Dr. Martin-Plank will provide her experiences in advocating for older adults in Pennsylvania and nationally through professional organizations, meeting with coalition partners to promote access to care for vulnerable older adults in rural areas by promoting full practice authority for nurse practitioners, and advocating for full home health authority for nurse practitioners. Dr. Martin-Plank will share how she is active in advocacy and policy at the local, state and federal levels, and how to build a presence and relationship with legislators on The Hill and State Capitol. Dr. Martin-Plank is a family, gerontological, and mental health nurse practitioner, practicing in Pennsylvania, New Jersey, and Arizona.

Access to oral health care is essential for promoting and maintaining overall health and well-being, yet oral health disparities exist among vulnerable and underserved populations. While nurses make up the largest portion of the health care work force, educational preparation to address oral health needs of elders and persons with disabilities is limited across nursing curricula. This descriptive study reports on the interdisciplinary development, implementation, and testing of an oral health module that was included and infused into a graduate nursing curriculum in a three-phase plan. Phase 1 includes evaluation of a lecture presented to eight gerontological nurse practitioner (GNP) students. Phase 2 includes evaluation of GNP students’ perceptions of learning, skills, and confidence following a one-time 8-hour practicum infused into 80 required practicum hours. The evaluation data show promise in preparing nurse practitioner students to assess and address preventive oral health needs of persons aging with disabilities such that further infusion and inclusion in a course for nurse practitioners across five specialties will implemented and tested in Phase 3.

Abstract Background NHR are the frailest group of older people and require a gerontologically attuned approach to combat multiple challenges presented to the practitioner. The in-reach ANPc liaison service aims to confront such challenges, by providing comprehensive gerontological input to all nursing home residents admitted to hospital under all specialities medical, geriatric, and surgical. Methods This service commenced in September of 2018, working 9-5 Monday to Friday with limited leave cross-cover. All nursing home residents are reviewed by a Gerontology Advanced Nurse Practitioner candidate. Each patient received comprehensive geriatric assessment (CGA) with recommendations for care. All had follow up 2 weeks after discharge in telephone review clinic. Results 118 nursing home residents were admitted for acute care in the study period; 96/118 (82%) were reviewed by the ANPc. 16/118 (14%) were discharged prior to review and 6/118 (5%) died within 24 hours of admission. All other patients were reviewed within 72 hours. All assessed residents had >1 recommendation for intervention to enhance care following CGA: Interventions included 31% (30/96) undiagnosed delirium identified and management advice given. 21% (20/96) had recurrent falls work up and advice. 27% (26/96) had recommendations and changes to admission medications. 37% (36/96) referrals to other HSCP therapy disciplines for complete holistic care. 11% (11/ 96) had advanced care planning regarding future illnesses. 13% (13/96) had palliative care advice and referral to community palliative care. Follow up telephone review clinics have further resulted in reduced readmission rates through liaison with NH staff post-discharge. Conclusion The high complexity of this cohort of patients requires a timely, comprehensive gerontological approach in order to provide holistic care. They require a clearly defined approach to enhance care and minimise the need for unnecessary hospitalisations.

Purpose: This study examined nurse practitioners’ (NPs) practice patterns for heart failure (HF) management, specifically regarding the use of angiotensin-converting enzyme inhibitors (ACEIs), beta antagonists, diuretic monotherapy, and echocardiograms for older adults residing in long-term care facilities. Methodology: A Web-based survey was developed to identify NP practice patterns for HF management in long-term care settings. The Checklist for Reporting Results of Internet E-Surveys and expert opinion guided the survey design. The survey was distributed via e-mail to Gerontological Advanced Practice Nurses Association members. Results: Most NPs used ACEIs for ongoing therapy, initiated appropriate therapy for symptomatic patients, continued beta-antagonist therapy in patients with HF, used echocardiograms in the diagnosis and evaluation of HF, and prescribed monotherapy diuretics in accordance with HF care standards. Implications for Practice: Based on these findings, NP practice patterns for management of HF in the long-term care setting are in alignment with national standard of care regardless of certification or level of physician oversight. NPs need to be cognizant of the cardiovascular risk factors for HF and adjust treatment to add an ACEI or beta antagonist to diuretic monotherapy when the clinical diagnosis of HF is suspected or established.

Access to healthcare is needed and wanted by people of all ages and especially by those of the older population. The number of people in the 65 years of age and older population is rapidly growing with their needs expected to have a significant impact on the existing healthcare system and healthcare providers. The impact will be critical given the severe shortage of healthcare providers, especially of nurses and the rate of services being more often provided in non-hospital settings. The objectives of the study were to discover the plans of graduating nursing students as they choose their first place of employment, if they have future plans to pursue a nursing advance practice degree, and if they are very happy with their decision to become a nurse. Data for the study were obtained from a questionnaire presented to senior graduating nursing students. The findings were: (a) Most students prefer a hospital setting. (b) Younger students are three times as likely to seek out the hospital, and 1/3 of the students seek out the hospital setting because they were encouraged to become a nurse. (c) About 70% of the students want to work with their friends while 1/3 will seek the hospital worksite, as it is perceived as being the strongest resource in paying back loans. (d) Nearly 87% are considering the nursing advance practice role, and 52% have interest in the nurse practitioner role. The majority of students identified as very happy with their decision to become a nurse. This study provided insight for schools of nursing as they make curriculum decisions and to businesses as they learn of the preferences and plans of the new emerging nurses.

The role of an aged care nurse practitioner (ACNP) is well recognised internationally however, in Australia, the implementation of this advanced role is still in its infancy with few gerontological nursing experts registered as nurse practitioners (NP). This single Victorian facility 2002 study was the first to consider the role of an ACNP in Australia and the first to describe the clinical and social benefits or otherwise of ACNP interventions in an Australian context. NP Studies in the Australian Capital Territory (ACT) from 1999 – 2002 investigated the role in other nursing domains followed by an ACNP study conducted over 2004-2005. A subsequent national ACNP study in 2005 provided complementary results to this first Australian ACNP study which created the framework for these subsequent projects. This study aimed to establish: clinical or other outcomes that a gerontological nurse practitioner (ACNPs) could achieve for older persons in an Australian residential aged care facility, factors that impacted upon the introduction of such a role, a definition of the role and to establish whether such a role would benefit older persons in Australia. Various methods were used to determine the numerous outcomes which were to be studied in this project. A quantitative analysis of the functional and social status of residents who participated in the project, pre and post the ANCP interventions was undertaken. A quantitative analysis of the satisfaction of residents or their representatives pre and post the interventions is also presented. A qualitative analysis via focus groups, of the views of staff, residents and health professionals involved in the project was undertaken. Hospital rates pre and post the interventions and case studies are presented as additional information only. The team involved in this Victorian Government Department of Human Services funded aged care nurse practitioner project at Greensborough Private Nursing Home included this researcher, the ACNP candidate, the Director of Nursing (DON) and Deputy DON. The team jointly managed the complex legal framework, to ensure interventions were implemented safely for all residents with the support of the residents’ general practitioners and other health professionals working for the nursing home. Statistically significant improvements in the resident’s functional and social status were demonstrated for residents treated by the ACNP. Additionally, the resident and representative satisfaction survey revealed a higher overall level of satisfaction with the home following the project’s completion. The results demonstrated that the ACNPs’ interventions were of high quality, led to improvements in resident health outcomes, improved residents’ quality of life and reduced hospitalisation rates. This was achieved by intervening in a timely manner when residents required relief of their physical and psychological symptoms through targeted interventions and one-on-one specialist medical nursing attention. In summary, this study identified interventions an ACNP could undertake and therefore the role they could play in an Australian residential aged care facility, given the national legislation governing all aspects of an aged care facility. This study demonstrated that the role was feasible and achieved positive resident outcomes despite the factors that impeded its introduction.
