Background. Clinical research found that TCM is therapeutic in treating gastric cancer. Clearing heat is the most common method, while some antirheumatic medicines are widely used in treatment as well. To explore the pharmacological mechanism, we researched the comparison between heat-clearing medicine and antirheumatic medicine in treating gastric cancer. Methods. First, related ingredients and targets were searched, respectively, and are shown in an active ingredient-target network. Combining the relevant targets of gastric cancer, we constructed a PPI network and MCODE network. Then, GO and KEGG enrichment analyses were conducted. Molecular docking experiments were performed to verify the affinity of targets and ligands. Finally, we analyzed the tumor immune infiltration on gene expression, somatic CNA, and clinical outcome. Results. A total of 31 ingredients and 90 targets of heat-clearing medicine, 31 ingredients and 186 targets of antirheumatic medicine, and 12,155 targets of gastric cancer were collected. Antirheumatic medicine ranked the top in all the enrichment analyses. In the KEGG pathway, both types of medicines were related to pathways in cancer. In the KEGG map, AR, MMP2, ERBB2, and TP53 were the most crucial targets. Key targets and ligands were docked with low binding energy. Analysis of tumor immune infiltration showed that the expressions of AR and ERBB2 were correlated with the abundance of immune infiltration and made a difference in clinical outcomes. Conclusions. Quercetin is an important ingredient in both heat-clearing medicine and antirheumatic medicine. AR signaling pathway exists in both types of medicines. The mechanism of the antitumor effect in antirheumatic medicine was similar to trastuzumab, a targeted drug aimed at ERBB2. Both types of medicines were significant in tumor immune infiltration. The immunology of gastric tumor deserves further research.

Traditional Chinese medicine (TCM) is a rich resource of anticancer drugs. Increasing bioactive natural compounds extracted from TCMs are known to exert significant antitumor effects, but the action mechanisms of TCMs are far from clear. Proteomics, a powerful platform to comprehensively profile drug-regulated proteins, has been widely applied to the mechanistic investigation of TCMs and the identification of drug targets. In this paper, we discuss several bioactive TCM products including terpenoids, flavonoids, and glycosides that were extensively investigated by proteomics to illustrate their antitumor mechanisms in various cancers. Interestingly, many of these natural compounds isolated from TCMs mostly exert their tumor-suppressing functions by specifically targeting mitochondria in cancer cells. These TCM components induce the loss of mitochondrial membrane potential, the release of cytochrome c, and the accumulation of ROS, initiating apoptosis cascade signaling. Proteomics provides systematic views that help to understand the molecular mechanisms of the TCM in tumor cells; it bears the inherent limitations in uncovering the drug-protein interactions, however. Subcellular fractionation may be coupled with proteomics to capture and identify target proteins in mitochondria-enriched lysates. Furthermore, translating mRNA analysis, a new technology profiling the drug-regulated genes in translatome level, may be integrated into the systematic investigation, revealing global information valuable for understanding the action mechanism of TCMs.

Colorectal cancer (CRC) is the third most diagnosed cancer worldwide and is a significant cause of cancer-related deaths. Previous studies have observed that Coptis chinensis (CC) and Mume Fructus (MF) are effective against CRC, enteritis, and intestinal dysbiosis, but the chemical and pharmacological mechanisms remain poorly understood. In this study, we employed pharmacological network analysis to reveal mechanisms underlying the therapeutic effect of CC and MF against CRC. All compounds and targeted genes were obtained from the traditional Chinese medicine systems pharmacology database and analysis platform (TCMSP). Differentially expressed genes (DEGs) were identified based on GSE146587, GSE156720, and GSE184093 datasets. A protein-protein interaction (PPI) network was constructed to identify putative target genes of CC and MF. Ten key targeted genes were identified, including CCND1, ICAM1, IL1B, IL-6, MMP1, MMP3, MMP9, MYC, SERPINE1, and VEGFA. Among these genes, six (ICAM1, IL1B, IL-6, MMP1, MMP3, MMP9, and SERPINE1) were positively correlated with levels of effector memory CD4 T cells and natural killer T cells, and three (CCND1, MYC, and VEGFA) were negatively correlated with type 17 T helper cells and CD56dim natural killer cells. Molecular docking analysis showed that four compounds of CC and MF (kaempferol, oleanolic acid, quercetin, and ursolic acid) could affect CRC by interacting with target genes. Our study proved that pharmacological analysis could reliably assess the mechanism of traditional Chinese medicines for treating cancer.

The purpose of the study was to research the attitudes of the Bulgaria citizens to alternative medicine methods among students at the Medical university in the city of Varna and administrative personnel - working people who are under a risk of professional deseases. Methods Randomly selected Bulgarian citizens were interviewed and divided in two groups: Group A – 129 students from the medical university of Varna age 18 to 30 years old and Group B – 121 people working in the in the administrative departments age 31 to 66 years old. Results The 37,2% of the people from Group A and 52, 1% of the people in group B were aware of the essence of the methods of the alternative medicine. The 62% of the students and the 75,2% of the administrative personnel thought that alternative methods used in the countries from the East are protecting and preventing diseases. The actual personal experience in using those methods in both groups was very low –below 10% for Group A and below 25% for Group B. Conclusions Bulgarian citizens are still not informed enough for the non-conventional methods and their application which needs to be directed in informing, educating and more practical work towards the targeted groups.

Background and Purpose: LCS101 is a botanical formula extracted from 14 botanical components. While conventional oncology focuses on targeted medicine, research on LCS101 adopts a multi-targeted approach, examining its preclinical (in vitro, in vivo, and ex vivo) and clinical (randomized controlled trial, pragmatic) effects. This includes examining the formula’s impact on the immune system, selective anticancer effects, and improved chemotherapy-related symptoms and quality of life. Effects on the Immune System: In murine splenic cell cultures, LCS101 significantly increased T-cell proliferation and macrophage tumor necrosis factor-α production. Blood samples from healthy volunteers exposed to LCS101 showed a dose-dependent increase in natural killer cell activity; and a randomized controlled trial showed significantly lower rates of leucopenia/neutropenia and anemia in patients with breast cancer undergoing chemotherapy. Selective Anticancer Effects: In vitro LCS101 demonstrated selective growth inhibition (on XTT viability assay) in human breast and prostate cancer cell lines, without any harmful effects on normal human epithelial cells. The anticancer effects were attributed to reactive oxygen species activity. Cytotoxic effects of doxorubicin and 5-fluorouracil on breast cancer cell lines were significantly increased following exposure to LCS101, with a protective effect in normal cells. Symptom Relief and Quality of Life: Clinical research shows that patients taking LCS101 during chemotherapy are less likely to report symptoms such as fatigue, pain, nausea and vomiting. Conclusion: LCS101 exhibits multi-targeted effects, with significant implications for cancer care. Further research is needed to better understand the impact of these findings.

Background. Korean medicine (KM) interventions are attractive for children with idiopathic short stature (ISS). We investigated the real-world clinical practice patterns of KM doctors in ISS treatment. Methods. The survey targeted KM doctors who have treated patients with ISS in KM clinical settings for >3 years. We included questions on the diagnosis and treatment patterns, effects, cost, and opinions of doctors on KM treatment for ISS. A frequency analysis was performed. Results. There were 58 respondents, and “heights of parents” (n = 57, 98.3%), “height, weight, and growth rate of child” (n = 55, 94.8%), and “amount of meals, digestive function” (n = 52, 89.7%) were frequently used as indicators for diagnosis and treatment. The most frequently used KM interventions were “herbal medicine” (n = 58, 100%), “acupuncture” (n = 49, 84.5%), “moxibustion” (n = 38, 65.5%), “dry cupping” (n = 26, 44.8%), and “physiotherapy” (n = 22, 37.9%). Herbal medicines were generally prescribed to tonify the spleen or kidney, and the most frequently used individual herbs were Acanthopanacis cortex, Astragali Radix, and Cervi parvum cornu. The most common acupuncture points were ST36, GB34, SP6, EX-LE5, and LI4. Conclusions. This study showed the use of KM for ISS in real-world clinical settings. In the future, well-designed clinical studies to verify the effect of KM treatment on ISS based on real-world clinical practice patterns should be actively performed.

Le syndrome métabolique (SMet) se caractérise par une obésité abdominale qui induit un cercle pernicieux auto-entretenu, dans lequel un état inflammatoire de bas grade et un stress oxydatif vont favoriser l'insulino-résistance pour conduire à terme au diabète de type II. Pour tenter d’empêcher ces troubles physiologiques, un régime alimentaire et une activité physique régulière sont proposés aux personnes à risque. Souvent, le traitement médicamenteux s’avère indispensable et expose le patient à une poly médication prolongée et a ces effets préjudiciables à long terme. Ce travail de thèse consiste à étudier une voie alternative par une médecine douce ciblée consistant en une prise régulière d’une boisson infusée d’Astéracées amères choisies pour leur richesse en sesquiterpènes lactones et en dérivés caffeoyls. Les premiers sont connus pour leur activité anti-inflammatoire et les seconds pour leurs effets à la fois antioxydant/anti-inflammatoire et insulino-sensibilisant. Une étude in vivo sur des rats « syndrome métabolique induit » a été conduite avec pour objectif de tester la pertinence d’une infusion d’Astéracées riche en dérivés caffeoyls dans la prévention du SMet. L’expérimentation a été menée sur le modèle de rat « fructose », un modèle nutritionnel prédisposant au SMet en quatre semaines. La comparaison des paramètres physiologiques de la cohorte d’animaux "témoin" nourris avec une alimentation normale et la cohorte d’animaux nourris avec une alimentation "riche en fructose" supplémentée ou non par l'infusion, nous a permis de montrer des effets patents de l’infusion sur la sensibilité à l’insuline et sur le poids des animaux. Par ailleurs, nous avons recherché de nouvelles substances naturelles toujours plus efficaces dans la lutte contre le stress oxydatif. Pour cela, nous avons procédé à un criblage cellulaire sur 8 extraits hydro alcooliques d’Astéracées amères visant à évaluer leurs effets protecteurs contre un stress oxydatif généré par H2O2. L’extrait de feuille de bardane présentait l’activité protectrice la plus puissante et a été retenu pour un fractionnement séquentiel visant à purifier et à identifier la molécule bioactive par RMN. Une analyse RT-qPCR et une approche d’amarrage moléculaire ont permis d’identifier la cible responsable de l’effet de protection. L’ensemble des résultats à conduit à l’identification d’un nouveau sesquiterpène lactone activateur de la glucose-6-phosphodeshydrogenase
Metabolic syndrome (MetS) is characterised by abdominal obesity that induces a self-perpetuating pernicious cycle in which low-grade inflammation and oxidative stress promote insulin resistance and ultimately lead to type II diabetes. In order to prevent these physiological disorders, diet and regular physical activity are recommended for patients at risk. Often, medical treatment is essential and exposes the patient to prolonged poly-drug therapy and its undermining consequences in the long term. This thesis work consists in studying an alternative way of targeted alternative medicine consisting in a regular intake of a drink infused with bitter Asteraceae chosen for their richness in sesquiterpene lactones and caffeoyl derivatives. The former are known for their anti-inflammatory activity and the latter for their antioxidant/anti-inflammatory and insulin-sensitising effects. An in vivo study on "induced metabolic syndrome" rats was conducted with the aim of testing the relevance of an infusion of Asteraceae rich in caffeoyl derivatives in the prevention of MetS. The experiment was conducted on the "fructose" rat model, a nutritional model predisposing to SMet in four weeks. The comparison of physiological parameters of the cohort of "control" animals fed with a normal diet and the cohort of animals fed with a "high fructose" diet supplemented or not by the infusion, allowed us to show clear effects of the infusion on the insulin sensitivity and on the weight of animals. In addition, we have researched new natural substances that are more effective in the fight against oxidative stress. To do this, we carried out a cellular screening on 8 hydroalcoholic extracts of bitter Asteraceae in order to evaluate their protective effects against oxidative stress generated by H2O2. The burdock leaf extract shows the most potent protective activity and was selected for a sequential fractionation aimed at purifying and identifying the bioactive molecule by NMR. RT-qPCR analysis and a molecular docking approach were used to identify the target responsible for the protective effect. The results led to the identification of a new sesquiterpene lactone that activates glucose-6-phosphodeshydrogenase

Este Trabalho Final de Mestrado tem como principal objetivo a elaboração de um Plano de Marketing estratégico para uma clínica de medicinas alternativas e complementares, a Natural Care, que irá prestar serviços no âmbito do modelo alternativo. A Natural Care pretende penetrar no mercado no início do ano 2019 e este projeto funcionará como um guia tático para definir as estratégias de marketing e para realizar ações que irão permitir gerar notoriedade e promover o serviço junto do seu público-alvo. A metodologia utilizada foi o método por inquérito (survey), com o objetivo de tomar conhecimento de sentimentos subconscientes dos consumidores, neste caso, as motivações. Numa primeira fase, foi realizada uma entrevista semi-estruturada de carácter informal a um profissional de medicina alternativa e complementar e, uma outra, a um consumidor deste tipo de medicinas, mais propriamente, de Acupuntura. Para uma maior compreensão da viabilidade do projeto foi realizado um inquérito por questionário online para identificar o potencial target da Natural Care e para dar respostas aos objetivos específicos referentes às motivações dos consumidores. Foram obtidas 715 respostas ao questionário, das quais apenas 605 foram consideradas válidas para dados estatísticos. Este trabalho permitiu concluir que, o que mais motiva os consumidores de medicinas alternativas e complementares a realizar os tratamentos é o facto de se sentirem renovados/energizados e aliviados do stress da vida quotidiana, porém, o que menos os motiva, é a perda de peso.
This Final Master Thesis has as main objective a Strategic Marketing Plan elaboration for an alternative and complementary medicine clinic, Natural Care, a company that will provide services in the alternative model. The Natural Care clinic intends to enter in the market in early 2019 and this plan will be a tactical guide to define the marketing strategies and to implement actions that will give notoriety and promote the service with its target audience. The research method used was the "survey", in order to obtain all knowledge of the consumer's behavior, more exactly the motivations. Initially, a semi-structured informal interview was conducted with an alternative and complementary medicine practitioner. In a second phase, an interview was also made to an alternative and complementary medicines patient, more properly Acupuncture. For a better understanding of the project feasibility an online questionnaire survey was conducted to identify the potential target of Natural Care and respond at the specific objectives regarding the consumer's motivations. A total of 715 responses to the questionnaire were collected, which only 605 were considered valid for the statistical data. With this case study we can conclude that the feeling of renewal and energized is what motivates patients to use alternative medicine. Helping the stress release of daily life is also one of the main reasons for the demand for these treatments. On the other hand, the weight loss is what least motivates consumers of alternative and complementary medicines to consume these treatments.

Acquired Immunodeficiency Syndrome (AIDS) is a human disease caused by the human immunodeficiency virus type 1 (HIV-1) and it is one of the biggest social, economic and health challenges in the world. The Joint United Nations Programme on HIV/AIDS (UNAIDS) and World Health Organization (WHO) estimated that between 33.4 to 46.0 million people around the world were living with HIV/AIDS in December 2005 and the highest estimates are in the Sub-Saharan Africa (around 25 million). In more developed countries a combined antiretroviral therapy called highly active antiretroviral therapy (HAART) is used, which results in reduced progression to AIDS in most patients. Despite the beneficial effects of HAART, significant side effects are experienced by treated patients. In addition, most infected people live in countries where the treatment is very expensive or, in many cases, not available at all. These people therefore rely on medicinal plants for health care. In this study, aqueous extracts from Sutherlandia frutescens and Lobostemon trigonus were screened for potential anti-HIV activities in a series of in vitro enzymatic assays, including reverse transcriptase, HIV-1 protease and glycohydrolases. Two extracts of Sutherlandia leaves (SFL-1 and SFL-2) were prepared that inhibited HIV reverse transcriptase and a Lobostemon leaf extract (LTL) was shown to also inhibit this enzyme. All extracts were assayed at 1.25mg/ml. Tannin content was determined for all active extracts using a tannic acid assay. SFL-1 and SFL-2 were found to contain about 6 percent and 7 percent tannins, respectively, and LTL contained 31% tannins by weight. Tannins were removed using polyamide columns and three fractions were collected for each. The extracts were also fractionated with Sephadex G-25, Amberlite IR 120 and Dowex 1-X8 as size exclusion, cation exchange and anion exchange, respectively. Extracts were also fractionated by preparative thin layer chromatography where two compounds were separated from S. frutescens extract with high activity against reverse transcriptase while showing insignificant inhibition towards other enzymes tested. SFL-BFW-10 and SFL-WEF-7 inhibited reverse transcriptase by almost 100 percent and the IC50 values calculated for these compounds were 0.34 and 0.23mg/ml, respectively. Cytotoxicity of these compounds was evaluated on Chang liver cells and peripheral blood mononuclear cells (PBMCs). None of these compounds showed any significant inhibition of cell proliferation. The purity of these compounds could not be confirmed because there was insufficient material to use in the techniques required to show purity and identification. Therefore, TLC was used to determine the nature of these compounds. SFL-BFW-10 was identified as an organic acid and SFL-WEF-7 was identified as flavonoid.

The dissertation describes a novel method for plant drug discovery based on mutation and selection of plant cells. Despite the industry focus on chemical synthesis, plants remain a source of potent and complex bioactive metabolites. Many of these have evolved as defensive compounds targeted on key proteins in the CNS of herbivorous insects, for example the insect dopamine transporter (DAT). Because of homology with the human DAT protein some of these metabolites have high abuse potential, but others may be valuable in treating drug dependence. This dissertation redirects the evolution of a native Lobelia species toward metabolites with greater activity at this therapeutic target, i.e. the human DAT. This was achieved by expressing the human DAT protein in transgenic plant cells and selecting gain-of-function mutants for survival on medium containing a neurotoxin that is accumulated by the human DAT. This created a sub-population of mutants with increased DAT inhibitory activity. Some of the active metabolites in these mutants are novel (i.e. not detectable in wild-type cells). Others are cytoprotective, and also protect DAergic neurons against the neurotoxin. This provides proof-of-concept for a novel plant drug discovery platform, which is applicable to many different therapeutic target proteins and plant species.

Current research suggests resveratrol, a phytoalexin found predominately in grapes, may function as a chemopreventive and/or chemotherapeutic agent for various cancers, including colorectal cancer. However, the underlying mechanism(s) involved in these activities remain elusive. Thus, the objective of the studies discussed here sought to investigate the effect of resveratrol treatment on gene modulation in human colorectal cancer cells in order to identify and characterize novel molecular targets that contribute to the observed anticancer activities of resveratrol. Here, we identify activating transcription factor 3 (ATF3) and early growth response-1 (Egr-1) as novel targets of resveratrol and provide data to elucidate the mechanism(s) of regulation and how each target contributes to the anticancer effect of resveratrol in colorectal cancer cells. We demonstrate the involvement of resveratrol in ATF3 transcriptional regulation, which is facilitated by Egr-1 and Krüppel-like factor 4 interactions, and show that ATF3 contributes, at least partially, to resveratrol-induced apoptosis (Chapter 3). Moreover, we suggest that increased Egr-1 transcriptional activity by resveratrol requires posttranslational acetylation of Egr-1 in a SIRT1-independent manner. This acetylation by resveratrol may contribute to Egr-1-mediated expression of the pro-apoptotic protein nonsteroidal anti-inflammatory drug (NSAID)-activated gene-1 (NAG-1) induced by the phytoalexin (Chapter 4). Taken together, the work presented here provide (1) novel mechanisms by which resveratrol induces ATF3 and Egr-1 expression and (2) represent additional explanations for the anti-tumorigenic/anti-carcinogenic effects of resveratrol in human colorectal cancer cells.

Management of Sjögren’s syndrome (SS) encompasses confirmation of diagnosis, disease assessment, and treatment of glandular and systemic manifestations including special situations such as pregnancy and SS-related lymphoma. The American European Consensus Group (AECG) classification criteria 2002 are the current gold standard for the diagnosis of SS. Salivary gland sialometry, sialochemistry, and ultrasound and tear osmolarity may be useful adjuncts. Recently, preliminary classification criteria of the American College of Rheumatology have been introduced as an alternative to the AECG criteria. Symptoms of SS are non-specific and must be actively explored. When assessing patients with SS, it is important to consider not only objective parameters such as abnormalities in blood tests and changes in tear and salivary flow, but also patient-reported outcome measures and impact on quality of life. Current management of patients with SS is hampered by the lack of evidence-based strategies. The symptoms experienced by patients with SS are often not fully appreciated by clinicians, which may contribute to the suboptimal management of the condition. Management of fatigue remains a major challenge and a holistic, multidisciplinary approach is recommended. Factors that may contribute to fatigue should be fully addressed. Recent advances in the understanding of the pathogenic mechanisms of SS have informed more targeted therapeutic strategies with some promising data. Optimal management of SS requires expertise from different disciplines. Combined clinics with rheumatology, oral medicine, and ophthalmology input will improve care and communications as well as reduce the number of clinic visits for patients and healthcare-related cost. Effective link between pSS specialists, dentists, opticians, and general practitioners will facilitate early diagnosis and reduce risk of long-term disability of SS.

In this chapter, the main causes of male factor infertility, specifically endocrine abnormalities and effect of endocrine disrupters, will be outlined. For many patients, conventional therapy presents considerable financial strain and moral dilemma. In this context, many patients are preferring complementary medicine (CAM). Hence, the promises offered by herbal medicine including Ayurveda, Arab, and Chinese medicine will be explored in this chapter. Many naturopathic medicaments such as Withania somnifera, Asparagus racemosus, Curculigo orchioides, Zingiber officinale, etc. are being routinely used as part of traditional medicine practice in Ayurveda and Arab medicine and gaining wider acceptance in other countries. Common herbs, targeted infertility condition, and results thereafter shall be discussed. In the concluding part of the chapter, the potential deleterious contraindications of alternate medicine such as lead toxicity from Ayurvedic medications shall be briefly discussed.

This chapter discusses effective diagnostics and treatment of canine hepatic carcinoma (CHC), where state-of-the-art global technologies are complemented by traditional Chinese veterinary medicine (TCVM). The biokinetic Ga-67 model of CHC is proposed to clarify the Ga-67 metabolic mechanism among various organs. It is aimed at identifying the best routine for detecting the metastatic or primary CHC and substantiating the optimal further treatment. The routine examination of CHC can be performed via Ga-67 nuclear examination or MRI, biological index, X-ray, and abdominal ultrasound. The available methods of animal cancer treatment imply separate or combined application of surgery, radiation therapy, and chemotherapy targeted at the particular cancer cells. However, there is also a general concern on the quality of life of pets/canine patients. This leaves enough space to the TCVM (including acupuncture and famous herbal drugs) with a long application history in Asia and growing usage as alternative treatment in other regions. However, its current applications to domestic animals/pets suffering from carcinomas are based on individual expert opinions, while there are no outlined veterinary treatment strategies and guidelines for clinical practice in this field. A comprehensive combination of state-of-the-art global technologies and TCVM is considered instrumental in curing canine hepatic carcinoma.

Indigenous knowledge (IK) has been viewed as local knowledge that has been developed and accumulated, over time, by a community and has been passed down over generations. Such knowledge is represented in most spheres of human activity, such as in agriculture, traditional and alternative medicine, human and animal health, forestry and botany, among others. The purpose of this chapter is to discuss how IK is accessed and used by agricultural extension workers in Zimbabwe. The study reviews the relevant literature and focuses largely on Indigenous Agricultural Knowledge (IAK). The study utilises both quantitative and qualitative methods; a questionnaire was distributed and extension workers drawn from eight provinces of Zimbabwe. Mashonaland Central Province produced the highest number of respondents because the population for the province included ward and village extension workers in addition to the district and provincial extension officers and supervisors targeted in each province. From the projected sixty (60), forty four (44) districts participated. The study observed that indigenous knowledge is relevant in modern day agriculture and should be given sufficient attention in extension work. The study recommends that IK be documented and integrated into research, education and training for posterity.

Phytochemicals derived from different plants are promising therapeutic agents. Herbal compounds can be used under diseases, etiological causes of which are alterations of carbohydrate, protein, and lipid metabolisms, along with increased oxidative stress and chronic low-grade inflammation. Potential sources of biologically active substances may be grape wine, rich in phenolic compounds. Well-studied examples of polyphenols are phenolic acids, catechins, anthocyanins, and flavonoids, etc. Another source of biologically active compounds is yacon (Smallanthus sonchifolius Poepp. & Endl.). The aboveground part of yacon is rich in phenolic compounds and terpenes. Main biologically active substances from tuberous roots of yacon are fructooligosaccharides and phenolic compounds. The section will be devoted to the analysis of hypoglycemic and antioxidant effects, and molecular targets of the complex of biologically active substances derived from red wine and yacon.

Target drug delivery is one of the core issues in modern medicine. Although both thermo-sensitive and magnetic drug vehicles have been developed for this purpose, reliable drug targeting is yet to achieve, because it’s hard to control local temperature in body for thermo-sensitive drugs, and it’s also difficult to control the colloidal sizes of magnetic vehicles to meet the requirements for both long-time circulation and magnetic responsibility. Here we present a new technology to solve these two problems. The drug (taxel) was combined with hyper-branched thermo-sensitive polymer and magnetic nanoparticles. Magnetic responsibility of the complex can be remotely controlled by applying an alternating magnetic field. The untreated complexes were stable in dispersion, while after exposing to the alternating magnetic filed for 5 minutes, the complexes were destabilized and deposited from dispersion quickly. Unlike the commonly used water bath, the technology present here can heat up the drug vehicles remotely from outside, which must have great potentials in the applications of magnetic or thermo-sensitive drug delivery.

Abstract Nuclear reactors, cyclotrons and generators are being used to produce nuclear medicine radionuclides. Radiopharmaceuticals based on Gallium-68 are widely used both experimentally and clinically in Positron Emission Tomography (PET). Currently Ga-68 is mainly produced via 68Ge/68Ga generators, which are expensive. An alternative method is by cyclotron using high enriched Zinc-68 via the 68Zn(p,n)68Ga reaction. Investigations have been conducted for producing Ga-68 using a solid target impacted with a proton beam in energies between (13–14) MeV or higher. The main interest of this work is to produce Ga-68 using energies within a lower range, therefore Ga-68 can be accessed to locations with cyclotrons of lower energy. In this article, an approach to a novel target design to get Ga-68 with low energy protons is conducted. Adequate shielding to avoid extra doses outer the cyclotron is also considered. Monte Carlo code FLUKA is used to simulate the processes of interaction of radiation with matter and the radioactive isotope production. For a beam energy of 11 MeV and current on target of 40 μA the simulated produced activity is about 69.1 GBq. The results show the feasibility of producing Ga-68 with low energy protons and self-shielding system guarantees the regulation-based dose exposition outer the cyclotron.

Magnetic microcapsules are often used as vesicles in targeted drug delivery systems, where focused magnetic field propels the capsules to highly specific locations in tissue. To fully realize this potential it is important to understand the dynamics of magnetically-responsive micrometer sized particles in viscous fluids and the effect of boundaries on particle motion. Furthermore, for practical biomedical applications, it could be useful to create synthetic micrometer-sized vesicles able to perform controlled self-propelled motion. Herein, using computer simulations, we examine the motion of magnetically-responsive synthetic microcapsules that able to crawl along walls in microchannels filled with a viscous fluid. The compliant fluid-filled capsules considered in this study encompass superparamagnetic nanoparticles in their solid shells and, therefore, can be manipulated by alternating magnetic forces.

Tuberculosis (TB) is one of the most widely spread diseases. In 2006, 9.2 million new TB cases were reported with 1.7 million victims [1]. To diagnose TB, Mycobacterium tuberculosis (MTB) is identified in clinical samples. The challenge of TB diagnostics is high-performance screening conducted by nontrained personnel. Currently, nucleic acid testing with target-amplification strategy such as polymerase chain reaction (PCR) is available for detection of TB. However, this entails cumbersome procedures run by skilled operators with expensive instrumentation and reagents. To overcome these challenges, this paper presents a nanotip sensor to diagnose TB rapidly without target-amplification. The proposed methodology uses a nanostructured tip as a biosensor to detect target analytes. The novelty of this approach is in the superior concentration and detection mechanisms of nucleic acids on the terminal end of a nanotip using an alternating current (AC) electric field, specific chemical binding, and capillary action. Confirmatory identification of MTB is achieved by detecting MTB strains on a nanostructured tip through DNA hybridization. In this paper, the working principle is presented with the demonstration of amplification-free detection of MTB genomic DNA using the nanotip sensor. The performance of the tip sensor is characterized.

Good Manufacturing Practices (GMP), a component of Pharmaceutical Quality Systems, is aimed primarily at managing and minimizing the risks inherent in pharmaceutical manufacture to ensure the quality, safety and efficacy of products. Provision of adequate number of personnel with the necessary qualifications/practical experience and their continuous training and evaluation of effectiveness of the training is the responsibility of the manufacturer. (World Health Organization [WHO], 2014; International Organization for Standardization [ISO], 2015). The classroom method of training that has been used for GMP capacity building in the pharmaceutical manufacturing industry in Nigeria over the years, delivered by experts from stringently regulated markets, have not yielded commensurate improvement in the Quality Management Systems (QMS) in the industry. It is necessary and long over-due to explore an alternative training method that has a track record of success in other sectors. A lot of studies carried out on mentoring as a development tool in several fields such as academia, medicine, business, research etc., reported positive outcomes. The aim of this study was to explore mentoring as an alternative GMP training method in the pharmaceutical manufacturing industry in Nigeria. Specifically, the aim of this study was to evaluate the impact of mentoring as a GMP capability building tool in the pharmaceutical manufacturing industry in Nigeria, with focus on GMP documentations in XYZ pharmaceutical manufacturing company located in South-Western region of Nigeria. The methodology comprised gap assessment of GMP documentation of XYZ company to generate current state data, development of training materials based on the identified gaps and use of the training materials for the mentoring sessions. The outcome of the study was outstanding as gap assessment identified the areas of need that enabled development efforts to be targeted at these areas, unlike generic classroom training. The mentees’ acceptance of the mentoring support was evident by their request for additional training in some other areas related to the microbiology operations that were not covered in the gap assessment. This result portrays mentoring as a promising tool for GMP capacity building, but more structured studies need to be conducted in this area to generate results that can be generalized.

After initiating health sector reforms in 1994, the Bénin government established the Integrated Family Health Project, known as PROSAF. Funded by the U.S. Agency for International Development, PROSAF operates in the Borgou region, which is mostly rural and has the country’s most severe health problems. PROSAF managers wanted to understand why local people were not using health services, despite their poor health. As noted in this brief, managers requested that the African Population and Health Research Centre (APHRC) study the way households and communities make decisions on health care. In a study conducted in 2000 with support from the Population Council, APHRC identified sociocultural factors that might impede access to health care in the region and recommended approaches to overcome these obstacles. Study findings included that adult men make unilateral decisions in household resource allocation and health care; people prefer alternative health care, mainly traditional healers or self-medication, because of their low cost; use of modern medicine tends to occur as a last resort, and only when symptoms are advanced; communication about FP is limited, both between husbands and wives and between parents and children.
