Reverend Father Professor Amechi Nicholas Akwanya is one of the towering scholars of literature in Nigeria and elsewhere in the world. For decades, and still counting, Fr. Prof. Akwanya has worked arduously, professing literature by way of teaching, researching, and writing in the Department of English and Literary Studies of the University of Nigeria, Nsukka. To his credit, therefore, this genius of a literature scholar has singularly authored over 70 articles, six critically engaging books, a novel, and three volumes of poetry. His PhD thesis, Structuring and Meaning in the Nigerian Novel, which he completed in 1989, is a staggering 734-page document. Professor Akwanya has also taught many literature courses, namely: European Continental Literature, Studies in Drama, Modern Literary Theory, African Poetry, History of Theatre: Aeschylus to Shakespeare, European Theatre since Ibsen, English Literature Survey: the Beginnings, Semantics, History of the English Language, History of Criticism, Modern Discourse Analysis, Greek and Roman Literatures, Linguistics and the Teaching of Literature, Major Strands in Literary Criticism, Issues in Comparative Literature, Discourse Theory, English Poetry, English Drama, Modern British Literature, Comparative Studies in Poetry, Comparative Studies in Drama, Studies in African Drama, and Philosophy of Literature. A Fellow of Nigerian Academy of Letters, Akwanya’s open access works have been read over 109,478 times around the world. In this wide-ranging interview, he speaks to Andrew Bula, a young lecturer from Baze University, Abuja, shedding light on a variety of issues around which his life revolves.

The research is presented in the form close to a fundamentally annotated bibliography demonstrating how European literary experience was advanced in the first quarter of the 19th c. in Russia at the time when contemporary Russian literature was being shaped. Six parts are devoted successively to French, German, English, Italian, Spanish, and classical literatures. The major aspects of research are outlined in an extensive foreword (E. Dmitrieva, M. Koreneva). Highlights include: Comparative analysis of the international contacts of Russian literature; a new interest in the novel, the genre that manifested a new literary taste; publishing and the audience in Russia compared to other European cultures; the birth of literary criticism on the margins of rhetoric; the evolution of a literary taste where gallomania was being substituted by anglo- and germanophilia; the change in the forms of contacts from imitation to stylization in accordance with the formula suggested by Konstantin Batyushkov ‘The stranger’s treasure is mine.’

This paper deals with corruption debates as a political factor in the First Portuguese Republic. Criticism of corruption is a hitherto hardly considered aspect for understanding the instability of the Republic. Criticism of corruption as a critique of parliamentarism existed in almost all European countries in the first third of the 20th century. This essay offers a systematic examination of corruption debates in Portugal and aims to emphasise the international commonalities. Similar to the rest of Europe, these criticisms contributed to the bad image and destabilisation of the parliamentary system. The essay mainly uses political newspapers and pamphlets as sources. After an assessment of the relevant research literature and a very short section on anticorruption in the late monarchy, we will concentrate on three groups of critics: monarchical Catholic voices, radical republican commentaries, and anarchist left-wing contributions. The aim is to reconstruct patterns of argumentation of the aforementioned political directions that were typical throughout the republican period. We will also take up the alleged connection between cultural backwardness and corruption in the Portuguese self-description. In the last section, we will shortly focus on the (almost non-existent) defence strategies of the ruling Republicans.

The paper aims is to present and evaluate the Review the History of Serbian Literature A. N. Pipin's as a classical history of Serbian literature that became part of the national culture. The development of the history of literature among Serbs, as an independent discipline and its modest beginnings, can be found in the first decades of the 19th century, in the time of Dositej and Vuk. In its beginnings, the history of literature was a "story" about the literary past of a nation and at its core was - criticism. This main idea as an axiom is a signpost that leads from the history of literature, which has long performed the function of criticism, to the genesis of literary criticism as the youngest branch of literary science and the way it formulated and exercised its functions in conditions when literary history was in a certain measures and history of the people. The Serbs received the first History of Serbian Literature (1865) from the pen of Pavel Jozef Šafarik (1795–1861), a Protestant and German student who served in Novi Sad. The next history of Serbian literature was also written by a foreigner, the Russian Alexander Nikolaevich Pipina (1833–1904). His Review the History of Serbian Literature (1865) has not been fully translated into Serbian. When marking questions from the new Serbian literature, Pipin's approach leads to a synthesis of ideas about cultural and political and national development. Slavery replaced the idea of revival "among Orthodox Serbs who fled to Austria". From that perspective, he views the development of national literature as an important part of culture and identity. Pipin also deals with the issue of national identity and the awakening of the national consciousness of the Slavs in his extensive study "Panslavism in the Past and Present" (1878), in which "the Serbian national question is incorporated into the general critique of Russian official policy and Slavophile orientation in the Balkans during Eastern Europe crisis". In this paper, we value his competence, cultural mission, the gift of the comparator, without which there is no great literary historian, and his practical contribution to classifying Serbian literature and culture in the European context.

The ‘dramatic rise’ of Titus Andronicus ‘among critics and directors’ in the 1990s was primarily linked to ‘the growth of feminist Shakespeare criticism’.1 Its recent stage popularity, however, lies in its responses to the new geopolitical realities and the shifting physical and mental borders of the European Union. This article examines four European productions of Titus Andronicus in their engagement with political change, migration, rising nationalism and xenophobia, which, I argue, refocus attention onto this play’s position in the Shakespeare canon.

The dissertation titled “Central European Literary ‘Escapes’ from History (Vladimir Bartol, Witold Gombrowicz, Sándor Márai)” is an outcome of the interdisciplinary research project conducted at the crossroads of literary studies, history, and anthropology. Inspired by contemporary methodology exploring the category of experience, the author aims to provide new insights into the writers’ narrative self-identifications, diaristic practices, and their common background of a Central European community of historical fate. This comparative study attempts to replace geopolitical conceptualisations of Central Europe in terms of regional identity with a geopoetic map of this area focusing on self-identifications of writers and their sensual experiences of this space. Whereas geopolitical Central Europe has been a laboratory of ideologies nourished by modernist dialectical tradition, the geopoetic Central European condition can be articulated in life writing and particularly in a diary. The dissertation’s overarching theme regards the three writers’ attitudes to the History of the twentieth century, its accelerated pace as well as the changeable spatial coordinates of Central Europe and temporary places of stay in exile. The author argues that to the post-war historical circumstances enclosed within the ideologised dialectical thought and thus reverberating with the absurd overtone, Bartol, Gombrowicz and Márai responded with a hermeneutic laboratory of self, explored in diary and exile. They embarked on an exilic odyssey and diaristicwriting which allowed them not only to maintain a certain distance from History (with a specific exception for Bartol) but also to reconfigure their experience of time and in the end also self-identifications. The main sources are analysed using the anthropological approach which regards the diaries in terms of practice, existentially crucial for their authors in the process of redefining their selves in the face of rapidly shifting spatiotemporal contexts. The diaristic reconfiguration of time puts the kairotic dimension of temporality to the foreground which consequently undermines for a while its chronological, impersonal side.

This study evaluated moral dilemmas and Lawrence Kohlberg's (1975) stages of moral reasoning of protagonists in a sample of children's translated European war novels. The sample, consisting of fourteen books, was defined as all children's European war novels published between 1950 and 1984. The content analysis first determined the moral dilemmas in each of the novels by identifying those story situations where two or more moral issues were in conflict. A second procedure evaluated the protagonists' response to the dilemma, making possible the assignment of a Kohlberg level and stage of moral judgement. The collected data were evaluated following two steps. First, the Issues, Levels and Stages were quantitatively analyzed for representation, number, and frequency. In addition, the Issues and Stages were evaluated for those moral issues most frequently paired with each moral stage. The second procedure examined the relationship between the data and selected variables: Era (Era I 1952-1962, Era II 1963-1973, Era III 1974-1984), Sex of author and Sex of protagonist. The findings revealed that moral dilemmas in the European war novels were most often related to issues of Affiliation Roles, Morality and Mores, and Truth. No dilemma situations arose out of conflicts of the moral issues of Sex or Law. All other Kohlberg moral issues were represented at least once in the sample. The predominant stage of moral reasoning in the sample was Stage 2 (serving one's own needs), closely followed by Stage 1 (blind obedience to authority) and Stage 3 (playing the good role). Significantly, these stages reflect the general moral reasoning capabilities of the intended reading audience, ages 8 12 years. While higher stages were represented, they accounted for substantially fewer protagonist resolutions to dilemma situations. With regard to sex of the protagonist, the findings revealed that male characters more frequently resolved their dilemma situations with sophisticated levels of moral reasoning than did female, a factor which may be linked to the type of story. The relationships between moral development and Era appeared to reflect the transition from traditional realism to modern realism in children's fiction. Books written in Era I (1952-1962) contained few or no moral dilemmas. As with other traditional realistic fiction, child protagonists in that era were insulated from the world around them and thus remained relatively unaffected by World War II. Books written in Era II (1963-1973) and Era III (1974-1984), however, showed evidence of portraying children in the modern mode of realism. Unlike Era I, protagonists of these periods encountered large numbers of moral dilemmas and were highly involved in and affected by the war.

The femme fatale is a marker for the past is evident in the film noir work of German exile directors. These directors created a femme fatale character similar to Weimar examples of the sexual woman icon, using Weimar cultural constructions as a template for their work in Hollywood. The femme fatale figure in film noir is specifically an enigma or duplicitous mystery, a woman with a gun who threatens the male protagonist. She represents a piece of the male character's past often seen through the structure of the voice-over and flashback. These narrative devices enable the male protagonist to rework this jaded past vis-a-vis his relationship to the femme fatale and his fatal attraction to her. The film noir femme fatale is linked to German exile directors' desire to review a past that has been lost and cannot be recuperated. In the case of the Weimar femme fatale, she is a sign for the trauma of World War I and the ensuing political/social crises of the Weimar republic. The femme fatale in her castrating capacity is a marker for historical upheaval and male subjectivity in flux. She is ultimately the scapegoat for male questions of self and a split subjectivity brought on by historical events such as war and the experience of exile. Her various guises include the criminal woman, the technological entity, and the double.

This thesis consists of two parts: a creative work and two critical essays on translingual writing. The creative component, Then the Cicadas Sang, is a novel set in 1940s Malta. It is a story about love and aspiration. As a teenage girl, Mari vouches she will do anything to leave the tiny island she lives on. Foreigners – the British who governed the island at the time – and books give her a glimpse of the world beyond her shores. But she craves for more, unaware of what she risks losing by chasing her dreams. The novel deals with how books shape our imagination, how the languages we speak give us access to different systems of conceptualizing the world and how we navigate the spaces in between. It does this through the protagonist, Mari, and the people who help shape who she is, in particular Mrs Applegate, a British evacuee who sought shelter in Gozo in the midst of the Blitz. But as much as it is a story of a girl turning into a woman, the novel is also the story of an island. It sits between Junot Díaz’s The Brief Wondrous Life of Oscar Wao and Elena Ferrante’s The Neapolitan Series. The extract submitted is Book One in a series of two. The critical essays explore the poetics of multilingual writing. They analyse the linguistic, political and cultural stratifications in multilingual writing, with a focus on the perception and reception of Maltese literature written in English. I ask if a multilingual writer’s role can be akin to that of a cultural translator. They investigate whether, unlike the monolingual writer, a writer’s multilingual background gives him or her access to different systems of conceptualizing the surrounding environment and how this informs the creative process. This study informs my own process of writing Then the Cicadas Sang, with a particular regard to self-translation and how one language can carry another on the page. In this case the languages I am working with are English, Maltese and Italian.

To read a work by a different author living in a different era and setting, people tend to imagine works of literature to be something they are not. To avoid this fateful tendency, I hope to elucidate what it means to read a work of literature and interpret it: love it to the point of wanting to foremost discuss its excellence of being a piece of art. Rather than this being a defense, I would rather call it a musing, an examination on two texts that I adore: Beowulf and the Anglo-Saxon Exodus

This thesis is the first book-length study of Johannes Schiltberger's Reisebuch, the German-language account of the Bavarian crusader's approximately 30-year captivity and enforced travels in the Middle East under the Ottoman sultan Bayezid I (r. 1389-1403), the Turko-Mongolian warlord Timūr (r. 1370-1405) and his successors. It investigates the Reisebuch's representation of the Orient. My reexamination of the manuscript and print transmission from c. 1450 to 1600 has shown that there were various versions of the Reisebuch in circulation, and I base my reading of the work on this basic multiplicity and variety. This variance is pertinent for a literary interpretation of the Reisebuch because the manifest narrative and material transformations go hand in hand with differing thematic foci, narratological and representational strategies, so that one can confidently contend each version of the Reisebuch negotiates a distinct relationship with the eastern 'other'. I argue that the transmission diverges into three strands as (1) an abridged chronicle that emphasises the complexity of political and military relations in the East; (2) a proto-ethnographic account with a focus on religious diversity; and (3) six paratextual reconceptualisations of the Reisebuch as Turcica in early print. In short, my study engages with the transmission and transformation of material, on the one hand, and with the consequences of this creative process for cultural ideas, on the other: it asks what role 'rewriting' - deliberate and inventive adaptation - plays in the Reisebuch's portrayals of the East. My thesis therefore provides not only the first in-depth reading of Schiltberger's Reisebuch as a 'medieval multi-text' (a term coined by Iain Macleod Higgins to indicate the inter-textuality and intra-textuality common to the medieval travel account) but highlights its representational process as multiple, polyvocal and dynamic.

This project explores the nature and extent of the relationship between Blake and Whitman. I examine their works to find affinities in tone, style and themes and seek to understand the origin of these affinities. The resultant discoveries, however, lead to the conclusion that, because of Whitman’s lack of exposure to Blake’s work, these affinities must be accounted for through a coterie of indirect influences on Whitman. Over the course of the introductory chapter, I establish the critical proclivity of connecting William Blake and Walt Whitman, providing examples of such critical interpretation; in addition, I provide an introduction to the key figures, terms, and works with which this thesis engages. The work of the second chapter of this project is to uncover in Whitman’s work, before he could have read Blake, those elements that are read as points of contact between them. Through close readings, I show that those aspects of Whitman’s work which are read as points of contact between Blake and Whitman predate Whitman’s exposure to Blake’s work, and so necessitate an engagement with influences shared by Blake and Whitman. The third chapter articulates the notion that a variety of influences affected Whitman’s composition of Leaves of Grass, and these various influences serve as an explanation for those apparent similarities between Blake and Whitman discussed in chapter two. The final element this chapter engages with is that of nineteenth-century periodical culture; this aspect of the influences articulated in this chapter provides a secondary explanation for the similarities discussed in the second chapter. The fourth and fifth chapters focus on the 1860 and 1867 iterations of Leaves of Grass and the 1867 and 1871–72 versions of Leaves of Grass, respectively, both with special emphasis on the poem that would become “Song of Myself.” The changes seen throughout these iterations will be used to understand Whitman’s evolving prosody as well as his changing public persona. These chapters also engage with the work of Swinburne, in chapter five, and of Gilchrist, in chapter four, as integral elements of this mediated influence of Blake on Whitman. In the final chapter of this work, I summarize my findings, suggest possible avenues for further inquiry, and discuss the implications of this research. There is a trend in Anglo-American literary criticism to see the relationship between America and England as adversarial rather than generative. The concluding chapter of this work will explore the idea of the Anglo-American literary tradition as a continuum—a complex of acceptance, extension, transformation, and refusal—and place the relationship of Whitman to Blake accurately on this continuum.

The article highlights the history of aksakology of the XX-XXI centuries. The beginning of scientific practice is associated with the name of S.I. Mashinsky and his monograph on the work of S.T. Aksakov. The research of E.L. Voitolovskaya, O.N. Belokopytova, A.V. Chicherin became the most systematic research of the 1950s - 1970s. It is shown that the main vectors of aksakology, since the 1980s, were set in the works of E.I. Annenkova, V.A. Koshelev, Yu.V. Mann. The efforts of Ufa linguists and literary critics, as well as participants of the Samara conferences of 2017 and 2020, were particularly noted. The monograph by V.E. Ugryumov and the developments on poetics made in recent years by A.A. Churkin are considered significant at the present stage.

The paper deals with the exile view of Czech literary criticism in the past decades, reflected in two essays: one by Igor Hajek, and the other one by Kvetoslav Chvatik. Igor Hajek (1931�1995), a Czech literary critic, who went to exile in 1969, played a significant role in presenting Czech literature abroad. Kvetoslav Chvatik (1930�2012) was a Czech philosopher, aesthetician, art historian, and literary theorist. Hajek taught at universities in the English-speaking world, while Chvatik worked in a German-speaking environment. Two periods are covered and compared in the paper: the first period, the period of pluralistic democracy and the resulting cultural structure when the literary criticism contributed to the fact that Czech literature reached the European level, and the period after February 1948 when the ruling ideology started to interfere in the development of literature. Two completely contradictory conceptions are described showing the radical changes that took place in literary criticism after 1948. The text looks at the role of literary criticism in an era when plurality of opinion is possible, and at the impact of the suppression of freedom of speech on the work of literary critics. It also shows how the process of the shift of literary criticism towards its true function in the spirit of democratising tendencies had gradually won its way.

This paper focuses on the field of literary history in order to show what approach to the historiography of Czech literature was taken by the representative of Czech exile literary criticism, Igor Hajek. The context which Hajek entered during his study stays in the USA and Great Britain, and later in exile, was the reception horizon of the late 1960s, when the events of the �Prague Spring� attracted the attention of the West and turned attention to the Czech liberalisation movement, in which literature played a significant role. Hajek assumed the role of a mediator of the fundamental values of Czech literary production to the Western audience from the position of an expert in the Anglo-American cultural environment and Czech and foreign literary approaches. The specificity of his perspective is due to the fact that he tried to present the image of Czech national literature with respect to a non-Czech reader and that he aimed to clarify the main features of the development of Czech literature to international students and readers. The paper presents the conclusions of the analysis of Hajek�s literary-historical essays, which show that Igor Hajek relied mainly on the views of Arne Novak, a Czech literary historian and critic. The paper further assumes that Igor Hajek, due to his background in English studies, methodologically drew on some of the approaches that were being promoted in the West in his time and notes the connections between Hajek�s methods and the methodologies these approaches are based on.

The reintegration of the classical values in the cultural life of postwar Bessarabia was a long and difficult path, because of the involvement of the political factor, the powerful impact of the sociological mentality, and from here – the selective exegetical approach, influenced by the ideology of power and the enforcing of restrictive and dogmatically norms, with focus on certain writers and works, certain aspects of those. Among the most active and competent representatives of the literary heritage in the area between the rivers Prut and Nistru from the second half of the 20th century were the literary historians Efim Levit, Eugeniu Russev, Haralambie Corbu, Pavel Zavulan, Ion Vasilenco, Lazăr Ciobanu, Vasile Ciocanu, Pavel Balmuş, researchers from the Classical literature section (today the Premodern and modern Romanian literature Section of the Institute of Romanian Philology „Bogdan Petriceicu-Hasdeu”). Their achievements in the literary and historical areas constitutes the subject of this article.

The poetry inscription, with Bai Juyi in the Middle Tang Era as its representative, began to express purely personal emotions in terms of content, which reflects the poet’s creative individuality. Bai takes stone as his friend, loves it, chants it, and inscribes poems on it, endowing it natural and personal qualities. Bai was the first poet to consciously combine “poetry” and “stone” with nearly 20 kinds of poetry inscriptions. Compared with book documents, Bai’s poetry inscriptions not only have the philological value of text criticism, but also have multiple functions, i. e., reproducing the historical context of poetry creation and transmitting as a “linguistic landscape”. Such humanistic connotation determines the significance of Bai’s poetry inscription in the history of Chinese literature and culture.

The literary critic Mihai Cimpoi lets the curtain fall and presents us with some dialogues with the representatives of China and Japan. Reading these scholarly discussions, we can dive among the Alpha and Omega of Mihai Eminescu’s works subtleties, we also manage to pick from the precious seeds of universality and crystallize interculturality – the basis of precious axiology. Academician Mihai Cimpoi’s dialogues with the representatives of China and Japan profile the attitude of society in the Far East towards the values identified in Romanian literature, towards our Light (which the Chinese consider to be theirs). These discussions develop the props of universal literary criticism and offer a new grid for presenting interculturality, a spectrum of ideas woven in a friendly atmosphere, a constructive dialogue between personalities of phenomenal erudition.

Terrorist attacks using explosives and CBRN (Chemical, Biological, Radiological and Nuclear) materials have been present throughout history. While the frequency of CBRN terrorist attacks is relatively low compared to those with explosives and other types of weapons, it is crucial to treat the efforts of both terrorist organizations and individuals with appropriate gravity in order to avert catastrophic consequences. Identifying warning signs that indicate criminal behaviour is crucial for preventing planned crimes or terrorist attacks, and there is a need for more precise coverage of potential risk indicators related to CBRN and explosive crimes. This research aimed at examining and scrutinizing possible warning signs associated with planning and conducting terrorist attacks using CBRN and explosive materials. The research was implemented in three phases. First, comprise the systematic literature review. In the second phase, the case studies and CCTV records from past cases from Europe, USA, Australia and Asia were analysed and the aim was to create a list of risk indicators and categories for future reference by developing a methodological tool. The last phase represented a survey in which the practitioners from European Law enforcement and Intelligence Agencies critically assessed the list of risk indicators and their categories created based on the previous two steps of the research. The last goal was to gain the agreement and endorsement of law enforcement officials from different European nations regarding the validity and importance of recognized risk indicators and their categories, as well as their ranking for use in operational tasks, investigations, and training. The majority of the respondents found the identified categories and risk indicators as reliable and relevant for their operational activities and investigations. For the second research question, the survey results prioritized categories of risk indicators that are most suitable for the detection tactics of investigators and intelligence officers. The third research question examined the ease of observing identified risk indicators, with the category of technological detection/air sampling alarm risk indicators ranking as the easiest to detect. Finally, the survey found that the identified risk indicators are useful for training activities of security entities. Several final comments and recommendations from participants were also discussed, emphasizing the importance of considering multiple factors when identifying risk indicators and the value of the comprehensive list of identified risk indicators. The publication also examines some terrorist theories, the advantages, limitations, and the ongoing debate surrounding the use of profiling in protective security.

The article reveals and characterizes the methodological features of teaching the discipline «Intellectual and Psychological Foundations of Mass Media Functioning» on the third year of the Faculty of Journalism at Ivan Franko National University of Lviv. The focus is on the principles, functions, and standards of journalistic creativity during the full-scale war of the Russian Federation against Ukraine. As the Russian genocidal, terrorist, and ecocidal war has posed acute challenges to the education and upbringing of student youth. A young person is called not only to acquire knowledge but to receive them simultaneously with comprehensive national, civic, and moral-spiritual upbringing. Teaching and educating students, the future journalists, on Ukrainian-centric, nation-building principles ensure a sense of unity between current socio-political processes and historical past, and open an intellectual window to Ukraine’s future. The teaching of the course ‘Intellectual-Psychological Foundations of Mass Media Functioning’ (lectures and practical classes, creative written assignments) is grounded in the philosophy of national education and upbringing, aimed at shaping a citizen-patriot and a knight, as only such a citizen is capable of selfless service to their own people, heroic struggle for freedom, and the united Ukrainian national state. The article presents student creative works, the aim of which is to develop historical national memory in students, promote the ideals of spiritual unity and integrity of Ukrainian identity, nurture the life-sustaining values of the Ukrainian language and culture, perpetuate the symbols of statehood, and strengthen the moral dignity and greatness of Ukrainian heroism. A methodology for assessing students’ pedagogical-professional competence and the fairness of teachers who deliver lectures and conduct practical classes has been summarized. The survey questions allow students to express their attitudes towards the content, methods, and forms of the educational process, which involves the application of experience from European and American countries, but the main emphasis is on the application of Ukrainian ethnopedagogy. Its defining ideas are democracy, populism, and patriotism, enriched with a distinct nation-building potential, which instills among students a unique culture of genuine Ukrainian history, the Ukrainian language and literature, national culture, and high journalistic professionalism. Background Lung cancer is the number one cause of cancer death worldwide.(1) It is the fifth most commonly diagnosed cancer in Australia (12,741 cases diagnosed in 2018) and the leading cause of cancer death.(2) The number of years of potential life lost to lung cancer in Australia is estimated to be 58,450, similar to that of colorectal and breast cancer combined.(3) While tobacco control strategies are most effective for disease prevention in the general population, early detection via low dose computed tomography (LDCT) screening in high-risk populations is a viable option for detecting asymptomatic disease in current (13%) and former (24%) Australian smokers.(4) The purpose of this Evidence Check review is to identify and analyse existing and emerging evidence for LDCT lung cancer screening in high-risk individuals to guide future program and policy planning. Evidence Check questions This review aimed to address the following questions: 1. What is the evidence for the effectiveness of lung cancer screening for higher-risk individuals? 2. What is the evidence of potential harms from lung cancer screening for higher-risk individuals? 3. What are the main components of recent major lung cancer screening programs or trials? 4. What is the cost-effectiveness of lung cancer screening programs (include studies of cost–utility)? Summary of methods The authors searched the peer-reviewed literature across three databases (MEDLINE, PsycINFO and Embase) for existing systematic reviews and original studies published between 1 January 2009 and 8 August 2019. Fifteen systematic reviews (of which 8 were contemporary) and 64 original publications met the inclusion criteria set across the four questions. Key findings Question 1: What is the evidence for the effectiveness of lung cancer screening for higher-risk individuals? There is sufficient evidence from systematic reviews and meta-analyses of combined (pooled) data from screening trials (of high-risk individuals) to indicate that LDCT examination is clinically effective in reducing lung cancer mortality. In 2011, the landmark National Lung Cancer Screening Trial (NLST, a large-scale randomised controlled trial [RCT] conducted in the US) reported a 20% (95% CI 6.8% – 26.7%; P=0.004) relative reduction in mortality among long-term heavy smokers over three rounds of annual screening. High-risk eligibility criteria was defined as people aged 55–74 years with a smoking history of ≥30 pack-years (years in which a smoker has consumed 20-plus cigarettes each day) and, for former smokers, ≥30 pack-years and have quit within the past 15 years.(5) All-cause mortality was reduced by 6.7% (95% CI, 1.2% – 13.6%; P=0.02). Initial data from the second landmark RCT, the NEderlands-Leuvens Longkanker Screenings ONderzoek (known as the NELSON trial), have found an even greater reduction of 26% (95% CI, 9% – 41%) in lung cancer mortality, with full trial results yet to be published.(6, 7) Pooled analyses, including several smaller-scale European LDCT screening trials insufficiently powered in their own right, collectively demonstrate a statistically significant reduction in lung cancer mortality (RR 0.82, 95% CI 0.73–0.91).(8) Despite the reduction in all-cause mortality found in the NLST, pooled analyses of seven trials found no statistically significant difference in all-cause mortality (RR 0.95, 95% CI 0.90–1.00).(8) However, cancer-specific mortality is currently the most relevant outcome in cancer screening trials. These seven trials demonstrated a significantly greater proportion of early stage cancers in LDCT groups compared with controls (RR 2.08, 95% CI 1.43–3.03). Thus, when considering results across mortality outcomes and early stage cancers diagnosed, LDCT screening is considered to be clinically effective. Question 2: What is the evidence of potential harms from lung cancer screening for higher-risk individuals? The harms of LDCT lung cancer screening include false positive tests and the consequences of unnecessary invasive follow-up procedures for conditions that are eventually diagnosed as benign. While LDCT screening leads to an increased frequency of invasive procedures, it does not result in greater mortality soon after an invasive procedure (in trial settings when compared with the control arm).(8) Overdiagnosis, exposure to radiation, psychological distress and an impact on quality of life are other known harms. Systematic review evidence indicates the benefits of LDCT screening are likely to outweigh the harms. The potential harms are likely to be reduced as refinements are made to LDCT screening protocols through: i) the application of risk predication models (e.g. the PLCOm2012), which enable a more accurate selection of the high-risk population through the use of specific criteria (beyond age and smoking history); ii) the use of nodule management algorithms (e.g. Lung-RADS, PanCan), which assist in the diagnostic evaluation of screen-detected nodules and cancers (e.g. more precise volumetric assessment of nodules); and, iii) more judicious selection of patients for invasive procedures. Recent evidence suggests a positive LDCT result may transiently increase psychological distress but does not have long-term adverse effects on psychological distress or health-related quality of life (HRQoL). With regards to smoking cessation, there is no evidence to suggest screening participation invokes a false sense of assurance in smokers, nor a reduction in motivation to quit. The NELSON and Danish trials found no difference in smoking cessation rates between LDCT screening and control groups. Higher net cessation rates, compared with general population, suggest those who participate in screening trials may already be motivated to quit. Question 3: What are the main components of recent major lung cancer screening programs or trials? There are no systematic reviews that capture the main components of recent major lung cancer screening trials and programs. We extracted evidence from original studies and clinical guidance documents and organised this into key groups to form a concise set of components for potential implementation of a national lung cancer screening program in Australia: 1. Identifying the high-risk population: recruitment, eligibility, selection and referral 2. Educating the public, people at high risk and healthcare providers; this includes creating awareness of lung cancer, the benefits and harms of LDCT screening, and shared decision-making 3. Components necessary for health services to deliver a screening program: a. Planning phase: e.g. human resources to coordinate the program, electronic data systems that integrate medical records information and link to an established national registry b. Implementation phase: e.g. human and technological resources required to conduct LDCT examinations, interpretation of reports and communication of results to participants c. Monitoring and evaluation phase: e.g. monitoring outcomes across patients, radiological reporting, compliance with established standards and a quality assurance program 4. Data reporting and research, e.g. audit and feedback to multidisciplinary teams, reporting outcomes to enhance international research into LDCT screening 5. Incorporation of smoking cessation interventions, e.g. specific programs designed for LDCT screening or referral to existing community or hospital-based services that deliver cessation interventions. Most original studies are single-institution evaluations that contain descriptive data about the processes required to establish and implement a high-risk population-based screening program. Across all studies there is a consistent message as to the challenges and complexities of establishing LDCT screening programs to attract people at high risk who will receive the greatest benefits from participation. With regards to smoking cessation, evidence from one systematic review indicates the optimal strategy for incorporating smoking cessation interventions into a LDCT screening program is unclear. There is widespread agreement that LDCT screening attendance presents a ‘teachable moment’ for cessation advice, especially among those people who receive a positive scan result. Smoking cessation is an area of significant research investment; for instance, eight US-based clinical trials are now underway that aim to address how best to design and deliver cessation programs within large-scale LDCT screening programs.(9) Question 4: What is the cost-effectiveness of lung cancer screening programs (include studies of cost–utility)? Assessing the value or cost-effectiveness of LDCT screening involves a complex interplay of factors including data on effectiveness and costs, and institutional context. A key input is data about the effectiveness of potential and current screening programs with respect to case detection, and the likely outcomes of treating those cases sooner (in the presence of LDCT screening) as opposed to later (in the absence of LDCT screening). Evidence about the cost-effectiveness of LDCT screening programs has been summarised in two systematic reviews. We identified a further 13 studies—five modelling studies, one discrete choice experiment and seven articles—that used a variety of methods to assess cost-effectiveness. Three modelling studies indicated LDCT screening was cost-effective in the settings of the US and Europe. Two studies—one from Australia and one from New Zealand—reported LDCT screening would not be cost-effective using NLST-like protocols. We anticipate that, following the full publication of the NELSON trial, cost-effectiveness studies will likely be updated with new data that reduce uncertainty about factors that influence modelling outcomes, including the findings of indeterminate nodules. Gaps in the evidence There is a large and accessible body of evidence as to the effectiveness (Q1) and harms (Q2) of LDCT screening for lung cancer. Nevertheless, there are significant gaps in the evidence about the program components that are required to implement an effective LDCT screening program (Q3). Questions about LDCT screening acceptability and feasibility were not explicitly included in the scope. However, as the evidence is based primarily on US programs and UK pilot studies, the relevance to the local setting requires careful consideration. The Queensland Lung Cancer Screening Study provides feasibility data about clinical aspects of LDCT screening but little about program design. The International Lung Screening Trial is still in the recruitment phase and findings are not yet available for inclusion in this Evidence Check. The Australian Population Based Screening Framework was developed to “inform decision-makers on the key issues to be considered when assessing potential screening programs in Australia”.(10) As the Framework is specific to population-based, rather than high-risk, screening programs, there is a lack of clarity about transferability of criteria. However, the Framework criteria do stipulate that a screening program must be acceptable to “important subgroups such as target participants who are from culturally and linguistically diverse backgrounds, Aboriginal and Torres Strait Islander people, people from disadvantaged groups and people with a disability”.(10) An extensive search of the literature highlighted that there is very little information about the acceptability of LDCT screening to these population groups in Australia. Yet they are part of the high-risk population.(10) There are also considerable gaps in the evidence about the cost-effectiveness of LDCT screening in different settings, including Australia. The evidence base in this area is rapidly evolving and is likely to include new data from the NELSON trial and incorporate data about the costs of targeted- and immuno-therapies as these treatments become more widely available in Australia.
