Since the seminal report by the Institute of Medicine, To Err Is Human, was issued in 1999, significant efforts across the health care industry have been launched to improve the safety and quality of patient care. Recent advances in the safety of health care delivery have included commitment to creating high-reliability organizations (HROs) to enhance existing quality improvement activities. This article will explore key elements of the HRO concept of deference to expertise, describe the structural elements that support nurses and other personnel in speaking up, and provide examples of practical, evidence-based tools to help organizations support and encourage all members of the health care team to speak up.

Highway Authorities in the United Kingdom have jurisdiction to control, maintain and improve the local highway network, and the Road Traffic Act 1988 places a duty on such authorities to take preventative measures to reduce road casualties. As such, engineers working for the Highway Authority are on the ‘front-line,’ and are required to deal directly with lay concerns relating to road safety. This study investigates the nature and characteristics of how local road safety issues are raised and how engineers respond to such issues in a local authority setting. A grounded theory methodology was applied in the collection and analysis of this data, and in the generation of subsequent emergent themes. Datasets were established containing textual data from correspondence between the lay public and the authority, and from local press reporting. This was augmented by 47 semi-structured interviews with engineers. The analysis demonstrates that road safety issues and their construction, form a distinct genre. There are certain characteristic structural elements and argumentative approaches, which are oft repeated, in lay formulations of road safety. Road safety issues are played out in a contested field, although engineers may have, in theory, the ‘expertise’ that grants them authority to assess, diagnose and implement mitigation measures; in practice they have little autonomy or control. Regulatory restrictions, political interference, resource impoverishment and a volatile public, severely limit engineers’ independence and discretion. In dealing with the exigencies and pressures of day-to-day front-line public service, engineers deploy certain strategies for ‘managing’ the public. These pragmatic strategies are examined in order to establish how engineers can best effect practical action, in the face of competing and often conflicting demands. In examining the rhetorical organisation of lay argumentative strategies, a ‘popular epidemiology’ of road safety is recreated. This term, borrowed from Brown (1992), encapsulates a folk philosophy with respect to accident causation and the measures that are considered necessary or appropriate to ameliorate/eliminate identified issues. It is suggested that in vivo formulations of road safety issues, such as the ‘accident waiting to happen’ are founded on vague premises, and constitute a category mistake. Projections from phenomenally troubling, yet largely unsubstantiable events, to those with profound material consequences, are neither necessary nor certain. In making decisions on substantial capital investments, engineers, by necessity, are required to assess competing sites on a more epistemically secure metric, namely the police road casualty record.

International organisations such as the World Health Organisation highlight the road crash problem in less motorised (or developing, or low income) countries like those in Southeast Asia and recommend the adoption of Western road safety measures. However, there are many differences between highly motorised and less motorised countries which raise questions about how successfully Western road safety knowledge and expertise can be transferred.----- A review of the statistical information on road crashes shows a great deal of uncertainty about both the scale and likely trajectory of road fatalities globally, in less motorised countries and in Asia. It is generally agreed, however, that Asia accounts for around half of all road fatalities, and analysis of the limited available data shows both that Southeast Asia is not an atypical region of Asia in road safety terms, and that Thailand and Vietnam are not atypical of Southeast Asian countries.----- A literature review of recommended practice approaches to road safety transfer in Asia shows that there are many economic, institutional, social and cultural factors which potentially influence the success of transfer. The review also shows that there is no coherent, comprehensive approach which either conceptualises these factors and their relationship to transfer outcomes, or uses an analysis of these factors to plan or modify transfer. To address this gap, this thesis develops a 'road safety space' model as a tool for conceptualisation and analysis, based on a biological metaphor which views the transfer of road safety measures from one context to another as analogous to the transfer of a species into a new ecological space. The road safety space model explicitly considers economic, institutional, social and cultural factors (from specific to broad) which influence the particular road safety issue which a particular road safety transfer effort seeks to address. A central contention of this thesis is that the road safety space model is both a feasible and useful tool to improve the process of road safety transfer to less motorised countries. Road safety space analysis is seen to have a role in a broader process of selection of road safety measures for transfer, along with knowledge of how the measures are considered to operate.----- The research reported in this thesis is comprised of three studies. Study 1 reviewed evaluations of road safety transfer to Thailand and Vietnam. Studies 2 and 3 were case studies of road safety transfer to Thailand and Vietnam respectively.----- Study 1 was an analysis of existing evaluations of road safety transfer to Thailand and Vietnam. The aims were to analyse the evaluations for their consideration of contextual factors, as described in the road safety space model, and to discuss whether the road safety space model assisted in understanding the reasons for success or failure of transfer. However, very few such evaluations exist, and those that were found generally lacked information on whether contextual factors were considered. This indicated the need for a more detailed, in-depth qualitative investigation of particular cases of road safety transfer, in order to investigate the feasibility and utility of the road safety space model.----- Two case studies (Study 2 and Study 3) were conducted to test whether the road safety space approach was both feasible and useful as a means of improving road safety transfer efforts. Study 2 was a case study of the development and implementation in Thailand of a road safety education program for school children, which involved the transfer of Western research and techniques. The transfer was funded by the World Bank and managed by the Thai Ministry of Education (MOE). Study 3 was a case study of the development and implementation of a motorcycle helmet wearing program in Vietnam, which involved the transfer of Western knowledge, techniques and technology. The transfer agents were staff of Asia Injury (AI), a non-government organisation (NGO), and the program was funded initially by a charitable fund, with the intention of becoming self-funding through operation of a helmet factory.----- The case studies employed background research into existing information on economic, institutional, social and cultural factors relevant to the road safety issues (road use behaviour of school children in Thailand and motorcycle helmet purchase and wearing in Vietnam), and collected data through interviews with key informants, analysis of secondary sources and observations. This information was used to derive the road safety space for each road safety issue, to identify the road safety space recognised and addressed by the transfer agents (ARRB and AI), and to determine which factors they missed, or were aware of but took no action on. The focus of this analysis was on the processes used in transfer, not on the road safety outcomes of transfer, although these provided information on the processes as well. Available evaluation information was used to draw links between the omissions and the success of the transfer processes. It was noted that information on how the transferred measures operate should come from a road safety space analysis in the originating country, although this raised questions about selection of country and time (when the measure was first introduced, or in its maturity).----- The feasibility and utility of the road safety space model were discussed. It was clear that the model provided information on the cases which was missed by the transfer agents. The questions examined next were whether this information could have been obtained from an exercise conducted before the transfer had commenced, whether the required effort and cost justified the potential benefits, and whether the information on the road safety space could have been useful for the transfer agents. Comparisons between the road safety spaces for the two cases showed some areas of commonality, e.g. perceptions of police corruption, but also many differences. It was considered likely that some broad factors could be generic, and the possibility was mooted that less motorised countries share issues with police enforcement. This requires further research, however, and at this stage it is better to treat each road safety space as a unique combination of contextual factors influencing the road safety issue of interest.----- It is concluded that the road safety space model is feasible if used in such a way as to minimise the research involved, and useful, although the degree of utility needs to be further explored in a prospective study. The limitation introduced by restricting informants to those who could speak English are discussed. An approach using road safety space analysis is recommended, emphasising analysis of the country to which the road safety measure is being transferred, supplemented by analysis of the originating country road safety space. Gaps in knowledge are identified for further research and development, in particular the theoretical and practical understanding of road use behaviours and their modification in less motorised countries in Southeast Asia. Elaboration of the model is also recommended, to take into account the influence of the type of measure transferred, the role of the transfer agent, the area of road safety (education, engineering or enforcement), and the time dimension (the time which might be needed for a transfer to show its effects).----- The findings of this research are likely to be applicable to road safety transfer in other less motorised regions of the world, however prospective testing is needed. They may also be relevant to issues of transfer for areas other than road safety, in particular public health and traffic engineering, where similar economic, institutional, social and cultural issues come together.

The Offshore Installation Manager (OIM) performs one of the most complex and critical roles within the UK offshore oil and gas industry. The OIM is responsible for the safety and well-being of all persons onboard and within 500m of the installation whilst operating within an isolated and hostile physical environment within the UK Continental Shelf . The persistence of major incidents in the industry highlights the importance of OIM competence in controlling emergencies, consequently the research aims to contribute to new knowledge by identifying the potential barriers that can prevent the effective competence assessment of an OIM in this situation. The research utilised Engeström's Expanded Activity Theory to establish the holistic socio-cultural environment within which the OIM operates during emergency response to establish the key documents relevant to the role and associated competence assessment. The documents were then analysed using a critical hermeneutic approach based on the work of Phillips and Brown (1993) to determine the potential barriers to effective competence assessment of the OIM in controlling emergencies. The research is interdisciplinary and aims to develop new knowledge within the domains of safety engineering and law. The output from the research will assist industry Duty Holders improve the competence assessment of their appointed OIMs and allow safety engineers to consider the probable competence of an OIM at a given point in time. The research has identified 8 potential barriers to the effective competence assessment of OIMs in controlling emergencies. These are associated with the multiple definitions of competence that exist; the predominance of competence assessment by observation; the lack of definitive competence requirements for trainers and assessors; the complexity in UK legislation and the interaction between UK Government departments; concerns associated with goal setting legislation when applied to competence assessment and the lack of statutory competence requirements for OIMs in controlling emergencies; the lack of ownership of OIM competence and competence assessment; and the lack of regulation and independent audit of the competence assessment process of OIMs in controlling emergencies.

Entre 1946 et 2016, ce sont plus de vingt cas de recours à des experts par les instances de représentation du personnel qui ont été créés, dont huit sur les trois dernières années. La possibilité sans cesse étendue de recourir à l’assistance d’experts pourrait laisser penser que le régime du recours à l’expertise est efficient. La facilité avec laquelle la loi associe désormais l’expert à chaque nouvelle mesure prise en faveur du dialogue social masque cependant mal l’incapacité croissante du législateur à préserver la fonction initiale de l’expertise : une fonction informative. Signe des dysfonctionnements qui grèvent le régime actuel du recours à l’expertise, le sujet est hautement polémique et fait l’objet d’un contentieux florissant. L’étude du périmètre ainsi que des modalités du recours à des experts exhorte à la construction d’un régime plus cohérent, de nature à assurer l’effet utile de l’expertise tout en préservant la compétence et la parole des représentants du personnel
Between 1946 and 2016, this is more than twenty new cases in which the employee representatives were granted the right to resort to the services of experts, and only eight in the last three years. The ever-expanded possibility to be assisted by external experts might suggest that the expertise statutory regime is efficient. The ease with which the law associates experts to each new measures taken in favor of the social dialogue cannot hide the persistent difficulties encountered by the legislator to preserve the original function of expertise : an informative function. The controversial nature of the right to resort to the services of experts and the increasing amount of litigation in this area are manifestations of its failures. A study of the scope and the conditions under which the experts might be appointed urges to the construction of a more coherent system, that would ensure the effectiveness of the right to resort to experts while maintaining the employee representatives’ competence and voices

<p>Society expects that buildings and other structures are safe for the people who use them or who are near them. The failure of a building or structure is expected to be an extremely rare event. Thus, society implicitly relies on the expertise of the professionals involved in the planning, design, construction, operation and maintenance of the structures it uses.<p>Structural engineers devote all their effort to meeting society’s expectations effi ciently. Engineers and scientists work together to develop solutions to structural problems. Given that nothing is absolutely and eternally safe, the goal is to attain an acceptably small probability of failure for a structure, a facility, or a situation. Reliability analysis is part of the science and practice of engineering today, not only with respect to the safety of structures, but also for questions of serviceability and other requirements of technical systems that might be impacted by some probability.<p>The present volume takes a rather broad approach to safety and reliability in Structural Engineering. It treats the underlying concepts of safety, reliability and risk and introduces the reader in a fi rst chapter to the main concepts and strategies for dealing with hazards. The next chapter is devoted to the processing of data into information that is relevant for applying reliability theory. Two following chapters deal with the modelling of structures and with methods of reliability analysis. Another chapter focuses on problems related to establishing target reliabilities, assessing existing structures, and on effective strategies against human error. The last chapter presents an outlook to more advanced applications. The Appendix supports the application of the methods proposed and refers readers to a number of related computer programs.<p>This book is aimed at both students and practicing engineers. It presents the concepts and procedures of reliability analysis in a straightforward, understandable way, making use of simple examples, rather than extended theoretical discussion. It is hoped that this approach serves to advance the application of safety and reliability analysis in engineering practice.<p>The book is amended with a free access to an educational version of a Variables Processor computer program. FreeVaP can be downloaded free of charge and supports the understanding of the subjects treated in this book.

Empirical evidence shows that economic regulators uncritically accept this advice. Such outsourcing raises the possibility of significant impact on technical regulatory outcomes.

Abstract This book chapter focuses on a programme on improving human health through livestock research in three areas: (i) animal-source foods for nutrition; (ii) zoonoses (diseases transmitted between animals and people); and (iii) FBD. This was the first CGIAR group with an explicit food safety mandate (rather than focusing on specific hazards) and with expertise in using research methods for food safety rather than diseases in general. ILRI was also one of the first groups to focus on food safety in the 'informal markets' of developing countries, and by the 2010s, had become the lead research institute globally in this emerging area. ILRI research on FBD has resulted in many science outputs, including some genuinely innovative tools and approaches, and has already demonstrated outcomes at community, national and regional levels. These include substantial inputs into global, regional and national strategies and national training programmes. The major development-oriented approach - the triple-path for training, motivating and enabling of informal market agents - has been shown to be both scalable and sustainable. While questions remain about its lasting effects on food safety and its application outside those few countries where its success has been demonstrated, the next few years should bring further evidence about this, with benefits lasting for many decades to come.

In 2019, the 72nd World Health Assembly issued a call to action, The Global Action on Patient Safety, that called for Member States to democratize healthcare by engaging with the very users of the healthcare system—patients, families, and community members—along with other partners—in the “co-production” of safer healthcare.The WHO’s Patients for Patient Safety (PFPS) Programme, guided by the London Declaration, addresses this global concern by advancing co-production efforts that demonstrate the powerful and important role that civil society, patients, families, and communities play in building harm reduction strategies that result in safer care in developing and developed countries. The real-world examples from the PFPS Programme and Member States illustrate how civil society as well as patients, families, and communities who have experienced harm from unsafe care have harnessed their wisdom and courageously partnered with passionate and forward-thinking leaders in healthcare including clinicians, researchers, policy makers, medical educators, and quality improvement experts to co-produce sustainable patient safety initiatives. Although each example is different in scope, structure, and purpose and engage different stakeholders at different levels, each highlights the necessary building blocks to transform our healthcare systems into learning environments through co-production of patient safety initiatives, and each responds to the call made in the London Declaration, the WHO PFPS Programme, and the World Health Assembly to place patients, families, communities, and civil society at the center of efforts to improve patient safety.

The French utility has organized the aging degradation assessment for nuclear plants in operation in maximum safety rules. Also the utility decided to engaged large program of expertises, for each degradations identified, based on research and development activities and prediction criteria program of Nuclear Plants in function of several actions: expertises, data bases on characteristics, etc. This paper shows the expertise results and thermal aging for cast duplex stainless steel studies on reactor coolant circuit components engaged to evaluate and to monitor the toughness evaluation and increasing factor. Also this paper presents the strategy associated. This paper shows the applications were evolved for 3-loop PWR plants. The monitoring was mainly oriented on evaluation of the ratio computation and measurement. This integrity assessment and expertises results values available periodically were performed considering: • the life evaluation of the plants and alternative maintenance actions, • the large database from cast reactor coolant component assessed after removed from nuclear power plants, • the identification of degradation for different components and prediction criteria proposed. The results obtained are updated in the periodic maintenance program and in volume of expertise database for life management.

Risk management processes follow relatively well established steps, but the hazard identification and risk assessment steps often depend heavily on “experts” with relevant expertise and operating experience. This paper presents a tool developed to help novice researchers identify and assess hazards more efficiently. The hazard identification tool was developed in the form of an updatable algorithm, based initially on previous risk assessments for similar situations as well as checklists and guidelines available from academic texts, Occupational Safety and Health Act (OSHA), and industry. Major risk categories included in the algorithm include: a) Ergonomics and Mechanical, b) Chemical, c) Physical (including Electrical Hazards, Fire Safety, Noise Hazards, and Radiation Hazards), d) Psychological and Organizational, and e) Biological. In the initial test of the algorithm, a team of non-experts used the algorithm to identify risks for two Ohio Coal Research Center (OCRC) projects dealing with solid oxide fuel cell and electrostatic precipitator testing. Their results were comparable to the list of risks generated by a group of “research experts” without the algorithm. Future plans include making the algorithm available on a wiki platform to collaboratively develop it with the combined knowledge, experience and perspectives of all participating researchers. This paper also describes the overall safety plan for the Ohio Coal Research Center (OCRC), which operates within the Institute for Sustainable Energy and the Environment (ISEE) at Ohio University. An OCRC safety flow diagram is presented that has been successfully used to improve the safety of new research projects being led by new researchers.

On-line equipment condition monitoring is a critical component of the world-class production and safety histories of many successful nuclear plant operators. From addressing availability and operability concerns of nuclear safety-related equipment to increasing profitability through support system reliability and reduced maintenance costs, Predictive Maintenance programs have increasingly become a vital contribution to the maintenance and operation decisions of nuclear facilities. In recent years, significant advancements have been made in the quality and portability of many of the instruments being used, and software improvements have been made as well. However, the single most influential component of the success of these programs is the impact of a trained and experienced team of personnel putting this technology to work. Changes in the nature of the power generation industry brought on by competition, mergers, and acquisitions, has taken the historically stable personnel environment of power generation and created a very dynamic situation. As a result, many facilities have seen a significant turnover in personnel in key positions, including predictive maintenance personnel. It has become the challenge for many nuclear operators to maintain the consistent contribution of quality data and information from predictive maintenance that has become important in the overall equipment decision process. These challenges can be met through the implementation of quality training to predictive maintenance personnel and regular updating and re-certification of key technology holders. The use of data management tools and services aid in the sharing of information across sites within an operating company, and with experts who can contribute value-added data management and analysis. The overall effectiveness of predictive maintenance programs can be improved through the incorporation of newly developed comprehensive technology training courses. These courses address the use of key technologies such as vibration analysis, infrared thermography, and oil analysis not as singular entities, but as a toolbox resource from which to address overall equipment and plant reliability in a structured program and decision environment.

As road safety issues become increasingly complex and involve various stakeholders, designing of safety actions as on-board vehicle safety systems or public policy measures needs knowledge that describes the performance level to fulfil the objectives. Performance is assessed by evaluating designed actions as regards their functional specifications. By its ability to create new knowledge, evaluation is also used to improve existing actions or to specify road safety strategies. However, given the complexity of the evaluation activity and its context, experts consider that the existing knowledge is insufficient. One considers that this situation reflects the lack of a theoretical framework. In particular, there is no way to identify emerging assessment issues due to the insertion of new safety systems and the emergence of new users’ behaviours. In this paper, we introduce a framework that describes the design of the evaluation methods in the field of road safety. It is used as a guide to build new evaluation models for specific stakeholders and viewpoints. This framework is based on the identification of the objectives to be achieved and the use of the evaluators’ expertise.

Abstract This paper details experiences gained while developing a novel technology-driven approach to Risk Assessment methodologies, e.g., Process Hazard Analysis (PHA), Hazard Identification (HAZID) and Hazard Operability (HAZOP), in oil & gas. Emphasis has been placed on combining encoded human knowledge with Artificial Intelligence techniques, in a way which fosters safer designs and operations, while maintaining Subject Matter Experts (SMEs) at the center of decision making. Encoding of human knowledge (e.g., Subject Matter Expertise, Industry best practices) in digital applications has traditionally been associated with creating static pieces of information, such as lessons learned documentation and validation activities for hazard analysis. New digital technologies, however, make it possible to create truly dynamic knowledge representations, which capture key concepts and their relationships, creating a new type of "source of truth." As a result, corporate and external knowledge can be made more readily accessible to engineers and operations personnel participating in decision making. Digital corporate knowledge can also be supplemented with Artificial Intelligence (AI) techniques which can help uncover latent threats and better guide optimal decision making. This is particularly relevant in Workforce, Health & Safety (WH&S) and Process Safety contexts, where the impact of flawed or suboptimal decisions can lead to catastrophic consequences. Practical examples from an oil & gas major show how the risk assessment domain can be represented in a computational knowledge graph, in a format which is comprehensible not only to software developers, but more importantly, to oil & gas SMEs. A presentation of different AI techniques overlaid on top of this computational knowledge graph, can also offer a glimpse of the possibilities of marrying SME expertise with emerging digital technologies.

Rapid technological innovation is reshaping the UK food system in many ways. FSA needs to stay abreast of these changes and develop regulatory responses to ensure novel technologies do not compromise food safety and public health. This report presents a rapid evidence assessment of the emerging technologies considered most likely to have a material impact on the UK food system and food safety over the coming decade. Six technology fields were identified and their implications for industry, consumers, food safety and the regulatory framework explored. These fields are: Food Production and Processing (indoor farming, 3D food printing, food side and byproduct use, novel non-thermal processing, and novel pesticides); Novel Sources of Protein, such as insects (for human consumption, and animal feedstock); Synthetic Biology (including lab-grown meat and proteins); Genomics Applications along the value chain (for food safety applications, and personal “nutrigenomics”); Novel Packaging (active, smart, biodegradable, edible, and reusable solutions); and, Digital Technologies in the food sector (supporting analysis, decision making and traceability). The report identifies priority areas for regulatory engagement, and three major areas of emerging technology that are likely to have broad impact across the entire food industry. These areas are synthetic biology, novel food packaging technologies, and digital technologies. FSA will need to take a proactive approach to regulation, based on frequent monitoring and rapid feedback, to manage the challenges these technologies present, and balance increasing technological push and commercial pressures with broader human health and sustainability requirements. It is recommended FSA consider expanding in-house expertise and long-term ties with experts in relevant fields to support policymaking. Recognising the convergence of increasingly sophisticated science and technology applications, alongside wider systemic risks to the environment, human health and society, it is recommended that FSA adopt a complex systems perspective to future food safety regulation, including its wider impact on public health. Objectives: (1) Evaluate Immunocompetence-OTL-containing Chromosomal Regions (ICRs), marked by microsatellites or candidate genes, for magnitude of direct effect and for contribution to relationships among multiple immunocompetence, disease-resistance, and growth traits, in order to estimate epistatic and pleiotropic effects and to predict the potential breeding applications of such markers. (2) Evaluate the interaction of the ICRs with genetic backgrounds from multiple sources and of multiple levels of genetic variation, in order to predict the general applicability of molecular genetic markers across widely varied populations. Background: Diseases cause substantial economic losses to animal producers. Emerging pathogens, vaccine failures and intense management systems increase the impact of diseases on animal production. Moreover, zoonotic pathogens are a threat to human food safety when microbiological contamination of animal products occurs. Consumers are increasingly concerned about drug residues and antibiotic- resistant pathogens derived from animal products. The project used contemporary scientific technologies to investigate the genetics of chicken resistance to infectious disease. Genetic enhancement of the innate resistance of chicken populations provides a sustainable and ecologically sound approach to reduce microbial loads in agricultural populations. In turn, animals will be produced more efficiently with less need for drug treatment and will pose less of a potential food-safety hazard. Major achievements, conclusions and implications:. The PI and co-PIs had developed a refined research plan, aiming at the original but more focused objectives, that could be well-accomplished with the reduced awarded support. The successful conduct of that research over the past four years has yielded substantial new information about the genes and genetic markers that are associated with response to two important poultry pathogens, Salmonella enteritidis (SE) and Escherichia coli (EC), about variation of immunocompetence genes in poultry, about relationships of traits of immune response and production, and about interaction of genes with environment and with other genes and genetic background. The current BARD work has generated a base of knowledge and expertise regarding the genetic variation underlying the traits of immunocompetence and disease resistance. In addition, unique genetic resource populations of chickens have been established in the course of the current project, and they are essential for continued projects. The US laboratory has made considerable progress in studies of the genetics of resistance to SE. Microsatellite-marked chromosomal regions and several specific genes were linked to SE vaccine response or bacterial burden and the important phenomenon of gene interaction was identified in this system. In total, these studies demonstrate the role of genetics in SE response, the utility of the existing resource population, and the expertise of the research group in conducting such experiments. The Israeli laboratories had showed that the lines developed by selection for high or low level of antibody (Ab) response to EC differ similarly in Ab response to several other viral and bacterial pathogens, indicating the existence of a genetic control of general capacity of Ab response in young broilers. It was also found that the 10w-Ab line has developed, possibly via compensatory "natural" selection, higher cellular immune response. At the DNA levels, markers supposedly linked to immune response were identified, as well as SNP in the MHC, a candidate gene responsible for genetic differences in immunocompetence of chickens.

A geologic map poster illustrates the geology GRI GIS data set and serves as a primary figure for this GRI report. No poster was prepared for the geohazards GRI GIS data set. Additionally, figure 7 of this report illustrates the locations of the major geologic features in the parks. Unlike the poster, which is divided into a northern and southern portion to show detail while accommodating the parks’ length, figure 7 is a single-page, simplified map. The features labeled on figure 7 are discussed in the “Geologic History, Features, and Processes” chapter. To provide a context of geologic time, this report includes a geologic time scale (see "Geologic History, Features, and Processes"). The parks’ geologic story encompasses 200 million years, starting in the Jurassic Period. Following geologic practice, the time scale is set up like a stratigraphic column, with the oldest units at the bottom and the youngest units at the top. Organized in this manner, the geologic time scale table shows the relative ages of the rock units that underlie the parks and the unconsolidated deposits that lie at the surface. Reading the “Geologic Event” column in the table, from bottom to top, will provide a chronologic order of the parks’ geologic history. The time scale includes only the map units within the parks that also appear on the geologic map poster; that is, map units of the geohazards data are not included. Geology is a complex science with many specialized terms. This report provides definitions of geologic terms at first mention, typically in parentheses following the term. Geologic units in the GRI GIS data are referenced in this report using map unit symbols; for example, map unit KJfrc stands for the Cretaceous (K) and Jurassic (J) Franciscan Complex (f), Redwood Creek schist (rc), which underlies a portion of the Redwood Creek watershed (see “GRI Products”).

Driving is one of the riskiest work tasks, accounting for around one third of fatal crashes in the UK. Organisations are expected to manage work-related road safety (WRRS) in the same way that they manage other health and safety risks. The Health and Safety Executive (HSE) and Department for Transport (DFT) issue joint guidance on this in INDG382 ‘Driving at work: managing work-related road safety’. HSE and DFT were seeking to update INDG382 to include reference to vehicle safety technologies that could enable employers to monitor safety related events or driver behaviours, to support learning and safety improvements. They commissioned TRL to - Conduct a literature review focused on evaluations of the impact of these technologies on work-related road safety (specifically, crash risk) Lead in-depth interviews with representatives of organisations who had implemented technology-based safety monitoring in their fleet and stakeholders and experts who provided further insights into factors affecting successful implementation. TRL found that telematics systems, drowsiness and distraction recognition systems, and collision warning systems have significant potential safety benefits, but rigorous published evaluation of safety-focused telematics in the fleet context is limited. There is good evidence for the safety benefits of intelligent speed assist in private and fleet vehicles. Successful implementation relies on procuring systems that match needs, managing the potential for data to overwhelm and embedding monitoring and driver feedback within good management systems and strong safety leadership. This report provides recommendations for updating guidance for organisations considering implementing vehicle safety monitoring technologies (telematics).

The objective of the SARex exercise, conducted north of Spitzbergen in ice-infested water in late April 2016, was to identify and explore the gaps between the functionality provided by the existing SOLAS (International Convention for Safety of Life at Sea) approved safety equipment and the functionality required by the Polar Code. The exercise was a joint collaboration between the Norwegian Coast Guard (using the Coast Guard vessel KV Svalbard as the exercise platform), experts from industry, governmental organizations and academia. The exercise scenario was to be along the lines of a “Maxim Gorkiy scenario”, where an expedition cruise ship sinks in the marginal ice zone north of the coast of Svalbard.

Public, fixed-route transit services most commonly operate on public streets. In addition, transit passengers must use sidewalks to access transit stops and stations. However, streets and sidewalks are under the jurisdiction of municipalities, not transit agencies. Various municipal policies, practices, and decisions affect transit operations, rider convenience, and passenger safety. Thus, these government entities have an important influence over the quality, safety, and convenience of transit services in their jurisdictions. This research identified municipal policies and practices that affect public transport providers’ ability to deliver transit services. They were found from a comprehensive literature review, interviews and discussions with five local transit agencies in the U.S., five public transportation experts and staff from five California cities. The city policies and practices identified fall into the following five categories: Infrastructure for buses, including bus lanes, signal treatments, curbside access; Infrastructure for pedestrians walking and bicycling to, and waiting at, transit stops and stations; Internal transportation planning policies and practices; Land development review policies; Regional and metropolitan planning organization (MPO) issues. The understanding, acknowledgment, and implementation of policies and practices identified in this report can help municipalities proactively work with local transit providers to more efficiently and effectively operate transit service and improve passenger comfort and safety on city streets.

As research continues into how fire department interventions affect fire dynamics in the modern fire environment, questions continue to arise on the impact and implications of interior versus exterior fire attack on both occupant survivability and firefighter safety. This knowledge gap and lack of previous research into the impact of fire streams has driven the need for further research into fire department interventions at structure fires with a focus on hose streams and suppression tactics. As the third report in the project “Impact of Fire Attack Utilizing Interior and Exterior Streams on Firefighter Safety and Occupant Survival”, this report expands upon the fire research conducted to date by analyzing how firefighting tactics, specifically suppression methods, affect the thermal exposure and survivability of both building occupants and firefighters in residential structures. • Part I: Water Distribution • Part II: Air Entrainment • Part III: Full-Scale Residential Fire Experiments. This report evaluates fire attack in residential structures through twenty-six full-scale structure fire experiments. Two fire attack methods, interior and transitional, were preformed at UL’s large fire lab in Northbrook, IL, in a single-story 1,600 ft2 ranch test structure utilizing three different ventilation configurations. To determine conditions within the test structure it was instrumented for temperature, pressure, gas velocity, heat flux, gas concentration, and moisture content. Ad- ditionally, to provide information on occupant burn injuries, five sets of instrumented pig skin were located in pre-determined locations in the structure. The results were analyzed to determine consistent themes in the data. These themes were evaluated in conjunction with a panel of fire service experts to develop 18 tactical considerations for fire ground operations. As you review the following tactical considerations it is important to utilize both these research results and your per- sonal experience to develop your department’s polices and implement these considerations during structural firefighting.

Objective. To conduct a systematic review of the literature on the safety of vaccines recommended for routine immunization in the United States, updating the 2014 Agency for Healthcare Research and Quality (AHRQ) report on the topic. Data sources. We searched MEDLINE®, Embase®, CINAHL®, Cochrane CENTRAL, Web of Science, and Scopus through November 9, 2020, building on the prior 2014 report; reviewed existing reviews, trial registries, and supplemental material submitted to AHRQ; and consulted with experts. Review methods. This report addressed three Key Questions (KQs) on the safety of vaccines currently in use in the United States and included in the Centers for Disease Control and Prevention’s (CDC) recommended immunization schedules for adults (KQ1), children and adolescents (KQ2), and pregnant women (KQ3). The systematic review was supported by a Technical Expert Panel that identified key adverse events of particular concern. Two reviewers independently screened publications; data were extracted by an experienced subject matter expert. Studies of vaccines that used a comparator and reported the presence or absence of adverse events were eligible. We documented observed rates and assessed the relative risks for key adverse events. We assessed the strength of evidence (SoE) across the existing findings from the prior 2014 report and the new evidence from this update. The systematic review is registered in PROSPERO (CRD42020180089). Results. A large body of evidence is available to evaluate adverse events following vaccination. Of 56,608 reviewed citations, 189 studies met inclusion criteria for this update, adding to data in the prior 2014 report, for a total of 338 included studies reported in 518 publications. Regarding vaccines recommended for adults (KQ1), we found either no new evidence of increased risk for key adverse events with varied SoE or insufficient evidence in this update, including for newer vaccines such as recombinant influenza vaccine, adjuvanted inactivated influenza vaccine, and recombinant adjuvanted zoster vaccine. The prior 2014 report noted a signal for anaphylaxis for hepatitis B vaccines in adults with yeast allergy and for tetanus, diphtheria, and acellular pertussis vaccines. Regarding vaccines recommended for children and adolescents (KQ2), we found either no new evidence of increased risk for key adverse events with varied SoE or insufficient evidence, including for newer vaccines such as 9-valent human papillomavirus vaccine and meningococcal B vaccine. The prior 2014 report noted signals for rare adverse events—such as anaphylaxis, idiopathic thrombocytopenic purpura, and febrile seizures—with some childhood vaccines. Regarding vaccines recommended for pregnant women (KQ3), we found no evidence of increased risk for key adverse events with varied SoE among either pregnant women or their infants following administration of tetanus, diphtheria, and acellular pertussis vaccines during pregnancy. Conclusion. Across this large body of research, we found no new evidence of increased risk since the prior 2014 report for key adverse events following administration of vaccines that are routinely recommended. Signals from the prior report remain unchanged for rare adverse events, which include anaphylaxis in adults and children, and febrile seizures and idiopathic thrombocytopenic purpura in children. There is no evidence of increased risk of adverse events for vaccines currently recommended in pregnant women. There remains insufficient evidence to draw conclusions about some rare potential adverse events.

The National Institute of Justice (NIJ) and its Forensic Technology Center of Excellence (FTCoE) hosted the National Opioid and Emerging Drug Threats Policy and Practice Forum on July 18–19, 2019, in Washington, DC. The forum explored ways in which government agencies and programs, law enforcement officials, forensic laboratory personnel, medical examiners and coroners, researchers, and other experts can cooperate to respond to problems associated with drug abuse and misuse. Panelists from these stakeholder groups discussed ways to address concerns such as rapidly expanding crime laboratory caseloads; workforce shortages and resiliency programs; analytical challenges associated with fentanyl analogs and drug mixtures; laboratory quality control; surveillance systems to inform response; and policy related to stakeholder, research, and resource constraints. The NIJ Policy and Practice Forum built off the momentum of previous stakeholder meetings convened by NIJ and other agencies to discuss the consequences of this national epidemic, including the impact it has had on public safety, public health, and the criminal justice response. The forum discussed topics at a policy level and addressed best practices used across the forensic community.

To address the challenges in pandemic preparedness and response (PPR), the World Health Assembly (WHA), at a special session in November 2021, established an Intergovernmental Negotiating Body (the INB) and tasked it with drafting a new legal instrument for PPR. During its second meeting in July 2022, the INB decided to develop the accord under Article 19 of the WHO Constitution, which grants the WHO the authority to negotiate a legally-binding Convention or Agreement and requires ratification by countries according to their local laws to enter into force. The aim is to complete negotiations and adopt a new pandemic instrument at the WHA in May 2024. The new legally binding agreement aims to address many of the failures exposed by the COVID-19 pandemic. However, the adoption of such an agreement is not the end of the process but the beginning. The negotiations on the instrument must establish a mechanism to monitor countries' compliance with the accord, particularly on the legally-binding elements. In this paper, we recommend creating such a mechanism as part of the accord: an independent committee of experts that monitors state parties' compliance with the pandemic accord and the timeliness, completeness, and robustness of states’ reports on their obligations. Its primary purpose would be to verify state self-reports by triangulating them with a range of publicly available information, making direct inquiries, and accepting confidential submissions. It would report its findings to a body consisting of or that is directly accountable to heads of state, with a particular focus on elevating instances of non-compliance or inadequate reporting. Its reports would also be available to the public. The proposed design builds on the analysis of strengths and weaknesses of existing monitoring approaches to 11 international treaties and mechanisms within and outside of health, a review of the literature, and interviews and input from more than 40 experts from around the world.
