.5 to 78.5 mos); 9 mos for the lymphomatous subtype (range 1 to 63 mos), 17 months for the chronic subtype (range 5 to 22 mos) and 34 mos for the smoldering subtype (range 16 to 48 mos). Conclusion: This retrospective series represents one of the largest North American experiences to date among primarily Caribbean descendants. Interestingly, when placed into the context of other experiences published over the past 30 years, it is clear there has been little to no change in the outcome of patients diagnosed with this disease. In fact, this population, with a median overall survival of only 6 mos, represents one of the poorest outcomes reported for any sub-type of lymphoma. These data suggest that radically new ATLL directed therapies are needed. The recent development of several new T-cell active agents, in addition to other strategies (ex: transplantation) need to be specifically studied in this population.

Abstract A 50-year-old woman of south Asian descent presented with a 4-year history of hair loss, itching and follicular prominence at her frontal hairline, in association with increasingly sparse eyebrows and loss of body hair. She was otherwise well and on no medication. Clinical examination revealed typical features of frontal fibrosing alopecia (FFA), with associated papules on the forehead. In addition, striking hyperpigmentation was evident over the forehead, face and neck. This was felt to be in keeping with FFA-associated dyspigmentation–lichen planus (LP) pigmentosus. Initial management was lymecycline 408 mg once daily, tacrolimus 0.1% ointment twice daily plus mometasone furoate 0.1% two times weekly to the neck, and pimecrolimus cream twice a day to the face, as well as a sun protection factor 50 sunscreen. Hydroxychloroquine was avoided initially due to the potential side-effect of altered skin pigmentation with this drug. While the hairline recession remained unchanged, the pigmentary changes worsened despite treatment and affected areas became increasingly itchy. Lymecycline was stopped and hydroxychloroquine 200 mg once daily was started. After 12 months, the cutaneous pigmentation continued to worsen and was extending onto the arms and torso. A trial of low-dose isotretinoin (20 mg once daily) was considered as this has been reported to be of benefit in LP pigmentosus. However, after 6 months, there was no improvement. A trial of mycophenolate mofetil 500 mg twice daily was then commenced. After 3 months of treatment, skin itching had settled and no new areas of hyperpigmentation were noted. After 9 months, there had been a significant improvement in skin pigmentation, which has been maintained. The extent of frontal hairline recession has remained stable. While FFA was initially described exclusively in postmenopausal White women, it has since been reported in Hispanic, African, Afro-Caribbean and Asian individuals, premenopausal women, and men. Dyspigmentation associated with FFA has largely been a feature observed in patients with darker skin types, and it is not clear whether this pigmentation is part of the spectrum of FFA in individuals with darker skin or whether LP pigmentosus is occurring coincidently with FFA in patients with darker skin types. Low-dose isotretinoin has been reported as being of benefit in LP pigmentosus but was ineffective in this case. Mycophenolate mofetil is also reported to be of benefit in the management of LP pigmentosus, and led to a significant improvement in both pigmentation and itch in this case. This case highlights the therapeutic benefit of mycophenolate mofetil in in LP pigmentosus associated with FFA.

This study considers the social and economic lives of free women in Jamaica from 1760 to 1834. Throughout the period studied Jamaica was Britain's most important imperial holding. The colony's slave economy, driven by the labour of hundreds of thousands of enslaved men and women, generated incredible wealth. Still, Jamaica was the deadliest place to live in British America. Due to the endemic nature of tropical disease and atrocious mortality rates, neither the enslaved population nor the white population maintained itself naturally prior to emancipation. However, an environment characterized by death and demographic crisis engendered heightened opportunities for women to take part in tropical enterprise and to shape the futures of their families. Inheritance norms were weakened by the omnipresence of death, precipitating more generous inheritance bequests for women and a greater role given to wives and daughters-both white women, and those of mixed-race descent-in tropical commerce. Additionally, as slave ownership was not limited by gender or race, free women of all races took part in the slave economy. Free women's visibility in the island's formal as well as informal economies, and the wealth accumulated by some, was unsurpassed in a British American context. However, in this slave society, free women's prosperity rested upon the exploitation and oppression of others. In contrast to familiar historical trajectories that have presented Caribbean participation in Atlantic markets of slavery and capital as male-driven ventures, this study argues that free women of all races were vital participants in the slave economy and principle beneficiaries of plantation profits. This project moves beyond previous studies on women in colonial Jamaica by revealing how women's enterprise and relations with one another shaped the nature of this economy and society, including the commercial, familial and kin networks that bound it together. In doing so, it enhances our understanding of this colony and the operation of race and gendered power within it.

This thesis analyses the themes of history, memory, and authorship in the works of four women playwrights from the French Caribbean islands of Martinique and Guadeloupe. In doing so, it aims to reveal the three levels of marginalization to which Caribbean women theatre practitioners are subjected: being a woman, being a French Caribbean woman, and being a French Caribbean woman who writes theatre. The thesis seeks to contribute to the expansion of the field of French Caribbean literary and drama studies, endeavours to redress the gender balance in studies on French Caribbean literature, and aspires to add to the existing body of work on French Caribbean women's writing. Therefore, the thesis aims to reveal and to analyse the world of French Caribbean women's theatre and to study how the playwrights address socio-political issues that affect their communities and influence their own writings and careers. The corpus consists of plays by Gerty Dambury, Ina Césaire, Maryse Condé, and Suzanne Dracius from the 1980s to the early 2000s. While focussing on a different theme, each chapter rests its analysis on theatrical works of a similar genre. The analysis of the plays deploys theories of the theatre pertaining to postcolonial drama and gender. The first chapter serves as an introduction to a group of female French Caribbean writers and their predecessors. The second chapter is a study of two historical plays, focussing on the collective experience of historical events and the role played by women in those events. The third chapter analyses plays that problematize the relationship between the collective and the individual. The fourth chapter looks at the image of the French Caribbean female artist and the multiple barriers she encounters in achieving creative independence.

American history has celebrated the involvement of black women in the "underground railroad," but little is said about women's everyday resistance to the institutional constraints and abuses of slavery. Many Americans have probably heard of and know about Harriet Tubman and Sojourner Truth - two very prominent black female resistance leaders and abolitionists-- but this thesis addresses the lives of some of the less-celebrated and lesser-known (more obscure) women; part of the focus is on the common tasks, relationships, burdens, and leadership roles of these very brave enslaved women. Resistance history in the Caribbean and Americas in its various forms has always emphasized the role of men as leaders and heroes. Studies in the last two decades Momsen 1996, Mintz 1996, Bush 1990, Beckles and Shepherd, Ellis 1985, 1996, Hart 1980, 1985) however, are beginning to suggest the enormous contributions of women to the successes of many of the resistance events. Also, research revelations are being made correcting the negative impressions and images of enslaved women as depicted in colonial writings (Mathis 2001, Beckles and Shepherd 1996, Cooper 1994, Campbell 1986, Price 1996, Campbell 1987). Some of these new findings portray women as not only actively at the forefront of colonial military and political resistance operations but performed those activities in addition to their roles as the bearers of their individual original cultures. Their goal was achievement of freedom for their people. Freedom can be seen as a magic word that politicians, propagandists, psychologists and priests throw around with ease. Yet, to others freedom has a different meaning which varies with the individual's sense of associated values. Freedom without qualification is an abstract noun meaning, "not restricted, unimpeded", or simply, "liberty"; but when it is concretized in individual situations its meaning is narrowed, and it becomes clear that no one can be fully free. Yet the love of freedom is one of our deepest feelings, a truly heartfelt cry, freedom of wide open spaces, liberty to enjoy the taste, in unrestricted fashion, of the joys of nature, to live a life free from external anxieties and internal fears; freedom to be truly ourselves. All living creatures, even animals seem to value their freedom above all else. Enslaved people were not submissive towards their oppressors; attempts were made both subtly, overtly and violently to resist their so-called "masters" and slavery conditions. Violent and non-violent resistance were carried out by the enslaved throughout colonial history on both sides of the Atlantic, and modern historical literature shows that women oftentimes displayed more resistance than men. Enslaved Africans started to fight the transatlantic slave trade as soon as it began. Their struggles were multifaceted and covered four continents over four centuries. Still, they have often been underestimated, overlooked, or forgotten. African resistance was reported in European sources only when it concerned attacks on slave ships and company barracoons, but acts of resistance also took place far from the coast and thus escaped the slavers' attention. To discover them, oral history, archaeology, and autobiographies and biographies of African victims of the slave trade have to be probed. Taken together, these various sources offer a detailed image of the varied strategies Africans used to defend themselves and mount attacks against the slave trade in various ways. The Africans' resistance continued in the Americas, by running away, establishing Maroon communities, sabotage, conspiracy, and open uprising against those who held them in captivity. Freed people petitioned the authorities, led information campaigns, and worked actively to abolish the slave trade and slavery. In Europe, black abolitionists launched or participated in civic movements to end the deportation and enslavement of Africans. They too delivered speeches, provided information, wrote newspaper articles and books. Using violent as well as nonviolent means, Africans in Africa, the Americas, and Europe were constantly involved in the fight against the slave trade and slavery. Women are half the human race and they're half of history, as well. Until recent years, Black women's history has been even less than that. Much work has been done studying the lives of slaves in the United States and the slave system. From elementary school in the USA on through college we are taught the evils of slavery that took place right here in the Land of the Free. However, how much do we know about the enslaved in other places, namely the Caribbean? The Caribbean was the doorway to slavery here in the New World, and so it is important that we study the hardships that enslaved people suffered in that area. Slaves regularly resisted their masters in any way they could. Female slaves, in particular, are reported to have had a very strong sense of independence and they regularly resisted slavery using both violent and non-violent means. The focus of my research is on the lives of enslaved women in the Caribbean and their brave resistance to bondage. Caribbean enslaved women exhibited their strong character, independence and exceptional self worth through their opposition to the tasks they performed in the fields on plantations. Resistance was expressed in many different rebellious ways including not getting married, refusing to reproduce, and through various other forms as part of their open physical resistance. The purpose of this project is to identify the role enslaved women in both the Caribbean and the USA played in some of the major uprisings, revolts, and rebellions during their enslavement period. The research identifies individual female personalities, who played key roles in not only the everyday work on plantations, but also in planning resistance movements in the slave communities. This study utilizes plantations records, archival material, and official sources. Archival records from plantations located in archives and county clerks' offices; interviews with sources such as researchers and experts familiar with the plantations of slave communities in designated areas; and research in libraries, as well as other sources, oral histories, written and oral folklore, and personal interviews were used as well.

Writers in the Caribbean, like writers throughout the postcolonial world, return to colonial texts to rewrite the myths that justified and maintained colonial control. Exemplary of a widespread, regional phenomenon that begins at mid-century, writers such as Aimé Césaire and George Lamming take up certain texts such as Shakespeare's The Tempest and recast them in their own image. Postcolonial literary theory reads this act of rewriting the canon as a political one that speaks back to power and often advocates for political and cultural independence. Towards the end of the twentieth century and at the beginning of the twenty-first century, Caribbean women writers begin a new wave of rewriting that continues in this tradition, but with certain differences, not least of which is a focused attention to gender and sexuality and to the literary legacies of romance. In the dissertation I consider a number of novels from throughout the region that rewrite the romance, including Jean Rhys's Wide Sargasso Sea (1966), Maryse Condé's La migration des coeurs (1995), Mayra Santos-Febres's Nuestra señora de la noche (2006), and Dionne Brand's In Another Place, Not Here (1996). Romance, perhaps more than any other literary form, exerts an allegorical force that exceeds the story of individual characters. The symbolic weight of romance imagines the possibilities of a social order--a social order dependent on the sexual behavior of its citizens. By rewriting the romance, Caribbean women reconsider the sexual politics that have linked women with metaphorical constructions of the nation while at the same time detailing the extent to which transnational forces, including colonization, impact the representation of love and desire in literary texts. Although ultimately these novels refuse the generic requirements of the traditional resolution for romance (the so-called happy ending), they nonetheless gesture towards a reordering of community and a revised notion of kinship that recognizes the weight of both gendered and sexual identities in the Caribbean.

As policy makers and educators from varying philosophical platforms develop strategies for facilitating inclusive education, there is a subsequent realization that this involves inclusive leadership. Such leadership may be addressed through ethical decision-making, exceptionality in learning, equity, effective programming, and partnerships (Crockett, 1999). Related to the moral and ethical aspects of decision making is the issue of caring leadership. Among the educational leaders who have demonstrated caring leadership, and who have had transformative influences over followers are the four women who constitute this study. To understand how they evolved as educational leaders, testimonies of their experiences and perceptions were developed. These testimonies are presented as reconstructed narratives. The discussion on these narratives explores relationships between who these women are in character, their experiences of the ethic of care, and leadership. The study directs focus on the lives of these women with a view to documenting their contributions and sharing their voices about the education systems of Trinidad and Tobago, and the broader Caribbean area. Oral history interviewing, within the biographical tradition, is the methodology used for data collection. The data as transcribed narratives and topical life histories were then content-analyzed to identify common themes and link these with contemporary research on leadership, women, and the ethic of care as discussed in the review of the literature. Findings from the study revealed caring leadership as an evolutionary process, and the importance that spirituality, community, and a sense of gender-equity and inter-relatedness played in the lives of the participants.

This can be seen as the first step in a path that is going to be enriched and further developed in order to demonstrate how oral history can be used to reflect on reading, its multiple formative values and the various contexts in which it is practised.

Chapter two focuses on well-known Afro-Caribbean women writers, Jamaica Kincaid and Edwidge Danticat, who reside in the United States and make a significant contribution to “resistance culture.” Through narratives of return, Kincaid and Danticat challenge exploitative consumption and tourism in their literary works by exposing and utilizing the power that lies in the production of history. They do this by using their mobility and prominence in North American literary markets to inform potential tourists and fellow Caribbeans abroad of the injustices of the tourist industry that are rooted in the history of slavery and colonialism. Kincaid directly confronts and criticizes the tourist industry in her satirical essay/memoir A Small Place; while Danticat participates in and critiques the tourist industry with her travel guide/memoir After the Dance. They produce alternative travel narratives that resist the travel guide genre, which has historically defined “natives” (the other) outside of history, modernity, and humanity.

Abstract This chapter provides an overview of food as a central yet undertheorized component of Caribbean religions. In the history and anthropology of religion, food has been approached most often according to the social function of its consumption and factors that may restrict it. The eminently social dimensions of food consumption are considered alongside the cultural implications and contingent histories of its preparation. The chapter reveals how women and gay men usually occupy the space for cooking in Caribbean religions, but their culinary labor has remained invisible. It highlights the importance of socialization in religious kitchen spaces as they played roles in converting newcomers into practitioners.

This chapter traces the history of the Zika virus. Viruses are usually messengers of bad news. The bad news emerging as a specialty of infection by Zika viruses and the harm they cause is the long-term disability of the most vulnerable populations: pregnant women and their babies. Zika virus is a member of the flavivirus group, whose fellow members are yellow fever and West Nile viruses; all three are transmitted by mosquitoes. Attacks by Zika virus and the disease it caused unexpectedly exploded in 2015–2016, mainly in Brazil and surrounding countries in Central and South America and the Caribbean. Zika was then transported to the United States. Zika infections recorded in the United States were linked primarily to airplane or ship travelers from the areas of Zika outbreaks. Adult males and females infected with Zika virus may develop an autoimmune disease termed Guillain-Barré syndrome (GBS). GBS describes persons whose own immune system attacks their nerves (polyneuropathy), leading to symmetrical weakness of the extremities requiring hospitalization.

Previous research suggests that bisexual women’s rate of cannabis use is 2 to 7 times higher than their heterosexual peers; however, factors contributing to this are unclear. Trait impulsivity (i.e., tendency to act without forethought) and history of childhood physical abuse (CPA) are two risk factors that may be relevant for bisexual women’s cannabis use. Specifically, bisexual women indicate high levels of risk-taking and commonly report histories of CPA. While both impulsivity and CPA have been identified as predictors of cannabis use in heterosexual women, research has yet to explore these factors as predictors of cannabis use among bisexual women. Consequently, the present study examined CPA and trait impulsivity as predictors of cannabis use in a sample of bisexual women. It was hypothesized that both trait impulsivity and exposure to CPA would predict greater frequency of cannabis use. Participants were 225 bisexual women aged 22.77 years (SD = 3.45) recruited from a southeastern university and community area. Participants completed an online survey including questions about their past 30-day frequency of cannabis use (5-point scale ranging from 0 = never to 4 = daily), CPA using the Child Maltreatment Scale-physical abuse subscale, and impulsivity using the Barratt Impulsiveness Scale-version 11. A majority of the sample reported cannabis use in the past 30 days (60.3%), with 23.2% using once or twice, 12.9% using weekly, 11.2% using almost daily, and 12.9% using daily. To account for the large number of zero values on the cannabis use score, we tested a Poisson hurdle model to evaluate the effects of CPA and impulsivity on cannabis use. Frequency of cannabis use was modeled first as a binary logistic model (0 versus any use) and then as a truncated regression model for non-zero responses. Results indicated that across all participants, greater impulsivity predicted any cannabis use in the past 30 days (p = .003), but CPA did not (p = .942). Among participants who reported past 30-day cannabis use only, CPA was associated with increased frequency of cannabis use (p = .003), and impulsivity did not predict frequency of cannabis use (p = .683). Overall, results indicated that greater trait impulsivity was associated with past 30-day cannabis use, but not frequency of cannabis use. Conversely, greater CPA exposure was not associated with whether one used cannabis in the past 30 days but instead was related to increased frequency of past 30-day cannabis use. Thus, although trait impulsivity may identify cannabis users, CPA may identify sexual minority women who are at risk for increased frequency of cannabis use. Given the potential long-term harms associated with increased cannabis use (e.g., changes in brain morphology, cognitive impairment, and respiratory issues), prevention efforts targeting bisexual women may want to consider impulsivity and CPA.

Introduction: Breast cancer is the most common cause among women, with invasive ductal carcinoma (ICD) being the most prevalent and having great phenotypic and genotypic heterogeneity and the ability to metastasize. In turn, mastitis is an inflammation in the breast region, infectious or noninfectious causes, most commonly affecting lactating women. Objectives: It is intended to report a case of ICD accompanied by infectious mastitis and its reverberations. Case Report: A 27-year-old woman presents with a clinical history of mastitis in her left breast that occurred about 3 months ago after weaning her son. She also explained about the use of antibiotic therapy, multiple previous drainages, and local complication with engorgement, edema, erythema, and purulent drainage in the surgical ostium of the left breast. A new drainage and material collection was carried out. Computed tomography of the chest indicated a solid heterogeneous lesion, dense, irregular contours, with areas of air trapping, liquefaction, and cleavage plane with the left pectoralis major muscle and ipsilateral axillary adenomegaly. Pathological pathology confirmed the hypothesis of neoplasia, indicating grade III ICD in comedonecrosis. Immunohistochemistry demonstrated triple-negative character and culture, positivity for Corynebacterium renale. She started neoadjuvant chemotherapy with reduced breast volume and absence of secretion today. Discussion: This is a common and problematic situation in health systems. Repeated outpatient referrals, invasive processes without resolution, aggravating the patient’s case. Moreover, it is noted that mastitis has delayed the diagnosis of the neoplasm, which, depending on the delay, may result in a worse prognosis or a more aggressive or expensive treatment. Conclusion: The diagnostic investigation of neoplasms is of great importance in case of prolonged mastitis not responsive to treatment, due to the fact that many mastitides may come from neoplastic processes that generate the lesion, which can make this injured area conducive to bacterial proliferation.

Introduction: Primary angiosarcoma of the breast (PAOB) is a class of extremely rare sarcomas, with an incidence rate of 1/2,000 cases of breast cancer worldwide. It is more frequent in 20- and 50-year-old women without history of previous cancer and commonly described in the left breast. Clinical presentation can be the same as usual breast cancer and histology can mimic poorly differentiated ductal carcinoma, which is why immunohistochemistry should be performed. Swelling, a feeling of fullness and exponential growth within the breast are frequent complaints, as noted by Kunkiel et al. in their series of case reports. The natural history of PAOB is only partly understood, suggesting that the lesion begins within the mammary parenchyma and then infiltrates skin and subcutaneous tissue nearby. The predominant management has been mastectomy, mainly, or sectorectomy with clear margins in cases of conservative breast surgery. Adjuvant therapies are not associated with improved survival, except for adjuvant chemotherapy in localized tumors of 5 cm or more. Case report: S.O.S., a 32-year-old woman, identified breast asymmetry in 2017, during the lactation period, presence of mild pain and swelling in the left breast. She was admitted to the breast cancer and benign lesions outpatient clinic at Professor Alberto Antunes University Hospital in February 2019. She held a BI-RADS 4 breast magnetic resonance imaging (MRI) in January 2019, which suggestedan irregular mass in the left breast, probably of vasculolymphatic nature; also showed core biopsy in February 2019: low-grade PAOB. In April 2019, she underwent a modified radical mastectomy of the left breast with ipsilateral lymphadenectomy. Due to the large extent of the lesion, an entire cutaneous area of left anterior hemithorax was resected, and thoracoepigastric flap was used to close the left hemithorax. An anatomopathological report diagnosed PAOB grade I. In July 2019, immunohistochemistry corroborated the diagnosis of PAOB with CD31 positive; positive von Willebrand factor (Factor VIII - polyclonal Rabbit) and ki67 positive for 25% of neoplastic cells. In the fourth month after the surgery, the patient started adjuvant radiotherapy, concluding it in October 2019. In post-treatment follow-up, in January 2021, she was referred to the breast reconstruction program, awaiting the procedure until this report was made.

Tears are flowing. Whether Yvonne Catterfeld, Kazim as-Sahir, Unati Msenga-na, Liu Huan, Simon Cowell or Lira – they are all part of a jury of global music casting show formats such as The Voice, Idol or Got Talent and show their tears in front of the camera, seemingly ashamed and yet completely uninhibited. Their tears flow in reaction to ‘particularly soulful’ music titles or to the candidates’ tragic personal stories, paired with the ‘right’ song selection. The display of great emotions is an essential element of reality TV formats. With Sara Ahmed, they can be understood in the sense of an ‘affective economy’ as an effect of their circulation, their staging as a specific ‘emotional style’ of dealing with emotions (Eva Illouz). The circulation of affects in casting shows is a global one, since the formats, developed in Europe, have produced local versions in over 60 countries worldwide. Emotions play an important role in the successful localization of the formats and define a complex area of conflict between a sensitization to socio-cultural characteristics and the ‘reproduction of culturalistic concepts’ (Laura Sūna) or clichés. In European cultural history, tears have developed a special significance as guarantors of the authenticity of empathy (Sigrid Weigel), and are generally associated with femininity, however at the same time have been film-historically recoded as ‘gender-neutral’ (Renate Möhrmann). Keeping in mind that all these casting show formats have been exported from Europe, these observations are of special interest, especially since one can see men and women crying equally in the Arabic, German or South African versions of e. g. The Voice. This article questions the concurrence of musical performance, display of tears, gender performance and the translocal dramaturgy of music casting shows.

Introduction: Cervical cancer is the fourth most frequently found cancer among women worldwide. Numerous studies have underlined that persistent infection with human papilloma virus is the most important risk factor, two strains of the same virus – 16 and 18 being responsible for approximately 70% of the cases. Cervical cancer rarely metastasizes in the cervical lymph nodes and this indicates a poor prognosis. Literature data reports an incidence for left supraclavicular M1LYm of 0.1-1.5%. Material and Method: We bring attention to the case of a 44-year-old patient from the rural area who was diagnosed in January 2019 with stage IIIB cervical cancer, represented morphologically by a poorly differentiated squamous cell carcinoma. When admitted, the patient presented clinically with vaginal hemorrhage, intense abdominal and pelvic pain, fatigue, a dynamic, significant weight loss. The physiological personal history revealed nine pregnancies, the first one when she was 16. After pretherapeutic evaluation, the multidisciplinary committee decides performing simultaneous radio-chemotherapy with platinum salts. During the second week of treatment clinical examination revealed left cervical and supraclavicular adenopathy, both documented through imaging evaluation. Lymph node biopsy is done and its histopathological aspect, correlated with the immunohistochemistry profile supports the diagnosis of poorly differentiated squamous cell carcinoma lymph node metastasis. The initial treatment scheme is maintained, the patient being discharged with clinical remission of cervical and supraclavicular lymph node metastasis. Conclusions: The peculiarity of the case is determined by the distant metastases in the left cervical and supraclavicular lymph nodes, a rare finding during treatment, which was associated with a poor prognosis; in this case treatment was done for palliative purposes. Rapid diagnosis is the main factor that conditions the therapeutic results and chances for healing.

ABSTRACT Background: Indonesia is in the fourth position with the largest population in the world (274 million people) after China, India, and the United States. Currently, Indonesia is experiencing a demographic bonus and also has a high dependency ratio (46.6%). It resulting in heavy burdens that must be borne by the productive age population to finance the lives of the unproductive population. The high population in Indonesia is determined by the high number of children born alive. The purpose of this study was to examine the determinants of fertility in Indonesia. Subjects and Method: A cross-sectional study was conducted using Indonesian Population Demographic Survey year 2017. A sample of 49,627 reproductive women aged 15-49 years who had ever give birth was selected for this study. The dependent variable was fertility (based on number of children born alive). The independent variables were contaceptive use, contraceptive method, source of information, knnowledge toward contraception, history of birth delivery, and residence. The data were analyzed by path analysis run on Stata 13. Results: Fertility increased with traditional contraceptive use (b= 0.51; 95% CI= 0.41 to 0.61; p<0.001), information from government (b= 0.59; 95% CI= 0.46 to 0.72; p <0.001), low education toward contraceptive (b= 0.89; 95% CI= 0.49 to 1.29; p <0.001), birth delivery <1 year (b= 0.10; 95% CI= -0.05 to 0.25; p= 0.187), health assurance participant (b= 0.54; 95% CI= 0.44 to 0.64; p<0.001), living in urban area (b= 0.32; 95% CI= 0.22 to 0.41; p<0.001), hormonal contraceptive use (b= 0.08; 95% CI= -0.10 to 0.25; p= 0.408), and living in west Indonesian (b= 0.57; 95% CI= 0.47 to 0.66; p<0.001). Fertility decreased with family decision on contraceptive use (b= -0.31; 95% CI= -0.42 to -0.21; p<0.001), education ≥Senior high school (b= -1.25; 95% CI= -1.35 to -1.16; p<0.001), and high family wealth (b= -0.50; 95% CI= -0.60 to -0.40; p<0.001). Conclusion: Fertility increases with traditional contraceptive use, information from government, low education toward contraceptive, birth delivery <1 year, health assurance participant, living in urban area, hormonal contraceptive use, and living in west Indonesian. "Breast cancer is a major concern in women’s health in Mainland China. Literatures demonstrates that women with breast cancer (WBC) need to pay much effort into resisting stigma and the impact of treatment side-effects; they suffer from overwhelming consequences due to bodily disfigurement and all these experiences will be unbeneficial for their mental and sexual health. However, related studies in this area are rare in China. The objectives of this study are 1) To understand WBC’s treatment experiences, 2) To understand what kinds of support should be contained in a transdisciplinary intervention framework (TIP) for Chinese WBC through the lens that is sensitive to gender, societal, cultural and practical experience. In this study, the feminist participatory action research (FPAR) approach containing the four cyclical processes of action research was adopted. WBC’s stories were collected through oral history, group materials such as drawings, theme songs, poetry, handicraft, storytelling, and public speech content; research team members and peer counselors were involved in the development of the model. This study revealed that WBC faces difficulties returning to the job market and discrimination, oppression and gender stereotypes are commonly found in the whole treatment process. WBC suffered from structural stigma, public stigma, and self-stigma. The research findings revealed that forming a critical timeline for intervention is essential, including stage 1: Stage of suspected breast cancer (SS), stage 2: Stage of diagnosis (SD), stage 3: Stage of treatment and prognosis (ST), and stage 4: Stage of rehabilitation and integration (SRI). Risk factors for coping with breast cancer are treatment side effects, changes to body image, fear of being stigmatized both in social networks and the job market, and lack of personal care during hospitalization. Protective factors for coping with breast cancer are the support of health professionals, spouses, and peers with the same experience, enhancing coping strategies, and reduction of symptom distress; all these are crucial to enhance resistance when fighting breast cancer. Benefit finding is crucial for WBC to rebuild their self-respect and identity. Collaboration is essential between 1) Health and medical care, 2) Medical social work, 3) Peer counselor network, and 4) self-help organization to form the TIF for quality care. The research findings are crucial for China Health Bureau to develop medical social services through a lens that is sensitive to gender, societal, cultural, and practical experiences of breast cancer survivors and their families."

"Throughout the second half of the seventeen and the eighteenth centuries, tea remained an expensive exotic drink for Britain that “preserved” its overseas nature. It was only in the Victorian era (1837-1903) that tea became the English national drink. The process attracts the attention of academics from various humanities. Despite an impressive amount of research in the UK, in Russia for a long time (in the Soviet years) the English tradition of tea drinking was considered a philistine curiosity unworthy of academic analysis. Accordingly, the English tea party in Russia has become a leader in the number of stereotypes. The issue became important for academics only at the turn of the twentieth and twenty-first centuries. Currently, we can observe significant growth of interest in this area in Russia and an expansion of research into tea drinking with regard to the history of society, philosophy and culture. Despite this fact, there are still serious lacunas in the research of English tea parties in the Victorian era. One of them is related to the analysis of visualization of this practice in Victorian painting. It is a proven fact that tea parties are one of the most popular topics in English arts of the nineteenth and the twentieth centuries. No other art school in the world referred to the topic so frequently: painting formed the visual image of the English tea party, consolidated, propagandized and spread ideas of the national tea tradition. However, this aspect has been reflected neither in British nor Russian studies. Being descriptive and analytical, the present research refers to the principles of historicism, academic reliability and objectivity, helping to determine the principal trends and social and cultural features and models in Britain during the period. The present research is based on the analysis of more than one hundred genre paintings by British artists of the period. The paintings reflect the process of creating a special “truly English” material and visual context of tea drinking, which displaced all “oriental allusions” from this ceremony, to create a specific entourage and etiquette of tea consumption, and set nationally determined patterns of behavior at the tea table. The analysis shows the presence of English traditions of tea drinking visualization. The canvases of British artists, unlike the Russian ones, never reflect social problems: tea parties take place against the background of either well-furnished interiors or beautiful landscapes, being a visual embodiment of Great Britain as a “paradise of the prosperous bourgeoisie”, manifesting the bourgeois virtues. Special attention is paid to the role of the women in this ritual, the theme of the relationship between mothers and children. A unique English painting theme, which has not been manifested in any other art school in the world, is a children’s tea party. Victorian paintings reflect the processes of democratization of society: representatives of the lower classes appear on canvases. Paintings do not only reflect the norms and ideals that existed in the society, but also provide the set patterns for it."

This edition of the Caribbean Economics Quarterly (Q2-2022) is a collaboration between the IDBs Caribbean Country Department and IDB Invest, which focuses on firms access to finance. The report begins by considering both the nature and history of the regions financial sector development, highlighting key measures of financial access and adequacy. It then leverages enterprise survey data developed by the Compete Caribbean partnership to assess legacy and emerging challenges facing firms from across the region, including those owned and/or operated by women. Newly available data from 2020 are compared with a previous vintage of the surveys from 2014, providing important insights into how circumstances have evolved, especially considering the COVID-19 shock. The analysis suggests that: (i) financial sectors and firms across the Caribbean face outsized challenges, particularly when compared to peers across the globe; (ii) the COVID-19 crisis appears to have further constrained access to finance; (iii) smaller firms appear to face more significant hurdles than larger ones; and, (iv) women-owned and/or operated firms face more severe challenges with respect to financial access than other firms across the region. Policies and reforms with the potential to improve financial development, access, and inclusion are highlighted, as well as successful examples of IDB support and collaboration in related areas across the Latin American and Caribbean region.

This policy brief uses data from the 2014/2015 Latin American Public Opinion Project survey to examine attitudes toward intimate partner violence and child physical discipline in six Caribbean countries. Although Latin America has a reputation for a particularly macho culture, Caribbean adults were 10.8 percent more likely to tolerate a man beating his wife if she neglects the household chores and 5.7 percent more likely to if she is unfaithful. Characteristics of those who were more tolerant of intimate partner violence included being lower income, younger, resident of a rural area, and not completing secondary education. Similarly, those who say it is necessary to physically punish children in the Caribbean - and those who experienced physical punishment frequently themselves - were more prevalent than in Latin American countries. Experiencing frequent physical punishment during childhood was found to be a statistically significant correlate of male tolerance of intimate partner violence after controlling for other individual characteristics. Policy options to prevent intimate partner violence and childhood violence are examined.

The starting point for this document was the Inter-American Development Bank (IDB) technical cooperation project Gender and Municipal Development (NI-T1002), which was financed by the Bank's Capacity Building Fund for Local Institutions in Central America (CABILICA) and implemented in coordination with the Nicaraguan Institute for Municipal Strengthening (INIFOM) from 2004 to 2005. The purpose of the project was to promote municipal development interventions that better address the needs and priorities of women and men, and to facilitate full participation in municipal planning and development processes. It supported the development of a module on gender in municipal development to help build capacity among local authorities, planners, and project managers; a national diagnostic study on gender in municipal development; and a compilation of good practices in the Latin American region in this area of work.

The Free Trade Area of the Americas (FTAA) process was launched during the Miami Summit of Heads of State in December 1994. It was the centerpiece of a broader hemispheric initiative of political and socio-economic cooperation among 34 countries of the Americas with the objective to negotiate a hemispheric free trade agreement by the year 2005. The preparatory phase began in January 1995 and formal negotiations were launched in April 1998. The creation of an FTAA would clearly be the most important chapter in the history of regional cooperation in the Western Hemisphere and mark a fitting culmination to a fast maturing trade policy framework in Latin America and the Caribbean. The FTAA process is the result of progressive globalization of the world economy and a profound transformation in the region based on: (i) structural economic reforms in almost all the countries directed at stimulating market activity and a better articulation with the world economy; (ii) the emergence, or strengthening, of democratic regimes almost everywhere and (iii) political commitments to foster peace and cooperation among neighbors with a history of rivalry and conflict. Regional integration has been a fundamental complementary tool for achieving these ambitious national objectives, which permeate the entire region. Latin America and the Caribbean have a long tradition of interest in regional integration. An intense amount of activity in this area emerged out of the Post-War period. However, the initiatives in the first three decades following the War inserted themselves in the prevailing state-led import substitution strategy of the time, itself to a large extent a product of "market skepticism" derived from the Great Depression. In the 1990s, however, a "new" regionalism emerged in Latin America and the Caribbean that conformed to the new national strategies for economic and political transformation and preparation for globalization.
