Background There is encouraging evidence that interdisciplinary teams of Health and Social Care Professionals (HSCPs) can enhance patient care in the Emergency Department (ED), especially for older adults with complex needs. However, no formal process evaluations of implementations of ED-based HSCP interventions are available. The study aimed to evaluate the development and delivery of a HSCP team intervention for older adults in the ED of a large Irish teaching hospital. Methods Using the Medical Research Council (MRC) Framework for process evaluations, we investigated implementation and delivery, mechanisms of impact, and contextual influences on implementation by analysing the HSCP team’s activity notes and participant recruitment logs, and by carrying out six interviews and four focus groups with 26 participants (HSCP team members, ED doctors and nurses, hospital staff). Qualitative insights were analysed thematically. Results The implementation process had three phases (pre-implementation, piloting, and delivery), with the first two described as pivotal to optimise care procedures and build positive stakeholders’ involvement. The team’s motivation and proactive communication were key to promote acceptability and integration in the ED (Theme 1); also, their specialised skills and interdisciplinary approach enhanced patient and staff’s ED experience (Theme 2). The investment and collaboration of multiple stakeholders were described as essential contextual enablers of implementation (Theme 4). Delivering the intervention within a randomised controlled trial fostered credibility but caused frustration among patients and staff (Theme 3). Discussion This process evaluation is the first to provide in-depth and practical insights on the complexities of developing and delivering an ED-based HSCP team intervention for older adults. Our findings highlight the importance of establishing a team of HSCPs with a strong interdisciplinary ethos to ensure buy-in and integration in the ED processes. Also, actively involving relevant stakeholders is key to facilitate implementation. Trial registration ClinicalTrials.gov, NCT03739515; registered on 12th November 2018.

Abstract Bacterial contamination of haematopoietic stem cell products (HSCP) during collection and processing is a potential risk and has been described as cause of serious morbidity and mortality. The rate of contamination is reported in the range of 0 to 4.5% in peripheral blood progenitor cell (PBPC) apheresis to as high as 26% in bone marrow (BM) harvests. Systematic microbiological testing is an important component of the HSCP quality assessment and identification of the bacteria involved helps in the management of early infective complications. Here we report the rate of contaminated HSCP collected in a single transplant centre, our policy of the management of this complication and the relevant clinical outcomes. This is a retrospective study of prospectively collected results of microbiological analyses of 246 collections performed from January 2015 till December 2015. Stem cells were collected by BM harvesting under general anaesthesia (N=47) and by PBPC apheresis (N=199) using Optia separator system. BM harvest rather then PBPC was performed on donor request or with donor consent where BM was clinically preferable over PBPC transplant. (eg. red cell disorders). Samples of HSCP for bacterial cultures were taken pre processing, post processing, including post filtration and at defined intervals during complex procedures. Cryoprotectant is also tested to check that no contamination has occurred during manufacture of the freeze mix. Samples were cultured in BACTEC™ Lytic/10 Anaerobic/F culture vials (pre-reduced enriched Soybean-Casein Digest broth with CO2) and in anaerobic blood cultures; BACTEC Peds Plus™/F culture vials (enriched Soybean-Casein Digest broth with CO2) under aerobic conditions, both for 14 days. Organisms were specifically identified in all positive HSCP. Sixteen bacterial contaminations of HSCP were recorded (6.09% of total collections). The most frequent product contaminated was BM (N=15, 31.9%); only one PBPC apheresis product was positive (0.5%). The following bacteria were isolated: propionibacterium (N=7), coagulase negative staphylococcus (N=4), micrococcus (N=2), staphylococcus capitis (N=1), Staphylococcus epidermidis (N=2). Seven HSCP were not infused: one patient died before undergoing to transplant, two products have been collected as stem cell rescue in case of graft failure from allogeneic donors, four collections were stored for future use. Nine patients received contaminated HSCPs. After being contemporaneously alerted to the contamination, the clinical team performed daily blood cultures (BC). From these 9 patients only two minor clinical events were recorded; One, a non-neutropaenic fever on day +2 (S. Capitis cultured from HSCP, BC negative) and the other a positive BC (micrococcus cultured from both HSCP and BC) in a patient without fever or signs of infection. Both were treated with IV vancomycin on microbiology advice. These data showed a rate of bacterial contamination of HSCP comparable with reports from other groups. Moreover, at our centre no major clinical events were recorded after the infusion of contaminated HSCP. In this small study, the most frequent source of contaminated HSCP was BM and the most frequently isolated pathogens were skin commensals. No consensus exists on the requirement for antibiotics after the infusion of contaminated HSCP. Our experience supports a strategy of symptomatic management based on fevers or positive blood cultures with targeted antibiotics based on in vitro sensitivities rather than pre-emptive empirical treatment of all patients. Abstract Hematopoietic stem cell/progenitor (HSCP) transplantation (HSCT) is routinely used for the treatment of cancer and inborn hematopoietic defects. The bone marrow (BM) microenvironment (ME) is a major regulator of hematopoietic function and fate. Clinical data supports osteoblastic regeneration after HSCT despite the inability of BM mesenchymal stem cells (BM-MSC) to engraft. Therefore, understanding the hematopoietic-dependent mechanisms controlling ME mesenchymal regeneration is expected to provide molecular targets for intervention in the context of HSCT. Hematopoietic connexin-43 (H-Cx43) mediates HSCP survival and efficient blood formation by scavenging damaging excess reactive oxygen species (ROS) through transfer to BM mesenchymal stromal cells (BM-MSC) after chemotherapy, preventing lethal hematopoietic failure (Taniguchi-Ishikwawa E et al., PNAS 2012), while the expression of Cx43 on BM-MSC regulates CXCL12 secretion and HSCP homeostasis (Schajnovitz A et al., Nat. Immunol., 2011). Since Cx43 is expressed in mitochondria, we hypothesized that H-Cx43 mediated ROS transfer upon stress depends on hematopoietic mitochondria transfer and uptake by the BM-MSC. We created chimeric mice by transplanting Vav1-CreTg/-, Cox8 mitochondrial localization signal-Dendra2Tg/- wild-type (mDendra2/WT) or Cx43fl/fl(mDendra2/Cx43Δ/Δ) HSCP to lethally irradiated, congenic WT mice and assessed the recovery of stromal cell regeneration via transfer of mitochondria to BM-MSC. H-Cx43Δ/Δchimeric mice have delayed lympho-hematopoietic recovery after irradiation or chemotherapy which can be reversed by restoration of hematopoietic Cx43 expression. H-Cx43Δ/Δchimeric mice exhibit decreased (~60-80%) and delayed colony-forming-unit-fibroblast (CFU-F) and osteoblast (CFU-Ob) regeneration and hematopoietic recovery. The delayed hematopoietic response in H-Cx43Δ/Δchimeras associated with ~40% reduction in mitochondrial transfer from HSCP to Lin-/CD45-/PDGFRα+/Sca1- BM stromal cells (MSC/P). Reverse transplantation experiments indicate that stromal Cx43 is dispensable for mitochondrial transfer from BM stroma to HSCP. Impaired mitochondrial uptake in H-Cx43Δ/Δchimeras associated with ~30-40% decreased mitochondrial ROS (mROS), membrane potential (MMP) and proliferation (assessed by in vivo BrdU uptake) of recipient MSC/P, suggesting that the transferred mitochondria reprogram the recipient mesenchymal progenitor metabolism. Defects of mitotransfer from H-Cx43Δ/ΔHSCP to BM MSC/P and in recipient BM MSC/P mitochondrial activity were recapitulated in in vitro co-cultures. Interestingly, intracellular [ATP] is upregulated (~2 fold) in MSC/P from chimeric H-Cx43Δ/ΔBM that received donor-derived mitochondria, as compared to donor mitochondria containing MSC/P from WTchimeras. Hemichannel opening causes loss of ATP, we therefore speculated that ATP released from MSC/P upon irradiation and transplantation is uptaken by HSPC, activating mitochondrial transfer as part of BM regeneration. Forced glycolysis-dependent restoration of [ATP] in MSC/P but not in HSCP enhances transfer of mitochondria from HSCP to MSC/P, suggesting that BM stromal [ATP] is an irradiation-responsive positive regulator of mitochondria transfer. Hemichannel-derived exogenous ATP suppresses AMPK activation, which regulates cellular metabolic homeostasis by modulating mitochondrial ROS, mitochondria dynamics and the fate of mitochondria. We found that MSC/P recipient of H-Cx43Δ/Δ mitochondria have increased AMPK activity as assessed by increased phosphorylation of AMPK and its downstream effectors ULK1 and ACC (~2-fold) when compared with MSC/P recipient of H-WT mitochondria, whereas MSC/P containing no donor-derived mitochondria from either chimeric mice are insensitive to the effect of Cx43 deficiency. In vivo administration of the AMPK inhibitor BML-275 dramatically increased the mitochondria transfer from HSCP to MSC/P in WT and H-Cx43Δ/Δ chimeras, and completely restores the negative effect of H-Cx43 deficiency on BM mesenchymal and hematopoietic regeneration. Our data indicate that hematopoietic mitochondrial Cx43 is required to control both mitochondrial transfer and BM ME energetic balance and regeneration after myeloablative irradiation. Background Over 65s are frequent attenders to the Emergency Department (ED) and more than half are admitted for overnight stays. Early assessment and intervention by a dedicated ED-based Health and Social Care Professionals (HSCP) team reduces ED length of stay and the risk of hospital admissions among older adults while improving patient health-related quality-of-life and satisfaction with care. This study aims to evaluate whether augmenting the treatment as usual for older adults admitted to ED is cost-effective. Methods and findings Cost-effectiveness analysis (CEA), conducted alongside the OPTI-MEND randomised controlled trial of 353 patients aged ≥65 with lower urgency complaints compared the effectiveness of early assessment and intervention by a dedicated HSCP team in the ED to treatment as usual (TAU). An economic analysis estimated the average cost per older adults randomised to the HSCP team, and compared to TAU, how contact with HSCP team changed health care use, and associated total costs, and estimated the effect of HSCP on Quality-Adjusted Life Years (QALYs). Within the OPTI-MEND trial, the average cost of a contact with the HSCP team during ED attendance is estimated to be €801 per patient. Compared to TAU, the incremental QALY of intervention is 0.053 (95% CI: 0.023 to 0.0826, p<0.0001). Accounting for cost savings because of contact with HSCP team, the average incremental saving in the total cost, compared to TAU, is -€6,128 (95% CI: -€9,217 to -€3,038, p<0.0001). Given the incremental health gains and significant cost savings, bootstrapped cost CEA suggests that dedicated HSCP care dominates over TAU for low urgency older adults attending the ED. Conclusions A dedicated HSCP team in the ED significantly improves overall health for lower acuity older adults and, by reducing inpatient length of stay, results in staggering cost savings. This economic evaluation conducted on the OPTI-MEND trial provides convincing evidence that HSCP should be adopted as part of treatment as usual in Irish EDs. Abstract Background Introducing Health and Social Care Professional (HSCP) teams to the emergency department (ED) has increasingly demonstrated benefits for ED patient and process outcomes. However, there is a dearth of research exploring the views of key ED stakeholders on the role of HSCP teams in care delivery the ED. This qualitative study investigated the perspectives of a wide range of ED stakeholders about HSCPs teams working in the ED. Methods A total of 65 participants including older adults who had recently attended the ED and their carers/relatives, ED doctors and nurses, HSCPs and pre-hospital staff participated in four World Café style focus groups and individual interviews across two Irish hospital sites. Written and audio-recorded data were transcribed and thematically analysed. Results Overall, participants expressed positive views on HSCPs working in teams in the ED, with benefits for patients, staff members and the hospital (Theme 1). Having an ED-based HSCP team was described as promoting effective and timely decision-making and a more integrated approach to patient care, particularly for frail older adults with complex needs (Theme 2). Barriers and enablers for effective implementation were identified at multiple levels (Theme 3) including the ED physical environment, (e.g., space and equipment), operational factors (e.g., working hours), and relations (e.g., patient-staff or staff-staff communication); factors at system level included availability of community resources and financial pressures. Conclusion Our study indicates overall acceptability of HSCPs working in teams in the ED and positive views on their contribution to enhance the quality care of older adults. However, a number of operational and relational factors need to be considered to ensure feasibility and effectiveness. This information is crucial to inform implementation.

IntroductionHealth and social care professionals (HSCPs) have increasingly contributed to enhance the care of patients in emergency departments (EDs), particularly for older adults who are frequent ED attendees with significant adverse outcomes. For the first time, the effectiveness of a HSCP team intervention for older adults in the ED has been tested in a large randomised controlled trial (Clinicaltrials.gov,NCT03739515), providing an opportunity to explore the implementation process for this type of intervention. This protocol describes a process evaluation that will to investigate the implementation, delivery and impact of an HSCP team intervention in the ED.Methods and analysisUsing the Medical Research Council Framework for process evaluations, we will employ a mixed-methods approach to provide a description of the process of implementation and delivery of the HSCP intervention in the ED, evaluate its fidelity, dose and reach and explore the perceptions of key staff members in relations to the mechanisms and contexts of impact at the levels of individuals, physical environment, operations, communication and the broader hospital and healthcare system.Ethics and disseminationEthical approval for this study was received from the HSE Mid-Western Regional Hospital Research Ethics Committee (Ref: 103/18). All participants will be invited to read and sign a written consent form prior to participation. The results of this review will be disseminated through publication in a peer-review journal and presented at relevant conferences.

Abstract. The Henry's law solubility (HS) and liquid-phase loss rate constants (kl) of the tropospheric trace gas constituents peroxyacetic nitric anhydride (PAN; CH3C(O)O2NO2, commonly known as peroxyacetyl nitrate) and peroxypropionic nitric anhydride (PPN; C2H5C(O)O2NO2, also known as peroxypropionyl nitrate) in deionized (DI) water and of PPN in n-octanol were measured using a flow bubble apparatus at temperatures between 5.0 and 25.0 ∘C. For PAN in DI water, the observed values for HS,aq are consistent with the literature, whereas the solubility of PPN in DI water is slightly lower than literature values, ranging from HScp(PPN)aq = (1.49 ± 0.05) M atm−1 at 25.0 ∘C to HScp(PPN)aq = (7.01 ± 0.25) M atm−1 at 5.0 ∘C (stated uncertainties are at the 1σ level). The data are best described by ln⁡(HScp(PAN)aq/[Matm-1]) = -(17.8±0.3) + (5620±85)/T and ln⁡(HScp(PPN)aq/[Matm-1]) = -(19.5±1.7) + (5955±480)/T, where T is in kelvin. For n-octanol, the PPN solubility ranges from HScp(PPN)oct = (88±5)Matm-1 at 25.0 ∘C to HScpoct = (204±16)Matm-1 at 5.0 ∘C and is best described by ln⁡(HScp(PPN)oct/[Matm-1]) = -(6.92±0.75) + (3390±320)/T. n-Octanol–water partition coefficients (KOW) for PPN were determined for the first time, ranging from 59 ± 4 at 25.0 ∘C to 29 ± 3 at 5.0 ∘C. Observed loss rate constants in DI water are consistent with recent literature and larger than the thermal dissociation rates for both PAN and PPN, consistent with a hydrolysis mechanism, whereas kl values in n-octanol are significantly smaller than gas-phase dissociation rate constants, likely owing to a “cage effect” in the organic liquid. The results imply that uptake of either PAN or PPN on cloud water and organic aerosol is negligible but that uptake of PPN may constitute an overlooked source of peroxy radicals in organic aerosol.

AbstractObjective:To disrupt cycles of health inequity, traceable to dietary inequities in the earliest stages of life, public health interventions should target improving nutritional wellbeing in preconception/pregnancy environments. This requires a deep engagement with pregnant/postpartum people (PPP) and their communities (including their health and social care providers, HSCP). We sought to understand the factors that influence diet during pregnancy from the perspectives of PPP and HSCP, and to outline intervention priorities.Design:We carried out thematic network analyses of transcripts from ten focus group discussions (FGD) and one stakeholder engagement meeting with PPP and HSCP in a Canadian city. Identified themes were developed into conceptual maps, highlighting local priorities for pregnancy nutrition and intervention development.Setting:FGD and the stakeholder meeting were run in predominantly lower socioeconomic position (SEP) neighbourhoods in the sociodemographically diverse city of Hamilton, Canada.Participants:All local, comprising twenty-two lower SEP PPP and forty-three HSCP.Results:Salient themes were resilience, resources, relationships and the embodied experience of pregnancy. Both PPP and HSCP underscored that socioeconomic-political forces operating at multiple levels largely determined the availability of individual and relational resources constraining diet during pregnancy. Intervention proposals focused on cultivating individual and community resilience to improve early-life nutritional environments. Participants called for better-integrated services, greater income supports and strengthened support programmes.Conclusions:Hamilton stakeholders foregrounded social determinants of inequity as main factors influencing pregnancy diet. They further indicated a need to develop interventions that build resilience and redistribute resources at multiple levels, from the household to the state.

Objective:Due to a lack of validated assessment instruments, this study aimed to pilot test the newly developed Health and Social Care Professionals’ Knowledge & Attitudes Towards Later Life Sexuality (HSCP-KALLS) instrument. The HSCP-KALLS instrument is designed to assess health and social care professionals’ knowledge (46 items) and attitudes (40 items) towards later life sexuality including components related to dementia, sex worker services and Lesbian, Gay, Bisexual, Transgender, Intersex or Queer/Questioning (LGBTIQ+).Methods:A group of health and social care professionals (n = 22) and Healthcare-related educator (n = 2) were invited to complete the HSCP-KALLS instrument. Feedback on items phrasing and the experience of completing the instrument was sought.Results:Written feedback regarding either phrase of items or use of the instrument was not specifically addressed by participants. A high level of internal consistency was revealed for both the knowledge and attitude items (α = 0.84 & 0.88, respectively). A decent level of knowledge (M=39.75, SD=4.90) and positive attitudes (M=161.04, SD=13.50) towards later life sexuality were demonstrated by participants. Participants had greater knowledge on items related to ageing, intimacy, and sexuality (95%), with a lower level of knowledge on items related to sexuality diversity (e.g., LGBTIQ+). Providing more trainings about later life sexuality was frequently addressed in the knowledge written feedback. Participants generally demonstrated positive attitudes towards later life sexuality. However, a high proportion of ambivalent responses were noted on some attitude items (e.g., A9 & A18) that participants indicated in written feedback that their responses would depend on circumstances.Conclusion:Preliminary reliability and feasibility of using the HSCP-KALLS instrument has been encouraging, with further testing in large samples now, required to robustly establish psychometric properties. Supporting later life sexuality is essential and the use of HSCP-KALLS instrument can inform and identify professional development needs of health and social care professionals to improve care provision for older people by supporting their expression of sexuality in healthcare settings.

Анотація:
The purpose of this study was to investigate the direct and indirect effects of information and communication technology (ICT) on a hybrid lean-agile supply chain performance (HSCP) in terms of leanness, agility and leagility of automotive supply chains (SCs) in Egypt. A conceptual framework was proposed and tested using questionnaires, which were filled through face-to-face interviews. A mixed-methods approach was used to achieve a comprehensive understanding of research relationships. The population was 101 international organizations in Egyptian automotive industry within its three sub-sectors (manufacturers of auto-feeding and automotive industries and distributors). Structural equation modelling was used to examine the proposed relationships. Results indicated that ICT positively/significantly affects both SC integration (SCI) and HSCP. SCI positively/significantly affects SC information sharing (SCIS). SCIS affects HSCP in terms of mass customization and postponement positively/significantly. The three sub-sectors are using a new blended SC strategy that hybridizes attributes of each approach according to the SC node’s position. This article adds value to the current debate on the applicability of implementing a blended lean-agile SC strategy by integrating two main lines of management research (leagility approach and hybridized lean-agile manufacturing systems) into one new hybrid approach. It is the first study that investigated empirically the direct/indirect ICT–HSCP relationships of multiple nodes in a heterogeneous market. It maps the hierarchical auto SCs of an emerging market. It provides automotive SC players with practical insights on using ICT for better SCI and SCIS and choosing the appropriate aspects of leanness, agility and leagility based on the organization’s position across its SC.

Resumo: A utilização eficiente da banda passante em redes de alta velocidade e grandes atrasos, denominadas redes com produto banda-atraso elevado (PBA), tornou-se um grande desafio. Isto ocorre devido aos ajustes do protocolo Transmission Control Protocol (TCP). O High Speed TCP (HSTCP), uma variante do TCP para redes com PBA elevado, emprega ajustes mais agressivos permitindo, assim, que a utilização da banda seja escalável. As políticas de Gerenciamento Ativo de Filas ou Active Queue Management (AQM), monitoram o nível de ocupação das filas nos roteadores e notificam o congestionamento incipiente aos emissores TCP através do descarte/marcação de pacotes. O sistema de controle de congestionamento apresenta natureza de retroalimentação, na qual a taxa de transmissão dos nós fontes é ajustada em função do nível de ocupação da fila. Os controladores AQM determinam a probabilidade de descarte/marcação para maximizar a vazão e minimizar perdas, garantindo, assim, a estabilidade do tamanho da fila independentemente das variações das condições da rede. Neste trabalho, define-se a política de gerenciamento ativo de filas HSTCP-H2 para redes com PBA elevado que utilizam o protocolo HSTCP. Para a derivação de HSTCP­H2: são utilizadas técnicas de Teoria de Controle Ótimo. A principal característica desta política é considerar o atraso do sistema o que permite melhor utilização dos recursos disponíveis. A estabilidade e os objetivos de desempenho do sistema são expressos e solu­cionados através de Desigualdades Matriciais Lineares, permitindo que os parâmetros do controlador possam ser calculados através da solução de um problema convexo simples. Diferentes controladores foram derivados considerando-se diferentes objetivos de de­sempenho, os quais consideram as características de redes com produto banda-atraso elevado. Através de simulações, os desempenhos dos controladores derivados são avalia­dos e a eficácia do controlador que apresentou o melhor desempenho foi comparado com o desempenho da política de AQM RED. São considerados cenários com enlace gargalo único e com múltiplos gargalos.
Abstract: The efficient utilization of bandwidth in high speed and large delay networks, called high bandwidth-delay product networks (BDP), has become a major challenge. This is due to adjustments of the Transmission Control Protocol (TCP). The High Speed TCP HSTCP): a TCP variant to high BDP networks, employs more aggressive adjustments, allowing scalable bandwidth utilization. The Active Queue Management (AQM) policies monitor the queue length in the routers and notify incipient congestion to TCP source by marking or dropping packets. The congestion control system presents intrinsic feedback nature, where the transmission rates of the sources are adjusted according to the level of congestion inferred by the queue occupancy. The AQM controllers determine the dropping marking probability values to maximize throughput and minimize losses, giving guarantees to stabilize the queue length independent of network conditions. In this work, it is defined HSTCP-H2, an active queue management policy to high BDP networks, which adopt the HSTCP as their transport protocol. Optimal control theory is used to conceive HSTCP-H2. The novelty of the proposed approach lies in consider the delay of the system which allows better use of available resources. Furthermore, in the proposed approach, stability and performance objectives are completely expressed as Linear Matrix Inequalities (LMIs), thus requiring the solution of a single convex problem for the computation of the controller parameters. Different controllers are derived considering different design goals, which take into ac­count the characteristics of the high bandwidth-delay product networks. The performance produced by different optimal controllers was investigated. The efficacy of the control­ler with the best performance was then compared to the performance of RED policy. The simulation experiments were carried out using topologies with single and multiple bottleneck.

Cette thèse présente les résultats obtenus en utilisant deux stratégies différentes de recherche de particules lourdes, chargées et à long temps de vie dans l'expérience CMS auprès du LHC, utilisant des données issues de collisions proton-proton à une énergie de √s= 13 TeV, collectées entre 2017 et 2018 et qui correspondent à une luminosité intégrée de 101 fb-1. Cette recherche est divisée en deux stratégies, la première se concentre sur l'utilisation de signature s exotiques de dépôts de haute énergie dans le trajectographe en silicium, et la seconde utilise le temps de vie comme signature additionelle. Les bruits de fond associés sont estimés dans les données par deux méthodes différentes, et des procédures de validation sont effectuées dans des regions de contrôle. Les résultats de la première stratégie ont été soumis à une interpétation statistique puis rendus publics, tandis que les résultats de la seconde stratégie ne le sont pas encore
This thesis presents the results obtained using two different strategies in the search for heavy stable charged particles in the CMS experiment at the LHC, using proton-proton collision data at an energy of √s= 13 TeV corresponding to an integrated luminosity of 101 fb-1 collected between 2017 and 2018. This research is divided into two strategies: the first focuses on using exotic signatures of high-energy deposits in the silicon tracker, and the second uses lifetime as an additional signature. Backgrounds are estimated in the data using two different methods, and validation procedures are performed in control regions. The results of the first strategy have been subjected to statistical interpretation and made public, while the results of the second strategy are not yet public

Os tumores de mama são a principal causa de morte em cães e isso vem despertando maior interesse no desenvolvimento de estudos relacionados a este distúrbio. A proximidade com os seres humanos, tanto na convivência quanto aos hábitos, podem influenciar o aparecimento das neoplasias. A semelhança dos tumores mamários caninos com os tumores de mama da mulher leva a um interesse no estudo da patologia comparada, estimulando o uso de modelos animais. Apesar dos muitos estudos, pouco se conhece sobre o prognóstico e as causas dos tumores mamários caninos, observando-se um esforço crescente na tentativa de acrescentar aos fatores prognósticos clássicos novos parâmetros, de natureza molecular, que auxiliem a decisão clínica, à semelhança do verificado em Medicina Humana, estando entre eles os marcadores moleculares, como a proteína P53 e as proteínas de estresse. A expressão de proteína P53 e das proteínas de estresse tem sido observada em muitas neoplasias, incluindo o câncer de mama. Nesse sentido, o objetivo do presente estudo foi investigar a imunomarcação de HSP 27, HSP 72, HSP 90 e proteína P53 em tecido mamário normal e neoplásico de cadelas e estabelecer uma relação entre a expressão destas proteínas e o grau histológico das neoplasias. Foi realizada análise estatística e o nível de significância (α) adotado foi de 5%. Dentre os tumores malignos, os carcinomas simples foram o tipo histológico predominante. Para a proteína P53, não houve diferença significativa em sua expressão entre os grupos de tumores malignos avaliados, ocorrendo o mesmo para as HSPs 27, 72 e 90. A sensibilidade do teste de imuno-histoquímica para a proteína P53, nesta amostra, foi de 67,5%, a especificidade foi de 100%, o valor preditivo positivo foi de 100%, o valor preditivo negativo foi de 25% e acurácia do teste foi de 92%. Breast tumors are the leading cause of death in dogs and this has aroused great interest in developing studies related to this disease. The proximity with humans, much as in living habits, may influence the onset of tumors. The similarity of canine mammary tumors with breast tumors of women take an interest in the study of comparative pathology, stimulating the use of animal models. Despite many studies, little is known about the prognosis and causes of canine mammary tumors, observing a growing effort in trying to add to the classic prognostic factors new parameters of molecular nature, that help the clinical decision, like that seen in Human Medicine, and among them the molecular markers such as P53 and stress proteins. The expression of P53 protein and the stress proteins has been observed in many cancers, including breast cancer. Accordingly, the purpose of this study was to investigate the immunostaining of HSP 27, HSP 72, HSP 90 and P53 protein in normal and neoplastic breast tissue of female dogs and establish a relationship between the expression of these proteins and the histological grade of tumors. Statistical analysis was performed and significance level (α) was 5%. Among the malignant tumors, simple carcinomas were the predominant histologic type. Protein P53, presented no significant difference in expression between the evaluated groups of malignant tumors, the same occurring for HSPs 27, 72 and 90. The test sensitivity of immunohistochemistry for protein P53 in this sample was 67,5%, specificity was 100%, positive predictive value was 100%, negative predictive value was 25% and accuracy of test was 92%. This thesis deals with theoretical analysis of First Hop Redundancy Protocols. It describes Hot Standby Router Protocol, Virtual Router Redundancy Protocol and Gateway Load Balancing Protocol. It also shows examples of configuration of each protocol on Cisco devices with supported version of the Cisco IOS. Furthermore, this thesis includes design of two of these protocols, Hot Standby Router Protocol and Gateway Load Balancing Protocol, and their implementation in discrete event simulator OMNeT++ and Automated Network Simulation and Analysis library. Finally, the thesis presents results of testing of the implementations in comparison with actual Cisco devices.

AbstractHematopoietic stem cell transplantation (HSCT) was not available in the United Arab Emirates (UAE) until July 2020, when the first adult HSCT was done in Abu Dhabi by the Abu Dhabi Stem Cell Center. HSCT is increasingly indicated for various non-malignant and malignant diseases. In the UAE, adults and pediatric patients that could benefit from HSCT were commonly travelling abroad for such procedures, with the associated cost, logistics of travel, delay in delivering HSCT, and social challenges of spending an extended period of time outside the UAE. In this chapter, we will highlight the progress of HSCT services in the UAE and also share our experiences and lessons learned during the establishment of a comprehensive pediatric and adult HSCT service in the UAE that is designed to cater for UAE citizens and residents as well as those in neighboring countries facing similar HSCT accessibility challenges.

"Research has shown the frequent use of complementary and alternative medicine (CAM), its potentialities and risks. Aim: To characterize adults’ knowledge and practices of CAM in order to identify the need to develop lifelong learning programs. After obtaining the necessary authorizations, between 1/10-5/12/2021, 115 adults living in Portugal, mostly women with a higher education degree, working full-time, with a monthly income perceived as medium, answered a Google Forms questionnaire on health (self-)care practices (HsCP). Most considered their health as good (44.3%), being satisfied with it (53%). The majority: took some care of their health (60.5%; 28.1% a lot); had no physical (56.1%) or psychological (78.3%) health problem diagnosed by a conventional health care professional. Regarding the extent to which the participants had knowledge of non-conventional health (self-)care practices (NCHsCP), most (39.5%) knew little about it (10.5% had no knowledge). The majority: had already looked for information about NCHsCP (55.8%), was interested in having (more) knowledge about it (77.2%), and had someone close to them who used and/or was using some non-conventional practice - NCP (57%). Only 14.2% had done training in some NCP and 5.3% considered it impossible to reconcile conventional and NCHsCP. The majority (65.8%) stated that knowledge about NCP is important to them as (future) health professionals (not applicable to 26.3%). As for the last time participants performed any HsCP, 30.2% considered it a NCP (19.8% were not sure). In terms of HsCP, 66.7% reported using conventional and NCP. Among those who have ever used any NCP: 47.1% did it with a professional and 34.3% by themselves and with a professional; 45.8% use it rarely; 93.2% had never done it against the recommendation of a (conventional) health professional. Among those regularly (at least once a year) accompanied by one or more conventional health care professionals, 54% had never talked about NCP. Regarding negative experiences with NCP, 89.8% never had any. The minor children of 13.9% had done some NCP (not applicable to 60.2%). Among the health care professionals, 80.2% had no course during their training mentioning NCP; in their professional experience, 64.2% had knowledge of situations in which conventional and NCP were used simultaneously; in the context of their professional activity: 51.9% had recommended, and 40.7% had referred someone to NCP. Even though these preliminary results are not worrying, they suggest adults living in Portugal would greatly benefit from lifelong learning programs focusing on the potentialities and risks of CAM."

Objective: The objective of this study was to standardize the Ficoll gradient technique for the circulating hematopoietic stem cell (HSC) isolation for the assembly of the peripheral blood mononuclear cell (PBMC) biorepository of breast cancer (BC) patients attended in the Clinical Oncology Service of Instituto Mário Penna. Methods: The study protocol was approved by the Ethics Committee of Instituto Mário Penna (CAEE 82703418.8.0000.5121). In recommended protocols, 15 mL of blood was used. At first, we adapted this volume due to the limited amounts of samples for research available. Blood was collected in a 9-mL sodium heparin tube. The experiments were performed in 50-mL conical tubes, but with reduced blood volume, and no PBMC ring was formed. It was necessary to change to 15 mL conical tubes. Finally, the remaining red blood cells were lysed with ammonium chloride. However, with the reduced volume, this solution lysed the PBMC too. Then, we decided to remove this step from the protocol. Results: We obtained 8.06×106 cells/mm3 with 80% viability. Data were confirmed by a Neubauer camera and an automatic cell counter. The HSCs were labeled with antibodies against CD34 and CD133 by flow cytometry. Conclusion: The characterization of HSCs is important to link tumor-associated HSCs with malignant and immunosuppressive phenotypes. Studies are in progress with this standardization, and they will permit us to perform the HSC characterization of BC patients with a better knowledge of tumor microenvironment.

Abstract This paper reports on the development of novel hybrid soft cable-driven parallel robots (HSCP). As the growth and refinement of the soft robotic field progresses, new methods of soft robotic actuation and control will continue to be developed. While most of the existing soft robotic systems have a serial structure, soft parallel robots have received a lot of attention recently. This is due to the wide range of applications of these systems in different industries such as the medical and the food industry. To the best of our knowledge, most of the existing soft parallel robots have active flexible legs. However, in this work for the first time, we have proposed a novel soft parallel robot with only passive flexible legs. In our hybrid design the stiffness of the robot is maintained through the passive soft legs and the actuation is based on a cable-driven mechanism. This will simplify the design and modeling of the robot. As an example of our design approach, a 6 degrees of freedom (DOF) hybrid soft parallel robot is designed which resembles the rigid Stewart mechanism. The mechanics of the robot are unique, being controlled only through cable actuation through six servo motors positioned around the base of the robot. The supports which were 3D printed with deformable Ninja Flex filament supply a nonlinear force to the robot, combined with the cable control enabling the robot to be able to execute basic movement within the workspace as well as being safer for human-machine interaction due to the soft support system. This paper also compares the kinematics and controls of this new robotic system to a typical rigid steward robot.

Haematopoietic stem cell (HSC) transplantation has been widely used to treat patients that have undergone high-dose chemotherapy or radiotherapy for haematological or non-haematological malignancies, and is currently investigated for the treatment of several other pathologic conditions. Nevertheless, present and expected clinical applications are hindered by the shortage of cells available for transplantation. Hence, many researchers have attempted to achieve an in vitro expansion of HSCs, using different experimental set-ups and approaches, which range from traditional static monolayer cultures to three-dimensional (3D) dynamic systems. Specifically, several bioreactor systems have been proposed, including perfusion chambers, stirred, rotating, hollow fiber, and packed bed reactors [1]. Taken together, literature studies suggest that a dynamic 3D culture system may provide superior expansion of HSCs.

Current hematopoietic stem cell (HSC) separation methods utilize immunomagnetic techniques where antibodies are conjugated to paramagnetic or iron-dextran particles so that the cells can be precipitated using a magnetic field. Antibodies are usually targeted against CD34 antigen, a known HSC marker that is lost as the cell matures into a terminally differentiated blood cell. While this method is able to produce stem cell purities greater than 80% in most cases, cell yield is usually low (<50%) and there is evidence to suggest that not all HSCs express CD34 and hence, cannot be selected using these methods. Our long-term aim is to move away from immunologic isolation methods and instead focus on general HSC behavior in the presence of specific proteins. Selectins are a group of adhesion molecules that have been shown to selectively retard HSC rolling in comparison to more mature blood cells so that, under the right conditions, HSC isolation from whole blood should be possible. Since this functional method is independent of specific HSC surface marker, it would enable isolation of all HSC sub-populations as well as improve the overall yield. We have used two geometries for HSC isolation using selectins. The first assembly was a commercially available parallel plate flow chamber with rectangular cross-section. This proved to be inefficient for cell separation due to the emergence of dead zones within the chamber and excessive cell accumulation along the tubing and fittings that comprised the inlets and outlets. We have evaluated this chamber and two redesigns using finite elements software. Ultimately we decided on a simpler approach — to replace the flow chamber with a capillary tube, which reduced unwanted cell accumulation due to gravity or dead zones while increasing the effective area for separation, and hence allowing for better HSC isolation.

Haematopoietic stem cells (HSCs) circulate in the bloodstream under flow conditions, but the mechanical mechanisms of governing their physiological trafficking in mammals are still not yet clearly understood. The mobilization of HSCs and their progenitors into the circulation represents the basis for modern bone marrow transplantation procedures [1]. Recently, Mendez-Ferrer et al. performed experimental investigations on mice [2]. They demonstrated that the circulating HSCs and their progenitors exhibit robust circadian oscillations, and the circadian fluctuations could also be significantly altered when the HSCs were subjected to different time of lighting. These results indicated that the photic cues could affect the trafficking of HSCs in healthy animals. This implies that the light is the stimulus of HSCs release. When the HSCs are exposed to light, the HSCs release would markedly increase; when the HSCs are in darkness, the HSCs release keeps low efficiency. In this study, we numerically simulate this phenomenon to study the mechanical mechanism of circadian fluctuations regulated HSCs release under the influence of periodic lighting time.

It also offers practical advice on how to address many of the challenges involved in implementing HSP, how to evaluate its impact, and how to complement it with other policing approaches to help sustain decreases in crime. Included throughout the publication are case studies from the region that help illustrate how HSP is applied. The guide also answers questions that are often asked about HSP, such as issues associated with the displacement of crime.

Leaks attributed to hydrogen-stress cracking (HSC) initiating in regions of mild mechanical damage have been reported in cathodically protected pipe lines constructed from high-strength, microalloyed, controlled-rolled steels. The hydrogen is believed to be present in service from the cathodic potential applied. Laboratory studies were initiated to determine the factors that contributed to those unexpected failures. Strain aging at ambient temperatures as a result of deformation introduced during the mechanical damage, was found to be a significant factor. Smooth-bar specimens that were strained and then aged failed by HSC within one week, whereas specimens that were not strain aged did not fail by HSC. Result: The findings of this research indicate a potential sequence of events which may lead to hydrogen-related failures in regions of mild mechanical damage: (1) Following the damage, ambient-temperature strain aging which promotes sensitivity to HSC takes place in the mechanically damaged region, in a surface layer of the pipe wall which has been subjected to a critical level of strain. The time period for this step would be on the order of several years. (2) Electrochemical conditions which promote hydrogen charging develop at the pipe surface from the cathodic current applied (or possibly corrosion). (3) Local stresses in the mechanically damaged region are elevated above the threshold stress for HSC by the moderate stress concentration provided by the mechanical damage. For the X70 pipe studied, the stress elevation should be at least 20 percent above the nominal hoop stress. (4) An HSC crack initiates and grows in the strain-aged surface layer. (5) The crack propagates further by HSC, through the non-strain-aged portion of the wall, as a result of the high stress concentration at the crack tip. (6) When the crack grows to a critical depth, it propagates rapidly through the wall by overload and causes a leak.

Fifty six diverse spring wheat cultivars were evaluated for genetic variation and heritability for thermotolerance in terms of cell-membrane stability (CMS) and triphenyl tetrazolium chloride (TTC) reduction. The most divergent cultivars for thermotolerance (Danbata-tolerant and Nacozari-susceptible) were crossed to develop an F8 random onbred line (RIL) population. This population was evaluated for co-segragation in CMS, yield under heat stress and HSP accumulation. Further studies of thermotolerance in relations to HSP and the expression of heterosis for growth under heat stress were performed with F1 hybrids of wheat and their parental cultivars. CMS in 95 RILs ranged from 76.5% to 22.4% with 71.5% and 31.3% in Danbata and Nacozari, respectively. The population segregated with a normal distribution across the full range of the parental values. Yield and biomass under non-stress conditions during the normal winter season at Bet Dagan dit not differ between the two parental cultivar, but the range of segregation for these traits in 138 RILs was very high and distinctly transgressive with a CV of 35.3% and 42.4% among lines for biomass and yield, respectively. Mean biomass and yield of the population was reduced about twofold when grown under the hot summer conditions (irrigated) at Bet Dagan. Segregation for biomass and yield was decreased relative to the normal winter conditions with CV of 20.2% and 23.3% among lines for biomass and yield, respectively. However, contrary to non-stress conditions, the parental cultivars differed about twofold in biomass and yield under heat stress and the population segregated with normal distribution across the full range of this difference. CMS was highly and positively correlated across 79 RILs with biomass (r=0.62**) and yield (r=0.58**) under heat stress. No such correlation was obtained under the normal winter conditions. All RILs expressed a set of HSPs under heat shock (37oC for 2 h). No variation was detected among RILs in high molecular weight HSP isoforms and they were similar to the patterns of the parental cultivars. There was a surprisingly low variability in low molecular weight HSP isoforms. Only one low molecular weight and Nacozari-specific HSP isoform (belonging to HSP 16.9 family) appeared to segregate among all RILs, but it was not quantitatively correlated with any parameter of plant production under heat stress or with CMS in this population. It is concluded that this Danbata/Nacozari F8 RIL population co-segregated well for thermotolerance and yield under heat stress and that CMS could predict the relative productivity of lines under chronic heat stress. Regretfully this population did not express meaningful variability for HSP accumulation under heat shock and therefore no role could be seen for HSP in the heat tolerance of this population. In the study of seven F1 hybrids and their parent cultivars it was found that heterosis (superiority of the F1 over the best parent) for CMs was generally lower than that for growth under heat stress. Hybrids varied in the rate of heterosis for growth at normal (15o/25o) and at high (25o/35o) temperatures. In certain hybrids heterosis for growth significantly increased at high temperature as compared with normal temperature, suggesting temperature-dependent heterosis. Generally, under normal temperature, only limited qualitative variation was detected in the patterns of protein synthesis in four wheat hybrids and their parents. However, a singular protein (C47/5.88) was specifically expressed only in the most heterotic hybrid at normal temperature but not in its parent cultivars. Parental cultivars were significantly different in the sets of synthesized HSP at 37o. No qualitative changes in the patterns of protein expression under heat stress were correlated with heterosis. However, a quantitative increase in certain low molecular weight HSP (mainly H14/5.5 and H14.5.6, belonging to the HSP16.9 family) was positively associated with greater heterosis for growth at high temperature. None of these proteins were correlated with CMS across hybrids. These results support the concept of temperature-dependent heterosis for growth and a possible role for HSP 16.9 family in this respect. Finally, when all experiments are viewed together, it is encouraging to find that genetic variation in wheat yield under chronic heat stress is associated with and well predicted by CMS as an assay of thermotolerance. On the other hand the results for HSP are elusive. While very low genetic variation was expressed for HSP in the RIL population, a unique low molecular weight HSP (of the HSP 16.9 family) could be associated with temperature dependant heterosis for growth.

The Houston Ship Channel (HSC) is one of the busiest deep-draft navigation channels in the United States and must be able to accommodate increasing vessel sizes. The US Army Engineer District, Galveston (SWG), requested the Engineer Research and Development Center, Coastal and Hydraulics Laboratory, perform hydrodynamic and sediment modeling of proposed modifications in Galveston and Trinity Bays and along the HSC. The modeling results are necessary to provide data for hydrodynamic, salinity, and sediment transport analysis. SWG provided three project alternatives that include closing Rollover Pass, Bay Aquatic Beneficial Use System cells, Bird Islands, and HSC modifications. These alternatives and a Base (existing condition) will be simulated for present (2029) and future (2079) conditions. The results of these alternatives/conditions as compared to the Base are presented in this report. The model shows that the mean salinity varies by 2–3 ppt due to the HSC channel modifications and by approximately 5 ppt in the area of East Bay due to the closure of Rollover Pass. The tidal prism increases by 2.5% to 5% in the alternatives. The tidal amplitudes change by less than 0.01 m. The residual velocity vectors vary in and around areas where project modifications are made.
