The matter of the distinctiveness and the association between anxiety and depression in childhood is an empiric, theoretical, and clinical issue. The principal objective of the present thesis was to describe the development of the association between AS and DS during the school-age period, first by documenting changes and continuity in their association in a first article, and then by documenting the longitudinal contributions between AS and DS and their etiological underpinnings in a second article. Teachers assessed AS and DS in 1112 twins at 5 time points from Kindergarten to Grade 6 (6 to 12 years of age). First, scale means, variances and inter-scale correlations were compared through time to assess the stability of AS and DS and of their association. Different factorial structures were compared. Analyses revealed that AS and DS were modestly stable at best from the beginning to the end of elementary school and that a bidimensional conceptualization, i.e. AS and DS as two distinct constructs, was supported by the data even though they remain highly correlated within this developmental period. Second, a genetic cross-lagged model was used to examine the genetic and environmental contributions to cross-sectional associations, cross-age and cross-lag effects of AS and DS from Kindergarten to Grade 6. Results suggested that AS and DS were associated throughout the elementary school-years and relatively stable due to common genetic and unique environmental influences. In the early school-years, DS are more stable than AS, and more central to the development of both subsequent AS and DS; AS only marginally contributed to subsequent DS in this sample. The innovative results of the thesis underline the importance of certain explanatory hypotheses of the association between anxiety and depression, inform about the distinctiveness of the two sets of symptoms and allow to generate additional hypotheses concerning the mechanisms implicated in the association.

Perinatal mental health is a major societal concern; suicide has become the second cause of maternal mortality after cardiovascular disease, representing 13.4% of maternal deaths. The objective of this research was to study psychopathology among mothers during the transition to motherhood, in relation to contextual and relational variables as well as attachment representations.A first study was aimed at identifying trajectories of depression and anxiety during the transition to motherhood. Our sample consisted of 148 Canadian women (mean age: 21,66 years). They filled in a socio-demographic questionnaire, an inventory of psychological symptoms (Symptom Check-List, SCL-90), a measure of psychological stress (PSM), a dyadic adjustment scale (The Dyadic Adjustment Scale, DAS) and a social support questionnaire (SSQ). In a second study, we examined the links between burnout and anxious-depressive symptomatology. Our sample consisted of 70 French women (mean age: 31,19 years) who completed the Edinburgh Postnatal Depression Scale (EPSD), an Anxiety Inventory (STAI Trait/Etat), and the Parental Burnout Assessment (BPA). In a third study, we examined the links between attachment, anxious-depressive symptoms, and burnout. Thirty-five women (mean age: 31.32 years) filled in the EPSD, the STAI Trait/Etat and the PBA. The Attachment Multiple Model Interview was also administered to assess attachment to each parent and partner. Finally, to illustrate the results of our final study, we used the same sample with a qualitative approach.The results showed: (1) the existence of three distinct trajectories of depression and anxiety, one of them with a high level of symptoms that continued to increase during the transition to motherhood ; (2) that mothers’ family and personal environments can be linked to a set of correlated factors (family income, psychological stress, marital satisfaction, perception of social support) that predict mental health trajectories throughout the transition to motherhood ; (3) that disorganized attachment in the relationship with one’s own mother is linked to a composite measure of depressive and trait anxiety symptoms in the postpartum period ; (4) that deactivating attachment strategies with the partner are associated with having an anxious state in the postpartum period ; (5) that trait anxiety mediates the link between maternal attachment and parental burnout.Our results highlight the importance of:- prioritizing trajectory studies in the transition to motherhood. They make it possible to consider inter-individual differences in maternal profiles of adaptation, highlighting the need to pay particular attention to mothers at risk.- taking into account the personal and family context in which pregnancy takes place to anticipate the trajectories of depression and anxiety during the transition to motherhood.- a prenatal assessment allowing systematic early detection of mothers at risk.- systematic screening for anxiety during the prenatal examination to prevent burnout.- pay specific attention to attachment models and strategies in the postpartum period in order to intervene early through effective psychotherapies

The study aim is to propose a complementary lecture of an epidemiological study which insists on psychological resource suitable for protecting the development or the aggravation of depressive and/or anxiety disorders. From rigorous methodology, 500 participants from 4 European countries have been interviewed during their consultation or visit at a primary care service. Firstly, differentiation of depressive and/or anxiety disorders prevalence (SCID-I/NP) risk factors from protective factors have been underlined. Contrary to literature, our study has emphasized a low number of factors linked with these disorders. Secondly, the presence of depressive and/or anxiety disorders is associated with intense psychological distress, assessed by psychological distress scales (GHQ-12 and MHI-5), particularly with comorbidity and depressive disorders. Thirdly, the study assesses the screening utility of both psychological distress scales and both diagnosis instruments, CIDI-SF and CIS-R, for diagnosable depression and anxiety disorders in a general population sample. Both scales and the CIDI-SF perform in detection of these disorders. Because the psychological distress scales are unable to produce precise diagnosis, we recommend the use of these scales and particularly the MHI-5 as a screening instrument which could be linked with the CIDI-SF in order to produce diagnosis. Finally, our study points out the sense of mastery such as a psychological resource susceptible for coping with depressive and/or anxiety disorders

Whereas a happy motherhood is a collective aspiration, it is known that many women are reporting psychological distress during the perinatal period. Yet, women’s psychological health can not be considered regardless of their roles in society and norms associated. This research work aims to test a psychosocial and cultural model to understand adjustment disorder regarding motherhood (postpartum depressive symptoms and burnout). In a first study, various steps with different populations (N = 250, 22, 474, 249 and 231) have led to the validation in French of a scale assessing beliefs associated with the maternal role. Then, a longitudinal follow up was settled with women who were seen during late pregnancy and at 2 and 4 months postpartum (N = 129). This work has served to demonstrate the convergent, predictive and construct validity of maternal burnout. Furthermore, the impact of motherhood psychosocial and normative characteristics in the development of depressive and burnout symptoms have been explored. Finally, cluster analyses were conducted to identify more or less adaptive patterns regarding articulation of investment in motherhood and working life. This research work has opened research avenues regarding maternal burnout in the postpartum period and considerations of socio-normative aspects of the mothering role regarding prevention and care of those troubles

Pour les femmes, le choix de devenir parent a beaucoup évolué dans les dernières décennies, puisqu'elles ont désormais une plus grande liberté de choix. Toutefois, la transition à la maternité peut amener les femmes à vivre beaucoup de pression liée aux recommandations des experts de la santé, aux médias, aux attentes de l'entourage ou encore à leurs propres perceptions quant à l'atteinte du standard de « bonne mère ». Cette étude a pour objectif d'examiner la transition à la maternité afin de mieux comprendre les différents facteurs pouvant être liés au développement de symptômes dépressifs postnataux et à l'ajustement au nouveau rôle de mère. Au total, 252 femmes ont rempli des questionnaires en période prénatale et postnatale. Les résultats confirment que les événements de vie stressants durant la grossesse, l'estime de soi, la perception du tempérament de l'enfant et la qualité de la relation conjugale suite à la naissance de l'enfant sont liés significativement au développement de symptômes dépressifs postnataux et à l'ajustement maternel. Un nouveau facteur de risque au développement de symptômes anxieux postnataux et des difficultés d'ajustement maternel a également été identifié : la motivation envers les tâches parentales. Les mères qui font les tâches par sentiment d'obligation pour correspondre aux standards de « bonne mère » ou pour répondre aux attentes de leur entourage auront davantage de symptômes anxieux et un moins bon ajustement maternel. En ce qui concerne la motivation à avoir un enfant, celle-ci n'aurait pas de contribution significative pour expliquer l'ajustement au rôle de mère. Finalement, les résultats de cette étude relèvent un biais de désirabilité sociale chez certaines femmes puisqu'elles peuvent avoir tendance à minimiser l'ampleur de leurs problèmes et de leurs insatisfactions. Cette tendance à l'autoduperie pourrait avoir un impact sur leur bien-être psychologique en entravant d'éventuelles demandes d'aide.

Facing with adversity, individuals may turns to spirituality and use it as a personal resource to cope difficult life events. Complex to analyze, spirituality is an area of research, relatively recent across the Atlantic, revealing a growing interest in the scientific literature. In France, this matter’s being little explored, so it is worth to provide a better understanding of its effect on health.The aim of this study was to access the potential effect of spirituality in relation to health’s indicators, such as anxiety, depression and quality of life, and to identify spiritual resources that may have a role in the quality of life as well.A mixed methodology was used. The qualitative component consists in three studies: the first concerns a student population (n = 219), the second consists in a population suffering from cancer (n = 42) and the third is a cardiac population (n = 46). Research instruments included assessment of spirituality (DSES, IWSRI and SWBS), anxiety and depression (HADS) and quality of life (WHOQOL-BREF). The quantitative component is based on semi-directive interviews conducted with both student population (n = 14) and cardiac patients (n = 9).Generally, results demonstrate that spirituality, as measured by the existential well-being, involve a relationship with psychological distress and quality of life with respect to those three populations. Spirituality also seems, through some indicators, to have an impact on the quality of life, either alone or in combination with psychological distress. It should be noted that spirituality may influence the quality of life positively but negatively as well.The spirituality reveals therapeutic potential by its influence on quality of life and raise the usefulness of studying spirituality in the context of health

As the prevalence of Alzheimer’s disease (AD) rises in Canada and worldwide, it is imperative to increase our understanding of its prodromal stages. This dissertation contributes to the body of research in this field by exploring the cognitive characteristics of three at-risk groups: those with amnestic mild cognitive impairment (aMCI), late-life depression (LLD) and aMCI with concomitant depressive symptoms (aMCI/D+). The cognitive performance of these groups is compared to that of healthy elderly control subjects (CONT) on three experimental tasks. The first explores the influence of emotional valence on memory for positive, negative and neutral word lists. Results reveal that, compared to neutral words, CONT and aMCI/D+ subjects recall more emotional (positive and negative) words, aMCI subjects recall more positive words, and LLD subjects recall more negative words. The second experiment investigates the effect of valence and intensity on memory for emotional images. Results show that valence is associated with recall of positive and negative images in the aMCI and CONT groups, and with recall of negative images in the aMCI/D+ group. Valence is not associated with recall in the LLD group. In addition, the stimuli’s emotional intensity is positively associated with recall of positive images in CONT subjects, but negatively associated with recall of positive images in aMCI subjects. Intensity is positively associated with recall of negative images in aMCI/D+ and LLD subjects. A third experiment compares these same groups on their semantic knowledge of biological and man-made objects. Only aMCI/D+ are impaired on this task, and their perofmrance is particularly impaired for biological items. Performance of aMCI and LLD groups, on the other hand, is normal. This dissertation provides compelling evidence that aMCI, aMCI/D+ and LLD individuals present distinct cognitive characteristics, namely on tests of episodic and semantic memory. The results have important theoretical and clinical implications, in that they contribute to clarifying the semiological features of each group, and corroborate the notion that each of these syndromes is cognitively distinct. In addition, these results contribute to clarifying the nosology of aMCI/D+, which paves the way for future research exploring treatment opportunities for this condition.

Compte tenu du nombre grandissant de cas de maladie d'Alzheimer (MA), il est impératif d'accroître notre compréhension de sa phase prodromique. Cette thèse vise à contribuer aux travaux dans ce domaine en caractérisant le fonctionnement cognitif d’individus à risque de développer la MA : ceux avec un trouble cognitif léger amnésique (TCLa), une dépression gériatrique (DEP) et un TCLa avec symptômes dépressifs concomitants (TCLa/D+). Leurs performances sont comparées à celles de participants contrôles (CONT) dans trois expériences. La première explore l’influence de la valence émotionnelle des stimuli sur les performances mnésiques pour des mots positifs, négatifs et neutres. Comparativement aux mots neutres, les CONT et TCLa/D+ rappellent davantage de mots émotionnels (positifs et négatifs), les TCLa rappellent davantage de mots positifs et les DEP rappellent davantage de mots négatifs. Dans la deuxième expérience, l’influence de la valence et de l’intensité émotionnelles des stimuli sur les performances mnésiques est testée à l’aide d’images. Les résultats révèlent une association entre les images émotionnelles et les performances des TCLa et CONT, et entre les images négatives et les performances des TCLa/D+. La valence n'est pas associée au rappel chez les DEP. Par ailleurs, l’intensité émotionnelle des stimuli est positivement associée au rappel d’images positives chez les CONT, et au rappel d’images négatives chez les TCLa/D+ et DEP. L’intensité est négativement associée au rappel d’images positives chez les TCLa. Dans une troisième étude comparant ces groupes sur leurs connaissances sémantiques d’objets biologiques et manufacturés, seuls les TCLa/D+ présentent des déficits, surtout en ce qui concerne les objets biologiques. En revanche, les TCLa et DEP obtiennent des résultats normaux. Globalement, cette thèse démontre que les individus avec TCLa, TCLa/D+ et DEP présentent des caractéristiques distinctes au plan de la mémoire épisodique et de la mémoire sémantique. Les conclusions ont d’importantes retombées théoriques et cliniques. Notamment, elles éclaircissent les caractéristiques sémiologiques de chaque groupe et appuient la notion qu’ils correspondent à des syndromes distinguables sur le plan du fonctionnement cognitif. De plus, les résultats contribuent à clarifier la nosologie du TCLa/D+, ce qui ouvre la voie aux recherches futures sur les traitements adaptés à cette condition.

Postnatal depression, very frequent during the postpartum period, is likely to influence child development ; few studies investigated the specific impact of early postnatal depressive symptoms (PNDS). To assess i) the impact of obstetrical complications (OC's) and of behavioral characteristics of the new-born on PNDS at 6 weeks ii) whether 6 week PNDS predict impaired child development independently from later maternal depressive symptoms iii) direct and indirect effects of these factors on child development at 6 months and 2 years. 598 dyads were followed up over 2 years, with repeated measures of maternal PNDS and child development. The children of mothers with PNDS at 6 w. Were more likely to present with poor cognitive outcome, association partly explained by the existence of maternal depressive symptoms over the follow-up. OC's and new-born behavioral characteristics didn't change these results. Causal pathways analyses show effects of new-born behavioral characteristics and of anxious disorders during pregnancy on child's development. Child developmental outcome might be influenced by both antenatal and postnatal maternal factors as well as by early infants’ factors

Depression is highly common throughout the life course and dementia is common in late life. Depression has been linked with dementia, yet the direction and pathological mechanisms of this association (whether depression is a prodromal feature or consequence of, or a risk factor for dementia) remains unclear. Vascular risk factors are associated to the risk of incident cognitive impairment and dementia and comorbid vascular disease is a feature of depression in latelife. Therefore, the hypothesis that vascular risk factors are the ultim denominator of psychological perturbations and dementia is to be verified. The aim of this work was to study the links between blood pressure, depression and dementia. In a cohort of 9294 community-dwelling elderly individuals aged 65 years and over, participating to the longitudinal population-based 3 City Study, followed up for 4 years, we found an association between baseline depressive symptoms’ severity and the incident dementia risk (particularly of vascular type). Conversely, we found no association between history of depression and incident dementia. Moreover, our cross sectional analyses exhibited an inverse association between systolic and diastolic blood pressure values and depression. Overall, however, the association between depressive symptoms and dementia was not mediated by hypertension. These results indicate that depression is rather a prodromal symptom of vascular dementia than a risk factor for it. The explanation for the inverse association found between blood pressure values and depression is not straightforward. Low blood pressure may lead to cerebral hypoperfusion found to be associated with white matter lesions in cerebral regions vulnerable to alterations of cerebral blood flow, and associated with cognitive impairment and depression. This work offers the opportunity for the definition of group of populations at high risk to develop dementia, vascular one in particular. It also gives the perspective of research on the biological mechanisms linking blood pressure and depression. Currently in Canada, more than one million people are living with the sequelae of an acquired brain injury. The incidence of brain injury outnumber breast cancer, AIDs, spinal cord injury and multiple sclerosis - combined. Traumatic brain injury (TBI) is now considered a chronic medical condition and a major public health concern whose consequences can persist and progress. Although depression is one of the psychological disorders with the highest prevalence rates post-TBI, there are few studies of nonpharmacological interventions for this issue. This lack of empirical evidence has a direct impact on TBI patients, leaving them without recognized efficacious treatment adapted to the specificities of their condition. Behavioural Activation (BA) has been demonstrated to be as effective as cognitive behaviour therapy (CBT) but is less cognitively demanding and thus potentially more suitable for people with brain impairment. However, no study has been yet done to verify its feasibility and efficacy in the TBI population. The objective of this single-case study was to preliminary evaluate the feasibility and efficacy of BA administered in a low-intensity guided self-help format for depressive symptoms in adults (18 years and older) having sustained TBI, using a single-case experimental design. This study also aimed to evaluate the potential benefits of BA regarding quality of life and social participation. Five participants with clinically significant depressive symptoms were enrolled in the study and were asked to complete mood assessments during the baseline, treatment and follow-up. Only two of them completed the entire protocol. This project demonstrated the feasibility of BA intervention on mild to moderate depression symptoms in adults with mild TBI. Modest evidence in support of BA to decrease depressive symptoms and quality of life was found but BA had no effect on social participation.

Currently, there exists no psychological theory or multifactorial model that adequately explains the causal factors that contribute to the development of gambling disorder. Progress in this domain is hindered by the fact that there is no scientific consensus about the conceptualization of this disorder. Blaszczynski and Nower (2002) were among the first to state that gamblers are a heterogenous group and to study subgroups of pathological gamblers. Their work and that of their successors has shown that the popular hypothesis according to which pathological gamblers are a homogenous group is false, and that treatments derived from these models cannot be uniformly applied to all gamblers without consideration for certain crucial aspects, such as type of gambling game, gender, developmental history and neurobiology. Comprised of two studies, the current thesis aims to further the knowledge in this vein of work. Inspired by the Pathways model of pathological gambling (Blaszczynski, & Nower, 2002), and by the psycho-structural interaction theory (Griffiths, 1993), this thesis aims to develop a better understanding of the influence of the structural characteristics of gambling games on gambling behaviours by comparing individuals who gamble on video lottery terminals (VLT) or at poker. More specifically, the first study aims (1) to compare poker gamblers and VLT gamblers with respect to the severity of the gambling problem and psychological characteristics related to gambling behaviours, such as narcissism, sensation-seeking, depressive or anxiety symptoms, and cognitive distortions, (2) to test a predictive model of gambling problem severity and verify its invariance with respect to the type of gambling game, and (3) to test a mediating effect of cognitive distortions between psychological characteristics and gambling problem severity. The results indicate that in comparison to VLT gamblers, poker players present significantly higher levels of narcissism and sensation-seeking, and significantly lower levels of depression and gambling problem severity. Bootstrap analyses reveal that cognitive distortions have a mediating effect on gambling problem severity for both groups of gamblers. In addition, the level of depression in VLT players significantly predicts gambling problem severity both directly and indirectly via the mediating effect of cognitive distortions. Multigroup invariance analyses reveal a non-invariance in the configuration of the predictive model of the gambling problem between the two groups. These profiles could be linked to the player's interest for a specific type of gambling game, but also to problem gambling. The second study aims to assess the reliability of a French adaptation of a self-report measure of cognitive distortions, the Gambling Related Cognitions Scale (GRCS; Raylu, & Oei, 2004, French version: Grall-Bronnec et al., 2012), by comparing the answers of poker players to those of VLT players. Depending on the method used, nearly half of the GRCS items were flagged as showing differential item functioning (DIF) with a medium to large effect. The five items with most significant DIF were more positively endorsed by poker players as compared to VLT gamblers, independent of their total score. These items were related to skill, knowledge, learning and probabilities. This artefact in the instrument consequently increases the total score of poker players, thus suggesting a higher level of cognitive distortions in this group, and has the reverse effect with VLT players. Clinical and theoretical implications derived from these findings are discussed.

Attention-deficit and hyperactivity/impulsivity (ADHD) symptoms and depressive symptoms frequently co-occur in the same child. However, no theoretical model currently accounts for this pattern of co-occurrence. The principal goal of this dissertation is to test whether the dual failure model accurately accounts for the development of the association between ADHD symptoms and depressive symptoms in childhood. The dual failure model postulates that the association between externalizing behaviors and internalizing behaviors is mediated by peer difficulties and academic underachievement. This dissertation first investigates whether ADHD symptoms predict depressive symptoms throughout childhood, consistent with the dual failure model. The contribution of genetic and environmental factors to these associations is also investigated. Then, the mediating role of academic achievement in the association between inattention and depressive symptoms is investigated, as well as the contribution of risk factors relating to the dual failure model. The current study relies on a sample of 650 twin pairs, some of which have been seen between kindergarten and sixth grade. In the first paper, a greater association between depressive symptoms and inattention is seen than between depressive symptoms and hyperactivity/impulsivity. These associations are mainly accounted for by genetic factors in Kindergarten but nonshared environmental factors contribute increasingly to these associations as children get older. In the second paper, academic achievement is found to mediate the association between inattention and depressive symptoms. This mediation model is mostly accounted for by shared genetic factors between inattention and academic underachievement. Disruptive behaviors and relational difficulties do not account for the association between inattention, academic underachievement and depressive symptoms. These results show that inattention is an early risk factor for depressive symptoms and that risk factors relating to the dual failure model do not account for this association. The discussion focuses on the implications of these results for ADHD, the dual failure model and the contribution of genes to human development.

Nous observons, depuis une dizaine d’années, un regain d’intérêt pour les interventions comportementales dans le traitement de la dépression. Des études se penchent sur l’efficacité de l’activation comportementale, une sous-composante de la thérapie cognitivo-comportementale, pour le traitement de la dépression. Dans le domaine de l’activité physique, de plus en plus d’études démontrent l’impact bénéfique de l’activité physique sur la santé mentale. Bien que l’activation comportementale et l’activité physique impliquent toutes les deux une réactivation de l’individu, ces interventions n’ont à ce jour jamais été comparées en terme d’efficacité pour le traitement de la dépression. L’essai clinique randomisé contrôlé décrit dans cette thèse et dans les articles qui la composent évalue l'efficacité et la faisabilité de l’activation comportementale et de l'activité physique sous forme d'auto-traitements guidés pour le traitement de la dépression. Cinquante-neuf participants présentant des symptômes de dépression légers à modérés ont été assignés aléatoirement à une intervention d’auto-traitement guidé d’activation comportementale (n = 20), à une intervention d’auto-traitement guidé d’activité physique (n = 19), ou à un groupe contrôle en liste d’attente (n = 20) pendant huit semaines. Des analyses de variance à modèle mixte et des contrastes déterminées a priori ont été réalisées. L'activation comportementale et l'activité physique se sont avérées significativement supérieures pour réduire les symptômes dépressifs comparativement au groupe contrôle. Des améliorations significatives ont été détectées concernant les variables d’auto-efficacité, de satisfaction de vie, d’affect positif et négatif, et de perception de la solitude. Les implications de ces résultats et des pistes de recherches futures sont discutées. Notamment, l’efficacité de ces interventions minimales offre des options prometteuses pour favoriser la dissémination de traitements supportés empiriquement pour le traitement des symptômes dépressifs.

Mental health issues are common in adolescents and can have a lasting negative impact ontheir overall health. Several risk factors for depressive symptoms in this population are wellknown, such as being a girl or being bullied. School connectedness appears to be a protective factor in adolescents’ depressive symptoms. However, its positive impact among vulnerable groups remains unknown. This master’s thesis has three distinct objectives. The first one isto assess the influence of school connectedness on depressive symptoms among adolescents.The second is to see if its influence varies among vulnerable groups, such as girls, victims of bullying and those who combine these two vulnerabilities. The third is to examine the hypothetical relative impact whether public health interventions aimed at increasing school connectedness, eliminating bullying or eliminating the gender gap could achieve on significant depressive symptoms based on the associations found above. To answer these questions, datafrom COMPASS-Quebec’s 2017-2018 cohort of 17,367 students from 37 high schools in Eastern Quebec were used ( Chapter 1). The results suggest that the students’ reported school connectedness has a protective influence on their depressive symptoms and its influence appears evengreater among young people cumulating vulnerabilities, such as bullied girls. However, evenwith the highest school connectedness scores, this influence may not be sufficient in the mostat-risk students for depressive symptoms.

The broad goal of my research is to better understand how cytokines produced by the immune syqtem induce depressive disorders as reported in patients treated by cytokine immunotherapy. The hypothesis is based on activation of the indoleamine 2,3-dioxygenase (IDO) a tryptophan-catabolism enzyme upregulated by cytokines mainly IFN-gamma. Because tryptophan (Trp) is the precursor of the biosythesis of serotonin (5-HT), a neurotransmiter which plays a role in depression occurence, we suggested that IDO-induced depletion may cause depressive symptoms. In mice, we developed txo models of acute and chronic brain IDO activation based on either administration of lipopolysaccharide (LPS) or inoculation of Bacille Calmette-Guerin (BCG). Our data show that LPS and BCG induce depressive-like symptoms in mice and affect Trp and 5-HT metabolism. Our results reveal that IDO activation in the brain by cytokines induce depressive-like responses such as observed following cytokine immunotherapy

Objectives: First-year students undergo a certain amount of stress, which is due to difficulties in coping with university and a new young adult life. Despite many symptoms of various somatic (tiredness, headaches, backaches. . . ), psychological (depression, suicidal tendencies. . . ), and behavioral disorders (eating habits, addictive behaviors. . . ) among this population, very few studies have been dedicated to students' stress and health. Existing studies in this area tend to be descriptive and cross-sectional rather than explanative and prospective. Moreover, academic failure is high (50%) amongst 1st year students. The model that we adopted for this study is multifactorial, integrating various antecedents being able to predict directly or indirectly (transactions have mediator effects) the various issues considered here, whether somatic, psychological or academic. In order to find out whether this model, inspired by the transactional model of stress -in which appraisal processes and coping play a major part- could explain students' differences in terms of academic performance and physical and mental health, we carried out a longitudinal study on a sample of 556 first-year students. Method: Measures related to socio-biographic, personality and environmental characteristics, as well as transactional processes (perceived stress, control, social support, and satisfaction, coping strategies implemented by students to face the stressful events of the start of a new academic year) were completed at the University Medical Care Centre, during the mandatory medical check. The criteria (depressivity, somatic symptoms, eating disorders and academic performance) were evaluated at T2 (the end of the year). We have built up a specific perceived stress scale and a specific coping scale for this population. We found four underlying factors of perceived stress: academic stress, university dysfunctioning, loneliness and interpersonal relation failures. Results: We have produced some integrative models which reveal the indirect effects of the antecedents on the four issues, carried by transactional variables. Tobacco smoking seems to predict the depression. An anger-hostility and a low self-esteem have indirect effects may be followed by the increase in some dysfunctional transactions (academic perceived stress, loneliness, emotion-oriented coping) and the decrease in functional transactions (problem-oriented coping, family social support). The anxiety-trait predicts the emergence of somatic symptoms, this effect is carried by the academic stress and the emotional coping. A low feeling of adaptation at university T1 and an emotion-oriented coping predict eating disorders. Being younger, better adapted at the T1 stage and more satisfied with incomes predicts a better academic performance. The implied transactional variables are the problem-oriented coping, the festive-addictive coping (negative relation) and the social support from university. The beneficial role of the contacts with the family and the harmful role of contact with friends on success are carried by the adoption of festive-addictive strategies. Conclusion: This study reveals the interest of applying the transactional model to the health and performance of students. The compromise variables are flexible and modifiable which allows reflection concerning the prospects of each student, whether is in terms of prevention or therapy

Depression is a common mental disorder and the leading cause of disability worldwide. It has been associated with poor quality of life, physical decline, and increased risk of premature death. In addition, depression leads to a considerable economic burden, making it a major global public health. Protective and risk factors are diverse and include modifiable lifestyle indicators. Several studies have suggested that diet may play an important role in the risk of depression, but few of these studies have investigated the relationship between « a priori » dietary scores and depression. This thesis thus aims to investigate the relationship between overall diet quality evaluated using various nutritional « a priori » scores and incident depressive symptoms, using the data from SU.VI.MAX and NutriNet-Santé cohorts.Our findings showed an inverse association between adherence to the Mediterranean diet or nutritional recommendations (French and international) and the risk of incident depressive symptoms. However, the study on adherence to the Mediterranean diet showed significant results in men only. Moreover, « pro-inflammatory » diet was positively associated with the risk of incident depressive symptoms, but in the SU.VI.MAX cohort, the associations were significant only in some subgroups of the population: men, smokers and participants with low physical activity. We also showed that an increasing number of « healthy » lifestyle indicators was inversely associated with incident depressive symptoms.All of these findings suggest that promoting « healthy » lifestyles (including « healthy » diet) could be an important strategy in primary prevention of depressive symptoms

Introduction: Major depression, along with suicide, tops the list of health problems affecting the well-being of the Inuit. Improvement in their mental wellness would imply primarily the reduction of gaps in the continuum of services, treatment and support offered. This thesis focuses on the quality of care offered to those individuals in Nunavik who present with a common mental disorder. Three objectives are pursued to verify whether these health care have the potential to provide support to these individuals in their recovery process: 1) assess the quality of clinical processes, using indicators that are both valid and adapted to the context of Nunavik, 2) identify the delays and disruptions in the continuums of care, and 3) identify individual and organizational factors liable to influence the quality of care. Method: This research adopts a descriptive correlational design based on quantitative methods of data collection and analysis. The study population comprises users of primary health services aged 14 years and over, who present with an anxiety disorder or depression. Using clinical records, information on the care offered was colligated for a two-year observation period. Ninety-three individuals from 10 communities in Nunavik made up the sample. Results: A total of 18 clinical process indicators were deemed relevant, measurable and valid indicators to demonstrate the strengths and gaps in the clinical processes. Study of the continuums of care subsequently located the majority of discontinuations around the first follow-up visit (planning and implementation), which indicates that treatment of common mental disorders in Nunavik is built on an acute disease management model. Finally, the principal factors influencing the quality of care were associated with individual clinical factors (type of disorder) and socio-demographic factors (age). Conclusion: This study provides new information pertaining to the clinical processes and continuums of care for mental health in Nunavik. This information has the potential to allow policy makers to base their decisions concerning the organization of mental health primary care services on contextualized information and in so doing, to implement effective and adapted solutions to improve care for the long term.
