Still, it was only with the development of modern scientific medicine in the late 19th and early 20th centuries that its use in therapy began to be based on more solid scientific evidence. Today, we know that music can have a powerful effect on the heart, and research has shown that certain types of music can help improve heart function. This review explores the latest scientific findings on how music affects the heart and the different music types that may benefit heart health.

This work considers the nature of therapeutic relationships between nursing staff and patients on acute mental health inpatient wards. Section A is a literature review, exploring the psychological theories behind the care delivered by nurses through the medium of therapeutic relationships in inpatient settings and providing a meta-synthesis of studies investigating the nature of therapeutic relationships between nursing staff and patients from the perspectives of nurses. Section B presents a phenomenological study in which nursing staff completed in-depth interviews providing descriptions of their therapeutic relationships with patients. The results suggest a great variance in the nursing staff ability to get to know and understand patients and their needs from a psychological perspective. The knowledge gained about patients through the medium of relationships did not seem to be shared by the staff team and did not seem to be integrated into a coherent treatment plan.

The addiction subject is nowadays a valid one, as well as in the past century. Not only because of the increase of people that are addict, but also because of the important effects that cause on people and their environments. There are many theoretical perspectives to approach the addiction problem, but the most convenient because of its therapeutic results is the one that issupported by the psychology of health. lt is based on the integral approach to the person. This paper describes a general therapeutic scheme to work with addicts from the cognitive behaviora lperspective.
El tema de las adicciones sigue tan vigente como en el siglo pasado, no solo por el incremento del número de personas que lo padecen sino también por las importantes secuelas que produceen el individuo y en su entorno. Existen diferentes enfoques teóricos para abordar la problemáticade las adicciones, sin embargo el que nos parece más conveniente por sus resultados terapéuticos es el que proviene desde la Psicología de la salud, basado en un enfoque integral en el abordaje del sujeto. El artículo fundamentalmente describe un esquema terapéutico general de su aplicación a los adictos, desde el enfoque cognitivo-conductual.

Cette thèse de doctorat met l’emphase sur les défis rencontrés par les patients atteints de maladies rares. Elle est structurée en trois parties, chacune d’entre elles dédiée aux enjeux d’un acteur au cœur de l’Odyssée diagnostique et thérapeutique des patients atteints de maladies rares. La première partie de la thèse s’intéresse au patient et à son réseau social. Le chapitre 1 considère les sources de délai à l’accès au diagnostic, et explore notamment l’effet du capital social sur le délai d’obtention du diagnostic. Le chapitre 2 évoque les externalités négatives sur la santé maternelle d’un diagnostic d’une maladie chronique chez l’enfant. La seconde partie de la thèse est dédiée à l’industrie pharmaceutique et s’intéresse aux décisions d’investissements de R&D ciblant les maladies rares. Le chapitre 3 évalue l’effet causal de l’Orphan Drug Policy sur l’effort de recherche, et le chapitre 4 envisage les inégalités d’allocation des investissements de R&D entre les maladies rares. La partie 3 est dédiée aux décideurs publics et discute des enjeux d’évaluation des bénéfices de l’innovation thérapeutique et de la définition des conditions d’accès à cette innovation. This dissertation emphasizes the challenges raised by the management of rare diseases and is structured around three key actors of the diagnostic and therapeutic “odyssey” of patients with rare diseases. Part I is devoted to patients and their social networks. Chapter 1 considers demand-side sources of delay in receiving a diagnosis; Chapter 2 explores the health spillover effects from patients’ health to their direct support structure. Part II considers pharmaceutical firms and examines how firms’ decisions to allocate R&D investment to rare diseases are impacted by innovation policies in rare arenas. Chapter 3 evaluates the causal impact of the EU Orphan Drug policy on R&D efforts in orphan drugs, while Chapter 4 investigates the inequality in allocation of R&D investment within rare diseases. Part III focuses on policymakers and addresses the issues in measuring pharmaceutical innovation benefits along with costs in rare disease arenas, while considering the opportunity cost of healthcare expenditures. Chapter 5 measures the causal impact of pharmaceutical innovation in rare diseases on longevity, while Chapter 6 is a critical discussion of decision-making tools for rational allocation of healthcare resources, and the use of a cost-effectiveness threshold

This phenomenological study describes the therapeutic potential of change for women who come together in collective creative process to perform their stories. The author examines women, aged 30-72, and their experiences of collective performance, spanning 29 years, in response to their life circumstances, emotional health, personal relationships, professional life, and community connections. Roles inside and outside of the group are explored, as are their group and individual processes. Research on women, collectives, applied theatre, and therapeutic theatre is presented. Perspective is gained through the lenses of feminist theory, social constructivism, and psychodramatic role theory. Comparisons are made between applied theatre and drama therapy, and the mutual exclusion of group versus individual, socio-political versus therapy, is questioned. Implications for the use of socio-political community drama in a therapeutic theatre format in drama therapy are formulated.

Research has shown that plants, gardens, and nature can promote and enhance human health and well-being. A review of the relevant literature examined human-environment relationships and the possible therapeutic benefits derived from these relationships. In addition to the research literature, therapeutic gardens designed by landscape architects were examined. A comparative analysis between various theories within the research literature, and between the research literature and executed designs was performed. This analysis resulted in design suggestions and guidelines from the point of view of fostering human health and well-being.

The first study examined the interaction between low doses of the multi-kinase inhibitor sorafenib and the histone deacetylase inhibitor vorinostat in colon cancer cells. Sorafenib and vorinostat synergized to kill HCT116 and SW480 cells. In SW480 cells, sorafenib+vorinostat toxicity correlated with CD95 activation and CD95-stimulated autophagy. Drug lethality in SW480 cells was blocked by knock down of CD95. In SW620 cells that are patient matched to SW480 cells, sorafenib+vorinostat toxicity was significantly reduced that correlated with a lack of CD95 activation and lower expression of ceramide synthase 6 (LASS6). Overexpression of LASS6 in SW620 cells enhanced drug-induced CD95 activation and tumor cell killing, whereas knock down of LASS6 in SW480 cells suppressed CD95 activation. In HCT116 cells, sorafenib+vorinostat did not increase CD95 plasma membrane levels, weakly induced caspase 8 association with CD95, and knock down of CD95 enhanced drug lethality. In HCT116 cells sorafenib+vorinostat treatment caused CD95-dependent autophagy that was a protective signal. Thus, treatment of tumor cells with sorafenib+vorinostat activates CD95 that promotes viability via autophagy or degrades survival via extrinsic or intrinsic pathways. Drug-induced activation of the de novo ceramide synthesis pathway plays a key role in CD95 activation. The second project explores the mechanism by which the combination of 17AAG, an hsp90 inhibitor, and PD184352, a MEK1/2 inhibitor alters survival in colon cancer cells. 17AAG and PD184352 synergized to kill HCT116 and SW480 cells. In HCT116 cells drug-exposure increased CD95 plasma membrane levels In SW620 cells, 17AAG and PD184352 toxicity was significantly reduced that correlated with a lack of CD95 activation and lower expression LASS6. Overexpression of LASS6 in SW620 cells enhanced drug-induced CD95 activation and tumor cell killing. In Mia Paca2 cells, a pancreatic cell line, inhibition of caspase 8 or overexpression of c-FLIP-s suppressed cell killing by PD184352 and 17AAG exposure. Drug lethality in Mia Paca2 cells was blocked by knock down of CD95. Additionally, overexpression of Bcl-xL or knockdown of caspase 9 decreased cell killing in 17AAG and PD184352 combination treatment. Thus, 17AAG+PD184352 exposure activates the extrinsic and intrinsic apoptotic pathways to kill Mia Paca2 cells. This document was created in Microsoft Word 2000.

Duchenne muscular dystrophy (DMD) is common and fatal X-linked genetic disorder. The overall objective of this thesis was to carry out a prospective study for dystrophin gene therapy in dystrophic muscles, using the X-linked muscular dystrophy (mdx) mouse model. Recombinant adenovirus (AdV) is presently the vehicle of choice for gene therapy for a number of diseases including DMD. However AdV possess two major limitations when utilized as vectors in skeletal muscles: (i) the maturation-dependency of AdV-infectivity in skeletal muscles, (ii) the host immune response against adenoviral proteins as well as the transgene product. Thus, the work presented in this thesis addresses these two major limiting factors. By modifying either the host or the vectors, we have attempted to optimize AdV-mediated therapeutic gene transfer in dystrophic muscle. Our strategy has consisted firstly to evaluate the regenerative response of dystrophic muscle with advanced disease after experimentally-induced regeneration, in an attempt to recapitulate the myogenic program. We report that mdx mice with severe dystrophic pathology can still show a substantial level of muscle repair with attendant generation of immature myofibers. Secondly, by taking advantage of this level of regeneration and the concomitant generation of immature myofibers, we have upregulated expression of the coxsackie adenovirus receptor (CAR) present in mdx muscles. We have delivered AdV containing a dystrophin gene (AdV-Dys) at a period corresponding to this peak level of CAR expression and we have reported a significant increase of the number of dystrophin expressing myofibers with a net trend toward muscle function amelioration.
The work of this thesis also reports that the combined blockade of calcineurin and CD28 signaling, two key and distinct elements needed for an effective immune response, effectively blunted the immune-mediated destruction of dystrophin expressing myofibers expressed after AdV-Dys delivery. As an alternative to host modification (regeneration and immunosuppression) that can be associated with potential toxic effects, we have explored a strategy where by the recombinant AdV vector contains a less immunogenic transgene utrophin. We report that overexpression of utrophin and dystrophin by AdV-mediated gene transfer in adult immunocompetent mdx mice produces differential effects on muscle cell function in adult immunocompetent (mdx) mice. Anxiety is a diffuse fear of an object, rather potential than present, it is detached from concrete and projected in the future. It associates psychomotor restlessness and has neurovegetative response. Anxious symptoms create a discomfort that patients experience with great difficulty. Whether we are talking about generalized anxiety, or we are talking about anxious paroxysms, patients call for help, sometimes in Emergency Room (ER) because of irrational fear of death, or fear of madness. The anxiety disorder is common in all medical healthcare offices, but especially in psychiatry. The therapeutic attitude is based on the same principles everywhere, but there are situations in which the treatment differs and psychotic anxiety, the particular form requiring admission into specialized service, is under discussion here. If in the past, the first intention was benzodiazepine (BZD) anxiolytics at the moment, they are increasingly finding their place in the therapeutic scheme. The beneficial effect installs quickly, but when balancing the balance versus risk, balances often tend to overcome the anxiolytic classics. Nowadays, more frequently, protocols recommend administering SSRI antidepressants to treat anxiety. In the case of emergency in which anxiety occupies a main place, such as psychotic anxiety, it is necessary to prescribe antipsychotics, especially atypical antipsychotics. For these reasons, we aim to share our experience for patient benefit.

Spread across Brazil and attaining an unparalleled political force, therapeutic communities are as inescapable in the debate on drug policy as they are complex to define. Although they are not a Brazilian creation, they have been operating in that country for decades, and their dissemination intensified in the 1990s. In 2011, they were officially incorporated into Brazil's Psychosocial Care Network (Rede de Atenção Psicossocial, or RAPS). Since then, therapeutic communities have been at the center of public debates about their regulation; about how they should—or even if they should—be a part of the healthcare system; about the level of supervision to which they should be submitted; about their sources of funding, particularly whether or not they should have access to public funding; and, most importantly, about the quality of the services they offer and the many reports of rights violation that have been made public. However, a well-informed public debate can only flourish if the available information is based on sound evidence. The SSRC’s Drugs, Security and Democracy Program is concerned with the policy relevance of the research projects it supports, and the debate around therapeutic communities in Brazil points to a clear need for impartial research that addresses different cross-cutting aspects of this topic in its various dimensions: legal, regulatory, health, and observance of human rights, among others. It is in this context that we publish this working paper series on therapeutic communities in Brazil. The eight articles that compose this series offer a multidisciplinary view of the topic, expanding and deepening the existing literature and offering powerful contributions to a substantive analysis of therapeutic communities as instruments of public policy. Although they can be read separately, it is as a whole that the strength of the eight articles that make up this series becomes more evident. Even though they offer different perspectives, they are complementary works in—and already essential for—delineating and understanding the phenomenon of therapeutic communities in Brazil.

Although phytochemicals are plant-derived toxins that are primarily produced by plants as a form of defense against insects or microbes, several lines of studies have demonstrated that phytochemicals (e.g., polyphenols, carotenoids, and amino acids) have several beneficial biological actions for human health, such as anti-oxidative, anti-inflammatory, and cardioprotective effects. Recent studies have demonstrated that phytochemicals can modulate neuronal excitability in the nervous system, including nociceptive sensory transmission, so it is possible that phytochemicals could be complementary alternative medicine candidates; specifically, therapeutic agents against pain. The focus of this review is to elucidate the mechanisms underlying the modulatory effects of phytochemicals on neuronal electrical signals, such as generator potentials, action potentials, and postsynaptic potentials, in the nociceptive pathway neurons resulting in potential local anesthetic effects, intravenous anesthesia and analgesic effects, and inflammatory pain relief effects. In addition, we discuss the contribution of phytochemicals to the relief of nociceptive and/or pathological pain and their potential clinical application on the basis of our recent studies in vivo.

Traditionally, the interactions of drugs and toxicants with human tissue have been investigated in a reductionist way—for example, by focusing on specific molecular targets and using single-cell-type cultures before testing compounds in whole organisms. More recently, “systems biology” approaches attempt to enhance the predictive value of in vitro biological data by adopting a comprehensive description of biological systems and using computational tools that are sophisticated enough to handle the complexity of these systems. However, the utility of computational models resulting from these efforts completely relies on the quality of the data used to construct them. Here, we propose that recent advances in the development of bioengineered, three-dimensional, multicellular constructs provide in vitro data of sufficient complexity and physiological relevance to be used in predictive systems biology models of human responses. Such predictive models are essential to maximally leveraging these emerging bioengineering technologies to improve both therapeutic development and toxicity risk assessment. This brief outlines the opportunities presented by emerging technologies and approaches for the acceleration of drug development and toxicity testing, as well as the challenges lying ahead for the field.

Review question / Objective: The aim of this study was to compare the therapeutic effects of various stem cell types, sources and routes of administration on chronic decompensated cirrhosis by using network meta-analysis. Condition being studied: Liver cirrhosis is an important public health problem that puzzles the world. It is divided into compensatory stage and decompensated stage. Once the patient enters decompensated stage, the treatment is very limited, and liver transplantation is currently the best and only approach to improve the survival rate of decompensated cirrhosis4. However, liver transplantation is difficult to be widely applied due to the lack of donor organs and high cost. Therefore, it is very important to study the alternative treatment of liver transplantation. Stem cell therapy as a promising frontier treatment for decompensated cirrhosis, is becoming one of the best feasible alternatives to liver transplantation in recent 20 years. It is very important and necessary to optimize the factors such as cell sources, types, and delivery route, etc. before taking stem cell therapy as a routine clinical treatment. It is believed that the network meta-analysis of the efficacy of various types of stem cells from different sources and routes of administration in the treatment of chronic decompensated cirrhosis can provide useful very clues for clinical practice.

Review question / Objective: The objective of this systematic review is to summarise different approaches reported in studies to evaluate guideline adherence in physiotherapy care. Further, we aim to identify clinical and methodological factors that may explain the assumed heterogeneity of guideline adherence among physiotherapists. Condition being studied: Clinical practice guidelines are systematically developed statements that summarise the current state of knowledge from research and practice. They are intended to support clinicians and patients to make decisions about appropriate health care for specific clinical circumstances (2). Various studies show that evidence-based physiotherapy care can lead to improved patient outcomes (e.g. pain, function, quality of life) and at the same time contribute to a lower utilisation of medical services and a reduction in health care costs. The degree of agreement between medical or therapeutic care and the recommendations made in guidelines is often described in studies with the term "guideline adherence". However, the heterogeneous use of the term guideline adherence and the lack of a standardised research methods or operationalisation lead to limited comparability of the study results.

Highly acute mental illness and acute severe behavioural disturbance (ASBD) are two of the most challenging problems faced by Mental Health Intensive Care Units (MHICU). ASBD is defined as behaviour that places the patient or others at imminent risk of injury or death. It includes extreme distress, aggression, and serious self-harm, in the context of mental illness. This Evidence Check assessed the literature on models of care and treatment strategies for these conditions, aiming to refine the model used in MHICU in NSW. It attempted to find the most effective models of care for high acuity and ASBD—and the barriers and enablers to implementing them. A total of 58 relevant papers were found, spanning 2015 to 2020. They were rated on a hierarchy of evidence designed for models of care and interventions in complex settings. Two models of care, ‘Safewards’ and ‘Improving the therapeutic milieu of the wards’, were rated as best practice. These were followed by five models of care, themes and groups of treatments at middle levels of the hierarchy, and nine themes and treatment practices at the lowest level. There were several features common to numerous papers in the review: therapeutic engagement, meaningful activities, safe spaces, and welcoming spaces. Barriers and enablers to implementation were generally not addressed specifically. However, themes emerging from the papers showed several enabling factors: training, buy-in from stakeholders at all levels of the organisation, and assessment of progress. Barriers to implementation included lack of support from management and lack of engagement from frontline staff. Complex, multilevel practice change interventions appear necessary for effective implementation.

Review question / Objective: This systematic review aims to comprehensively assess the efficacy and safety of full-endoscopic cervical laminectomy and decompression versus anterior cervical decompression with fusion in treating cervical spondylotic myelopathy (CSM) patients. Condition being studied: Cervical spondylotic myelopathy (CSM) is a degenerative disease associated with cervical cord compression, which has increased significant health-related social costs and derived disabilities. Anterior cervical discectomy and fusion (ACDF) is the "gold standard" for the treatment of CSM. However, the application of ACDF may cause some complications. Recently, full-endoscopic cervical laminectomy and decompression have shown potential therapeutic effects for CSM. However, no systematic review or meta-analysis has focused on the effects of full-endoscopic cervical laminectomy and decompression in the treatment of CSM. This systematic review aims to comprehensively assess the efficacy and safety of full-endoscopic cervical laminectomy and decompression versus anterior cervical decompression with fusion in treating CSM patients.

Review question / Objective: We provide a protocol to evaluate the efficacy of integrated Shoutai Pill and Western medicine to update the evaluation for the best available and security treatment for recurrent pregnancy loss(RPL). Condition being studied: Recurrent pregnancy loss (RPL) is a distinct disorder defined by two or more consecutive pregnancy failures before 20 gestational weeks infertile couples. The incidence of this disease accounts for about 1%-5% of women of reproductive age and seriously affects their physical and psychological health. At present, the known etiology of this disease mainly includes abnormal anatomic structures, genetic abnormality, endocrine disorders, prethrombotic status, abnormal immune function, infection, etc. Excluding the above factors, approximately 40-50% of RPL remain unexplained, known as unexplained recurrent pregnancy loss (URPL). At present, the main therapeutic methods of RPL are surgical therapy, preimplantation genetic diagnosis (PGD), hormone therapy, anti-infection therapy, anticoagulation, and immunoregulatory therapy, etc. However, there is no effective treatment has been identified for URPL. Therefore, we still need to investigate effective treatments to reduce pregnancy losses and maintain successful pregnancy preservation in these patients.

Review question / Objective: To assess the efficacy and safety of the two approaches as a treatment for patients with OVCF. Condition being studied: Osteoporotic Vertebral Compression Fractures(OVCFs) is one of the most common health problems in the elderly population. Percutaneous kyphoplasty is a minimally invasive technique that has gained widespread recognition. Transpedicular and extrapedicular are two approaches for kyphoplasty. But over the last decade, the safety and effect of two approaches remain unclear, and there is still a lack if evaluation of their therapeutic effects. Information sources: We searched CENTRAL; MEDLINE; EMBASE; Chinese Biological Medicine Database; VIP Journals Database; Wan-fang database, CNKI; and Chinese Evidence-Based Medicine Database from the their inception to December 2020. The search terms used were: the transpedicular approach and extrapedicular approach for treating OVCF in both English and Chinese. We also searched our own files manually for relevant articles. We hand searched Chinese language journals and conference proceedings. We included dissertations and abstracts, provided they contained sufficient detail for critical evaluation.

This rapid review synthesises the literature on how community-based management of acute malnutrition (CMAM) programmes could be adapted in settings of conflict and fragility. It identifies multiple factors affecting the quality and effectiveness of CMAM services including the health system, community engagement and linkages with other programmes, including education, sanitation, and early childhood development. Family MUAC (Mid-Upper Arm Circumference) is a useful tool to increase community participation and detect early cases of moderate acute malnutrition (MAM) and severe acute malnutrition (SAM) more effectively and less likely to require inpatient care. The literature does not say a lot about m-Health solutions (using mobile devises and applications) in data collection and surveillance systems. Many of the above-mentioned issues are relevant for CMAM programmes in settings of non-emergency, emergency, conflict and fragility. However, there are special circumstance in conflict and fragile settings that need adaptation and simplification of the standard protocols. Because of a broken or partly broken health system in settings of conflict and fragility, local governments are not able to fund access to adequate inpatient and outpatient treatment centres. NGOs and humanitarian agencies are often able to set up stand-alone outpatient therapeutic programmes or mobile centres in the most affected regions. The training of community health volunteers (CHVs) is important and implementing Family MUAC. Importantly, research shows that: Low literacy of CHVs is not a problem to achieve good nutritional outcomes as long as protocols are simplified. Combined/simplified protocols are not inferior to standard protocols. However, due to complexities and low funding, treatment is focused on SAM and availability for children with MAM is far less prioritised, until they deteriorate to SAM. There is widespread confusion about combined/simplified protocol terminology and content, because there is no coherence at the global level.
