Objective To study the epidemiology and outcomes of delirium among hospitalized patients in Zambia. Methods We conducted a prospective cohort study at the University Teaching Hospital in Lusaka, Zambia, from October 2017 to April 2018. The primary exposure was delirium duration over the initial 3 days of hospitalization, assessed daily using the Brief Confusion Assessment Method. The primary outcome was 6-month mortality. Secondary outcomes included 6-month disability, evaluated using the World Health Organization Disability Assessment Schedule 2.0. Findings 711 adults were included (median age, 39 years; 461 men; 459 medical, 252 surgical; 323 with HIV). Delirium prevalence was 48.5% (95% CI, 44.8%-52.3%). 6-month mortality was higher for delirious participants (44.6% [39.3%-50.1%]) versus non-delirious participants (20.0% [15.4%-25.2%]; P < .001). After adjusting for covariates, delirium duration independently predicted 6-month mortality and disability with a significant dose-response association between number of days with delirium and odds of worse clinical outcome. Compared to no delirium, presence of 1, 2 or 3 days of delirium resulted in odds ratios for 6-month mortality of 1.43 (95% CI, 0.73–2.80), 2.20 (1.07–4.51), and 3.92 (2.24–6.87), respectively (P < .001). Odds of 6-month disability were 1.20 (0.70–2.05), 1.73 (0.95–3.17), and 2.80 (1.78–4.43), respectively (P < .001). Conclusion Among hospitalized medical and surgical patients in Zambia, delirium prevalence was high and delirium duration independently predicted mortality and disability at 6 months. This work lays the foundation for prevention, detection, and management of delirium in low-income countries. Long-term follow up of outcomes of critical illness in resource-limited settings appears feasible using the WHO Disability Assessment Schedule.

The aim of this work was to assess the diversity of Hymenoptera in an orange orchard / Brazilian savannah fragment interface in Descalvado, State of São Paulo, Brazil, using Moericke, Malaise and pitfall traps. The sampling was carried out from February to June 2006, when 5,148 specimens of Hymenoptera, from 12 superfamilies and 36 families, were caught: Chalcidoidea (1,885 specimens; 36.6% out of the total; 14 families), Ichneumonoidea (715; 13.9%; 2), Vespoidea (554; 10.8%; 5), Apoidea (444; 8.6%; 2), Diaprioidea (430; 8.4%; 2), Chrysidoidea (366; 7.1%; 3), Platygastroidea (340; 6.6%; 2), Ceraphronoidea (211; 4.1%; 2), Cynipoidea (107; 2.1%; 1), Evanioidea (83; 1.6%; 1), Tenthredinoidea (7; 0.1%; 1) and Trigonaloidea (6; 0.1%; 1). The Moericke and Malaise traps installed in the Brazilian savannah fragment were responsible for capturing the highest number of Hymenoptera (2,158 specimens; 41.9% out of the total collected and 1,739; 33.8%, respectively), followed by the Moericke traps placed in the orange orchard (1,123; 21.8%) and by the pitfall traps (128; 2.5%). The results indicate that the diversity of families of Hymenoptera in the Brazilian savannah fragment is greater than in the culture of citrus and expresses the greatest diversity of hosts existing in that environment. The study made clear that the combined use of different kinds of traps allows the capture of a large diversity of Hymenoptera. O objetivo deste estudo foi avaliar a diversidade de himenópteros de uma interface cultura de citros/fragmento de Cerrado em Descalvado, SP, Brasil, através do uso de armadilhas de Moericke, de Malaise e de pitfall. As amostragens ocorreram entre fevereiro e junho de 2006, quando foram obtidos 5.148 exemplares de himenópteros pertencentes a 12 superfamílias e 36 famílias: Chalcidoidea (1.885 exemplares; 36,6% do total; 14 famílias), Ichneumonoidea (715; 13,9%; 2), Vespoidea (554; 10,8%; 5), Apoidea (444; 8,6%; 2), Diaprioidea (430; 8,4%; 2), Chrysidoidea (366; 7,1%; 3), Platygastroidea (340; 6,6%; 2), Ceraphronoidea (211; 4,1%; 2), Cynipoidea (107; 2,1%; 1), Evanioidea (83; 1,6%; 1), Tenthredinoidea (7; 0,1%; 1) e Trigonaloidea (6; 0,1%; 1). As armadilhas de Moericke e de Malaise instaladas no fragmento de Cerrado capturaram o maior número de exemplares de himenópteros (2.158 exemplares; 41,9% do total coletado e 1.739; 33,8%, respectivamente), seguidas pelas de Moericke instaladas na cultura de citros (1.123; 21,8%) e pitfall (128; 2,5%). Os resultados obtidos indicam que a diversidade de famílias de himenópteros no fragmento de Cerrado é maior do que na cultura de citros e expressa a maior diversidade de hospedeiros existente naquele ambiente. O uso combinado de diferentes tipos de armadilhas possibilitou a captura de maior diversidade de grupos de himenópteros.

Remote sensing technology is one of the best methods for the large-scale monitoring of chlorophyll in crops. This study analyzes the feasibility of estimating the contents of chlorophyll by means of narrow band normalized difference vegetation indices (NDVInb). The reflectance of the two bands forming the NDVInbis from simulations run on the PROSPECT model. A traversal of possible combinations of NDVInbare examined from 400 nm to 800 nm. Our results indicate that, at the leaf level, estimation of chlorophyll content can be identified in NDVInb. Ranges for these bands include: 1) 720-735 nm combined with 400-428 nm; 2) 550-615 nm, 692-701 nm or 707-715 nm combined with 400-432 nm or 462-496 nm; 3) 562-589 nm, 616-662 nm or 729-737 nm combined with 434-454 nm; and 4) 664-687 nm combined with 550-615 nm or 692-701 nm.

In GEMINI-1/-2, dolutegravir + lamivudine was non-inferior to dolutegravir + tenofovir disoproxil fumarate/emtricitabine (TDF/FTC) in achieving viral suppression (viral load [VL] < 50 copies/mL) in treatment-naive adults. Abbott’s RealTime HIV-1 assay provides quantitative VL (40–10,000,000 copies/mL) and qualitative target detected or target not detected (TND) for VL < 40 copies/mL. This post hoc analysis assessed very-low-level viremia and “blips” through Week 144. Proportions with VL < 40 copies/mL and TND are presented overall and by baseline VL and CD4+ cell count. “Blips” (single VL ≥ 50 to <200 copies/mL with adjacent values < 50 copies/mL) were assessed from Day 1 after VL suppression and from Weeks 48 through to 144. Proportions with TND increased through Week 48 and were similar between groups at all visits (Week 144: dolutegravir + lamivudine, 451/716 [63%]; dolutegravir + TDF/FTC, 465/717 [65%]). By observed analysis, TND rates were similar between groups across baseline subgroups. Through Week 144, proportions with ≥1 “blip” were generally comparable for dolutegravir + lamivudine vs. dolutegravir + TDF/FTC from Day 1 (15% vs. 20%) and from Week 48 (7% vs. 11%). Through 144 weeks, the proportions with TND or “blips” were similar between dolutegravir + lamivudine and the three-drug comparator, reinforcing the efficacy and durability of dolutegravir + lamivudine.

ImportanceDespite federal initiatives encouraging the enrollment of individuals from racial and ethnic minority groups in US clinical trials, no studies to date have specifically examined demographic disparities among participants in phase 1 drug development trials for patients with metastatic cancer.ObjectiveTo assess trends in the enrollment of patients from racial and ethnic minority groups in US phase 1 therapeutic drug trials for metastatic cancer from 2000 to 2018.Design, Setting, and ParticipantsIn this cross-sectional study, ClinicalTrials.gov was queried in July 2021 to identify completed phase 1 drug trials for metastatic cancer in the US from January 1, 2000, to December 31, 2018, with published results, yielding 221 phase 1 trials with 8309 participants aged 18 years or older with metastatic solid tumors. Proportions of each racial and ethnic group of trial participants were compared with that from the North American Association of Central Cancer Registries’ Cancer in North America (CiNA) database. Statistical analysis was performed from July 12, 2021, to March 15, 2022.Main Outcomes and MeasuresFor each racial and ethnic group, the difference between trial and CiNA proportions was examined using a 2-sample test for equality of proportions with continuity correction.ResultsThe 8309 phase 1 trial participants (4198 men [50.5%]; median age, 59 years) included 23 American Indian or Alaska Native participants (0.3%), 371 Asian or Pacific Islander participants (4.5%), 514 Black participants (6.2%), 401 of 5076 Hispanic or Latinx participants (7.9%), and 7154 White participants (86.1%). Industry funded 165 of the 221 trials (74.7%). White patients were overrepresented overall compared with the corresponding CiNA cohort (7154 of 8309 [86.1%] vs 4 113 096 of 4 891 486 [84.1%]; difference, 2.0 percentage points; P &amp;lt; .001). There was an increase in overrepresentation of White patients from 2000 to 2011 (trials, 2780 of 3245 [85.7%]; CiNA, 2 378 019 of 2 800 711 [84.9%]; difference, 0.8 percentage points; P = .23) to 2012-2018 (trials, 4374 of 5063 [86.4%]; CiNA, 1 735 077 of 2 090 775 [82.9%]; difference, 3.5 percentage points; P &amp;lt; .001) and corresponding worsening representation of American Indian or Alaska Native patients (2000-2011: trials, 10 of 3245 [0.3%]; CiNA, 10 905 of 2 800 711 [0.4%]; difference, −0.08 percentage points; 2012-2018: trials, 13 of 5063 [0.3%]; CiNA, 9484 of 2 090 775 [0.5%]; difference, −0.20 percentage points), Asian or Pacific Islander patients (2000-2011: trials, 121 of 3245 [3.7%]; CiNA, 75 033 of 2 800 711 [2.7%]; difference, 1.1 percentage points; 2012-2018: trials, 151 of 5063 [3.0%]; CiNA 70 535 of 2 090 775 [3.4%]; difference, −0.75 percentage points), Black patients (2000-2011: trials, 244 of 3245 [7.5%]; CiNA, 322 701 of 2 800 711 [11.5%]; difference, −4.0 percentage points; 2012-2018: trials, 270 of 5063 [5.3%]; CiNA, 255 625 of 2 090 775 [12.2%]; difference, −6.9 percentage points), and Hispanic or Latinx patients (2000-2011: trials, 161 of 1792 [9.0%]; CiNA, 169 297 of 2 800 711 [6.0%]; difference, 3.0 percentage points; 2012-2018: trials, 240 of 3295 [7.3%]; CiNA, 156 118 of 2 090 775 [7.5%]; difference, −0.2 percentage points). Similar disparities were observed when comparing industry-funded and academic center–sponsored trials.Conclusions and RelevanceIn this cross-sectional study of participants in phase 1 clinical trials of drugs for metastatic cancer, worsening disparities were observed over time in the accrual of patients from racial and ethnic minority groups. These findings may represent widening inequalities in access to trial sites and worsening systemic biases. More efforts are needed to diversify phase 1 cancer drug trials to improve equity in access to new treatments and to ensure that safety and efficacy findings from early drug trials are generalizable across populations.

BackgroundTelomere dysfunction can underly the development of idiopathic pulmonary fibrosis (IPF), and recent work suggests that patients with telomere syndromes might benefit from treatment with androgens, such as danazol.MethodsThis was a prospective observational cohort study. 50 patients with IPF received off-label treatment with danazol after they showed progressive disease under treatment with pirfenidone or nintedanib. The primary outcome was the difference in yearly decline in forced vital capacity (FVC) prior to (pre) and after (post) start of treatment with danazol.ResultsThere was no significant difference in FVC-decline between 1 year pre and 1 year post start of danazol treatment (mean decline pre 395 mL (95% confidence interval (CI) 290–500) compared to post 461 mL (95% CI 259–712); p=0.46; paired t-test). 11 patients (22%) were still on danazol after 1 year, and 39 patients had stopped danazol, mainly because of side-effects (56%) or death (33%). In patients who were still using danazol after 1 year, FVC-decline significantly slowed down under danazol treatment (mean pre 512 mL (95% CI 308–716)versuspost 198 mL (95% CI 16–380); p=0.04). Median survival post danazol was 14.9 months (95% CI 11.0–18.8).ConclusionDanazol as a treatment of last resort in patients with IPF did not lead to slowing of lung function decline and was associated with significant side-effects. It remains to be determined if earlier treatment or treatment of specific patient subgroups is beneficial.

Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang trên 98 bệnh nhân (BN) tuổi trung bình 75,5 ± 3,62, cao nhất 95 tuổi, thấp nhất 55 tuổi. Bảng điểm ISPP trước phẫu thuật: 27,3 ± 2,8; Bảng điểm chất lượng cuộc sống (CLCS) trước phẫu thuật: 5,2 ± 0,7; Chỉ số IIEF trước phẫu thuật: 17,8 ± 5,8; Kết quả: Biến chứng sau phẫu thuật: Hẹp niệu đạo 3,1%; đái rỉ tạm thời 3,1%; xuất tinh ngược dòng 4,1%; Điểm IPSS trung bình sau mổ 1 tháng, 6 tháng và 12 tháng lần lượt là 7,6 ± 3,3; 5,1 ± 2,1 và 4,6 ± 1,8; Điểm CLCS trung bình sau mổ 1 tháng, 6 tháng và 12 tháng lần lượt là: 1,4 ± 0,7; 0,88 ± 0,5 và 0,8 ± 0,4; Chỉ số IIEF trung bình sau mổ 1 tháng, 6 tháng và 12 tháng lần lượt là 12,18 ± 5,8; 12,51 ± 7,11 và 12,9 ± 7,1. Kết luận: Phẫu thuật nội soi điều trị UPĐLTTTL là phương pháp an toàn, hiệu quả, cải thiện CLCS của BN.

Abstract Background: Medical financial hardship, encompassing material, behavioral, and psychologic domains, is becoming an increasingly common consequence of illness in cancer patients. Identifying at-risk patients is the first step to develop proactive approaches to mitigate this problem. To try and address this need, Mayo Clinic Breast Disease Registry (MCBDR) is prospectively collecting data about financial concerns in addition to the usual sociodemographic and clinical information. Methods: We used data from Mayo Clinic Breast Disease Registry, a prospective cohort of consenting patients seen at Mayo Clinic Rochester within one year of initial breast cancer diagnosis. Participants completed baseline and annual follow-up surveys rating their financial concerns on a linear analogue scale from 0 (“none”) to 10 (“constant concerns”). We compared patient-reported financial concern at baseline to that on each patient’s most recent survey, with worsening concerns defined as a 1+-point increase. Logistic regression evaluated for predictors of worsening financial concerns. Results: 1,957 participants responded to financial concern questions on a baseline and at least one follow-up survey between 2015 and 2020. Mean age was 58.5 years (SD 12.5), and mean time between diagnosis and the most recent follow-up was 25.6 months (SD 16.2). 357 (18.2%) reported worsening financial concerns. Only lower baseline financial status was associated with a greater likelihood of worsening financial concerns (see Table). Conclusions: More than one in seven breast cancer survivors develop worsening financial concerns within 5-years of diagnosis, and those with less financial security at baseline appear to be most vulnerable. Поражение нижних конечностей (НК) при сахарном диабете (СД) является частой причиной ампутации, что приводит к значительному снижению качества жизни пациентов. Несмотря на широкий спектр совре- менных методов лечения СД, прогрессивный рост количества ампутаций у данных пациентов является актуальной проблемой. ЦЕЛЬ: изучить клинико-лабораторные характеристики пациентов центра спасения конечностей (ЦСК) с сахарным диабетом 2 типа (СД2). МАТЕРИАЛЫ И МЕТОДЫ: проведен ретроспективный анализ 729 историй болезни пациентов, имею- щих СД2 и госпитализированных в ЦСК ГБУЗ ГКБ им. В.П. Демихова в период с 2019 по 2020 гг. РЕЗУЛЬТАТЫ: всего в анализ вошло 729 пациентов. Медиана возраста – 66,0 лет [59,0; 73,0]. У 50,89% (n=371) пациентов стаж СД2 более 10 лет. Средний ИМТ составил 31,82±0,43 (95% ДИ 30,96-32,67). Наиболее частой сопутствующей патологией являлась артериальная гипертензия (АГ) – 89,57% (n=653). У 63,24% (n=461) пациентов имелась ИБС, из них у 19,62% (n=143) был инфаркт миокарда в анамнезе. У 16,87% (n=123) наблюдалось острое нарушение мозгового кровообращения, у 44,31% (n=323) – хрони- ческая болезнь почек (ХБП). Медиана гликированного гемоглобина (HbA1c) составляла 7,8% [6,7; 9,1]. Медиана гликемии – 9,9 [7,2; 13,8] ммоль/л. На инсулинотерапии находились 75,99% (n=554) пациентов, из них 14,9% получали только инсулин (n=83), а 85,01% (n=471) – базальный инсулин и пероральные сахароснижающие препараты (ПССП). Только ПССП получали 14,95% (n=109) пациентов. 14,13% (n=103) получали метформин, 6,31% (n=46) – ингиби- торы дипептидилпептидазы-4 (иДПП-4), 0,41% (n=3) – агонисты глюкагоноподобного пептида (аГПП-1), 3,70% (n=27) – ингибиторы натрий-глюкозного ко-транспортера (иНГЛТ-2), 16,46% (n=120) – препараты сульфонилмочевины. Лечение статинами получали 38,59% пациентов (n=225). Среди имевших АГ терапия ингибиторами ренин-ангиотензин-альдостероновой системы была назначена 79,46% (n=472) пациентам, блокаторами кальциевых каналов – 20,55% (n=128). При поступлении у 72,7% пациентов (n=530) был диагностирован СДС. У 27,2% (n=199) – облитериру- ющий атеросклероз артерий НК, из них у 66% (n=133) – критическая ишемия НК. У 26% пациентов с СДС (n=244) имела место гангрена, у 34,5% (n=183) – язва, у 23,9% (n=127) – флегмона, у 9,05% (n=48) – абсцесс, остеомиелит встречался у 11,8 (n=63) пациентов данной группы. В отделении 15,5% (n=113) пациентам провели реваскуляризацию, 14,4% (n=105) было проведено стен- тирование, 1,1% (n=8) – шунтирование. Ампутация НК была выполнена 51% пациентов (n=372), из них 9,95% (n=37) – высокая ампутация, 90,05% (n=335) – низкая ампутация. Среди пациентов с СДС только местное лечение получили 33,5% (n=244), антибактериальную терапию получали 7,2% пациентов (n=53). Выписаны с улучшением 98,5% (n=718) пациентов, летальность стала исходом госпитализации в 1,5% случаев (n=11). ВЫВОДЫ: большинство пациентов ЦСК с СД2, госпитализированных в 2019-2020 г., были старше 60 лет (68,86% (n=502)), имели ожирение (64,14% (n=127)), а также атеросклеротические сердечно-сосудистые за- болевания (АГ – 89,57% (n=653), ИБС – 63,24% (n=461), ОНМК в анамнезе – 16,87% (=123)). У 50,89% (n=371) пациентов стаж СД2 составлял более 10 лет, у 44,31% (n=323) имелась ХБП. Исходом 51% (n=372) госпита- лизаций стала ампутация НК, что требует более тщательного подхода к профилактике и своевременной диагностике поражения НК у пациентов с СД.

PA Inhibitor activity (PAIact-Verheijen’s method-U/ml) and PA Inhibitor 1 antigen (PAIlAg- Kruithof’s radioimmunoassay -ng/ml) were evaluated, on blood samples before (B) and after (A) venous occlusion (VO) (10 min) in order to analyse, in B-VO samples, the ratio (R=PAIlAg/PAIact) ' between the immunological and enzymatic material, and the change of PAIlAg levels after VO.The B-VO values (m ± SD) were determined from 111 plasmas : -86 with normal ( 12U/ml) PAIact levels = 3.7 ± 2.5 ; PAIlAg = 17.4 ± 10.1 ; R = 7.1 ± 6.2 (range 1.5 - 24 )-25 with high PAIact levels (15 post surgery, 10 obese patients)= 30.9± 19.3; PAIlAg = 75.3 ± 44.7 ; R = 2.7 ± 1.5 (range 1.3- 4.4). The correlation between the 2 dosages was r = 0.82 (p 0.01). In the 2 kinds of patients with high PAIact levels, a parallel high PAIlAg level was found. As the range of the ratio R was very large, patients were divided in 2 Groups : GrI : Normal R ( 7), n=81 (normal PAIact : n=57 ; high PAIact : n=24) and GrII : high R, n=30. The results for PAIact/PAIlAg/R were = GrI :12.6± 16.3/ 31.5±34.4/ 3.2 ± 1.5. GrII : 2.6±4.1/ 27.9 ±30.1/ 13.8±6.1. In GrII the platelet origin of inactive PAIlAg from in vitro activation of platelets could be demonstrated (high level of BTG lug/ml) in 50 % of cases. No in vitro platelet activation could be demonstrated in GrI. These results point out the necessity of a good preparation of the plasma (mainly 0-4°) for PAIlAg determination.The B and A-VO values were analysed from 60 subjects. Plasmas with in vitro activation of platelet determined by BTG levels had been discarded. No platelet activation occured with the VO. The results were not corrected with the PCV. PAIlAg B-VO/A-VO = 19.2 ± 15.1/ 26.9 ± 18.3. 36 subjects (60 % of total) had an increase in PAIlAg A-VO 20 % (mean increase = 62 %) . There was no correlation between basal values of PAIlAg and the increase after VO, and between PAIlAg increase and t-PA-Ag release. It is concluded that a weak increase of PAIlAg may occur after VO ; this increase is not parallel with t-PA release. The physiopathological significance of this increase of PAIlAg as yet to be evaluated.

Introdução: A doença hemolítica perinatal (DHPN) é um tipo de anemia hemolítica causado por incompatibilidade sanguínea materno-fetal. Trata-se de uma doença de elevada morbimortalidade perinatal, que pode ter seus riscos reduzidos por uma boa assistência pré-natal, que permite diagnóstico e intervenção precoce nos casos de maior gravidade. Objetivos: Analisar a população de gestantes aloimunizadas assistidas em centro de referência no Rio de Janeiro e descrever dados de seu acompanhamento pré-natal, antecedente obstétrico, parto e principais desfechos perinatais. Materiais e métodos: Foi realizado um estudo observacional, retrospectivo, descritivo, com obtenção de dados por análise de prontuários de gestantes assistidas na Área de Atenção à Gestante da instituição que tiveram resultado do teste de Coombs indireto positivo, anti-D, realizado na própria instituição entre os anos de 2015 e 2020 e de seus respectivos recém-natos. Resultados e conclusão: A média de idade gestacional do início do pré-natal na instituição foi de 23,89 semanas e a de número de consultas de pré-natal foi de 6,14. Quanto à paridade, a média foi de 3,56. A respeito da dosagem da maior titulação de anti-D, a mediana foi de 1:32 e 81,4% tiveram titulação de anticorpo maior ou igual a 1:16. Em 66 casos (68%) as gestantes foram submetidas à avaliação de Doppler da ACM, das quais 11 tiveram medida maior do que 1,5 desvios da mediana — achado compatível com alto risco de anemia fetal. Entre elas, cinco foram submetidas a TIU e duas tiveram o diagnóstico de hidropsia fetal, A idade gestacional média de interrupção da gestação foi de 37 semanas. Vinte e seis (32,5%) partos ocorreram antes de 37 semanas e quatro (5%), antes de 34 semanas. Na amostra analisada, em nove casos não havia informação sobre o acompanhamento neonatal e, entre os 71 restantes, não houve nenhum caso de óbito durante a internação. A assistência pré-natal prestada na instituição resulta em desfecho neonatal satisfatório, com baixas taxas de TIU e de óbito fetal e neonatal. No entanto, a assistência pré-natal à população geral ainda está aquém do ideal, refletindo em atraso na chegada para atendimento no centro de referência e, sobretudo, em falha na administração da profilaxia com imunoglobulina anti-D.

Infrared-pumped upconversion lasing of praseodymium ions in a fluorozirconate glass fiber at room temperature has been observed in our laboratory at 491,520,605,635, and 715 nm. For all these transitions the initial state is the 3P0 level at 21 000 cm-1, populated by a two-step pumping process using Ti:sapphire lasers at diode compatible wavelengths; 1.01 μm and 835 nm. The observed threshold powers suggest that diode-pumped operation of these upconversion laser transitions should be readily achievable. Elsewhere the use of codoping to pump this system at a single wavelength has also been demonstrated. In waveguide geometry modest pump powers can achieve large transition rates, an important advantage in this system where the intermediate pump level has a lifetime limited by nonradiative decay to 100 μs. The 4-level 635-nm transition, which exhibited the highest gain, yielded a maximum output of 185 mW when pumped by a total of 2.7 W of infrared power. The efficiency of the pumping process was further demonstrated by operation on the quasi-3-level blue transition at 491 nm. Output powers of up to 40 mW have been achieved at this wavelength.

Fibrinolytic parameters were studied before and after a 10 minute venous occlusion (VO) in 48 patients with confirmed deep vein thrombosis (DVT), at least 3 months after the last thrombotic episode. Congenital antithrombin III or protein C deficiencies were ruled out in these patients. The following tests were performed : euglobulin clot lysis time (ECLT), diluted whole blood lysis times (DWBLT), tissue plasminogen activator (t-PA) antigen (Biopool kit) and activity (fibrin plates), PAI activity (Verheijen et al.) and PAI i antigen (RIA, Kruithof et al.).Patients were divided in 2 groups : group 1 (27 patients) with normal fibrinolytic response (ECLT and DWBLT less than 90 min. after VO), group 2 (21 patients) with defective fibrinolytic response at least twice with a few weeks time interval. In group 1, the mean t-PA antigen release after VO was 32.0 ± 27.2 ng/ml (post-occlusion value minus preocclusion value) and was associated with low or normal levels of basal PAI activity (7.1 ±3.1 IU/ml), and PAI 1 antigen (15.1 ±4.1 ng/ml, n = 7). These results could explain the good response to VO, and a PAI activity suppression after stasis (PAI activity undetectable in 20 out of 27 patients).In contrast, in group 2, a low t-PA Ag increase after VO:10.5 ± 8.2 ng/ml as compared to group 1 (p <0.001) was associated with high levels of basal PAI activity : 22.4 ± 15.4 IU/ml (p <0.001), and PAI 1 Ag : 43.6 ± 24.9 ng/ml (n = 11, p <0.001). This association may account for the impaired fibrinolytic response aftep V0, and for persistant high PAI activity levels after stasis (17.6 ± 22.5 IU/ml). Before V0, PAI 1 Ag levels were in good correlation with PAI activity (r = 0.66, p <0.01), and , were significantly higher in group 2 as compared to group 1 (only 3 patients belonging to group 2 had normal PAI levels).Since elevated PAI 1 antigen and PAI activity levels were associated with an abnormal fibrinolytic response to venous stasis, VO test could be restricted to patients with normal PAI levels, in order to detect hypofibrinolysis related to insufficient t-PA release.

Recently a model of primary hemostasis has been introduced (Kratzer & Bom, Haemostasis 15:357;1985). This in vitro method detects very sensitively pathological platelet functions (Kratzer, Bellucci & Caen, Haemostasis 15:363; 1985). In order to use this technique routinely in the hospital the instrument now called “Thrombostat 4000” has been completely computerized and standardized by Kratzer and von der Goltz, VDG (8221 Seeon, West Germany).An artificial vessel (Aperture: cellulose acetate coated with collagen typ I, 2.2 ug/ mm2) with a single 150 urn hole; capillary: teflon, 200 urn, length 20 mm) was perfused with anticoagulated whole blood (Na-citrate 1 to 10) using a pressure of 40 mbar. To determine the reproducibility of the new technique, in vitro bleeding volume (V), time (T) and flow at the beginning of the experiment (IF), depending on blood viscosity, was measured in a healthy control person at different days (mean +− standard deviation, n= number of experiments).The mean of the intraassay variance (%) was: V (7.1), T(6.8) and IF(3.3). It was possible to measure the mean from day to day with the following variances (%): V(6.2), T(4.7) and IF(4.1). The low variance, which approaches enzymatic determinations is astonishing because platelet cell functions depend on a complex interaction of several thousands of enzymes.The excellent assistance of P. Abstract. Introduction. The relevance of the research topic is due to the need to develop and scientifically substantiate physiological criteria for assessing working conditions in difficult cases according to indicators of the functional state of the body (in accordance with the section "General hygienic assessment of working conditions" of Guideline R 2.2.2006-05). The aim of the work is to determine changes in the functional state of the body in workers of mental, visual-intense, physical activity on the basis of complex physiological and hygienic studies to substantiate quantitative physiological criteria for different classes of labor intensity. Materials and methods. Complex industrial physiological and hygienic studies included an assessment of the degree of labor intensity with the analysis of retrospective data, physiological studies of the central nervous system, visual analyzer, neuromuscular apparatus, cardiovascular system of mental workers (52 groups), visually stressful (32), physical labor ( 71 professional group). Statistical processing of the obtained data was carried out using the statistical software Statistika 10, Microsoft Excel 2010. Research results. It is shown that the depth and time of the onset of physiological changes depend on the degree of labor intensity, which makes it possible to calculate the percentage of decrease in functions, taking into account the intensity class of the labor process. The list of physiological criteria for confirming the class of working conditions is determined by the characteristics of labor activity, with mental work these are indicators of concentration of attention and the volume of working memory, with visually intense labor, the critical frequency of fusion of light flashes, with physical labor, muscle endurance and heart rate. Conclusions. Quantitative physiological criteria have been established for various types of labor activity, which provide a valid evidence-based assessment of the class of working conditions according to the characteristics of the intensity of the labor process and control of the levels of the functional state.

Human plasma fibrinogen (ϕ) is heterogeneous with respect to its γ’ chain population (γn 93%; γ’ 7%) which differ in that C-terminal residues 407-411 of γn are replaced in γ’ by a unique 20 amino acid sequence. DEAE-cellulose chromatography yields two major $ peaks (peak 1 and peak 2 ϕ). Peak 1 ϕ molecules each contain two γn chains whereas peak 2 ϕ molecules are heterodimeric with respect to γn and γ’ chains. Using washed human platelets (PL), we confirmed that the number of peak 2 ϕ molecules binding to PL in the presence of ADP is about half the number of peak 1 ϕ molecules (18,962 ± 2,298 vs. 44,366 ± 16,096 molecules per PL) although Kds were similar − 0.20 µM and 0.26 µM, respectively. We also confirmed that isolated S-carboxymethyl (SCM)γn chains supported PL aggregation well (85% of peak 1 ϕ @ 0.94 mg/ml). In contrast to γn chains, SCM γ’ chains supported aggregation poorly (12% of peak 1 ϕ), and a 50:50 mixture of γn and γ’ supported aggregation to only 65%. Nevertheless, we found that peak 2 ϕ (n=15) supported aggregation well in the presence of ADP (5-20 mM) to an extent that was ≥ 93% of that of peak 1 ϕ (n=8). Since the C-terminal region of the Aγ chain has been shown to play a role in ϕ-PL interactions, we evaluated whether this region in peak 2 ϕ molecules was particularly important for maintaining its ability to support PL aggregation. Peak 2 ϕ from which C-terminal Aγ chain fragments had been removed by digestion with plasmin from >96% to 93 or 90% coagulability, showed decreased PL aggregation potential (71% and 54%, respectively) compared to comparably digested peak 1 ϕ (90% coagulable; 90% aggregation). In contrast to plasmin digested peak 2 ϕ, an unchromatographed plasma ϕ fraction (I-9) lacking C-terminal regions of Aγ chains but rich in γn chains, supported PL aggregation to the same extent as did intact peak 2 ϕ. These findings indicate that ϕ molecules containing Yγn chains can approach the aggregation potential of ϕ molecules containing 100% γn only if intact Aγ chains are also present.

Introdução: Pacientes com COVID-19 apresentam sinais precoces de deterioração, pois a piora do quadro é identificada pelos sinais vitais. Sistemas de alerta precoce identificam a necessidade de intervenções rápidas e podem ser usados em unidades de internação, para prever os recursos necessários. No entanto, há uma lacuna sobre sua aplicação no contexto da COVID-19 em hospitais brasileiros. Objetivo: Analisar a performance prognóstica e o poder discriminatório de uma escala de alerta precoce modificada, em unidades de internação adulto COVID-19 de um hospital universitário paulista quanto a admissões em unidades de terapia intensiva (UTI) e óbitos. Métodos: Tratou-se de estudo de coorte retrospectivo que avaliou informações de 300 participantes hospitalizados com COVID-19 de março/2020 a março/2022. Foi utilizado o National Early Warning Score 2 (NEWS2) para obter o escore de risco de deterioração clínica precoce; e o Índice de Comorbidades de Charlson (ICC), para ajustar o risco estimado pelo NEWS2 de acordo com a prevalência de condições clínicas prévias. Resultados: O ICC sugeriu que 26% possuíam taxa de mortalidade em 1 ano e 48% em 10 anos; e demonstrou bom desempenho nas associações com o NEWS2. O escore do NEWS2 foi de 4,1 unidades, sugerindo moderada complexidade; com sensibilidade de 71,1%, especificidade de 56,3%, Valor preditivo positivo de 22,4%, Valor preditivo negativo de 91,7%, Razão de verossimilhança positiva de 1,6 e Razão de verossimilhança negativa de 0,5. A análise da área sob a curva ROC (66%; IC95%=0,57-0,54; p=0,0009) evidenciou que o NEWS2 foi modestamente capaz de predizer a probabilidade de óbito ou admissão na UTI. Conclusão: O NEWS2 apresentou bom poder discriminatório para avaliar a gravidade de pacientes com COVID-19 e moderada capacidade de predição de óbitos e de admissão na UTI.

В рамках отечественных исследований закономерностей формирования и распространения растительного покрова и оптимизации способов его использования на урбанизированных территориях наблюдается рост актуальности изучения малых и средних городов. В результате анализа порядка 60 публикаций и статистических материалов за период с 2010 по 2022 гг., представленных в российских электронных научных библиотеках Elibrary, ResearchGate, Google Scholar, обсуждаемые сегодня вопросы изучения растительного покрова, рассматриваемой категории городов России были объединены в несколько направлений: Видовое богатство урбанофлоры и ее структуры. За указанный период найдено и проанализировано 50 публикаций и статистических материалов, в которых отражены полные списки видов урбанофлоры для 86 городов (18 малых и 68 средних). Видовое богатство колеблется от 140 (г. Анива, Сахалинская область) до 786 (г. Ливны, Орловская область). В 6 городах отмечено до 200 видов растений, в 37 – от 201 до 400, в 27 – от 401 до 600 и только в 12 зарегистрировано свыше 600 видов [3]. Как отмечают специалисты, разница в количестве зависит от многих факторов: площади и численности населения города, возраста населенного пункта, природно-климатических условий и др. [1, 5, 6]. Однако нами не выявлено тесных корреляционных связей между факторами. В структуре урбанофлоры преобладают семейства: Asteraceae, Poaceae, Rosaceae, Fabaceae, Brassiaceae, Cyperaceae, Сaryophyllaceae, Apiaceae, Polygonaceae, Ranuncllaceae [3]. Аборигенная и адвентивная (чужеродная) фракция урбанофлоры. В анализируемых научных публикациях отмечено, что в среднем на долю аборигенной и адвентивной фракции приходится 71% (317 видов) и 29% (130) соответственно. В урбанофлоре 7 городов отмечается до 200 аборигенных видов, в 37 – от 201 до 400 и в 8 городах свыше 400. Что касается адвентивных, то в 42 городах отмечено не больше 200 видов и только в 5 – от 201 до 400. В связи с тем, что в некоторых городах процесс адвентизации достигает 40-50%, актуальным является организация охраны аборигенных, в том числе редких и охраняемых видов. Это связано с их приспособленностью к внешней среде, а также они поддерживают устойчивость биоразнообразия [3]. Охрана урбанофлоры. В урбанофлоре малых и средних городов - Абазы (Республика Хакасия) 8 видов занесены в Красную книгу (6 – Красная книга России и 2 – региональная), Кинешма (Ивановская область) 2 вида в региональной Красной книге, Кумертау (Республика Башкортостан) 11 видов занесено в Красную книгу России, Мелеуз (Республика Башкортостан) 10 видов и другие. В некоторых городах отмечены редкие и охраняемые виды: Гаврилов Посад (Ивановская область) 24 вида, Кадников (Вологодская область) 74 вида отнесены к статусу «редкие», 34 – «охраняемые» и для 40 видов необходим ботанический контроль, Сенгилей (Ульяновская область) – 19 редких видов и другие. В настоящее время в городах активно организуется сеть ООПТ различного типа (памятники природы, ботанические сады, дендрарии, парки и другие). Однако большинство публикаций по данному направлению касается больших и крупных городов. Отмечено всего 4 средних города, имеющих ООПТ – Туапсе и Геленджик (Краснодарский край), Биробиджан (ЕАО), Магадан (Магаданская область) [4, 6].

If most of them are convenient enough for readers (if the journal is open access), then many have problems informing authors about the conditions, the process of submitting and reviewing the manuscript. The Editorial Offices empirically populate the journal's website with content that can change dramatically (both in terms of website design and content) as the journal develops. Aim of Study: To develop a website model for an academic journal that takes into account the basic requirements for the preparation, publication, and archiving of high quality scientific manuscripts. La República de Panamá cuenta con una estructura para la gestión del riesgo que inició bajo el esquema de Protección Civil característico de la institucionalidad latinoamericana entre los años 1960 y 2000, y que luego fue evolucionando hacia esquemas más participativos y desconcentrados. En 2005 se adoptó una ley que introdujo una visión preventiva, aunque manteniendo un énfasis en la preparación y la respuesta, y en 2010, con la inspiración dada por la Política Centroamericana para la Gestión Integral del Riesgo de Desastres (PCGIR), el país aprobó, por decreto ejecutivo, una Política Nacional en la cual se integraron elementos novedosos, más participativos y con una visión de responsabilidades extendida a todos los actores públicos. En este sentido, Panamá puede ser considerado un país en un proceso de transición activa, donde aún la normatividad de mayor fuerza jurídica se soporta en una vision centralizada dentro de la estructura institucional y territorial. Esta visión tiene su origen en el enfoque orientado a la respuesta que primaba en el contexto histórico en que se establecen las primeras normativas. La reforma del año 2005 no alcanzó a generar una orientación claramente preventiva ni participativa. Los resultados de la aplicación del iGOPP en Panamá (2013) arrojan un nivel general de avance del 32.93%, considerado dentro del rango incipiente en la clasificación propuesta por este índice. El análisis de los resultados por componentes de política pública para la GRD (ver Gráfico 1) revela progresos diferenciales que van del nivel bajo para el grupo de indicadores de "identificación y conocimiento del riesgo" (15%), hasta notable para el componente de "marco general de gobernabilidad" (cumplimiento del 71%). "protección financiera" se ubica en el rango apreciable (41%), mientras que "preparativos para la respuesta" (32%) y "reducción del riesgo" (38%) se ubican en el nivel incipiente. Por su parte, el nivel más bajo (0%), lo obtuvo el componente de "planificación de la recuperación post-desastre", en el cual se evidencia que el país no cuenta con normatividad expresa que oriente los procesos de rehabilitación y reconstrucción. Por otro lado, el análisis general desde la dimensión de las fases de la política pública muestra un nivel incipiente para las fases de "definición de responsabilidades territoriales" (40%), "definición de responsabilidades sectoriales" (40%), "coordinación y articulación central de la política" (38%) y de "control, rendición de cuentas y participación" (32%). Finalmente, en un rango de avance calificado por el iGOPP como bajo, se ubican los aspectos relacionados con "evidencias de progreso en la implementación" (15%).

1.1 Resumen macroeconómico En el segundo trimestre la economía enfrentó varios choques, principalmente de oferta y de costos, la mayoría de los cuales no fueron anticipados, o los previstos fueron más persistentes de lo esperado, y que en conjunto interrumpieron la recuperación de la actividad económica observada a comienzos de año y llevaron la inflación total a niveles superiores a la meta. La inflación básica (sin alimentos ni regulados: SAR) aumentó, pero se mantuvo baja y acorde con lo esperado por el equipo técnico. A comienzos de abril se inició una tercera ola de pandemia, más acentuada y prolongada que la anterior, con un elevado costo en vidas humanas y algún impacto negativo en la recuperación económica. Entre mayo y mediados de junio los bloqueos de las carreteras y los problemas de orden público tuvieron un fuerte efecto negativo sobre la actividad económica y la inflación. Se estima que la magnitud de estos dos choques combinados habría generado una caída en niveles en el producto interno bruto (PIB) con respecto al primer trimestre del año. Adicionalmente, los bloqueos causaron un aumento significativo de los precios de los alimentos. A estos choques se sumaron los efectos acumulados de la disrupción global en algunas cadenas de valor y el incremento en los fletes internacionales que desde finales de 2020 vienen generando restricciones de oferta y aumentos de costos. Todos estos factores, que afectaron principalmente el índice de precios al consumidor (IPC) de bienes y de alimentos, explicaron la mayor parte del error de pronóstico del equipo técnico y el aumento de la inflación total a niveles superiores a la meta del 3 %. El incremento en la inflación básica y de los precios de los regulados fue acorde con lo esperado por el equipo técnico, y se explica principalmente por la eliminación de varios alivios de precios otorgados un año atrás. A todo esto se suma la mayor percepción de riesgo soberano y las presiones al alza que esto implica sobre el costo de financiamiento externo y la tasa de cambio. A pesar de los fuertes choques negativos, el crecimiento económico esperado para la primera mitad del año (9,1%), es significativamente mayor que lo proyectado en el informe de abril (7,1%), signo de una economía más dinámica que se recuperaría más rápido de lo previsto. Desde finales de 2020 las diferentes cifras de actividad económica han mostrado un crecimiento mayor que el esperado. Esto sugiere que los efectos negativos sobre el producto de las recurrentes olas de contagio estarían siendo cada vez menos fuertes y duraderos. No obstante, la tercera ola de contagio del Covid-19, y en mayor medida los bloqueos a las vías y los problemas de orden público, habrían generado una caída del PIB durante el segundo trimestre, frente al primero. Pese a lo anterior, los datos del índice de seguimiento a la economía (ISE) de abril y mayo han resultado mayores que lo esperado, y las nuevas cifras de actividad económica sectoriales sugieren que el impacto negativo de la pandemia sobre el producto se sigue moderando, en un entorno de menores restricciones a la movilidad y de mayor avance en el ritmo de vacunación. Los registros de transporte de carga (junio) y la demanda de energía no regulada (julio), entre otros, indican una recuperación importante después de los bloqueos en mayo. Con todo lo anterior, el incremento anual del PIB del segundo trimestre se habría situado alrededor del 17,3 % (antes 15,8 %), explicado en gran parte por una base baja de comparación. Para todo 2021 el equipo técnico incrementó su proyección de crecimiento desde un 6 % hasta el 7,5 %. Este pronóstico, que está rodeado de una incertidumbre inusualmente elevada, supone que no se presentarán problemas de orden público y que posibles nuevas olas de contagio del Covid-19 no tendrán efectos negativos adicionales sobre la actividad económica. Frente al pronóstico del informe pasado, la recuperación de la demanda externa, los niveles de precios de algunos bienes básicos que exporta el país y la dinámica de las remesas de trabajadores han sido mejores que las esperadas y seguirían impulsando la recuperación del ingreso nacional en lo que resta del año. A esto se sumaría la aún amplia liquidez internacional, la aceleración en el proceso de vacunación y las bajas tasas de interés, factores que continuarían favoreciendo la actividad económica. La mejor dinámica del primer semestre, que llevó a una revisión al alza en el crecimiento de todos los componentes del gasto, continuaría hacia adelante y, antes de lo esperado en abril, la economía recuperaría los niveles de producción de 2019 a finales de 2021. El pronóstico continúa incluyendo efectos de corto plazo sobre la demanda agregada de una reforma tributaria de magnitud similar a la proyectada por el Gobierno. Con todo eso, en el escenario central de este informe, el pronóstico de crecimiento para 2021 es del 7,5 % y para 2022 del 3,1 %. A pesar de esto, el nivel de la actividad económica seguiría siendo inferior a su potencial. La mejora en estas proyecciones, sin embargo, está rodeada de una alta incertidumbre. En junio la inflación anual (3,63 %) aumentó más de lo esperado debido al comportamiento del grupo de alimentos, mientras que la inflación básica (1,87 %) fue similar a la proyectada. En lo que resta del año el mayor nivel del IPC de alimentos persistiría y contribuiría a mantener la inflación por encima de la meta. A finales de 2022 la inflación total y básica retornarían a tasas cercanas al 3 %, en un entorno de desaceleración del IPC de alimentos y de menores excesos de capacidad productiva. En los meses recientes el aumento en los precios internacionales de los fletes y de los bienes agrícolas, y las mayores exportaciones de carne y el ciclo ganadero han ejercido presiones al alza sobre el precio de los alimentos, principalmente de los procesados. A estas fuerzas persistentes se sumaron los bloqueos de las vías nacionales y los problemas de orden público en varias ciudades registrados en mayo y parte de junio, los cuales se reflejaron en una fuerte restricción en la oferta y en un aumento anual no esperado del IPC de alimentos (8,52 %). El grupo de regulados (5,93 %) también se aceleró, debido a la baja base de comparación en los precios de la gasolina y a la disolución de parte de los alivios a las tarifas de servicios públicos otorgados en 2020. Como se proyectaba, la inflación SAR repuntó al 1,87 %, debido a la reactivación de los impuestos indirectos de algunos bienes y servicios eliminados un año atrás, y por las presiones al alza que ejercieron los alimentos sobre las comidas fuera del hogar (CFH), entre otros. En lo que resta del año se espera que el aumento en los alimentos perecederos se revierta, siempre y cuando no se registren nuevos bloqueos duraderos a las vías nacionales. El mayor nivel de precios de los alimentos procesados persistiría y contribuiría a mantener la inflación por encima de la meta a finales de año. La inflación SAR continuaría con una tendencia creciente, en la medida en que los excesos de capacidad productiva se sigan cerrando y registraría un aumento transitorio en marzo de 2022, debido principalmente al restablecimiento del impuesto al consumo en las CFH. Con todo esto, para finales de 2021 y 2022 se estima una inflación total del 4,1 % y 3,1 %, y una inflación básica del 2,6 % y 3,2 %, respectivamente. El comportamiento conjunto de los precios del IPC SAR, junto con continuas sorpresas al alza en la actividad económica, son interpretados por el equipo técnico como señales de amplios excesos de capacidad productiva de la economía. Estos persistirían en los siguientes dos años, al final de los cuales la brecha del producto se cerraría. El mayor crecimiento económico sugiere una brecha del producto menos negativa que la estimada hace un trimestre. Sin embargo, el comportamiento de la inflación básica, especialmente en servicios, indica que el PIB potencial se ha recuperado de forma sorpresiva y que los excesos de capacidad siguen siendo amplios, con una demanda agregada afectada de forma persistente. Esta interpretación encuentra soporte en el mercado laboral, en donde persiste un desempleo alto y la recuperación de los empleos perdidos se estancó. Adicionalmente, los aumentos en la inflación en buena medida están explicados por choques de oferta y de costos y por la disolución de algunos alivios de precios otorgados un año atrás. Los pronósticos de crecimiento y de inflación descritos son coherentes con una brecha del producto que se cierra más rápido y es menos negativa en todo el horizonte de pronóstico con respecto al informe de abril. No obstante, la incertidumbre sobre los excesos de capacidad es muy alta y es un riesgo sobre el pronóstico. Las perspectivas de las cuentas fiscales de Colombia se deterioraron, Standard & Poor’s Global Ratings (S&P) y Fitch Ratings (Fitch) redujeron su calificación crediticia, los bloqueos y problemas de orden público afectaron el producto y el país enfrentó una nueva ola de contagios de Covid-19 más acentuada y prolongada que las pasadas. Todo lo anterior se ha reflejado en un aumento de las primas de riesgo y en una depreciación del peso frente al dólar. Esto ha ocurrido en un entorno favorable de ingresos externos. Los precios internacionales del petróleo, del café y de otros bienes básicos que exporta el país aumentaron y han contribuido a la recuperación de los términos de intercambio y del ingreso nacional, y han mitigado las presiones al alza sobre las primas de riesgo y la tasa de cambio. En el presente informe se incrementó el precio esperado del petróleo para 2021 a USD 68 por barril (antes USD 61 bl) y para 2022 a USD 66 bl (antes USD 60 bl). Esta mayor senda presenta una convergencia hacia precios menores que los observados recientemente, como resultado de una mayor oferta mundial esperada de petróleo, la cual más que compensaría el incremento en la demanda de este bien básico. Por ende, se supone que el aumento reciente de los precios tiene un carácter transitorio. En el escenario macroeconómico actual se espera que las condiciones financieras internacionales sean algo menos favorables, a pesar de la mejora en los ingresos externos por cuenta de una mayor demanda y unos precios del petróleo y de otros productos de exportación más altos. Frente al informe de abril el crecimiento de la demanda externa fue mejor que el esperado, y las proyecciones para 2021 y 2022 aumentaron del 5,2 % al 6,0 % y del 3,4 % al 3,5 %, respectivamente. En lo corrido del año las cifras de actividad económica muestran una demanda externa más dinámica de la esperada. En los Estados Unidos y China la recuperación del producto ha sido más rápida que la registrada en los países de la región. En estos últimos la reactivación económica ha estado limitada por los rebrotes del Covid-19, las limitaciones en la oferta de vacunas y el poco espacio fiscal para enfrentar la pandemia, entre otros factores. La buena dinámica en el comercio externo de bienes se ha dado en un entorno de deterioro en las cadenas de valor y de un aumento importante en los precios de las materias primas y en el costo de los fletes. En los Estados Unidos la inflación sorprendió al alza y su valor observado y esperado se mantiene por encima de la meta, al tiempo que se incrementó la proyección de crecimiento económico. Con esto, el inicio de la normalización de la política monetaria en ese país se daría antes de lo proyectado. En este informe se estima que el primer incremento en la tasa de interés de la Reserva Federal de los Estados Unidos se dé a finales de 2022 (antes del primer trimestre de 2023). Para Colombia se supone una mayor prima de riesgo frente al informe de abril y se sigue esperando que presente una tendencia creciente, dada la acumulación de deuda pública y externa del país. Todo esto contribuiría a un incremento en el costo del financiamiento externo en el horizonte de pronóstico. La postura expansiva de la política monetaria sigue soportando unas condiciones financieras internas favorables. En el segundo trimestre la tasa de interés interbancaria y el índice bancario de referencia (IBR) se han mantenido acordes con la tasa de interés de política. Las tasas de interés promedio de captación y crédito continuaron históricamente bajas, a pesar de algunos incrementos observados a finales de junio. La cartera en moneda nacional detuvo su desaceleración anual y, entre marzo y junio, el crédito a los hogares se aceleró, principalmente para compra de vivienda. La recuperación de la cartera comercial y de los desembolsos a ese sector fue importante, y se alcanzó de nuevo el elevado saldo observado un año atrás, cuando las empresas requirieron niveles significativos de liquidez para enfrentar los efectos económicos de la pandemia. El riesgo de crédito aumentó, las provisiones se mantienes altas y algunos bancos han retirado de su balance una parte de su cartera vencida. No obstante, las utilidades del sistema financiero se han recuperado y sus niveles de liquidez y solvencia se mantienen por encima del mínimo regulatorio. A partir de este informe se implementará una nueva metodología para cuantificar y comunicar la incertidumbre que rodea los pronósticos del escenario macroeconómico central, en un entorno de política monetaria activa. Esta metodología se conoce como densidades predictivas (DP) y se explica en detalle en el Recuadro 1. Partiendo del balance de riesgos que contiene los principales factores que, de acuerdo con el juicio del equipo técnico, podrían afectar a la economía en el horizonte de pronóstico, la metodología DP produce distribuciones de probabilidad sobre el pronóstico de las principales variables (v. g.: crecimiento, inflación). Estas distribuciones reflejan el resultado de los posibles choques (a variables externas, precios y actividad económica) que podría recibir la economía y su transmisión, considerando la estructura económica y la respuesta de política monetaria en el futuro. En este sentido, permiten cuantificar la incertidumbre alrededor del pronóstico y su sesgo. El ejercicio DP muestra un sesgo a la baja en el crecimiento económico y en la brecha del producto, y al alza en la inflación. El balance de riesgos indica que las disyuntivas para la política monetaria serán potencialmente más complejas que lo contemplado en el pasado. Por el lado de las condiciones de financiamiento externo, se considera que el mayor riesgo es que se tornen un poco menos favorables, en un escenario en el cual la Reserva Federal de los Estados Unidos incremente con mayor prontitud su tasa de interés. Esto último, ante un crecimiento económico y del empleo mayor que el esperado en los Estados Unidos que genere presiones significativas sobre la inflación de ese país. A esto se suma la incertidumbre sobre el panorama fiscal en Colombia y sus efectos sobre la prima de riesgo y el costo del financiamiento externo. En el caso del crecimiento, la mayoría de los riesgos son a la baja, destacándose los efectos de la incertidumbre política y fiscal sobre las decisiones de consumo e inversión, la aparición de nuevas olas de contagio de la pandemia del Covid-19 y sus impactos sobre la actividad económica. En el caso de la inflación, se incorporó el riesgo de una mayor persistencia de los choques asociados con la disrupción de las cadenas de valor, mayores precios internacionales de las materias primas y de los alimentos, y una recuperación más lenta que la esperada de la cadena agrícola nacional afectada por los pasados bloqueos a las vías. Estos riesgos presionarían al alza principalmente los precios de los alimentos y de los bienes. Como principal riesgo a la baja se incluyó un alza de los arriendos menor que el esperado en el escenario central, explicada por una demanda débil y por una mayor oferta en 2022 dadas las altas ventas de vivienda observadas en el presente año. Con todo, el crecimiento económico presenta un sesgo a la baja y, con el 90 % de confianza, se encontraría entre un 6,1 % y 9,1 % para 2021 y entre el 0,5 % y 4,1 % para 2022. La brecha del producto tendría un sesgo a la baja, principalmente en 2022. El sesgo de la inflación es al alza, y se encontraría entre el 3,7 % y 4,9 % en 2021, y el 2,2 % y 4,7 % en 2022, con un 90 % de probabilidad. 1.2 Decisión de política monetaria En las reuniones de junio y julio la JDBR decidió mantener la tasa de política monetaria inalterada en 1,75 %.

Banco de la República is celebrating its 100th anniversary in 2023. This is a very significant anniversary and one that provides an opportunity to highlight the contribution the Bank has made to the country’s development. Its track record as guarantor of monetary stability has established it as the one independent state institution that generates the greatest confidence among Colombians due to its transparency, management capabilities, and effective compliance with the central banking and cultural responsibilities entrusted to it by the Constitution and the Law. On a date as important as this, the Board of Directors of Banco de la República (BDBR) pays tribute to the generations of governors and officers whose commitment and dedication have contributed to the growth of this institution.1 Banco de la República’s mandate was confirmed in the National Constitutional Assembly of 1991 where the citizens had the opportunity to elect the seventy people who would have the task of drafting a new constitution. The leaders of the three political movements with the most votes were elected as chairs to the Assembly, and this tripartite presidency reflected the plurality and the need for consensus among the different political groups to move the reform forward. Among the issues considered, the National Constitutional Assembly gave special importance to monetary stability. That is why they decided to include central banking and to provide Banco de la República with the necessary autonomy to use the instruments for which they are responsible without interference from other authorities. The constituent members understood that ensuring price stability is a state duty and that the entity responsible for this task must be enshrined in the Constitution and have the technical capability and institutional autonomy necessary to adopt the decisions they deem appropriate to achieve this fundamental objective in coordination with the general economic policy. In particular, Article 373 established that “the State, through Banco de la República, shall ensure the maintenance of the purchasing power of the currency,” a provision that coincided with the central banking system adopted by countries that have been successful in controlling inflation. In 1999, in Ruling 481, the Constitutional Court stated that “the duty to maintain the purchasing power of the currency applies to not only the monetary, credit, and exchange authority, i.e., the Board of Banco de la República, but also those who have responsibilities in the formulation and implementation of the general economic policy of the country” and that “the basic constitutional purpose of Banco de la República is the protection of a sound currency. However, this authority must take the other economic objectives of state intervention such as full employment into consideration in their decisions since these functions must be coordinated with the general economic policy.” The reforms to Banco de la República agreed upon in the Constitutional Assembly of 1991 and in Act 31/1992 can be summarized in the following aspects: i) the Bank was assigned a specific mandate: to maintain the purchasing power of the currency in coordination with the general economic policy; ii) the BDBR was designatedas the monetary, foreign exchange, and credit authority; iii) the Bank and its Board of Directors were granted a significant degree of independence from the government; iv) the Bank was prohibited from granting credit to the private sector except in the case of the financial sector; v) established that in order to grant credit to the government, the unanimous vote of its Board of Directors was required except in the case of open market transactions; vi) determined that the legislature may, in no case, order credit quotas in favor of the State or individuals; vii) Congress was appointed, on behalf of society, as the main addressee of the Bank’s reporting exercise; and viii) the responsibility for inspection, surveillance, and control over Banco de la República was delegated to the President of the Republic. The members of the National Constitutional Assembly clearly understood that the benefits of low and stable inflation extend to the whole of society and contribute mto the smooth functioning of the economic system. Among the most important of these is that low inflation promotes the efficient use of productive resources by allowing relative prices to better guide the allocation of resources since this promotes economic growth and increases the welfare of the population. Likewise, low inflation reduces uncertainty about the expected return on investment and future asset prices. This increases the confidence of economic agents, facilitates long-term financing, and stimulates investment. Since the low-income population is unable to protect itself from inflation by diversifying its assets, and a high proportion of its income is concentrated in the purchase of food and other basic goods that are generally the most affected by inflationary shocks, low inflation avoids arbitrary redistribution of income and wealth.2 Moreover, low inflation facilitates wage negotiations, creates a good labor climate, and reduces the volatility of employment levels. Finally, low inflation helps to make the tax system more transparent and equitable by avoiding the distortions that inflation introduces into the value of assets and income that make up the tax base. From the monetary authority’s point of view, one of the most relevant benefits of low inflation is the credibility that economic agents acquire in inflation targeting, which turns it into an effective nominal anchor on price levels. Upon receiving its mandate, and using its autonomy, Banco de la República began to announce specific annual inflation targets as of 1992. Although the proposed inflation targets were not met precisely during this first stage, a downward trend in inflation was achieved that took it from 32.4% in 1990 to 16.7% in 1998. At that time, the exchange rate was kept within a band. This limited the effectiveness of monetary policy, which simultaneously sought to meet an inflation target and an exchange rate target. The Asian crisis spread to emerging economies and significantly affected the Colombian economy. The exchange rate came under strong pressure to depreciate as access to foreign financing was cut off under conditions of a high foreign imbalance. This, together with the lack of exchange rate flexibility, prevented a countercyclical monetary policy and led to a 4.2% contraction in GDP that year. In this context of economic slowdown, annual inflation fell to 9.2% at the end of 1999, thus falling below the 15% target set for that year. This episode fully revealed how costly it could be, in terms of economic activity, to have inflation and exchange rate targets simultaneously. Towards the end of 1999, Banco de la República announced the adoption of a new monetary policy regime called the Inflation Targeting Plan. This regime, known internationally as ‘Inflation Targeting,’ has been gaining increasing acceptance in developed countries, having been adopted in 1991 by New Zealand, Canada, and England, among others, and has achieved significant advances in the management of inflation without incurring costs in terms of economic activity. In Latin America, Brazil and Chile also adopted it in 1999. In the case of Colombia, the last remaining requirement to be fulfilled in order to adopt said policy was exchange rate flexibility. This was realized around September 1999, when the BDBR decided to abandon the exchange-rate bands to allow the exchange rate to be freely determined in the market.Consistent with the constitutional mandate, the fundamental objective of this new policy approach was “the achievement of an inflation target that contributes to maintaining output growth around its potential.”3 This potential capacity was understood as the GDP growth that the economy can obtain if it fully utilizes its productive resources. To meet this objective, monetary policy must of necessity play a countercyclical role in the economy. This is because when economic activity is below its potential and there are idle resources, the monetary authority can reduce the interest rate in the absence of inflationary pressure to stimulate the economy and, when output exceeds its potential capacity, raise it. This policy principle, which is immersed in the models for guiding the monetary policy stance, makes the following two objectives fully compatible in the medium term: meeting the inflation target and achieving a level of economic activity that is consistent with its productive capacity. To achieve this purpose, the inflation targeting system uses the money market interest rate (at which the central bank supplies primary liquidity to commercial banks) as the primary policy instrument. This replaced the quantity of money as an intermediate monetary policy target that Banco de la República, like several other central banks, had used for a long time. In the case of Colombia, the objective of the new monetary policy approach implied, in practical terms, that the recovery of the economy after the 1999 contraction should be achieved while complying with the decreasing inflation targets established by the BDBR. The accomplishment of this purpose was remarkable. In the first half of the first decade of the 2000s, economic activity recovered significantly and reached a growth rate of 6.8% in 2006. Meanwhile, inflation gradually declined in line with inflation targets. That was how the inflation rate went from 9.2% in 1999 to 4.5% in 2006, thus meeting the inflation target established for that year while GDP reached its potential level. After this balance was achieved in 2006, inflation rebounded to 5.7% in 2007, above the 4.0% target for that year due to the fact that the 7.5% GDP growth exceeded the potential capacity of the economy.4 After proving the effectiveness of the inflation targeting system in its first years of operation, this policy regime continued to consolidate as the BDBR and the technical staff gained experience in its management and state-of-the-art economic models were incorporated to diagnose the present and future state of the economy and to assess the persistence of inflation deviations and expectations with respect to the inflation target. Beginning in 2010, the BDBR established the long-term 3.0% annual inflation target, which remains in effect today. Lower inflation has contributed to making the macroeconomic environment more stable, and this has favored sustained economic growth, financial stability, capital market development, and the functioning of payment systems. As a result, reductions in the inflationary risk premia and lower TES and credit interest rates were achieved. At the same time, the duration of public domestic debt increased significantly going from 2.27 years in December 2002 to 5.86 years in December 2022, and financial deepening, measured as the level of the portfolio as a percentage of GDP, went from around 20% in the mid-1990s to values above 45% in recent years in a healthy context for credit institutions.Having been granted autonomy by the Constitution to fulfill the mandate of preserving the purchasing power of the currency, the tangible achievements made by Banco de la República in managing inflation together with the significant benefits derived from the process of bringing inflation to its long-term target, make the BDBR’s current challenge to return inflation to the 3.0% target even more demanding and pressing. As is well known, starting in 2021, and especially in 2022, inflation in Colombia once again became a serious economic problem with high welfare costs. The inflationary phenomenon has not been exclusive to Colombia and many other developed and emerging countries have seen their inflation rates move away from the targets proposed by their central banks.5 The reasons for this phenomenon have been analyzed in recent Reports to Congress, and this new edition delves deeper into the subject with updated information. The solid institutional and technical base that supports the inflation targeting approach under which the monetary policy strategy operates gives the BDBR the necessary elements to face this difficult challenge with confidence. In this regard, the BDBR reiterated its commitment to the 3.0% inflation target in its November 25 communiqué and expects it to be reached by the end of 2024.6 Monetary policy will continue to focus on meeting this objective while ensuring the sustainability of economic activity, as mandated by the Constitution. Analyst surveys done in March showed a significant increase (from 32.3% in January to 48.5% in March) in the percentage of responses placing inflation expectations two years or more ahead in a range between 3.0% and 4.0%. This is a clear indication of the recovery of credibility in the medium-term inflation target and is consistent with the BDBR’s announcement made in November 2022. The moderation of the upward trend in inflation seen in January, and especially in February, will help to reinforce this revision of inflation expectations and will help to meet the proposed targets. After reaching 5.6% at the end of 2021, inflation maintained an upward trend throughout 2022 due to inflationary pressures from both external sources, associated with the aftermath of the pandemic and the consequences of the war in Ukraine, and domestic sources, resulting from: strengthening of local demand; price indexation processes stimulated by the increase in inflation expectations; the impact on food production caused by the mid-2021 strike; and the pass-through of depreciation to prices. The 10% increase in the minimum wage in 2021 and the 16% increase in 2022, both of which exceeded the actual inflation and the increase in productivity, accentuated the indexation processes by establishing a high nominal adjustment benchmark. Thus, total inflation went to 13.1% by the end of 2022. The annual change in food prices, which went from 17.2% to 27.8% between those two years, was the most influential factor in the surge in the Consumer Price Index (CPI). Another segment that contributed significantly to price increases was regulated products, which saw the annual change go from 7.1% in December 2021 to 11.8% by the end of 2022. The measure of core inflation excluding food and regulated items, in turn, went from 2.5% to 9.5% between the end of 2021 and the end of 2022. The substantial increase in core inflation shows that inflationary pressure has spread to most of the items in the household basket, which is characteristic of inflationary processes with generalized price indexation as is the case in Colombia. Monetary policy began to react early to this inflationary pressure. Thus, starting with its September 2021 session, the BDBR began a progressive change in the monetary policy stance moving away from the historical low of a 1.75% policy rate that had intended to stimulate the recovery of the economy. This adjustment process continued without interruption throughout 2022 and into the beginning of 2023 when the monetary policy rate reached 12.75% last January, thus accumulating an increase of 11 percentage points (pp). The public and the markets have been surprised that inflation continued to rise despite significant interest rate increases. However, as the BDBR has explained in its various communiqués, monetary policy works with a lag. Just as in 2022 economic activity recovered to a level above the pre-pandemic level, driven, along with other factors, by the monetary stimulus granted during the pandemic period and subsequent months, so too the effects of the current restrictive monetary policy will gradually take effect. This will allow us to expect the inflation rate to converge to 3.0% by the end of 2024 as is the BDBR’s purpose.Inflation results for January and February of this year showed declining marginal increases (13 bp and 3 bp respectively) compared to the change seen in December (59 bp). This suggests that a turning point in the inflation trend is approaching. In other Latin American countries such as Chile, Brazil, Perú, and Mexico, inflation has peaked and has begun to decline slowly, albeit with some ups and downs. It is to be expected that a similar process will take place in Colombia in the coming months. The expected decline in inflation in 2023 will be due, along with other factors, to lower cost pressure from abroad as a result of the gradual normalization of supply chains, the overcoming of supply shocks caused by the weather, and road blockades in previous years. This will be reflected in lower adjustments in food prices, as has already been seen in the first two months of the year and, of course, the lagged effect of monetary policy. The process of inflation convergence to the target will be gradual and will extend beyond 2023. This process will be facilitated if devaluation pressure is reversed. To this end, it is essential to continue consolidating fiscal sustainability and avoid messages on different public policy fronts that generate uncertainty and distrust. 1 This Report to Congress includes Box 1, which summarizes the trajectory of Banco de la República over the past 100 years. Introducción En 2023 el Banco de la República celebra 100 años de su fundación. Este es un aniversario de gran significado, el cual ofrece la oportunidad de resaltar el aporte que el Banco ha hecho al desarrollo del país. Su trayectoria como garante de la estabilidad monetaria lo ha consolidado como la institución estatal independiente que genera mayor confianza entre los colombianos por su transparencia, capacidad de gestión y el cumplimiento efectivo de las funciones de banca central y culturales encomendadas en la Constitución y la Ley. En una fecha tan importante como esta, la Junta Directiva del Banco de la República (JDBR) hace un reconocimiento a las generaciones de directivos y funcionarios que con su compromiso y dedicación contribuyeron a engrandecer esta institución1. El mandato del Banco de la República se consolidó en la Asamblea Nacional Constituyente de 1991, para cuya integración los ciudadanos tuvieron la oportunidad de elegir a las setenta personas que tendrían como tarea redactar una nueva constitución. Los dirigentes de los tres movimientos políticos más votados fueron elegidos presidentes de la Asamblea, y esta presidencia tripartita reflejó la pluralidad y la necesidad de consenso entre las diferentes fuerzas políticas para sacar adelante la reforma. Entre los asuntos considerados, la Asamblea Nacional Constituyente le otorgó especial importancia a la estabilidad monetaria. Por esta razón decidió incluir el tema de banca central y dotar al Banco de la República de la autonomía necesaria para utilizar los instrumentos a su cargo sin injerencia de otras autoridades. El constituyente entendió que velar por la estabilidad de precios es un deber del Estado y que la entidad responsable de este cometido debe estar consagrada en la Constitución y contar con la capacidad técnica y autonomía institucional necesaria para adoptar las decisiones que considere pertinentes para alcanzar este objetivo fundamental, en coordinación con la política económica general. En particular, el artículo 373 estableció que “el Estado, por intermedio del Banco de la República, velará por el mantenimiento de la capacidad adquisitiva de la moneda”, disposición que coincidía con el esquema de banca central adoptado por países exitosos en el control de la inflación. En 1999, mediante sentencia 481, la Corte Constitucional indicó que “el deber de mantener la capacidad adquisitiva de la moneda no solo se predica de la autoridad monetaria, crediticia y cambiaria, esto es de la Junta del Banco de la República, sino también de quienes tienen responsabilidades en la formulación y ejecución de la política económica general del país” y que “la finalidad constitucional básica del Banco de la República es la protección de la moneda sana, pero esa autoridad debe tomar en consideración en sus decisiones los otros objetivos económicos de la intervención del Estado, como el pleno empleo, pues sus funciones deben coordinarse con la política económica general.” La reforma al Banco de la República concertada en la Constituyente de 1991 y en la Ley 31 de 1992 se puede resumir en los siguientes aspectos: i) asignó al Banco un mandato específico: mantener la capacidad adquisitiva de la moneda, en coordinación con la política económica general; ii) designó a la JDBR como autoridad monetaria, cambiaria y crediticia; iii) otorgó al Banco y a su Junta Directiva un importante grado de independencia frente al Gobierno; iv) prohibió al Banco otorgar crédito al sector privado distinto del financiero; v) estableció que para otorgar crédito al Gobierno se requería del voto unánime de su Junta Directiva, a menos que se trate de operaciones de mercado abierto; vi) determinó que el legislador, en ningún caso, podrá ordenar cupos de crédito a favor del Estado o de los particulares; vii) designó al Congreso, en representación de la sociedad, como principal destinatario del ejercicio de rendición de informes del Banco; y viii) delegó en el presidente de la República la función de inspección, vigilancia y control sobre el Banco de la República. Los miembros de la Asamblea Nacional Constituyente entendieron claramente que los beneficios de una inflación baja y estable se extienden a toda la sociedad y contribuyen al buen funcionamiento del sistema económico. Entre los más importantes cabe mencionar que una inflación baja promueve el uso eficiente de los recursos productivos, al permitir que los precios relativos guíen de mejor forma la asignación de recursos, lo cual promueve el crecimiento económico y aumenta el bienestar de la población. Igualmente, una inflación baja reduce la incertidumbre sobre la rentabilidad esperada de la inversión y sobre el precio futuro de los activos, lo que aumenta la confianza de los agentes económicos, facilita la financiación de largo plazo y estimula la inversión. Una inflación baja evita redistribuciones arbitrarias del ingreso y la riqueza, debido a que los estratos de ingresos bajos de la población no pueden protegerse de la inflación mediante la diversificación de sus activos, y concentran una elevada proporción de su ingreso en la compra de alimentos y otros bienes básicos, ítems que generalmente son los más afectados por los choques inflacionarios2. Por otra parte, una baja inflación facilita las negociaciones salariales, lo cual crea un buen clima laboral y reduce la volatilidad del nivel de empleo. Finalmente, una inflación baja contribuye a que el sistema de impuestos sea más transparente y equitativo, al evitar las distorsiones que la inflación introduce sobre el valor de los activos y de los ingresos que componen la base tributaria. Desde el punto de vista de la autoridad monetaria, uno de los beneficios más relevantes de una inflación baja es la credibilidad que los agentes económicos adquieren en la meta de inflación, lo que la convierte en un ancla nominal efectiva sobre el nivel de precios. Al recibir su mandato, y en uso de su autonomía, el Banco de la República empezó a anunciar metas puntuales de inflación anual a partir de 1992. Si bien en esta primera etapa las metas de inflación propuestas no se lograron cumplir de forma precisa, sí se consiguió imprimirle a la inflación una tendencia descendente, que la llevó desde un nivel del 32,4% en 1990 al 16,7% en 1998. Para aquella época la tasa de cambio se mantenía dentro de una banda, lo cual limitaba la efectividad de la política monetaria, que buscaba cumplir simultáneamente una meta de inflación y un objetivo de tasa de cambio. La crisis asiática se contagió a las economías emergentes y afectó de manera importante a la economía colombiana. La tasa de cambio presentó una fuerte presión a la depreciación al cerrarse el acceso al financiamiento externo en condiciones de un elevado desequilibrio externo. Lo anterior, junto con la falta de flexibilidad cambiaria, impidió hacer una política monetaria contracíclica, lo que condujo a una contracción del PIB del 4,2% en dicho año. En este contexto de desaceleración económica, la inflación anual se redujo al 9,2% a finales de 1999, situándose por debajo de la meta del 15% que se había fijado para ese año. Este episodio reveló plenamente lo costoso que podría ser, en términos de actividad económica, el tener simultáneamente metas para la inflación y para la tasa de cambio. Hacia finales de 1999 el Banco de la República anunció la adopción de un nuevo régimen de política monetaria que denominó Esquema de Inflación Objetivo. Este régimen, conocido internacionalmente como ‘Inflation Targeting,’ venía ganando creciente aceptación en países desarrollados, al haber sido adoptado a partir de 1991 por Nueva Zelanda, Canadá e Inglaterra, entre otros, logrando importantes avances en el manejo de la inflación, sin incurrir en costos en términos de actividad económica. En América Latina, Brasil y Chile también lo acogieron en 1999. En el caso colombiano, el último requisito pendiente por cumplir para adoptar dicho esquema de política era la flexibilidad de la tasa de cambio, la cual se materializó hacia septiembre de 1999, cuando la JDBR decidió abandonar las bandas cambiarias para permitir que la tasa de cambio se determinara libremente en el mercado. De forma coherente con el mandato constitucional, el objetivo fundamental de este nuevo esquema de política consistía en “el cumplimiento de una meta de inflación que contribuya a mantener un crecimiento del producto alrededor de su capacidad potencial”3. Dicha capacidad potencial se entendía como aquel crecimiento del PIB que la economía puede obtener si utiliza plenamente sus recursos productivos. Para cumplir este objetivo la política monetaria debe cumplir necesariamente un papel contracíclico en la economía. Ello porque cuando la actividad económica está por debajo de su potencial y existen recursos ociosos, la autoridad monetaria puede reducir la tasa de interés ante la ausencia de presiones inflacionarias para estimular por esa vía la economía y, de manera inversa, cuando el producto supera su capacidad potencial. Este principio de política, que está inmerso en los modelos para guiar la postura de política monetaria, hace que, en el mediano plazo, sean totalmente compatibles los objetivos del cumplimiento de la meta de inflación y de un nivel de actividad económica compatible con su capacidad productiva. Para alcanzar este propósito, en el esquema de inflación objetivo se utiliza la tasa de interés del mercado monetario (a la cual el banco central suministra liquidez primaria a los bancos comerciales), como el instrumento primordial de política. Con ello se sustituyó la cantidad de dinero como meta intermedia de política monetaria, que el Banco de la República, al igual que varios otros bancos centrales, utilizaron por mucho tiempo. En el caso colombiano, el objetivo del nuevo esquema de política monetaria implicaba, en términos prácticos, que la recuperación de la economía, luego de la contracción ocurrida en 1999, debía lograrse al tiempo que se cumplían las metas decrecientes de inflación establecidas por la JDBR. De manera notable este propósito se cumplió. En la primera mitad de la década del 2000 la actividad económica logró una recuperación importante, hasta alcanzar un crecimiento del 6,8% en 2006. Entretanto, la inflación fue descendiendo gradualmente, en línea con las metas de inflación. Fue así como la tasa de inflación se redujo desde el 9,2% en 1999 al 4,5% en 2006, cumpliendo con la meta de inflación establecida para ese año, mientras que el PIB alcanzó su nivel potencial. Después de lograrse este equilibrio en 2006, la inflación repuntó al 5,7% en 2007, por encima de la meta del 4% fijada para ese año, debido a que el crecimiento del PIB del 7,5% superó su capacidad potencial4. Luego de comprobarse la eficacia del esquema de inflación objetivo en sus primeros años de operación, este régimen de política continuó consolidándose a medida que la JDBR y el equipo técnico ganaron experiencia en su manejo y se incorporaron modelos económicos de última tecnología para diagnosticar el estado presente y futuro de la economía, y evaluar la persistencia de los desvíos de la inflación y sus expectativas con respecto a la meta de inflación. A partir de 2010 la JDBR estableció la meta de inflación anual de largo plazo del 3%, que continúa vigente en la actualidad. La menor inflación ha contribuido a crear un entorno macroeconómico más estable, que ha favorecido el crecimiento económico sostenido, la estabilidad financiera, el desarrollo del mercado de capitales y el funcionamiento de los sistemas de pagos. Gracias a ello se lograron reducciones en la prima por riesgo inflacionario y menores tasas de interés de los TES y de crédito. A su vez, la duración de la deuda interna pública aumentó de forma importante pasando de 2,27 años en diciembre de 2002 a 5,86 años en diciembre de 2022 y la profundización financiera, medida como el nivel de la cartera como porcentaje del PIB, pasó de cerca del 20% a mediados de la década de los noventa a valores superiores al 45% en años recientes, en un contexto saludable de los establecimientos de crédito. Los logros tangibles alcanzados por el Banco de la República en el manejo de la inflación al haber contado con la autonomía que le otorgó la Constitución para cumplir con el mandato de preservar el poder adquisitivo de la moneda, junto con los importantes beneficios que se derivaron del proceso de llevar la inflación a su meta de largo plazo, hacen que el reto que actualmente enfrenta la JDBR de retornar la inflación a la meta del 3% sea aún más exigente y apremiante. Como es bien conocido, a partir de 2021, y especialmente en 2022, la inflación en Colombia volvió a convertirse en un serio problema económico, con elevados costos de bienestar. El fenómeno inflacionario no ha sido exclusivo de Colombia y es así como muchos otros países desarrollados y emergentes han visto alejarse sus tasas de inflación de las metas propuestas por sus bancos centrales5. Las razones de este fenómeno se han analizado en los recientes Informes al Congreso, y en esta nueva entrega se profundiza al respecto con información actualizada. La sólida base institucional y técnica que soporta el esquema de inflación objetivo bajo el cual opera la estrategia de política monetaria le da a la JDBR los elementos necesarios para enfrentar con confianza este difícil reto. Al respecto, en su comunicado del 25 de noviembre la JDBR reiteró su compromiso con la meta de inflación del 3,0%, la cual prevé alcanzar hacia finales de 20246. La política monetaria continuará enfocada en cumplir este objetivo, al tiempo que velará por la sostenibilidad de la actividad económica, tal y como lo ordena la Constitución. Las encuestas a analistas llevadas a cabo en marzo mostraron un incremento importante (del 32,3% en enero al 48,5% en marzo) en el porcentaje de respuestas que sitúan las expectativas de inflación a dos años o más en un rango entre el 3% y 4%. Este es un indicativo claro de recuperación de credibilidad en la meta de inflación a mediano plazo, lo cual guarda coherencia con el anuncio de la JDBR de noviembre pasado. La moderación de la tendencia alcista de la inflación que se observó en enero, y especialmente en febrero, contribuirá a reforzar esta revisión de expectativas de inflación, y ayudará a cumplir los objetivos propuestos. Luego de registrarse una inflación del 5,6% a finales de 2021, la inflación mantuvo una tendencia alcista a lo largo de 2022 debido a las presiones inflacionarias tanto de origen externo, asociadas con las secuelas de la pandemia y las consecuencias del conflicto bélico en Ucrania, como de origen interno, resultantes de: el fortalecimiento de la demanda local; los procesos de indexación de precios estimulados por el aumento de las expectativas de inflación; las afectaciones a la producción de alimentos provocadas por el paro de mediados de 2021, y el traspaso de la depreciación a los precios. Los aumentos del salario mínimo del 10% en 2021 y del 16% en 2022, que en ambos casos superaron la inflación observada y el incremento de la productividad, acentuaron los procesos de indexación al haber establecido un elevado referente de ajuste nominal. De esta forma, la inflación total aumentó al 13,1% a finales 2022. La variación anual de alimentos, que subió del 17,2% al 27,8% entre esos dos años, fue el factor que más influyó en la aceleración del Índice de Precios al Consumidor (IPC). Otro rubro que contribuyó de manera importante a las alzas de precios fue el de regulados, cuya variación anual aumentó del 7,1% en diciembre de 2021 al 11,8% a finales de 2022. Por su parte, la medida de inflación básica sin alimentos ni regulados subió del 2,5% al 9,5% entre finales de 2021 y finales de 2022. El aumento sustancial de la inflación básica muestra que la presión inflacionaria se extendió a la mayoría de los rubros de la canasta familiar, lo cual es característico de procesos inflacionarios con una indexación de precios generalizada, como ocurre en Colombia. La política monetaria empezó a reaccionar tempranamente a estas presiones inflacionarias. Fue así como a partir de su sesión de septiembre de 2021 la JDBR inició un cambio progresivo de la postura de la política monetaria a partir del mínimo histórico del 1,75% de la tasa de interés de política al cual se había llegado para estimular la recuperación de la economía. Este proceso de ajuste prosiguió sin interrupción a lo largo de 2022 y hasta inicios de 2023, cuando la tasa de política monetaria alcanzó el 12,75% en enero pasado, con lo cual acumuló un incremento de 11 puntos porcentuales (pp). El público y los mercados se han mostrado sorprendidos de que la inflación continuara aumentando, a pesar de los significativos incrementos de la tasa de interés. Pero como lo ha explicado la JDBR en sus diversas comunicaciones, la política monetaria actúa con rezago. Así como en 2022 la actividad económica se recuperó hasta alcanzar un nivel superior al de prepandemia, impulsada, entre otros factores, por el estímulo monetario otorgado durante el período de pandemia y de los meses subsiguientes, así también los efectos de la actual política monetaria restrictiva se irán dando paulatinamente, lo que permite esperar que hacia finales de 2024 la tasa de inflación converja hacia el 3%, como es el propósito de la JDBR. Los resultados de la inflación en enero y febrero de este año mostraron incrementos marginales decrecientes (13 pb y 3 pb respectivamente), en comparación con la variación observada en diciembre (59 pb). Esto sugiere que se aproxima un punto de inflexión en la tendencia de la inflación. En otros países de América Latina, como Chile, Brasil, Perú y México, la inflación llegó a su techo y ha empezado a descender lentamente, aunque con algunos altibajos. Es previsible que en Colombia ocurra un proceso similar durante los próximos meses. El descenso previsto de la inflación en 2023 obedecerá, entre otros factores, a las menores presiones de costos externos por cuenta de la progresiva normalización de las cadenas de suministro, a la superación de los choques de oferta por razones de clima y por los bloqueos viales de años anteriores, lo que se reflejará en menores ajustes en los precios de los alimentos, como ya se observó en los primeros dos meses del año y, por supuesto, al efecto rezagado de la política monetaria. El proceso de convergencia de la inflación a la meta será gradual y se extenderá más allá de 2023. Dicho proceso se facilitará si se revierten las presiones a la devaluación, para lo cual resulta esencial que se continúe consolidando la sostenibilidad fiscal y se eviten mensajes en diferentes frentes de la política pública que generan incertidumbre y desconfianza. _______________________________________ 1 Este Informe al Congreso contiene el recuadro 1 que resume la trayectoria del Banco de la República en estos 100 años. การสังเคราะห์งานวิจัยครั้งนี้ ประกอบด้วยการสังเคราะห์ด้วยการวิเคราะห์อภิมานงานวิจัยเชิงปริมาณที่ใช้แบบการวิจัยเชิงทดลอง แบบการวิจัยเชิงสหสัมพันธ์ และการเปรียบเทียบ จำนวน 144 เรื่อง และการสังเคราะห์งานวิจัยด้วยการวิเคราะห์เนื้อหางานวิจัยที่เป็นการวิจัยเชิงคุณลักษณะและการวิจัยแบบบรรยาย จำนวน 179 เรื่อง ในการวิเคราะห์อภิมานเป็นการเก็บรวบรวม และวิเคราะห์ฐานข้อมูล 2 ชุด งานวิจัยส่วนใหญ่ศึกษาด้านการเรียนการสอน การบริหารการศึกษา การศึกษาเพื่อพัฒนาบุคคล และการศึกษาภาคบังคับ แต่มีงานวิจัยน้อยมากเกี่ยวกับการศึกษาเอกชน การศึกษาสำหรับกลุ่มผู้ด้อยโอกาส ศึกษาปัญหาและวิธีแก้ไขเกี่ยวกับกระบวนการเรียนรู้ ผลการวิจัยพบว่ามีปัญหาระดับปานกลาง และมีปัญหาน้อยเกี่ยวกับคุณลักษณะนักเรียนและผู้สำเร็จการศึกษา มีงานวิจัย 10 เรื่องทำการประเมินวิธีการสอน และพบว่าคุณภาพการสอนอยู่ในระดับปานกลางถึงสูง และเสนอแนะให้มีการพัฒนาบุคลากร มีงานวิจัย 58 เรื่อง ทำการวิจัยด้านการบริหารการศึกษา และให้ข้อเสนอแนะว่ายังมีความต้องการจำเป็นเรื่อง การบริหารงานบุคคล การพัฒนาศักยภาพของบุคลากร แต่การวางแผนและการบริหารจัดการโดยทั่วไปมีความเหมาะสม มีงานวิจัย 9 เรื่องรายงานว่าคุณภาพการนิเทศการศึกษามีความเหมาะสม มีปัญหาระดับน้อยถึงปานกลาง และมีรายงานการพัฒนาชุดของตัวบ่งชี้สำหรับวัดความสำเร็จของการนิเทศ มีงานวิจัย 1 เรื่อง ศึกษาด้านการแนะแนว สรุปได้ว่านักเรียน ครูและผู้บริหาร เห็นด้วยกับการใช้ระเบียบข้อบังคับเรื่องวินัย และการลงโทษ และได้โปรแกรมคอมพิวเตอร์สำหรับสร้างธนาคารข้อสอบสองโปรแกรม มีงานวิจัย 6 เรื่อง ทำการวิจัยเกี่ยวกับวิธีวิทยาการวิจัย โดยศึกษาวิธีการวัดและประเมินผลการศึกษา และการเพิ่มอัตราตอบกลับแบบสอบถาม มีงานวิจัย 5 เรื่อง ทำการวิจัยเกี่ยวกับจิตวิทยาสังคม และพฤติกรรมศาสตร์ (แม้ว่าจะเป็นการวิจัยเชิงปริมาณ แต่ผู้ทำวิจัยเสนอรายงานในเชิงพรรณาเท่านั้น) และในกลุ่มสุดท้ายมีงานวิจัย 16 เรื่อง ศึกษาเชิงพรรณา/บรรยาย สภาวะ/วิถีชีวิต/ประเด็นสำคัญ ในชุมชน และการบริหารองค์กร จากข้อค้นพบในการวิจัย มีข้อเสนอแนะที่สำคัญ 4 ประการ ประการแรก ควรกระตุ้น และสนับสนุนให้นักการศึกษาทำการวิจัยขั้นสูงในสาขาที่มีการวิจัยน้อยให้มากขึ้น ประการที่สอง ควรมีการนำผลการวิจัยตามตัวแปรที่ให้ค่าเฉลี่ยขนาดอิทธิพลสูงไปใช้ให้เกิดผลในทางปฏิบัติ เพื่อส่งเสริมการเรียนรู้ของผู้เรียน และการขยายบริการการศึกษาสำหรับกลุ่มผู้ด้อยโอกาส ประการที่สาม องค์กรที่เกี่ยวข้องกับการวิจจัยควรได้รับการส่งเริมให้จัดกิจกรรมการฝึกอบรมทำวิจัย และจัดกิจกรรมการเผยแพร่ผลงานวิจัยย่างมีประสิทธิภาพ ตลอดจนจัดให้มีการพัฒนาระบบสารสนเทศเกี่ยวกับผลการวิจัย และประการสุดท้าย สำหรับการวิจัยต่อไปในอนาคต นักวิจัยควรต้องรายงานค่าสถิติที่เป็นผลการวิจัยเพื่อประโยชน์ในการวิเคราะห์อภิมานต่อไป ควรมีการพัฒนาเทคนิคการวิเคราะห์อภิมานให้ได้ผลการวิเคราะห์ถูกต้องและแปลความหมายได้ครอบคลุมมากยิ่งขึ้น และควรมีการนำวิธีการวิเคราะห์ทางสถิติขั้นสูง เช่น ลิสเรล เอช.แอล.เอ็ม มาใช้เพื่อให้ได้ผลการสังเคราะห์ที่ดียิ่งขึ้นกว่าเดิม

1. Macroeconomic Summary In December, headline inflation (13.1%) and the average of the core inflation measures (10.3%) continued to trend upward, posting higher rates than those estimated by the Central Bank's technical staff and surpassing the market average. Inflation expectations for all terms exceeded the 3.0% target. In that month, every major group in the Consumer Price Index (CPI) registered higher-than-estimated increases, and the diffusion indicators continued to show generalized price hikes. Accumulated exchange rate pressures on prices, indexation to high inflation rates, and several food supply shocks would explain, in part, the acceleration in inflation. All of this is in a context of significant surplus demand, a tight labor market, and inflation expectations at different terms that exceed the 3.0% target. Compared to the October edition of the Monetary Policy Report, the forecast path for headline and core inflation (excluding food and regulated items: EFR) increased (Graphs 1.1 and 1.2), reflecting heightened accumulated exchange rate pressures, price indexation to a higher inflation rate (CPI and the producer price index: PPI), and the rise in labor costs attributed to a larger-than-estimated adjustment in the minimum wage. Nevertheless, headline inflation is expected to begin to ease by early 2023, although from a higher level than had been estimated in October. This would be supported initially by the slowdown forecast for the food CPI due to a high base of comparison, the end anticipated for the shocks that have affected the prices of these products, and the estimated improvement in external and domestic supply in this sector. In turn, the deterioration in real household income because of high inflation and the end of the effects of pent-up demand, plus tighter external and domestic financial conditions would contribute to diluting surplus demand in 2023 and reducing inflation. By the end of 2023, both headline and core (EFR) inflation would reach 8.7% and would be 3.5% and 3.8%, respectively, by December 2024. These forecasts are subject to a great deal of uncertainty, especially concerning the future behavior of international financial conditions, the evolution of the exchange rate, the pace of adjustment in domestic demand, the extent of indexation of nominal contracts, and the decisions taken regarding the domestic price of fuel and electricity. In the third quarter, economic activity surprised again on the upside and the growth projection for 2022 rose to 8.0% (previously 7.9%). However, it declined to 0.2% for 2023 (previously 0.5%). With this, surplus demand continues to be significant and is still expected to weaken during the current year. Annual economic growth in the third quarter (7.1 % SCA)1 was higher than estimated in October (6.4 % SCA), given stronger domestic demand specifically because of higher-than-expected investment. Private consumption fell from the high level witnessed a quarter earlier and net exports registered a more negative contribution than anticipated. For the fourth quarter, economic activity indicators suggest that gross domestic product (GDP) would have remained high and at a level similar to that observed in the third quarter, with an annual variation of 4.1%. Domestic demand would have slowed in annual terms, although at levels that would have remained above those for output, mainly because of considerable private consumption. Investment would have declined slightly to a value like the average observed in 2019. The real trade deficit would have decreased due to a drop in imports that was more pronounced than the estimated decline in exports. On the forecast horizon, consumption is expected to decline from current elevated levels, partly because of tighter domestic financial conditions and a deterioration in real income due to high inflation. Investment would also weaken and return to levels below those seen before the pandemic. In real terms, the trade deficit would narrow due to a lower momentum projection for domestic demand and higher cumulative real depreciation. In sum, economic growth for all of 2022, 2023, and 2024 would stand at 8.0%, 0.2% and 1.0%, respectively (Graph 1.3). Surplus demand remains high (as measured by the output gap) and is expected to decline in 2023 and could turn negative in 2024 (Graph 1.4). Although the macroeconomic forecast includes a marked slowdown in the economy, an even greater adjustment in domestic absorption cannot be ruled out due to the cumulative effects of tighter external and domestic financial conditions, among other reasons. These estimates continue to be subject to a high degree of uncertainty, which is associated with factors such as global political tensions, changes in international interest rates and their effects on external demand, global risk aversion, the effects of the approved tax reform, the possible impact of reforms announced for this year (pension, health, and labor reforms, among others), and future measures regarding hydrocarbon production. In 2022, the current account deficit would have been high (6.3 % of GDP), but it would be corrected significantly in 2023 (to 3.9 % of GDP) given the expected slowdown in domestic demand. Despite favorable terms of trade, the high external imbalance that would occur during 2022 would be largely due to domestic demand growth, cost pressures associated with high freight rates, higher external debt service payments, and good performance in terms of the profits of foreign companies.2 By 2023, the adjustment in domestic demand would be reflected in a smaller current account deficit especially due to fewer imports, a global moderation in prices and cost pressures, and a reduction in profits remitted abroad by companies with foreign direct investment (FDI) focused on the local market. Despite this anticipated correction in the external imbalance, its level as a percentage of GDP would remain high in the context of tight financial conditions. In the world's main economies, inflation forecasts and expectations point to a reduction by 2023, but at levels that still exceed their central banks' targets. The path anticipated for the Federal Reserve (Fed) interest rate increased and the forecast for global growth continues to be moderate. In the fourth quarter of 2022, logistics costs and international prices for some foods, oil and energy declined from elevated levels, bringing downward pressure to bear on global inflation. Meanwhile, the higher cost of financing, the loss of real income due to high levels of global inflation, and the persistence of the war in Ukraine, among other factors, have contributed to the reduction in global economic growth forecasts. In the United States, inflation turned out to be lower than estimated and the members of the Federal Open Market Committee (FOMC) reduced the growth forecast for 2023. Nevertheless, the actual level of inflation in that country, its forecasts, and expectations exceed the target. Also, the labor market remains tight, and fiscal policy is still expansionary. In this environment, the Fed raised the expected path for policy interest rates and, with this, the market average estimates higher levels for 2023 than those forecast in October. In the region's emerging economies, country risk premia declined during the quarter and the currencies of those countries appreciated against the US dollar. Considering all the above, for the current year, the Central Bank's technical staff increased the path estimated for the Fed's interest rate, reduced the forecast for growth in the country's external demand, lowered the expected path of oil prices, and kept the country’s risk premium assumption high, but at somewhat lower levels than those anticipated in the previous Monetary Policy Report. Moreover, accumulated inflationary pressures originating from the behavior of the exchange rate would continue to be important. External financial conditions facing the economy have improved recently and could be associated with a more favorable international context for the Colombian economy. So far this year, there has been a reduction in long-term bond interest rates in the markets of developed countries and an increase in the prices of risky assets, such as stocks. This would be associated with a faster-than-expected reduction in inflation in the United States and Europe, which would allow for a less restrictive course for monetary policy in those regions. In this context, the risks of a global recession have been reduced and the global appetite for risk has increased. Consequently, the risk premium continues to decline, the Colombian peso has appreciated significantly, and TES interest rates have decreased. Should this trend consolidate, exchange rate inflationary pressures could be less than what was incorporated into the macroeconomic forecast. Uncertainty about external forecasts and their impact on the country remains high, given the unpredictable course of the war in Ukraine, geopolitical tensions, local uncertainty, and the extensive financing needs of the Colombian government and the economy. High inflation with forecasts and expectations above 3.0%, coupled with surplus demand and a tight labor market are compatible with a contractionary stance on monetary policy that is conducive to the macroeconomic adjustment needed to mitigate the risk of de-anchoring inflation expectations and to ensure that inflation converges to the target. Compared to the forecasts in the October edition of the Monetary Policy Report, domestic demand has been more dynamic, with a higher observed level of output exceeding the productive capacity of the economy. In this context of surplus demand, headline and core inflation continued to trend upward and posted surprising increases. Observed and expected international interest rates increased, the country’s risk premia lessened (but remains at high levels), and accumulated exchange rate pressures are still significant. The technical staff's inflation forecast for 2023 increased and inflation expectations remain well above 3.0%. All in all, the risk of inflation expectations becoming unanchored persists, which would accentuate the generalized indexation process and push inflation even further away from the target. This macroeconomic context requires consolidating a contractionary monetary policy stance that aims to meet the inflation target within the forecast horizon and bring the economy's output to levels closer to its potential. 1.2 Monetary Policy Decision At its meetings in December 2022 and January 2023, Banco de la República’s Board of Directors (BDBR) agreed to continue the process of normalizing monetary policy. In December, the BDBR decided by a majority vote to increase the monetary policy interest rate by 100 basis points (bps) and in its January meeting by 75 bps, bringing it to 12.75% (Graph 1.5). 1/ Seasonally and calendar adjusted. 2/ In the current account aggregate, the pressures for a higher external deficit come from those companies with FDI that are focused on the domestic market.
