Carcinoma of the external auditory canal (EAC) is known to be very rare with a rate of 1-6 per million people. It affects mostly people over the age of 50 regardless of gender. The most common type of carcinoma of EAC is squamous cell carcinoma (SCC). The aim of this paper is to present the case of a 56-year-old woman with carcinoma of EAC. She was admitted to otolaryngology clinic due to ear buzzing lasting for five months, periodic otorrhea and hearing deterioration in her right ear. In otoscopic examination a tumor in the right ear meatus was identified. It filled almost the entire canal. Biopsy confirmed the presence of squamous-cell cancer. Tumor advancement was diagnosed using computer tomography of the temporal bones, magnetic resonance imaging of the head and ultrasonography of the neck. Imaging examinations indicated T3 N0 stage. Doctors decided to perform lateral petrosectomy with elective neck dissection. During the operation, the results of histopathological examination of frozen specimens from marginal tissues revealed an invasion of the anterior wall of the ear canal, capsule of temporomandibular joint and parotid gland. The resection eventually included also parotid gland and temporo-mandibular joint with the caput of the mandible. After recuperation, the patient had complementary radiotherapy. The presented case shows that precise imaging is necessary before surgical treatment, but unfortunately the invasion of cancer may not be estimated properly. Based on literature, the best prognosis concerns wide resection with complementary radiotherapy, although the schedule of treatment is not determined.

Tinnitus produced by repetitive contraction of the middle-ear muscles is a rare condition. We present an interesting case of bilateral middle-ear myoclonus causing incapacitating tinnitus in a patient with multiple sclerosis. Otological examination demonstrated rhythmic involuntary movement of the tympanic membrane. These movements correlated with a rhythmic ‘rushing wind’ noise perceived by the patient. Oropharyngeal examination showed no evidence of palatal myoclonus. Impedance audiometry confirmed rhythmic change in the middle-ear volume. Medical management was unsuccessful. The patient’s tinnitus was subsequently cured with bilateral sectioning of the tensor tympani and stapedial tendons.

A two years old male cat domestic long hair was brought to the Tabby Pet Care clinic with the owner's complaint that the cat often scratches his right and left ears. The results of clinical examination showed scratching marks on the pinna, blackish yellow, wet, and smelly ear serumen on the ear canal. Investigations include microscopic examination of ear swabs and cerumen swab cytology. Ear swab examination results observed the presence of Otodectes cynotis and Otodectes cynotis eggs. The results of cytological examination of cerumen swabs using the rapid stain method were observed to have bacterial infections in the form of bacilli, coccus, and the presence of neutrophil infiltration. Based on clinical symptoms and the results of investigations, the patient was diagnosed with external otitis with a fausta prognosis. Treatment of otitis externa is carried out with topical therapy Ilium® ear drop. Therapy is carried out for seven days. On the eighth day the patient was declared cured and allowed to go home.

Dichotic listening of auditory stimuli is a method of assessing brain lateralisation. Different stimuli are presented simultaneously to the left and right ears, with the listener reporting which stimulus is perceived most clearly. To date, several studies that have examined the effects of aging on dichotic listening performance have indicated a pronounced right ear advantage (REA) with increasing age, but few studies have considered the effects of sex, and findings to date have been inconclusive. The aim of this research was to investigate whether the effects of age and sex resulted in a difference in the magnitude of the REA in both undirected and directed attentional tasks. Forty sex-matched, right-handed subjects with normal hearing or symmetrical bilateral sensorineural hearing loss participated in a series of directed and undirected dichotic listening tasks using consonant-vowel (CV) stimuli. The interaural intensity difference (IID) was modulated randomly during the undirected attention task. Results indicated that all groups (age & sex) showed a REA for both undirected and directed dichotic listening tasks. No age or sex-related differences were found. The findings were suggestive of a task-linked effect for dichotic listening performance. The use of CV stimuli, in combination with detailed testing via manipulation of the IID, appeared to minimize any possible age or sex-related differences. These findings have implications for theories on laterality and hemispheric asymmetry for older adults.

Otitis media is one of the most common childhood ailments seen in clinical practice today, yet the condition has rarely been identified in the skeletal remains of past populations. Other diseases of the middle ear and mastoid have been sporadically reported in the archaeological literature but no standardised criteria for their diagnosis in temporal bones have been established. A comparative study of different non-destructive methods of examination of human temporal bones was undertaken in order to determine the optimum strategy for future research. The material for the study consisted of 1244 temporal bones representing 688 individuals from seven English cemeteries dated to the Romano-British, Anglo-Saxon and medieval periods. The examination of such a wide range of material facilitated determination of optimum diagnostic criteria while eliminating single site bias. It also allowed a comparison to be made, of the prevalence of ear disease, between populations separated both geographically and chronologically. The inclusion in the study of the human skeletal meterial from a medieval leprosy hospital cemetery at Chichester afforded the opportunity to investigate the possibility that ear disease was more prevalent in those individuals who had suffered from leprosy. A review of the documentary evidence for the treatment of middle ear disease in the past suggests that, prior to the introduction of antibiotic therapy, no truly effective treatment was available and, therefore, palaeopathological evidence is unlikely to have been influenced by therapeutic intervention. In the preparation of skeletal reports the temporal bones should, ideally, be examined using a combination of methods. Two methods are recommended as being the most useful where resources are limited but in the identification of particular pathological conditions other methods proved useful. This research has demonstrated that middle ear disease in ancient populations can, with some certainty, be diagnosed using the proposed criteria, thus removing the barrier to future comparative research in this field.

Young adults 18-25 years of age in the United States are experiencing a new world as they transition to young adulthood in college. The purpose of this investigation is to examine the eating behaviors of the young college population. The present study will examine the kinds of foods college students eat and how they make decisions regarding food consumption. The methodology of the present study consists of a quantitative survey, which includes questions about gender, living arrangements, convenience, and student eating habits. The sample size is comprised of 228 students at the University of Central Florida. A regression model is used to explore the influence of the independent variables, which include gender; living arrangement; convenience; and health have on eating habits. Findings indicated that convenience significantly influence eating habits (p [less than] .001). Gender, living arrangements, and health, however, did not significantly influence the eating habits of the college student respondents. A second regression model examined the potential effects of gender, living arrangements, convenience, and health- weight gain on eating habits. Again, convenience was the only significant independent variable that has an influence on eating habits (p[less than].001) while gender, living arrangements, and healthweight gain were not significant predictors.

Background: Ramsay Hunt syndrome or herpes zoster oticus is a disorder caused by the reactivation of the varicella zoster virus in the geniculate ganglion, a bundle of nerve cells in the facial nerve. Symptoms include acute facial nerve palsy, otalgia, loss of taste in the anterior 2/3 of the tongue, dry mouth and eyes, and erythematous vesicular rash in the ear canal, tongue and / or palate. Objectives and Methods: Describe the case of a male patient, 26 years old, with complete Ramsay Hunt syndrome, reporting pain in the right ear with tinnitus, difficult to close his right eye, odynophagia and pain in the anterior 2/3 of the tongue at right side. Results: On examination, the presence of vesicles with erythema was observed in the region of the right external auditory canal and the ear, as well as on the right palate and tongue. Upon inspection, he had peripheral facial paralysis on the right, associated with loss of taste in the anterior 2/3 of the tongue. The patient was treated with antiviral and corticosteroids, followed for 3 months, obtaining partial recovery from facial paralysis. The anatomy of the facial nerve and its pathophysiology due to the involvement of herpes zoster will be discussed. Conclusion: Ramsay Hunt syndrome is often described as the presence of peripheral facial paralysis and vesicles in the auditory canal and the ear. However, this rare case presented the complete condition with the presence of vesicles on the palate and the tongue.

Context: Ramsay Hunt Syndrome (RHS), also named Optic Herpes Zoster is caused by the reactivation of Varicella-Zoster Virus that occurs in the geniculate ganglion. The classic syndrome is composed of vesicular lesions in the ear, ipsilateral otalgia and peripheral facial palsy. The onset of the syndrome with persistent hemicrania is absolutely atypical as a manifestation of RHS. Case report: A 69 year old woman with subacute onset of left exclusive hemicrania in moderate strong intensity without autonomic signs. Eight days after the onset of headache, she presented left peripheral facial paralysis and after 4 days, she noticed the appearance of vesicles in the left ear and pain in swallowing. She has developed nausea with several episodes of vomiting, important imbalance that made impossible gait. On physical examination she presented vesicles in the left ear and oropharynx, left peripheral facial paralysis House Brackmann grade IV, left hypoacusis, nystagmus and vestibular gait. The brain CT and cerebrospinal fluid analysis were normal. The patient has progressive improvement with Acyclovir. Conclusions: Otalgia or even intermediate neuralgia are more frequent painful manifestations of RHS. This case report highlights the need to consider the diagnosis of SRH in patients with persistent hemicrania, even if without initial classic signs of the syndrome.

Abstract: Relevance. Noise is the most common harmful factor in the production of titanium alloys. The aim of the study is to identify age and gender features of the formation of general somatic morbidity associated with the impact of industrial noise at a large metallurgical enterprise. Scope and methods. The study compared the prevalence of chronic pathology of 4681 people who have contact with noise above 80 dBA, and those who are not exposed to noise using the methodology of occupational risk analysis of the Research Institute of Occupational Medicine. academician N. F. Izmerov according to the results of a periodic medical examination The results. The formation of general somatic pathology of many systems of the body under the influence of industrial noise occurs from the first years of operation. An increased risk of developing ear pathology was identified with work experience of more than 7 years. Men under the influence of noise had a significantly higher prevalence of diseases of the ear and digestive organs than women, with higher risks compared to those who were not exposed to noise and a medium to high relationship with working conditions. Conclusion. The revealed features of the formation of general somatic pathology under the influence of industrial noise allow us to recommend the introduction of health and rehabilitation programs from the first years of working in noise, even during the period of adaptation of young workers to the production environment.

Abstract: Relevance. Local vibration is a significant factor in the production environment of metallurgical production, which affects health and efficiency. The aim of the study is to identify the exposure, gender and age features of the formation of general somatic morbidity associated with the impact of local vibration in the production of titanium alloys. Scope and methods. The prevalence of general somatic pathology was compared in 2596 workers who have contact with local vibration, with varying degrees of harmfulness of working conditions, and non-exposed persons according to the results of periodic medical examination using the methodology of occupational risk analysis of the Research Institute of Occupational Medicine. academician N. F. Izmerov. Results. At comparison of prevalence of a somatic pathology at workers in contact to local vibration to not exhibited persons according to medical inspection, and at different degree of harm of working conditions, are revealed the raised risks of formation of diseases nervous, kostno-muscular, endocrine systems, respiratory organs, digestion, an ear, a skin, an arterial hypertensia, the mental frustration, the raised weight of a body, gynecologic diseases with a tendency of growth of the majority of them at the big levels of vibration, more often at small, less often - average degree of communication with working conditions. The frequency of pathology in exposed workers was higher in all age groups. Average degree of communication with working conditions of a gynecologic pathology at the age of 18-25 years was marked. At men frequency of a pathology of respiratory organs, digestion is revealed authentically big, than at women; an ear; skin at the big relative risks 1,2-1,3, of the raised arterial pressure and hyperglycemia. Conclusion. A higher frequency of visceral pathology of a number of body systems under the influence of local vibration of high levels may justify the need to develop differentiated preventive programs to minimize its negative impact, taking into account the identified features in sex and age groups.

Annotation. On the example of railway and air transport, a number of new issues of occupational hearing loss are shown, requiring study and subsequent implementation in practical health care, as new etiological factors in the development of hearing loss. This concerns the factor of labor intensity, which has a potentiating, pathogenetic significance, along with noise, in the development of hearing loss due to the formation of chronic stress, leading to additional ischemia of the vascular stria and damage to the neuroepithelium of the inner ear. There is a need, along with the development of a methodology for assessing the factor of labor intensity for SOUT of workplaces, to determine the criteria for its negative action when used for the examination of the connection between the disease of the organ of hearing and the profession. In the «List of occupational diseases» in clause 2.4.2. indicated diseases associated with exposure to infrasound (IZ), among the manifestations of which, called sensorineural hearing loss bilateral. However, IZ, as well as low-frequency noise (LFN), were not previously considered as significant adverse factors in relation to the hearing organ in mass clinical trials, therefore, there are no expert criteria for the relationship between hearing loss and their impact, which could be applied in practice, which requires accumulation of evidence base for the subsequent substantiation of an independent nosological form of a disease of the organ of hearing, associated with mechanoacoustic exposure.

Context: Vogt-Koyanagi-Harada syndrome (VKH) is a rare, multisystemic, autoimmune disease mediated by a Th1 response against melanocytes in the eye, inner ear, central nervous system, skin and hair. In this article, we report a case of VKH with severe visual impairment and discuss the therapeutic response to corticotherapy followed by the use of Adalimumab, a tumor necrosis factor (TNFα) inhibitor. Case report: A 61-year-old black woman started bilateral frontal headache of severe intensity, associated with bilateral eye pain, hyperemia and watery eyes, progressing with visual turbidity with gradual worsening, seeing only figures after eight days. After ten days bilateral hypoacusis started, also progressive. She denied eye movement pain, diplopy, dizziness, fever, joint pain or skin injuries. On examination, visual acuity (VA) in RE: hand movement for 30 cm, LE: light perception, fundus of the eye with serous bilateral retinal detachment. CSF with 155 lymphomonocyte predominance cells, proteins: 73, negative bacterioscopy and cultures. Pulsotherapy was performed for 7 days followed by 1g of cyclophosphamide and maintenance therapy with fortnightly Adalimumab. Two months after discharge, she presented VA in RE: 20/200 and LE: counting fingers at 1 meter. Conclusions: Aggressive and early treatment with immunosuppression is key to the effective treatment of VKH. Immunotherapy can be used in patients who are unresponsive to corticosteroid doses. Biological agents that target TNFα have effective results in non-infectious uveitis. Adalimumab is a safe and effective option, which also reduces the need for chronic corticosteroid therapy. The prognosis depends on the early diagnosis and treatment.

European Pressurized Reactor (EPR) nuclear power plant is now under construction in China. One of the biggest changes in the EPR reactor pressure vessel (RPV) is the nozzle to shell welds are designed as “set-on” principle instead of “set-in” principle in the CPR1000 units. The nozzle to shell welds of the EPR pressure vessel must be inspected according to the RSE-M code. This is necessary in order to guarantee the integrity of the primary circuit. The In-Service Inspection (ISI) program of EPR nuclear power plant demands the automatic ultrasonic inspection of the nozzle to shell welds from nozzle inner surface. This paper presents the technical characteristics of the EPR reactor vessel, and analyzes the in-service examination requirements of nozzle to shell welds. Technical solutions have been designed to perform the examination of the component. The qualification process of the ultrasonic examination of the nozzle to shell welds is also presented.
