The purpose of this study was to compare nurses' knowledge of diabetes and nursing interventions in a home health care agency (HHCA) that employed a diabetes nurse educator (DNE) and an agency that did not employ a DNE. Nurse knowledge of diabetes mellitus was measured using the Ditibetes: Basic Knowledge Test (DBKT). Nursing care interventions for patients with diabetes were measured using a researcher-developed tool, the Nursing Care Intervention Tool (NCIT). The data analysis showed that the nurses in the agency with the diabetes nurse educator were significantly more knowledgeable about diabetes. The data also showed that the nurses in the HHCA with a diabetes nurse educator provided a significantly higher standard of patient care than the nurses in the agency without a diabetes nurse educator.

This article is an exploratory study of perceptions in mental health nurses who are qualified to prescribe yet choose not to do so. In-depth semi-structured face-to-face interviews, field notes and analysis of documents were used to investigate the perceptions of the non–prescribing nurse prescriber. A mapping exercise was conducted to identify potential participants. Interview data analysis was based on the principles of descriptive phenomenology and the research was theoretically framed within concepts of power, structure/agency and culture. This study has contributed to understanding the views of non-prescribing mental health nurse prescribers on why they do not use their prescribing qualification. The findings from this study suggest that there are complex, interlocking factors: power and knowledge; culture; and structure and agency, which may enable or prevent mental health nurse prescribers from independently prescribing.

In 2011, a structured electronic system for the staffing of agency nurses was implemented in the Western Cape as part of the Nursing Information Management System (NIMS). This electronic system was developed to ensure a fair tendering process for the procurement of agency staff, providing information on agency expenditure, maximum wage rates and adherence to principles and rules of supply chain management. Although technology is used more and more in healthcare services within the Western Cape, some professional nurses could lack the skills needed to implement the electronic system appropriately. A descriptive and quantitative design was followed to determine the competence levels of professional nurses in utilising the electronic staffing system for agency nurses in public hospitals in the Metropole, Western Cape. The aim of the study was to develop guidelines for nurse managers according to which they can train professional nurses in hospitals to utilise the electronic system for staffing of agency nurses. Assumptions were adapted from the theoretical framework of Benner (1984) on different levels of competencies and used as theoretical departure of the study. The accessible population for this study was all the professional nurses in hospitals within the Metropole of the Western Cape Government Health registered as users on the database, called the Nursing Information Management System (NIMS) (N=278). This population served as the total inclusive sample for the study. The method of data collection was a self-administered structured questionnaire. The data was analysed by using the SPSS Version 24 software program and descriptive and inferential statistics were conducted.

Previous research has suggested that critical care nurses' close and prolonged contact with patients places them in a unique position of improving patient outcomes and reducing the demand on critical care beds by detecting subtle signs of deterioration in a timely manner and identifying patient progression and readiness for less intensive care. Nonetheless, nurses rarely take decisions about patients' treatment modalities on their own, and constantly need to seek advice or authorisation. Indeed, several studies suggested that one of the most frequent decisions taken by nurses during clinical practice is that of referring to medical, nursing or other health care practitioners. However, there is very Iim ited research around the factors related to, and the actual process of, such referring. In view of this gap in the literature, the purpose of this study was to explore and explain the process of referring in the context of an intensive care unit by means of a substantive theory derived from the data. The principal research question was: What are the factors associated with critical care nurses' decisions to seek help from medical practitioners? The study took place in a general intensive care unit in a state hospital in a Mediterranean island nation. Data generation and analysis took place concurrently and iteratively, and were guided by the dimensional analysis approach to grounded theory. Data collection involved : (1) twenty hours of preliminary non participant observation; (2) fifty hours of participant observation and informal interviewing; (3) fifteen hours of formal semi-structured interviews with ten critical care nurses working in this unit selected by means of purposive and theoretical sampling; (4) two 2-hour focus group sessions aimed at enhancing theoretical sufficiency and verification of the emerging substantive theory. The analytical process was characterised by a series of inductive-deductive cycles, during which increasingly conceptual labels were attached to data segments. Working hypotheses and theoretical memos were used to interrogate the data and look for positive and negative evidence for the inductively derived labels. The findings suggest that nurses' decisions to seek help from doctors are complex and frequently mediated by individual or organisational factors which are unrelated to the actual clinical situation, such as experience, asse11iveness and willingness to take risk. They involve nurses weighing up several occasionally conflicting motivators, including a desire to act with some degree of independence; attempting to prevent personal and professional risk; and being constantly mindful of their asymmetrical decision making power relative to doctors. A central consideration is that of balancing their moral obligation to safeguard critically ill patients' interests with their duty to respect medical practitioners' preferences. Subsequently, nurses find themselves in a position of dual agency as they need to concurrently act as an agent to two different principals, namely the medical practitioner and the patient, a situation which is potentially morally distressing. The emergent substantive theory underscores the significance of the nurse's role in acting on behalf of the patient in a scenario of significant patient vulnerability; the factors that enhance, mediate and potentially suppress this advocacy role; and the interrelatedness of nurses' relationship with doctors and their effectiveness in safeguarding critically ill patients' interests. Subsequently, the study should provide valuable insight into the type of leadership and education that is required to assist nurses in placing patients' interest at the forefront of their actions and interactions, while fostering collaboration within multidisciplinary teams.

This study describes nurses' ethical perspectives on nursing dying patients in a critical care setting, as well as nurses' responses to their perspectives. The design involved a phenomenological approach, with unstructured interviews with eight critical care nurses used to generate data. The results indicated that nurses' ethical perspectives centered around a theme of senselessness; a senseless decision-making process, the experiences of patients and family members as senseless, and nurses' activities as senseless. Senselessness illustrated the multiple ethical dilemmas inherent in nurses' experiences. Nurses' ethical perspectives also involved their attempts to cope with senselessness by finding new meanings through shifting focus to patient comfort, support of the family, and to nurses' personal philosophies. The situational context of nurses' perspectives was explored in terms of influences on their perspectives. This study supports other recent nursing research identifying prolongation of the process of dying as a significant ethical problem engendering moral distress.

ENGLISH ABSTRACT: Agency-nursing work refers to nursing services provided by agency nurses employed on a casual, contracted basis. Agency-nursing has become more common in nursing practice in South Africa. This group of nurses play a significant role within the health care industry for both public and private sector. Given the pivotal role that nurses play in caring for the health of others, it is important to understand what motivates agency nurses in terms of job satisfaction. The aim of the study was to explore the perceptions of agency nurses regarding job satisfaction within critical care units in private health care institutions in the Western Cape. A qualitative research design with a phenomenological approach was applied. A sample size of n=10 was drawn from a total population of N=553, using random sampling technique. A semi-structured interview guide was designed based on the objectives of the study and validated by experts in the field before data collection. Ethical approval for the study was obtained from the Ethics Committee at the Faculty of Health Sciences, University of Stellenbosch. Permission to conduct the research was obtained from the participating nursing agency and consent was given in writing. The presentation of the results was categorised into themes and sub-themes emerged from the data analysis. The five clusters of themes that emerged were reasons for doing agency work, experiences with hospital staff, allocation of agency nurses, communication of information and staff development. The findings support Herzberg’s theory on motivation factors in terms of what motivates workers to excel in their performance. Agency nurses experienced both positive and negative aspects with regards to job satisfaction. The results of the study suggest the need to include revision and implementation of human resource policies and practices to include the nurses’ “voice” in an effort to provide a more supportive work environment. AFRIKAANSE OPSOMMING: Agentskapverplegingswerk verwys na verplegingsdienste wat voorsien word deur agentskapverpleegsters wat op ’n tydelike, gekontrakteerde basis in diens geneem word. Agentskapverpleging het meer algemene gebruik in die verpleegpraktyk in Suid-Afrika geword. Hierdie groep verpleegsters speel ’n belangrike rol in die gesondheidsbedryf vir beide die openbare en die private sektor. Gegee die deurslaggewende rol wat verpleegsters speel in die gesondheidsversorging van ander mense, is dit belangrik om te verstaan wat agentskapverpleegsters motiveer vanuit die oogpunt van werksbevrediging. Die doel van die studie was om die persepsies en ervarings van agentskapverpleegsters ten opsigte van werksbevrediging te ondersoek binne die waakeenhede in private gesondheidsorg-inrigtings in die Wes-Kaap. ’n Kwalitatiewe navorsingsontwerp met ’n fenomenologiese benadering is toegepas. ’n Monstergrootte van n=10 is geneem uit ’n populasie van N= 553 deur die ewekansige steekproeftegniek te gebruik. ’n Semi-gestruktureerde onderhoudgids was ontwerp en gebaseer op die doelwitte van die studie en was deur deskundiges op hierdie gebied, voor die insameling van data, gelding verklaar. Etiese goedkeuring vir die studie was verkry van die Etiese Komitee van die Fakulteit van Gesondheidswetenskappe, Stellenbosch Universiteit. Goedkeuring om die navorsing te doen, was verkry van die deelnemende agentskap en toestemming was op skrif geplaas. Die voorgelegde resultate was gekategoriseer in temas en subtemas wat uit die data-analise voortgespruit het. Die vyf groepe van temas wat hieruit gespruit het, is redes vir die keuse om agentskapwerk te doen, ervarings met hospitaalpersoneel, die toewysing van agentskapverpleegsters, kommunikasie van inligting en personeelontwikkeling. Die bevindinge het onthul dat Herzberg se teorie oor motiveringsfaktore werkers aangemoedig het om te presteer in die uitvoering van hulle pligte. Werksbevrediging is volgehou deur die agentskapverpleegster. Die bevindings dui op die behoefte om die hersiening en implementering van menslike hulpbronbeleidsverklarings en -praktyke in te sluit sodat die verpleegster se stem gehoor kan word in ’n poging om ’n meer ondersteunende werkomgewing te voorsien. The general Australian workforce is becoming increasingly reliant on agency staff due to the diminishing of full-time jobs and the rise in part-time jobs since the recession of 1991-1992. This pattern is also seen in Western countries such as America and the United Kingdom (Peerson et al., 2002:505). The excessive costs of health service adopting a 100% staffing model has created the use of agency nursing to help meet fluctuating and unpredictable health care demands in Australia and overseas; agency staff are used to meet the shortfalls in staff-patient ratios (Peerson et al., 2002:505). Hurst & Smith (2011:287) agree that agency nurse staffing form a large part of the health service in the UK and state that this was unlikely to change because of recruitment and retention problems, high absenteeism, and staff wanting to work casually. In their study, they identified three types of agency nurses: A bank nurse – the hospital’s own employees or those supplied by NHS Professionals, who work when required, usually at short notice; agency nurses provided by commercial organisations, who are equally flexible but less familiar with the ward patients and procedures; and permanent ward staff working paid overtime. In South Africa, this also remains a problem in public and private institutions. The World Health Organization (WHO) states that the minimum ratio for nurses to population is 200:100 000 or 500 people per nurse. According to South African Nursing Council statistics of nursing manpower as at the end of 2008, South Africa has 437 nurses for every 100 000 people. This assumed that all nurses registered with SANC are working in South Africa at the time. Therefore, if the assumed number of staff not working in South Africa is subtracted, the ratio can increase to between 600 and 678 people per nurse (WHO, 2006). Joubert (2009:2) shows that a shortage of nurses contributes to deaths in hospitals in South Africa that would otherwise have been avoidable. The use of agency staff is becoming an appropriate means of providing cost-effectiveness and flexibility to staffing needs.

AbstractA change of government in 2010 brought fresh NHS reforms and a new Health and Social Care Act (HSCA, 2012). Both, along with the 2014, Five Year Forward View (NHSE) set the tone for this Chapter. We discuss how the continued emphasis on competition between providers, and the introduction of Clinical Commissioning Groups (CCGs) which replaced Primary Care Trusts (PCTs) as commissioners of community services, impacted on community nursing service management and delivery. Policy shifted in favour of a more co-operative approach to service provision and familiar agendas were set out for keeping people out of hospital with reform based around integration between health care sectors and between health and social care services. There was little change on the ground for district nurses in this era despite increasing emphasis on integrated care, collaborative, cross-sector working (i.e. with LA social care) and multi-disciplinary team management of complex patients. The HSCA 2012 began to unravel almost as soon as it was enacted, with the emphasis on competition undermined by the Five Year Forward View shift towards integration between sectors as a dominant organising principle. Community Health Services (CHS) were, to some extent, protected from the fragmentation associated with the Act, and in terms of district nursing practice, this era generated little change with patterns of service provision remaining very much as they were following the upheaval generated by the Transforming Community Services agenda.

Quality care and improving health outcomes are cornerstones of healthcare provision, yet quality improvement (QI) preparation and assessment in health-professional education has been found to inadequately prepare graduates for their future roles (Robb et al., 2017). Toi Ohomai Institute of Technology’s Bachelor of Nursing (BN) year three teaching team looked to address this by reviewing and improving QI teaching and assessment modules within a course in the programme. Alongside this redevelopment, research with students was undertaken to investigate the efficacy and outcomes of this work. This paper presents findings from reviews of QI assessment projects completed by BN students (n = 93), with particular attention to identifying student experiences and their approach to this project using a detailed content analysis (Krippendorff, 1989). Particular attention was paid to highlighting a shift in thinking from quality assurance (QA) approaches that prevailed in the prior teaching of this module, to QI. We found 41% of students selected and undertook projects that reflected QI concepts focused on improved patient outcomes, and 59% of students selected and undertook projects that were concerned with standards, auditing and compliance improvement, more in keeping with QA. In addition, seven student QI projects addressed enhancing te ao Māori (Māori worldview), including language activities through music and exercise, bilingual labelling, and culturally safe care for Māori residents. Key findings address the ongoing challenges of embedding QI concepts and engagement in practice and professional development needs; and policy, practice and procedural improvements and the need for more time to enact and evaluate QI projects. Recommendations from this study are: (1) enhancing te ao Māori and Te Tiriti o Waitangi responsiveness throughout the BN curriculum; (2) ongoing preparation for student-nurse educators to ensure they are confident to support student-led QI initiatives; (3) further shared professional development with agency staff prior to practice placements; and (4) replication of this research to identify longitudinal outcomes. This research reinforces the importance of education–practice partnerships to enhance effective QI education for preparing graduates to transition to their new roles in the workplace.

Abstract This study presents a game developed through the Scratch programming language, making it possible to be an educational support tool for the teacher. In an increasingly technological world, it is natural that this resource can also be inserted in schools, however, teachers need pedagogical tools to help them in the classroom. Thus, the objective of this work was to develop, using Scratch, a quiz that could facilitate the study of nuclear energy production. For this, the work was developed in three moments: 1) a bibliographic survey was carried out to evaluate how Scratch is used in physics teaching; 2) preparation of a script containing which aspects would be addressed and the story behind the game; 3) a question and answer game was programmed using Scratch. The contribution of this work will support teachers during the preparation of classes for basic education. It is also expected to contribute to the awareness of nurses about the risks of the disease and how it can be prevented since they are multiplier agents of the education and health of the population.

Abstract: Introduction. In the course of their work, medical workers are exposed to a biological factor, including bacterial, viral nature. Medical personnel come into contact with patients with measles, rubella, diphtheria, tuberculosis, hepatitis, coronavirus infection and other infectious diseases. The aim of the study is to assess the humoral immunity by the presence antibodies to the measles, rubella, hepatitis B viruses, to the causative agent COVID-19, tuberculosis and diphtheria bacteria in health care workers. Methods. Antibodies to measles, rubella, hepatitis B viruses, diphtheria and tetanus pathogens were measured in blood serum samples of 1221 MW; total antibodies to mycobacterium tuberculosis - in 120 MW; antibodies to the nucleocapsid protein of the SARS-CoV-2 virus – in 301 MW. The study was carried out by the method of enzyme immunoassay using commercial test systems; antibodies to diphtheria toxoid were detected in the passive hemagglutination reaction. The control group consisted of persons of engineering and technical personnel, comparable in age, gender and work experience. Results. Medical personnel were found to have significantly more frequent detection of seronegative reactions to the presence of antibodies to the hepatitis B virus (40.9% and 13.5%, p<0.001) of measles (28.8% and 3.9%, p<0.05); significantly high prevalence in the presence of total antibodies to mycobacterium tuberculosis (7.5% of cases in medical, 0% of cases of workers in the control group, p<0.05). In comparison with doctors, nurses had a significantly higher prevalence of antibodies to the nucleocapsid of the SARS-CoV-2 virus (38.9% and 23.7%, p<0.05). Conclusions. The study of post-vaccination immunity in medical workers showed the presence of a high proportion of seronegative individuals among vaccinated (viral hepatitis B, measles) medical workers and, accordingly, significant biological risks. A higher seroprevalence in total antibodies to Mycobacterium tuberculosis may also indicate insufficient immune protection among MW. The biological significance of seroprevalence to SARS-CoV-2 virus proteins (for nurses) requires further study.

This brief summarises key considerations about the social, political and economic context shaping the outbreak of Ebola in the N’Zérékoré prefecture, Guinea, as of March 2021. The outbreak was declared on 14 February 2021, two weeks after the death of the first known case, a health agent (Agent Technique de Santé) from Gouécké. Gouécké is located 40km north of N’Zérékoré via the paved Route Nationale 2. The nurse sought care at a health centre in Gouécké, a clinic and then a traditional healer in N’Zérékoré. She died in N’Zérékoré on 28 January. When they became sick, the relatives of the first known case referred themselves to N’Zérékoré regional hospital, where the disease was transmitted to healthcare workers. Although the potential for transmission in rural areas of the Gouécké subprefecture was high, to date, most cases have been reported in the urban setting of N’Zérékoré, which is the focus of this brief. At the time of writing (22 March), the total number of cases was 18 (14 confirmed, four probable), with nine deaths and six recoveries. The last new case was reported on 4 March.

Helytoky is a type of parthenogenesis whereby females produce only female offspring without the involvement of males, even where males are occasionally produced. In the last few years, strong circumstantial evidence has implied that thelytoky can be caused by micro-organisms called Wolbachia in at least some species of wasps. The thelytoky can be "cured" by treatment with antibiotics. Further Wolbachia-like organisms can be found in microscopic examinations and genetically identified through their DNA. The aphelinid wasps, and especially species in the genus Aphytis, are among the most important of all classical biological control agents. Aphytis species are critical in the biological control of scale insect pests in commercial orchards and ornamental plantings. About 30% of Aphytis species are thelytikous, of which we were able to study three in detail. In all three, thelytoky was curable by treatment with antibiotics and Wolbachia were identified morphologically and through their DNA. In contrast, Wolbachia were not detectable in biparental species of Aphytis. Studies of Wolbachia gene sequences obtained from Aphytis showed that they were most closely related to those from a very distantly related wasp, Muscidifurax uniraptor, strongly implying that the Wolbachia can be horizontally transferred. As revealed by electron microscopy, the Wolbachia show a strong association with the nurse and follicle cells of the female wasps.

This report provides findings of a rapid appraisal of the Auxiliary Nurse Midwife (ANM) training program in Uttar Pradesh, India. The appraisal was undertaken by the Population Council and State Innovations in Family Planning Agency (SIFPSA). As SIFPSA is in the process of planning a training program to strengthen the technical skills and competence of ANMs and update their contraceptive knowledge, it was felt that a rapid appraisal of the training program would help in understanding the strengths and weaknesses of previous training efforts. The ANM training initiatives of SIFPSA would then build upon the experiences gained from earlier training programs and the existing training needs of ANMs. The emphasis was on improving ANMs' technical and counseling skills by providing on-the-job practical training. The program was a three-week skill-based training, during which ANMs were attached to a CHC/PPC or a district hospital. This was followed by one week of training at an ANM Training Centre in IEC and counseling. The objective was to review how the training program was organized and how it helped improve the technical skills and competence of ANMs.

In September 1999, the Zimbabwe National Family Planning Council (ZNFPC) and the Population Council initiated a one-year study to assess the performance of ZNFPC’s community-based distribution (CBD) program. A continual decline in the program’s productivity, combined with the expanding HIV/AIDS epidemic, indicated a need to reconsider ZNFPC’s objectives and revise the roles and responsibilities of its full-time, salaried CBD agents. The study examined the productivity, costs, and potential sustainability of the CBD program. Researchers gathered information by reviewing program documents through 1999. They also interviewed program managers, district and community nurses, and community leaders and CBD agents from eight regions. They assessed community views through focus group discussions with female CBD clients, male and female users and nonusers of family planning, and youth. As noted in this brief, the study revealed a need to reorient the program to fit the country’s changing reproductive health needs—specifically the country’s growing HIV/AIDS crisis. The organization incorporated the study findings in a pilot program to improve organizational efficiency while focusing agents’ efforts on the HIV/AIDS crisis.

The various initiatives to eradicate FGC in Mali over the past two decades have had little impact on this traditional practice. This study assessed the use of health personnel to combat FGC, as recommended by the World Health Organization. Conducted in 1998 by the Association de Soutien au Developpement des Activites de Population, a nongovernmental organization, and the Ministry of Health, the study covered 14 urban and rural health centers in Bamako and Segou. In the eight health centers that served as experimental sites, 59 health providers, including physicians, midwives, nurses, and aides, attended a three-day training course on identifying and treating medical complications related to FGC and counseling clients about FGC. In the six centers that served as control sites, 48 providers were interviewed. As noted in this brief, health providers are an important potential resource in campaigns to eradicate female genital mutilation, but a concerted effort is needed to ensure that they can become effective behavior change agents. After a three-day training course, providers’ knowledge about FGC increased, but few of them counseled their clients about FGC.
