Background/Aims: To investigate the prevalence and trajectories of depressive symptomatology at 1-year follow-up, and the severity of depressive symptoms, by dementia diagnostic group, as well as to determine the predictors of depressive symptomatology at 1-year follow-up. Methods: In rural and remote patients of an interdisciplinary memory clinic between 2004 and 2014, 144 patients diagnosed with no cognitive impairment (NCI), mild cognitive impairment, dementia due to Alzheimer's disease (AD), or non-AD dementia completed the Center for Epidemiologic Studies of Depression Scale to assess depressive symptomatology at both time points. Results: Among patients with data at both time points, persistence of depressive symptomatology at follow-up occurred in 22.2%, remission in 17.4%, incidence in 13.2%, and absence in 47.2%. The prevalence of depressive symptomatology at baseline and persistence at follow-up were significantly greater in the NCI group than in the other diagnostic groups, but there were no differences in severity. Depressive symptomatology at follow-up was independently associated with depressive symptomatology, lower independence in activities of daily living, and lower self-rating of memory at baseline, as well as with decreased independence in activities of daily living between time points. Conclusion: Future studies should further examine short-term postdiagnostic trajectories in depressive symptomatology in multiple dementia diagnostic groups to inform prognoses and treatment decisions.

Abstract Objective To examine the relationship between athletes with and without Autism diagnosis and self-reported symptoms as measured by ImPACT. Method Participants were selected from an archival de-identified sports medicine ImPACT database. The sample (N = 232) was primarily male (72.4%) student athletes with a mean age of 15.41 years (SD = 1.292). Participants were divided into two groups: Autism diagnosis (n = 130); No diagnosis (n = 102). An independent samples t-test was conducted to analyze the variation of self-reported symptoms between athletes with and without an Autism diagnosis. Results The independent samples t-test revealed significant differences between diagnostic groups and self-reported cognitive (t = 5.832, p < .001, d = .72); sleep (t = 4.040, p < .001, d = .51); vestibular somatic (t = 4.154, p < .001, d = .53); and affective symptomatology (t = 3.988, p < .001, d = .51). Athletes with a diagnosis of Autism reported more symptoms overall in comparison to athletes without a diagnosis. Conclusions These findings suggest that prior psychological diagnoses play a significant role in symptom scores; however, the degree to which these symptoms can be attributed to the diagnosis itself or the concussion is unknown. It should be noted that individuals who have been diagnosed with Autism may already be at an increased risk of sleep difficulties, emotional regulation, and sensory stimulation within the environment. Therefore, differentiating the symptom scores from preexisting symptoms of Autism from a concussion can be beneficial when helping athletes return to play.

Abnormal uterine bleeding (AUB) is one of the most common gynaecological disorders in women of all age groups, it affects quality of life (QoL) and carries high direct and indirect socioeconomic costs. The present article aims to summarise the different aetiologies of AUB, using the unambiguous terminology and systematic approach provided by the International Federation of Gynecology and Obstetrics (FIGO) PALM-COEIN classification system with its periodic evolutions; the objective is also to evaluate the clinical impact and the most accurate diagnostic management, due to continuous improvements in imaging and outpatient hysteroscopy in order to improve patient care and reduce healthcare costs.

The aim of the present investigation was to provide insight into how postconcussion symptomatology may be altered in individuals exhibiting attention-deficit/hyperactivity disorder (ADHD)–related behaviors and examine factors that may be responsible for driving such relationships. A total of 99 individuals were assessed during the subacute phase of concussion recovery. Inattentive symptomatology, but not diagnosis of ADHD, was related to greater concussion-symptom severity and overall symptoms endorsed. Cluster and factor analyses highlighted that the relationship between ADHD symptomatology and concussion symptomatology was not a function of overlapping constructs being assessed (i.e., concussion-related symptomatology was not a proxy of ADHD-related symptomatology). These relationships were not mediated by parental observations of impairments in behaviors associated with executive functioning (i.e., executive dysfunction was not driving the greater concussion-related symptomatology associated with ADHD-related symptomatology). These findings highlight the importance of moving beyond categorical frameworks of ADHD to, instead, consider the continuum of underlying behaviors.

The Positive and Negative Syndrome Scale (PANSS) has long been widely used to assess the extent and severity of psychiatric symptomatology in research studies with samples diagnosed with schizophrenia, particularly in pharmacological studies. Recently it was used in a federally funded multisite study with a mixed-diagnosis sample of persons with serious mental illness. Though numerous factor analytic studies of the PANSS (both exploratory and confirmatory) have been undertaken in samples with schizophrenia, its use in mixed-diagnosis samples characteristic of those encountered in research on psychiatric rehabilitation has not been much studied. This paper reports on a series of confirmatory factor analyses undertaken to explore the fit of various models found in the schizophrenia literature to data from the more heterogeneous multisite sample. Two of the models tested attained marginal fit to the data (using the conventional fit criteria of CFI > .90 and RMSEA), however, both failed to represent a broad range of symptoms because they incorporated only a few PANSS items. An alternative model was developed using all 30 PANSS items and tested using parceling to improve item fit. This 5 factor model demonstrated adequate fit to the data. These findings are discussed in light of the need for a model that fits a mixed-diagnosis sample.

Discospondylitis is defined as an infection of the intervertebral disc, as well as the two vertebrae adjacent to it. This disease is found in several species, however, dog is the most affected one. It may result of a hematogenous infection, paravertebral infection, pre/post surgery or foreign body. Clinical symptomatology usually has a wide rang, from pain, fever hyperthermia and paresis to a non-ambulatory paraplegia without deep nociception. A complete neurological examination is essential, as the neurological symptomatology occurs on 50% of affected animals. The diagnosis is based on imaging studies and laboratory tests and the ideal treatment is supported in the results of culture and antibiotic sensitivity test (AST). In this retrospective study there were analyzed dog clinical files referred for a neurology consult in Referência Veterinária (RRV) from March 2015 to June 2017, and there were selected 16 cases. Of all the cases confirmed with discoespondylitis, 69% of the pacients were older than 8 years when the first clinical signs appeared. It was selected about 62,5% of the sample which corresponded large breed dogs, such as, german shepherd, golden retriever and labrador. The prognosis depends on the etiology, neurological signs and response to treatment.

Abstract: Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a complex condition characterized by a broad spectrum of overlapping symptoms and manifesting in multiple systems of the body. Patients experience symptoms such as profound fatigue, post-exertional malaise (PEM), unrefreshing sleep, and cognitive impairments. It affects millions worldwide, yet much is still unknown about its etiology and pathophysiology. The condition's onset is frequently linked to infectious triggers, including viral infections, suggesting a dysregulated immune response as a central component. Diagnosing ME/CFS poses significant challenges due to many unspecific symptoms that overlap with various other conditions. Current treatment strategies focus primarily on symptomatic relief and lifestyle modifications to manage disease impact. The COVID-19 pandemic has further spotlighted ME/CFS, drawing parallels between long COVID and ME/CFS symptomatology and underscoring the urgent need for comprehensive research. Background: breast cancer is considered as one the most common type among women worldwide and for Jordanian citizens according to Jordan Ministry of health that there was around 1292 (38.4%) new cases of breast cancer reported in 2020, and The diagnosis of breast cancer is usually associated with psychological stress such as Anxiety and depression that is resulting from the diagnosis itself, where if it's was incurable diseases, fear of death, fear of loss, at the end previous studies show that there is a higher prevalence of depressive disorder which is up to two to three times more than the general population. Objective: Our goal in this cross-section study is to find out the prevalence of psychological and depressive disorders correlates to breast cancer (BC) in outpatient and inpatient setting as it was approved that depression is comorbid to cancer that should not be neglected. We also aim to identify risk factors of depression among study participants like cancer stage ( initial or later), income and marital status, and type of intervention chemotherapy or radiation therapy plus their treatment sessions Method: This study was conducted at king Abdullah university hospital in Irbid, King Hussein cancer center (KHCC), and queen Alia Military hospital in Amman, Jordan. More than 400 formed the study sample include inpatient and outpatient setting of breast cancer using in them Depression and anxiety assessment scale. In the inpatient setting The Hospital Anxiety and Depression Scale (HADS) instrument used, which is a 14-question instrument given to patients in a secondary care setting to screen for the presence and severity of depression and anxiety. Also, the beck depression Inventory (BDI) used, which is a self-report rating inventory that measures characteristics, attitudes, and symptoms of depression. In the outpatient setting The PHQ-9 instrument which includes nine questions given to the patient in primary care settings. The anxious symptomatology defined by using the GAD-7 instrument with a total score of 15 and above indicating a case with severe anxious symptomatology. Results: Our study findings demonstrated a higher prevalence of depressive and anxious symptomatology in the inpatient setting and advanced disease stages. In addition, the underutilization of antidepressant therapy was observed. there for we need to consider mental disorder as part of the treatment protocol for breast cancer patient. Introduction: Ramsay Hunt Syndrome, a rare clinical entity, is caused by the reactivation of the varicella zoster virus, which results in inflammation of the geniculate ganglion of the facial nerve. Characterized by the triad: ipsilateral facial paralysis, otalgia and herpetic vesicles in the external ear. Due to the broad symptomatology, which encompasses atypical symptoms and present in several other pathologies, it is often underdiagnosed. Case report: A 62-year-old woman admitted to Hospital Evangélico de Londrina due to paresthesia in the left hemiface, associated with vertigo and severe headache. On physical examination, peripheral facial paralysis on the left and vesicles on the palate and left external ear were observed. AVC protocol exams were requested (normal) and liquor was collected (clear, colorless appearance, 92% lymphocytes). New serologies and magnetic resonance imaging of the skull with contrast were requested. Prescribed acyclovir, prednisone, eye drops and ocular occlusion. The patient evolved with maintenance of headache and otalgia. Prednisone was replaced by intravenous dexamethasone, gabapentin and topical lidocaine were added, and other medications were maintained. Non-reactive requested serologies. Analysis of the herpes simplex virus by polymerase chain reaction > 30. In view of the data, a diagnosis of viral meningitis due to herpes associated with Ramsay Hunt syndrome was diagnosed. Maintained treatment, patient evolved with improvement of the condition. In this paper we examine the evolving roles of digital technology in shaping the professional identity and therapeutic encounters of homeopathic practitioners. Homeopathy is a form of alternative medicine that relies almost exclusively in making a diagnostic on patients’ verbal description of their symptoms. In contrast to the biomedical diagnosis, the homeopathic diagnosis does not place the patient in a disease category, but in a remedy category. The software that aids homeopathic professionals in their practice is based on a digitalization of books that organized the homeopathic knowledge of remedies and symptoms (repertories and Materia Medica). Organized as an inventory of descriptions of remedies and symptoms, the software operates algorithmically in helping the homeopath to match patients’ symptoms to possible remedies and select the most adequate remedy. During the consultation, the homeopath makes a patient file by writing a selection of words extracted from patients’ talk that express symptoms in great detail – including feelings, signs of suffering and various lifestyle preferences. In this process the homeopath and the software co-create the patient’s “person” as a collection of more-or-less idiosyncratic symptoms, in their attempt to match the patient’s symptomatology with the description of one single remedy. Drawing on interviews with patients and homeopaths, observations of consultations and homeopathic seminars, we argue that the use of digital technology is practiced as boundary work. Firstly, digital technology becomes a boundary marker between the patient and the homeopath. Secondly, digital technology helps homeopaths to legitimize their practice as a rigorous one, being one of the strategies used to gain acceptance in the health-care ecosystem. Last but not least, by presenting the software as only a time-saving device, a neutral aide for making “intelligent suggestions” that can be confirmed but also rejected by the homeopath’s judgment, homeopaths create a distinct professional identity oriented around a central principle of their practice: “Treat the patient as a whole person”.

Introduction: Psychiatric disorders occurring during endocrine dysfunction and, conversely, endocrine dysfunctions associated with mental disorders were the emergence of a new discipline, psychoendocrinology. Psychiatric disorders correlated with endocrine diseases are defined as psychopathological manifestations of variable intensity and clinical symptomatology, determined by complex psycho-neuro-endocrinological interrelationships. Defining elements consist of the association between diagnosis of mental disorders and specific symptoms for endocrine dysfunction. Methods: We conducted a prospective one-year study (January 2018 - December 2018 on 112 patients hospitalized in the Clinic of Psychiatry who also had an endocrinological comorbidity. We investigated the frequency and severity of psychoendocrinological associations by studying a number of demographic and clinical items. Results: The results showed that the highest incidence belongs to thyroid disorder - 55.36%, followed by gonadal disorders - 24.11%, and, rarely, pituitary diseases and diabetes. Hyperthyroidism was associated most frequently with manic episodes, while unipolar depression prevailed in patients with hypothyroidism. In gonadal disorders, present in majority in female patients (secondary amenorrhea, menopause or erectile dysfunction in males), depression accompanied by anxiety, often severe in intensity, was the most frequent psychiatric diagnosis. Psychotic disorders were met in a smaller number of cases, especially in patients with long history of endocrine disorders and instability of biological constants. Conclusions: We may state that affective disorders are the most frequent nosologically category in patients with endocrine dysfunctions. It requires a better collaboration between specialists in endocrinology and psychiatry, to highlight the determinants which contribute to the development of psychopathological manifestations in endocrine diseases and to individualize the treatment depending on cases’ particularities.

Context: Opsoclonys-myoclonus syndrome (OMS) is a rare neurologic disorder characterized by acute or chronic subcortical myoclonus and cerebellar ataxia at 6 months to 3 years-old children with rates of incidence expressed as 0.18 per 1.000.000 person- year. With nonspecific physiopathology, the only definitive finding is an elevated lymphocyte and positive B-cells count on the cerebrospinal fluid (≥11 cels/mm³) along with 50 to 93% cases reported from the National Pedriatric Myoclun Center (1989-2013) presenting oligoclonal bands. The diagnosis is established by clinical evaluation with exclusion criteria based on the presence of structural central nervous system damage and the aggressive treatment includes immunomodulatory therapy for behavior and cognitive stabilization. Case report: This paper aims to describe a case of a 1-year-old premature pediatrician patient presenting OMS in the absence of fetus distress due to pre- eclampsia condition with long-term hospitalization. After hypotonia, psychomotor agitation and vomit episodes, the patient was referred to Campo Grande (MS) where worsened to globus myoclonus, opsoclonus and nystagmus after 25 days of hospitalization, symptomatology responsive to Propranolol 10mg a day with regression of the clinical and neurological condition. Conclusion:Although OMS is a rare condition with variable prognosis, children appear to respond to pharmacological and non-pharmacological treatment improving the quality of life.

According to the World Health Organization (WHO), the echinococcosis-hydatidosis complex is one of the 17 Neglected Tropical Diseases in humans, caused by parasites of the genus Echinococcus spp. Occurring worldwide, it prevails in rural areas and precarious regions, with low sanitary and socioeconomic conditions, contributing to the evolution and subsistence of parasitic diseases. Among the species of Echinococcus, the species E. granulosus is the main responsible for damage to public health. In the echinococcosis-hydatidosis cycle, the adult form of the parasite is found in the small intestine of the definitive host (HD), generating eggs that will be eliminated together with the feces. The symptomatology depends on factors such as: affected organs, size of the cyst, location in the tissue, complications associated with the presence of the cyst and bacterial infections. Humans are infected due to poor sanitary conditions, and the parasite's eggs are ingested in water and/or food contaminated with HD feces. The diagnosis of hydatidosis begins with epidemiological precedents, followed by suspicion due to clinical signs; confirmation by imaging tests; identification of cyst constituents and confirmation by detection of antibodies by ELISA or indirect immunofluorescence. For treatment, surgical techniques can be used in conjunction with antiparasitic treatment. Considering that this is a disease with the possibility of recurrence, a multifactorial approach is necessary, associating drug and surgical treatment and long-term follow-up

The paper describes three conditions and their interrelationship - burnout syndrome, negative stress (distress) and managerial disease, which have a negative impact on both work efficiency and the psychophysical health of employees. Although the origin of the aforementioned conditions is prolonged stress, their symptomatology differs, which is important from the point of view of differential diagnosis, and for the purpose of applying adequate measures of prevention and therapy. Burnout manifests itself through three basic components: emotional exhaustion, cynicism in the context of a negative attitude towards work and clients, and significantly reduced work efficiency. These components arise from the meeting of internal, intrinsic personality factors (first of all, perfectionism and unrealistically high expectations and assessments related to oneself and one's own work) and external, extrinsic ones, which relate to the organization of work and the behavior of management staff. On the other hand, managerial disease is characteristic of certain psychological (so-called A and D) personality types who are chronically irritable, quarrelsome, nervous, prone to control, which results in eating, sleeping and, often, cardiovascular problems which, if not treated treat, can lead to severe and irreversible health outcomes. The prevalence of the described conditions requires the education of employees in order to prevent them, as well as the presence of experts, the so-called. stress-manager for preventive and therapeutic action, and together, as research shows, it has a positive psychological-health, social and even economic effect on the individual and the community as a whole.

Introduction: Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL), is a non-atherosclerotic, nonamyloid, hereditary cerebral disease of small vessels and capillaries caused by mutations in the NOTCH-3 gene located on chromosome 19. The presence of granular osmophilic material (GOM) deposition in the smooth muscle cells of vessel walls is the pathological hallmark of arteriopathy in CADASIL. GOM deposits in the basal lamina of smooth muscle of small vessels are pathognomonic for CADASIL. The presence of GOM in capillary blood vessels of the skin and muscle in biopsy and genetic studies (NOTCH-3 analysis) plays a key diagnostic role. Biopsy tests have high specificity (up to 100%) and low sensitivity (less than 50%). The NOTCH-3 test has been proposed as the primary diagnostic approach, allowing detection of 90% of affected individuals. CADASIL has an estimated prevalence between 2 and 5 in 100,000, and the phenotypic study demonstrates different clinical symptoms in the course of the disease within the same family. The average age of onset of clinical symptoms varies between 48.3 years in men and 52.2 years in women. Characteristic symptoms of migraine, stroke or TIA (transient ischemic attack), behavioral changes, early and progressive cognitive changes associated with leukoencephalopathy on imaging studies. The large association of symptoms often causes the diagnosis of CADASIL to be delayed. In this case, we had an association of gait ataxia within the framework of motor alterations, demonstrating the wide range of symptomatology of the pathology. This case report presents a familial course that started outside the most prevalent age group in the studies described and with an atypical presentation in an affected generation. Case reports: Case 01: woman, 68 years old, started progressively forgetting to perform household activities after the age of fifty, associated with primarily generalized myoclonic epileptic seizures, evolving rapidly within five years to walking apraxia with the use of a wheelchair and tonic-tonic epileptic seizures. bilateral clonic disorders, comprehension aphasia and bradypsychism. Relatives report previous migraine without chronic aura and REM (rapid eye movement sleep) sleep behavior disorder (RMSD). On neurological examination, severe ataxia with bilateral dysdiadochokinesia associated with bilateral hypometric index-index. Bilateral ROT 4+/4+ with bilateral Hoffman and Babinski signs. MMSS and MMII with FGM 4/5 proximal and distal. Case 02: woman, 36 years old, pastry chef, had episodes of forgetting about everyday activities of her work, progressive in the last three years (cake recipes, budget accounts, orders placed) associated with confusion for spatial location on the way home/ work, evolving to apraxia in writing letters and words and difficulty with calculations associated with monoparesis of the right lower limb for twelve months with progression to paresis of the lower limbs after six months and evolution to paresthesia of the upper limbs for three months. Associated with the condition, he has migraine without chronic aura and RMSD. The neurological examination showed Mini-Mental State Examination 22/30 (expected score of 29), list of animals in one minute: 09 animals; list of words starting with “F”: 03 words; clock test: 2/4; difficulty with calculations and digital agnosia with right/left apraxia; Upper limbs: eutrophic, FMG 4/5, bilateral distal; FMG 5/5 bilateral proximal; Lower limbs: eutrophic, bilateral FMG 4/5 distal and proximal with positive Mingazini; atypical gait with evidenced weakness in heel, toe and tandem gait; Bilateral dysdiadochokinesia with eumetric, slowed indexindex; ROT 4+/4+ in the right side with positive Hoffman and Babinski signs. MRI Brain (17/09/2020): extensive area of hypersignal on T2 and FLAIR (T2- weighted-Fluid-Attenuated Inversion Recovery) involving the periventricular white matter in all lobes without atrophic or expansive effect; Case 03: woman, 45 years old, started behavioral arrest epileptic seizures at the age of thirty-two, progressing to focal dysperceptive seizures with progression to bilateral tonic clonic seizures after eight months and multiple episodes of anterograde amnesia, presenting forgetfulness related to everyday work activities (exchanged worksheets , payments, calculation errors and budgets); associated with the condition presented migraine without chronic aura and RMSD. Genetic Test (04/06/2017): Heterozygous alteration in exon 8 of the NOTHC – 3 gene. Discussion: CADASIL presents a rare cause of cognitive decline and is often overlooked in diagnosis, except in cases of high clinical suspicion in a familial course. Access to imaging tests becomes fundamental for the diagnostic segment and the primordial genetic test for etiological elucidation and family planning, in the report described the family presentation with the same course of satellite symptoms (migraine without aura, and RMSD) associated with cognitive alteration with anticipation of age of onset are hallmarks of clinical thinking. Cases described in the literature show that the clinical symptomatology is not necessarily related to the level of brain injury observed in the imaging exam, which could be explained by personal factors and which exon is affected. The NOTCH-3 gene has 24 exons, in which the literature reports exon 4 as the most common mutation, followed by 3, 5 and 6, mainly in the Caucasian population. In an Asian population, the most affected exon is 4 and 11, which is also found in Italian descendants. The mutation in exon 8, described in the clinical case, is found in a population of Portuguese origin, being the second most common mutation in this nationality, behind the mutation in exon 4. Brain MRI studies have tried to elucidate the most affected brain regions, aiming to trace a line of evolution. Involvement of the temporal lobe, external capsule and corpus callosum are described as probable markers of CADASIL, and can be used as an aid in the diagnosis due to its specificity of 86% and sensitivity of 89%. Studies also show that the frontoparietal area has frequent findings of hyperintensity (100%), followed by the temporal lobe (83%), less frequently affecting the brainstem, occipital lobe and cerebellum. Despite the great advances in the specialized literature, the causes of the important cognitive dysfunction presented in the course of the evolution of CADASIL remain unclear. However, studies have suggested that the process of cognitive decline is more related to the loss of cortico-subcortical connections than to brain atrophy itself, with these disconnections resulting from repeated transient ischemic accidents. Science has been looking for ways to change the prognosis of CADASIL, recent studies in gene therapy and neurogenetics show the importance of thinking about this pathology as a genetic disease of great importance to change the prognosis of this pathology.

The attention-deficit/ hyperactivity disorder (ADHD), is a neurobiological disorder and belongs to those called neurodevelopmental disorders, since it is commonly diagnosed in childhood, and persists into adulthood. The ADHD is divided by its central symptomatology in hyperactivity, and inattention, variable according to the case. It is one of the most diagnosed disorders in the child and education psychology fields, it has been considered that every child and adolescent must have an integral treatment plan, where technology advances can be part of it, and being available at home, becoming psycho-educative, and supportive in the treatment process. Thus, in this article, a revision of current applications about ADHD is presented.

Wilt and crown rot are two important diseases of tomato caused by different strains ("formae speciales") of the fungus, Fusarium oxysporum. While both pathogens are members of the same fungal species, each differs genetically and resistance to the diseases is controlled by different genes in the plant. Additionally, the formae speciales differ in their ecology (e.g. optimal temperature of disease development) and epidemiology. Nevertheless, the distinction between these diseases based on symptoms alone may be unclear due to overlapping symptomatology. We have found in our research that the ambiguity of the pathogens is further confounded because strains causing tomato wilt or crown rot each may belong to several genetically and phylogenetically distinct lineages of F. oxysporum. Furthermore, individual lineages of the pathogen causing wilt or crown rot may themselves be very closely related. The diseases share the characteristic that the pathogen's inoculum may be aerially dispersed. This work has revealed a complex evolutionary relationship among lineages of the pathogens that makes development of molecular diagnostic methods more difficult than originally anticipated. However, the degree of diversity found in these soil-borne pathogens has allowed study of their population genetics and patterns of dispersal in agricultural settings.
