Introduction. Practices of occupational therapists, particularly those supporting older persons with physical impairments, remain overly focused on remediating impairments, and implementation of occupation-centered practices remains fraught with difficulties. In Japan, this issue exists across the continuum from acute care to rehabilitation settings and into the community. This is despite the existence of international models and frameworks that place occupation at the core of the profession. Accordingly, there is a need to better understand how occupational therapists respond to the call for occupation-centered practices across the said continuum of care with this population. The aim of this study was at exploring and understanding occupational therapists’ experiences of supporting the resumption of occupations among older persons with physical impairments, in Japan. Methods. Embedded in a constructivist world view, this was a qualitative focus group study. Four focus groups (two in urban areas and one each in rural and semirural areas), consisting of seven or eight occupational therapists with at least three years of relevant practice experience, convened twice to narrate and explore their support of older persons. All were participating voluntarily with confidentiality of their participation being guaranteed by the researchers. They met for a third time to verify emerging analytic results. Data were analysed using a reflective thematic analysis. Results. Identified were three themes, namely, calling forth powers of occupations, imagining client’s future, and cocreating plots, which we synthesized into recurring cocreations from emerging opportunities. Discussion. Supporting the resumption of occupations among older persons with physical impairments hinges on repeated processes of identifying possibilities for occupation, followed by actions to bring these (e.g., images of clients’ future) into reality. Occupations’ healing properties (i.e., occupations’ powers) can be used to assist clients in experiencing health and well-being. The results suggest a reframing of occupational therapy practices as recurring processes of recognizing opportunities for occupation, followed by actions whereby these possibilities are turned into reality. Occupational therapy effectiveness might be enhanced when goals and methods are repeatedly and creatively aligned with the evolving plots cocreated between the client, therapist, and stakeholders.

Nonlinear dynamics systems theory, also referred to as chaos theory, is infiltrating the biomedical and social sciences, including the practice of occupational therapy. Consequently, the Neuro-occupation model has emerged for the treatment of different illnesses, in which a therapist strives to apply the benefits of this theory. This model provides occupational therapists with a method of anticipating the natural flux between underlying or hidden order and chaos in occupations, which allows individuals to function at their highest level. These occupations typically involve the interaction of numerous variables over time. By considering a nonlinear dynamics approach in illnesses such as alcoholism, occupational therapists may understand how meaningful occupations serve as a catalyst to facilitate change, which may assist in better understanding the effect of the therapeutic relationship with the clients they serve. This paper explains the nonlinear dynamics of the interventions in a single case study on alcoholism using the Neuro-occupation model.

This study is the first to examine the cultural sensitivity of the Canadian Occupational Performance Measure (COPM), as reported by occupational therapists who have used it with people from an ethnic minority background. The research was conducted as part of a Master's degree, held in Boston, USA, and funded by the Elizabeth Casson Trust, UK. Twenty-five practising occupational therapists from North America answered an emailed survey and, from this sample, three were interviewed by telephone. The findings showed that the practitioners perceived the COPM as able to address the cultural occupations of ethnic minorities; however, they felt that including examples of cultural occupations and children's occupations and modifying the rating scales would improve the tool. Two of the three therapists interviewed worked with children and, because of this, there was a focus on children's occupations that was only revealed in the second part of the research. The nature of the tool, its ability to focus on occupation and the therapist's interview skills were identified as the reasons that the COPM is effective in addressing the cultural occupations of ethnic minorities. Additional research is needed using a larger sample to validate these findings.

Maintaining relationships and interacting socially are essential aspects of the occupational performance of childhood and adolescence. Social participation occurs during many childhood occupations, such as play and school work. Occupational therapists assess and treat children with difficulties in social participation. Rather than assessing a child's social skills deficits in isolation, the use of occupation-based theoretical models guides clinicians to consider the individual in his or her environments and during occupations. Familiarity with existing models of social competence and available assessment tools provides occupational therapists with the basis for a comprehensive assessment of children. This paper presents an occupational therapy model (the Model of Human Occupation), models of social competence from cognate fields and a range of assessment tools in order to guide occupational therapists in assessing and treating children with social participation difficulties in a more occupation-centred manner. The paper also presents a rationale for the use of multiple methods for a comprehensive assessment of a child's social competence.

INTRODUCTION: Previous studies on bereavement have focused on the implications for bereaved individuals from behavioural and psychological perspectives. Limited literature discusses occupational therapists' perceived ability to treat bereaved individuals and how activities can be used to create new meaning to their lives. METHOD: A descriptive qualitative design was employed in the study to elicit occupational therapists' perceived ability to treat bereaved individuals find new meaning in their life. Thirty-five occupational therapists were recruited, and nine participated in two focus groups. Data collected were thematically analysed. FINDINGS: Three themes emerged, namely: (1) activities used in occupational therapy creates opportunities for new meaning to life after loss of a loved one, (2) appropriate occupational therapists' helping skills and professional behaviour to assist and treat the bereaved, and (3) perceived barriers to positive outcomes in treating the bereaved. CONCLUSION: Bereaved individuals often have impaired performance in occupations and emotional processing. The effects of bereavement were addressed using the appropriate occupational therapist helping skills and professional behaviour to guide holistic, occupation-based and client-centred treatment. Bereaved individuals were treated through purposeful activities, facilitation of therapeutic groups and self-management skills training which seemed to create opportunities for new meaning in their life. Occupational therapists need to be cognisant of the barriers that negatively influence the positive outcome of occupational therapy services.

This article proposes that the idea of meaningful occupations can be used by occupational therapists and occupational scientists to create organizational development interventions to help build sustainable business organizations. Review of recent organizational development literature indicates a trend toward an increasing lack of concern for individual employee needs. A conceptual model of organizational development that reinstates focus on the well-being of the individual as a basis for building sustainable organizations in which employee and organizational goals match is proposed for use by occupational therapists and occupational scientists who may be interested in contributing to this area of human endeavor. An occupation-based worker evaluation formula based on participation in occupations that are meaningful to an employee is suggested.

The importance of considering an individual's spirituality and the confusion over definition have been emphasised by the interest shown in this subject over the past decade. A working definition to help to reduce confusion was presented at the College of Occupational Therapists' annual conference in 2004 and it was felt that it would be useful both to review the attitudes and practices of occupational therapists in this area and to evaluate how they assess and meet spiritual needs. It appeared that practical spiritual needs were often thought of only in terms of religious and cultural traditions or rituals and, as a result, the potential of everyday meaningful occupations was not considered fully. Therefore, the use of occupation was reviewed in relation to meeting spiritual needs. Finally, the perceived barriers to incorporating spirituality into practice were investigated. The review showed that: ▪ Although a majority of participants recognise the importance of spirituality to health and illness and the potential benefits to treatment, the number of therapists actually incorporating spiritual needs into daily practice does not demonstrate this recognition ▪ Some occupational therapists feel confident about addressing and assessing spiritual concerns explicitly, but this tends to be situation based and dependent on factors such as client-therapist relationship, therapist awareness of spiritual issues and the expression of spiritual need by the client ▪ Further exploration of the potential of everyday occupations that address the spiritual needs of both religious and non-religious clients needs to be undertaken ▪ Spirituality needs to be addressed more fully in undergraduate occupational therapy programmes.

In recent years, the biomedical approach in the delivery of health care has been challenged by the ‘new public health’ model which promotes a humanistic, holistic approach to health care. Whilst occupational therapy has traditionally embraced this latter approach, evidence suggests that within the profession there is concern that technical advances (for example, the introduction of increasingly sophisticated equipment) have eroded many of the characteristics of a humanistic orientation to therapy. The extent to which occupational therapists endorse this new model was addressed in a survey of attitudes of practising occupational therapists (n = 378). A 22-item questionnaire, including aspects of therapist's knowledge and techniques, client characteristics, client-therapist relationships and approaches to professional practice, was modified and adapted for use with occupational therapists. The findings indicated that attitudes reflecting the new public health model — emphasising client-therapist interaction, client responsibility and holistic attitudes toward health care — prevailed, together with the therapist's knowledge and skills. Age and years of professional experience yielded significant differences, with older (30 or more years of age) and more professionally experienced (6 or more years) therapists endorsing a humanistic approach to therapy to a greater extent and with a greater emphasis on promoting the resources of the client than their younger and less professionally experienced counterparts. Recommendations for the education of occupational therapists and for professional practice are made, together with suggestions for further research.

Recent changes in the philosophy and structure of the National Health Service give greater emphasis to the prevention of ill health within locally defined communities. Occupational therapists, by virtue of their unique philosophy, have an opportunity to influence primary care strategy and practice by highlighting the links between environment, occupation and health. The recent changes in the structure of the National Health Service are described and the philosophy of occupational therapy is discussed in relation to these changes. This description provides the basis for recommendations as to how occupational therapists can work to build a recognition of the fundamental importance of adaptive occupation to individual health and, hence, to health at a community and population level. Working at a community and population level will require occupational therapists to strengthen links with health promotion and public health in order to help promote health through meaningful occupations within local settings.

The purpose of this study was to examine pediatric occupational therapists attitudes towards family-centered care. Specific attributes identified by the literature (professional characteristics, educational experiences and organizational culture) were investigated to determine their influence on these attitudes. Study participants were 250 pediatric occupational therapists who were randomly selected from the American Occupational Therapy Association special interest sections. Participants received a mail packet with three instruments to complete and mail back within 2 weeks. The instruments were (a) the Professional Attitude Scale (b) the Professional Characteristics Questionnaire, and (c) the Family-Centered Program Rating Scale. There was a 50% return rate. Data analysis was conducted in SPSS using descriptive statistics, correlations and regression analysis. The analysis showed that pediatric occupational therapists working in various practice settings demonstrate favorable attitudes toward family-centered care as measured by the Professional Attitude Scale. There was no correlation between professional characteristics and educational experiences to therapists' attitudes. A moderate correlation (r=.368, p These study findings suggest that organizational culture has some influence on occupational therapists attitudes toward family-centered care (R2 =.16). These findings suggest educators should consider families as valuable resources when considering program planning in family-centered care at preservice and workplace settings.

Dubin's and Cohen's (1970) Empirical Motivational Model of Updating (EMMPU) guided the design of this research, leading to the examination of the relationships among lifelong learning characteristics, job satisfaction, and engagement in professional updating activities for occupational therapists. Occupational therapists completed the Characteristics of Lifelong Learners in the Professions Questionnaire (Livneh, 1986), the Index of Organizational Reactions (Smith, 1976), the Professional Activity Engagement Survey, and participated in qualitative interviews.Statistically, no relationships were found among job satisfaction, characteristics of lifelong learning, and engagement in professional updating activities. The qualitative data suggested that these relationships existed. Characteristics of lifelong learning significantly predicted 7% of the variation in time spent engaged in professional updating activities. There were significant differences found on characteristics of lifelong learning and engagement in professional updating activities among therapists with varying educational 1eve1s.Utilizing grounded theory methodology, a theory of Professional Learning Management emerged from the data, contributing to the understanding of professional updating that was not addressed by the EMMPU model. In the theory of Professional Learning Management, patient treatment interaction triggered the need for updating by occupational therapists. The patient interaction determined the specifics of the necessary learning in regards to the content, timing, context, and methods. Occupational Therapists were reliant upon off-the-job learning in order to provide quality patient care. There was a cyclical feedback loop between off-the-job learning and application of that learning on the job within the patient treatment situation.Unlike the EMMPU model, updating occurred under both positive and negative contexts. The high engagers in professional updating were able to learn in spite of the barriers restricting learning efforts. The barriers to learning were not managed as well by the medium and low engagers in professional updating activities. Regardless of level of updating, occupational therapists utilized a variety of updating methods in accordance with the way in which the learning needs were defined by the patient treatment situation.3

Changes in health care delivery practices are impacting the provision of care in all venues. Occupational therapists working in hospital settings strive to meet professional mandates for occupation-based practice within a medical-model system. Ethical practice is another aspect of service provision vulnerable to contextual influences. The aims of the two studies reported here were to explore occupational therapists’ experiences with occupation-based practice, and with ethical issues, in hospital settings. Grounded theory methods were employed for both studies. Data were collected via individual, semi-structured interviews with 22 participants for the first study. For the second study, nine participants participated in individual, semi-structured interviews, journaling, and follow up interviews. Data analysis resulted in four emergent themes for each study. The main themes of the first study were Occupation-based practice expresses professional identity; Occupation-based practice is more effective; Occupation-based practice can be challenging in the clinic; and, Occupation-based practice takes creativity to adapt. The four themes of the second study were Anything less would be unethical: Key issues; I trust my gut: Affective dimension of ethical practice; Ethical practice is expected but challenging; and, It takes a village. Occupational therapists negotiate challenges inherent in contemporary hospital-based practice to provide occupation-based services and to practice ethically. Occupation-based practice is perceived to be more effective than biomedical approaches to intervention. Therapists must employ creative strategies to overcome challenges presented by medical-model service delivery contexts in order to provide occupation-based interventions. In comparison to other health care professionals working in adult rehabilitation practice, occupational therapists experience both common and unique ethical issues. A discovery of this study was that occupational therapists also experience ethical tensions related to team members’ and families’ sometimes subtle, and less frequently explicit, requests to falsify recommendations in documentation. Experiences with ethical issues include an inherent affective component in the form of moral distress and a strong sense of caring. The impact of systemic/organizational and relational forces is a reality that contemporary occupational therapists must negotiate in order to provide occupation-based and ethical practice.

Introduction: The prevalence of mental disorders is rising across the world and in South Africa driving the need for effective, occupation-centred practice in acute mental health care. In the acute setting, however, many occupational therapists experience challenges to practising in an occupation-centred manner. Occupation though remains the core construct of occupational therapy and occupational therapists everywhere are being urged to rediscover the power of occupation and embrace, develop, and maintain an occupation-centred practice. Aim: This study aimed to describe occupation-centred practice from the perspective of occupational therapists working in acute mental health care, in and around the City of Tshwane, South Africa. Methodology: A qualitative, explorative, descriptive design was used. Through maximum variance purposeful sampling nineteen participants were recruited to two focus groups. Transcriptions were analysed using the six steps of thematic analysis as described by Braun and Clarke to construct themes. Results: Four themes were constructed namely, 1. The process of occupation-centred practice, 2. Activities enable occupation-centred practice, 3. The theoretical underpinnings of occupation-centred practice and 4. Influencers of occupation-centred practice. Conclusion: Occupational therapists confirmed the centrality of occupation in their practice and further described occupation-centred practice as a process that entails the use of activities to facilitate experiences. Theoretical constructs that were helpful in guiding occupation-centred practice were highlighted. Influencers were experienced as either supporting or constraining occupation-centred practice. Significance: This study contributes to the evidence base of the profession in South Africa, ensuring that Occupational Therapy maintains its unique role and contribution to acute mental health care.

Occupational therapists working within Australia’s dynamic and complex publicly-funded health sector may experience ethical dilemmas as they fulfill their professional responsibilities, compromising client care, straining collegial relationships and negatively affecting their personal well-being. This thesis describes and explores the nature of the ethical dilemmas experienced by public practice occupational therapists. Section One: LITERATURE REVIEW The literature review comprises two parts. First a background to ethics in health care is outlined, where the Person-Environment-Occupation Model is presented as a way of evaluating the ethical dilemmas experienced by occupational therapists. This is followed by a review of literature exploring the ethical dilemmas experienced by health professionals including occupational therapists, speech pathologists, physiotherapists, social workers, rehabilitation counsellors and nursing and medical professionals. However, no literature was identified exploring the nature of ethical dilemmas experienced by public practice occupational therapists, providing an opportunity for further research. Section Two: JOURNAL MANUSCRIPT The journal manuscript outlines a qualitative, descriptive study exploring the ethical dilemmas experienced by five public practice occupational therapists. Findings indicate that public practice occupational therapists experience an array of ethical dilemmas which reflect the complexities of working within Australia’s publicly-funded health care context. Ethical dilemmas reported by public practice occupational therapists are encompassed by five themes: defining boundaries in professional relationships, fair access to quality services, professional status, life choices and the complex client at the heart of the dilemma. Person, environment and occupation factors were all found to contribute to the ethical dilemmas experienced by participants.

This qualitative study uncovered the identity construction of occupational therapists located in two regions of the United States. In utilizing an organizational communication studies lens, it was discovered that occupational therapists possess multiple, conflicting, and intersecting identities. The four central categories that emerged from the data were: Therapist as catalyst of functional activity, Therapist as envoy of profession, Therapist as navigator of bureaucracy, and Therapist as member of interdisciplinary team. Therapist as catalyst of functional activity proves to be the preferred identity of occupational therapists and the subsequent identities work to support or interfere with occupational therapists ability to see themselves as a catalyst of functional activity for their patients. This study further problematizes the notion that individuals have preferred identities and the effects this has on identity construction. The examination also provides useful insights for the development of Occupational Therapy as a profession.

Purpose. The purpose of the study was to describe Occupational Therapists’ experiences of working in Irish schools.  Method. Convenience sampling was used to recruit the eight participants in the study. A semi-structured interview was used to gain data about their experiences. The data was analysed through latent and manifest content analysis inspired by Olsson and Sörensen (2011) and meaning units were identified, condensed and labelled with codes. Result. The analysis resulted in three categories: “Resources impact the delivery of service” which shows how resources, within and outside of the participants control, impact the delivery of service and the challenges that might lead to. “Importance of communication and understanding” describes the dynamic between the participants’, school-staff and parents. ”Comparing school-based and clinic-based occupational therapy” where the participants share their experiences and opinions of school-based and clinic-based Occupational Therapy.  Conclusion. Resources and communication have a beneficial effect on Occupational Therapy in schools. Many factors add to the complexity of delivering the service. A preference of school-based Occupational Therapy is seen, however, Occupational Therapy today is generally more clinic-based. More research on the topic is needed.

Анотація:
In this paper, we propose to design, develop, and study a cyber-physical system that enables patients and therapists to virtually interact for rehabilitation activities with assistive robotic devices. The targeted users of this system are post-stroke patients. On the patient’s side, an assistive robotic device can generate the force that the therapist applies to the patient. On the therapist’s side, another robotic device can reproduce the responsive force generated by the patient. With this system, the interaction can be virtually established. In addition, by integrating real human trajectories, the proposed assistive robotic system can help patients to perform rehabilitation activities in their own pace. Such an assistive robotic system and virtual interacting scheme can minimize both patient’s and therapist’s traveling time. The assistive functions of this light weight design can also help patients to in their ADLs.

Abstract Attention Deficit Hyperactivity Disorder (ADHD) is characterized by "a persistent pattern of inattention and/or hyperactivity-impulsivity that interferes with functioning or development." (Diagnostic and Statistical Manual of Mental Disorders, 2014, p. 59). According to the Diagnostic and Statistical Manual of Mental Disorders (2014, p. 59) the inattention present in ADHD manifests itself "as rambling on tasks, lack of persistence, difficulty maintaining focus, and disorganization. In adults, these symptoms significantly affect occupational performance, leading to difficulties performing occupations that are "necessary activities that people need, want, and are expected to do" (World Federation of Occupational Therapists, 2012a, p. 2). Most have difficulty staying in their jobs, inconstancy in relationships, and when performing adult responsibilities, these instabilities can interfere with and cause feelings of frustration and guilt (Children and Adults with Attention-Deficit/Hyperactivity Disorder, 2022). Occupational performance is characterized as "the achievement of the selected occupation resulting from the dynamic transaction between the client, the context and environment, and the activity or occupation" (American Occupational Therapy Association, 2015, p. 14). In this case the games aim to provide strengthening of the ability to perform these actions, working the executive functions, the "executive functions (EF) refer to the ability to engage in goal-oriented behavior, performing voluntary and self-organized actions" (CAPOVILLA; ASSEF; COZZA, 2007, p. 1).
