Анотація:
The paper deals with the origins and major principles of the Unitarian religion that began to spread in Great Britain in the 18th century. The author aims to reveal the impact of the ethics of this Non-conformist (Dissent) Christian religious thought on the literary works of Elizabeth Gaskell (1810–1865), whose family background was largely Unitarian. The study shows the way that ethical principles of the Unitarian doctrine influence the problem-theme facet of her novels, which is evident in the artistic interpretation of the idea of strengthening the role of women in the Victorian society, in the author’s new approach to the solution to ‘the fallen woman’ problem, based on the possibility to atone for the sin through the service to the good of people and maternal love. The article focuses on the artistic depiction of the evil nature of a lie, the ideas of pacifism, religious tolerance, social justice, and resolution of social problems on the basis of the Christian idea of mutual dependence of humans, as presented in the novels written by Gaskell. The characters of her works, being new for Victorian literature, are also developed on the moral principles of Unitarianism. They are a socially active young woman from the middle class whose efforts are aimed at the resolution of the social conflict and a church minister (a dissenter) suffering from religious or moral doubts. The latter circumstance determines the shift from the depiction of the external social conflict to the internal one, which results in the in-depth psychological insight into the character in Gaskell’s narration. Particular attention is also given to the artistic interpretation of the key Unitarian idea of moral development and perfection of humans and continuous social progress in the novels Ruth (1853), North and South (1855), Sylvia’s Lovers (1863).

UNAIDS aimed to reach 90% antiretroviral therapy uptake among people living with HIV by 2015. Despite free provision of antiretroviral therapy to pregnant women living with HIV, the uptake remains low. This study describes views of pregnant women about low uptake of antiretroviral therapy among pregnant women living with HIV in Mbeya City Council.The study used a descriptive cross-sectional design and applied mixed research approaches. 44 pregnant women on antiretroviral therapy and 12 health providers were conveniently selected. Four key informants were purposively selected. In-depth interviews with pregnant women and key informants; exit interviews with pregnant women; and observation of health providers were conducted. Audio tape recording from in-depth interviews was transcribed into Swahili to English. Social ecological framework was used in data coding based on individual, social and health service aspects then analysed using Atlas. ti 7. Quantitative data was analyzed in Excel spreadsheet, and presented using tables.The findings from interviews and observations showed that the main individual factors (like treatment-related side effects and religious faith); social factors (like traditional medicine and lack of partner/husband support) and health service factors (like long waiting time and discrepancy in information given to clients during adherence counseling on antiretroviral therapy) were more likely to be associated with low uptake of antiretroviral therapy.This study indicates that various factors at individual, social and health service levels were more likely to cause low uptake of ART. Reasons for discrepant information during counseling by providers remain a question which needs further research.

The internet is the one of the most important global network and information source in information age. The internet has changed employee’s life enormously. The purpose of this study is to clarify the benefitting situations of employees from e-employee services. For this purpose, a 20-item data collection tool, based on the e-employee services put forward by the Information Council Turkey was developed. Maintained measurement tool was applied on 515 employees residing different regions and provinces of Turkey in the 2014 academic year. As a result, it was observed that employees benefitted from social, health and education oriented services of e-employee at “medium frequency”, whereas they “rarely” benefitted from the economic-oriented services of e-employee. It was found that women used e-employee services more than men. 30-year of age and younger people used e-employee services more often than older people. In addition, those who have higher incomes use e-employee services more frequently than those who have medium or low incomes. Those working in education and finance sectors benefit from the e-employee services more often than those in other sectors.

This study examined how women and men cope with stress during a period of organizational change and how the coping strategies are related to health. Questionnaire responses were obtained from 98 female and male administrators of health care service working in a Swedish governmental County Council during a period of downsizing. In line with earlier studies, the results showed women to be more likely than men to endorse the emotion-focused strategies of Seeking Social Support and Focus on and Venting of Emotions. Contrary to earlier studies the men did not report using problem-focused strategies more frequently than the women. The results of a regression analysis indicated that the reported use of problem-focused strategies did not contribute significantly to the explained variance in health. Instead, the emotion-focused strategy Denial explained a significant proportion of variance in the health problems. No sex differences in reported health problems were found.

Background. The modern philosophy of social life, where equal rights and opportunities are guaranteed to all, is a trend of world development. Gender approaches are implemented in all areas of society. This is evidenced, in particular, by the Sustainable Development Goals (Goal 5), UN Security Council Resolution 1325 "Women. Peace. Security", the Gender Equality Strategy of the Council of Europe for 2018–2023, the European Charter for the Equality of Women and Men in the Life of Local Communities, the Biarritz Partnership etc. The implementation of gender approaches in various spheres of social life allows us to see existing problems for various social groups and individuals, to offer alternative ways of solving them, which allow us to meet the needs of specific women and specific men. The very understanding of "governance" is based on new views on the work of civil power, which reflect its turning towards citizens, democratization, and are especially important in the conditions of decentralization. The modern understanding of this term, as I. Degtyareva notes, was formed and began to be actively used in the 90s years of the 20th century to denote a modern way of managing a democratic state, when all interested parties are involved in decision-making, which significantly expands the boundaries of traditional management of government bodies. Therefore, we can talk about the transition to gender-oriented governance in modern democratic countries. The purpose of this article is to analyze the gender orientation of modern public administration of Ukraine in the conditions of post-war reconstruction. Object of study. Democratic governance in the post-war reconstruction of Ukraine. Methods. To obtain scientific results, the method of comparative analysis was used to identify similarities/differences in Ukraine's development trends, readiness for joining the EU. A comparison of the study of the projects "Strengthening the role of women in politics and decision-making at the local level" of UNDP and "Gender aspects, development of agriculture and rural areas – Ukraine" of FAO gave grounds to conclude that the processes in the regions of Ukraine are similar to the global trend: the less authority and resources are in power, the more chances women have to come to power. The method of content analysis was also used as a quantitative-qualitative method of studying the speeches of the Head of the National Agency of Ukraine on Civil Service Issues, the Vice-Prime Minister of Ukraine on European and Euro-Atlantic Integration of Ukraine, and people's deputies. Results. In Ukraine, government officials, together with citizens, developed, wrote and implemented the State program for ensuring equal rights and opportunities for women and men, the Government approved the National Action Plan for the Implementation of the Convention on the Elimination of All Forms of Discrimination Against Women, the National Action Plan for the Implementation of UN Security Council Resolution 1325 "Women, peace, security". Departmental plans were developed and adopted for the implementation of this Plan. Amendments to normative legal acts that had discriminatory norms against women are planned and proposed for implementation. We can state that there is a real and rapid process of integration of gender approaches in all spheres of social life. The Government is also developing sectoral documents that will contribute to the development of gender-oriented public governance in the post-war period as well. At the same time, the problem remains the lack of experts on gender issues in public administration, as well as gender stereotypes that prevent effective change in society. Conclusions. Ukraine needs a systematic approach and consolidated actions of public authorities, civil society institutions, and business for a deeper understanding of gender approaches to the development of all spheres of our life. Post-war reconstruction will be a challenge for the implementation of a gender approach in all spheres of public life. At the same time, ensuring justice is the basic approach of public governance in European countries.

BackgroundSevere youth antisocial behaviour has been associated with increased risk of premature mortality in high-risk samples for many years, and some evidence now points to similar effects in representative samples. We set out to assess the prospective association between adolescent conduct problems and premature mortality in a population-based sample of men and women followed to the age of 65 years.MethodA total of 4158 members of the Medical Research Council National Survey of Health and Development (the British 1946 birth cohort) were assessed for conduct problems at the ages of 13 and 15 years. Follow-up to the age of 65 years via the UK National Health Service Central Register provided data on date and cause of death.ResultsDimensional measures of teacher-rated adolescent conduct problems were associated with increased hazards of death from cardiovascular disease by the age of 65 years in men [hazard ratio (HR) 1.17, 95% confidence interval (CI) 1.04–1.32], and of all-cause and cancer mortality by the age of 65 years in women (all-cause HR 1.16, 95% CI 1.07–1.25). Adjustment for childhood cognition and family social class did little to attenuate these risks. Adolescent conduct problems were not associated with increased risks of unnatural/substance-related deaths in men or women in this representative sample.ConclusionsWhereas previous studies of high-risk delinquent or offender samples have highlighted increased risks of unnatural and alcohol- or substance abuse-related deaths in early adulthood, we found marked differences in mortality risk from other causes emerging later in the life course among women as well as men.

The impact of early marriage and reproductive health for adolescents is very influential on all factors, both economic, social, health and educational factors. And early marriage seems to still be happening, including in Indonesia. Especially in Mojokerto Regency, the numbers are very fantastic, the people still do this. Usually the causes of early marriage are cultural and socio-economic factors. Some parents still have the notion that children can be a "savior" of family finances when they get married. There are also those who consider unmarried children to be an economic burden on the family. Actually, the bride who is still in her teens is the most disadvantaged party in early marriage. Because this event will sacrifice the physical and mental development of women. Getting pregnant at a young age and dropping out of school will limit women's career opportunities. In addition, early marriage can increase the risk of domestic violence. Community service activities were held at the P2KBP2 Office Hall, Mojokerto Regency, Jln RA. Basuni No. 19 Sooko Mojokerto. The purpose of this activity is to find out what are the factors behind early marriage and reproductive health for adolescents and what impact this has on the household and the social, economic, educational and health environment of couples who marry early. Partners for community service activities are IIDI Mojokerto Branch and Puspa Majapahit, Mojokerto Regency. The participants of the activity are OSIS representatives for high school students or the equivalent in Mojokerto Regency. The methods in this community service are lectures, discussions, and questions and answers. The result of this drawing is the existence of counseling ambassadors representing 1 youth mass organization or 1 student council member in each high school or equivalent to be able to conduct counseling or socialization in every secondary school in each of these schools or the equivalent in all Mojokerto district.

Climate change presents a considerable threat to human security, with notable gender disproportions. Women’s vulnerability to climate change has implications on agro-based livelihoods, especially the rural populace. The primary purpose of this study was to assess women’s vulnerability to climate change and the gender-skewed implications on agro-based livelihoods in rural Zvishavane, Zimbabwe. A qualitative approach that used purposive sampling techniques was adopted. Data was collected through 20 in-depth interviews with 11 de jure and 9 de facto small-scale female-headed farmer households. Two focus group discussions with mixed de facto and de jure small-scale female-headed farmer households were also conducted. Five key informant interviews were held with departmental heads of the Ministry of Women’s Affairs, Gender and Community Development; the Agriculture Technical Extension Service Department; the Livestock Production Department; the Runde Rural District Council and the Meteorological Services Department. Gendered effects were noted in terms of increased roles and responsibilities for women. Observations showed that there was an increase in distances travelled by women to fetch water owing to a depleted water table. Climate-induced migration of men due to depleted livelihoods in rural areas has also increased roles and responsibilities for women. The traditional male responsibilities assumed by women included cattle herding and ox-driven ploughing. This study concluded that adaptation strategies towards vulnerability to climate change have to be gender-sensitive and area-specific. This study also recommended that response programmes and policies meant to curb existing gendered vulnerabilities should be informed by evidence because climate-change effects are unique for different geographical areas. Moreover, adaptation activities should be mainstreamed in community processes so as to reduce the burden on women and increase sustainability opportunities.

IntroductionOne of the main harms from breast cancer screening is the anxiety caused by false positive results. Various factors may be associated with false-positive anxiety. One modifiable factor may be the method of communication used to deliver results. The aim of this study is to measure the effect on anxiety of receiving benign biopsy results in-person or by telephone.Methods and analysisThis is a multi-centre cluster randomised crossover trial in the English National Health Service Breast Screening Programme (NHSBSP) involving repeated survey measures at four time points. Participants will be women of screening age who have a biopsy following a suspicious mammography result, who ultimately receive a benign or normal (B1) result. Centres will trial both telephone and in-person results on a month-by-month basis, being randomised to which communication method will be trialled first. Women will be blinded to the method of communication they will receive. The analysis will compare women who have received telephone results and women who have received in-person results. The primary outcome measure will be anxiety (measured by the Psychological Consequences Questionnaire) after receiving results, while controlling for baseline anxiety. Secondary outcome measures will include anxiety at 3 and 6 months post-results, understanding of results and patient preferences for how results are communicated. Qualitative telephone interviews will also be conducted to further explore women’s reasons for communication preferences. Qualitative and quantitative data will be integrated after initial separate analysis using the pillar integration process.Ethics and disseminationThis study has been approved by the Public Health England Breast Screening Programme Research Advisory Committee, (BSPRAC_0013, ODR1718_040) and the National Health Service Health Research Authority (HRA) West Midlands—Coventry & Warwickshire Research Ethics Committee (17/WM/0313). Objectives(1) To report maternal and newborn outcomes of pregnant women in areas of social deprivation in inner city London. (2) To compare the effect of caseload midwifery with standard care on maternal and newborn outcomes in this cohort of women.DesignRetrospective observational cohort study.SettingFour council wards (electoral districts) in inner city London, where over 90% of residents are in the two most deprived quintiles of the English Index of Multiple Deprivation (IMD) (2019) and the population is ethnically diverse.ParticipantsAll women booked for antenatal care under Guys and St Thomas’ National Health Service Foundation Trust after 11 July 2018 (when the Lambeth Early Action Partnership (LEAP*) caseload midwifery team was implemented) until data collection 18 June 2020. This included 523 pregnancies in the LEAP area, of which 230 were allocated to caseload midwifery, and 8430 pregnancies from other areas.Main outcome measuresTo explore if targeted caseload midwifery (known to reduce preterm birth) will improve important measurable outcomes (preterm birth, mode of birth and newborn outcomes).ResultsThere was a significant reduction in preterm birth rate in women allocated to caseload midwifery, when compared with those who received traditional midwifery care (5.1% vs 11.2%; risk ratio: 0.41; p=0.02; 95% CI 0.18 to 0.86; number needed to treat: 11.9). Caesarean section births were significantly reduced in women allocated to caseload midwifery care, when compared with traditional midwifery care (24.3% vs 38.0%; risk ratio: 0.64: p=0.01; 95% CI 0.47 to 0.90; number needed to treat: 7.4) including emergency caesarean deliveries (15.2% vs 22.5%; risk ratio: 0.59; p=0.03; 95% CI 0.38 to 0.94; number needed to treat: 10) without increase in neonatal unit admission or stillbirth.ConclusionThis study shows that a model of caseload midwifery care implemented in an inner city deprived community improves outcome by significantly reducing preterm birth and birth by caesarean section when compared with traditional care. This data trend suggests that when applied to targeted groups (women in higher IMD quintile and women of diverse ethnicity) that the impact of intervention is greater.

Introduction: Breast cancer is one of the most incidents and most common among women; surgical intervention is the main treatment, and physiotherapy plays an important role in the postoperative period, especially in the maintenance of range of motion and prevention/control of lymphedema. In the face of the contemporary scenario in which social isolation has been maintained due to the COVID-19 pandemic, the Federal Council of Physiotherapy and Occupational Therapy in Brazil released forms of remote care for patients who need clinical intervention. Objective: This study aims to report the experience of telecare in physiotherapy in the postoperative period of breast cancer surgery, focusing on aspects, pain, fatigue, mobility of the upper limb, and development of lymphedema. Case report: This is an experience report based on the telecare of a 61-year-old mastectomized patient, with 9 consultations, 8 in the remote format. The participant was evaluated regarding health habits, edema, sensation of pain, and tiredness using the Visual Scale of Pain (VAS), the Borg Rating of Perceived Exertion Scale, and a self-authored mastology questionnaire. In the initial evaluation, we found movement limitation (flexion-90º, extension-30º, and abduction-90º), the sensation of tightness in the scar, the sensation of a heavy limb, and dryness on the side of the surgery. In the final evaluation, we observed improvements when compared to the member opposed to surgery, in the gain of range of motion (flexion-180º, extension-45º, and abduction-180º), reduction of pain (VAS) and fatigue (BORG), and improvements in activities of daily living. Conclusion: The service in physical therapy was effective and obtained results similar to those expected in conventional face-to-face physical therapy treatments.

Reproductive tract infections (RTIs) are being increasingly recognized as a serious global health problem with impact on individual women and men, and their families and communities. RTIs can have severe consequences, including infertility, ectopic pregnancy, chronic pelvic pain, miscarriage, and increased risk of HIV transmission. For effective prevention and management of RTIs, accurate information is necessary and should be widely available. In addition to a brief introduction to RTIs, this document contains 13 factsheets addressing medical and social issues on a variety of topics related to RTIs. The document, produced by the Population Council with support from the Ford Foundation, aims to present up-to-date information related to RTIs in a clear and accessible manner. The factsheets are designed for health promoters, program managers, and service providers, and for anyone else involved in the dissemination of health information.

The Ministry of Health and Family Welfare (MOHFW) with financial support from the United States Agency for International Development (USAID) has sponsored the Innovations in Family Planning Services Project (under the management of the State Innovations in Family Planning Services Agency, Lucknow). The project aims to reduce fertility through increasing accessibility, improving quality, and generating demand for family planning services. It attempts to achieve its objectives by supporting service innovations in the public and nongovernmental sector and through social marketing of contraceptives. These intervention strategies are expected to increase the couple protection rate of the state in general and of Kanpur Nagar in particular. One of the prerequisites is to carry out a baseline survey in selected districts of Uttar Pradesh. The BSUP is primarily a household survey with an overall target sample size of 37,000 ever married women ages 13–49 years. The Population Council has collaborated with a number of Indian Consulting Organizations (COs) for survey implementation. Each CO has carried out the survey in one or more districts. The baseline survey was initiated in 15 out of the state’s 63 districts. This report pertains to district Kanpur Nagar.
