ENGLISH ABSTRACT: South Africa has transformed its mental health service provision from in-hospital care to community-based rehabilitation. Although the idea is sound, the process places the caregiving families under an immense pressure. The aim of this study was to explore the impact that the de-institutionalisation process has had on the families as they care for their child with intellectual disability. The study was conducted by means of qualitative, unstructured interviews with families that have had a child de-institutionalised from Alexandra Hospital in the Western Cape. All of the patients were diagnosed with a dual diagnosis of intellectual disability and mental illness. Even though the patients were in group-homes or attended a day care centre, final responsibility for the patients lay with the parents. Three main themes emerged from the interviews that describe the impact of deinstitutionalisation, viz. the characteristics of the family member with intellectual disability (aggressive, abusive and self-destructive behaviour of the patient), the effect that these characteristics had on the family (marital stress and health risks to the care giver), and community and resource factors. The study placed the family central to its environment and discussed the impact deinstitutionalisation had on its environment as a whole. It was concluded that the burden that de-institutionalisation places on the families far exceeded their ability to cope with these circumstances. This status quo could be improved if adequate resources and skills are given to families prior to de-institutional

ENGLISH ABSTRACT: The implementation of eugenic policies reached its peak during the zo" century when thousands of people with intellectual disabilities and other "undesirable qualities" were involuntary sterilized. Although most of the eugenic policies have been removed, countries such as South Africa, still make legally provision for the involuntary sterilization of people with intellectual disabilities. Torbjërn Tannsjë (1998) used the "argument from autonomy" to argue that involuntary sterilization practices are wrong because it involves compulsion. According to him, society should never interfere with people's reproductive choices and people should never be required to qualify for the right to have children. The aim of this assignment was to systematically assess the "argument from autonomy" as far as the policy of involuntary sterilization of people with intellectual disabilities is concerned. To this end, the concept of autonomy and the principle of respect for autonomy are discussed and applied to the intellectually disabled. It is argued that autonomy and respect for autonomy are useful concepts to apply to some people with intellectual disabilities. These individuals should not be automatically assumed to be incompetent, but their competence needs to be determined on an individual level, with reference to the complexity of the decision to be made. Special effort is needed from health care professionals to obtain (where possible) informed consent from people with intellectual disabilities. The application of the principle of respect for autonomy to matters of reproduction leads to the conclusion that people with severe to profound levels of disability, are unable to provide informed consent for sexual intercourse. Therefore some form of paternalistic protection is needed for these individuals. People with mild to moderate intellectual disabilities who are however competent to consent to sexual intercourse should never be prohibited from procreation by means of involuntary sterilization. State interference in matters of reproduction should be limited to interventions where (i) children are seriously harmed by parents and (ii) to protect those who are incompetent to consent to sexual interactions with others. Apart from these exceptions, the intellectually disabled is entitled to the same procreative rights as all other citizens.

ENGLISH ABSTRACT: South African Protective Workshops struggle to transform to align to a developmental human rights approach and to increase sustainability in the absence of a uniform model and an implementation strategy. This study applied qualitative research methodology in the form of Action Research with Co-operative Inquiry to answer the research question of what could be a framework of best practice for and strategy of transformation for protective workshops (PWs) operated by the South African Federation for Mental Health (SAFMH) in South Africa. It included the development of a transformation strategy for 31 PWs for adults with intellectual disability operated by SAFMH member organisations across South Africa. Seven representatives from the managers and service users of PWs were selected trough convenient sampling. The participants formed a co-operative inquiry group to determine the nature of the service to be delivered and how it should be implemented. The purpose was to develop a framework for possible and relevant services for persons with intellectual disability. Such services should preferably be aligned to the human rights based legislation and funding requirements of the Department of Social Development as to increase the long-term sustainability of the PWs. Given that the study was funded by the SAFMH, the directors requested that the Co-operative Inquiry Group use the exisisting best practice model used by Cape Mental Health as a template and point of departure. The study was done over a 12-month period (February 2012 – February 2013) and included a research initiation meeting, four search conferences and a presentation of the findings to the SAFMH Directors. The study contributed new knowledge on the nature of service provision to persons with ID and the ideal process to transform services within the SAFMH context. Through inductive content analysis two themes emerged namely: 1) Comprehensive service provision and 2) Coordinated transformation of services. The first theme determined that comprehensive services to persons with ID should consist of inclusive, appropriate, enabling and empowering services. The second theme suggested a new way of coordinating the transformation of services through systematic implementation, suitable regulation, sufficient capacity and ensuring sustainability. A central management structure was suggested to ensure coordinated implementation, to secure funding and to monitor and evaluate the implementation. A mind-shift towards new thinking was identified as a prerequisite for stakeholders buy-in on transformed service delivery. This mind-shift relates to the status of the service users with ID as adults in training towards employment in the open labour market and maximum integration into society. This study contributed new knowledge that informs the development of a new service delivery framework of best practice. The proposed implementation strategy could offer persons with ID the opportunity to progress and develop towards their maximal level of integration into society. It further provided PWs with possibilities for conceptualising different models of practice in the form of an implementable framework and a strategy to transform services. The findings were presented to the SAFMH Directors who adopted the concept framework and implementaimplementation strategy in theory as a proposal for future transformation without amendments.

South Africa has a growing rate of mental health care users. Because of the lack of health resources and personal beliefs many of these mental health care users consult with their church leaders. The treatment of mental illness by clergy in South Africa is largely undocumented. The aim of the study was to explore and describe the treatment of mental illness by Afrikaans speaking church leaders in Polokwane, Limpopo Province. To gain a deeper understanding of the views held by the Afrikaans-speaking church leaders, a qualitative approach was utilized. Ten participants agreed to participate in the study. The results tend to suggest that most of the respondents use a combination of supportive therapy and teachings from scripture to treat some mental illnesses, and that they feel positive towards collaboration with other mental health care professionals. Lastly the results indicated that most respondents emphasized the importance of homophily in referral criteria. The current study explored the formative processes of twelve student teachers constructing role understandings in the context of their experiences and interactions with people with disabilities. In particular, it examined the participants’ changing notions of self-as-teacher and their unfolding perceptions of an inclusive educator’s role in teaching children with disabilities. The research aimed to investigate personal and professional forms of knowledge linked with the prior subjective life experiences of the student teachers and those arising from their interactions in situated learning experiences in community settings. The contextual framework of the study focused on the development of the student teachers’ unique understandings and awareness of people with disabilities through processes of biographical situated learning. The investigation examined participants’ voluntary out-ofcourse experiences with people with disabilities across three community settings for the ways in which these experiences facilitated the participants’ emerging role understandings. These settings included respite experiences in families’ homes of young children with disabilities receiving early intervention, an after-school recreational program for primary and secondary aged children and adolescents with disabilities, and an independent living centre providing post-school options and activities for adults with disabilities. ii Two groups participated in the current study, each consisted of six student teachers in the Bachelor of Education Course at the Bathurst campus of Charles Sturt University. Group One participants were in the second year compulsory inclusive education subject and Group Two participants were in the third year elective early intervention subject. The investigation examines the nature of reflexive and reflective processes of the student teachers from subjective, conflict realities in an attempt to link community experiences with real-life issues affecting inclusive educational practices. The voluntary community experiences engaged the research participants in multi-faceted interactions with people with disabilities, providing thought-provoking contexts for their reflections on observations, responses and reactions to situations, such as critical incidents. The participants engaged in reflexive and reflective processes in records made in learning journals and in semi-structured interviews conducted throughout the investigation. Results were analysed from a constructivist research paradigm to investigate their emerging role understandings. Prior to this study there had been few practical components in the compulsory undergraduate inclusive education subject which meant that previously student teachers gained theoretical knowledge without the opportunity to apply their learning. Many student teachers had expressed their feelings of anxiety and uneasiness about what they should do and say to a person with a disability. Thus, the community experiences were selected in order to give a specific context for student teachers’ learning and to provide participants with expanded opportunities to consider their professional identity, social awareness and acceptance of people with disabilities. iii An analysis of the data demonstrated the centrality of reflection within a situated teaching and learning framework. Understandings of prior experiences and motivation were shown to interact with the outcomes of the community experiences through an on-going process of reflection and reflexivity. This reconstructing process encouraged learners to reflect on past, present and projected future experiences and reframe actions from multiple perspectives as a way of exploring alternatives within broader contexts. The data reveal the participants’ engagement in the community experiences facilitated their awareness of wider socio-cultural educational issues, while focusing their attention on more appropriate inclusive teaching and learning strategies. The reflective inquiry process of identifying diverse issues led participants to consider other possible alternatives to current community practices for better ways to support their changing perspectives on ideal inclusive classroom practices. The dialogic nature of participants’ on-going deliberations contributed to the construction of their deeper understandings of an inclusive educator’s role. The findings of the study identified external environmental and internal personal factors as contributing biographical influences which shaped the student teachers’ emerging role understandings. The results emphasised the value of contextual influences in promoting desirable personal and professional qualities in student teachers. Importantly, situated learning enhanced participants’ unique interpretations of their prospective roles. As a result of analysing their insights from interactions in community contexts, the student teachers had increased their personal and professional understandings of individuals with disabilities and broadened their perceptions of their roles as inclusive educators. Thus, the study found that encouraging a biographical reflexive and reflective orientation in participants was conducive iv to facilitating changes in their understandings. Overall, the outcomes had benefits for student teachers and teacher educators in finding innovative ways for integrating biographical perspectives into situated teaching and learning approaches. The study showed that contextual influences facilitated deeper understanding of role identity and produced new ideas about the nature of reflexivity and reflection in guiding student teachers’ learning. ENGLISH ABSTRACT: This study attempts to investigate the sources of stress and burnout in the social work profession and specifically in mental health settings. In order to achieve this, a literature study is undertaken which identified the sources of stress and burnout as external and internal stresses. External stresses can be identified as the administrative function of supervision, the educational function of supervision, and the relationship between the supervisor, the social worker and the client, aspects in the social work profession and the organisation. The personality and gender of the social worker are discussed as internal sources of stress. Social workers' experiences of stress and burnout are explored against the background of the socio-economic and political changes after the first democratic election in 1994 in South Africa. The literature review also indicated that there are peculiar stresses in mental health settings due to the clients' mental conditions. An exploratory study was undertaken in which 17 social workers in the Peninsula, working in mental health settings, participated. Information on the research topic was obtained by means of anonymous questionnaires. A group interview was arranged to ensure that all the questionnaires were received. It was found that since the 1994 democratic election, social workers worked under more stressful circumstances due to limited resources, restructuring, decreased manpower, a higher caseload, deinstitutionalisation of mental patients and an increased amount of uncertainty with regard to job security. Aspects in the social work profession that caused the most stress were identified as insufficient salaries and limited opportunities for promotion. The participants revealed uncertainty with regard to their future in the social work profession. Social workers acknowledge the importance of the supportive function of supervision but are also of the opinion that individuals should take responsibility to address burnout. Strategies that can be used by the social worker, the organisation and the supervisor to address burnout, are discussed. On the basis of the results and conclusion of the study, recommendations are made to supervisors, as well as further research.

People with intellectual disabilities are considered to be marginalised in our society. They continue to face prejudices, stigmas and are discriminated against resulting in social exclusion. This thesis develops a framework and methodology for pursuing inclusive environments and viable productivity within the workplace for persons with intellectual disabilities. The lack of opportunities within the workplace for individuals with intellectual disabilities intensi es their exclusion from society. The road to achieving inclusivity within the workplace has become a challenging one to navigate, as previous strategies which were developed to enable equality are far removed from what was intended. More often than ever before, employers are challenged from both a legal and political perspective for failing to diversify the workplace. Equality advocates as well as those who attempt to diversify their companies face obstacles within the workplace, namely inadequate resources or uncertainty about their appropriate role or approach in dealing with minorities. The building will serve as a learning hub, used by both persons with intellectual disabilities and people who are commonly referred to as able bodied. It will function as a level between education (if any) and the period before entering the workplace. The learning hub will afford persons with intellectual disabilities the opportunity to explore their capabilities in environments similar to that of a workplace and gain knowledge and experience prior to entering the workplace. The program envisaged in this thesis will demonstrate that this intervention is mutually bene cial to a company and the individual with an intellectual disability. In the first instance, the employer acquires knowledge and skills to act as a catalyst in enabling the transition of a person with an intellectual disability into the workplace to augment inclusivity. Secondly, the person with an intellectual disability through the program will be clothed with knowledge and expertise enabling meaningful participation amongst able-bodied employees. The transformation within workplaces as a result of the knowledge gained through this centre should create an enabling environment that incorporates the individual’s needs or one that best satis es their needs. The support by the employer is vital in the outcome and success of integrating people with disabilities into the social framework. The building is developed around a range of interactive programs between those with intellectual disabilities and people acting as the supervisors from workplaces. The supervisors, through training will be best placed to oversee progression and be empathetic toward staff with intellectual disabilities. This will enable them to transfer their knowledge and expertise of dealing with and accommodating employees with intellectual disabilities to their respective workplaces. A mock work environment will form the basis of the experiential training ground and in conjunction with job sampling will establish where a user will be best suited within the work environment. Public facilities encourage members of the public to become active participants within the space providing exposure and understanding towards those with intellectual disabilities. The centre is expected to be both accessible and lasting and will mediate experiential training and aligning it to workplace practices. The centre will be a vital space to enhance inclusion and for producing sustainable change for this marginalised group of persons. The building is located within an educational precinct on Melle Street, Braamfontein. The site which is in close proximity to the Central Business District allows for the centre to feed into businesses and vice versa. The location also allows for easy access to people from all over Johannesburg utilising various transport methods. The site being amid universities and schools ensures integration and optimum use, for educational facilities to link into this program. It also allows for social cohesion through exposure between the users of the building and other students in the surrounding area.

The aim of the study was to investigate the challenges faced by families living with mentally ill relatives in Thulamela municipality in the Vhembe district, Limpopo province. The objectives of the study were to investigate the impact that caring for a mentally ill person had on the family members; to explore the strategies adopted by families to enable them to cope better with the challenges they faced when caring for their mentally ill relatives; and to understand the support services that were provided to families by health care professionals to enable them to cope with such challenges. A Nonprobability sampling, in particular its subtype convenience samplings was used to select five families who participated in the study. A semi-structured interview schedule was used to collect data from the main care givers and a focus group discussion took place to gather more in-depth information from at least three members of each family. Those selected to participate in the study were all family members who were involved in the care giving responsibility. All participants were caring for mentally ill relatives diagnosed with schizophrenia, substance induced psychosis or bipolar affective. The care recipient had to be admitted at Hayani psychiatry hospital. Data extracted from the participants was analysed thematically following the six stages of thematic data analysis. The research findings obtained from family caregivers from Thulamela municipality found in the Vhembe district within the Limpopo province, highlighted that taking care of a family members with mental illness comes with a number of challenges. The impact was experienced by all members of the family irrespective of the age, social or financial status. The impact was experienced emotionally, physically, socially and financially. All participants also reported to be experiencing burnout. They prefer their loved ones to remain living in mental institution as it gives them a chance to live normal lives.

Mashigo, Refilwe Junior. "Educational programs for the mildly mentally disabled children in the black communities of the Witwatersrand." Thesis, 2014. http://hdl.handle.net/10210/12748.
Lack of appropriate facilities for the mildly mentally disabled children in the black communities of the Witwatersrand was a source of concern for parents. These parents did not want their children to attend the "training centres for the mentally handicapped children" as they felt their children functioned above the criteria used at these centres. Some parents, as a result, choose to keep their children at home. This study was undertaken to research the hypothesis that mildly mentally disabled children could benefit from educational facilities and programs. Respondents were parents of the affected children and teachers from the training centres in Soweto, East Rand and Alexandra. An exploratory study was conducted. Forty teachers administered the questionnaires and trained interviewers assisted 93 parents to complete their questionnaires. Problems were encountered due to wrong addresses (many houses were then unnumbered in Soweto) and reluctance from some respondents to participate. The results confirmed the hypothesis as both teachers and parents felt that alternative facilities to the training centres were needed for the mildly mentally disabled children, that teachers should be qualified in that special field of teaching, and more contact between teachers and parents was recommended, A multi-disciplinary approach in the treatment program was also an important factor, unfortunately none of the centres had more than three professionals. Social workers were used by other professionals to provide services from the birth of the disabled child as support systems, catalysts and advisers to parents. The social worker's role is to educate, empower, support, mediate, liaise with the external resources and nurture the person to independence and self-reliance.

Mental health is known to be adversely affected by gender inequality and poverty. The World Health Organization (WHO) reported that psychological disorders affect half the global population (WHO, 2010) and South African estimates posited that 16.5% of the population presented with common mental disorders in 2007 (Williams et al., 2008). Depression and anxiety disorders in women, specifically, are posing a major public health concern in developing countries due to inadequate treatment (Aidoo & Harpham, 2001). Protective factors, namely intrapersonal, interpersonal, community and cultural factors, were identified by Ungar (2008) as a mechanism that promotes resilience and alleviates the effects of adversity. In order to develop knowledge geared toward intervention strategies to promote mental health in socio-economically disadvantaged women in urban communities, this study employed an explanatory mixed methods research design (which included both quantitative surveys and qualitative interviews) to establish an incidence of depressive symptoms, and levels of ego-resilience, among women in Doornkop, Soweto. This was done in order to explore, first, the relationships between selected socio-demographic variables and depression and ego-resilience, and second, to investigate if there was a relationship between depression and ego-resilience. Finally, the study aimed to gain an understanding of how women perceived the role of protective resources in their lives which might promote positive mental health outcomes. The statistical component of the study found a weak negative correlation between the two constructs of depression and ego-resilience. Symptoms of depression appeared to be related by exposure to a high number of difficult life experiences, and particularly in instances of having been a victim of crime or violence. While ego-resilience seemed to be related to increased participation in community groups, it also correlated with adversity, namely, unemployment. This led the researcher to the conclusion that for this sample, depression and ego-resilience were independent constructs, and although they were divergent concepts, both were shaped by exposure to adversity. The interview data further explored exogenous sources of resilience as set out by Ungar (2008). These findings highlighted the importance of interpersonal relations on feelings of well-being. While the protective factors varied in the sources from which they arose (from children, romantic partners, parents, peers and community groups to music and television, and faith in God), they all provided a sense of purpose, belonging and self -worth, which enabled positive feelings. The main recommendation that arose from the research was the need for community education about mental illness, the services available to communities, and the importance of developing and maintaining family and community systems of support. These measures may go some way toward enhancing resilience in women and reducing vulnerability to mental disorders arising from experiences of adversity. Furthermore, a focus on community-based interventions, such as education and life skills, is vital in shifting the focus of interventions from mental illness to the promotion of mental health. Ongoing research is fundamental to developing our existing knowledge of both psychopathology and the promotion of mental well-being in women in socio-economically deprived communities. Research, in light of the challenges facing community mental health services in poor areas, could include foci on the beliefs of professionals including the knowledge and skills required to deliver effective mental health services. Knowledge of this nature should serve to enhance our understanding of the complexities of mental health and inform the development of innovative and appropriate treatment modalities that are oriented to the particular needs of women in resource-poor communities.

Moetseloa, Mpolokeng Cecilia. "The challenges experienced by the people living with HIV on the termination of temporary disability grant in a semi-urban area in Gauteng." Diss., 2018. http://hdl.handle.net/10500/25102.
The South African government provides people living with HIV Temporary Disability Grants to assist them with money when they cannot work due to being disabled by HIV. The toll of the disease has contributed to the inability to be employed among black South Africans. The aim of this exploratory qualitative study was to investigate the challenges experienced by people living with HIV when their Temporary Disability Grants are terminated in the semi-urban area of Gauteng. The temporary disability grant is terminated after six months of receiving it. In-depth interviews were conducted with people who live with HIV who are members of Ekupholeni Mental Health and Trauma Centre support group. Thematic analysis was used to analyse data. The findings of the study revealed that the termination of Temporary Disability Grant affects the running of the households of people living with HIV, causes poor management of the disease, non-adherence to treatment which leads to viral rebound, poor nutrition as a result of poor finances. The findings of this study are significant for the policy review on how long a person should receive the disability grant and the criteria used to apply for a disability grant, and to encourage the people living with HIV to start their own gardening programmes for food supply. Moreover, to encourage the introduction of a Chronic Disease Grant (CDG) for people who live with HIV.
