Whole-body cryotherapy (WBC) has been frequently used to supplement the rehabilitation of patients with rheumatoid arthritis (RA). The aim of this study was to compare the effect of WBC and traditional rehabilitation (TR) on clinical parameters and systemic levels of IL-6, TNF-αin patients with RA. The study group comprised 25 patients who were subjected to WBC (−110°C) and 19 patients who underwent a traditional rehabilitation program. Some clinical variables and levels of interleukin-6 (IL-6) and tumor necrosis factor-α(TNF-α) were used to assess the outcomes. After therapy both groups exhibited similar improvement in pain, disease activity, fatigue, time of walking, and the number of steps over a distance of 50 m. Only significantly better results were observed in HAQ in TR group (p< 0.05). However, similar significant reduction in IL-6 and TNF-αlevel was observed. The results showed positive effects of a 2-week rehabilitation program for patients with RA regardless of the kind of the applied physical procedure.

Background:Rehabilitation methods and standards for patients with rheumatoid arthritis (RA) have significantly changed due to more efficient medication improving the course of the disease. Therefore, physical activity, participation, disease management and patient education are most important goals in rehabilitation of patients with RA.Objectives:Aim of this study was to evaluate the significance and impact of rehabilitation methods according to the subjective attitudes and views of experts and professionals in the field of RA. Opinions of members of the task force (TF) “Rehabilitation” of the Austrian Society of Rheumatology (ÖGR) were compared to the estimation of the other members of the ÖGR.Methods:All members of the ÖGR were invited to participate in an online survey to rate the impact of rehabilitation for patients with RA between 0 (no impact) and 10 (high impact). Besides sociodemographic and experience related data about the experts and professionals, two main issues were investigated: (1) Impact of rehabilitation related to specific interventions (2) Impact of rehabilitation methods for patients with RA according to different disease and treatment points.Results:129 members (50% male, 50% female) of the ÖGR participated in the online survey. 12 persons were members of the TF “Rehabilitation” of the ÖGR. 11 (8.6%) respondents were general physicians, 66 (51.6%) specialists in internal medicine with further expertise in rheumatology, 15 (11.5%) specialists in internal medicine, 14 (10.9%) specialists for physical medicine with further expertise in rheumatology, 2 (1.6%) specialists in orthopaedics, 13 (10.2%) health professionals and 7 (5.5%) persons were from other profession categories such as researchers for example. The majority of respondents (80%) worked already more than five years with patients with RA in a stationary setting. Results of the online survey demonstrate that the ranking of the impact of specific rehabilitation interventions did only marginally differ between the two person groups: Both groups ranked the importance of occupational and physical therapy the highest. Only the subjective importance of splints and assistive technologies was higher assessed by the general members of the ÖGR. Further, the ranking about the estimated impact of rehabilitation methods for patients with RA was very similar between the two person groups: The importance of rehabilitation for patients with functionality restrictions and for patients with RA in the first years of their disease was ranked the highest by both groups. Conclusion:Results of the online survey demonstrate that ratings related to the impact of rehabilitation interventions for RA patients do only slightly differ between the investigated member groups of the ÖGR. Finally, the results indicate that rehabilitation methods for RA patients and rehabilitation related knowledge are well accepted and successfully transferred into disease management of patients with RA by professionals and experts in rheumatology in Austria.Disclosure of Interests:Claudia Oppenauer: None declared, Elke Böttcher Grant/research support from: UCB,Roche, MSD, Speakers bureau: UCB, Pfizer, BMS,MSD,Roche,Amgen, Lilly, Gabriele Eberl: None declared, Albrecht Falkenbach: None declared, Winfried Habelsberger: None declared, Johannes Kirchheimer: None declared, Werner Kullich Shareholder of: Roche Stocks, Grant/research support from: MedTec Company, Germany - MedizinTechnik, Erich Mur: None declared, Christa Oliveira-Sittenthaler: None declared, Georg Stummvoll Speakers bureau: Lilly, Roche, gsk, Christian Wiederer Consultant of: Firma Grünenthal, Thema Capsaicin, Dezember 2019, Harald Zeindler: None declared, Valerie Nell-Duxneuner Speakers bureau: MSD, Pfizer, Jansen, Abbvie, Lilly, Novartis

Rheumatoid arthritis (RA) is an autoimmune disease that affects joint tissues and causes severe physical and functional impairments on quality of life due to muscular and articular pain. The involvement of temporomandibular joint in RA interferes with mouth opening and masticatory process. However, no studies addressed the impact of RA on oral health-related quality of life (OHRQoL) and satisfaction with prostheses use in elderly people. Aim: This study assessed the impact of oral rehabilitation with conventional dentures on the OHRQoL and prostheses satisfaction in elderly patients with RA, associated or not with temporomandibular disorder (TMD). Methods: Forty-five elderly were enrolled and divided into three groups: (1) RA and TMD (n=15, experimental), (2) RA without TMD (n=15, experimental), and (3) without RA and without TMD (n=15, control). The OHRQoL and the prostheses satisfaction were evaluated before and after new oral rehabilitation with partial and/or complete dentures. The OHRQoL and prosthesis satisfaction were assessed and verified through OHIP-14 questionnaire and visual analogue scale, respectively. Results: TMD group exhibited the worst mean values (P<0.05) for all OHIP-14 domains before insertion of new dentures. Group 2 showed worst means (P<0.05) compared to controls for functional limitation and physical pain domains of the OHIP-14, but not in the general score. Patients showed better outcomes of satisfaction with prostheses use only after the new rehabilitation. Conclusion: The use of new and well-fitted dentures improves all domains of OHRQoL in patients with RA and TMD and all groups were satisfied with prostheses use after the new rehabilitation with conventional dentures.

Introduction: This open, uncontrolled study examined the effects of physical therapy and rehabilitation on the quality of life in patients with rheumatoid arthritis (RA) and psoriatic arthritis (PsA). Material and methods: The study included a total of 109 patients (69 with RA and 40 with PsA). Patients came from Norway for a four-week rehabilitation period at the Institute of Physical Medicine, Rehabilitation & Rheumatology - Igalo from June till October, 2003. This was a self-controlled, pretest/posttest study. All patients had six days of physical therapy per week, during a four-week stay, which made a total of 24 therapy days. Basic therapy included mud packs/baths, kinesitherapy, hydrokinesitherapy and electrotherapy with analgesic effects. Quality of Life measurements were conducted two times (on admission and discharge) using questionnaire EuroQoL (EQ-5D). The research also included evaluation of ACR improvement. Results: Pain/disability scale and the well being scale showed that quality of life in patients with PsA was significantly lower in comparison with RA patients. However, after 4 weeks, quality of life was much better in most dimensions of the EuroQoL questionnaire. Patients showed no improvement in self-care activities (in both groups) and daily activities (in group with PsA). Significant improvement was measured also in ACR improvement criteria (around 30%). Conclusions: Physical therapy at the Igalo Institute and good climate conditions have significantly improved the Health-Related-Quality-of-Life in both groups of patients. ACR index showed great improvement after a four-week rehabilitation period.

Early detection of Rheumatoid Arthritis (RA) and other neurological conditions is vital for effective treatment. Existing methods of detecting RA rely on observation, questionnaires, and physical measurement, each with their own weaknesses. Pharmaceutical medications and procedures aim to reduce the debilitating effect, preventing the progression of the illness and bringing the condition into remission. There is still a great deal of ambiguity around patient diagnosis, as the difficulty of measurement has reduced the importance that joint stiffness plays as an RA identifier. The research areas of medical rehabilitation and clinical assessment indicate high impact applications for wearable sensing devices. As a result, the overall aim of this research is to review current sensor technologies that could be used to measure an individual’s RA severity. Other research teams within RA have previously developed objective measuring devices to assess the physical symptoms of hand steadiness through to joint stiffness. Unfamiliar physical effects of these sensory devices restricted their introduction into clinical practice. This paper provides an updated review among the sensor and glove types proposed in the literature to assist with the diagnosis and rehabilitation activities of RA. Consequently, the main goal of this paper is to review contact systems and to outline their potentialities and limitations. Considerable attention has been paid to gloved based devices as they have been extensively researched for medical practice in recent years. Such technologies are reviewed to determine whether they are suitable measuring tools.

INTRODUCTION Rheumatoid arthritis (RA) is a chronic constantly deteriorating disease of unpredictable clinical course, with exacerbations, remissions and damaged joints. It leads to the loss of self-sufficiency, independence in performing many daily activities, decrease of working ability and invalidity. Beside physical factors, which are regarded as most responsible for the poorer quality of life of RA patients, psychological changes are also significant, such as the feeling of helplessness, hopelessness and depression. The goal of the treatment of patients with RA is to decrease illness symptoms, slow down the development of illness progression, improvement of physical functioning and provision of expert help to the RA patients to adapt to life. OBJECTIVE The aim of the study was to assess the influence of spa therapy on the functional condition and life quality of RA patients. METHODS The study involved 69 patients with RA (51 female and 18 male, on average aged 55.2?11.4 years, with illness duration 12.5?7.5 years), and were a part of a cohort from Norway, suffering of inflammatory rheumatism. All the patients came for four-week rehabilitation at the Institute for Physical Medicine, Rehabilitation and Rheumatology "Dr. Simo Milosevic" in Igalo, Montenegro. The RA patients underwent treatment with mud compresses, mud, mineral and pearl baths, as well as with underwater shower massage (balneotherapy) kinesitherapy and certain forms of electrotherapy with analgesic effects. The evaluation was done on admission and after completed physical therapy when we assessed RA patients' functional condition and quality of life. The functional condition was determined using the Modified Health Assessment Questionnaire (MHAQ), and the quality of life using the Medical Outcomes Study Short Form 36-item Questionnaire (SF-36), which encompassed eight life domains. RESULTS After completion of 28-dayspa therapy, RA patients showed a significant improvement in functional condition. Their quality of life was significantly improved in all dimensions of SF-36 Questionnaire (p<0.01), and the functional status (MHAQ score) was also significantly better (p<0.01). CONCLUSION Balneotherapy, together with climatic factors in Igalo, leads to a significant improvement of functional status and quality of life in patients suffering from RA.

In this case series, three elite college-level female volleyball players between 21 and 22 years old experienced acute abdominal pain during an overhead swinging motion. All three athletes were diagnosed with acute rectus abdominis (RA) muscle strain using musculoskeletal ultrasound, without the need for MRI. Each athlete sustained severe RA injury resulting in substantial loss of playing time and warranted a focused rehabilitation program, which emphasized core strengthening, physical modalities, and altering athletes’ hitting technique. RA muscle strain is a relatively infrequent, yet potentially severe, injury in elite volleyball players that necessitates early diagnosis and treatment to avoid prolonged or incomplete recovery.

With an increasing ageing population, it is important to explore factors that can contribute to healthy ageing. Physical activity (PA) and sedentary behaviour (SB) are associated with psychological and physical health in older adults. This thesis therefore aims to explore the associations between PA as well as SB and a range of health, motivational, environmental factors in older adults from assisted living facilities (ALFs) and community settings using various methodological approaches. Study 1 used latent profile analyses to group people based on PA, SB, and physical function in resident of ALFs. It was found that those classed as high physical function with an active lifestyle had better mental health compared to those who had lower physical function and an inactive lifestyle. These findings suggest that in order to improve mental health in older adults, interventions should take all these variables into account. Study 2 used latent profile analyses to classify people based on the degree of autonomy support from important others and perceptions of the physical environment, and subsequently examined differences in engaging in light PA and moderate-to-vigorous PA (MVPA) between these groups. Results suggest that perceptions of the physical environment should be taken into account along with support from important others to facilitate increases in levels of PA. Study 3 employed ecological momentary assessment to examine the within-person association of light PA, MVPA, and SB in relation to bodily pain and fatigue in older community dwelling adults. This study revealed associations between bodily pain and PA, as well as SB. Furthermore, daily fatigue was influenced by typical fatigue and physical health. The overall conclusion of the studies presented in this thesis implies that individual, social, and environmental factors and its interactions can all contribute to mental health, PA, SB, bodily pain, and fatigue outcomes in older adults.

This thesis incorporates five studies investigating physical activity and community mobility in individuals with multiple sclerosis (MS). 1. Pedometer step count accuracy was investigated whilst examining activity levels in individuals with neurological disease (n=43). The pedometers significantly under-estimated counts in this group. 2. The test-retest reliability of the Physical Activity Scale for the Elderly (PASE) was examined. People with MS (n=20) were found to have low activity levels with a test-retest reliability coefficient total PASE score of 0.934 (95% CI=0.62-0.97). 3. An observational study examined physical activity levels in individuals with MS using the PASE. Participants appeared to be less active than healthy individuals (69.6±50.11, 154.3±80.4 respectively). 4. A questionnaire (n=80) and focus group discussions (n=24) were used to gather user opinions regarding physical activity participation. Emerging themes were the desire to be active and the barriers and facilitators to participation. 5. A phase II RCT investigated the feasibility of a physical activity provision support system in people with MS (n=18). The approach successfully resulted in attendance at fitness centres and increased activity levels (but did not reach statistical significance). The 5 studies each present new data in the field. The implications are discussed with suggestions for further research.

Introduction Rehabilitation activities of a sufficient intensity are necessary for optimal recovery in individuals with spinal cord injury (SCI). Optimizing rehabilitation and activity prescription requires quantification of physical activity and its predictors during this time. Purpose To determine, during inpatient rehabilitation, the: 1) reliability and validity of measures of physical activity. 2) level of physical activity using objective and self-report measures. 3) level of cardiovascular stress experienced during physical therapy (PT) and occupational therapy (OT). 4) variables associated with greater time spent at higher heart rate during PT. 5) number of active movement repetitions occurring during PT and OT. Methods Design: A test retest design was used to determine the reliability of physical activity measures. A longitudinal observation design was used to determine movement repetitions and physical activity levels. A cross-sectional observational design was used to determine the level of cardiovascular stress. Subjects: Participants (n=108) were recruited from consecutive admissions to rehabilitation. Results Good reliability for accelerometry and step counts, and moderate reliability for self-report, was demonstrated. Validity was demonstrated for wrist accelerometry and step counts but not self-report physical activity. For most groups and variables, no changes occurred during therapy time from admission to discharge. Outside of therapy all groups increased from admission to discharge in accelerometer measured activity kilocounts but not self-report minutes, where the majority of time was spent in leisure time sedentary activity (~4.5 hours). The average time spent at a heart rate within the cardiovascular training zone was 6.0±9.0 minutes in PT and lower in OT. Lower spasticity, higher exercise self-efficacy, and better orthostatic tolerance correlated with a greater amount of time within a cardiovascular training zone. Average repetitions for PT and OT combined did not exceed 300 for the upper or lower extremity. Most repetition variables remained unchanged over the inpatient rehabilitation stay while clinical outcomes improved significantly. Conclusions Individuals report that a large amount of time is spent engaged in higher intensity activities. Measurement of heart rate during therapy sessions shows little time is spent at intensities sufficient to accrue cardiovascular benefits. Repetitions in therapy are low compared to the motor learning literature.

The development of serious games, requires the participation and collaboration of several disciplines; as in the following thesis, showing the collaboration between physiotherapists and developers. Obviously good collaboration and understanding requires a tool that allows passage of information from one discipline to another. One example of such a tool is the concept of patterns. The concept has been adopted in many communities, one of which is game design where they have been called game design patterns, to offer the same advantages with additional benefits. In the following thesis we use game design patterns in order to translate the requirements of physiotherapists into a serious game. Or more precisely, the aim is to satisfy the requirements of physiotherapists as movements, motivation, and other features and map them into patterns and translate them in a serious game. The validation of the mapping was carried out in three different ways: with the physiotherapists, patients, and finally with game designers. The work carried out, shows an example of how game design patterns can be used to satisfy the requirements of physiotherapists in a game for rehabilitation.

Because many patients reduce exercise following outpatient cardiac rehabilitation (CR), we developed an intervention to assist with the transition and evaluated its feasibility and preliminary efficacy using a one-group pretest–posttest design. Five CR patients were enrolled ~1 month prior to CR discharge and provided an activity tracker. Each week during CR they received a summary of their physical activity and steps. Following CR discharge, participants received an individualized report that included their physical activity and step history, information on specific features of the activity tracker, and encouraging messages from former CR patients for each of the next 6 weeks. Mixed model trajectory analyses were used to test the intervention effect separately for active minutes and steps modeling three study phases: pre-intervention (day activity tracking began to CR discharge), intervention (day following CR discharge to day when final report sent), and maintenance (day following the final report to ~1 month later). Activity tracking was successfully deployed and, with weekly reports following CR, may offset the usual decline in physical activity. When weekly reports ceased, a decline in steps/day occurred. A scaled-up intervention with a more rigorous study design with sufficient sample size can evaluate this approach further.

Aim: This study aimed to explore how qualitative data about rehabilitation for patients with intermittent claudication do provide an enhanced understanding of the quantitative experimental results. Background: The study was a randomized clinical trial comparing a rehabilitation intervention with usual care. A statistically significant difference between rehabilitation and usual care was found in walking distance, physical activity, quality of life and diet. The findings from the quantitative and qualitative analyses were analysed separately on their own tradition. In this study, mixed methods address whether the qualitative results could help explain the quantitative results and bring forward additional information. Design: Complex mixed-method intervention design with a convergent questionnaire variant. Methods: From April 2017- May 2019, patients diagnosed with intermittent claudication were included in a randomized clinical trial (N=118). In addition, qualitative interview informants from the intervention group were sampled from the quantitative study population for a survey (N=43) and focus group interviews (N=10). Interviews were conducted from April 2018-August 2019. Results: Integrated analyses identified how improvement in walking distance, physical activity, diet and quality of life was affected by team spirit, pedometer, education and fun exercise in a local setting. Quantitative and qualitative findings primarily confirmed and expanded each other; however, two discordant results were also evident. Conclusion: Our study adds empirical evidence regarding how a mixed-methods study can be used to obtain a more nuanced understanding of complex healthcare problems. The study provides new knowledge concerning how to set up a rehabilitation programme for patients with intermittent claudication.

Review question / Objective: Coronavirus disease 2019 (COVID-19) is a novel infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which has caused a huge impact in many countries and has attracted great attention from countries around the world. However, since the outbreak of the COVID-19 pandemic, most attention has focused on containing transmission and addressing the surge of critically ill patients in acute care settings. As we enter the second phase of the pandemic, emphasis must evolve to post care of COVID-19 survivors. A variety of persistent symptoms, such as severe fatigue, shortness of breath, and attention disorder have been reported at several months after the onset of the infection. We urgently need to identify safe and effective COVID-19 rehabilitative strategies. Overwhelming evidence exists that physical activity produces short-, middle- and long-term health benefits that prevent, delay, mitigate and even reverse a large number of metabolic, pulmonary and cardiovascular diseases. The purpose of this study was to evaluate the effects of physical activity interventions for rehabilitation of post-covid-19 patient and provide a reliable method and credible evidence to improve the prognosis of post-COVID-19 patients via systematic review and meta-analysis.

Objectives. Although the health benefits of physical activity are well described for the general population, less is known about the benefits and harms of physical activity in people dependent upon, partially dependent upon, or at risk for needing a wheelchair. This systematic review summarizes the evidence for physical activity in people with multiple sclerosis, cerebral palsy, and spinal cord injury regardless of current use or nonuse of a wheelchair. Data sources. We searched MEDLINE®, CINAHL®, PsycINFO®, Cochrane CENTRAL, Embase®, and Rehabilitation and Sports Medicine Source from 2008 through November 2020, reference lists, and clinical trial registries. Review methods. Predefined criteria were used to select randomized controlled trials, quasiexperimental nonrandomized trials, and cohort studies that addressed the benefits and harms of observed physical activity (at least 10 sessions on 10 different days of movement using more energy than rest) in participants with multiple sclerosis, cerebral palsy, and spinal cord injury. Individual study quality (risk of bias) and the strength of bodies of evidence for key outcomes were assessed using prespecified methods. Dual review procedures were used. Effects were analyzed by etiology of impairment and physical activity modality, such as treadmill, aquatic exercises, and yoga, using qualitative, and when appropriate, quantitative synthesis using random effects meta-analyses. Results. We included 146 randomized controlled trials, 15 quasiexperimental nonrandomized trials, and 7 cohort studies (168 studies in 197 publications). More studies enrolled participants with multiple sclerosis (44%) than other conditions, followed by cerebral palsy (38%) and spinal cord injury (18%). Most studies were rated fair quality (moderate risk of bias). The majority of the evidence was rated low strength. • In participants with multiple sclerosis, walking ability may be improved with treadmill training and multimodal exercise regimens that include strength training; function may be improved with treadmill training, balance exercises, and motion gaming; balance is likely improved with postural control exercises (which may also reduce risk of falls) and may be improved with aquatic exercises, robot-assisted gait training, treadmill training, motion gaming, and multimodal exercises; activities of daily living may be improved with aquatic therapy; sleep may be improved with aerobic exercises; aerobic fitness may be improved with multimodal exercises; and female sexual function may be improved with aquatic exercise. • In participants with cerebral palsy, balance may be improved with hippotherapy and motion gaming, and function may be improved with cycling, treadmill training, and hippotherapy. • In participants with spinal cord injury, evidence suggested that activities of daily living may be improved with robot-assisted gait training. • When randomized controlled trials were pooled across types of exercise, physical activity interventions were found to improve walking in multiple sclerosis and likely improve balance and depression in multiple sclerosis. Physical activity may improve function and aerobic fitness in people with cerebral palsy or spinal cord injury. When studies of populations with multiple sclerosis and cerebral palsy were combined, evidence indicated dance may improve function. • Evidence on long-term health outcomes was not found for any analysis groups. For intermediate outcomes such as blood pressure, lipid profile, and blood glucose, there was insufficient evidence from which to draw conclusions. There was inadequate reporting of adverse events in many trials. Conclusions. Physical activity was associated with improvements in walking ability, general function, balance (including fall risk), depression, sleep, activities of daily living, female sexual function, and aerobic capacity, depending on population enrolled and type of exercise utilized. No studies reported long-term cardiovascular or metabolic disease health outcomes. Future trials could alter these findings; further research is needed to examine health outcomes, and to understand the magnitude and clinical importance of benefits seen in intermediate outcomes.

Training cadets-weightlifters at all stages has a multipurpose orientation, that is why it is important to define and plan a rational combination of the training means use. Distribution of such micro structures in the cycle of training, as the days, months of training, provides effective volume, intensity and other values of physical load distribution. The structure of training cadets-weightlifters is based on taking into account the regularities and principles of sports training as the condition for physical readiness and working capacity increase. Any power oriented sports demands components characteristics in the structure of micro cycles. We consider the methodology of the training lessons organization by the example of the micro cycle of training taking into account bioenergetic profile of cadets-weightlifters. We revealed the necessity to distribute the macro cycle to structural components as the condition for the effectiveness of different variants of the training effects distribution. Materials and methods. We analyzed the range of training lessons among cadets-weightlifters in order to create the variants of gradual training problems solution according to the kinds of training. We analyzed training programs of cadets taking into consideration the level of readiness and their bioenergetic profiles. We created the content of the training work in the micro cycle of the preparatory period for cadets-weightlifters with different bioenergetic profiles. The main material of the research includes the ratio of the training effects volume in one micro cycle taking into account cadets’ bioenergetic profile. Cadets-weightlifters from Tyumen Higher Military-Engineering Command College (military Institute) took part in the research (Tyumen, Russia). Results. We created the content of the training work by the example of one micro cycle for cadets-weightlifters taking into account bioenergetic profile. The created variant of the training loads structure includes the main means of training taking into account the kind of training. Realization orientation in five regimens of work fulfillment with the effectiveness estimation of a total load within one lesson and a week in general is estimated according to a point system. Conclusion. The created variant of a micro cycle considers kinds of training realization taking into account the percentage of the ratio. Taking into account bioenergetic profiles helps to discuss strong and weak sides of muscle activity energy supply mechanisms. We consider the ability to fulfill a long-term aerobic load among the representatives of the 1st and the 2nd bioenergetic profiles. The representatives of the 3rd and the 4th biotype are inclined to fulfill the mixed load. The representatives of the 5th biotype are characterized by higher degree of anaerobic abilities demonstration. The technology of planning the means taking into account the regimens of work realization with point system helps to increase physical working capacity and rehabilitation processes in cadets’ organisms.
