The purpose of this study is to define traditional Native education for three Algonquian speaking nations using ethnographic skills of cognitive anthropology. An understanding of traditional Native education from a First Nations' perspective through dialogue using individual audio-taped interviews and an audio-taped group consensus-building dialogue is provided. The Algonquian elders involved are from the Algonquin, Cree and Ojibway Nations. Ten case studies and a group consensus-building conversation with elders constitutes this study. Each case study contains an individual audio-taped dialogue transcription with contextual remarks. The audio-taped dialogues and group consensus-building conversation are transcribed and analyzed using verbal protocol techniques. The emergent themes across the interviews and group consensus-building dialogue are analyzed and the findings tabulated. Six female elders and four male elders whose ages cover a fifty year age span, is the composition of the ten case studies. Nine elders, two women and seven men make up the membership of the group dialogue. Some of the participating elders conveyed their thoughts using the assistance of a translator. Consensus emerges across the individual dialogues and group interview. Elders tell of the existence of a different epistemology for Algonquian speakers that originates in the circle of life and is represented by the medicine wheel. In the cosmology of the circle each person is a whole world and a member of the larger circles of life; the family, the community, the world and the universe. According to the elders the concept of traditional Native education and the process of traditional Native education are embedded in the medicine wheel. Traditional Native education includes learning the Algonquian customs, traditions, values and beliefs and languages. Traditional Native education is the process of acquiring a First Nation identity. The importance of the land to all First Nations People is a recurring theme across all the dialogues. Elders disclose that the land holds knowledge and wisdom, and that it is capable of offering direction. They also iterate the interconnectivity of all of life in the recorded dialogues. No apparent difference in the thought patterns of the contributing Algonquian elders to this study is evident. This study has implications for First Nations' education in particular and for education in general. First, it gives direction to educators involved in educating First Nations children pointing out the need to provide traditional Native education and delineating the components of such an education. Second, it indicates that different epistemologies exist for First Nation Peoples and non-First Nations People and suggests ways of bridging the cultural differences to encourage understanding amongst all people. Third, it offers direction to educators involved in developing cross-cultural education programs.

Littorals in the in the Exuma Cays, Bahamas and the Bath Plantation, Barbados are comparative in many ways. These edges of the sea have provided critical services to local people during the time of slavery and since. More than food and medicine, the littoral is the nightly sea bath, where children are instructed, and the last ecosystem effectively used by the elderly. Independence and self- respect derive from use and protection of these littoral by individuals and communities. Local patterns of conservation and use are argued to be essential in the ecological structure and functions of the littoral. Development projects and marine protected areas alike are seen as potentially breaking local ties with the littoral causing trophic skew and damaging local society. If development occurs, mitigation solutions potentially derive from legally recognizing local people as partners in the co-management of their traditional littoral. Included with this article is a presentation prepared by Drs. Brent and Richard Stoffle.

In recent years, growing numbers of Mainland Chinese elders are participating in Christian congregations during short visits or extended stays in the U.S. This thesis explores social and cultural factors that explain these immigrant elders’ involvement with Christianity while in the U.S. Based on face-to-face interviews with 20 Mainland Chinese elders living in Atlanta as well as participant observation at a Chinese Christian church and with a Bible study group, this study finds that loneliness and social isolation in a foreign land are the main reasons for Mainland Chinese elders’ participation in Christian activities. Besides religious services, social service functions of churches and opportunities for fellowship are other factors explaining church involvement. Long immersion with China’s religious history of syncretism renders these elders receptive to spiritual alternatives, such as Christianity, especially when dominant ideologies of Confucianism and Communism seem to be in decline in China. But most elders approach their Christian involvement with pragmatic selectivity based more on secular than religious considerations. In conclusion, I argue that social isolation and minority ethnic status, Chinese Christian Churches’ social functions, and current social contexts in China are all factors that help explain Chinese elders’ religious involvement in the U.S.

The proposed research study explored the extent and nature of financial abuse crimes amongst elders in San Bernardino County. This study utilized a descriptive research method. This study identified characteristics of victims and perpetrators, contributing factors associated with financial abuse, the degree of trauma an elder will experience depending on the type of financial crime committed. This study will provide an overview of the different types of financial abuse cases reported and determine if there is a significant difference and/or similarities of these types of crimes committed against male and female elderly in San Bernardino County.

Анотація:

This paper examined patterns of decumulation and the role that health events and marital disruption play in forming those patterns. Study data were drawn from six biennial waves of the HRS (1998 - 2008), and merged RAND HRS data files for the period 1998–2008. The a priori expectation was that there will be variation in drawdown strategies households employ. Findings suggest that patterns of dissaving are heterogeneous. The five most prevalent patterns were discussed. Households predominantly transitioned between oversaving and overspending. Households are expected to have a goal of on target spending therefore the observed cycle's dissaving will influence the next cycle's draw down rate in an attempt to maintain a sustainable drawdown rate. Markov model results suggest that households do recalibrate their depletion rate as a function of their last depletion rate. This study hypothesized that the onset of a health condition or a spouse's admission to a nursing home would be associated with an excessive decumulation of assets. These hypotheses were unsupported by the research. Marital transitions as predictors of decumulation were only partially borne out by the results. Divorce was also expected to increase the likelihood of overspender however this relationship was not significant. Loss of spouse was associated with an increased likelihood of verspending. One of the major contributions of this study is the identification of patterns of dissaving in retirement. Various life course, demographic and decumulation factor variables were determinants of these patterns. Overall results suggest that elders have a difficult time managing to an on target drawdown. This study concludes with a national decumulation policy directive outline.

The old-age population is increasing at a considerably faster rate than the worlds total population. According to the United Nation report (2017), currently Europe has the highest proportion of elderly people of all the continents, but it is remarkable that in 2050 there will be an increase in the number of people over 60 also in China, in some nations of Africa and in Latin America. However, the increase in elderly people is notable because aging brings important changes in health status: cognitive and physical functioning, together with the degree of disability, are important indicators used to try to quantify the quality of health, in the later stages of life. Moreover aging is also frequently related to non-communicable diseases. Among brain disorders and non-communicable diseases, dementia, which is an umbrella term for several diseases, is the major cause of disability among older adults having major consequences on cognitive impairment and on mortality. The general aim of this thesis was to study in depth the health status using trajectory models of cognitive function, physical activity, and disability in a cohort of elderly people. Its specific aims were to identify trajectory and multi-trajectory trends of cognitive, physical functioning and disability on non-demented elderly people living in the North of Italy in order to profile each multi-trajectory group identified. Besides another aim was to describe trajectories of cognitive decline for a part of the older adults of the Italian population-based cohort and for the Swedish population-based cohort. This PhD thesis is based on data gathered from the two population-based studies: the Italian InveCe.Ab (Invecchiamento Cerebrale in Abbiategrasso, i.e., Brain aging in Abbiategrasso) study conducted by the Golgi-Cenci Foundation, Abbiategrasso (Milan) and the Swedish database of SNAC-K study (Swedish National Study on Aging and Care-Kungsholmen) carried out by the Stockholm Gerontology Research Center in collaboration with Aging Research Center (ARC), Karolinska Institutet. To achieve the aims, three outcomes were used in the present work. Cognitive function was assessed using the Mini Mental State of Examination (MMSE) scale administered at every follow-up time in both studies.The physical status was assessed using the walking test as gait speed. To test the preservation from disability during all the follow-up times the Activity Daily Living scale was administered (ADL). This scale takes into account difficulty in daily living activities, such as bathing, using the toilet, dressing, feeding and moving from one room to another. To describe different courses of the three outcomes, the statistical methodology of Group-Based Trajectory Model (GBTM) was applied. This method uses the trajectory groups as a statistical device to find the unknown distribution of trajectories across subjects. The groups identified by using the GBTM were profiled and compared using a multinomial logistic regression model. The results described by the multi-trajectories are good for most of the elderly: they have quite stable cognitive, physical function and they are free from disability in daily activities. Aging is a real, concrete and complex issue for human beings. Constancy, perseverance and well planned research are the keys to deepen in order to have a better knowledge of this phenomenon and its lifespan.

The National Policy on Integrative and Complementary Practices (PNPIC) was implemented by the National Health System (SUS) in 2006, creating a range of possibilities to ensure the integrality in the health care practises. The medical training needs to be done so the student learn to look the person who he is taking care in a holistic way, in view of the current reality of showing the direction of the medical knowledge, because the practice is grounded in the biomedical conception. One starts from the assumption that the Traditional Chinese Medicine approaches enable to qualify the anamnesis, especially in elderly patients, in order to take into account their lifestyles, their emotions and their spirituality. The themes linked to the integrative practises should be debated in the academia, since the new medical training program searches for new perspectives of professional education. The goal of this thesis is to analyze the use of the Traditional Chinese Medicine on broadening the anamnesis done by medicine students, on medical visits for the elderly people on the Family Health Strategy. Its specific goals were: to know the anamnesis meaning for medicine students on elderly patients; to offer training to the students for the clinical handling under the Traditional Chinese Medicine light and to identify changes on the medical practice after the training offered. It is an action-research, with qualitative approach, with eight medicine students from a private college as subjects, during their internship of public health, carried out in a unit of family health in the cities of Cabedelo and João Pessoa, both on Paraíba State. In relation to the data collecting, in the first stage, recorded interviews were performed with the use of a semi-structured script. In the second phase, training was offered to the students to develop the anamnesis of the elderly, according to the techniques of Traditional Chinese Medicine. In the third stage, newly recorded interviews were conducted. To analyze the data, content analysis technique was used, on the theme mode. It was identified through the results that students have weaknesses in their approach with the patient and in his training, at the prospect of comprehensive, humane care and practiced with the use of knowledge of the integrative practices. We also observed little appreciation of the semiology knowledge needed to develop a wider anamnesis, according to the holistic concepto of health. However, the students were receptive to the new knowledge, when invited to attend the training in Traditional Chinese Medicine. In this respect, it was verified that the training conducted caused students to include new information about anamnesis theme, which leads to recommend educational strategies focused on action-research, could be employed in medical education in order to strengthen changes in the health care model.
A Política Nacional de Práticas Integrativas e Complementares foi implementada pelo Sistema Único de Saúde em 2006, abrindo um leque de possibilidades para garantir a integralidade na atenção à saúde. A formação médica necessita ser realizada de forma que o estudante aprenda a ver a pessoa de quem cuida de forma holística, tendo em vista a presente realidade de mostrar o direcionamento do saber médico, em função da prática ser fundamentada na concepção biomédica. Parte-se do pressuposto que as abordagens da medicina tradicional chinesa possibilitam qualificar a anamnese, sobretudo em pacientes idosos, de modo a levar em consideração, seus hábitos de vida, suas emoções e a espiritualidade. Os temas vinculados às práticas integrativas devem ser discutidos no meio acadêmico, uma vez que o novo currículo médico busca novas perspectivas na formação do profissional. Este estudo tem como objetivo geral analisar o uso de técnicas da Medicina Tradicional Chinesa, na ampliação da anamnese realizada por estudantes de medicina, na consulta à população idosa na Estratégia Saúde da Família. Como objetivos específicos foram: conhecer o significado da anamnese para estudantes de medicina na consulta ao idoso; oferecer capacitação aos estudantes para o manejo clínico à luz da Medicina Tradicional Chinesa e identificar mudanças em sua prática médica, após a capacitação oferecida. Trata-se de uma pesquisa-ação com abordagem qualitativa, que teve como sujeitos, oito alunos de uma faculdade de medicina da rede privada, em seu internato de saúde coletiva, realizado em uma unidade de saúde da família, cenários de Cabedelo e João Pessoa, PB respectivamente. No que concerne à coleta de dados, no primeiro momento foram realizadas entrevistas gravadas, mediante o emprego de roteiro semidirigido. No segundo momento, foi oferecida capacitação aos acadêmicos para desenvolverem a anamnese da pessoa idosa segundo a aplicação de técnicas da Medicina Tradicional Chinesa. Na terceira etapa foram feitas novas entrevistas gravadas. Para análise dos dados, foi utilizada a técnica análise de conteúdo, modalidade temática. Foram identificados através dos resultados obtidos que os alunos apresentam fragilidade em sua abordagem com o paciente e em sua formação na perspectiva do cuidado integral, humanizado e praticado com o uso de saberes de práticas integrativas. Observou-se ainda pouca valorização de conhecimentos de semiologia necessários ao desenvolvimento de um anamnese ampliada, segundo a concepção holística em saúde. No entanto, os alunos se mostraram abertos ao novo conhecimento, quando convidados a participar da formação em medicina tradicional chinesa. A esse respeito, verificou-se que a capacitação realizada fez com que os alunos incluíssem novos elementos sobre o tema anamnese, o que leva a recomendar que estratégias educativas, centradas na pesquisa-ação, possam ser empregadas na formação médica de modo a fortalecer mudanças no modelo de atenção em saúde.

This study examined how Japanese American elders conceptualize depression. Japanese American elders age 65 years and older (N = 120) were recruited from a senior center in Los Angeles, CA. Participants read a brief vignette that described an individual who met the criteria for major depression and then filled out a series of questionnaires. Counter to prior theories, these Japanese American elders emphasized that the interpersonal criteria contributed to the individual's problem to a lesser degree than the somatic, emotional, and cognitive criteria. These elders expressed Explanatory Models of depression that were similar to Western Conceptualizations of depression. The results also indicated that having an important role within one's family and higher activity levels tended to be related to lower Geriatric Depression Scale scores. Finally, acculturation, generational status, sex, educational level, and income were not related to problem conceptualization. Clinical implications and directions for future research were discussed.

Delirium is a state of acute confusion and is common in hospitalized older adults. Delirium is associated with significant increases in morbidity and mortality, as well as healthcare costs. Delirium also is associated with functional and cognitive decline, as well as need for institutionalization and rehabilitation. Delirium can cause psychosocial distress for patients and families. While much is understood about the epidemiology of delirium, the pathophysiological mechanisms that lead to the development of delirium are less clearly defined.The purpose of this study was to investigate the relationship of allostatic load (AL), a composite measure of primary (i.e. acute) stress mediators and secondary (i.e. chronic) stress outcomes and delirium in the hospitalized older adult. Development of the Allostatic Load & Delirium in Hospitalized Elderly model provided a theoretical framework for the study.Forty- four participants, ranging from 66 to 93 years of age (M = 76 years of age) were recruited from three intensive care units and enrolled once they were determined not to have a cognitive deficit or prevalent delirium, as assessed by the Standardized Mini-Mental State Examination and Confusion Assessment Method (CAM), respectively. Ten AL components reflective of acute and chronic stress were collected upon admission. Allostatic load was calculated as the sum of the number of components for which the participant was rated in the highest risk quartile. Allostatic load subsets based on acute and chronic components were also calculated. Incident delirium was assessed 48 -72 hours after admission with the CAM.Findings indicated that the incidence of delirium was 29.2%. The subset AL score based on components considered primary stress mediators was significantly related to delirium; however, no other variables were associated with delirium. Logistic regression modeling indicated that an AL subset of primary stress mediators did predict the incidence of delirium (OR 2.5, 95% CI = 1.12, 5.79; X2 (1) = 5.668, p < .05).The findings from this study exploring the relationship between AL and delirium in the hospitalized older adult suggest that an AL score based on primary mediators may be useful in predicting delirium in the hospitalized older adult.

A descriptive study was conducted to explore the relationship among three measures of decision making capacity: (1) clinicians' opinions, (2) Pfeiffer's (1975) SPMSQ, and (3) a developmental instrument by Fitten et al. (1990) designed to measure patient clinical decision making ability. Another purpose of the study was to replicate portions of the work by Fitten et al. (1990). A convenience sample of 41 patients, four physicians, and three nurse practitioners was studied. Quantitative research techniques were employed for data collection and analysis. Significant correlations were found among all measurements of decision making capacity indicating that the developmental instrument appears to be a valid method of determining decision making capacity according to results obtained for convergent validity. The findings of this study support those reported by Fitten et al. (1990).

Aging population is a worldwide phenomenon and Hong Kong is of no exception. Around 13% of the total population was aged over 65 in mid-2011 and it is projected to follow a remarkable rising trend in the future. In view of this and the increasing housing demand of the elderly people, the Hong Kong Government has adopted the concept of “Aging in Place” and “Community Care” to initiate a serious of housing policies and provisions to cater for the various special needs of the low-income elderly. For those middle-income elderly who are outside the “welfare net” of the government, the Hong Kong Housing Society has implemented the “Senior Citizen Residences Scheme” a decade ago which offers a choice of residence for them to live a safe, comfortable and independent lifestyle with provision of integrated elderly care services. In this paper, the eligibility, scheme features, housing facilities, provision of services, operation arrangements and community supports of Housing for Senior Citizens (HSC) provided by the HKHA and Senior Citizen Residences Scheme (SEN) provided by the HKHS will be studied to evaluate the effectiveness of the housing schemes provided by both organizations. Factors that contribute to life satisfaction and successful aging of the elderly will be further looked into. Based on the comments collected from the questionnaires distributed to the elderly residents of the HKHA and HKHS housing estates under study, recommendations will then be given for future improvement of the housing policies.

In Canada, falls are one of the leading causes of injury and deaths for seniors. These types of injuries can typically be avoided through falls prevention programs, and past studies suggest that these health services have significantly reduced seniors’ falls risk and rates in Canada. Despite the abundance of falls prevention research, practices and programs available in the country, Aboriginal Elders remain overrepresented in fall-related injury and fatality rates. The elevated rates of falls for Aboriginal Elders indicate that current falls prevention programs and standards may not be reaching those most vulnerable to fall hazards and injuries. My thesis is written in the publishable paper format and is comprised of two papers. Using an exploratory case study methodology in paper one, I investigated the social determinants of health that Inuvialuit Elders and LFPPs identify as factors that increase, decrease, or have no effect on the likelihood of an Inuvialuit Elder experiencing a fall. Together, we found that personal health status and conditions, personal health practices and coping skills, physical environments, social support networks, and access to health services increase Inuvialuit Elders likelihood of experiencing a fall, health practices and coping skills and access to health services decrease Inuvialuit Elders likelihood of experiencing a fall, and culture has no affect on the likelihood of Inuvialuit Elders experiencing a fall. In paper two, I used a participatory action research approach informed by postcolonial theory to examine what current falls prevention recommendations are offered by local falls prevention programmers (LFPPs) in order to reduce fall rates among Inuvialuit Elders in Inuvik, Northwest Territories, Canada; and to understand how falls prevention programs for Inuvialuit Elders can be co-created with participants to be culturally safe. In it, I provide the recommended strategies of developing and implementing a culturally safe falls prevention program for Inuvialuit Elders, as suggested by the LFPPs and Inuvialuit Elders who participated in the research. Taken together, the papers in this thesis make it apparent that research concerning falls prevention for Aboriginal Elders and falls prevention programs continues to be influenced by colonial practices. As a result, there is a demonstrated need for program development and research in this area to work towards reducing health disparities and challenging colonial practices.

This study explored the emerging Self-Imposed Activity Limitation (SIAL) theory among community dwelling elders. This theory was examined using the proposed Aging Well Conceptual model which was guided by Baltes' Selection, Optimization with Compensation model, Markus and Nurius' Envisioned Possible Selves theory, Kuypers and Bengtson's Social Breakdown Syndrome model, Bandura's Self-Efficacy theory, and Rotter's Locus of Control theory. The objectives of this study were to explore the relationships among multiple variables in a hierarchical model and to examine the explanatory power of the SIAL variables in predicting elders' well-being.A correlational descriptive design with a causal modeling approach was used employing Structural Equation Modeling (SEM) techniques. The Aging Well model was tested through a secondary analysis of the National Survey of Midlife Development in the United States (MIDUS) database selecting respondents aged 65-74 years.Two research questions guided this study. Research question one, how well does the Aging Well model fit with empirical sample data, was explored. The Aging Well model statistically approximated the MIDUS data after theoretical and statistical modifications and explained 76% of the variance of elder's well-being. The mediating effects of SIAL variables were determined by nested alternative model testing. Research question two, are the proposition statements in the Aging Well model valid, and was demonstrated empirically by the expected patterns of correlation and covariance among most of the variables in the Aging Well model.SIAL as a composite factor had a large positive effect on elder's well-being. Elders' perceived constraints and perception of aging had no direct effect on well-being. The influences of these two factors on well-being were mediated by a common factor, SIAL. These findings supported the emerging SIAL theory by suggesting that the optimal use of SIAL would lead to adaptive outcomes promoting elders' well-being. In addition, SIAL mediated the effects of elders' sense of control and perception of aging on well-being. The full range of SIAL could not be examined due to limitations inherent in secondary data analysis.

A basic health and lifestyle questionnaire was given to a convenience sample of 103 Ute elders, age 50 and older. Fifty-three percent reported being diagnosed with diabetes. The mean BMI (body mass index) was 33.31 (SD=8.72). For descriptive purposes, BMI was divided into three categories: less than 25, 25-29.99, 30 or above. Eleven percent were in the BMI category of less than 25. Thirty percent of Ute elders reported a BMI between 25-29.99. Fifty two percent had a BMI of 30 or above. Diabetes rates among this Ute elder sample were significantly lower with a lower income, lower education level, older age, higher BMI, and having a family history of diabetes. A binary logistic regression revealed family history (Exp [B]=3.06; p Based on this survey, the Ute Tribe should focus future wellness programs on prevention and control of diabetes and obesity among their tribe. Prevention for these chronic diseases needs to begin with the youth as well as with the older members of the tribe. Currently, the Ute tribe has two programs to treat and prevent diabetes among their tribe. The Diabetes Prevention and Control Program is a clinic to provide intervention for those Ute members with diabetes. It also provides a small gym furnished with exercise equipment for members of the tribe to use at no cost to them. The other program for the youth of the tribe teaches about the importance of nutrition and exercise in their lifestyles. Future programs should expand upon existing programs in attempts to reach the whole tribe.

place attachment, and respectively to examine the significance of the place attachment in determining degree of older individuals&rsquo
satisfaction with their life. Institutional environment was assessed under three headlines
design, social and institutional aspects. It was also aimed to analyze the way those three aspects are interacted with each other to conclude with place attachment. The sample consisted of 120 older individuals who were from two different institutions
Seyranbaglari Rest Home and Year of 75 Resting and Caring Home of Retired Organization. The data were gathered via applying the following measures
measures of design aspect, social aspect and institutional aspect
place attachment scale, life satisfaction scale and demographic information form. A set of exploratory and confirmatory factor analyses were conducted on the data, to determine whether the items are reliable indicators of latent variables. In this study, structural equation modeling technique was used to analyze the data by using LISREL. Investigating structural relationships among same variables in the light of the related literature, two different models were specified. The results of the study indicate that, the hypotheses of the first model, saying that
there is relationship between environmental variables of an institution and the life satisfaction of the elderly, and this relation is mediated by the place attachment, were supported except for the one assuming the direct relationship between design aspect and place attachment. The second model specifies the design aspect as the predictor of the social and institutional aspects of the institutional environment. The rest of the model specifies the same structural relationships with the first model. The second model was supported completely, and was confirmed all of the relationships specified by it. In the light of the related literature, the findings of the study were discussed. Additionally, limitations of the study and suggestions for further research were investigated.

This study draws upon recorded conversations with 21 Nattilingmiut elders and relies on a close research mentorship with Nattilingmiut elder Nilaulaaq Miriam Aglukkaq. The Nattilingmiut were one of the last groups in the Canadian Arctic to become settled into communities initiated by the federal government, a project that began in the 1950s and ended in the late 1960s. While early literature about Inuit strongly suggests that Inuit pre-settlement society was extraordinarily peaceful and harmonious, there is little literature available that focuses on Inuit elders' perspectives on changes to their social experience in post-settlement times. The Nunavut Government and Inuit agencies are currently working to develop systems that reflect Inuit Qaujimajatuqangit (Inuit epistemology) towards self governance and a consciously developed syncretism. As the Inuit Qaujimajatuqangit initiatives rely on the input of the Inuit elders, this study meets a current need to enhance elders' participation by offering some of their perspectives on dealing with conflict and change. It also offers a decolonizing methodology and is based on local protocols of the East Kitikmeot communities.

This research focuses on working with fifteen local youths, one elder, and two teachers in the town of Churchill, Manitoba to document intergenerational knowledge transfer. According to Tsuji (1996) there has been a significant loss of traditional ecological knowledge in First Nation communities between generations for both males and females. Traditional knowledge is important because it relies on an individual’s own knowledge, beliefs, and attitudes towards certain issues that he or she has experienced in the past. Through conducting interviews with Elders, youths, and teachers along with observation and participant observation, this research shows knowledge from the Indigenous elder is being transferred to the younger generation through stories, presentations, and education. There is a moderate level of intergenerational knowledge transfer from youth to elders and elders to youth, as well as a high level of interest in incorporating traditional and local knowledge in education.

Expected to turn into an aged society in 2018 and a super-aged society in 2025, Taiwan is a country that has been aging in a fast rate. Elderly people have a high demand for healthcare services. While the average individual expense for traditional Chinese medicine healthcare services has been decreasing year by year, and developing diverse traditional Chinese medicine healthcare services is the government’s policy for promoting aging in place, it is necessary to devise service innovation strategies to increase the strengths and opportunities for the industry, boost the value of healthcare, and satisfy the needs of elderly people. The objectives of this study were to: (1) investigate the current status of traditional Chinese medicine healthcare services in Taiwan; (2) summarize the strengths, weaknesses, opportunities, and threats of the traditional Chinese medicine healthcare system in Taiwan; (3) leverage the healthcare provision value chain matrix to analyze the healthcare service innovation strategies. Through an analysis of the healthcare industry, government policies, and global trends of aging in place, this study first identified the development direction of traditional Chinese medicine healthcare services and then applied the healthcare provision value chain to build a theoretical model and four basic dimensions of service innovation. Later, this study reviewed literature and conducted in-depth interviews with elite in the industry to capture the current status of the industry and assess the potential development models and their feasibilities. Results showed that the traditional Chinese medicine healthcare industry has been active in developing integrated services that support aging in place and geriatric medicine for elderly people to strengthen its competitive advantage. Through an analysis of the healthcare provision value chain matrix, this study identified new opportunities for the healthcare industry and 16 innovation strategies for four stages, including prevention, diagnosis, therapeutic measure, and supervision and management. Traditional Chinese medicine clinics were advised to increase their involvement in the community, take advantage of organizational mechanisms provided by local and national associations, and develop value-added services to satisfy the needs of elderly people.

Background: The elderly often have chronic diseases, and most people think that traditional Chinese medicine (TCM) is mild with fewer side effects. As a result, it is an issue worth our attention whether the correctness of the elderly taking TCM will be affected by their awareness and attitude when people use TCM frequently. Objectives: (1) Understand the elderly’s awareness, attitude, and correct practice of taking TCM. (2) Analyze the impact of the elderly’s characteristics on their awareness, attitude, and correct practice of taking TCM. (3) Examine the association between elderly’s awareness, attitude and correct practice of taking TCM. Methods: A cross-sectional study was conducted on subjects aged 65 or over who lived in Nangang District, Taipei City, without dementia, being able to take care of themselves and communicate in Mandarin and Taiwanese with consciousness. The questionnaire includes four parts: the characteristics of the elderly, the awareness of taking TCM, the attitude towards taking TCM, and the status of taking TCM. Data analysis was performed with descriptive statistics, analysis of variance (ANOVA), Mann-Whitney U test and Linear regression analysis. Results: In this study, 165 subjects were enrolled. The average scores of awareness and attitude of the elderly on taking TCM were 3.48 ± 0.55 and 4.04 ± 0.40 respectively. The mean score of the status of taking TCM was 3.40 ± 0.53. The factors influencing awareness of taking TCM: age, marital status, educational level, chronic illness, main medication choice during physical discomfort, the experience of taking TCM and the purpose of taking TCM. Important predictor variables included "educational level" and "during physical discomfort, choose TCM and western medicine". Factors influencing attitude towards taking TCM included educational level, monthly disposable income, presence or absence of arthritis, the experience of taking TCM and the purpose of taking TCM. The factors influencing the practice of taking TCM included main medication choice during physical discomfort, and whether the TCM was obtained from hospital, clinics and TCM stores. The elderly's awareness of taking TCM affects their attitudes towards taking TCM and the status of taking TCM which shows a positive correlation. The higher of the elderly's awareness score on taking TCM, the more positive attitude and correct practice occured. However, there’s no correlation between the attitude of elders toward taking TCM and the status of taking TCM. Conclusions and Suggestions: The elderly’s awareness of taking TCM may affect their attitude and correct practice of taking TCM while their attitudes may not affect their correct practice. However, the average score of awareness of the elderly is on the low side. Therefore, it is necessary to provide information on medication safety for the elderly through the combination with the activity centers for the elderly in all counties and cities and the Chinese Medicine Safety and Health Education Resource Center of the Ministry of Health and Welfare. On the basis of the Theory of Planned Behaviors (TPB), this research explored into the factors that affecting the intention of the elderly in the demand for traditional physical massage. Purposive sampling method was applied to collect data with a self-administered structured questionnaire. The target samples were the members of Kaohsiung General Labor Union and the Chinese Federation of Labor who participated in certain labor education activities in the research period. Some class members of the traditional massage department in a university in southern Taiwan were also included as samples. 558 responses out of 800 questionnaires were collected, and 429 valid responses left (53.6%) after data cleaning. Statistical analyses applied were descriptive statistics, the t-test, one-way ANOVA, Pearson Product-Moment Correlation, and multivariate regression analysis with a SPSS software package. The findings of the data analysis indicated that the attitude, subjective novms, perceived behavioral control and behavioral intention of the elderly varied significantly in part along with the demographic factors. The attitudes of the elderly positively affected the behavioral intention of accepting traditional physical massage. The subjective norms of the elderly in accepting traditional physical massage positively affected behavioral intention. The perceived behavior control of the elderly of accepting traditional physical massage positively affected their behavioral intentions. The attitude, subjective norms, and perceived behavioral control of the elderly of accepting traditional physical massage altogether positively affected behavioral intention with a variance explained at 74.8%. Test result of the current study has shown that the TPB can be a reliable theory in explaining the elderly’s behavioral intention of accepting traditional physical massage. This study has also suggested several future study directions, 1. Since samples included were limited to the citizens of Kaohsiung city, test result may not be generalizable to entire Taiwan. Surveys at a national level should be conducted in the future to make the data complete. 2. This study took behavioral intention, no the actual behavior, as the dependent variable. It will be worthy for future study to explore further into the real behavior. 3. The current study also suggested that the education authorities to establish disciplines of traditional physical massage widely in the university levels of education to train more healthcare professionals who are qualified as a massage therapist in the setting of traditional Chinese medical clinics. In addition, the authorities are obliged to formulate an accrediting system to include the current massage practitioners as legitimate professionals through license qualification process. By doing this, the government may supervise the national’s health as well as assure the labor right of the massage practitioners, and accordingly help the traditional techniques can be inherited to the next generations.

Previous nursing research has not examined the extent of collaboration between nurses and Indian Elders, and contact with traditional healers in Manitoba. An exploratory-descriptive two phase research design was implemented to identify factors affecting collaborative efforts between nursing staff and Indian Elders, and to explore nurse-healer interaction. Phase I consisted of semi-structured, face to face interviews with field nurses (N=10) and Elders (N=13) on three Indian reserves in northern Manitoba. Phase II consisted of a close-ended, structured, census survey mail questionnaire administered to nurses (N=64) working autonomously with Indian clients. Findings based on Phase I interviews and an 81.2% return rate of Phase II questionnaires indicated that nurses who were appreciative of the traditional health care system were collaborating with Elders and healers on reserves and health centres in Manitoba. The need for ongoing and indepth communication between nurses and Elders/healers was established. A more detailed cultural orientation was indicated as needed by the majority of nurses. Factors influencing the collaborative process were identified as: past transcultural experiences, knowledge of who/what Elders are, influence of the nurse's own culture and the attitude of the Elders and community towards the nurse. Client conditions identified as appropriate for Elder referral included: lack of self worth, family discipline problems, social interaction problems, child neglect, and parent-child conflict.

In Taiwan, according to the long history of Chinese medicine, people often seek traditional Chinese medicine as a complementary treatment. The purpose of this study is to explore whether there is a difference in the risk of subsequent stroke between patients with diabetes who are treated with Chinese medicine or not. In this study, a cross-sectional study will be conducted using Taiwan National Health Insurance database. We will collect patients with newly elderly diabetes mellitus diagnosis from one million randomly selected subjects aged equal to or above 65 years old of database in 2000. Patients received TCM will be defined as patients ever used TCM within one year after elderly diabetes mellitus was diagnosed. We will compare the characteristics between TCM users and non-users, as well as describe the incidence of stroke in elderly diabetes mellitus generations.

There are now a wide variety of eBooks on the market. Nevertheless, senior citizens tend to feel less affinity for these devices and find them a very different experience from their conventional reading habits. In this study, in-depth interviews and questionnaire surveys were used to identify the actual functions that senior citizens require from eBooks. This was then integrated with their conventional reading habits to design the software and hardware interface. The goal is to help older people establish a sound mental model for the use of eBooks and reduce the level of frustration during use. An experiment was conducted during the study using three sets of push button user interfaces, users with and without computer experience and four types of task complexity. The time and number of button presses required to perform the tasks were recorded and the NASA-TLX rating scale used after each test to determine their workload. The results showed that in terms of task execution time, those with computer experience performed better than those with no computer experience; for task complexity, the best rating was for "Low" difficulty, followed by "Medium", "High (Forward)" then "High (Reverse)"; there was no significant difference between a stylus or one-handed user-interface but both were preferred over a two-handed user interface. When it came to the task error rate performance: there was no significant difference between those with computer experience and those with no computer experience; for task complexity, there was no significant difference between "High (Forward)", "Medium" and "High (Reverse)". These were all superior to the performance for "Low" complexity; there was no significant difference in the task error rates for the one-handed or stylus user interfaces, and both performed better than the two-handed user interface. The NASA-TLX rating scale showed that in terms of mental load, physical load, temporal load, effort, performance and frustration level, the rankings for all 6 indicators were significantly better for those with computer experience than those with no computer experience; the stylus interface performed better than the one-handed user interface and this was in turn better than the two-handed user interface. These differences all met the test for significance as well. Correlation analysis indicated that the only correlation was between task execution time and task error rate performance, with no correlation between other combinations. Thanks to the improvement medical progress and the living standard, the elderly population in Taiwan significantly increased especially in recent year. According to the prediction from the government, in 2018, Taiwan will become the aged society, an elderly population of 14 or more by the definition of the UN. The ageing related issues have attracted the attention by the whole society in Taiwan, and the issues of long-term care specifically for the elderly are most important. In the field of long-term care, functionality is the most important goal in practice and the necessary outcomes in relevant research. In this way, a comprehensive assessment tool with the characteristics of multiple dimensions, sound psychometry, time-saving, and handiness is crucial to provide individual care and research. The aims of the present study are to translate the Multidimensional Observation Scale of Elderly Subjects, MOSES into traditional Chinese, to examine the reliability and validity, and explore the feasibility of its use in the elderly in the long-term care facility. A total of 60 older adults were enrolled in the study, the average age of the participants was 80.72±8.25, with female predominant (N=34, 56.7%), and the majority of them were never institutionalised (N=52, 86.7%). Most of the enrolled elderly were rather healthy with relatively low (median=5.00) in comorbided severity as indicated by Charlson Comorbidity Index (CCI). In reliability testing, the inter-rater reliability was assessed with Krippendorf's method, and K-alpha was 0.87, indicating a good reliability between raters. The internal validity testing showed the Cronbach's alpha of 0.93 for self-care functioning, 0.93 for disoriented behaviours, depression/anxiety, and irritable behaviour, and 0.78 for withdraw behaviours. We performed content validity and criterion-related validity to examine the construct validity. Content validity was assessed with item-level and scale-level content validity indices (I-CVI and S-CVI). Both I-CVI and S-CVI were 0.99, judged as excellent validity for content validity. The Pearson's r between targeted criterion and self-care functioning was -0.90, and -0.86 for disoriented behaviours. The Pearson's r of self-report criteria and depression/anxiety was 0.65 for Geriatric Depression Scale, 0.65 and 0.53 for DASS-D and DASS-A respectively. The Pearson's r in rater-rated scale was higher, and was 0.75 and 0.65 for NPI-dysphoric and NPI-anxiety respectively. The Pearson's r for the criterion and irritable behaviour was 0.82, and 0.72 for withdraw behaviours. Based on our results, the MOSES Traditional Chinese version showed good reliability and validity, and was similar to the psychometric properties in the original study. In conclusion, we believed that the MOSES Traditional Chinese version can be a reliable assessment tool for future practice and research in the field of long-term care.

Tao is the only tribe that has no liquor-making tradition among the fourteen indigenous peoples of Taiwan. Their ancestors did not make or drink liquors, and the word ‘liquor’ even does not exist in Tao people’s language. Many studies, however, suggested that liquors are now playing an important role in Tao people’s daily life. What started to bring liquors into the everyday life of a tribe that has no tradition of liquor-making? The objective of the present study is to describe the hsitorical and interpersonal context at the time when the first contact between Tao people and alcohol happened, and to know Tao people’s motivation and situation of drinking and their self-perception of health. The study adopts field observation of quality technique to understand alcohol use and the concept of drinking control in the tribe. This research subjects are three Tao elders who had no prior experience with alcohol until the first appearance of it in Lanyu. In order to reach the validity and the affection of study, data from an additional priest and a brethren in Ivalino (野銀) village were also included for the triangle confirmation in data analyses. The results showed that Tao people did not drink alcohol during the Japanese colonial rule period. After Taiwan restoration, the tobacco liquor regime of the monopoly was executed, and currency was introduced into Lanyu. The Tao people experienced drinking alcohol when they worked in Taiwan and brought the habit of alcohol drinking back to Lanyu. This was the key period when Tao people started to drink. Through the life experience of drinking of three Tao elders, this study describes the meanings of drinking and the self-perception of health. The study adopt field observation to document when and where Tao people drink and how the church tries to advocate the concept of control drinking and healthy drinking in the tribe. According to the result, it is suggested that health professionals in Lanyu should understand Tao tribe’s history contexts. In order to effectively advocate the concept of healthy drinking and to make it suitable for the local culture, the tribe health system should also include church, health centers, and the social support from the Tao tradition in the future.

Abstract Purpose: The main purpose of the study is to discuss the living quality effect of subjects who accept the training of traditional Taichiquan. Method: the study recruited general elderly volunteers (30 persons) living in Taipei and New Taipei City as participants. Before the start of experiment (sf-36), the evaluation of score before intervention (pre-test) via questionnaire was first conducted, and then the participants were asked to accept “traditional successive martial Taichiquan forms training” for 60 minutes each time, once every week over 8 weeks. After 8 weeks, the same questionnaire (sf-36) was adopted to evaluate the score after intervention(post-test). The t-test of dependent samples was adopted to exam the difference of participants before and after the training. Results: as comparing the sf-36 evaluation results before and after the training, the living quality effect of elderly after the intervention of Taichiquan training improve significantly, as well as the effect of physical functions. The enhancement for effect of character restriction due to physical function was insignificant. The enhancement for effect derived from emotional character restriction was insignificant. The enhancement for social function effect was insignificant. The effect of improving physical pain was significant. The vitality effect was significant. The psychological health improvement effect was significant. The general health effect improvement was significant. Conclusion : The training of traditional Taichiquan enhanced the perception of living quality on the elderly with no reqular physical activity. Key words: taichiquan, elderly, living quality
