European clinical guidelines recommend the use of Exposure and Response Prevention (ERP) and Comprehensive Behavioral Intervention for Tics (CBIT) as first-line treatments for tic disorders. Although ongoing efforts in research are being made to understand the mechanisms underlying these behavioral approaches, as of yet, the neurophysiological mechanisms behind behavioral interventions are poorly understood. However, this is essential to tailor interventions to individual patients in order to increase compliance and efficacy. The Theory of Event Coding (TEC) and its derivative BRAC (Binding and Retrieval in Action Control) provide a theoretical framework to investigate cognitive and neural processes in the context of tic disorders. In this context, tics are conceptualized as a phenomenon of enhanced perception–action binding, with premonitory urges constituting the perceptual and the motor or vocal expression constituting the action part of an event file. Based on this, CBIT is assumed to strongly affect stimulus–response binding in the context of response selection, whereas the effects of ERP presumably unfold during stimulus–response binding in the response inhibition context. Further studies are needed to clarify the neurophysiological processes underlying behavioral interventions to enable the individualization and further development of therapeutic approaches for tic disorders.

Trauma is universal, affecting health and behavioral choices. Trauma Informed Care (TIC) principles guide healthcare providers to avoid re-traumatization. States now mandate Trauma Informed Care (TIC) education. When implemented consistently, TIC benefits all. Adequate information integrating TIC principles in forensic nurse education exists. However, the application of TIC principles in forensic nurse practices remains elusive to measurement. The study's purpose is to explore forensic nurse knowledge before and following basic TIC interventional education, and recognition of TIC responses necessary to promote changes in behavior. The design is pre-, educational intervention, post-, and post-post survey that measures change in TIC intervention recognition. The analysis is a descriptive, correlational study to discover learning trends in practicing forensic nurses, determining the validity and reliability of the survey questions using an item response theory (IRT) model. Nineteen forensic nurses participated in the pre-test, intervention, post-, and/or post-post-test. The results revealed that nurses with >3 years’ and <10 years’ experience in nursing and forensic nursing are more likely to recognize TIC learning in a variety of situations. The authors summarize that moral conflict may influence choice in answers, reflecting a lack of recognition of TIC.

Grounded theory methodology revealed a central theme of OTs addressing TIC through direct and indirect interventions. The study concluded that therapists consider trauma and formulate interventions on the basis of the child’s needs from a holistic approach. Trauma-informed care (TIC) initiatives in state child welfare agencies are receiving more attention, but little empirical evidence exists as to their efficacy. The purpose of this study was to assess changes in self-reported practices and perceptions of child welfare staff involved in a multifaceted, statewide TIC intervention. Ten child welfare offices were matched and randomized to an early or delayed cohort. Staff were surveyed at Time 1 prior to any intervention, Time 2 postintervention for Cohort 1, and Time 3 postintervention for Cohort 2. The survey covered six domains: trauma screening, case planning, mental health and family involvement, progress monitoring, collaboration, and perceptions of the state’s overall system performance. Linear mixed modeling assessed the effect of the intervention. Cohort by time interaction was significant for three intervention targets. We demonstrate, using a rigorous study design, the mixed results of a multimodal intervention to improve trauma-informed attitudes, practices, and system performance. TIC initiatives must account for complex, dynamic contextual factors.

Over the past decade, behavioral interventions have become increasingly recognized and recommended as effective first-line therapies for treating individuals with tic disorders. In this article, we describe a basic theoretical and conceptual framework through which the reader can understand the application of these interventions for treating tics. The three primary behavioral interventions for tics with the strongest empirical support (habit reversal, Comprehensive Behavioral Intervention for Tics, and exposure and response prevention) are described. Research on the efficacy and effectiveness of these treatments is summarized along with a discussion of the research evaluating the delivery of these treatments in different formats and modalities. The article closes with a review of the possible mechanisms of change underlying behavioral interventions for tics and areas for future research.

AbstractPurpose of review:To summarize behavioral interventions for the treatment of primary tic disorders.Recent findings:Although tics were attributed to a disordered weak volition, the shift towards neurobiological models of tic disorders also transformed nonpharmacologic treatment practices. Current international guidelines recommend habit reversal training, comprehensive behavioral intervention, and exposure and response prevention as first-line therapies for tics. Appropriate patient selection, including age and presence of comorbidities, are salient clinical features that merit consideration. Evidence for further behavioral interventions is also presented.Summary:Currently recommended behavioral interventions view tics as habitual responses that may be further strengthened through negative reinforcement. Although availability and costs related to these interventions may limit their effect, Internet-based and telehealth approaches may facilitate wide accessibility. Novel nonpharmacologic treatments that take different approaches, such as autonomic modulation or attention-based interventions, may also hold therapeutic promise.

Aim: The main objective of this research was the effectiveness of mentalization-based therapy on parent-child interaction and parenting stress in a randomized controlled trial study on children with symptoms of tic disorders and their mothers. Methods: The method of the current research was quasi-experimental with a pre-test, post-test and follow-up plan with a control group. The research sample consisted of 30 mothers and their children with symptoms of tic disorders who had referred to counseling centers and clinics in Mako city in the second half of 2022. They were selected as available and randomly divided into two groups. Experiment and control (15 people in each group) were assigned. The experimental group participated in 15 sessions of the mentalization-based therapy group intervention program, and the control group did not receive any intervention. The subjects of both groups answered the questions of the parent-child relationship scale and the short form of the parental stress scale in the pre-test and post-test phases. Data analysis was done using mixed multivariate analysis of variance test. Results: The results of mixed multivariate analysis of variance indicated the effectiveness of mentalizing group therapy on improving the dimensions of parent-child relationship and parenting stress of children (P<0.001). Also, the results showed that mentalization intervention reduces the parenting stress scores of children with tic disorder symptoms (P<0.001). Conclusion: It can be concluded that the therapeutic mentalization group intervention is effective in improving the dimensions of the parent-child relationship and parenting stress of children with symptoms of tic disorder, and this treatment can be used to reduce the psychological problems of children with tic disorder. Introduction: Introduction: Transitions in care (TiC) interventions have been proposed to improve the management and outcomes of patients in emergency departments (ED). The objective of this review was to examine the effectiveness of ED-based TiC interventions to improve outcomes for adult patients presenting to an ED with acute atrial fibrillation or flutter (AFF). Methods: Methods: A comprehensive search of eight electronic databases and various grey literature sources was conducted. Comparative studies assessing the effectiveness of interventions to improve TiC for patients presenting to the ED with acute AFF were eligible. Two independent reviewers completed study selection, quality assessment, and data extraction. When applicable, relative risks (RR) with 95% confidence intervals (CIs) were calculated using a random effects model and heterogeneity was reported among studies using I-square (I2) statistics. Results: Results: From 744 citations, seven studies were included, consisting of three randomized controlled trials (RCT), three before-after (B/A) studies, and one cohort study. Study quality ranged from unclear to low for the RCTs according to the risk of bias tool, moderate in the BA trials according to the BA quality assessment tool, and high quality of the cohort study according to the Newcastle Ottawa scale. The majority of interventions were set within-ED (n=5), including three clinical pathways/management guidelines and two within-ED observation units. Post-ED interventions (n=2) included patient education and general practitioner referral. Four studies reported a decreased overall hospital length of stay (LoS) for AFF patients undergoing TiC interventions compared to control, ranging from 26.4 to 53 hours; however, incomplete and non-standardized outcome reporting precluded meta-analysis. An increase in conversion to normal sinus rhythm among TiC intervention patients was noted, which may be related to increased utilization of electrical cardioversion among the RCTs (RR=2.16; 95% CI: 1.42, 3.30; I2=%), B/A studies (RR=2.69, 95% CI: 2.17, 3.33), and cohort study (RR=1.39; 95% CI: 1.24, 1.56). Conclusion: Conclusions: Within-ED TiC interventions may reduce hospital LoS and increase use of electrical cardioversion. However, no clear recommendations to implement such interventions in EDs can be generated from this systematic review and more efforts are required to improve TiC for patients with AFF.

IntroductionThis paper outlines the study protocol for the Dutch Tackle Your Tics study in youth with tic disorders. Tourette syndrome and chronic tic disorders are prevalent neurodevelopmental disorders, placing considerable burden on youth and their families. Behavioural treatment is the first-line, evidence-based intervention for tic disorders, but tic reduction and availability remain relatively low. Patient associations stress the need for more accessible high-quality treatments, also focusing on improving quality of life. Therefore, the brief, intensive group-based treatment Tackle Your Tics was developed.Methods and analysisTackle Your Tics is a 4-day intensive and comprehensive group-based intervention for children and adolescents (9–17 years) with Tourette syndrome or a chronic tic disorder. The programme encompasses exposure and response prevention treatment and additional supporting components (coping strategies, relaxation exercises and parent support). To study the effectiveness of Tackle Your Tics and identify predictors/moderators at baseline, a single-blinded randomised controlled trial (n=104) is conducted, comparing Tackle Your Tics (n=52) with a waiting list condition lasting 3 months (n=52). Assessments are performed at similar time points for both groups: at baseline, after 4 weeks, and at 3 and 6 months of follow-up, on tic severity, quality of life and other psychosocial variables.Ethics and disseminationEthics approval has been obtained from the medical ethical committee of the Amsterdam Medical Centre (METC nr NL66340.018.18, v3 June 2020). Findings will be presented on national and international conferences, peer-reviewed scientific journals, patient organisation meetings and public media. Patient representatives are fully integrated as part of the research team. If Tackle Your Tics proves to be effective, it can expand evidence-based treatment possibilities for children and adolescents with tic disorders. Identifying the psychosocial predictors/moderators for the effectiveness of this intervention can provide personalised treatment advice in the future.Trial registration numberNL8052.

Tic disorders can have a significant emotional and social impact on children and their families. There is increasing support for the use of parenting programmes in childhood conditions; however research into the applicability of such programmes in tic disorders is limited. This thesis therefore aimed to investigate the topic of parenting interventions in tic disorders. The thesis presents five chapters, written as a series of self-contained papers and prepared in accordance with selected journal submission guidance. Paper 1 is a systematic literature review of the implementation and effectiveness of behavioural parent training programmes across neurodevelopmental disorders. Twenty-two randomised controlled studies were included in the review. Neurodevelopmental disorders included attention deficit hyperactivity disorder, autistic spectrum conditions, intellectual difficulties and developmental disorders and tic disorders. Training programmes included Triple P, Barkley’s Defiant Children, Parent Child Interaction Therapy, New Forest Parenting, Parenting your Hyperactive Preschooler, Parents Plus Children, Preschoolers with Autism, Incredible Years and an idiosyncratic programme. Effects of interventions on child outcomes were examined alongside intervention characteristics and content. Results indicated robust evidence for effects of parent training on attention-deficit/hyperactivity difficulties and externalising difficulties, across neurodevelopmental disorders and training programmes. Evidence to support the effect of parent training on internalising, social and global difficulties across disorders was less robust, with fewer and inconsistent outcomes reported. Clinical and research implications resulting from the findings are discussed. Papers 2 and 3 present empirical studies. Both studies are Q-methodological investigations into opinions on parenting interventions in tic disorders. Q-methodology is a technique which enables participants’ subjective viewpoints to be grouped using by-person factor analysis. Views on the acceptability, effectiveness, feasibility and utility of parenting interventions were explored across parents of children with tic disorders (Paper 2) and professionals (Paper 3). Across both studies, seven main factors were identified (parents four, professionals three). Findings highlighted that interventions were generally considered acceptable, justified and perceived as needed. However, important differences in opinions were found within and between parent and professional groups, highlighting key clinical considerations for possible intervention format, delivery and content. Paper 4 provides a commentary of the clinical implications of these findings when the two studies are considered together, and provides guidance to further develop and implement interventions. Paper 5 presents a critical review, including discussion of the strengths, limitations and implications of the findings, alongside personal reflections on the research process.

Chronic tic disorders (CTDs), including Tourette’s disorder and persistent motor or vocal tic disorder, are neurobiological conditions affecting an estimated 3 to 4 percent of children and adolescents. These disorders include the presence of motor and/or phonic tics, which can range in number, frequency, and severity. Although CTDs are typically treated through medications, the available medications have the potential of adverse side effects, do not result in long-term coping strategies, and may not be effective or preferable for all individuals. Habit reversal training (HRT), a behavioral intervention for tics, has been identified as a well-established treatment. The purpose of habit reversal is to build an individual’s awareness of his or her tics and disrupt tics through developing a competing behavioral response. One avenue or service delivery that has not yet been explored for individuals with CTDs is use of a parent-mediated approach to habit reversal. Parent-mediated interventions have been used successfully with children with challenging behavior and autism. They are based on a triadic model, in which a therapist works directly with a parent to teach the therapeutic techniques. Parents, in turn, work directly with their children while receiving feedback from the therapist. The current study investigated a parent-mediated habit reversal intervention for the treatment of chronic tic disorders in two children. A delayed multiple baseline design was used. Baseline data were collected for three sessions. Intervention was delivered over six sessions, during which time coaching was faded. Follow up data collection occurred one month after the end of treatment. Based on changes in parental fidelity of implementation from baseline to intervention, this study provides preliminary evidence for parents being trained as therapists and providing habit reversal training strategies to their children. This treatment was reported as acceptable by both parent participants and by one of the child participants. Treatment motivation remained high and stable throughout baseline, intervention, and follow up. Child motivation was more variable during baseline, intervention, and follow up. Tic severity was also variable and more research would be needed to determine the impact of treatment for tics. Limitations and implications for future research are provided.

La question que nous nous posons dans cette thèse est dans quelle mesure les TIC peuvent favoriser l'émergence de projets innovants entre acteurs de la recherche publique et de la recherche privée ? Plus spécifiquement, nous reconnaissons dans cette recherche, à l'instar de nombreux auteurs, que c'est moins la dimension technique que les modalités de mise en œuvre de la solution qui importent [Benghozi, 2001]. Dans le cas où les pratiques sont incertaines et complexes parce que mêlant des acteurs différents, une réflexion sur la notion d'usage nous conduira ainsi à mettre en évidence la nécessité d'une co-conception des solutions TIC et à proposer la mise en œuvre de cette démarche à travers une recherche intervention dans le cadre du projet KMP au sein de Telecom Valley. Cette expérimentation nous permettra alors de mettre en exergue le rôle de la construction d'objets frontières dans la co-évolution de la conception et des usages.

Robots play a critical role in our everyday lives, performing tasks as diverse as maintenance, surveillance, exploration in harsh environments, or search and rescue operations. Concerning the different environments where they operate, the submarine is one of those that has increased its activity the most. Nowadays, there are three types of robots: ROVs, AUVs and HROVs. Despite the differences in structure and capabilities, there is a problem common to all three: the human-robot interaction has various deficiencies, and the user continues to play a central role from the point of view of decision-making. This thesis is focused on research related to human-robot interaction: the use of algorithms to assist the user during the mission specification (making the user interface easy to use), the exploration of a multimodal interface and the proposal for a robot control architecture (allowing change from autonomous to teleoperated, or vice versa).
Los robots desempeñan un papel fundamental en nuestra vida cotidiana, realizando tareas tan diversas como mantenimiento, vigilancia, exploración en entornos hostiles u operaciones de búsqueda y rescate. De entre todos los entornos donde actúan, el submarino es uno de los que más ha aumentado su actividad. Los tipos de robots utilizados son: ROVs, AUVs y HROVs. Existe un problema común a los tres: la interacción hombre-robot presenta diversas deficiencias y el usuario sigue jugando un papel central desde el punto de vista de la toma de decisiones. La presente tesis está centrada en la investigación relacionada con la interacción hombre-robot: el uso de algoritmos para asistir al usuario durante la especificación de la misión (haciendo que la interfaz de usuario sea fácil de usar), la exploración de una interfaz multimodal y la propuesta de una arquitectura de control del robot (permitiendo cambiar desde autónomo a teleoperado, o viceversa).

Cette thèse se compose de trois essais qui contribuent collectivement à notre compréhension de la gestion de la solitude chez les personnes âgées en perte d’autonomie et chez les soignants professionnels grâce à des interventions basées sur la technologie. Le premier essai identifie plusieurs voies de recherche dans la littérature concernant les interventions des TIC pour les personnes âgées bénéficiant de soins à domicile professionnels. Il met en évidence le besoin de recherches supplémentaires et reconnaît le rôle central des soignants professionnels dans ces interventions. Le deuxième essai explore le point de vue des aidants professionnels et révèle leur contribution substantielle à l'atténuation de la solitude des personnes âgées grâce à un accompagnement émotionnel et au soutien cognitif. Dans le troisième essai, des solutions de santé mobile, telles que la musique et la messagerie instantanés, sont tester pour traiter la solitude au travail des aidants professionnels, ouvrant la voie à des solutions pratiques permettant aux organisations d’améliorer le bien-être et la rétention des aidants. Dans l'ensemble, ces essais enrichissent notre compréhension théorique et pratique du potentiel de la technologie pour atténuer la solitude chez les personnes âgées et, dans le contexte des aidants professionnels, fournissent ainsi des perspectives précieuses pour améliorer la qualité des soins et le bien-être global
This thesis comprises three essays that collectively advance our understanding of loneliness mitigation among the elderly with loss of autonomy and professional caregivers through technology-based interventions. The first essay identifies a literature gap in ICT interventions for elderly recipients of professional home caregiving. It highlights the need for further research and recognizes the pivotal role of professional caregivers in these interventions. L’avènement des TIC accentue le phénomène des communautés virtuelles. Privilégiant plutôt l’aspect social que technologique, l’objectif de cette thèse est de comprendre la communication et l’organisation dans ces communautés. Le problème terrain a été d’étudier ce phénomène dans un contexte communautaire. Les investigations se sont alors focalisées sur l’analyse d’un site sportif d’e-commerce primé par de nombreux prix d’innovation en terme de modèle de constitution communautaire: Sportmalin. Com Dès lors sur le plan théorique, nous envisageons les TIC comme « des artéfacts communicationnels ». Le principal intérêt est de révéler que le phénomène communautaire provient majoritairement d’une médiation entre trois objets antinomiques du système mais néanmoins interreliés : les outils communautaires, les enjeux mercantiles et la volonté des membres de faire partager leurs connaissances. Pendant 20 mois, l’analyse nous apprend qu’une adéquation est possible entre une problématique mercantile et communautaire. Tel un rouage, chaque partie (communautés virtuelles, outils communautaires, TIC, marketing et multinationales) se complète tout en restant dépendante les unes des autres. L’ensemble du phénomène de communauté virtuelle résulte de stratégies d’acteurs et de choix rationnels. Bien que les communautés virtuelles semblent être le produit d’efforts délibérément coordonnés d’un ensemble d’acteurs, le facteur humain constitue la variable instable de ce système communautaire. La conclusion révèle que la « communauté virtuelle commerciale », telle que nous allons la décrire au cours de cette étude, constitue le modèle universel de toute communauté virtuelle artefact
The increasing of ICT accentuates the phenomenon of the virtual communities. Rather privileging the social aspect more than the technological aspect, the aim of this thesis is to understand communication and organization in these communities. The ground problem has been to study this increasing phenomenon in a community context. The investigations then focused on the analysis of a community site of e-business in sports awarded by numerous innovation prizes as model of a community constitution: sportmalin. Com. Therefore from a theoretical point of view, it is necessary to understand the ICT as “communicational actefacts”. The principal interest was to reveal that the community phenomenon mainly results from a mediation between three antinomic objects of the system but nevertheless linked together : the community technical tools, the mercantile stakes and the will of the members to share and exchange their knowledge. The survey led during twenty months teaches us that an adequacy is possible between a mercantile and a community problematics. Just like a cog of a system, every part (virtual communities, community tools, the ICT, marketing initiative and the multinationals) completes the other as well as remaining dependent with the others. The analysis grants that the whole phenomenon of virtual community is the sole result of strategy of actors and rational choices. Although the virtual communities seem to be the product of deliberately coordinated efforts of a set of actors, the human factor is the unstable variable of this community system. The conclusion of this research reveals that the “ virtual commercial community ”, such as we are going to describe it in this study, establishes the universal model of any virtual community artefact

Abstract The purpose of the study was to explain the effects of risk factor targeted lifestyle counselling intervention on quality of lifestyle counselling and on adherence to lifestyle change in stroke patients. Content and characteristics of stroke patient counselling was described (I) with narrative literature review (n=33). Lifestyle habits of hospital-admitted stroke patients were described and explained (II) with cross-sectional design (n=150). Quasi-experimental, nonequivalent control group pretest-posttest design explained (III) the effects of lifestyle counselling intervention on working-age stroke patients’ adherence to lifestyle change (n=75/group). Cross-sectional, explorative between-groups prospective design explained (IV) associations between the quality of counselling and adherence to lifestyle change during the 12-month follow-up (n=98). The literature data was gathered from the databases and analysed with content analysis (I). The empirical data was collected with Lifestyle (II), Adherence to Lifestyle Change (III, IV) and Counselling Quality (IV) instruments. Descriptive statistics, factor analysis, cross-tabulation of variables, logistic regression, and ANCOVA were used for data analysis. Counselling was described with seven themes: information about the disease and concerns regarding stroke, aims of counselling, counselling methods, interaction as a method for counselling, stroke nurse as a counsellor, emotional support and decision making in patient care (I). Patients were overweight or obese (75%) and 86.7% had high waist circumference levels. They generally had rather unhealthy eating habits, were physically inactive and quite highly stressed, and 33.5% drank to excess every month or every week, and 32.9% were current smokers (II). After intervention the experimental group lost their weight at 3 and 6 months, reduced cigarette consumption at 3 months, and increased smoking cessation at 6 months. The experimental group received more support from nurses, family and friends (III). Lifestyle counselling quality was estimated rather good, with the exception of patient-centred counselling, and significantly better by the experimental group. Interactiveness, resources, and advantages of counselling were related to adherence variables (IV). Intervention had effects on lifestyle counselling quality and on adherence to lifestyle change in the short term
Tiivistelmä Tutkimuksen tarkoituksena oli selittää aivoverenkiertohäiriöpotilaiden (AVH) riskitekijöihin kohdistuvan elintapaohjausintervention vaikutuksia elintapaohjauksen laatuun ja elintapamuutokseen sitoutumiseen. Ohjauksen sisältöä ja ominaispiirteitä kuvattiin (I) narratiivisen kirjallisuuskatsauksen avulla (n=33). AVH-potilaiden elintapoja (n=150) sairaalaan tulovaiheessa kuvattiin ja selitettiin (II) poikkileikkaustutkimuksella. Kvasikokeellinen tutkimus selitti (III) elintapaintervention vaikutuksia työikäisten AVH-potilaiden elintapamuutokseen sitoutumiseen (n=75/ryhmä). Ohjauksen laadun ja elintapamuutokseen sitoutumisen välistä yhteyttä 12 kuukauden seurannan aikana selitettiin (IV) poikkileikkaus- ja pitkittäistutkimuksen avulla (n=98). Seitsemän teemaa kuvasi ohjausta: tieto sairaudesta ja siihen liittyvistä tekijöistä, ohjauksen tavoitteellisuus, ohjausmenetelmät, vuorovaikutus ohjausmenetelmänä, sairaanhoitaja ohjaajana, emotionaalinen tuki, sekä potilaan hoitoon liittyvä päätöksenteko (I). Potilaista 75% oli ylipainoisia tai lihavia ja suurella osalla vyötärönympärys oli liian suuri (86.7%). He söivät jokseenkin epäterveellisesti, liikkuivat liian vähän ja olivat stressaantuneita. Potilaista 33.5% joi humalahakuisesti kuukausittain tai joka viikko ja 32.9% tupakoi (II). Koeryhmän potilaiden paino laski kolmen ja kuuden kuukauden kuluttua kotiutumisesta. Savukkeiden määrä väheni kolmen kuukauden kuluttua kotiutumisesta ja tupakoinnin lopettaminen lisääntyi kuuden kuukauden kuluttua. Koeryhmän potilaat saivat enemmän tukea hoitajilta, perheeltä ja ystäviltä (III). Ohjauksen laatu arvioitiin hyväksi, poikkeuksena oli ohjauksen potilaskeskeisyys. Stillasittande framställs allt mer som en oberoende riskfaktor och är ett vanligt förekommande fenomen på arbetsplatsen. Detta är sammankopplat med en förhöjd risk för de stora folksjukdomarna, exempelvis hjärt- och kärlsjukdomar samt olika cancerformer. Syftet var att undersöka interventioner för att motverka långvarigt stillasittande på arbetsplatsen. Detta examensarbete är en litteraturstudie och tio stycken vetenskapliga artiklar inkluderades. Artiklarna hämtades från databaserna Pubmed och Cinahl, genom sökorden ”sedentary behavior”, ”sedentary behaviour”, ”sitting time” och ”workplace”. Vid databearbetningen genomfördes en temaanalys och tre kategorier fastställdes, ”e-hälsa avbryter stillasittande”, ”arbetsstationer reducerar tid i stillasittande” och ”multikomponenta interventioner påverkar stillasittande i större utsträckning”. De huvudsakliga resultaten tyder på att vid interventioner med e-hälsa som tillvägagångssätt, förekom ökningar i antal pauser från långvarigt stillasittande. Vidare kunde olika arbetsstationer tydligare minska tiden i stillasittande och därmed även öka tiden i stående ställning. Slutligen påvisade de multikomponenta interventionerna mest kompletta förbättringar gällande stillasittande beteenden inom flera avseenden. Därmed påvisades multikomponenta interventioner överlag vara mer fördelaktiga och kompletta. I framtiden bör forskningen framförallt inrikta sig på att kartlägga interventioners effektivitet på längre sikt, i syfte att säkerställa bibehållandet av förbättrat stillasittande beteende.

L’approche Orientation Cognitive au Rendement Occupationnel Quotidien (CO-OP) est reconnue pour améliorer le rendement des tâches entraînées chez les enfants avec un trouble d'acquisition de la coordination (TAC). Récemment, son efficacité pour le transfert d’habiletés apprises sur des tâches non entrainées durant les séances d’intervention (transfert intertâche) a été démontrée chez les adultes, ayant subi un accident vasculaire cérébral (AVC). À ce jour, comme aucune étude n’avait exploré le transfert intertache chez les enfants avec TAC, cette étude à cas unique visait à déterminer si CO-OP favoriserait l'amélioration d’une tâche non entraînée, comme chez les adultes. Quatre enfants (7 et 12 ans), avec un TAC, ont reçu 10 séances d’intervention CO-OP, où trois tâches ont été travaillées et une quatrième fut utilisée uniquement pour mesurer le transfert. Le Performance Quality Rating Scale avec définitions (PQRS-OD) a permis de mesurer la performance. Une bande à trois écarts types fut la méthode d’analyse choisie. Contrairement aux adultes, seulement deux des quatre participants ont obtenu un transfert intertâche. Ces résultats indiquent que des recherches supplémentaires sont nécessaires pour déterminer la façon de mieux favoriser un transfert intertâche chez les enfants.

This chapter provides a broad overview of tic disorders. Specifically described are different tic disorders and the psychiatric and psychosocial difficulties commonly experienced by children with tic disorders. Common conceptual models of tic disorders and neurobiological and behavioral research that support these models are reviewed. Recommended practices for assessing children with tic disorders are discussed, commonly used measures for assessing tic-related severity and impairment are detailed, and both evidence-based and experimental treatments for tic disorders are described. A case example is provided to demonstrate how clinicians may implement the recommended assessment practices and one of the evidence-based treatments discussed in the chapter (Comprehensive Behavioral Intervention for Tics). The chapter concludes by considering theoretical and practical challenges and implications specific to tic disorders.

Many psychiatric disorders of childhood have a chronic course. As such, they impact multiple developmental epochs and negatively influence developmental trajectories. While early identification and intervention may minimize, or even prevent, symptoms being carried into adulthood, the availability of evidence-based treatments is sparse in children and adolescents compared to adult populations. Establishing effective interventions for psychiatric symptoms presenting in childhood is critical given the chronic course of most psychiatric disorders. This chapter describes psychopharmacological and psychosocial interventions used for the treatment of childhood psychiatric conditions, with an emphasis on empirically supported treatments. Both symptom- and diagnosis-specific approaches are described as well as the use of combined interventions for the following childhood psychiatric conditions: autism spectrum disorder (ASD), intellectual disability (ID), attention-deficit/hyperactivity disorder (ADHD), anxiety, depression, obsessive compulsive disorder (OCD), chronic tic disorders, eating disorders, and conduct problems.

This chapter briefly describes tic disorders and trichotillomania (TTM) and reviews the pharmacotherapy and psychosocial treatment outcome literature for each of these conditions. In contrast to anxiety or depression, distorted or maladaptive cognitions do not appear to play a central role in the etiology or maintenance of tic disorders and TTM, and therefore cognitive therapy is not emphasized in the psychosocial treatments studied to date. Treatment protocols are best characterized as “behavioral,” although some include ancillary cognitive interventions. Behavioral treatments that include habit reversal training (HRT) appear to hold the greatest promise for each of these conditions, and these are described in some detail. Future directions in treatment research are suggested.

The therapy manuals included in this volume—the Unified Protocols for Transdiagnostic Treatment of Emotional Disorders in Children (UP-C) and Adolescents (UP-A)—include evidence-based treatment strategies to assist child and adolescent clients to function better in their lives. The manuals include specific guidelines for treatment delivery, and they also contain information about how to introduce parent-directed strategies to help promote long-term uptake of youth-directed therapy skills. The evidence-based treatment skills presented may be applied by therapists to children and adolescents with a wide variety of emotional disorders. This treatment guide takes a transdiagnostic approach to the treatment of emotional disorders. Some of the disorders that may be targeted include anxiety disorders and depressive disorders. This treatment is flexible enough for use with some trauma and stress-related disorders (including adjustment disorders), somatic symptom disorders, tic disorders and obsessive-compulsive disorders. The transdiagnostic presentation of evidence-based intervention techniques within these treatments may be particularly useful for children and adolescents presenting with multiple emotional disorders or mixed/subclinical symptoms of several emotional disorders.

The Unified Protocols for Transdiagnostic Treatment of Emotional Disorders in Children and Adolescents (UP-C and UP-A) are evidence-based, transdiagnostic intervention programs that target core emotion regulation processes that may be shared across varying presentations of internalizing distress or disorders in youth. Given their popular transdiagnostic and modular structures, the UP-C and UP-A have quickly been disseminated and implemented with a variety of populations and in differing treatment settings. This volume aims to aid UP-C and UP-A therapists in understanding varied applications and modifications of these approaches and assist them in applying such in their own practice. To that end, chapters are offered on not only standard UP-C and UP-A research and practice but also applications for youth with obsessive-compulsive symptoms, tic-related concerns, substance use, serious mental illness, and eating disorders. Structural modifications to the UP-C and UP-A using a stepped care model and when delivering care in pediatric or community practice settings and in differing cultures or languages are also discussed. To aid in their use, each chapter includes a brief, user-friendly description of the modifications or adaptations of the UP-C and/or UP-A that are described therein.

Implanted venous ports in children are commonly placed by pediatric interventional radiologists, primarily through the internal jugular vein to allow long-term delivery of intravenous access. The ideal location of the catheter tip is the junction of the superior vena cava and right atrium. High catheter tip positions may result in failure of the port catheter due to venous thrombosis, catheter occlusion, or fibrin sheath formation, whereas low catheter tip positions may cause catheter occlusion, cardiac arrhythmias, or perforation. This chapter describes indications, contraindications, equipment, techniques, and complications of the endovascular snaring technique of central venous port tip revision in children.

Abstract Objectives/Scope The purpose of this paper is to share knowledge from utilizing a novel behavioral change management approach, that incorporates agile, ideation and experimentation methods, to improve Asset Integrity and Process Safety Management (AIPSM) at Petroleum Development Oman. This paper includes an overview of the methods used to achieve positive shifts in safety leadership Behaviors observed during team AIPSM engagements or "huddles". Methods, Procedures, Process Our Thriving in Change (TIC) approach was utilized, and the following methods were deployed: 1) Observable Behaviors identification: This involved identifying the leadership Behaviors critical to AIPSM success. 2)Ideation: This creative process involved co-creating possible behavioral intervention ideas or "nudges" with key stakeholders. 3)Prioritization: Ideas were prioritized based on the most passionate first since people support what they are most passionate about. The selected idea was then further improved to fit within typical cost, time and do-ability constraints. 4)Behavioral Experimentation: The idea was then converted into an experiment card to facilitate the planning, measurement and learning around the behavioral experiment Results, Observations, Conclusions The primary outcomes of this process were: 1) Behavior Observation card: a simple and cost effective co-created solution to nudging leaders at all levels towards the Behaviors that are required in creating a culture that supports AIPSM 2)Minimal resistance: People support what they help to develop. The approach of selecting the most passionate idea first helped drive higher levels of engagement and participation in the execution of the idea. 3)Create a learning organization: The PDCA nature of the experimentation card promotes cycles of learning and experimentation until the goal is achieved. 4) Speed of implementation: The agile approach encouraged quicker doing and results. Learning our way into it rather than suffering "analysis paralysis". The observations from our behavioral experiment: The first round of experimentation was not successful, however it provided us with several learnings such as shifting the language used and making the observation card simpler and easier to use. The second round of experimentation in 4 different areas provided a more positive result. Within a month we observed improvements in the targeted leadership Behaviors and huddle leaders viewed this as a valuable coaching tool. The observation card is now being rolled out across all PDO AIPSM "huddles". As a conclusion, the observation card helped to improve observable leadership Behaviors such as communication, transparency, respect etc, which improved the overall AIPSM management. Novel/Additive Information Holistically the approach used can be transferred to any other business with some customization to suit each organization business nature. The utilization of change management tools such as ideation and behavioral experimentation is not common in the oil field sector. Looking to the future we can expand the utilization of the change management tools to other HSE project and furthermore to support some PDO strategic projects.

At first, we referred to the values within the European dimension of education, as they were established by the European Commission for every state. Here we made special reference to the plan of the educational contents, as it has to contain elements of proximity and coincidence. Secondly, we talked about the new paradigms met with the educational policies. Here we stress the importance not only of knowledge, but also of competences and values the students will achieve. In order to fulfil this task, the usage of the TIC and of the educational resources opened for every type of educational contexts should be intensified. Here we also mentioned the series of activities which help the learning of the common language for a European citizenship and the defining of the new educations, adapted to the dimension of education, the European Commission and the Council of Europe propose. The next treated aspect was to establish the defining of the problem mentioned at the beginning. Here we reach the following objectives: to describe the nature of the problem, to establish the scale of the problem, to identify the affected categories, to establish the causes of the problem, to argue the need for intervention, to estimate the risks and the uncertainty of the problem discussed and also to present the healing activities. In order to get real information, we used the following methods: questionnaires (were disseminated to two different categories of respondents: students who want to get a job on one hand and employers on the other hand) and observations. The activities supposed data collecting, processing, analyzing and interpretation. In the end we draw the conclusions.

Abstract Reliable evaluation of the cement-bonding quality and identification of isolation zones of a cased-hole well are challenging problems, particularly for a plugged and abandoned (P&A) well. Ultrasonic tools have been developed to conduct pitch-catch and/or pulse-echo measurements for cement evaluation at high spatial resolutions. Recently, extended data processing of pitch-catch measurements has been developed to identify third-interface echoes (TIE) from flexural mode waveforms. The derived information of TIEs can be integrated with flexural attenuation rates of casing and acoustic impedances of annulus materials to enhance the accuracy and confidence of evaluations of cement quality and zone isolation. However, there are limitations in conventional pitch-catch measurements. The conventual pitch-catch measurements are longitude measurements. Their vertical resolution is limited by the spacing between transducers. A utilized piezoelectric transducer used by such measurements needs a liquid couplant. The received signals of this kind of sensor are sensitive to the mud density. The heavy mud may cause strong attenuations of intensities of received flexural mode waveforms. Additionally, a piezoelectric sensor is sensitive to the direction of wave propagation. Therefore, a TIE can be missed if two walls of the annulus of a well are not parallel, such as a deviated well. This paper introduces a new compensated pitch-catch measurement method for reliably detecting the eccentricity of the inner pipe and annulus material in a cased-hole environment. The electromagnetic acoustic transducers (EMATs) are utilized to excite and acquire Lamb and shear horizontal waves, respectively, which propagate circumferentially. The operation parameters of this new measurement method are optimized to excite and acquire waves for more reliably extracting TIEs from received waveforms. Compared with piezoelectric sensors, EMAT sensors do not require couplants and are not sensitive to the wave propagation angle, the mud density, and the rugosity of the pipe surface. The vertical resolution of the Lamb wave measurements is controlled by the vertical sampling rate of the tool and the sensor size. This new measurement method has been validated with Lab measurements. The test fixtures with varied annulus spacings were designed, constructed, and cemented. Multiple tests were designed and conducted to verify the modes of Lamb and shear horizontal waves, existences of TIEs with different operation parameters of measurements, and the relations between arrival times of TIE and annulus spacing, as well as filled in materials of annulus. The visibility of TIE for a deviated inner pipe has also been confirmed. The tests results confirmed the optimal operation parameters of this new measurement method. The detected arrival times of TIEs are consistent with their predicted values. This new measurement method has some key technical advantages. The tool measurements do not require a liquid-filled inner casing for acoustic coupling. The arrangement of the transducers in the tool enables fully compensated measurements. Furthermore, the vertical resolution of detected tubing eccentricity is governed by the vertical sampling rate of the tool rather than the physical transmitter-receiver spacing. The long length of received waveforms can provide the time window to exposure the trainlet of TIE for revealing the types of filled-in materials of annulus and acoustic impedance contrast of filled materials and well barriers.

The intervention of the irregular settlements has evolved upon the successes and errors of policies that normally do not consider an integral vision of the urban system. Among the numerous integral interventions in Latin American, Venezuela has developed three scopes of intervention proposed by Consejo Nacional de la Vivienda (CONAVI) by creating Pograms for the improvement and the rehabilitation of these areas. Currently two scenes are superimposed: one national that proposes Centers of Endogenous Development to decentralize and disperse; and the other Regional, made up of the Urban Land Committees directed towards regularize the ownership of the land; and the Local Councils of Public Planning who guarantee the main participation. These scenarios look toward eradicating the precariousness tie to a phenomenon that, instead of being looked at as a threat, bust be looked at as an opportunity to incorporate them into a suitable urban plan

Abstract Well intervention requires reliable pressure data and real-time monitoring of multiple annuli simultaneously to confirm appropriate pressure response of actuated valves and components. The current industry standard uses traditional monitoring methods which involve reading digital crystal gauges and manually recording the values at increments for interpretation. This paper presents a case study where the operator reduced the probability of human error, and improved assurance and safety by utilizing a digital solution during an intervention campaign. During operations, personnel used digital pressure testing software to monitor annulus pressures while performing acid stimulation pressure pumping operations and verified test outcomes during additional intervention operations. To conduct these tests, pressure transducers were tied into the locations where a digital gauge would normally be located, and cables connected to a data acquisition unit for interpretation by operations personnel. The software monitored and analyzed the data in real time to validate the operator's criteria, and all real-time data was able to be viewed both on-site and remotely. When tests completed, reports were auto generated and stored for ease of access. The process described above resulted in a safer testing environment by utilizing transducer cables and a data acquisition unit, allowing personnel to remain a safe distance away, as opposed to the traditional method of taking manual readings near pressurized areas. High resolution digital data was constantly monitored, instead of taking readings in increments, allowing for the improved visualization of annulus or tubing pressures in real time and improving the efficiency of operations. This on-site and remote real-time visualization allowed for better communication and interpretation of appropriate pressure response when shifting sleeves, equalizing pressures, or performing coiled tubing cleanouts. When all intervention work was completed, a comprehensive end of well report was generated to provide a record of all operations and reduce communication errors during handover or subsequent operations throughout the well lifecycle. This digital software solution and equipment tie in used for intervention operations provides a safer environment, more accountability, improved decision making, and more efficient communication in handover. The ability for real time monitoring of pressure response improves participation and collaboration between personnel both on-site and in remote locations. Comprehensive documentation provides an auditable history of events which can be used to improve future operations and promote best practices.

The way in which entrepreneurship is taught has a high impact on students' entrepreneurial capacity. In this sense, there have been several studies that analyze the impact that different forms of teaching have on the development of entrepreneurship skills. Different research indicates, in a general sense, that the more education concerning entrepreneurship, the more entrepreneurship skills are developed. Other studies report that students that were involved in a more practical and reflective educational model, reported greater intentions of undertaking an entrepreneurial experience. However, none of these studies use the development of “case studies” as a means of developing entrepreneurship skills. In this sense, this research project proposes an experimental study, with a control group of 30 students and three intervention groups of 90 students (from fifth semester forward), in order to measure the impact that the development of real entrepreneurs case studies (either of success or failure) had on students. All of this, in order to improve the attitudinal competencies of: (i) problem analysis mentality before proposing early solutions, ii) mentality of testing low-cost solutions before investing, and (iii) data-driven decision-making mentality rather than intuition. Results suggest that there were noticeable improvements in the soft skills learned by the students. There was a percentage change in the results of the intervention groups in comparison with the control group (even when the control group also registered improvements). Further studies may test this intervention in different settings or measure other soft/hard skills using the same intervention. Abstract Deep critically sour fields played a key role in PDO's production portfolio for past two decades particularly in the south of Oman. Cost-effective solutions to develop these fields was certainly needed to ensure that oil produced from these fields is economically attractive. Completion design optimization was successfully implemented leading to significant savings in life-time cost of these well. Completion optimization journey was performed in stages based on the challenges encountered throughout the life of the field. Due to risky nature of the reservoirs in these fields, wells were completed with tubing and permanent packers to ensure well integrity requirements are fulfilled and all operation risks are controlled. These permanent packers induced a big challenge during well intervention & workover as they have low success chances when it comes to milling. This base completion design was optimized including eliminating the use of permanent packers to achieve savings of about 5% in well cost at least 80% in well interventions. At early stage of the field life, combining drilling caprock and reservoir section and casing them with the same liner was considered as a good saving in well drilling cost leading to total cost reduction of about 5% of total cost. However, this optimization increased cost of well intervention dramatically in these wells as the tubing and packer need to be retrieved to gain full access. In addition, challenges in milling the permanent packer have increased intervention cost even further. This first optimization done was to introduce a liner across the reservoir section of the same size as the tubing (Mono-bore completion). This optimization made the completion more Rig-less friendly and reduced well intervention cost by at least 80%. The second optimization done was to replace the permanent packers with Tie-back Polished Bore Receptacle (PBR). This has eliminated the requirement for packer milling without jeopardizing the sealing functionality of the permanent packer. This design was even optimized further to eliminate the possibility of failure of the permanent packer or PBR by introducing cemented completion. Cemented completion has also reduced total cost of drilling & completing the wells by 5% while maintaining the advantage to perform well activities and minimum cost. These improvements in well completion strategy have reduced life-time cost of the well dramatically without jeopardizing well basic functional requirements and well integrity requirement. As a result of this work, production from these field became more economically attractive.

Our research addresses the Sierra Leonean government’s need to optimise and validate an effective model for in-service TPD. As noted, a distributed TPD model is favoured (building on the World Bank’s Free Education project and the Sierra Leone Education Attendance Monitoring System (SLEAMS), using a ‘one-tablet-per-school’ (OTPS) model). Evidence indicates that this low-tech approach to TPD can improve learning outcomes in emergencies (⇡Haßler, et al., 2019). In contrast, COVID-19 interventions that focus on ‘remote learning for children’ are catastrophically failing (⇡Asadullah, 2020; ⇡Asanov, et al., 2020; ⇡BRAC, 2020; ⇡Le Nestour, et al., 2020; ⇡Uwezo Kenya, 2020). Our research provides crucial and timely evidence to shore up current government plans for a scalable, effective TPD intervention. Our research identifies whether this TPD model addresses the needs of rural / disadvantaged teachers. Our study focuses on a sample of teachers and students from schools in rural / semi-rural areas to understand whether and how school-based TPD initiatives promote student learning (as an alternative to traditional centralised and cascade models). Such traditional models are severely disrupted by COVID-19, highlighting our study’s relevance to education continuity in emergencies.

There is significant interest in sustainable food production practices in Canada and worldwide due to the challenges caused by the Russia-Ukraine war, land degradation, and climate change. Sustainable food production is a food system that provides affordable, nutritious food while preserving and restoring natural resources and generating robust ecosystem services such as carbon sequestration, water filtration, and retention. This report explores multiple routes to foster improved social, ecological, and economic impacts associated with alternative practices promoting sustainable food production. It identifies core problems that prevent agricultural systems and their food chains from implementing (more) sustainable practices. The report mobilizes a Theory of Change (TOC) to outline possible interventions and metrics to implement (community-based) interventions to promote shared principles of sustainable production and create communities of practice. The TOC was developed in consultation with a set of actors in the food chain (including farmers, financial institutions, municipal governments, food processors, NGOs and industry associations) during a nine-month research intervention in Canada (2023), complemented by a literature review. Thanks to this co-creation process, the proposed interventions and metrics to measure and track improvements at the farm and societal levels presented in this report are outcomes-based and bottom-up. This enables agricultural communities and actors in the food chain to pursue alternative routes to improve outcomes. The report also discusses incentives to pursue sustainable food production, either explicit (e.g. monetary payments, contractual clauses) or implicit (e.g. social norms, cultural values, network-based engagement of food chain actors). Lastly, it outlines a potential research design to test the suggested interventions, metrics, and incentives in a Randomized Control Trial (RCT).

Overview This is the third quarterly progress report for an ongoing living systematic review on cannabis and other plant-based treatments for chronic pain. The first progress report was published in January 2021 and the second in March 2021. The draft systematic review was available for public comment from May 19 through June 15, 2021, on the Agency for Healthcare Research and Quality (AHRQ) Effective Health Care website. The systematic review synthesizes evidence on the benefits and harms of plant-based compounds (PBCs), such as cannabinoids and kratom, used to treat chronic pain, addressing concerns about severe adverse effects, abuse, misuse, dependence, and addiction. The purpose of this progress report is to describe the cumulative literature identified thus far. This report will be periodically updated with new studies as they are published and identified, culminating in an annual systematic review that provides a synthesis of the accumulated evidence. Main Points In patients with chronic (mainly neuropathic) pain with short-term treatment (4 weeks to <6 months): • Studies of cannabis-related products were grouped based on their tetrahydrocannabinol (THC) to cannabidiol (CBD) ratio using the following categories: high THC to CBD, comparable THC to CBD, and low THC to CBD. • Comparable THC to CBD ratio oral spray is probably associated with small improvements in pain severity and may be associated with small improvements in function. There was no effect in pain interference or serious adverse events. There may be a large increased risk of dizziness and sedation, and a moderate increased risk of nausea. • Synthetic THC (high THC to CBD) may be associated with moderate improvement in pain severity and increased risk of sedation, and large increased risk of nausea. Synthetic THC is probably associated with a large increased risk of dizziness. • Extracted whole-plant high THC to CBD ratio products may be associated with large increases in risk of withdrawal due to adverse events and dizziness. • Evidence on whole-plant cannabis, low THC to CBD ratio products (topical CBD), other cannabinoids (cannabidivarin), and comparisons with other active interventions was insufficient to draw conclusions. • Other key adverse event outcomes (psychosis, cannabis use disorder, cognitive deficits) and outcomes on the impact on opioid use were not reported. • No evidence on other plant-based compounds, such as kratom, met criteria for this review.

Prior to the Taliban takeover in August 2021, the World Food Programme (WFP) Afghanistan had been working to see how its strategic outcomes in the 2018–23 Country Strategic Plan (CSP) were aligned with viable peace and development efforts nationally, and to investigate plausible pathways by which its interventions could support broader goals to contribute to the humanitarian–development–peace (triple) nexus. With the Taliban regaining control, these pathways have become less clear and difficult for WFP to make firm strategies. This Theory of Change (TOC) document looks at each of the relevant strategic outcomes from the 2018 CSP in turn (temporarily rolled over as a result of the uncertainty) and proposes theoretical ways in which the interventions can be best aligned with medium- to longer-term goals. Whilst these can no longer be simplistically labelled ‘development’ and ‘peace’, preserving national systems and institutions remains vitally important to ensure minimum humanitarian suffering. The aim of developing these TOCs is to help best align current programming and develop future programming options, as well as better generating evidence on what works. The TOCs are also intended to be a bridge to the next CSP, generating evidence to inform future strategic outcomes and helping the organisation think through realistic contributions to national systems.

Barn och unga upp till 18 år som lever i familjer där barnets förälder eller någon annan vuxen som barnet varaktigt bor tillsammans med har en psykisk störning eller funktionsnedsättning, har en allvarlig fysisk sjukdom, har missbruksproblem, utsätter barnet eller en närstående för våld eller oväntat avlider omfattas av begreppet barn som anhöriga. Barnen har en förhöjd risk att utveckla egna psykosociala eller psykiska problem. Därför är det viktigt att hälso- och sjukvården beaktar deras behov av information, råd och stöd. Sedan drygt tio år sedan finns en bestämmelse i Hälso- och sjukvårdslagen om detta (HSL 2017:30, 5 kap. 7§). Primärvården har unika förutsättningar för att identifiera och stödja dessa barn, men forskningen inom området är hittills mycket begränsad, både nationellt och internationellt. Studiens övergripande syfte var att följa upp en tidigare kartläggning av kliniskt verksamma medarbetares kunskapsläge och arbete med barn som anhöriga samt att ta fram ett forskningsbaserat kunskapsunderlag för Region Värmlands fortsatta arbete med barn som anhöriga i primärvården där medarbetares erfarenheter tas tillvara. 133 kliniskt verksamma medarbetare inom primärvården i Region Värmland deltog i en digital enkät. Endast två tredjedelar av deltagarna angav att de känner till bestämmelsen och ännu färre kände till Region Värmlands rutin VIDA RUT-26224 som reglerar arbetet med bestämmelsen på regional nivå. Efter att ha tagit del av bestämmelsens innehåll ansåg en majoritet av deltagarna att alla patientgrupper förekommer på vårdcentralen. Men endast hälften uppgav att de själva har träffat någon av dessa patienter under den senaste månaden. Av dessa hade en majoritet även frågat om patienten hade barn i hemmet, och cirka en tredjedel av dem som hade frågat uppgav att insatser för att möta barnets behov hade planerats eller genomförts. Medarbetarna gav en rad förslag för att utveckla arbetet med barn som anhöriga i primärvården. Kompetensutveckling, implementering av lokala rutiner på vårdcentralen och samverkan med andra parter nämndes. Jämförelsen med en liknande studie från år 2013 visade att tendenser till en positiv förändring över tid kan noteras. Det gäller främst att en högre andel av medarbetarna tycks ha frågat om familjesituationen inklusive att identifiera om hemmavarande barn finns, dels att en högre andel därefter tycks ha tagit upp frågan om barnets situation och behov av stöd. Med tanke på att det är tio år mellan undersökningarna och att lagstiftningen på området under den tiden har skärpts hade tydligare skillnader kunnat förväntas. Sammantaget kvarstår istället många av de brister som tidigare identifierats. Arbetet för barn som anhöriga behöver intensifieras och anpassas till primärvårdens specifika förutsättningar. Barnens behov av information, råd och stöd beaktas även här. Primärvården skulle framför allt kunna fokusera på att identifiera barn som anhöriga, att öppna upp för samtal med den vuxne patienten om barnens situation och att utveckla fungerande samarbeten för de barn som behöver fortsatt stöd. Även korta interventioner som ges på vårdcentralen och informationsmaterial med lokal anknytning behöver utvecklas och användas i större utsträckning. För detta behövs kompetenshöjande insatser. Det är även viktigt att följa upp och utvärdera arbetet, till exempel genom systematiska utvärderingar eller samverkansprojekt.

This evaluation from the Office of Evaluation and Oversight (OVE) documents and draws lessons from what the Inter-American Development Bank (IDB) has done over the past decade with a view to making a strong future contribution to increasing climate-change resilience and reducing GHG emissions in Latin America and the Caribbean (LAC). The evaluation will be organized into three sections and nine chapters. The first section will address the big picture, including climate change challenges and what the IDB is doing to support countries in overcoming these challenges. The second section will address the adaptation and mitigation interventions and actions that the IDB has supported through loans and TC operations in key sectors and areas, taking into consideration mainstreaming and any associated co-benefits, when possible. The final section will assess project performance and results to gauge whether or not projects are economically and environmentally sustainable and whether the promoted techniques are robust to evolving climatic conditions.
