Purpose: To optimize collimator setting to improve dosimetric quality of pancreas volumetric modulated arc therapy plan for stereotactic body radiation therapy. Materials and Methods: Fifty-five volumetric modulated arc therapy cases in stereotactic body radiation therapy of pancreas were retrospectively included in this study with internal review board approval. Different from the routine practice of initializing collimator settings with a template, the proposed algorithm simultaneously optimizes the collimator angles and jaw positions that are customized to the patient geometry. Specifically, this algorithm includes 2 key steps: (1) an iterative optimization algorithm via simulated annealing that generates a set of potential collimator settings from 39 cases with pancreas stereotactic body radiation therapy, and (2) a multi-leaf collimator modulation scoring system that makes the final decision of the optimal collimator settings (collimator angles and jaw positions) based on organs at risk sparing criteria. For validation, the other 16 cases with pancreas stereotactic body radiation therapy were analyzed. Two plans were generated for each validation case, with one plan optimized using the proposed algorithm ( Planopt) and the other plan with the template setting ( Planconv). Each plan was optimized with 2 full arcs and the same set of constraints for the same case. Dosimetric results were analyzed and compared, including target dose coverage, conformity, organs at risk maximum dose, and modulation complexity score. All results were tested by Wilcoxon signed rank tests, and the statistical significance level was set to .05. Results: Both plan groups had comparable target dose coverage and mean doses of all organs at risk. However, organs at risk (stomach, duodenum, large/small bowel) maximum dose sparing ( D0.1 cc and D0.03 cc) was improved in Planopt compared to Planconv. Planopt also showed lower modulation complexity score, which suggests better capability of handling complex shape and sparing organs at risk . Conclusions: The proposed collimator settings optimization algorithm successfully improved dosimetric performance for dual-arc pancreas volumetric modulated arc therapy plans in stereotactic body radiation therapy of pancreas. This algorithm has the capability of immediate clinical application.

PurposeThe hearing aid microphone setting (omnidirectional or directional) can be selected manually or automatically. This study examined the percentage of time the microphone setting selected using each method was judged to provide the best signal-to-noise ratio (SNR) for the talkers of interest in school environments.MethodA total of 26 children (aged 6–17 years) with hearing loss were fitted with study hearing aids and evaluated during 2 typical school days. Time-stamped hearing aid settings were compared with observer judgments of the microphone setting that provided the best SNR on the basis of the specific listening environment.ResultsDespite training for appropriate use, school-age children were unlikely to consistently manually switch to the microphone setting that optimized SNR. Furthermore, there was only fair agreement between the observer judgments and the hearing aid setting chosen by the automatic switching algorithm. Factors contributing to disagreement included the hearing aid algorithm choosing the directional setting when the talker was not in front of the listener or when noise arrived only from the front quadrant and choosing the omnidirectional setting when the noise level was low.ConclusionConsideration of listener preferences, talker position, sound level, and other factors in the classroom may be necessary to optimize microphone settings.

This paper presents a response surface methodology approach in the optimizationof the carbon dioxide temperature-programmed adsorption process using a new materialreferred as nitrogen-functionalized graphene oxide. This material was synthesized byloading nitrogen groups to graphene oxide using aqueous ammonia in supercriticalcondition. Later on, it was utilized as a sorbent for carbon dioxide adsorption. This process was optimized by implementing a response surface methodology coupled with a Box- Behnken design for the effects of three factors: adsorption temperature, carbon dioxide flow rate, and the amount of adsorbent. In analyzing the response surface, a model equation was generated based on the experimental data by regression analysis. This model equation was then utilized to predict optimum values of response. Furthermore, response optimizer was also conducted in identifying factor combination settings that jointly optimize the best response.

Background: Quality measures relating to diabetes care in America have not improved between 2005 and 2016, and have plateaued even in areas that outperform national statistics. New approaches to diabetes care and education are needed and are especially important in reaching populations with significant barriers to optimized care. Methods: A pilot quality improvement study was created to optimize diabetes education in a clinic setting with a patient population with significant healthcare barriers. Certified Diabetes Care and Education Specialists (CDCES) were deployed in a team-based model with flexible scheduling and same-day education visits, outside of the traditional framework of diabetes education, specifically targeting practices with underperforming diabetes quality measures, in a clinic setting significantly impacted by social determinants of health. Results: A team-based and flexible diabetes education model decreased hemoglobin A1C for individuals participating in the project (and having a second A1C measured) by an average of −2.3%, improved Minnesota Diabetes Quality Measures (D5) for clinicians participating in the project by 5.8%, optimized use of CDCES, and reduced a high visit fail rate for diabetes education. Conclusions: Diabetes education provided in a team-based and flexible model may better meet patient needs and improve diabetes care metrics, in settings with a patient population with significant barriers.

Дисертаційна робота присвячена дослідженню впливу регульованого температурного стану поршнів дизеля різного призначення на його техніко-економічні показники. Розроблено та реалізовано методику оцінки якості дизеля при регулюванні інтенсивності масляного охолодження поршнів та керуванні моментом початку впорскування палива в камеру згоряння, а також при врахуванні моделі експлуатації енергетичної установки. Проведено експериментальні дослідження впливу температурного стану поршнів з низькотеплопровідним покриттям поверхні камери згоряння транспортного дизеля, а також зміни кута випередження впорскування палива на показники токсичності відпрацьованих газів, паливної економічності та температурний стан деталей камери згоряння. В результаті виконаного оптимізаційного дослідження запропоновано характеристичні карти керування масляним охолодженням поршнів та моментом початку впорскування палива для комплексного покращення техніко-економічних показників дизеля. Оцінено ефективність впровадження запропонованих заходів в автомобільних та тракторних дизелях та стаціонарних дизельгенераторах. Запропоновано методику врахування температурного стану найбільш теплонавантажених зон поршня в загальній методиці оцінки якості дизеля. Проведені розрахункові дослідження дозволили визначити ефективність регулювання температурного стану поршнів на ресурсну міцність його камери згоряння.

Dissertation is devoted to research of complex influence of pistons temperature state regulation on the diesel engine technical and economic performance. In the dissertation work a method of estimating the quality of the diesel engine when regulating of the pistons temperature state, which takes into account indicators of exhaust gases toxicity and ICE fuel efficiency in each mode of the power plant operation is proposed. Experimental study of the effect of temperature condition of pistons with low conductive coating combustion chamber surface and changes the fuel injection timing angle on the exhaust gases toxicity, fuel economy and thermal condition of combustion chamber parts is done. As a result of the optimization research the characteristic cards control of pistons oilcooling and the fuel injection start for complex diesel engine technical and economic indicators improvement are proposed, the effectiveness of their use in automobile and tractor diesel engines and stationary diesel generators are estimated. The method of accounting of the most heat-loaded piston zones temperature state in general procedure of diesel quality assessment is proposed. Conducted estimated researches have allowed to define the effectiveness of pistons temperature state regulation on the combustion chamber resource strength.

L’H2, vecteur d’énergie prometteur, peut être synthétisé par les Thermococcales. La haute pression (HP) influencerait le métabolisme associé, mais n’a pas été envisagée en pratique. Après criblage d’isolats pour dégradations de substrats et productions d’H2, T. barophilus MPT, croissant préférentiellement à 40 MPa, a été choisi comme modèle, et sa fermentation a été décrite dans un contexte appliqué. Des méthodes HP ont été optimisées pour étudier l’H2. Un bioréacteur de 400mL de culture continue a été amélioré, maintenant des fluides corrosifs à HP hydrostatique (jusqu’à 120 MPa) et gazeuse (jusqu’à 40 MPa) jusqu’à 150 °C. Il a permis de mesurer la production d’H2 de notre souche à HP gazeuse. Un tube compressible pour culture discontinue à phase gaz étanche a été inventé, et a servi à mesurer la production d’H2 de T. barophilus en HP hydrostatique. Le métabolisme HP de la souche a été étudié grâce à des délétions préalables de gènes clés (mbh, mbs, co-mbh, shI, shII). Les rôles des enzymes liées ont été précisés via des mesures de croissances, produits (H2, H2S, acétate) et expressions génétiques des mutants, à 0,1 et 40 MPa. La tolérance à l’H2 de T. barophilus a été augmentée par évolution adaptative en laboratoire.« Evol », la souche fille acclimatée durant 76 générations à une saturation d’H2, a crû dans 10% d’H2, contrairement à la souche mère. Pour comprendre ces adaptations, les produits (H2, H2S, acétate), transcriptomes et génomes des deux souches ont été comparés. Avec 119 mutations génomiques, le métabolisme de l’H2 a été modifié dans le variant. Ce projet souligne l’intérêt du caractère piézophile des Thermococcales dans la bio-production d’H2 et permet de proposer des stratégies d’H2 et permet de proposer des stratégies d’optimisation
H2, a promising energetic vector, can be synthesized by Thermococcales. High pressure (HP) could influence the associated metabolism, but was not practically considered. After having screened isolates for assets in substrate degradation and H2 yields, T. barophilus MPT, growing optimally at 40 MPa, was chosen as a model and its metabolism was characterized in an applied context. Methods for HP culture were optimized for H2 studies. Our HP bioreactor for continuous culture underwent major improvements. This 400 mL container, able to maintain corrosive fluids at hydrostatic (up to 120 MPa) and gas (up to 40 MPa) pressures, at up to 150 °C, served to assess H2 production of our strain at high gas pressure. We also created a compressible device for discontinuous leak-free gas-phase incubations, allowing to measure T. barophilus HP H2 production (hydrostatic). HP adaptations of T. barophilus were observed thanks to previous deletions of key genes (mbh, mbs, co-mbh, shI, shII).We refined the roles of each concerned enzyme by assessing growths, end-products (H2, H2S, acetate), and gene expressions of the mutants, at 0.1 and 40 MPa. Additionally, we enhanced H2 tolerance in our model by adaptive laboratory evolution. “Evol”, the ensuing strain acclimatized to H2-saturating conditions for 76 generations, grew in 10% H2, contrarily to the parent strain. To understand such adaptation, we compared both strains’ end-products (H2, H2S, acetate), transcriptomes, and genomes.119 mutations were detected and the H2 metabolism was changed in the new variant. This work underlines the interest of Thermococcales’ piezophily for H2 bio-production and permits to propose optimization strategies

Based on cost considerations, the transformers connected to generators use no voltage tap changers.They are only operated without voltage conditions, so they must be optimized before the on-line phase. This thesis optimizes the setting of no voltage tap changer transformers connected to generators.The optimal goals are the maximum of reactive power absorbtion and the minimum of active power loss. The optimized setting of tap positions can stabilize the system voltage and reduce the active power loss. First, the thesis refers the IEEE C57.116 standard to learn the relationship between the generator voltage and the reactive power, and analysis the range of reactive power output to the system caused by the system voltage, unit transformer impedance and transformer taps. Second, the thesis use the reactive power corresponds to the system voltage and the generator voltage derivatives the currents cross the transformers high side and low side. The result of active power loss is the sum of the copper loss caused by the currents and the iron loss caused by the operation of transformers. In this thesis, the first simulation uses the full load situation to analysis the range of reactive power and active power loss under the conditions of generator voltage, generator excitation limit and system voltage. The second simulation uses actual generation situation to analysis the maximum of reactive power absorbtion and the minimum of active power loss under every tap position by power flow. Finally, the operators can use the result of the simulations in different generation situation to learn the reactive power output and the active power loss under every tap position.

Temperature setting of air-conditioned office is related thermal comfort of occupants and energy saving. This study focuses on suitable thermal emviromements in air-condition office. Field surveys were conducted 481 occupants in 56 offices (classified into affair offices and service offices) during the summer time in three metropolitans in subtropical Taiwan. The results show the standard effective temperature (ET*) in the offices in metropolitans, Taipei (north), Taichung (center) and Kaohsiung (south) at 25.3℃, 26.1℃, and 25.5℃. The comfort temperature in affair offices relates with the ceiling height, and in service offices relates with the planar density of occupants by multiple regression analysis. The setting temperature in affair offices and service office follows the analysis result in regression equations. The temperature of air-condition sets at 27 ℃ in order to conserve 6% energy(based on 26℃ by government recommends). These results provide a reference of temperature setting to achieve the energy saving.

Echocardiography is a diagnostic imaging technique by which ultrasound is used to display anatomic and physiologic characteristics of the cardiovascular system. Echocardiography consists of several different imaging modalities that require appropriate settings. In this chapter the most important system settings are discussed in the context of the basic physics of ultrasound image formation. Setting-up the echo machine to optimize patient examination is discussed in detail. All controls are covered. Continuous-wave, pulsed-wave, and colour flow Doppler are explained, as well as more advanced techniques including myocardial velocity imaging and speckle tracking and 3D imaging. Understanding these basic principles will allow optimizing image quality for each individual patient.

Disparities exist in the rate of incarceration of minorities, with substantial elevations occurring in African American, Latino, and Native populations. Cultural competence is an essential aspect of providing mental health care in any setting. An understanding of culture is even more important in correctional settings, as several unique factors may lead to conflict and misunderstanding if not adequately addressed. First, minority ethnic groups are vastly overrepresented in prisons and jails, so a familiarity with the predominant culture of those groups is necessary to engage inmates in treatment and diagnose them accurately. Second, mental health clinicians may be unfamiliar with law enforcement culture, which heavily influences the practices of corrections officers and differs significantly from health care culture. Third, many correctional psychiatrists grow up and train outside the United States, bringing their own cultural beliefs about crime and punishment into the American health care system. As the field of cultural psychiatry has developed, scholars have attempted to apply its principles to the correctional setting to deliver competent care in prisons and jails. These papers have provided guidance to correctional mental health clinicians on matters such as immigrant populations, language barriers, validity of psychological testing in different ethnic groups, stigma of mental illness in prison, religion’s role in coping with the stress of incarceration, and many others. This chapter reviews the evolution of cultural competence skills in correctional settings and current best practices in jails and prisons to optimize effective treatment outcomes.

Uterine compression of the great vessels in the second–third trimesters may contribute to ineffective resuscitative measures. In addition, in the setting of ongoing maternal compromise, fetal outcome may be optimized by delivery. The rarity of maternal cardiac arrest poses a multitude of challenges in the effective application of a perimortem cesarean delivery. Nevertheless, perimortem cesarean delivery remains an essential rescue maneuver for women with periviable fetuses who do not respond to initial resuscitative maneuvers.

Research has shown aggression toward others is a problem in a subset of military veterans. Predicting this kind of aggression would be immensily helpful in clinical settings. To our knowledge, there currently are no risk assessment tools or screens that have been validated to specifically evaluate acute violence among veterans. This chapter reviews what we do and do not know about violence in veterans so that clinicians who are making decisions about acute violence can be informed by the existing scientific knowledge base. Examining these empirically supported risk and protective factors using a systematic approach may optimize clinical decision making when assessing acute violence in veterans.

SummaryThis chapter discusses the issues relative to so-called configuration spaces that need to be set up before initiating the search for a solution. It starts by introducing some basic concepts, such as discrete and continuous subspaces. Then it discusses certain criteria that help us to determine whether the given configuration space is (or is not) adequate for the tasks at hand. One important topic which is addressed here is hyperparameter importance, as it helps us to determine which hyperparameters have a high influence on the performance and should therefore be optimized. This chapter also discusses some methods for reducing the configuration space. This is important as it can speed up the process of finding the potentially best workflow for the new task. One problem that current systems face nowadays is that the number of alternatives in a given configuration space can be so large that it is virtually impossible to gather complete metadata. This chapter discusses the issue of whether the system can still function satisfactorily even when the metadata is incomplete. The final part of this chapter discusses some strategies that can be used for gathering metadata that originated in the area of multi-armed bandits, including, for instance, SoftMax, upper confidence bound (UCB) and pricing strategies.

AbstractIn this chapter, the relation of salutogenesis to maternity care is discussed by giving a critical overview of studies in perinatal care, primarily measuring and promoting parental sense of coherence (SOC) and well-being.An overview is given on salutogenic approaches to neonatal and infant service provision. Important aspects of and salutogenic interventions for parent–child attachment in the first year of a child’s life are examined. Parents’ and caregivers’ relationship with their infants and newborns plays a critical role in shaping the emotional, cognitive, and social development of their child. Different interventions of early support to optimize parenting capacity and their impact are also discussed.Although the chapter focuses only on examples of salutogenic approaches based on reasonable evidence, there is a growing awareness of the value of salutogenic approaches to the provision of maternity care, and to facilities and services to enhance parenting and well-being in infancy and early childhood. Research gaps are identified, and suggestions for the direction of future research are outlined.

AbstractAlgorithmic recourse is concerned with aiding individuals who are unfavorably treated by automated decision-making systems to overcome their hardship, by offering recommendations that would result in a more favorable prediction when acted upon. Such recourse actions are typically obtained through solving an optimization problem that minimizes changes to the individual’s feature vector, subject to various plausibility, diversity, and sparsity constraints. Whereas previous works offer solutions to the optimization problem in a variety of settings, they critically overlook real-world considerations pertaining to the environment in which recourse actions are performed.The present work emphasizes that changes to a subset of the individual’s attributes may have consequential down-stream effects on other attributes, thus making recourse a fundamcausal problem. Here, we model such considerations using the framework of structural causal models, and highlight pitfalls of not considering causal relations through examples and theory. Such insights allow us to reformulate the optimization problem to directly optimize for minimally-costly recourse over a space of feasible actions (in the form of causal interventions) rather than optimizing for minimally-distant “counterfactual explanations”. We offer both the optimization formulations and solutions to deterministic and probabilistic recourse, on an individualized and sub-population level, overcoming the steep assumptive requirements of offering recourse in general settings. Finally, using synthetic and semi-synthetic experiments based on the German Credit dataset, we demonstrate how such methods can be applied in practice under minimal causal assumptions.

AbstractBrownfield is a polysemic notion that encompasses a whole range of diverse spaces. Although there is no official definition at the European level, an emerging consensus has arisen around the urban character of brownfields and the need for intervention. Indeed, their location within metropolitan areas represents a strategic opportunity to densify and rejuvenate the urban fabric at the neighbourhood scale. Hence, we propose a definition of urban brownfields that is flexible enough to optimize the potential development of abandoned sites and precise enough to enable framing the discussion. Then, we take a look at the diversity of urban brownfields in European metropolitan areas by briefly explaining the factors that cause a site to become a brownfield and then attempting to classify different types of urban brownfields. The proposed classification aims to create a non-exhaustive reference framework by offering in-depth knowledge of the urban brownfield and regeneration projects phenomenon across Europe, without setting its meaning in stone.

The purpose of this study is to examine how idle time treatment in the single-machine scheduling problem with distinct (unequal) release dates affects schedules. For this purpose, two types of settings are considered: any idle time is permitted and unforced idle time is forbidden. In the latter setting, idle time is permitted only when no jobs are available, that is, release dates of unprocessed jobs are larger than the current time instant. Under these two idle time settings, the problem is solved both offline and online. In offline scheduling, all job information is known in advance and the schedule is optimized only once at time zero while in online scheduling, the schedule is re-optimized for only currently available jobs every time when a new job becomes available at its release date. Benchmark instances in the literature are solved by these approaches and the effect of idle time treatment on the obtained schedules is examined.

Design of engines and associated nominal settings are normally computed and optimized so as to provide maximum performance, i.e. compromise between efficiency and emissions. For Combined Heat and Power (CHP) applications fuelled by gas network, this conception is adapted for a large range of gas chemical composition, up to a limit. The indicator used to describe gas quality is the methane number (MN). Hence, engine manufacturers normally supply specific information such as nominal engine settings and its associated limit value for the methane number (MNL). When the engine is operating under its nominal settings, a low grade gas (MN < MNL) can lead to engine knock. Knock is caused by auto-ignition of the end gas ahead of the flame in spark-ignition engine. Heavy knock can severely damage engine piston, constituting a main constraint for optimization of engine operating conditions. For an engine setting ES, (the vector ES includes the spark advance SA, the air fuel ratio AFR and the load), methane number requirement (MNR) is defined such as the minimum value of MN above which no-knock is ensured. The objective of this paper is to predict MNR as a function of engine setting for three engines. Simulation results show that the critical value of the considered knock criterion varies from an engine to another. A new normalized knock indicator based on the energy ratio is proposed to enable this comparison: laminar flame speed is assumed to be more sensible to MN variation than internal fluid dynamics (swirl, tumble, and squish) due to engine design.

Recent development of autonomous and connected trucks (ACT) has provided the freight industry with the option of using truck platooning to improve fuel efficiency, traffic throughput, and safety. However, closely spaced and longitudinally aligned trucks impose frequent and concentrated loading on pavements, which often accelerates pavement deterioration and increases the life cycle costs for the highway agency. Also, effectiveness of truck platooning can be maximized only in dedicated lanes; and its benefits and costs need to be properly balanced between stakeholders. This paper proposes a network-design model to optimize (i) placement of dedicated truck-platoon lanes and toll price in a highway network, (ii) pooling and routing of ACT traffic from multiple origins and destinations to utilize these lanes, and (iii) configuration of truck platoons within these lanes (e.g., lateral displacements and vehicle separations). The problem is formulated as an integrated bi-level optimization model. The upper level makes decisions on converting existing highway lanes into dedicated platoon lanes, as well as setting user fees. The lower-level decisions are made by independent shippers regarding the choice of routes and use of platoon lanes vs. regular lanes; and they collectively determine truck traffic in all lanes. Link-cost functions for platoon lanes are obtained by simultaneously optimizing, through dynamic programming, pavement-rehabilitation activities and platoon configuration in the pavement's life cycle. A numerical case study is used to demonstrate the applicability and performance of the proposed model framework over the Illinois freeway system. It is shown that the freight traffic is effectively channelized on a few corridors of platoon lanes and, by setting proper user fees to cover pavement-rehabilitation costs, systemwide improvements for both freight shippers and highway agencies can be achieved.
