Анотація:

La neuromodulation des racines sacrées (NMS) est un traitement de l'incontinence urinaire et fécale proposé aux patients réfractaires aux traitements médicaux. Son efficacité à été démontrée dans le cadre du traitement de l'incontinence urinaire et fécale. Les mécanismes d'action de la NMS restent cependant mal connus. Dans un premier temps, nous avons voulu évaluer l'effet de la NMS sur la sensibilité viscérale dans un modèle de rat sain anesthésié. La sensibilité viscérale était évaluée par mesure de la variation de pression artérielle en réponse à a des distensions colorectales (DCR). La NMS appliquée chez les rats diminuait la variation de pression artérielle en réponse aux DCR. Cet effet était aboli par l'injection intrathécale de naloxone. Nous avons montré par immunohistochimie que la NMS réduisait l'activation neuronale dans des zones impliquées dans la sensibilité viscérale. Enfin, nous avons montré que la NMS augmentait l'internalisation du récepteur Mu opioïde en conditions de DCR. Dans un second temps, nous avons voulu évaluer l'effet de la NMS sur la sensibilité viscérale dans un modèle de sensibilisation croisée vessie-côlon chez le rat anesthésié. Une augmentation de la variation de pression artérielle était observée en comparaison avec des rats contrôles. La NMS diminuait cette variation et l'injection d'un antagoniste des récepteurs Mu opioïdes prévenait cette variation. Une diminution de l'activation neuronale était observée au niveau de la corne dorsale de la moelle épinière sacrée. Enfin, nous avons développé en parallèle un modèle de NMS chronique chez le Rat en réalisant des stimulateurs implantables. La validation fonctionnelle de ce modèle à été faite en mesurant la fréquence de miction après 3 jours de NMS. Une diminution de la fréquence de miction était observée
Fecal incontinence remains a therapeutic problem in patients when conservative measures fail and sphincter repair is unsuccessful or inappropriate. Sacral nerve stimulation (SNS) is an alternative surgical approach proposed initially to treat urinary incontinence, and has been used for the past decade as a successful treatment of fecal incontinence. Despite its overall efficacy, the mechanisms involved in this symptomatic effect remain unclear. The aim of this study was to evaluate the effect of SNS on rat visceral sensitivity. In an anesthetized rat model, mean arterial pressure variation was recorded in response to colorectal distension (CRD) in sham and effective SNS groups. Healthy and cross-organ sensitization models were studied. Characterization of the pathways involved in the effect of SNS was made by injecting pharmacological agents. Central neuronal activation and p-opioid receptor internalization were assessed by immunohistochemistry. In both models, SNS reduced mean arterial pressure variation. This effect was prevented by opioid receptor antagonist. CRD induced a rise in sacral dorsal horn of the spinal cord, parabrachial nucleus and solitary tract nucleus that was prevented by SNS. Finally, SNS induced an increase of p-opioid receptor internalization in a context of CRD. Concomitantly, we developed an implantable stimulating device in order to study chronic SNS on awaken rats. Implantation technique was validated by obtaining tail tremor response and by performing computed tomography imaging. Micturition frequency was assessed in rats undergoing 3 days of bipolar or unipolar SNS. In both groups, SNS reduced micturition frequency

Анотація:

La sclérose en plaques (SEP) est une maladie auto-immune du système nerveux central générant de nombreux symptômes neurologiques, parmi lesquels la douleur chronique qui est très invalidante et fréquente. A ce jour, la SEP est une maladie incurable avec une étiologie complexe, multifactorielle et encore mal comprise. De nombreuses données suggèrent que les polyphénols végétaux pourraient avoir des bénéfices thérapeutiques en régulant le stress oxydant et la neuroprotection dans la SEP. Dans ce contexte, ce travail de thèse a pour objectif d’évaluer l'effet d’un traitement curatif chronique à l'extrait de pépins de raisin (GSE pour Grape Seed Extract) dans un modèle murin reproduisant certaines des caractéristiques cliniques et neuropathologiques de la SEP, l'encéphalomyélite auto-immune expérimentale (EAE). Dans un premier temps, la composition biochimique du GSE a été évaluée. Par la suite, le traitement des souris EAE par le GSE 10 jours après l’induction du modèle (J10) a montré une amélioration à la fois du score neurologique et des troubles sensitifs chez les souris. Des analyses biochimiques et moléculaires au niveau du cerveau et de la moelle épinière ont montré dès J20 une correction des anomalies du stress oxydant permettant une restauration des altérations de la myéline, de la prolifération astrogliale et microgliale et des niveaux d’expression des sirtuines. Enfin, une analyse protéomique a permis de confirmer ces résultats et d’envisager des mécanismes d’action bénéfiques supplémentaires du GSE, notamment la correction de la dégradation des lipides. L’ensemble des effets du GSE décrits au cours de cette thèse soutient fortement l'idée que le GSE pourrait être une approche thérapeutique efficace pour le traitement de la SEP
Multiple sclerosis (MS) is an autoimmune disease of the central nervous system leading to many neurological symptoms, among which chronic pain is common and very disabling. To date, MS is an incurable disease with a complex, multifactorial and still poorly understood etiology. Numerous evidence suggest that plant polyphenols may have therapeutic benefits in regulating oxidative stress and providing neuroprotection in MS. In this context, this thesis work aimed to evaluate the effect of a chronic curative treatment with grape seed extract (GSE) in a mouse model reproducing some of clinical and neuropathological features of MS, the experimental autoimmune encephalomyelitis (EAE).First, the biochemical composition of GSE was evaluated. Subsequently, the treatment with GSE initiated from day 10 post-induction (D10) showed both an improvement in the neurological score and sensory disorders in mice. Biochemical and molecular analyzes in the brain and spinal cord showed from D20 a correction of oxidative stress abnormalities allowing restoration of myelin alterations, astroglial and microglial proliferation and levels of sirtuins expression. Finally, a proteomic analysis allowed to confirm these results and to identify new additional beneficial effect of GSE, such as the correction of lipid degradation. All the effects of GSE described during this thesis strongly supports the idea that GSE could be an effective therapeutic approach for the treatment of MS

Анотація:

Le syndrome de l'intestin irritable (SII) touche près de 10% de la population. Nous avons utilisé un modèle animal de SII induit par le butyrate développé au laboratoire afin de décortiquer les mécanismes de l'hypersensibilité colique (HSC) dans le SII. Le blocage du NGF par des anticorps anti-NGF prévient l'HSC induite par le butyrate, évaluée par le test de distension colorectale. Le NGF, quantifié par immunohistochimie (IHC), est surexprimé dans les ganglions rachidiens dorsaux (GRD) innervant le côlon des rats butyrate. Le blocage des canaux ASIC par amiloride prévient l'HSC induite par le butyrate. L'expression des ARNm ASIC1a et ASIC1b, évaluée par RT-PCR, est augmentée dans les GDR des rats butyrate. Cette augmentation est corrélée à une augmentation de l'expression de la protéine ASIC1A dans les neurones nociceptifs, quantifiée par IHC. Le blocage du NGF par des anticorps anti-NGF prévient la surexpression de ASIC1A dans les GRD. L'absence de variation d'expression du NGF et de ASIC1A au niveau colique suggère que ces moléciles ont une implication dans l'élément présynaptique plutôt que dans les terminaisons libres coliques. L'étude de l'expression spinale de la protéine Fos après stimulation des fibres coliques a montré que l'HSC induite par le butyrate est associée à une activation spécifique des segments thoraciques T10-T11-T12 de la moëlle épinière (MEp). Le blocage spinal du canal ASIC1A par la PcTx1 prévient l'HSC induite par le butyrate. L'expression de ASIC1a, évaluée par RT-PCR et Western blot, est augmentée dans la MEp des rats butyrate. Comme à la périphérie, l'expression de ASIC1a est modulée par le NGF puisque le blocage du NGF prévient la surexpression des ARNm et de la protéine ASIC1A dans la MEp des rats butyrate. En conclusion, ce travail de thèse suggère que le NGF et le canal ASIC1A jouent un rôle critique dans le développement de douleurs viscérales en contribuant à la fois à la sensibilisation périphérique et centrale.

AbstractThe introductory chapter ‘Staying proximate’ welcomes the reader to stay with more-than-human relations in present times of ecological crisis, known as the age of the Anthropocene. The chapter joins feminist, postcolonial, and Indigenous environmental scholars’ call for more nuanced alternatives to the Anthropocenic imaginary, ones that attend to the multiplicity, difference, and uneven distribution of more-than-human responsibilities, vulnerabilities, and sufferings in the world. We seek alternatives to the distancing, generalising, and even apocalyptic imaginaries of the Anthropocene by engaging with mundane beings, relations, and places in the north. By gathering around uncomfortable concerns, we develop modes of proximity as affirmative entry points underlining the commitment to stay with the trouble in caring, sensitive, and thoughtful ways. We suggest openness, affinity, engagement, irritation, middleness, and scopic modes of attuning to and engaging with more-than-human worlds. It is these modes of attuning to our proximate relations that provide a radical standpoint of proximity that intensifies, enriches, and complicates our research inquiries in such times of all-encompassing ecological turmoil.

Researchers forecast that autonomous vehicles will reduce accidents by 90%. The ethical interplay of autonomous vehicles, cybersecurity vulnerabilities, and societal demands further complicates the governments' decisions on self-driving cars. Autonomous vehicles have safety, economic, and societal benefactors; however, offsetting the unintended consequences of technologies requires governance initiatives to address cybersecurity threats and vulnerabilities. At issue is the lack of ethical consideration for autonomous vehicles from a cybersecurity perspective, given the amount of personal and sensitive data created, used, and stored by autonomous vehicles. The advancement of autonomous cars is increasingly reliant on software, consequently making them vulnerable to cyber-attacks. Software vulnerabilities remain a top cyber-attack vector in all industries. The deliberation concerning ethics for autonomous cars is ebb and flow, especially as opposing sides increase arguments as self-driving vehicles reach a reality.

Abstract This chapter considers the most serious objection to isomorphism invariance: overgeneration. As expressed by critics such as Feferman and Bonnay, this objection holds that isomorphism invariance renders logic in some sense sensitive to mathematics. In particular, if we accept isomorphism invariance, there will be sentences that are logically true just if the Continuum Hypothesis is true. It is argued here that the objection fails. The authors offer various precisifications and find that none of them succeeds in establishing a conclusion that should trouble the isomorphism invariantist. This discussion allows us to get much clearer on quite what it means for logic to be sensitive to mathematics, and offers much to help the defender of second-order logic.

Shows about psychic-mediums emphasize the consoling/healing function of middle-aged women as they allay lingering cultural anxiety regarding women wielding power. Defined as “sensitive,” women address issues of personal loss and individual trauma by presenting positive “messages from spirit” or helping troubled spirits move on. Although they communicate with the dead, they are normalized by being depicted interacting with middle class people in well-lit domestic spaces. Sometimes, though, older psychic-mediums openly challenge traditional definitions of gender as they supplement and correct male-dominated, empirical police methods by channeling victims of crime.

John McGahern was sensitive to the political shape of the communities he imagined in his fiction. These communities were scarred by the violence of historical events, the significance of which remained in recent memory. McGahern often chose the family as the social unit by which to gauge the effect of public events on the individual consciousness, and this rendered mass events, like the War of Independence and Civil War, as conflicts between people who could not escape their aftermath, even generations later. This chapter considers how for McGahern, as well as for the generation before him, the idea of the republic represented not only national separation from a larger power, but was the troubled symbol of a society that was divided uncomfortably between loyalty to community and to the state.

Sleep complaints are extremely common in clinical practice. They may present as a clinical feature or symptom of another co-occurring disorder or may represent a sleep–wake disorder. There are multiple types of sleep–wake disorders, which may involve, for example, trouble sleeping at night (insomnia), problems with breathing during sleep (sleep apnea), or abnormal events during sleep (nightmares). This chapter focuses on the assessment of insomnia disorder in adults. It begins with a review of the nature of the disorder, which is followed by a review of clinical assessment instruments designed for the assessment purposes of (a) diagnosis, (b) case conceptualization and treatment planning, and (c) treatment monitoring and evaluation. Recommendations are included for instruments with the greatest scientific support and for assessing insomnia disorder in a clinically sensitive manner.

Electronic nose systems are used to deliver a pattern response to a listed odor, and pattern recognition software is used to perform odor recognition and discrimination by using a series of sensors. The method of electronic noses generally includes time taking measurements in a non-standard test and error process. The sensory panel problem can be solved by electronic nose. For this purpose, a sensor model is used to design sensor array. The generated signal of these sensor array is used further to classify a mixture of two gases using principle component analysis (PCA)-based classification analysis. During classification, the efficiency of PCA classification has been checked over the different signal preprocessing technique. Continuous real monitoring of odor is done at specific sites in the field over hours, days, weeks, or even months. An electronic machine can also avoid many other troubles linked with the employ of human panels. Each and every variability, adaptation (becoming minimum sensitive during extended exposure), and revelation to hazardous compounds all come to mind.

Abstract Pipe-sticking during drilling operations causes severe difficulties, including economic losses and safety issues. Therefore, stuck-pipe predictions are an important tool to preempt this problem and avoid the aforementioned troubles. In this study, we have developed a prediction technique based on artificial intelligence, in collaboration with industry, the government, and academia. This technique was an unsupervised learning model built using an encoder-decoder, long short-term memory architecture. The model was trained with the time series data of normal drilling operations and based on an important hypothesis: reconstruction errors between observed and predicted values are higher around the time of pipe sticking than during normal drilling operations. The trained model was then applied to 34 actual stuck-pipe events, where it was found that reconstruction errors increased prior to the pipe sticking in some cases (thereby partly confirming our hypothesis) and were sensitive to large variations in the drilling parameters.

Abstract Case studies have shown the benefits of applying a unique wellbore stabilization technology to solve lost circulation events occurring in varying pressurize, permeable, and fragile formations. Leveraging this technology can increase successful drilling in geologically complex areas while avoiding conventional solutions that are both costly and and, in some cases, do not solve the causes of non-productive time. A unique wellbore stabilization technology has been proven effective in offshore wells in areas that are known to present small vugs, natural fissures, formation failure, wellbore erosion, and the associated drilling problems of lost circulation, poor cementing quality, problems running pipe, etc. When compared with the cost and operational lack of success offered by conventional "solutions" versus the relative low-cost and operational successes of the unique wellbore stabilization technology, the choice is clear – where use is appropriate, the stabilization technology has provided a greater success rate and trouble-free drilling operations at a lower cost than traditional solutions. The unique wellbore stabilization product is designed to address microfractures (as small as 3 µm) up to much larger fractures (3,000 µm). The product can be added to oil-based or water-based fluids at functional levels without adversely impacting the fluid rheology. Once in the drilling fluid, the stabilization product forms a pressure-sensitive layer on the formation wall to isolate the drilling fluid from the wellbore and stop fluid invasion that leads to formation failure, uncontrollable lost circulation, and well control issues. Once drilling is completed, the drop in pressure differential of the wellbore fluid allows the protective particles in the drilling fluid to lift off with the initiation of production. In the Campeche wells, one early study showed a 36% reduction in drilling time and 24% reduction in cost. This paper discusses the various options explored and detail the technical analysis that preceded the application of the wellbore stability technology. Case studies are presented to illustrate the effectiveness of the fluid technology in drilling three wells which encountered variable pressure regimes, from the highly pressurized Paleocene at around 5,000 m to underlying low-pressure sandstones. These three wells and their offsets represent the successful application of this low-invasion technology in major drilling basins where lost circulation events would normally incur major operational expenses and non-productive time (NPT).

Abstract A new approach to real-time automated drilling fluid measurement systems leads to enhanced collaboration and decision making between field and remote operations personnel. The approach is based on straight forward technology that can easily be understood by all, operated by field personnel, and robust enough for fast paced unconventional operations. The sensor information offers real-time actionable data without the demand to replicate the traditional daily mud report, streamlining wellsite activities for drilling fluid treatment as fluid conditions change. The automated drilling fluid measurement system (mud skid) was designed for reliability and simplicity. The idea is to use trending data over the single data point mud check to drive real time decision making across teams. Rather than replace the fluids specialist on location it enhances their role to drive operational excellence and consistency in their day-to-day rig support. Everyday across multiple rigs, drilling fluids specialists use the data to adjust treatment schedules, optimize activities, and capture unplanned events as early as possible to lower treatment cost with the support of the remote operations center (ROC). The ROC's monitoring activities drive fluid enhancements across multiple locations via data transparency and analysis, sharing of best practices, and event detection. With a few proven and fit for purpose sensors, the mud skid adds value within the cost limits of the unconventional well market.There is an opportunity for value generation via the use of economical drilling fluid sensors where costs traditionally limit full-scale automated drilling fluid measurements. The mud skid unit provides sufficient data to generate value without prohibitive expenses. The mud skid unit and the data collected are now part of standard operations across multiple rigs and basins. The data streams, available in real-time to everyone from the derrick hand to the drilling engineer, have created a new workflow for decision-making, fluid maintenance, trouble-shooting, and early event detection. As more events are captured by the sensors, there are additional opportunities to train machine learning algorithms and develop predictive models.This paper will review the development process, deployment, and delivery of the mud skid unit with case histories demonstrating the advantages and opportunities for simple, real-time data streams.

Examines non-conventional techniques for monitoring curvatures, displacement, or strains in buried pipelines. Internal devices, external devices, and fiber optic techniques were examined. Feasibility of each system is discussed and the most promising are identified. Two companion studies 'Guidelines Pipeline Strain Monitoring by Conventional Means' (Reference 1) and 'A Proposed Model for the Intervention Decision Making Process in Pipeline Movement Situations' (Reference 2) have already been completed, and an effort to determine appropriate failure criteria for pipelines in areas of soil instability is currently underway. The objective of this study is to describe methods of pipeline monitoring which are, for the most part, still in the conceptual and/or development phases. It is very likely that the techniques described herein will require extensive validation efforts and significant financial support before they will become reliable tools for routine use Improved pipeline strain monitoring techniques are needed because the conventional techniques (strain gages, inclinometers, and topographical surveying) meet all of the industry's needs. The conventional techniques are expensive, labor intensive, and require access to the pipeline for installation . As such they are limited in use to localized trouble spots that are known to be a problem. None of these techniques could be practically applied to a whole pipeline for any-time monitoring. Techniques that will better satisfy the industry's needs must be adequately sensitive, applicable to the whole pipeline, available at all times (or at least at reasonably frequent intervals), and capable of use without the need to excavate the pipeline or to interfere with its operation.Several non-conventional techniques for monitoring curvatures, displacements, or strains in buried pipelines that may meet these requirements are discussed herein. These fall into three generic classes: internal devices (instrumented pigs), external devices (involving moving a detecting device over the pipeline right-of-way), fiber-optic cables attached to the pipeline over its entire length.
