Even if it involves alternating between a filling phase and an emptying phase, the normal micturition cycle cannot be summed up as a binary operation but involves the constant consideration of multiple factors: the filling level of the bladder reservoir, the safety of the environment in which we live, the emotional context in which we evolve and the social constraints to which we are subjected.We now know that there are alterations and/or modifications in brain activity and connectivity, as well as changes in the regulation of the autonomic nervous system, in certain types of lower urinary tract dysfunction - notably in overactive bladder or urge urinary incontinence and in certain types of voiding dysfunctions. Among the therapies available today, electrical modulation/stimulation therapies (tibial neurostimulation and sacral neuromodulation) appear able to normalize and/or modify brain activity and connectivity, as well as ANS balance. They could thus provide at least a partial response to some of the etiopathogenies underlying these lower urinary tract dysfunctions. However, the deployment and positioning of these electrical modulation/stimulation therapies are still limited by an incomplete understanding of their mechanisms of action, imperfect identification of the indications and populations most likely to benefit from these therapies, a lack of consensus on the setting of the electrical current delivered, and a lack of medium and long-term evaluation. In the first part, we questioned the indications for these therapies, and particularly their place as a preventive approach for lower urinary tract dysfunctions due to spinal cord injury. We also questioned the relation, in terms of efficacy, between transcutaneous tibial neurostimulation and sacral neuromodulation, to better support patients in shared medical decision-making processe. Finally, we developed the first tool to predict the success of sacral neuromodulation as a treatment for voiding dysfunction. In the second part, we questioned the mechanisms of action, and more specifically the changes in the balance of the autonomic nervous system in response to an acute S3 sacral root stimulation.In the third part, we questioned the mid-term follow-up (5 years) after definitive implantation of sacral neuromodulation in a geographic population pool, looking for risk factors for discontinuation of follow-up. These data, although still to be supplemented by future research projects, will enable us to further optimize electrical modulation/stimulation therapies in the management of neurogenic and non-neurogenic lower urinary tract dysfunctions

Our work concerned 6 therapeutic targets. (1) Motivational enhancement : we developed a smoking cessation motivation scale. (2) Detoxification : we showed that bupropion is effective in smoking cessation. Sleep is improved by carbamates during alcohol detoxification and by nicotine patches during nicotine withdrawal. (3) Cognitive disorders : alcoholics recover after 6 months of sobriety. (4) Psychiatric comorbidity : alcohol induced and independent maniac/hypomaniac disorders are clinically similar. (5) Social distress : we developed a social distress scale in alcoholics. (6) Relapse prevention : amilsupride showed a lack of efficacy in alcoholics. We aimed at examining if the use of mental health services modified psychiatric comorbidity with substance use disorders and at studying substance use in response to psychiatric disorders (prevalences and associated factors). Our two first studies showed that the use of mental health services was associated with a decreased risk of problematic substance use among subjects with several non substance-related psychiatric disorders. The third study showed that substance use in response to psychiatric disorders was frequent. This use, with healthcare use or not, was more frequent among men, young people, low educational levels and those living alone. Subjects aged 30 or younger were more likely to use substances, without consulting a medical doctor. If mental health qtatus was severe, the combination of substance use and healthcare was more frequent. These studies might define and target interventions for drug problems prevention

This dissertation first part is an especially North-American comprehensive review about therapeutic alliance empirical studies that relate alliance to patient psychotherapy outcome. The second literature review emphasizes the ways and tools using for Pervasive Developmental and conduct disorders intermediate and long outcomes. The two-year prospective own study in a French Child Psychatry Day-Hospital assesses relationship between therapeutic alliance and clinical progress. Thirty-three 3-10 year-old children (18 with pervasive developmental disorders and 15 with mixed behaviour disorders) were assessed regarding their symptom status and development progress, while receiving multi modal intervention package. Independantly, the treatment staff assessed the therapeutic alliance with the parents by standardized ratings on the French modified Helping Alliance questionnaire from Luborsky. The statistical study shows that a parent's good therapeutic alliance correlate significantly with the social functionning improvement but not regarding the symptom status. As North-American therapeutic alliance adult's studies, this first child psychatry unit study seems to show that the parent therapeutic alliance is a predictor factor of child outcome independently of gender, age and pathological child status

Re-education practices are rarely pervaded with any philosophical thought, since those evry practices seem to hinder the development of suchan idea. Nevertheless, some elements may indicate thet more and more people are taking into account the need to implement such a reflexion within medical care so as to perceive re-education itself as a philosophical problem. The re-education of mentally impaired children implies mutually dependent areas of knowledge, namely : phenomenology, the philosophy of education, and the philosophy of cognitive sciences focusing on mental processes. Our work, which is based on this interdependence, consists in providing a conceptual framework thet we deem to be paramount for the support of our specific area of medical care. Mentally impaired children are involved in pathological events which definitely affect their lives. Those events tragically belong to a logical process involving trauma, cerebral plasticity, and resilience whose connections have to be scrutinized. Furthermore, the quality of the human relations involved in medical care relies on our recognition of the paradoxical value found in the strong desire for the children to maintain their autonomy. Finally, a philosophy of re-education cannot be complete without being linked to a philosophy of education

The cutaneous pigmentation mainly raises of a biochemical phenomenon called melanogenesis. It consists in producing melanin pigments which constitute for skin a natural protection against harmful effects of UV. Various dysfunction of melanogenesis due to external agressions (UV), to the hormonal disturbances or ageing result in appearance of hyperpigmentation spots which are particularly inesthetic. Ln theses cases, the use of chemical or cosmetics whitening agent can be helpfull to cure these disorders. Tyrosinase is the key enzyme implied in the first two stages of melanogenesis, then it represent an ideal target to achieve this goal. For several years the population concerned has had recourse to some tyrosinase inhibitors, but their use are not very effective or delicate because of unexpected side effects. We have in our program developped and synthetized polyphenols such aurone, chromones or cafeic and feruIic acid derivatives as tyrosinase inhibitor. Their in vitro evaluation on three models and particularly on human tyrosinase of cultivated melanocytes, permit us to identify some aurones, and some cafeic and ferulic acid derivatives with strong potential as human tyrosinase inhibitor. Structure activity relationship proves that their activity are overal related to hydroxyl group which allow them probably to mimic the enzyme substrate (L- Tyrosin or L-Dopa) or to chelate copper enzyme. Moreover, the selected molecules have natural occurence and are devoid of toxic effect. This predict a possible development for a use in dermocosmetics

The understanding of eating disorders's obese subjects and the treatment of obesity have changed during the last years. We have tried accross three studies to precise this evolution and to propose future research axis. The first study confirms that Binge Eating Disorder subjects correspond to a distinct subgroup of obese people who reports higher degree of general psychopathology and argue for the involvement of emotional eating and alexithymia in eating disorders in obese women. Results of the second study show the long-term weight loss unsuccess of a dietary program including very mow calorie diet and the consequence of a psychological attack against the body, depriving the subject of its main defense mechanism against impuluses. The third study shows that eatig loss control are not often the consequence of restriction episodes. Impulsivity has a rôle in the involvement of eating loss of control when subjects are not restraint and predicts the success or the insuccess of restriction

Depressive symptoms influence the individual’s psychological functioning. Among the related changes, the fluctuations of cognitive and affective states constitute essential information in understanding the functioning of depressed patients. Methods taking into account the specificities of temporal processes now allow the follow-up of affects and cognitions. However, only few works investigated the depressive state and the effects of a therapeutic intervention considering its evolution over time. Relying on the Ecological Momentary Assessment (Stone & Shiffman, 2008), this research aims to complete the current knowledge of the temporal evolution of cognitive and affective dimensions in depression (study 1) and to assess the effect of a physical activity program on this evolution (study 2). Study 1 compares the temporal evolution’s features of depressed mood, anxiety, brooding and self-esteem between depressed individuals (n = 14) and control ones (n = 26). Results demonstrate a specific evolution of these psychological dimensions in depression, independently of their levels, implying that the related information remains unconsidered through the usual assessments of depression. Study 2 highlights an antidepressant effect of a threemonth physical activity program in depressed patients (n= 10) and its influence on the temporal evolution and the functioning of cognitive and affective states related to depression. This work opens new windows in analyzing temporal processes involved in the depressive symptomatology and suggests interpretative leads regarding the psychological dynamics in Major Depressive Disorder

Obsessive Compulsive Disorder (OCD) is a mental illness defined by the presence of compulsions which are repetitive behaviors and obsessions which are intrusive thoughts. OCD may have a dramatic impact on the social and professional lifes of patients. The treatments for OCD are medication and cognitive-behavioral therapy. Nevertheless, symptoms remain in 25 to 40% of patients. Repeated Transcranial Magnetic Stimulation (rTMS) represent a novel therapeutic option for OCD patients however its effectiveness has been difficult to prove partly due to a multitude of factors involving the rTMS technique itself. The first study evaluated the impact of a two-week rTMS protocol on cognitive functions in healthy subjects using fMRI during a working-memory task. In the absence of behavioral modification, we observed that compared to sham, rTMS induced changes at multiple nodes of the cerebral network activated by the task beyond the stimulation site. This result represents an encouraging perspective for rTMS intervention on pathological brain network in OCD because hyperactive regions in this disorder (orbitofrontal cortex and anterior cingulate cortex) are too deep to be directly stimulated by rTMS. Our second study targeted the supplementary motor area (SMA), which had been suggested as candidate target region lying near the anterior cingulate cortex. We used for four week rTMS in a group of OCD patients while pre- and post-treatment fMRI was acquired during performance of a cognitive inhibition task. Stimulation of the SMA did not result in significant clinical improvement. These results shall guide future research to address the methodological challenges and the neurophysiological mechanisms of rTMS to improve its clinical efficacy for OCD

The re-education of human memory by daily life simulation systems is an important part of the current research trends providing advances in. Neuropsychology by introducing innovative aspects of computing. As a matter of fact, most of the psychometric tests used now involve a mostly verbal and little motivating formalism for the description of abstract situations and can not give a whole overview of the remaining memory capacities. Besides, the few "ecological » tests recently built are mainly used with goal of ''testing" rather than re-educating. The REMISSION project tries to fill this gap by presenting simulated situations according to adapted action programs, and usual chronological developments (waking up, preparing breakfast, etc. . . ). The patient is asked to carry out certain tasks on screen-displayed objects by means of the mouse (moves, state changes, etc. . . ). The first phase of the project consists in acquiring a whole set of knowledge chunks relative to the application domain in order to better analyse the real needs expressed by the medical team. It was followed by a conceptual analysis phase which allowed us to elaborate a precise rehabilitation model. Then the elaboration of a typical rehabilitation scenario allowed us to define an original system architecture, to formalize the whole treated data and to choose three powerful software products (Nexpert-Object, Oracle and Data Views) in order to develop a prototype of the system. Among the modules which compose this system, the "selector" plays a particularly important role in adapting the system to each patient case. Accord to the memory profile and the rehabilitation history of the patient, the system chooses the successive scenario and exercises

The proposed reflection on reading arose of a specific pedagogical practice: the teaching of literature to young people with severe psychic disorders, during their stay in an institution witch combines treatment with studies. This research deals with fields witch are ordinarily heterogeneous - pedagogy, literature and psychopathology - bringing them together and examining them with reference to psychoanalysis. My two main lines of study were: firstly, to define the pedagogical situation as a transference situation; secondly, to consider the literary experience as an analogy between the reader's experience and that of the writer. Marcel proust's experience, for instance, was invoked to throw light ont the psychic depts involved in the encounter with his work. This allows us to understand how literature deals with the traumatic experience and or catastrophic experience, to witch it gives shapes and representations. The encounter with literary texts within the psychopedagogical situation is capable of allowing young people, at odds with themselves, to be reconciled on the imaginary level with the symbolic dimension linked to their cultural heritage

Background: Parkinson’s disease (PD) is commonly viewed as a multi-systemic degenerative disorder. Alongside motor symptoms such as tremor, muscle rigidity and bradykinesia, up to 90% of people with PD develop speech disorders over the course of the disease. Phonation, prosody and articulation are the three main speech components altered in PD. These changes are grouped under the term “hypokinetic dysarthria”. Vowel production, an important aspect of speech for intelligibility, is commonly altered in PD. Various pharmacological and surgical interventions are now available to help manage the different motor symptoms of patients with PD. Speech therapies are also sometimes offered to patients to help manage communication disorders related to dysarthria. The impact of these therapeutic interventions on vowel production is, however, still largely unknown. Purpose: To investigate the impact of pharmacological (levodopa), chirurgical (deep-brain stimulation of the subthalamic nucleus, STN-DBS) and speech (Lee-Silverman Voice Treatment, LSVT®) interventions on the acoustic proprieties of vowels in PD. Results: Regarding the impact of levodopa, high daily doses may lead to vowel articulation impairment. At reduced doses, this adverse effect seems to disappear. As for the impact of STN-DBS, the surgical intervention itself may lead directly to vowel articulation impairment, while the electrical stimulations themselves may improve it. Also, STN-DBS may indirectly improve vowel articulation via the reduction of the daily levodopa dose required to manage the symptoms of PD following the intervention. Regarding the impact of LSVT®, this treatment may lead to an improvement of vowel articulation and coarticulation. Conclusions: Vowel production is modulated by the different therapeutic interventions offered to people with PD. Speech production and, more specifically, vowel articulation and coarticulation should be an integral part of patient evaluation profiles when choosing and optimizing therapeutic management interventions for these patients.

Scientific Background: Defined as a complex syndrome, combining attentional and behavioral difficulties, ADHD has nowadays become a widespread disorder affecting approximately 3.5% of children in France. The divergent approaches, as well as the various therapies, involve research in different fields. Biofeedback approaches have coughed our attention, since innovative and sensitive to the integration of the subject in his care journey. The approach based on cerebral activity (neurofeedback) has been explored in many aspects, however its respiratory counterpart has remained aside, thus a line of research to be explored. Problem: Does therapeutic support based on a biofeedback tool reduce the intensity of attentional dysfunction symptoms in children with ADHD? Method: The study focuses on the evaluation of a respiratory biofeedback protocol with an interactive and multisensory device (Garden - Ullo) in the reduction of the expression of symptoms related to AD / HD in children diagnosed and aged between 8 and 11 years. The 17 participants in this randomized, crossover clinical trial, are divided into two intervention groups (immediate and delayed) and the study is divided into two phases. During the intervention phase, activities involving attention are proposed through exercises involving three modalities: cooperation, cognition, and self-regulation. The evaluation phase allows it to determine the evolution of symptomatic manifestations during the study. Results : The results from this randomized trial highlights an overall decrease in the scores obtained on the different scales, testifying to the short-term benefits of the sessions carried out. The maintenance of these benefits in the medium term is however reduced, remaining perceptible for a sub-category of patients, those with an high potential profile. Conclusions: The proposed protocol allows to decrease symptomatic manifestations and functional impacts related to ADHD, in correlation with the patient's profile and comorbidities. This preliminary study leads to new questions about the functioning of ADHD, for which a covariance can be identified between behavioral and cognitive manifestations of the disorder

Hypnotic processes are the subject of transdisciplinary studies that attest to a specific state and involve relational and contextual dimensions (Rainville, 2004; Bioy, 2017). This research is carried out in the context of Persistent Grief Disorders (P.G.D.., Prigerson, 2009, Zech, 2006, Fasse, 2013). A pharmacological approach improves the depressive component without affecting the specific symptoms of PGD, questioning the place of psychotherapeutic interventions (Hensley, 2006). This exploratory research questions the meaning that hypnotic experience can take in a PGD context and describes a phenomenology. The ritual dimension of the hypnotic device could underlie the phenomena of "immediate healings" (Michaux, 2007), a blind task in the practice of hypnotherapy. This is a qualitative longitudinal action research using the Interpretative Phenomenological Analysis (Smith, 2009). The results show that hypnosis can assume a therapeutic ritual function that is characterized by a clear temporal scansion, as an act of closure of a mode of relationship. We have identified a hypnotic mythology of "common knowledge",dependent on the intensity of expectations and beliefs about hypnosis, especially its "magical" and "therapeutic" dimensions. The frame effect and the sequential ritualization of the session are major. People experiencing an "immediate healing" ritual were free of major psychiatric comorbidities. The central elements are: the therapeutic alliance, the absence of competition with other belief systems, respect for the psychological ecology of the subject, his involvement in a change of relationship with the deceased and the opening of a socially acceptable opportunity to produce it. Situations of inoperability show effects of cognitive dissonance induced, between temporary submission to a suggestion and resistance to a lived experience of abandonment of the deceased. The dimensions of "fidelity" and "loyalty" are major in these phenomena
