Purpose: To analyse factors behind the variation of patient doses from barium enema (BE) examinations. Material and Methods: The patients' (n=89) organ and effective doses (E) due to BE examinations were computed with the ODS-60 program. An average risk factor for BE examinations was derived using the BEIR V schema. The correlation of E with several independent variables was analysed. Results: Median Es at five hospitals were 4.4, 6.1, 7.1, 13 and 16 mSv. The E of the female patients (median 9.2 mSv) was higher than that of the males (median 5.4 mSv) ( p<0.001) due to the higher female doses to the gonads, bladder and uterus, resulting from different body structure. An average fatal risk factor of 0.02% per one BE examination was derived. Factors controlled by the radiologist (screening time, number of exposures) explained 40% and patient-related factors explained 16% of the total variation of E. The equipment-related factors are included in the residual 44%. Conclusion: Due to the large contribution of the radiologists' examination technique in the value of E, an optimal examination technique is essential in reducing doses and the stochastic risk to patients.

Background Currently, residency programs lack objective predictors for passing the sequenced American Board of Anesthesiology (ABA) certification examinations on the first attempt. Our hypothesis was that performance on the ABA/American Society of Anesthesiologists In-Training Examination (ITE) and other variables can predict combined success on the ABA Part 1 and Part 2 examinations. Method The authors studied 2,458 subjects who took the ITE immediately after completing the first year of clinical anesthesia training and took the ABA Part 1 examination for primary certification immediately after completing residency training 2 yr later. ITE scores and other variables were used to predict which residents would complete the certification process (passing the ABA Part 1 and Part 2 examinations) in the shortest possible time after graduation. Results ITE scores alone accounted for most of the explained variation in the desired outcome of certification in the shortest possible time. In addition, almost half of the observed variation and most of the explained variance in ABA Part 1 scores was accounted for by ITE scores. A combined model using ITE scores, residency program accreditation cycle length, country of medical school, and gender best predicted which residents would complete the certification examinations in the shortest possible time. Conclusions The principal implication of this study is that higher ABA/ American Society of Anesthesiologists ITE scores taken at the end of the first clinical anesthesia year serve as a significant and moderately strong predictor of high performance on the ABA Part 1 (written) examination, and a significant predictor of success in completing both the Part 1 and Part 2 examinations within the calendar year after the year of graduation from residency. Future studies may identify other predictors, and it would be helpful to identify factors that predict clinical performance as well.

A questionnaire was sent to 126 Fellows who had passed the Fellowship Examination in Intensive Care up to and including the examination of October, 1995. The major objectives were to assess the continuing involvement of Fellows in Intensive Care and obtain feedback on training and the examinations. Only six Fellows failed to respond. Ninety-six per cent of responders had some involvement in Intensive Care and 89% had a current formal Intensive Care appointment. The median percentage of the week spent in the Intensive Care was high. Forty-seven per cent were practising some anaesthesia. Although there was considerable individual variation, the Fellows had not changed their median amount of Intensive Care practice over time. The responders provided feedback on their work patterns in the public and private systems, and their training and examinations. Overall, the training/examination system appears to satisfy Fellows although some fine tuning is required.

Introduction. The dental health sector reform in Serbia has commenced in order to implement health policy changes. Assessment of legally regulated citizens rights to preventive dental examination, revision of those rights and their promotion are one of the dental health reform priorities. Objective. The objective of this study was to assess the utilization of preventive dental examination (PDE) in Serbia in 2006 among different subject groups and different districts and to analyze financial resources spent for those measures. Also, study aims were to compare the utilization of rights to preventive dental examination in Republic of Serbia in period between 2003-2006. Methods. The retrospective analytical study of Public Health Institute and Republican Statistical Office data were conducted. The comparative analytical method was used for assessment of preventive dental examinations within the given time frame from 2003 to 2006. Results. Results achieved in realization of PDE show a broad spectre of district discrepancies. The variation factor depending on preventive examination or population group it is related to varies from 26.73% to 90.88%. Conclusion. Realization of preventive dental examinations in the Republic of Serbia was significantly lower than projected in the period 2003-2006.

Abstract Quantifying the radiological risk from diagnostic radiography is essential for patients’ safety. Effective dose is considered the best quantity to determine the stochastic risk associated with the radiological examinations. Therefore, the present work investigated organ and effective dose for adult patients in the most common type of X-ray examinations, such as the chest (PA/LAT), abdomen (AP) and lumbar spine (AP/LAT) projections. The PCXMC version 2 Monte Carlo program was used to estimate the organ and effective dose for 547 patients who underwent a radiographic examination at the King Abdulaziz Hospital, Saudi Arabia. The results showed wide variation in organ doses. The largest organ dose in each X-ray examination were 2.72 mGy for the spleen in the lumbar spine (LAT), 1.98 mGy for the urinary bladder in the lumbar spine (AP), 0.40 mGy for the spleen in the chest (PA), 0.11 mGy for the kidney in the chest (LAT) and 1.78 mGy for the testicles in the abdomen (AP) examinations. Values for the effective dose were estimated for each investigated X-ray examination, and the results were compared with the published values available in the literature. The obtained values for effective dose were relatively lower than those reported by the European Commission and the United Nations Scientific Committee on the Effects of Atomic Radiation for the same examinations. The results presented here will help establish a database that will be useful for future comparisons and monitoring in radiographic procedures.

The aim of this research was to investigate possible phenotypic variation profile of Marsilea crenata Presl. cultivated in water and in the soil, to find alternative cultivation techniques to fulfill the increasing demand for pollutant-free plants. Phenotypic profile was investigated through macroscopic and microscopic examinations of all plant parts, and phytochemical screening on the ethanol extract of the leaves using thin layer chromatography technique was conducted to detect the terpenoid constituents. Results showed a variation of phenotypic profiles in the macroscopic examination caused by different cultivation methods. Phytochemical screening showed spots of terpenoid compounds with different color intensity. No differences were found in the microscopic examination of the leaves, petioles, and stems. Since major profile changes did not occur, M. crenata is recommended to be cultivated in water as its original habitat by providing a better caring and quality of water. Due to its phytoremediation property, it is necessary to grow M. crenata in pollutant-free water.

The article takes up the issue of a different approach to materials being subject to classical document examination and refers to repeatable forms, which have an important function in handwriting analysis. Basing on examinations of writing samples originating from different individuals, an occurrence of a repeatable form departing from the official definition has been observed, i.e. “inverted repeatability”. It constitutes a variation of the traditional repeatable form, i.e. the compared elements are inverted and executed in opposite directions. Below, examples and results of the research work are presented. The very nature of the “inverted repeatability” in classical document examination is emphasized with reference to writing habits and traditional repeatable forms.

Background: Medical education in Bangladesh is totally controlled by the Government and run a unique undergraduate curriculum throughout the country in both public and private sectors. This paper is aimed to briefly describe the medical education reform in Bangladesh and suggests further assessment changes. The present official form of undergraduate medical curriculum has first evolved in 1988 followed by revision in 2002 and 2012. Assessment and teaching are the two sides of the same coin. Assessment drives learning and learning drives practices. Following the curriculum reform since 2002, the assessment in undergraduate medical education has been greatly changed. There are a lot of in-course formative assessments which include item examination, card final and term final, designed to improve the quality of education. Ten percent marks of summative written examinations derive from formative assessment. Traditional oral examination has been changed to structured form to ensure greater reliability. Even then, teachers are not yet building up to conduct oral examination in such a structured way. Examiners differ in their personality, style and level of experience with variation of questioning and scoring from student to students. Weakness of reliability on oral examination still exists. Students also feel very stressful during the oral examinations. Moreover, to conduct such oral examination, three to four months times per year are lost by the faculties which can be efficiently utilised for teaching and research purposes. Worlds' leading medical schools now-a-days used oral examination only for borderline and distinction students. Bilateral agenesis of the long head of the biceps brachial tendon (LHB) is a very rare variation of the anatomy. We report a case of an 18-year-old man with bilateral agenesis of the long head of the biceps brachii tendon. We present initial findings, radiographical examinations and the follow-up of an unusual entity. Diagnosis of agenesis of the LHB can be challenging especially in cases of traumatic shoulder pain. It is not a very known entity because of its rareness. However, it could be associated with other congenital anomalies. The absence of the LHB is easily ignored in the diagnostic process. Clinical examination should be a pitfall, radiological examination is helpful to confirm the suspicion of LHB absence. MRI is often the first choice, although ultrasonography is cheaper and much easier to access and it is an excellent tool to visualise this anatomic variation with empty or shallow intertubercular groove.

Об’єкт дослідження: електронні варіативні опитування, що реалізовані на основі динамічних веб-форм із логічно пов’язаними полями. Мета дипломного проекту: створити програмне забезпечення для побудови гнучких варіативних форм із можливістю логічної взаємодії елементів опитування. У першому розділі було здійснено аналіз існуючих програмних продуктів, описано функціональні та не функціональні вимоги, поставлено комплекс задач. У другому розділі описано моделювання та конструювання программного забезпечення, проведено аналіз безпеки даних. У третьому розділі описано аналіз якості та тестування программного забезпечення. У четвертому розділі описано процесс розгортання та впровадження программного забезпечення, і також наведено інструкцію користувача.
The explanatory note of the diploma project consists of four sections, contains 22 tables and 7 sources - a total of 59 pages. Object of research: electronic variational surveys implemented on the basis of dynamic web forms with logically related fields. The purpose of the diploma project: to create software for the construction of flexible variational forms with the possibility of logical interaction of the survey elements. In the first section, an analysis of existing software products was performed, functional and non-functional requirements were described, a set of tasks was set. The second section describes the simulation and design of the software, conducted a data security analysis. The third section describes the quality analysis and software testing. The fourth section describes how to deploy and implement the software, and also the user's manual.

A critical overview of forensic science was performed by the National Academy of Sciences (NAS) in 2009 which resulted in several recommendations for the advancement of the hair analysis discipline within the field of trace evidence examination. Among them included improvements to training and proficiency testing requirements, adaptation of uniform protocols between all laboratories, and enhanced research and development efforts aimed at reducing the subjectivity between multiple analysts or within the same analyst. A survey was created for forensic hair analysts to determine what current microscopic hair examination procedures are being used and to identify variability between characteristics of individual examiners and laboratory practices in the field, three years following the NAS report. The survey was completed by 117 members of the American Society of Trace Evidence Examiners (ASTEE) and contained questions about the experience and training of the analyst, impact of the NAS report findings, type of hair examinations performed, and hair features referenced during microscopic hair comparisons. Most analysts surveyed received the same substantial amount of training and have several years of experience; however, very few reported that any laboratory procedures were affected as a result of the NAS report. Though hair analysis is not structured by mandated, uniform standard operating procedures, largely the same number and type of hair features are being referenced when microscopic hair comparisons are performed, indicating that little variability exists between laboratories or individual examiners. A trend toward more frequent root screenings for DNA viability and less frequent full microscopic hair comparisons was detected and supplemented by a shift of hair analysis from trace evidence sections to biology or DNA sections. Several technologies new to forensic hair analysis are currently being explored to supplement hair analyses with numerical data and, therefore, reduce subjectivity of forensic hair analysis.

This book investigates nominal arguments in classifier languages. A long-held claim is that classifier languages do not have overt article determiners (D). This book, however, brings to the forefront the theoretical investigation on the typologically unique Nuosu Yi, a classifier language that will be shown to have an overt article determiner. By comparing nominal arguments in Nuosu Yi to those in Mandarin, the book provides a parametric account of variation among classifier languages and extends the account to argument formation in general. This book begins with a detailed examination of bare numeral classifier phrases in Mandarin by comparing them with bare numeral noun phrases in number marking languages, such as English, French, and Russian. The book argues for a unified structure of bare numeral containing phrases with no reference to D across languages as well as for a D-less structure for various types of nominal arguments in Mandarin. It further studies nominal argument formation in Nuosu Yi. The facts from Nuosu Yi essentially alter the landscape of empirical data and constitute an immediate (prima facie) challenge to the proposed analysis of nominal arguments based on the Mandarin data. This book argues that despite the fact that Nuosu Yi has an overt article determiner, this should not force us to change anything about the proposed analysis of nominal arguments. Lastly, the book puts the analysis of Mandarin and Nuosu Yi nominal arguments in a broader, cross-linguistic perspective and develops a parametric account of variation in nominal argument formation in general.

States and employers are increasingly selecting highly skilled immigrants according to labour market qualifications and broad human capital attributes. This chapter considers the gender implications of the focus on skills through an examination of the different career trajectories of men and women. In particular, it considers the acknowledgement of part-time and non-continuous work in skilled immigration policy design as well as the potentially discriminatory effects of age limits. In doing so, it applies feminist theories from industrial relations and economics to the examination of skilled immigration policies in twelve countries, demonstrating variation across countries in their awareness to gender concerns.

Abstract The behavior of one-dimensional waves propagating through media that are not homogeneous will be the focus of this chapter. We start with an examination of the behavior of planewaves impinging on a planar interface between two fluid media with different properties and then extend that analysis to multiple interfaces and to waves that impinge on such an interface from an angle that is not perpendicular to that surface. The extent of those boundaries separating regions with different acoustical properties will be much larger than the wavelength of the sound. Many cases to be examined here will assume that the extent of the boundary is infinite and the wave incident on such an interface will be both reflected back into the medium from which it originated and be transmitted into the second medium on the other side of the interface. This exploration concludes with consideration of wave propagation through a medium whose properties change slowly and continuously through space resulting in curved ray paths. If the variation of sound speed is linear with height or depth, then the ray paths are arcs of circles. Complicated sound speed profiles will be approximated by piecewise-linear segments that have constant sound speed gradients.

Chapter 2 deals with the manner in which genetic variation is organized in random-mating populations, including such basics as the Hardy–Weinberg principle, multiple alleles and DNA typing, and X-linkage. These are basic, elementary concepts that set the stage for later chapters. It also includes detailed examination of two-locus linkage disequilibrium (LD), measures of LD, and the levels of LD observed in natural populations. The chi-square test for goodness of fit is discussed, together with its interpretation, along with the basics of hypothesis testing including type I error (false positive), type II error (false negative), power, and the need to correct for multiple comparisons. Also discussed are LD due to population admixture and Wahlund’s principle.

Jet-stabilized combustion is a promising technology for fuel flexible, reliable, highly efficient combustion systems. The aim of this work is a reduction of NOx emissions of a previously published two-staged MGT combustor [1, 2], where the pilot stage of the combustor was identified as the main contributor to NOx emissions. The geometry optimization was carried out regarding the shape of the pilot dome and the interface between pilot and main stage in order to prevent the formation of high temperature recirculation zones. Both stages have been run separately to allow a detailed understanding of the flame stabilization within the combustor, its range of stable combustion, the interaction between both stages and the influence of the modified geometry. All experiments were conducted at atmospheric pressure and an air preheat temperature of 650 °C. The flame was analyzed in terms of shape, length and lift-off height, using OH* chemiluminescence images. Emission measurements for NOx, CO and UHC emissions were carried out. At a global air number of λ = 2, a fuel split variation was carried out from 0 (only pilot-stage) to 1 (only main stage). The modification of the geometry lead to a decrease in NOx and CO emissions throughout the fuel split variation in comparison with the previous design. Regarding CO emissions, the pilot stage operations is beneficial for a fuel split above 0.8. The local maximum in NOx emissions observed for the previous combustor design at a fuel split of 0.78 was not apparent for the modified design. NOx emissions were increasing, when the local air number of the pilot stage was below the global air number. In order to evaluate the influence of the modified design on the flow field and identify the origin of the emission reduction compared to the previous design, unsteady RANS simulations were carried out for both geometries at fuel splits of 0.93 and 0.78, respectively, using the DLR in-house code THETA with the k-w SST turbulence model and the DRM22 [3] detailed reaction mechanism. The numerical results showed a strong influence of the recirculation zones on the pilot stage reaction zone.

Many material properties are statistical in nature. If one measures the same property of the same material repeatedly, ideally the result is a normally distributed “bell” curve about a mean value. This ideal case does not necessarily hold true for all mechanical properties of interest in steel weld metals. Tensile strength measurements tend to exhibit normal behavior for a given weld metal chemical composition deposited using a reasonable consistent welding procedure, Figure 1a. However, toughness measurements are not nearly as well-behaved or predictable. In a tensile test, assuming a defect free weld, the strength measurement is based on the bulk response of the material throughout the gage length. In a Charpy V-notch (CVN) impact test, again assuming a defect free weld, the toughness measurements are controlled largely by the very local response of the material at the point of highest stress where fracture initiates just below the notch. This paper presents a detailed assessment of a C-Mn weld metal and explains how CVN toughness can vary from less than 20 ft-lbf to over 200 ft-lbf in the same weld, often with test specimens located adjacent to one another in the test weld, Figure 1b. The much localized microstructure features that give rise to this degree of variation are a combined result of chemical composition, welding procedure, pass sequence, and individual welder technique. The evidence suggests that retained austenite in coarse grained regions of the as-deposited weld metal transform to martensite at the CVN test temperature, effectively creating local brittle zones in the weld metal. This example provides basis for examination of a broader range of microstructural discontinuities in steel weld metals and their potential influence on toughness measurement.

The weld thermal cycle in microalloyed pipeline and structural steels results in significant changes in microstructure and, consequently, mechanical properties of the weld heat-affected zone (HAZ). To have better control of the properties of the HAZ, knowledge of these changes and correlation with mechanical property variations is required. The first step in achieving this is to construct a methodology to examine different regions of the HAZ thoroughly, so that important characteristics such as grain size, microstructure, precipitate type and distribution, and mechanical properties are determined. The next step would be using this methodology to examine different regions of the HAZ under different welding conditions (the most important of which is heat input) and therefore to understand the effects of the welding process. In this paper, a methodology for studying HAZ microstructure/property relationships is demonstrated for a Grade 100 microalloyed steel welded autogenously (no filler metal). Microhardness measurements are used to assess the mechanical property variation across the HAZ. Optical microscopy (OM) and transmission electron microscopy (TEM) are used for grain structure examination and precipitate analysis. The relationship between microstructure and mechanical properties, with emphasis on precipitate reactions, is presented for different regions of the HAZ.

Virginia Power Surry Nuclear Station Pressurized Water Reactor (PWR) fuel was stored in a dry inert atmosphere Castor V/21 cask at the Idaho National Environmental and Engineering Laboratory (INEEL) for 15 years at peak cladding temperatures decreasing from about 350 to 150°C. Prior to the storage, the loaded cask was subjected to extensive thermal benchmark tests. The cask was opened to examine the fuel for degradation and to determine if it was suitable for extended storage. No rod breaches had occurred and no visible degradation or crud/oxide spallation were observed. Twelve rods were removed from the center of the T11 assembly and shipped from INEEL to the Argonne-West HFEF for profilometric scans. Four of these rods were punctured to determine the fission gas release from the fuel matrix and internal pressure in the rods. Three of the four rods were cut into five segments each, then shipped to the Argonne-East AGHCF for detailed examination. The test plan calls for metallographic examination of six samples from two of the rods, microhardness and hydrogen content measurements at or near the six metallographic sample locations, tensile testing of six samples from the two rods, and thermal creep testing of eight samples from the two rods to determine the extent of residual creep life. The results from the profilometry (12 rods), gas release measurements (4 rods), metallographic examinations (2 samples from 1 rod), and microhardness and hydrogen content characterization (2 samples from 1 rod) are reported here. The tensile and creep studies are just starting and will be reported at a later date, along with the additional characterization work to be performed. Although only limited prestorage characterization is available, a number of preliminary conclusions can be drawn based on comparison with characterization of Florida Power Turkey Point rods of a similar vintage. Based on this comparison, it appears that little or no cladding thermal creep and fission gas release from the fuel pellets occurred during the thermal benchmark tests or storage. Measurements of the cladding outer-diameter, oxide thickness and wall thickness are in the expected range for cladding of the Surry exposure. The measured hydrogen content is consistent with the oxide thickness. The volume of hydrides varies azimuthally around the cladding, but there is little variation across the thickness, of the cladding. It is most significant that all of the hydrides appear to have retained the circumferential orientation typical of prestorage PWR fuel rods.

Over the years, minor children’s family structure has experienced little change and has been well documented in previous family profiles (FP-17-15 & FP-13-19 & FP-19-25) and research. Consistent with prior work (Manning, Brown, & Stykes, 2014), U.S. children’s family structure has remained stable—with little to no change over the past ten years. Although changes to minor children’s family structure over time is small, examination of potential changes is still important. Using data from the 2021 ASEC Current Population Survey from IPUMS, this profile updates FP-19-25. We visualize variation in family structure of minor children (under the age of 18) in the U.S. by race/ethnicity. We also present state-level variation in the share of children living with two married biological parents.
