Анотація:
The aim of this study, carried out following qualitative research techniques –semi-structured interviews to dependant people, trained and un-trained caregivers, and public, private and third-sector employees in Castilla y León– is to analyse the similarities and differences that exist among the several discursive stances regarding how care services are provided to elderly dependants in terms of their place of residence. The results show that the lack or inadequacy of formal means in rural regions give place to situations that highlight the importance of caregiving by family members to dependant elderly people. Likewise, the traditional informal system of caregiving is considered to be debilitated in rural areas due to the ageing process, migration from rural to urban areas, as well as social, family and cultural transformations that have been taking place since the latter part of the 20th century.

The aim of this study was to characterize dependent elderly people and their main family caregivers and the association between functional capacity of the elderly and the burden of caregivers. This cross-sectional study was conducted with 112 elderly people and caregivers connected to a primary care service in Porto Alegre, Brazil. The applied instruments related to care and socioeconomic variables were Physical and Instrumental Activities of Daily Living (PADL and IADL) and the Burden Interview. The Spearman correlation coefficient was also used. Average age of the elderly individuals in this study was 81.41 years, while the average score for PADL was 10.36 and 6.25 for IADL. Of the 112 elderly individuals, 71.4% had severe dependence. Among caregivers, 75% were women, 61.6% were the sons or daughters with an average age of 57.98 and an average burden of 29.53, which is equivalent to moderate burden. Results revealed a significant correlation between functional capacity of the elderly and caregiver burden. It was verified that the higher the dependence of elderly people, the greater the burden of caregivers.

ABSTRACT‘Caring’ and ‘carers’ are words in frequent use in social policy, but their meaning is often vague and undefined, encompassing a wide range of activities and relationships. This paper discusses the meaning of caring and focuses particular attention on older carers. Secondary analysis of the 1985 Office of Population, Censuses and Surveys (OPCS) Informal Carers Survey data shows that of the estimated six million informal carers in Britain, the largest contribution is made by women in their early 60s, and that elderly men are more likely than younger men to be carers. The bulk of informal caring work, in terms of total time spent, is provided by co-resident carers, most of whom are a spouse or parent of the dependant. However, the largest number of those receiving help live in separate households, and these are mainly parents or parents-in-law of their carers. The emphasis on elderly people as a ‘social burden’ neglects their contribution as providers of informal care. Over a third of informal care to people over 65 is provided by elderly people. Elderly men and women provide equal amounts of co-resident care, reflecting gender equality in the care of elderly spouses. But among younger people marked gender differences are apparent in co-resident care, and in the provision of informal care to elderly people living in separate households.

This article emphasizes the importance of remittances on Moldovan income and economic growth. As a result of the increase in the flow of migrants, remittances have become an important factor in the country‘s GDP (in 2017, 20.6% of GDP). According to the Household Budget Survey the income from remittances directly affects the welfare of Moldovan households. In this study, the dependency of marginalized categories of the population (families with many children, elderly people and rural population) on the remittances of Moldovan migrants is analyzed. In rural areas, remittances account for more than 20% of the average disposable income.

In this paper, the authors propose software architecture to monitor elderly or dependent people in their own house. Many studies have been done on hardware aspects resulting in operational products, but there is a lack of adaptive algorithms to handle all the data generated by these products due to data being distributed and heterogeneous in a large scale environment. The authors propose a multi-agent classification method to collect and to aggregate data about activity, movements, and physiological information of the monitored people. Data generated at this local level are communicated and adjusted between agents to obtain a set of patterns. This data is dynamic; the system has to store the built patterns and has to create new patterns when new data is available. Therefore, the system is adaptive and can be spread on a large scale. Generated data is used at a local level, for example to raise an alert, but also to evaluate global risks. This paper presents specification choices and the massively multi-agent architecture that was developed; an example with a sample of ten dependant people gives an illustration.

AbstractObjectiveto profile the trajectory of, and risk factors for, functional decline in older patients in the 30 days following Emergency Department (ED) discharge.Methodsprospective cohort study of community-dwelling patients aged ≥65 years, discharged home from a metropolitan Melbourne ED, 31 July 2012 to 30 November 2013. The primary outcome was functional decline, comprising either increased dependency in personal activities of daily living (ADL) or in skills required for living independently instrumental ADL (IADL), deterioration in cognitive function, nursing home admission or death. Univariate analyses were used to select risk factors and logistic regression models constructed to predict functional decline.Resultsat 30 days, 34.4% experienced functional decline; with 16.7% becoming more dependent in personal ADL, 17.5% more dependant in IADL and 18.4% suffering deterioration in cognitive function. Factors independently associated with decline were functional impairment prior to the visit in personal ADL (Odds Ratio [OR] 3.21, 95% confidence interval [CI] 2.26–4.53) or in IADL (OR 6.69, 95% CI 4.31–10.38). The relative odds were less for patients with moderately impaired cognition relative to those with normal cognition (OR 0.38, 95% CI 0.19–0.75). There was a 68% decline in the relative odds of functional decline for those with any impairment in IADL who used an aid for mobility (OR 0.32, 95% CI 0.14–0.7).Conclusionolder people with pre-existing ADL impairment were at high risk of functional decline in the 30 days following ED presentation. This effect was largely mitigated for those who used a mobility aid. Early intervention with functional assessments and appropriate implementation of support services and mobility aids could reduce functional decline after discharge.

Drug induced liver injury is one of the main factor of liver failure and acute liver damage world wide with high incidence in western countries. Liver injury can be intrinsic (dose dependant) or idiosyncratic (dose independent). However idiosyncratic type is considered to be mainly responsible for drug induced liver damage. Binding of reactive metabolites of drugs to tissue proteins and oxidative stress is the possible cellular mechanism involved in this process. Moreover, some antibiotics, anti-epileptics, nonsteroidal anti-inflammatory drugs etc are more likely to induce liver damage in high risks groups that includes females, elderly and obese people. HLA halotype and variation in protein expression also plays an important role in this context. Various studies are available regarding clinical features, histopathological features, diagnosis and management related to antibiotics and acetaminophen induced liver damage. N acetylcysteine is commonly available antidote for drug induced hepatic damage. Role of other pharmacological agents as an antidote requires further studies. However, liver transplantation should be considered with drug induced lethal liver failure

Population aging is a global phenomenon which has important and far-reaching implications for many facets of human life. Although it is an achievement of public health policies and socioeconomic development; it is generally greeted with alarm as it is assumed that elderly will be ill and dependant due to age-related chronic diseases which will require long term care. Despite the fact that many older adults continue to work and contribute to the society, they are seen as a burden and a drain on resources, rather than as a resource in themselves. Ageism and discrimination towards elderly is prevalent at individual and institutional levels that affect their physical and mental health, putting them at risk of depression and social isolation. According to the World Health Organization’s (WHO) combatting ageism has great potential for achieving healthy ageing for all people as they age. Individual health and longevity is influenced by biological, environmental, and psychosocial factors, however modifiable risk factors are especially relevant as they are amenable to intervention. There is enough evidence that religiosity and spirituality (R/S) are among such factors which allow older adults to age in a more positive way. Further R/S activities are prevalent globally and elderly tend to have high rates of involvement in religious activities. Health benefits that may stem from R/S involvement could be important for the future of global health which suggests that they could be incorporated into the idea of positive ageing. Islamic perspective on old age is deeply rooted in the very texts of revelation and as such is divinely based. This review article intends to create awareness of the elderly on the ageing process, their status in the contemporary society and how by adopting positive attitude toward ageing they can enjoy better functional health, self-esteem and satisfaction in life.International Journal of Human and Health Sciences Vol. 03 No. 02 April’19. Dans un contexte de vieillissement rapide de la population, cette thèse explore les liens existants entre santé et modes de prise en charge des personnes âgées dépendantes et s'intéresse à la question du financement de la dépendance.La satisfaction des besoins d'aide des personnes âgées dépendantes constitue un objectif central de politiques publiques. Le Chapitre 1 estime l'effet de l'aide informelle (i.e. familiale) et de l'aide formelle (i.e. professionnelle) à domicile sur la santé mentale des personnes âgées dépendantes en France. Les résultats montrent que l'aide informelle réduit le risque de dépression et que l'aide formelle peut améliorer la santé mentale générale.De récentes études reconnaissent qu'aider un proche dépendant a des effets négatifs sur la santé des aidants et soulignent l'importance de les soutenir. Le Chapitre 2 s'intéresse à l'effet du soutien social sur la santé des aidants informels. Il montre que l'aide formelle et le soutien informel réduisent les problèmes de santé mentale associés à l'activité d'aide.Enfin, étant donné la pression financière et fiscale qui pèse sur les systèmes publics, le Chapitre 3 étudie dans quelle mesure les Européens seraient capables de financer leurs périodes de dépendance sur la base de leurs revenus et de leur patrimoine financier et immobilier. In the context of a rapidly aging population, this doctoral dissertation explores the relationship between health and long-term care arrangements and addresses the issue of the financing of long-term care.Meeting the needs of dependent elderly is an important objective of public policy. Chapter 1 estimates the effects of both informal (i.e. family) care and formal (i.e. professional) home care on the mental health of French dependent elderly. The results highlight that informal care decreases the risk of depression and that formal care can improve general mental health.Recent studies acknowledge that providing informal care has adverse health effects and emphasize the importance of supporting caregivers. Chapter 2 examines the effect of social support on caregivers' health. It shows that formal care and informal support limit the negative consequences of caregiving on mental health.Finally, given the increasing financial and fiscal pressure on public systems, Chapter 3 investigates to what extent Europeans elderly are able to pay for their periods of long-term care needs on the basis of their income, financial assets and home equity. It also studies the role of reverse mortgages. The simulations stress that only a small proportion of individuals would be able to finance totally their long-term care expenses and that housing assets may play an important role in long-term care financing

Cette thèse étudie comment des infirmier.es de nuit prennent la décision de recourir ou non à l'hôpital en cas d'urgence gériatrique en EHPAD. Le raisonnement clinique et la prise de décision chez des infirmiers sont rarement documentés. L'expérimentation « IDE de nuit » des projets paerpa offrait l'occasion de les étudier au prisme de l'âge et de l'incertitude dans un contexte d'urgence. Elle a aussi permis d'étudier l'épreuve organisationnelle que constitue une telle innovation. En mobilisant une approche interactionniste et sociocognitive et en nous fondant sur une enquête ethnographique trois années durant, nous avons saisi in situ le raisonnement et le jugement infirmiers à l'épreuve de l'urgence et du contexte expérimental. This thesis studies how night-shift nurses decide whether or not to resort to the hospital to deal with geriatric emergency cases in a nursing home. Clinical reasoning and decision-making among nurses are rarely documented. The “IDE de nuit” experimentation of Paerpa's projects offered the opportunity to study them through the prism of age and uncertainty in the context of an emergency. It also made it possible to study the organizational test that such an innovation constitutes. Through an interactionist and socio-cognitive approach and based on an ethnographic investigation lasting three years, we captured in situ nurses' reasoning and judgment while dealing with emergency cases in an experimental context. This research sheds light on several aspects: how care is mobilized in the service of the cure; how the emergency constitutes a social construction, which is influenced by the role played by the elderly patient and his caregivers; how the skills that drive innovation are developed and conditioned by the irreversibility of the actions and their results; how and when an organizational innovation begins and ends in an experimental context; finally, how all these elements contribute to defining the moral economy that has been made up and created around elderly people living in EHPADs, targets of this innovation.Keywords: geriatric emergency, decision-making, organizational innovation, IDE de nuit, experimentation, paerpa, le 15, SAMU, hospital, nursing assistants, EHPAD

La thèse porte sur le quotidien des personnes âgées soignées à l’hôpital gériatrique Bretonneau, établissement spécialisé de l’AP-HP situé dans le 18ème arrondissement de Paris. Elle a pour objet d’étudier certains modes de prise en charge de ces malades en perte d’autonomie dits « dépendants » ou « déments », en privilégiant les approches non médicamenteuses novatrices (art-thérapie, socio-esthétique, psychomotricité). L’enquête de terrain, effectuée entre 2008 et 2010, s’est appuyée sur la méthode du film d’exploration, faisant de la caméra le principal instrument de la recherche. Il en résulte trois films documentaires : Hôpital de Jour (117 min.), Court Séjour (67 min.) et Long Séjour (120 min.). Tout en éclairant par petites touches le fonctionnement général de l’institution, ces films décrivent la vie quotidienne de quelques malades et font des pensionnaires de Bretonneau les personnages centraux de la représentation cinématographique. Une partie écrite analyse les stratégies et les résultats de l’enquête de terrain. Elle met l’accent sur les enjeux éthiques soulevés par la présence de l’observateur-filmeur dans un milieu particulièrement sensible et auprès de sujets très vulnérables. This thesis deals with the daily life of the elderly who are patients at the Bretonneau Geriatric hospital, in Paris 18th arrondissement. It studies how these dependent or mentally-ill elders are cared for, focusing on innovative drug-free therapies (art therapy, esthetic therapy, psychomotricity).The field survey took place between 2008 and 2010 and used the method of exploratory filming, making the camera the main tool for the research. Three documentary films were made : Day Hospital (117 min), Short Stay (67 min) and Long Stay (120 min). While shedding some light on the way the institution works, these documentaries show the everyday life of a few patients and make them the main characters of the films. The written part analyses the strategy and the results of the field survey. It stresses the ethical issues raised by the presence of the filmmaker-observer in a highly sensitive environment and with very vulnerable subjects. The detailed analyses deal with the three main themes of the films : configuration of space and of material environment, physical techniques helping with loss of autonomy, interaction rituals between caregivers and patients

Анотація:
As a social worker practising in long-term residential care for people living with dementia, I am alarmed by discourses in the media and health policy that construct persons living with dementia and their health care needs as a threatening “rising tide” or crisis. I am particularly concerned about the material effects such dominant discourses, and the values they uphold, might have on the collective provision of care and support for our elderly citizens in the present neoliberal economic and political context of health care. To better understand how dominant discourses about dementia work at this time when Canada’s population is aging and the number of persons living with dementia is anticipated to increase, I have rooted my thesis in poststructural methodology. My research method is a discourse analysis, which draws on Foucault’s archaeological and genealogical concepts, to examine two contemporary health policy documents related to dementia care – one national and one provincial. I also incorporate some poetic representation – or found poetry – to write up my findings. While deconstructing and disrupting taken for granted dominant crisis discourses on dementia in health policy, my research also makes space for alternative constructions to support discursive and health policy possibilities in solidarity with persons living with dementia so that they may thrive.

AbstractOver the last decade, the range of robotic applications in the healthcare sector has expanded rapidly. These applications can range from dispensing medication to providing more personalized services to caretakers. However, this kind of robotization is associated with severe ethical and societal implications. To advance the design and acceptability of socially interactive robots it is, therefore, necessary to consider and analyze those concerns. The RoPHa research project aims at supporting care-dependent people to lead more independent life. This chapter examines potential ethical challenges and impacts in elderly care that were discussed during the design of the robotic system in the project. For evaluating the effect of assistive robotics on elderly care in practice, the MEESTAR model was applied. The ethical implications of the proposed applications were mapped to seven moral dimensions, such as autonomy, justice, privacy, etc. Each dimension were examined from three different perspectives (individual, organizational, and societal). All identified ethical implications were graded based on the degree of ethical justifiability. The results include ethically relevant questions regarding the role of the robotic system, its technical implications, economic and distrust barriers, occupational safety, data security as well as the legal and safety responsibilities of all involved parties.

The Middle Ages, despite its real existence, is often seen and used by fantasy writers as a tabula rasa to engrave their imagined stories. Its seeming state of blankness due to its distant time and lack of recorded histories allows the period to be used as a background for imaginative creations of creatures, peoples, and stories to engage with social issues pertaining to specific periods and places as well as the same issues that arise in different forms depending on the period and location. This article explores J.R.R. Tolkien's works on Middle-earth and Kazuo Ishiguro's The Buried Giant to illustrate how fantasy literature engages with the concept of (eternal-)youth and ageing. While Tolkien disrupts the idealization of (eternal-)youth through the fates of humans and imaginative races, Kazuo Ishiguro presents the value, growth and meaningfulness of life up until its very end by having readers follow the quest of an elderly couple. Both Tolkien and Ishiguro illuminate the meaningfulness and value of the lives of the aged up until the brink of death.

Abstract Intimate partner violence (IPV) is a health problem and criminal justice matter that includes elderly and disabled people, but is seldom recognized in that population. Abuse of elders and people with disabilities has the potential of presenting as many different kinds of crimes: assault, neglect, financial abuse, sexual assault, abandonment, and psychological abuse. When IPV affects elders and people with disabilities, reporting guidelines for both IPV and elder/dependent adult abuse must be followed. Dementia, depression, and chronic disabling health problems increase the risk of IPV among older people. Data substantiating incidence of IPV of older people is in its infancy, but suggests that IPV decreases with age.

Abstract Compared with Western countries, Israel is a unique feature on the map of ageing. While life expectancy in Israel matches the peak levels of Western Europe, birth rates in Israel are much higher. While Israel is considerably younger than most European countries, the elderly population itself is quite old and ageing rapidly. Between 1950 and 1985, the elderly population grew from 3 to 9 per cent of Israel's population. Most relevant to the planning of long-term care service is the growth of the dependent aged.

Based on fieldwork among the elderly in the Ik community in Northern Uganda, this chapter argues that the two apparently opposite concepts of vulnerability and human security are closely related in the sense that both phenomena fundamentally hinge on other people. Elderly Ik are often very vulnerable and dependent on family care. Cash transfers for elderly people are a mixed blessing in this overall fragile context of poverty and insecurity because the balance in intimate generational interdependence changes and this often creates friction in families. Following the cases of two elderly Ik who receive cash transfers, and the effect these have in their families, the chapter argues for relationality as a starting point for understanding vulnerability and human security.

Abstract Questions about age-related and age-dependent changes in functional abilities have been a focus of gerontological research for several decades (e.g. Katz et al., 1963; Katz, 1983; Lawton, 1972, 1990; Kane and Kane, 1981; Fillenbaum, 1985; Applegate et al., 1990). The prevalence of chronic conditions increases with advancing age. Only a minority, about 15 per cent, of 75-80-year-olds are healthy in terms of clinically diagnosed diseases (see Svanborg, 1988; Waters et al., 1989; Jylha et al., 1992). From this point of view the old saying ‘ipsa senectus morbus est ‘ suggesting that old age equals disease seems to be true. On the other hand about one-half of 75-80-year-olds regard themselves as healthy (e.g. Shanas et al., 1968; Heikkinen et al., 1983; Jylha et al., 1992). There are presumably several reasons for this discrepancy. The criteria for good health may change with age, elderly people may adapt to certain symptoms which they accept as the normal consequences of ageing, and not all diseases cause functional limitations capable of reducing an individual ‘s ability to cope with the routines of daily living. Among elderly people the level of functional abilities in particular seems to modulate their perceptions of health (e.g. Jylha et al., 1986).

It is common for elderly people with dementia to be confused about opening and closing doors. In the door opening and closing operation, various factors such as the way of door opening and closing , the shape of the handle, and the material of the door seem to affect the judgment and understanding of the opening and closing operation method. Therefore, in this study, we investigated the characteristics of door opening and closing operations of elderly people with dementia, and examine how to control the behavior of elderly people with dementia by door design and control problem behaviors in daily life.First of all, we examined the incidence of BPSD and other diseases and the measures being implemented through questionnaire surveys and on-site surveys.Next, we conducted an experiment to confirm the judgment, comprehension, and memory of elderly people with dementia, who are thought to be the cause of these occurrences, regarding the peculiar storage behaviors found in the on-site survey. The experiment was conducted on 10 elderly people with dementia at a special nursing home in Okayama Prefecture. We asked them to store daily life tools in multiple storage devices with different ways of opening and closing, and examined their opening and closing movements and memories of stored objects. It was found that the opening and closing of storage furniture doors in elderly people with dementia was influenced by long-term memory.In addition, we conducted an open/close operation experiment to investigate the current status of the opening and closing operation of the doors of elderly people with dementia. Using a miniature door samples of various ways of opening and closing, with various life-size handle We observed the opening and closing operation of the door of an elderly person with dementia, and confirmed how the judgment and understanding of the operation method change depending on the shape of the door handle and the opening and closing method. The experiment was conducted on 14 elderly people with dementia at a special nursing home in Okayama Prefecture. It was difficult to open and close a door that required unfamiliar operation such as a push plate with a single-hinged door that opened by pushing. It seems to be influenced by past experience with opening and closing.We hope that the results of these experimental studies will lead to measures to prevent problematic behaviors (troubled storage and wandering) seen in the daily lives of elderly people with dementia.

Background: Elderly people face health problems associated with reduced health and increased disability. Dependent elderly on caregivers who cause a burden affecting the quality of life of caregivers. The aim of this study was to analyze the correlation between the burden and quality of life among dependent elderly caregivers. Subjects and Method: A cross-sectional study was carried out in Pakuncen Village, Yogyakarta. The study subjects were 30 informal caregivers who cared for their family member aged ≥ 60 years with moderate dependence. Instruments used to screen dependent elderly included Activities of Daily Living (ADL) and Instrument Activities of Daily Living (IADL) to measure level of independence, MMSE and AD-8 to assess cognitive function of elderly. Caregivers aged ≥ 60 years underwent MMSE and those with scores <24 was excluded. Caregivers’ burden ware measured using The Zarit Burden Interview (ZBI) and Caregiver Reaction Assessment (CRA). Measurements of quality of life among caregivers was conducted by WHOQOL-BREF. Data were analyzed using Spearman-rank correlation. Results: Out of 30 caregivers, 24 were female and 6 were male with an average age of 49 years. Most respondents had light to moderate burden (Mean= 34.27; SD=18.94). Caregivers’ quality of life were low (<60) in psychological (Mean= 58.17; SD= 12.31), social relation (Mean= 59.77; SD= 9.04), and the environment domains (Mean= 51.90; SD= 10.00). Caregivers’ quality of life was high (≥60) in physical health domain (Mean= 63.30; SD=12.83). There were negative correlations between caregiver burden (ZBI and CRA) and the quality life of caregivers (WHOQOL-BREF). Conclusion: In Yogyakarta, the burden of caregivers (ZBI and CRA) and the quality of caregivers’ lives (WHOQOL-BREF) are negatively correlated. Over the last two decades, technology has evolved fast causing mass evolution in every aspect of life, among which health care domain and human-machine interaction. However, the benefits of the technology progress were limited for its elderly users, since the traditional human-machine interfaces were always a barrier between them and any new devices. Compared to those interfaces, multimodal interfaces offer a more natural way of interaction with the machines, coping with some inherent difficulties of elderly people. The paper presents a system dedicated to assist people with special needs, such as elderly people or people recovering from specific diseases, to perform rehabilitation exercises by using a multimodal interface. The system presents to the user a set of required exercises prescribed by the doctor or by the caregiver, and tracks the user exercise evolution, informs the user about his/her performances and mistakes, changes exercise levels depending on the users' physical condition and results. Furthermore, the system gives advice to its user to optimize the results of the exercise. To be easily accessible by elderly people, the interface is a key-factor of the system. The supervision of exercises is achieved by means of a Kinect camera, successive images of user's exercises being analyzed automatically. The feed-back to the user is given either by messages displayed on the screen or through voice output. The user is represented on the screen by an avatar, which is mirroring the user's movements during the exercise. The user can interact with the system by giving simple voice commands or by using the touch facilities of the screen. In this way, it allows the user to interact with the system without interrupting his exercise and in the most natural possible way. The interface can be adapted to user preferences by configuring the type and color of fonts, the arrangement of icons on the screen and the aspect of the display. After repeated interactions with the system, the icons corresponding to the most used commands given, either by touch or by voice, will become bigger on the screen or better placed. We claim that our proposed approach will help the elderly to perform their rehabilitation exercise in their preferred medium with a grade of independence and to improve the efficiency of the exercise and well-being.

The paper presents some results of the analysis of the Life Cycle Deficit (LCD) of the population aged 60 and over, based on the data frame of the National Transfer Accounts (NTA) for 2019. Based on age profiles of aggregate data is attested that the LCD of the elderly population in Moldova is about 41% of the LCD of all ages. The primary sources of livelihood for the elderly are not so much income from work but public transfers (payments for social programs, including pensions, healthcare and social services) – 37.8%. Redistribution of private asset-based reallocations plays a fairly significant role in financing the consumption of older people (30.7% of consumption). Along with advancing age, the economic activity of the population inevitably decreases. During the pre-retirement period, the self-employment labor income remains to have a special role in supplementing incomes and becoming the only source after age 75. People aged 60-75 continue to be donors in intergenerational transfers. Only after the age of 75+, do the elderly become recipients of private transfers. A gender characteristic is that older women remain the main beneficiaries of private transfers due to higher life expectancy. The aging of the population will accelerate in the coming decades, according to forecasts. Therefore, the share of the economically dependent population and the LCD will increase. More vigorous policy efforts are required to improve productivity and increase the potential of the working-age population, including by ensuring active and healthy aging and social security for the elderly. ABSTRACT Background: Hypertension is significantly associated with increased morbidity and mortality of cerebrovascular diseases, myocardial infarction, congestive heart failure and renal insufficiency. Hypertension is a major public health problem and an important research area due to its high prevalence and a major risk factor for cardiovascular disease and other complications. This study aimed to determine the effect of foot reflexology therapy on lowering blood pressure in elderly people suffering from hypertension in the working area of Sekupang community health center, Batam City. Subjects and Method: This study was an experiment with a pretest – posttest control group design. A sample of 15 elderlies was selected by simple random probability sampling. The dependent variable was elderly with hypertension. The independent variable was foot reflexology therapy. The data obtained from this study were the values of pre and post therapy blood pressure between the control group and the treatment group. The data was analyzed by Wilcoxon test. Results: After the intervention of foot reflexology was carried out, foot reflexology affected reducing headache intensity (Mean= 2; SD= 0.52) and it was statistically significant (p= 0.002). When compared to the control group that was not given therapy, indicating that pain intensity tended to increase with statistical results (Mean = 2.33; SD= 0.69) obtained (Z score = -2.64) with p = 0.008. Conclusion: Reducing the intensity of headaches and able to lower blood pressure in older people with hypertension. Background: Several studies demonstrate that healthy elderly people present impairments in different executive functions (for example, inhibition, updating and alternation). However, these works use tasks that measure reaction time as a dependent variable, and it is already known that processing speed decreases with age. Objective: As a consequence of that, this study aimed to test a battery of representative executive tests. This freely accessible battery includes 2 tests for each executive domain (inhibition, updating and alternation), controls the effects of processing speed, as the participants themselves regulate the time of stimulus presentation (paradigm - self-paced) and all responses were given verbally (thus controlling the effect of psychomotor speed). Methods: For this pilot study, 13 healthy elderly females (M=68.23, SD=6.13) were evaluated, each one performed a total of 6 executive tests. For the inferential statistical analysis, the t test of repeated measures with a bootstrap of 5000 resamplings was used. Results: As a result, we observed that in the executive blocks, participants obtained fewer correct answers per unit of time than in the control blocks, demonstrating that the executive block is in fact evaluating an executive function regardless of the processing speed. Conclusion: As a pilot study, this battery proved to be effective and easy to apply in elderly population.

ABSTRACT Background: Dementia is a collective term for several diseases that affect memory, other cognitive abilities, and behavior. These diseases can seriously interfere with people’s ability of daily living. This is not a normal phenomenon of aging. With the increasing prevalence of dementia in the elderly, the importance of dementia as a comorbidity of hypertension is increasing. However, several questions regarding the link between hypertension and dementia remain unresolved. This study aimed to determine Orientation-Memory-Concentration-Test (OMCT) can be used to assess cognitive function in the elderly as an early step in the early detection of dementia. Subjects and Method: This was a comparative study using a case-control design. The study was conducted in Jetis Sub-District, Yogyakarta from September 2019 to June 2020. A total sample of 110 of elderly was divided into two groups 42 elderly (case), and 68 elderly (control) selected by consecutive sampling using the Mini-Mental State Examination (MMSE), Clock-Drawing Test (CDT) and Orientation-Memory-Concentration Test (OMCT). The dependent variables were the sensitivity and specificity level of the OMCT instrument while the scores of the MMSE. The independent variable was CDT instruments. The collected data will then be processed by diagnostic analysis followed by analysis of ROC and Youden’s index to determine the optimal cut off. Respondents are categorized as having impaired cognitive function if the MMSE (cut off ≤ 24) or CDT (cut off <18) shows a positive result. Results: 110 elderlies were involved. A total of 42 elderlies were included in the case population, and 68 elderlies were included in the control population. In the total OMCT population (cut off> 11) it has a sensitivity (29%) and specificity (97%) to the combination of MMSE and CDT. In populations with hypertension OMCT (cut off> 2.5) has sensitivity (68%) with specificity (46%). In a population without hypertension OMCT (cut off> 7) has a sensitivity (55%) and specificity (90%) to the combination of MMSE and CDT. Conclusion: OMCT can be used as a screening tool for cognitive dysfunction in older people with hypertension because of its short duration, ease of use, and can be used in patients with visual impairments. With the aging of the population, the number of people aged 65 and over living alone is increasing every year. Accompanying this trend, the number of deaths due to falls is also on the rise. One of the characteristics of the walking style of people who fall down is that they walk with a frosted gait. The ultimate goal of this research is to construct a system that can issue a warning when it is determined from footsteps that a person is walking with a slipshod gait. The proposed method uses three types of audio data: normal footsteps, footsteps caused by a mildly slip walking, and footsteps caused by a slip walking. Two-step silence intervals and 1-12 dimensional MFCC were used as features, and P-values between footsteps were obtained for the silence intervals and MFCC of the three types of footsteps by Wilcoxon's signed-rank test. The results showed significant differences. Although significant differences were obtained when the silent interval was used as a feature, there is a possibility that footsteps are misrecognized as normal sounds because the silent interval varies depending on the speech data, even for the same walking style. On the other hand, with MFCC, significant differences were obtained, especially for the 6th, 7th, and 8th order coefficients, and the variation was small. Therefore, we decided to use MFCC in our identification experiments. As a discrimination model, we used Random Forest (RF), a type of machine learning, for three classifications and obtained a 91% correct response rate after evaluation by 10-fold cross-validation. The reason for the high correct response rate can be attributed to the fact that the footsteps were recorded at a stable volume in a quiet environment. It could also be attributed to the large differences among the three types of walking styles.

Demographic developments are not only increasing the proportion of older people in society, but also the number of people with physical and/or mental impairments becoming dependent on support, for example in nursing homes. Dementia is one of the most common and consequential psychiatric diseases among elderly. The progressive decline in cognitive and motor skills significantly impairs the quality of life of dementia patients. This also poses a great challenge for caregivers, especially in terms of communication and social interaction. Within the framework of psychosocial interventions, they try to maintain the cognitive performance and improve the emotional well-being of those affected. Music and reminiscence therapy have proven to be effective approaches in this context. While digital media are hardly used in practice so far, research already shows approaches how corresponding therapies can be supported by their use. This paper shows how tangible user interfaces (TUI) can be designed and used not only to support care work, but also to promote active participation and self-determined use by dementia patients. A first prototype is presented and shows the potentials of use and for future research.

What experiences of transitional hospital-home care were successful in supporting home caregivers of elderly people with functional dependence? The objective of this scoping review is to identify the hospital-home transitional care offered as support to the home caregiver of elderly people with functional dependence. Information sources: Medline/Pubmed, CINAHL, Scopus, Web of Science, LILACS and Embase will be used. We identify and study analytically three key channels that shape how inflation affects wealth inequality: (i) the traditional wealth (or Fisher) channel through which inflation redistributes from lenders to borrowers; (ii) an income channel through which inflation reduces the real value of sticky wages and benefits; and (iii) a relative consumption channel through which heterogeneous increases in the prices of different goods affect people differently depending on their consumption baskets. We then quantify these channels during the 2021 inflation surge in Spain using detailed, high-frequency customer-level data from one of the main commercial banks. The unexpected nature of the inflation shock and its perception as temporary in this period in particular closely fit the assumptions behind our theoretical decomposition. Results show that the wealth and income channels are an order of magnitude larger than the consumption channel. Middle-aged individuals were, in net terms, largely unaffected by inflation, while the elderly suffered the most. We find similar results when using representative surveys on households’ wealth, income, and consumption.
