Background Burnout has consistently been identified as an issue in health professionals, with paramedics in particular frequently noted to have high burnout rates. Objectives: The current study aimed to describe the prevalence of total work burnout in paramedics and of three subcategories: persona1, workrelated and patient-related burnout. Material and methods: This cross-sectional online survey presented the Copenhagen Burnout lnventory (CBI) to a national convenience sample (n=393) over a 5-week period from October-November 2021. Results: The primary outcome was prevalence of total burnout (%). Secondary outcomes were prevalence of subgroups of burnout and predictive factors. More than half (56.15%) of respondents were determined to have total burnout at the time of completing the survey; 46.4% had patientrelated burnout; 62.3% had work-related burnout; and 71.9% had personal-related burnout. Females, city work location, and having worked for 15-19 years as a paramedic were all predictive of total burnout. Conclusion: The study showed high prevalence of total burnout among this sample of paramedics.

AbstractIntroduction:In treating accident victims, actions by the Emergency Medical Personnel (EMP) at the scene may be the difference between life or death, full recovery or permanent disability. Development of selected profiles based on locale of services, tenure, and paramedic certification will provide valuable insight into the diversity within the Emergency Medical Services (EMS) profession. Not only will these profiles enable administrators to improve their recruitment, training, and retention of the emergency medical workforce, it potentially could enhance the quality of health care in the community.Population:Emergency medical personnel attending a statewide conference in Texas in late 1996 (n = 425).Hypotheses:1) There is no difference between the profiles of urban and rural emergency medical personnel; 2) There is no difference between the profiles of urban EMP with <9 years of experience and those with ≥9 years of experience; 3) There is no difference between the profiles of rural EMP with <9 years of experience and those with ≥9 years of experience. 4) There is no difference between the profiles of urban EMP with paramedic certification and those without certification; and 5) There is no difference between the profiles of rural EMP with paramedic certification and those without certification.Methods:EMP attending the conference completed 425 survey instruments measuring five demographic features, five work-related features, and two psychological features. Survey instruments were included in each registrant's conference package. Completed surveys were deposited anonymously in labeled receptacles throughout the statewide conference site. Data collection ceased at the end of the conference. Discriminant analysis identified distinct profiles for the urban and rural EMP.Results:The urban EMP, more than rural subjects, was younger (mean = 36 years), more likely to be compensated 100% for their services, had a higher level of education (mean = 13.8 years), and reported a lower level of burnout. Urban EMP with <9 years of experience tended to be younger, male, married, and reported less burnout. Urban paramedics were more likely to be compensated 100% for their services, and had achieved a higher level of education. The rural EMP with <9 years of experience were less likely to be paramedic, reported lower burnout scores, and was younger. The rural EMP without paramedic certification was more likely to be a volunteer, and have had fewer years of service.Conclusions:In Texas, locale of service (urban or rural), length of tenure as an EMP (>9 years), and paramedic certification appear to be significant factors that define the EMP population in Texas.

Introduction: Contemporary healthcare faces new challenges and expectations from society. The profession of a nurse, as well as a paramedic, is essential for the efficient functioning of healthcare. It has its importance not only in promoting and preserving health but also in prevention. With the increasing importance of providing medical care at the highest level, it is expected of these two professional groups to have more knowledge and skills than a few years earlier. The daily contact with patients and their families, the low level of control of the environment, the hierarchical system of professional dependence, and the dissatisfaction with remuneration are becoming extremely burdensome aspects of the nursing and paramedic professions. Long-term exposure to stressors associated with these medical professions may, in the long term, lead to the emergence of occupational burnout syndrome. The aim of this study is an attempt to answer the question of whether and how stress factors affect the occurrence of occupational burnout in the work of nurses and paramedics working in various medical entities. Material and Methods: The study covered a group of 434 respondents, including 220 nurses and 214 paramedics, working professionally in hospital departments and care and treatment facilities as well as in hospital emergency departments and ambulance services. The study was carried out using a diagnostic survey based on the questionnaire technique using the authors’ questionnaire and the standardized MBI Ch. Maslach. Two statistical values were used to statistically analyze the research results and verify the adopted hypotheses: the chi-square test and the Student’s t-test. Results and Conclusions: The current study showed that the phenomenon of occupational burnout among the studied group affects only nurses, while this problem does not apply to the studied paramedics. The main stressor among the nurses and paramedics is, above all, a very high level of responsibility. Nurses are overburdened by excessive demands and shift work, while paramedics are mostly burdened by an excess of duties. Both nurses and paramedics claim that their work is often stressful, which leads to physical and mental exhaustion.

Анотація:

Burnout among the paramedic profession is an increasing problem and not enough is being done to improve the current climate. It is for this reason that paramedics should individually contribute to the reduction of this issue by developing their own leadership characteristics, such as emotional intelligence, social intelligence, and stress and conflict management. Evidence suggests that these qualities and behaviours reduce the possibility of being subject to burnout symptomology. Implementing positive stress and conflict-management strategies will create a healthcare environment, which will be more enjoyable to work in. Emotional and social intelligence increase the resilience of prehospital clinicians who are subject to occupational challenges on a daily basis. Wider influences such as ambulance trusts, health services and governing bodies have implemented strategies to tackle this issue but clinicians can lead the change from the front line.

Анотація:

Healthcare delivery relies on human resource management to ensure quality. However, employee performance dropped from 2019 to 2022, highlighting the need to address job fatigue, which lowers performance. This study examines how mindfulness and emotional intelligence mediate the effects of occupational fatigue on medical and paramedic performance. This integrated study intends to understand healthcare job performance and improve medical professionals' competency and well-being in Indonesia. A 5% margin of error was used to choose 213 medical staff and paramedics from the Mojokerto City Government Health Agency in Indonesia using the Slovin algorithm. The Sobel Test and SEM with AMOS were used to analyse data. Mindfulness and emotional intelligence decreased considerably with job burnout, affecting job performance. Mindfulness and emotional intelligence also improved job performance. The results also showed that emotional intelligence and mindfulness moderate the job burnout-performance link. These findings show that emotional and psychological aspects are crucial to understanding how job fatigue affects employee performance.

AbstractIntroduction:The number of paramedics in Israel is increasing. Despite this growth and important role, the emergency medical organizations lack information about the characteristics of their work.Objective:The objective of this study was to examine the characteristics of the paramedics' work, the quality of their working lives, the factors that keep them in the profession, or conversely, draw them away from it.Methods:Cross-sectional study conducted through telephone interviews of a random sample of 50% of the graduates of paramedic courses in Israel (excluding conscripted soldiers).Results:The factors that attract paramedics to the profession have much to do with the essence of the job—rescuing and saving—and a love of what it involves, as well as interest and variety. Pressures at work result from having to cope with a lack of administrative support, paperwork, long hours, imbalance between work and family life, and salary. They do not come from having to cope with responsibility, the pressure of working under uncertain conditions, and the sudden transition from calm situations to emergencies. Dissatisfaction at work is caused by burnout, work overload, and poor health. Physical and mental health that impedes their ability to work is related to a sense of burnout and the intention to change professions.Conclusions:The findings about the relationships between health, job satisfaction, and burnout, coupled with the fact that within a decade, half of the currently employed paramedics will reach an age at which it is hard for them to perform their job, lead to the conclusion that there is a need to reconsider the optimum length of service in the profession. There also is a need to form organizational arrangements to change the work procedures of aging paramedics.

BackgroundParamedics are frequently exposed to traumatic events as part of their role. The implications of this constant exposure have suggested that paramedics are at high risk of developing long-term mental health issues such as depression, anxiety, burnout, trauma, PTSD, and suicide.PurposeTo synthesise studies to examine factors that play a role in the development of mental health difficulties in ambulance personnel. As well as, establishing gaps within the literature for further research.Data SourcesFrom the first 77 published journals reviewed from Psycinfo, PsychArticles, CINAHL, Education Research Complete, ERIC and grey literature, 15 studies met the inclusion criteria for this review and were included in the present study.ResultsFindings of this review suggest that the mental health and wellbeing of paramedics are significant issues. In reviewing the research, it was found that several different factors may play a role in the development of mental health difficulties in ambulance personnel. Four main themes were determined, (1) occupational stressors, (2) negative attitude towards emotional expression, (3) peer and social support, and (4) preparing paramedics for mental health challenges.ConclusionsIt is paramount for further research to explore paramedic students’ mental health experiences, as well as a focus on research to investigate a UK paramedic population due to most of the literature focusing on an international population. Psychological support and early interventions should be made a priority for all ambulance workers, as a way of improving the quality of the working life of paramedics.

Background Paramedics are at higher risk of anxiety, depression, burnout, and PTSD compared to the general population. Factors that have been associated with mental disorders include occupational stress, fatigue, sleep quality, chronic pain, physical activity, perceived social support, and overall quality of life. To date only a few of these factors have been investigated simultaneously in Australian paramedics. Aim This study aimed to investigate occupational stress, fatigue, sleep quality, chronic pain, physical activity, perceived social support, and overall quality of life, and their associations with anxiety, depression, burnout and PTSD in Australian paramedics. Methods Sixty-three paramedics, working for ambulance services in Australia, participated in this cross-sectional, self-reported study. Participants completed a questionnaire booklet composed of a demographics questionnaire, the Beck Anxiety Inventory, the Beck Depression Inventory-II, the Maslach Burnout Inventory - Human Services Survey, The Posttraumatic Stress Disorder Checklist - Civilian Version, the Emergency Medical Services Chronic Stress Questionnaire, the Chalder Fatigue Scale, the Pittsburgh Sleep Quality Index, the Orebro Musculoskeletal Pain Questionnaire, the Compendium of Physical Activities, the Multidimensional Scale of Perceived Social Support, and the Quality of Life Index. Results The prevalence of anxiety, depression, burnout and PTSD was higher in this study than what is reported in the general population of Australia. Moreover, the majority of these paramedics had one or more co-occurring mental disorders. Paramedics who reported higher levels of anxiety, depression and PTSD also reported increased occupational stress, greater levels of fatigue, poorer sleep quality, lower perceptions of social support, and significantly lower quality of life. Paramedics with burnout had significantly higher occupational stress and fatigue, and a lower quality of life when compared with paramedics without burnout. In addition, chronic pain was associated with increased levels of anxiety and depression. Conclusions Future research should be targeting interventions aimed at improving organisational and operational stress, fatigue, sleep quality, chronic pain and social support to improve anxiety, depression, burnout, and PTSD in Australian paramedics.

This diploma work deals with the problem of burnout syndrom and its prevention. It describes the burnout syndrom, factors which contribute to it and its course. Further it devotes to prevention, it shades the risk of crisis and importace of work with emotions. The important part of the prevention is formed by the system of the care for the stuff. The cotemporary situation at ZZS HMP (Prague rescue service) and possibilities which could be incorporated into this system and described. The aim of my diploma work is to show the main difficulties at ZZS HMP that the rescue team can encounter and suggest ways of possible care which are introduced in the theoretical part.

Introduction to the issue: This paper examines the impact of burnout syndrome, which is common among caring professionals, on the sexuality of male paramedics. The term "burnout" was coined in the US 25 years ago. Burnout is defined as a mental state resulting from constant and long-term exposure to stress, especially in relation to psychosocial work factors (Melamed and Shirom, 2006). The effect of burnout syndrome on sexuality is a topic that has received very little attention. That is why I decided to cooperate with PhDr. Šárka Dynáková, Ph.D. and doc. PhDr. Jan Kožnar, CSc, who are working on an international study in the Czech Republic on the effects of burnout syndrome on human health. Methodology: For the theoretical part of this diploma thesis, use was made of monographs and databases. For the practical part, quantitative data was collected using standardized questionnaires distributed among male paramedics. The first questionnaire focused on burnout syndrome using a validated questionnaire for the Czech Republic, one that applies the Shirom-Melamed scale from 2006 (Ptáček, 2013). The objective of this study was to assess the relationships between Sense of Coherence and Burnout (i.e. Emotional Exhaustion, Depersonalisation and Personal Accomplishment) and to investigate job-related stressors and support resources in a sample of privately employed eThekwini paramedics. A survey design was used and a convenience sample (N=56) was taken from one emergency care organisation. Biographical variables and qualitative information regarding stressors and support systems were obtained and two inventories, the Maslach Burnout Inventory-Human Services Survey and the Orientation to Life Questionnaire, were administered. Results showed a predominance of informal support systems and individual case-related stressors. There were relationships between Sense of Coherence and Burnout and a significant difference in Personal Accomplishment in the 36-40 age group was found. Regression results indicated that Depersonalisation and Sense of Coherence predicted Emotional Exhaustion and Emotional Exhaustion demonstrated a main effect on Depersonalisation. Sense of Coherence was the only variable that predicted Personal Accomplishment in the current sample of paramedies.

Problématique : Le rôle des paramédics se transforme partout dans le monde. Les besoins des aînés et des malades chroniques entraînent une forte proportion d’interventions préhospitalières non urgentes. La confrontation entre la perception et l’expérience du rôle peut entraîner un conflit correspondant à un stress organisationnel modéré et chronique chez ces travailleurs. Pour y faire face, différentes stratégies d’adaptation peuvent être adoptées. Objectifs : Cette étude vise à dépeindre la réalité des interventions préhospitalières non urgentes des paramédics québécois et à explorer son influence sur leur perception du rôle et les manifestations de cynisme et de désengagement. Méthode : Les données qualitatives obtenues en entrevues semi-structurées ont été codifiées et analysées à partir d’un modèle adapté des théories en psychosociologie du travail et d’administration de la santé (n=13, 3 régions, intervenants de - de 3 à + de 20 ans de carrière). Résultats : Les paramédics reconnaissent vivre un conflit de rôle alors que la formation et la définition de leur pratique se rapportent exclusivement à l’urgence, au contraire de leur expérience. Ils manifestent des attitudes de cynisme et de désengagement affectant la qualité des services, précisant qu’il s’agit d’une réalité inhérente à leur expérience professionnelle intimement reliée à l’épuisement professionnel, plus qu’à un manque fondamental de professionnalisme. Conclusion : Les paramédics décrivent différents mécanismes à instaurer qui visent à reconnaître la dualité conflictuelle de leur pratique dont, la mise en valeur de leur aptitude clinique à intervenir en première ligne dans un cadre préhospitalier non urgent ainsi que l’ajustement et le rehaussement des programmes de formation.
Problem: The paramedic’s role has been steadily evolving internationally. Elderly and chronically ill user’s demands have lead to an increase in prehospital non urgent interventions. Confronting the conflict between the perceived role of a paramedic and the shifting reality toward non urgent care may result in a role conflict that manifests as chronic-moderate organizational stress. Mechanisms of coping related to this stress may be adopted. Objectives: The objectives of this study are to depict the Quebec paramedic’s experience related to non urgent interventions and explore their role perception and resulting attitudes of cynicism and disengagement. Method: Qualitative data from semi-structured interviews have been codified and analyzed based on a model adapted from the literature on works in socio-psychology and health administration theories. (n=13, 3 administrative regions, less than 3 years to more than 20 years of career) Results: Paramedics acknowledge experiencing role conflict as they explain the formation of their work and field of practice as being exclusively oriented toward emergency response, contrary to their pervasive work reality. They demonstrate cynicism and disengagement, and specify it as being inherent to the professional experience and intimately linked with burnout, affecting quality of care despite fundamental professional intentions. Conclusion: Paramedics described different professional coping mechanisms aimed at acknowledging the conflicting duality of their practice, amongst them, showcasing their clinical capability to play an active role in primary health care in the prehospital, non urgent setting and adjusting and enhancing the training programs.

Abstract Ambulance services and paramedics perform critical roles in contemporary healthcare economies. Trained to work in the field and respond rapidly to emergencies, societies have come to increasingly rely on ambulance services to deliver urgent care, never more so than in recent years given intense social inequality, overstretched and underfunded health systems, and deadly pandemics. This monograph is the first book-length study of the paramedic profession in England. Based on in-depth interviews and ethnographic observation The Paramedic at Work provides a detailed account of the complex realities of work in this fascinating occupation. Empirical chapters explore the nature of work ‘out on the road’, the peculiarities of ambulance organizational culture, the intensity of workplace stress and burnout, and the current and future trajectory of paramedic professionalism. The book documents the unique paradoxes experienced by those employed in this line of work. Ambulance staff are trained to handle life-threatening trauma and disease, but most callouts consist of unplanned primary care. Paramedic work features wide autonomy but is also bound into an array of micromanaging performance indicators. Paramedics are trusted and respected in society but the profession is poorly understood and employers can be unsupportive. But, no matter how intense the personal struggles can be, paramedic work also offers rare opportunities for meaningful and socially valued work. The nature of the paramedic role is rapidly moving from a manual occupation rooted in first aid and transportation, to a clinical profession of increasing scope, versatility, and social respect.

Abstract This chapter delves into the most unpleasant and troubling elements of paramedic work, from the stark realities of operational overload, the uneasy realities of ‘coping mechanisms’ and to the prevalence of ill health, mental trauma, and even suicide among the paramedic community. It explores the enormous pressures placed on those working in the service, where paramedics risk having their wellbeing—in the words of one interviewee—‘slowly scratched away’. It documents how ambulance workers and employers try to handle the impact of distressing calls through formal and informal means. Alongside the gallows humour that famously operates in such ‘extreme’ settings, serious emergencies also provide opportunities for learning and debriefing, and they occupy a high position in the informal status hierarchy of paramedic work. However unpleasant they are, I show that very serious and challenging calls are part of the essence of emergency work, and that competent and ‘professional’ paramedics have to find appropriate ways to deal with them. The chapter moves on to explore the limits of coping mechanisms, drawing on academic concepts such as burnout, PTSD, and moral injury, but also the folk concepts emergent from paramedic street culture used to describe emotional labour, such as ‘five-year job’, ‘the bucket’, and the ‘grief mop’. Finally, it also describes endemic problems associated with ambulance service managerial and employment cultures. These represent yet another form of stress for paramedics, who often feel undervalued and neglected and sometimes live in fear of accusations of wrongdoing, complaints, and inspections.

The article introduces a new term in Russian science — territorial loyalty, which is a specific perceptual-behavioral phenomenon. The definition of this term is given, the relevance of the study, due to the existing research contradictions identified by the author, is substantiated, and an analogy with another loyalty type — organizational — is drawn. A comparison of organizational and territorial loyalty with reliance on the empirical research results leads to the idea of another perceptualbehavioral phenomenon — emotional burnout, the formation of which the individual affects its loyalty — organizational or territorial. The article gives a brief description of the author’s research tools pilot version for assessing the territorial loyalty, and also describes the questionnaire, which is one of the most Russian famous tools for studying emotional burnout. A pilot study of the relationship between emotional burnout and territorial loyalty was conducted on a small sample included 15 respondents (educational system employees and university students), in connection with which a quantitative data analysis was carried out using non-parametric methods. As a result, with a high level of statistical significance, it was found that the severity of territorial loyalty of non-burnt respondents is lower than the severity of this phenomenon in individuals characterized by an average, high or critical emotional burnout level, that is, the existence of a relationship between emotional burnout and territorial loyalty is confirmed. In addition, further research areas have been outlined: for example, the question remains as to which of the considered indicators is a predictor and which is a dependent variable

The purpose of this study is to investigate the effects of operation parameters on BWR fuel reactivity and nuclide inventory. The three high-priority parameters that are suggested by NRC/ORNL [1], that is, axial burnup profile, axial moderator density profile, and control blade insertion during operation, were examined in order to evaluate their impact on BWR burnup credit. All of the calculations were performed using SCALE6.1 based on the ENDF/B-VII nuclear data library and involved 238 energy groups. The results indicate that the axial burnup profile is the most critical factor in reactivity when the burnup value is greater than 10 GWd/MTU. The insertion of control blades during operation is also more important for the reactivity of spent fuel as compared to the influence of the axial moderator density profile. At a burnup of 60 GWd/MTU, these three operation parameters (i.e., axial burnup profile, axial moderator density profile, and control blade insertion during operation) in turn lead to multiplication factor differences of 18850, 7600, and 1980 pcm, respectively, for set2 nuclides. The influence between different parameters was also investigated in this study. The results reveal that multiple operation parameters affect the reactivity reduction tendency. In addition to the three parameters investigated in this study, other important factors related to burnup credit analysis are also currently under investigation.

Inventory estimation of long-lived fission products (LLFPs) in high-level radioactive wastes (HLW) from spent nuclear fuels of light water reactors is important for a safety assessment of their disposal. In order to develop an inventory estimation method of difficult-to-measure LLFPs (Se-79, Tc-99, Sn-126, and Cs-135), a parametric study was carried out by using a sophisticated burnup calculation code and data. In the parametric study, fuel specifications and irradiation conditions are changed in the conceivable range. The considered parameters are fuel assembly types (PWR / BWR), U-235 enrichment, moderator temperature, void fraction, power density, and so on. From the calculated results, we clarify the burnup characteristics of the target LLFPs and their possible ranges of generations. Finally, candidates of the key nuclide are proposed for the scaling factor method of HLW.

The pulverized coal injection (PCI) is widely utilized in the iron-making blast furnaces for its economic and environmental advantages. However, due its complexity, flow dynamics and chemical kinetics of PCI inside the raceway has not been well understood. Combustions of PCI and coke inside the raceway can be influenced by tuyere operation parameters. In this paper, a comprehensive three dimensional (3-D) multiphase flow computational fluid dynamics (CFD) model was utilized to investigate the PCI and coke combustion in the lower part of a blast furnace. Systematic parametric studies were conducted to analyze the effects of the natural gas injection, coal injection, PCI rate, and oxygen enrichment on the combustion performance, which include coal burnt-out rate, coke consumption rate, raceway shape, raceway temperature and etc.

In the eight-year, multicontractor program to develop a 220-MW coal-fired combustion turbine combined cycle plant, the team is now into its third year of subscale slagging combustor testing. Because of an ability to accept unbeneficiated, utility-grade coal, the slagging combustor is the key to the direct coal-fired combustion turbine. The projected plant will contain two 80-MW combustion turbines equipped with slagging combustors, two heat recovery steam generators, and a 65-MW steam turbine. In testing to date, the concept has demonstrated its ability to handle high- and low-sulfur bituminous coals, and low-sulfur subbituminous coal. Feeding the fuel in the form of pulverized coal has proven to be superior to coal-water mixture type feed. The program objectives relative to combustion efficiency, combustor exit temperature, NOx emissions, carbon burnout, and slag rejection have been met. Work continues to reduce alkali, particulate, and SOx levels leaving the combustor. Parametric studies have been made that focus on the latter two problems. They indicate that faster, more thorough slag removal between the rich and lean stages is the path toward achieving lower emission of SOx, alkali, and particulates. New components have been added to the subscale combustor for this purpose.

Abstract This paper provides an overview of the base model specifically developed to perform parametric sensitivity studies on the U-10Mo monolithic fuel system. U-Mo monolithic fuels are being considered for the conversion of test reactors into high-performance research reactors that operate using proliferation-resistant, low-enriched uranium (LEU) fuels. These plate-type fuels contain a high-density, low-enrichment fuel sandwiched between zirconium diffusion barriers and encapsulated in aluminum claddings. All U.S. high-performance research reactors have released the designs of their LEU monolithic fuel reactor cores. These designs include nearly 50 distinct fuel plate geometries with different operational parameters. Consequently, a single generic plate geometry representing all the extreme points in this design matrix is unrealistic. To evaluate the performance for various parameters, a set of sensitivity studies was performed. These studies considered various input parameters (i.e., geometric, operational, and material property-related). The results revealed valuable information about plate performance and the sensitivity of this performance to various modeling inputs. To establish a reference state for comparing these result, base model featuring representative irradiation conditions was developed. To capture in-reactor behavior accurately, incorporation of representative constitutive models capable of evolving properties with respect to temperature, irradiation time, and burnup was needed. The behavioral models considered burnup-dependent properties, swelling, creep, and degradation. This paper introduces the base model created for the parametric sensitivity studies. The detailed description of the procedure includes the model geometry, model discretization, thermo-mechanical coupling, material properties and behavioral models. This paper also provides selected results and assesses the performance of the base model.

The present study is motivated by interest in understanding of physical mechanisms that govern the effect of material and micro-structural characteristics of heat surface on boiling heat transfer and burnout at high heat fluxes. The effect was reported and investigated experimentally and analytically over several past decades. Only recently, with the advent of nanotechnology including microscale manufacturing, it becomes possible to perform high heat-flux boiling experiments with control of surface conditions. Of particular importance for practice is the potential for significant enhancement of boiling heat transfer (BHT) and critical heat flux (CHF) in pool and flow boiling on heaters with specially manufactured and controlled micro-structured surfaces. This enhancement is very important to a very wide range of engineering applications, like heat exchanger and cooling system, where maximum flux is needed. Currently, there are many controlled experiments that investigate such effect and they lend themselves a subject for detailed computational analysis. The focus of this study is micro-hydrodynamics of the evaporating thin liquid film at the receding triple contact line, corresponding to formation of dry spot in the footprint of a growing bubble. Parametric investigations are performed to assess the hypotheses that micro-structured surfaces enhance resilience to burnout due to residual liquid in the dry patch after contact line receding. Towards the study objective, a particle-based (mesh-less) method of computational fluid dynamics called Smoothed Particle Hydrodynamics (SPH) is adopted. The SPH method is selected for its capability to handle fluid dynamics in complex geometries and free surface problems without mass loss (characteristic of alternative interface capturing schemes used in mesh-based methods). Both surface tension and surface adhesion (hydrophilicity) are implemented and tested. The solid (heater) surface and manufactured micro-structures are represented by solid-type particles. Heat transfer, phase change (evaporation) and vapor dynamics are not included in the present simulation. The bouncing drop case measures the contact time of water droplet with solid surface. This case is used for “mesh” sensitivity (particle size) study and calibration of boundary conditions and surface tension coefficient. Subsequently, case studies are formulated and performed for contact line dynamics on heater surfaces with the fabricated Micro Pillar Arrays surfaces (MPA) and smooth surface. Variable characteristics include surface tension and pillar density on structured surface (modified by changing distance between pillars). First of all, residual fluid are found in all simulations with structured surface, while fluid are drained for smooth cases. For structured surface, it’s found that after the contact line recedes, fluid with higher surface tension resides in the dry patch more than fluid with lower coefficient, and the relation tends to be non-linear. While for smooth surface, all fluid will be drained after certain time and the relations are non-monotonic; it’s also found that the amount of residual fluid increase as the distance between pillars decreases until a limit. The fluid then starts to decrease with pillars being set further apart. The increase starts from 30 μm and the limit is around 10 μm.

Background Burnout is endemic to nursing and carries high personal, institutional, and social costs. Methods Previous studies have demonstrated that yoga may alleviate burnout in healthcare workers. A yoga-based intervention was created specifically for ICU nurses. The program was offered to all nurses working in critical care environments through recruitment emails and fliers. 28 nurses participated, but a high attrition rate led to only 9 people completing both the pre-and-posttest survey. Power analysis of 30 participants was not met and statistical significance could not be established, however, differences illustrated by non-parametric testing indicate potential clinical relevance. Results Pre-and-posttest Maslach Burnout Inventory (MBI) scores were compared via Wilcoxon signed-rank tests. Depersonalization and Personal Achievement domains of the MBI showed no difference, but there was a difference in SUM and MEAN/AVE Emotional Exhaustion scores (Z = -2.129, p = 0.33) and (Z = -2.113, p= 0.035), respectively. NDNQI data were collected before and after the intervention to determine if there was a change in nurse-sensitive patient outcomes and assessed using Wilcoxon signed-rank tests, showing no appreciable difference. Conclusions This pilot program reinforced that yoga may be a useful intervention in addressing burnout, however, the high attrition rate suggests that it is likely not a feasible option for most nurses.
