Повний текст джерелаMesenchymal Stem Cells (MSCs) are stromal cells present in a number of different tissue types. In addition to their ability to differentiate into multiple lineages (chondrocytes, adipocytes and osteoblasts), MSCs also display immunosuppressive properties. Whilst these mechanisms are far from fully understood, their immunosuppressive capacity has recently been shown to be modulated by miRNAs. OA is the most common form of joint diseases without curative treatment and mainly characterized by the degradation of articular cartilage, with subchondral bone alterations and synovial inflammation. MSC might provide therapeutic potential for treatment of OA.Here, we showed that an autologous injection of adipose-derived MSC (ASC) into an osteoarthritic joint improved pain and function levels in patients. We underscored the systemic immune tolerance induced following intra-articular injections of ASCs. Finally, we investigated the miRNA expression profile of human MSCs upon their stimulation by peripheral blood mononuclear cells. We identified miR-29a and PSAT1 as new candidates to regulate immunosuppressive activity mediated by MSCs

Повний текст джерелаThe lack of effective treatments to regenerate the brain led research into a new field: the cellular therapy. In this context, stem cells appear as a tool of choice to regenerate brain tissues and restore cognitive abilities. Olfactory ecto-mesenchymal stem cells (OE-MSCs), located in the nasal cavity, appear as an ideal candidate.In rats, we have demonstrated that transplantation of OE-MSCs restored memory capacities affected after ischemia and led to an increase in neurogenesis within the hippocampus.We then promoted the interest of these cells toward the scientific and veterinary community by characterizing the cells of eight species of interest. This work revealed strong similarities between the cells of all the species, which makes it possible to generalize in part their therapeutic properties from the data obtained mainly from human cells.To confirm these observations in vitro, we performed transplants in the fourth cerebral ventricle of old dogs with degenerative syndrome related to aging. The injection of stem cells partially restored disturbed cognitive abilities and was shown to have no deleterious effect.The results obtained during this thesis confirm the interest of OE-MSCs to treat central nervous system injuries and suggest that they could be a major tool for regenerative medicine

Повний текст джерелаThis psychology research work explores an especial clinical situation: one including an interpreter. It investigates how the presence of a third person affects a usually private meeting: a consultation between a psychologist and a patient. It also takes an interest in the effects of language co-existence and in the way one’s speech may get lost in translation.The clinical exercise in association with an interpreter has expanded in parallel to the increasing population flows, no matter their reasons and contexts. These particular consultations fit into a contemporary social evolution and they keep spreading, especially in the associations sector. The clinical interview with an interpreter has its roots in several schools of thought. Functionalism, culturalism, psychoanalitic anthropology, ethno-psychoanalysis, clinical ethno-psychiatry as the Ortigues’ clinical experience have influenced this kind of clinical practice and its intercultural professional context. Nowadays new clinical methods are being developed to include an interpreter in group an individual therapies. This particular clinical situation is the consequence of bringing people who do not share the same linguistic code, together. It lad psychologists and interpreters to question themselves: What are the characteristics and the issues of a medical interview with an interpreter? How can a psychologist practice in such a particular circumstance? These questions have been a starting point for both psychologists and interpreters to analyze this clinical exercise. Both professionals have respectively considered the issue according to the three following leads:- The specificity and the particularities of a clinical interview including an interpreter. - The clinical dimension of the speech in a clinical interview including an interpreter and led by a psychologist.- The singular inter-subjective bond that arises between both attending professionals: the clinical psychologist and the interpreter. The notions of translation and interpretation that are central in this type of interviews are not only present in their common meaning but also in a specific context linked to this particular consultation. Повний текст джерелаThe chilean school system has been among the first to introduce a « pilotage by the quality ». The Foundation Chile was one of the main actors of this institutionalization. The purpose of this doctoral research is to discover how the consultants in quality of the Foundation Chile have used the technical objects (Simondon, 1989) of their toolbox to influence the implication of school principals and their teams in the process of institutionalization of the pilotage by the quality. The field of scientific coherence of this research is that of the institutional analysis (Lamihi & Monceau, 2002). Seven of the ten former colleagues of the researcher are participated in this research. The research apparatus had for priority objective to allow the individual and collective expression of consultants, to make emerge their implications and by provoking transductions in their discourse. The collection of empirical data has been carried out through individual and collective interviews, all recorded in video (Pesce, 2017). The film apparatus facilitated the reflexivity (Lallier, 2009 ; Rouch, 1979) of consultants on their backgrounds and their practices within the Foundation Chile. To broaden the field of analysis, a few specific tools of the institutional analysis have been integrated to the research apparatus. The use of the analyzers which arose from interviews, that Lourau (1978) appointed of natural analyzers. I have also carried out a restitution of the results with that of participants who was my informant privileged, this in order to refine my analyzes. Finally, the writing of a diary study has been essential for the analysis of my own primary and secondary implications as a practitioner-researcher

Повний текст джерелаThis research explores Jean Laplanche's translational model from both a clinical and theoretical perspective. Laplanche spent his life translating Freud from the German and studying Freud in a non-dogmatic way. His critique of Freud's contradictions and theoretical impasses aimed to renew French psychoanalytic thought which in the 1960s and 70s was either stagnating or under the domination of Lacan's interpretation of Freud. His « translational model » reflects this heritage: his wish was to reinvent a theory open to discussion and criticism, opening new avenues for psychoanalytic research. The author's interest in Laplanche's theory stems from her own experience as a psychoanalyst and translator. The translational model is thus the result of rigorous research and experience; its aim is, among others, to rethink the origin and function of the sexual drives. Laplanche criticizes the Freudian theory whereby the source of the drives is to be found in bodily excitation. For Laplanche, the drives are not "mythical entities" but the result of the traumatic confrontation between the infant and the sexual adult. The infant's drive to "translate" thus comes from the "implantation" of sexual drives through enigmatic messages unwittingly transmitted by the caring adult. This adult can be related to the Nebenmensch Freud wrote about in his Project for a scientific psychology (1895). Indeed, in Laplanche's model, translation is the first psychic activity of the infant, born helpless, passive and dependent on adult care. What the child is unable to process becomes an untranslated "left-over", implanted in the child's body or in the deeper recesses of the unrepresented unconscious (the unconscious of primary repression), offering itself up to future translation. The question then arises: how can this "translational process" be clinically described? Is the term "translation" appropriate? Can translation be considered at once as the origin and a vicissitude of the drives? Laplanche based his exploration of the origins of the individual repressed unconscious on his close study of Freud's early works, in particular Freud's own "translational model" described in a letter to Fliess of 1896. His investigation led him to return to the theory of seduction as a "repressed theory" of psychoanalysis, extending its meaning to a general theory of the origins of infantile sexuality: seduction is at the root of all adult-child relationships, he says, and for this reason we have all been and continue to be "infant translators". His model is not a linguistic model in that translation comes before language. The clinical examples focus on what Laplanche called the process of « translation-detranslation-retranslation », with a special emphasis on "detranslation" in the context of the analytic situation, in the "après-coup" of the transference relationship between patient and analyst. This back-and-forth, circular movement between primary and secondary processes can be compared to the internal, psychic, and non-verbal work of the translator as s/he explores the infinite creative potential resulting from the encounter between source and target languages. This movement, triggered by the analytic frame and symbolized by what Laplanche calls the "fundamental anthropological situation" makes it possible for the patient to reignite processes frozen by infantile fixations. Can "translation" become a psychoanalytic concept, and if so, what could it mean, as opposed to existing concepts (such as symbolization or sublimation)? If one considers that the activity of translation itself is rooted in infantile sexuality and autoerotism, do we need the term "detranslation"? My research thus aims to explore the productivity of this model for psychoanalytic research. Can Laplanche's model be used not as a rigid tool but as an object of theoretical and clinical exploration... and further translation?

Повний текст джерелаThe deployment of technology in operating rooms dramatically accelerated over the last decades. More precisely, the surgeons’ ability to distinguish healthy from diseased tissues is still mostly based on their own subjective perception. As tissue status assessment is of upmost importance in oncologic surgery, both for tumor resection and reconstruction procedures, the ability to assess the tissues intraoperatively and in real-time over a large field is crucial for surgical act guidance. The lack of tools for biological intraoperative tissue status assessment has been the main source of motivation for this thesis work. A clinically-compatible imaging platform has been developed for oxygenation and fluorescence imaging in real-time. The capability of the platform to detect and quantify ischemia has been demonstrated through preclinical trials, by comparison with standard of care methods. Furthermore, the multimodal nature of the developed imaging device has been exploited by combining endogenous imaging of optical properties with exogenous fluorescence imaging, in the context of oncologic surgery. A fluorescence quantification technique was validated in preclinical trials with colorectal and pancreatic cancer models, highlighting the limitations of conventional fluorescence imaging

In addition, the rate of approval for anticancer drugs after entry in phase I clinical trial was reported as one of the lowest of all therapeutic areas. Thereby, this process has to be improved, and the use of new approaches fulfilling the gap between preclinical and clinical settings by anticipating human pharmacokinetics and efficacy could be an interesting solution.The work is focused on the building of strategies based on mathematical modeling of in vivo and in vitro preclinical data to anticipate the behavior of a new bcl-2 inhibitor developed by Servier laboratories in human to support clinical development. This project was elaborated following different steps:Firstly, a semi-mechanistic relationship was established in mice to describe the mechanism of action of the compound.PK extrapolation strategy using PBPK modeling was performed to anticipate human concentration-time profiles.PD extrapolation strategies based on different assumptions were proposed to predict human efficacy and doses to be tested in clinical trial.Predictions obtained were consequently compared to clinical results from a First in Human study confirming the usefulness of such approaches and the superiority of mechanism-based strategies compared to more empirical approaches.Therefore, this project highlights the large interest of elaborating interspecies translational approaches during drug development and could promote their use to accelerate new entities development, decreasing the risks of failure and financial costs

Повний текст джерелаThe tissue response of patients undergoing radiotherapy, despite identical treatment protocols, could have important variations. One of the questions asked to the radiobiology is to explain these variations (a posteriori approach) and if possible to predict it (a priori approach). The response of an organism and complex tissues to radiotherapy is the result of many determinants. Some belong to the tissue dynamics and homeostasis (inflammation, cytokines, etc.) and others to the sensitivity and cellular response (intrinsic sensitivity, DNA repair, regulation of cell death, etc.). About cellular determinants, our research team proposed to classify humans into three groups of different radiosensitivity levels, the first considered as normal is by far the largest (almost 75% of individuals), group II of intermediate radiosensitivity represents the majority of individuals having an abnormal response to radiotherapy. Group III gathers extremely rare conditions associated with marked or very marked hyper-radiosensitivity which are usually life threatening. Therefore patients in group II show either unexpected therapeutic response or severe radiation early or late side effects after doses for which normal tolerance is expected. Patients in group II can also be characterized by a strong predisposition to cancer and radiation-induced tumors. There is an inherent variability related to endogenous factors, such as the quality of DNA repair, or the spontaneous production of micronuclei; and exogenous factors. In this regard, certain medications may alter the cellular response to radiation therapy, such as statins, antiplatelet agents or anticoagulants. A posteriori approach: a single-center clinical study of 68 patients with prostate cancer treated by the same radiation oncologist in the same conditions, the frequency and severity of rectal side effects are compared to the use or not of statins : a radioprotective effect of statins toward radiation proctitis cannot be excluded. A priori approach: by studying the cellular and molecular radiation response in human fibroblasts (especially from healthy bowel tissue). Were studied: signaling and repair of DNA double-strand breaks by analysis of nuclear foci of MRE11; H2AX and pATM; radiation induced ATM protein transit from the cytoplasm to the nucleus was observed for the first time in rectal tissue fibroblasts. Statins appear to speed up this transit, making possible a radioprotective effect. These results open perspectives for further studies for the use of medications readily available that can modulate the severity of side effects of radiotherapy
