The Trump administration sought to erode confidence in the international system, which it regarded as an affront to national sovereignty. The COVID-19 pandemic, by raising collective awareness of the need for global cooperation, may help to restore that confidence and encourage renewed commitment to multilateralism. Public opinion polls show that people around the world have greater confidence in global institutions like the World Health Organization than in national leaders like Donald Trump or Xi Jinping. The United States will continue to be an indispensable partner after Trump, but the international order will rest on a stronger footing if Washington is no longer expected to be the prime mover and contributes instead as one among equals.

Cuba was a charter signatory of the GATT, transitioning to WTO membership in 1995. After its 1959 revolution, it sought to achieve economic independence from the US, and traded mainly outside GATT rules, in barter arrangements with the Soviet Union and later with Venezuela. This article investigates why Cuba chose to remain in the GATT and WTO as a centrally planned economy. GATT and WTO documents reveal that Cuba sought to gain influence among developing countries as a critic of existing trade rules and negotiations, while attempting to retain or seek new rules-based multilateral channels of market access. Yet Cuba has not been able to avoid dependence on major trading partners and a limited range of exports, as the collapse of the Soviet system, and the political deterioration in Venezuela led to major declines in Cuban exports and economic crisis. However, Cuba has begun to diversify its exports and expand its trade partnerships, and its trade interests now align more closely with WTO rules and negotiating agendas. Cuba will, however, need to implement domestic reforms and increased openness to global markets in order to realize the economic benefits of WTO membership.

The years immediately after the Second World War witnessed over-full employment in the Australian economy. The workforce, backed by public opinion, sought to reap gains in employment conditions after years of restraint. High on the unions' official agenda was reform of the federal arbitration system, which dominated the wage-fixing process. In 1947 the ALP government amended the Conciliation and Arbitration Act. Although endowed with unprecedented constitutional and political authority, the Chifley government failed to meet the main expectations of its union constituents. The system remained centralized and dominated by a small bench of judges. Chifley's appointments to the bench failed to depart from the conservative norm. The Labor government's overwhelming motivation was to hold down wage costs and cushion the economy from the shock of industrial labour's newfound bargaining power.

Combination of logic-based artificial intelligence with virtual reality in intelligent agent systems is an approach not extensively sought after to date. It is our belief that significant gain is to be expected if the technical challenges involved are overcome. In this paper, we describe the mVlTAL intelligent agent system, which is our latest effort towards this direction. The system is a contemporary intelligent agent system with applications in numerous areas, including intelligent virtual environments and formal artificial intelligence research. The system focuses largely on logic-based approaches, which are present in almost every aspect of it, including modeling, knowledge representation, definition of agent behaviors and inter-agent communication. In addition, virtual manifestation of the world and agents is also an inherent characteristic of the system. The system, even if still in a development and evaluation stage, has already been employed in experimental and educational applications, demonstrating the potential benefits of such an approach.

AbstractHistorical accounts of colonial science and medicine have failed to engage with the Colonial Office’s shift in focus towards the support of research after 1940. A large new fund was created in 1940 to expand activities in the colonies described as fundamental research. With this new funding came a qualitative shift in the type of personnel and activity sought for colonial development and, as a result, a diverse group of medical and technical officers existed in Britain’s colonies by the 1950s. The fact that such variety existed amongst British officers in terms of their qualifications, institutional locations and also their relationships with colonial and metropolitan governments makes the use of the term ‘expert’ in much existing historical scholarship on scientific and medical aspects of empire problematic. This article will consider how the Colonial Office achieved this expansion of research activities and personnel after 1940. Specifically, it will focus on the reasons officials sought to engage individuals drawn from the British research councils to administer this work and the consequences of their involvement for the new apparatus established for colonial research after 1940. An understanding of the implications of the application of the research council system to the Colonial Empire requires engagement with the ideology promoted by the Agricultural Research Council (ARC) and Medical Research Council (MRC) which placed emphasis on the distinct and higher status of fundamental research and which privileged freedom for researchers.

While children both understand the concept of, and have died by, suicide, little research has been conducted on children's experiences of healthcare systems during and after a suicidal crisis. This article focuses on three case studies of mothers with suicidal daughters and aims to describe the health service experiences of parents whose children have attempted suicide. The case studies were selected as exemplars of three different healthcare experiences of mothers with suicidal daughters younger than 16 years of age. Interviews were conducted with the mothers, focusing on their experiences when trying to find care for their daughters after a suicide attempt. A ‘dirty text’ analysis was undertaken on the transcripts, which aimed to find potential redemption within stories of trauma. Narratives were analysed to see how their stories were told, but also how experiences could be shared or be dissimilar. Significant gaps currently exist in the care and support provided to suicidal children, particularly in the critical post-discharge phase. Adults were not always able to recognise when a child was suicidal, or sometimes take that suicidality seriously. Support must often be proactively sought, and even organisations that are meant to target children and adolescents may not always provide appropriate care.

Natriuresis in direct response to a gastric sodium stimulus (upper-gut sodium monitor) has paradoxically only been demonstrated in humans and animals on a low-sodium diet preceding each study. It is possible that the low-sodium diet itself induces or suppresses systems that mediate or oppose the ensuing natriuresis. In this study, we sought to determine whether a system activated by this diet, the renin-angiotensin system, mediates the natriuretic response. Specifically, we sought to show whether changes in the circulating concentration of angiotensin II (ANG II) may mediate the renal response to stimulation of the gastric sodium monitor. Male New Zealand White rabbits were randomly assigned to low- (0.008%) or normal (2.2%) sodium diets. After 1 wk on the experimental diet, they received a sodium load intragastrically or intravenously, and plasma ANG II was measured at 0, 5, 10, 30, 60, and 120 min. Urine was collected for 4 h after the sodium load, and plasma sodium was measured at 0, 2, and 4 h. Urinary sodium excretion was greater in the 4 h after gastric than after intravenous sodium administration (P < 0.025) in the rabbits on the low-sodium diet. No significant difference was noted in the rabbits on the normal sodium. In rabbits on the low-sodium diet, there was an immediate and significant decline in plasma ANG II after sodium was administered both intragastrically (P < 0.025) and intravenously (P < 0.05). This decrease was greater after intragastric than intravenous sodium (P < 0.0025), and the difference was still evident at 120 min (P < 0.05). No significant difference in plasma ANG II was found in the normal diet group. We conclude, therefore, that a prolonged decrease in ANG II concentration may play a role in mediating the natriuretic response to the gastric sodium monitor.

3D cultures have gained attention in the field of regenerative medicine for their usefulness asin vitromodel of solid tissues. Bottom-up technology to generate artificial tissues or organs is prospective and an attractive approach that will expand as the field of regenerative medicine becomes more translational. We have characterized c-kit positive cardiac stem cells after long-term cultures and established a 3D-nanoculture system using collagen scaffolds. By combining informatics-based studies, including proteomic analyses and microarrays, we sought to generate methods that modeled cardiac regeneration which can ultimately be used to build artificial hearts. Here, we describe the use of biodegradable beads or 3D cultures to study cardiac regeneration. We summarize recent work that demonstrates that, by using a combination of molecular analyses with 3D cultures, it is possible to evaluate concise mechanisms of solid tissue stem cell biology.

A braced excavation, 23 m deep x 213 m long, for Civic Center Station on the Los Angeles Metropolitan Transit Redline was nearing completion when high strut loads were measured at mid-depth. About four months after the excavation bottomed out and the base slab was cast, four strut-to-wale connections failed. The contractor was directed to install a sister level of braces and later sought compensation from the owner. An investigation into the contractor's claim used the finite element method and determined that the high loads could not have been anticipated from conventional theory, and that they were due both to mixing tiebacks and struts in the same excavation and to preloading the struts. The method and results of the investigation are summarized.

In Imperial Russia, incorporation required an expensive special concession, yet over 4,000 Russian firms incorporated before 1914. I identify the characteristics of incorporating firms and measure the productivity gains and growth in machine power enjoyed by corporations using newly-constructed factory-level panel data compiled from Russian factory censuses. Factories owned by corporations were larger, more productive, and more mechanized than unincorporated factories. Higher productivity factories were more likely to incorporate and, after incorporating, added machine power and became even more labor productive. Russian firms sought the corporate form’s full set of advantages, not just stock markets access, to obtain scarce long-term financing. (JEL D24, G31, G32, L60, N43, N63)

This case presents an overview of the logistics industry, background on the business need to consider new systems, and the requirements of the company in its system selection. Details of the technologies considered are included. The reader is then faced with the challenge of analyzing the options, and making a recommendation for systems selection. After analyzing this case study, the reader should be able to: • Define logistics functions, supply chain management, and third party logistics (3PL) services • Describe LMS and RFID systems • Identify the expected costs and benefits of the proposed technologies • Develop a multi-factor evaluation for vendor selection • Make a recommendation based on the evaluation, financial data, and other considerations.

Experts have long sought systems to spur improvements in teaching and learning. PDK’s Joshua P. Starr reflects on how, in the 1990s, his Plainfield, New Jersey, district implemented walk-throughs to determine how well teachers and schools were implementing the district’s literacy program. Classroom visitors had a list of items to look for, and they made recommendations to school leaders based on what they saw. Although such a system has merit, Starr now questions whether it placed the expertise in the wrong place. Central offices have evolved to focus more on compliance and operations than on instructional leadership. As districts make decisions about how to get students back on track after the COVID-19 pandemic, they might be best served to look to teachers for understanding about how best to serve students.

The research objectives were to: 1) find out and analyze the productivity of cash crops in agroforestry and monoculture systems; 2) determine the selection of cash crops in Amborawang Laut Village. The study was conducted from September to November 2015. Determination of respondents using census methods. The number of respondents was 20 farmers, consisted of 10 farmers for each system (agroforestry and monoculture). The data collected was primary and secondary data. Data collection methods were carried out through interview, observation, and documentation. Data were analyzed using an independent t-test. The results showed that the productivity of cash crops in agroforestry system for three years (2013-2015) for string beans, beans, bitter melon, squash, cayenne pepper, eggplant pondoh, and cucumber respectively was 13,64; 11,86; 11,17; 14,00; 18,05; 11,75; dan 11,50 Mg ha-1, while the monoculture system was 7,01; 10,50; 10,50; 12,63; 10,45; 12,33; dan 10,58 Mg ha-1. The selection of cash crops by farmers in agroforestry and monoculture systems was based on market demand, namely types of plant that are easily marketed and sought after by consumers.

AbstractObjectivePolicy-makers and practitioners have a need to assess community resilience in disasters. Prior efforts conflated resilience with community functioning, combined resistance and recovery (the components of resilience), and relied on a static model for what is inherently a dynamic process. We sought to develop linked conceptual and computational models of community functioning and resilience after a disaster.MethodsWe developed a system dynamics computational model that predicts community functioning after a disaster. The computational model outputted the time course of community functioning before, during, and after a disaster, which was used to calculate resistance, recovery, and resilience for all US counties.ResultsThe conceptual model explicitly separated resilience from community functioning and identified all key components for each, which were translated into a system dynamics computational model with connections and feedbacks. The components were represented by publicly available measures at the county level. Baseline community functioning, resistance, recovery, and resilience evidenced a range of values and geographic clustering, consistent with hypotheses based on the disaster literature.ConclusionsThe work is transparent, motivates ongoing refinements, and identifies areas for improved measurements. After validation, such a model can be used to identify effective investments to enhance community resilience. (Disaster Med Public Health Preparedness. 2018;12:127–137)

AbstractOffering large muscle benefits despite low metabolic demand, continuous eccentric exercise appears to be an interesting alternative to concentric exercise. Nevertheless, further knowledge is needed about prolonged eccentric exercise. This work sought to investigate the cardiovascular responses to prolonged constant-load eccentric compared to concentric cycling. Ten healthy males performed two 45-min exercise sessions of either concentric or eccentric cycling separated by a month and matched for heart rate during the first 5 min of exercise. Cardiorespiratory, autonomic nervous system and vascular responses were assessed at rest, and during exercise and recovery. During cycling, oxygen uptake, cardiac output and systolic blood pressure were similar but heart rate and diastolic blood pressure were greater whereas stroke volume was lower during eccentric than concentric cycling (118±21 vs. 104±10 bpm; 77±9 vs. 65±8 mmHg; 122±12 vs. 135±13 mL). Baroreflex and noradrenaline concentration were altered during eccentric cycling, and after eccentric exercise, vascular tone was greater than after concentric cycling. We observed increased cardiovascular strain and altered baroreflex activity during eccentric compared with concentric exercise, suggesting eccentric cycling triggers greater sympathetic activity.

This research aimed to recognize the extent of reflecting the application of Islamic banking system on the financial performance of banks. It's also helps to find the feasibility sought by the conventional banks by directing to converting to the Islamic system through evaluating the financial performance of the conventional banks before and after its shift; by using the financial indicators and the personal interviews that answered the main question of the research which is; Is there a difference in the bank's performance before and after the shift from the conventional system to Islamic system? The most important research results were that the pre-preparation of the bank shift plan and the creation of its employees is one of the most important factors in the success of the process. Also the possibility of banks converting in the state of Kuwait is available and enacted with laws that regulate working therein. The financial ratios are also main indicators to identify the performance of the bank. The professional vision of the bank converting experience has also suggested the right decision to turn to the Islamic regime to achieve the desired end of all the technical aspects, including banking, financial and legal ones.

Caste system has remained an integral part of the Hindu social order. It has served to provide the uniqueness and the complexity to the latter. During colonial period as well as in the postcolonial period, it has been the most sought after issue. The colonial encounter added a new narrative to this system, as has been argued by the scholars such as Nicholas Dirks. However, apart from colonial officials, the non-official leaders of the ruling country also played an important role in the making of the present-day construct of the caste system. Annie Besant was one such leader, who came to India in 1893 as a leader of the Theosophical Society. She endorsed and extolled the caste system of the land as the best social structure this world has ever had. In this article, I attempt to throw light on Besant’s reading of the caste system and her role in strengthening the roots of this system.

Precision in orthodontic diagnosis can increase the chance of therapeutic success. The objective of this study was to describe the development of a computerized system (prototype), created from a printed table of the Cranial Facial Analysis and Total Dentition Space Analysis with Difficulty Index - Tweed-Merrifield Analysis - in order to aid orthodontic diagnosis. The analysis was transposed from the manual format to the digital format. A user-logical and clear interface was sought for the development of the prototype, consisting of tables and graphs, including automatic, fast and accurate calculations. The result was the immediate visualization of the resolution of the analysis after filling out the fields on the computer. This technological innovation can be a helpful instrument for the orthodontist that favors a more accurate dental-cranial-facial analysis, increases patient safety, orients conduct and may contribute to teaching and research.

Abstract Nature swimming pools are a sought-after alternative to conventional swimming pools. Their natural water purification system, without the need for chemicals, is beneficial to health and environmentally friendly. Designing such a nature swimming pool is a complex matter because it requires coordination of a number of effects, which can easily cause an imbalance in the entire system. This work uses the case study method to analyse the overall condition of three nature swimming pools in the South Moravian Region during the second half of the swimming pool operation season.

Arteriovenous malformations (AVMs) are unnatural connections in the vascular system. The treatment strategies are dictated by the angioarchitecture, location and presentation of AVMs. The present study sought to report a case report of a 35-year-old female patient diagnosed with a brain AVM and submitted to a transvenous AVM embolization, located at the corpus callosum's splenium, after a previous transarterial embolization failure. As a therapeutic approach, the patient was submitted to transarterial embolization. However, the procedure was performed too proximally in the arteries, which resulted in the total AVM persistency. The patient was submitted to angiographic control until complete AVM occlusion.

This study addresses the origins of multiwhisker-receptive fields of neurons in the thalamic ventral posterior medial (VPM) nucleus of the rat. We sought to determine whether multiwhisker-receptive field synthesis occurs in VPM through convergent projections from the principalis (PrV) and interpolaris (SpVi) nuclei, or in PrV by intersubnuclear projections from the spinal trigeminal complex. We tested these hypotheses by recording whisker-evoked responses in PrV and VPM before and after electrolytic lesion of the SpVi in lightly anesthetized rats. Before the lesion PrV cells responded, on average, to 3.2 ± 1.2 whiskers but responsiveness was reduced to 1.07 ± 0.31 whisker after the lesion. A similar reduction of receptive field size was observed in VPM, where neurons responded, on average, to 2.94 ± 0.95 whiskers before the lesion and to 1.05 ± 0.22 whisker after the lesion. Thus one can conclude that intersubnuclear projections mediate surround whisker-receptive fields in PrV, and therefore in VPM. However, it has previously been shown that parasagittal brain stem transection, which severed ascending projections from SpVi, but left intersubnuclear connections intact, rendered VPM cells monowhisker responsive. We wondered whether midline brain stem lesion modified receptive field properties in SpVi. In normal rats SpVi cells responded, on average, to 7.52 ± 4.25 whiskers, but responsiveness was dramatically reduced to 1.47 ± 1.07 whisker after the lesion. Together these results indicate that the synthesis of surround receptive fields in subcortical stations relies almost exclusively on intersubnuclear projections from the spinal trigeminal complex to the PrV.

This article describes how in this digital age, storage devices have been permitting the design of systems with the capacity to handle massive amount of diverse types of data. Real-time storage devices have been using complex architectures to accommodate the exponential increase of generated content. Hard drives have been at the core of most of storage systems. Estimating the reliability and the time to failure of such devices will be of great value. Weibull distribution has been widely used to assess the reliability of many systems. In this article, the three parameter Weibull distribution and Cuckoo search algorithm are combined to optimize the reliability and the maintenance cost of a RIAD 60 storage system. The numerical results show that using the proposed approach in this article, a RAID 60 system could reach its optimal reliable state by swapping less reliable drives by others more (used or new) reliable, whichever, is optimal according to the sought-after reliability threshold.

OBJECTIVES: We sought to use a clinical decision support system (CDSS) to save costs and to improve scheduling of vestibular patients in an otolaryngology clinic. STUDY DESIGN AND SETTING: We conducted a concurrent review of 50 vestibular patients scheduled in the University of Missouri otolaryngology clinic with or without testing based on the outcome of a CDSS. The CDSS was implemented using Web-based technology. Charges incurred by the health care system through tests determined by the CDSS were compared with those incurred using the standard procedure of ordering hearing tests and electronystagmography for all patients. RESULTS: Thirty-nine tests were prescheduled using the CDSS. Twenty-five additional tests were ordered after the visit. The CDSS resulted in savings of $37,904.00 in charges to the health care system. The CDSS showed high specificity and variable sensitivity. CONCLUSION: A Web-based CDSS can be used to better manage and coordinate patient encounters. SIGNIFICANCE: One important reason to use a CDSS in health care management is to lower costs.

In September 2015, the Virginia Department of Transportation instituted an active traffic management system on I-66 in Northern Virginia. I-66 is a major commuter route into Washington, D.C., that experiences significant recurring and nonrecurring congestion. The active traffic management system sought to manage existing capacity dynamically and more effectively with hard shoulder running, advisory variable speed limits, lane use control signs, and queue warning systems. An initial before-and-after analysis of the system’s operational effectiveness was performed with probe-based travel time data from the provider, INRIX, and used records from the active traffic management’s traffic operations center. On weekdays, statistically significant improvements were often observed during off-peak periods, but conditions did not improve during peak periods. Weekends showed the greatest improvements, with travel times and travel time reliability measures improving by 10% to 14%. Segment-level analysis revealed that most of the benefits were attained because of the use of hard shoulder running outside of the peak periods, which created additional capacity on I-66. Benefits due to advisory variable speed limits were inconclusive because of limited data.

Gastric sodium loading results in an increase in the portal venous concentration of vasoactive intestinal peptide (VIP) and down-regulation of both the intrahepatic and circulating renin–angiotensin systems. In the present study we sought to determine whether an increase in the concentration of VIP in the portal circulation might act to down-regulate the intrahepatic and/or circulating renin–angiotensin systems. Male Sprague–Dawley rats were infused intraportally with haemaccel vehicle or VIP in haemaccel for 60 min. Livers were harvested and blood was sampled. Angiotensin-converting enzyme (ACE) activity and angiotensinogen, angiotensin I, angiotensin II and renin concentrations were measured. VIP infusion decreased hepatic ACE activity (P < 0.05), the hepatic angiotensinogen concentration (P < 0.001) and the hepatic angiotensin I concentration (P < 0.05). The plasma angiotensinogen concentration and serum ACE activity were also decreased by intraportal VIP infusion (P < 0.05 for each). Plasma renin, angiotensin I and angiotensin II concentrations were unchanged by VIP infusion. We conclude that an increase in the portal venous VIP concentration down-regulates the intrahepatic renin–angiotensin system. These changes are similar to those reported after gastric sodium loading, and we suggest, therefore, that the increase in portal venous VIP that occurs after gastric sodium is the means by which the gastric sodium sensor signals the liver to effect these changes in the renin–angiotensin system.

190 Background: Breast cancer survivors face physical, psychological, medical, social, cultural and spiritual challenges. Services to address these needs are frequently not available or if available unknown. We sought to determine whether formation of a community-based comprehensive breast cancer survivorship program was feasible. Methods: After months of relationship building, the Women’s Breast Health Fund of the Community Foundation of Greater Birmingham (CFGB), awarded funding to support a systematic assessment of available services in the region. Survivors, their loved ones, providers and other national models of care were surveyed and interviewed. Focus groups including a Lesbian, Bisexual, Gay, Transgender, Queer (LGBTQ) group were held. Aggregated results were presented in monthly meetings to executive level hospital administrators from all health systems in the area, the UAB School of Nursing and CFGB. Results: Survivors seek advice from other survivors more than any other source. Gaps in services exist. Breast cancer survivors were often unaware of existing services. Services were not available to some cultural/ethnic groups or loved ones; staff of some services were not culturally sensitive to the needs of survivors. Some services were available to all, while others require payment. There was no source of authoritative, evidence-based information on breast cancer survivorship except for a few providers. After 12 months the group of executives from all health systems committed to support the formation of a community-based comprehensive breast cancer survivorship program designed to assist breast cancer survivors, their loved ones and institutions by providing reliable information about services. Conclusions: Breast cancer survivors, providers, and local health systems all support the development of a comprehensive breast cancer survivorship program. We feel that it will address unmet needs of breast cancer survivors, allowing each institution to address needs for individual patients. By using the breast cancer survivorship program to assess individual needs and to provide information about services for identified needs, we expect repetitive services will be reduced and quality of life for breast cancer survivors will improve.

Objective: To evaluate the long-term outcomes and satisfaction of nonurgent patients who seek care in the emergency department (ED) and are diverted to primary health services (PHS). Methods: Data were collected from 264 nonurgent patients diverted from the ED of a tertiary public university hospital in São Paulo, Brazil. The nonurgent patient definition was performed by Manchester triage system version II (MTS-II) associated to medical interview in the triage service. Satisfaction levels were evaluated by telephone interviews. The outcomes were assessed within 30 days after the ED visit. Results: Based on the MTS-II, 56.4 percent of the diverted patients were classified as green, 34.3 percent as blue, and 9.3 percent as white. Only one patient required a hospital admission and no deaths were registered within 30 days after ED diversion. After diversion, the majority of patients searched for PHS (62.7 percent), 14.4 percent sought out other EDs, and 22.9 percent did not seek out any other health services. Regarding patient satisfaction, 61.9 percent evaluated the triage team as fair, good, or very good. Conclusions: Our study suggests that diverting nonurgent patients from the ED to PHS may be carried out in a hierarchic system like the Brazilian public healthcare system. The MTS-II can be a useful triage system to support physician in the diverting process. In addition, patient satisfaction with the refusing was reasonable. Future studies should be designed to evaluate patient safety outcomes in a larger sample and in different healthcare systems.

A novel taxonomy of built-in self-test (BIST) methods is presented for the testing of micro-electro-mechanical systems (MEMS). With MEMS testing representing 50% of the total costs of the end product, BIST solutions that are cost-effective, non-intrusive and able to operate non-intrusively during system operation are being actively sought after. After an extensive review of the various testing methods, a classification table is provided that benchmarks such methods according to four performance metrics: ease of implementation, usefulness, test duration and power consumption. The performance table provides also the domain of application of the method that includes field test, power-on test or assembly phase test. Although BIST methods are application dependent, the use of the inherent multi-modal sensing capability of most sensors offers interesting prospects for effective BIST, as well as built-in self-repair (BISR).

Response is interpreted as a response, reaction and answer. A response will appear from receiving the message after a series of communications. Financing is one of the bank's main tasks, namely the provision of funds provision facilities to meet the needs of the parties which constitute a unit deficit. Employee response is used in this case to find out how much employees respond well or are interested in financing Islamic banks. This research is descriptive qualitative using a phenomenology approach. Therefore, the authors chose to use qualitative research methods to determine how to search, collect, manage, analyze data from research results and draw conclusions.The results of the study show that: 1) The financing system on Islamic bank products uses four different patterns or systems, namely: profit sharing system, buying and selling system, rental system, loan system. 2) Facilities provided by Islamic banks in the form of providing funds to meet the needs of parties with a profit sharing system. 3) Respondents' responses indicate that most respondents gave positive responses. This condition gives the impression that the employee's response to Islamic banking is perceived positively by respondents. Of the various types of products in Islamic banks, only the savings that are most sought after by sharia bank customers and their reasons for choosing these products because of sharia-compliant, lawful and safe Islamic bank products.

AbstractThe study sought to explore the attitudes of community pharmacists, university-based pharmacists, and pharmacy students before and after exposure to computerized systems of on-line information services. A 42-item attitudinal survey was administered to 21 community pharmacists, 7 university clinical pharmacist faculty, and 17 senior pharmacy students, prior to and at the end of a year of access to Grateful Med® and BRS Colleague®. Few significant differences were noted among the participants at baseline. No significant interaction-effect differences for type of participant or system used were found. Participants were generally positive about computers in general, the accuracy of on-line information services, their impact on knowledge and confidence, and their usefulness for pharmacists.

Background Preoperative neuroimaging assessment of collateral circulation is important for selecting acute ischemic stroke patients who are appropriate for endovascular treatment. We sought to validate the capillary index score system in an Asian population and compare its ability in predicting clinical outcomes with the American Society of Interventional and Therapeutic Neuroradiology/Society of Interventional Radiology grading system. Methods We continuously enrolled acute ischemic stroke patients from two neurological centers from March 2014 to March 2017. Multivariate analyses were performed to assess the capillary index score system with 90-day clinical outcome (modified Rankin scale score). The scoring systems were compared for predicting good (modified Rankin scale 0–2) and excellent (modified Rankin scale 0–1) functional outcomes using area under the receiver operating characteristic curves. Results We identified 157 patients (median age, 65 years; 96 (61.1%) males), of whom 71 (45.2%) patients with 90-day good functional outcomes were selected. Capillary index score was independently associated with clinical outcome after endovascular treatment (OR 0.63; 95% CI 0.43–0.92; P = 0.016) with its predictive ability for good functional outcome (area under the receiver operating characteristic curve 0.755). For excellent functional outcome, the capillary index score system was not inferior to the American Society of Interventional and Therapeutic Neuroradiology/Society of Interventional Radiology grading system (area under the receiver operating characteristic curve 0.748 versus 0.793, P = 0.09). Conclusions The capillary index score system is a potentially useful tool for predicting 90-day functional outcomes in acute ischemic stroke patients after endovascular treatment.

SummaryComponents of the adaptive immune system, in particular lymphocytes and immunoglobulin, play a major role in advanced atherosclerotic lesions. We sought to determine whether routine measurements of the relative number of circulating lymphocytes (%L) and γ -globulin (%G) reflecting immunoglobulin are related to event-free survival in patients with stable coronary artery disease (CAD). We prospectively studied the combined endpoint all-cause mortality, myocardial infarction and coronary revascularization procedures in 141 patients after successful percutaneous coronary intervention during a median follow-up time of 13.2 years. Using Cox regression, we found a significant influence of %L on event-free survival (P = 0.007) with a relative risk of 2.21 comparing third to first tertile. Subjects with higher %G values likewise had a shorter event-free survival (P = 0.008) with a relative risk of 1.67 comparing third to first tertile. The predictive value of %L and %G remained significant after adjustment for demographic data, cardiovascular risk factors, extent of CAD and other inflammatory markers. We conclude that the fraction of γ -globulin and in particular the relative lymphocyte cell count may serve as readily available and reliable prognostic tools for the long-term outcome in patients with stable CAD.

ABSTRACTIn the 1980s and early 1990s, sending remittances from South Africa posed major challenges for Ndebele migrants. As a result households receiving remittances only did so at irregular intervals. With increased diasporisation into South Africa, it was to be expected that new channels would open up. This article explores what is known as the malayisha system, its role and significance as an informal channel of remittances into Ndebele society. It argues that the system bridged the geographical gap between Matabeleland and Johannesburg, averting food insecurity and poverty for semi-proletarian households in Matabeleland. By facilitating the movement of goods and people between Matabeleland and South Africa, the system became instrumental in the quest of households to reconstruct their livelihoods after the destruction of their rural–urban-based livelihoods in Zimbabwe due to perennial droughts and ESAP. As a result, the services of omalayisha are highly sought-after, by both the migrant community in South Africa and households in Matabeleland.

Few studies sought to analyze the expression and function of the nonneuronal acetylcholine system in bone remodelingin vivodue to the lack of suitable models. We established a rat maxilla expansion model in which the midline palatine suture of the rat was rapidly expanded under mechanical force application, inducing tissue remodeling and new bone formation, which could be a suitable model to investigate the role of the nonneuronal acetylcholine system in bone remodelingin vivo.During the expansion, the expression pattern changes of the nonneuronal cholinergic system components and the mRNA levels of OPG/RANKL were detected by immunohistochemistry or real-time PCR. The value of the RANKL/OPG ratio significantly increased after 1 day of expansion, indicating dominant bone resorption induced by the mechanical stimulation; however after 3 days of expansion, the value of the RANKL/OPG ratio significantly decreased, suggesting a dominant role of the subsequent bone formation process. Increasing expression of Ach was detected after 3 days of expansion which indicated that ACh might play a role in bone formation. The mRNA expression levels of other components also showed observable changes during the expansion which confirmed the involvement of the nonneuronal cholinergic system in the process of bone remodelingin vivo. Further researches are still needed to figure out the detailed functions of the nonneuronal cholinergic system and its components.

Background: Involvement of the central nervous system (CNS) by mycosis fungoides (MF) is rare; however, it portends a poor prognosis. While aggressive multimodality therapy may improve outcomes, the role of radiation therapy (RT) is not well defined. Objectives: We sought to explore the efficacy of RT in the management of CNS involvement by MF. Method: We retrospectively identified five patients with MF and CNS involvement who received cranial or craniospinal RT at a single institution. Patient characteristics, disease features, radiographic findings, treatments delivered, and outcome data were extracted from the electronic medical record. Results: All 5 patients had neurologic deficits at RT initiation, and 4 experienced at least a partial improvement. Of 4 patients evaluated by MRI after RT completion, 3 had complete resolution of CNS disease within the irradiated field. At the time of last follow-up, all patients had died of MF. The median time to death was 7.4 months (range 1.0–21 months) from their diagnosis with CNS involvement and 1.2 months (range 0.4–7.1 months) from the end of RT treatment. Conclusions: We observed high rates of radiographic response and palliation of neurological symptoms. Nonetheless, all patients succumbed to their disease shortly after treatment, confirming the poor prognosis of this condition. Our findings suggest that RT may play a valuable palliative role for these patients.

Percutaneous mitral valve repair (PMVR) with the MitraClip system (Abbott Vascular, Santa Clara, CA, USA) is a valid therapeutic option for patients with severe mitral regurgitation (MR) deemed to be at high or prohibitive surgical risk. Despite the reassuring data on efficacy and long-term durability of the procedure, the proportion of patients with residual or relapsing severe MR after MitraClip therapy is not negligible. In light of the detrimental prognostic impact of severe MR, repeat interventions are increasingly performed in clinical practice using different techniques. In high-risk settings, percutaneous procedures have proven to be effective and safe at reducing MR. Building on this, we sought to summarize the current landscape and clinical experience of reinterventions after failed MitraClip therapy, so as to assist physicians facing the clinical hurdle of proper treatment management after failed PMVR.

ObjectiveWe sought to evaluate an electronic referral form to increase referral for genetic risk assessment of women with newly diagnosed epithelial ovarian cancer.MethodsA form summarizing referral for genetic counseling for women with ovarian cancer was introduced into the electronic medical record allowing gynecologic oncologists to electronically submit a request for genetic services. Analysis compared patient and provider characteristics for women newly diagnosed with ovarian, fallopian tube, and primary peritoneal cancer referred 1 year before and after introducing the form. All patients were seen in a single fee-for-service university-based cancer center clinic.ResultsThere were 86 newly diagnosed ovarian cancer patients seen before and 83 seen after the introduction of the electronic referral form. Most lived in the metropolitan area and had stage III to IV disease, serous histology, a documented family history, and a treating oncologist who was less than 10 years from completion of fellowship. Postintervention referral rates increased from 17% to 30% (P = 0.053). Factors best predicting referral were whether the patient was seen after the intervention (P = 0.009), resided in the metropolitan area (P = 0.006), and had been identified as at high hereditary risk (P < 0.0001). Sixty percent of the referred patients participated in counseling. There were no differences in baseline characteristics of the referred patients before and after the intervention.ConclusionsReferral rates increased with the introduction of an electronic medical record referral form suggesting that streamlining the physician referral process might be effective at increasing referrals for cancer genetic risk assessment.

Abstract - The problem of using Power System Stabilizer (PSS) in generator excitation is how to determine optimal PSS parameter. To overcome these problems, the authors use a method of intelligent bats based algorithm to design PSS. Bat Algorithm is an algorithm that works based on bat behavior in search of food source. Correlation with this research is, food sources sought by bats represent as PSS parameters to be optimized. Bat's algorithm will work based on a specified destination function, namely Integral Time Absolute Error (ITAE). In this research will be seen the deviation of velocity and rotor angle of each generator, in case of disturbance in bakaru generator. The analysis results show that the uncontrolled system produces a large overshoot oscillation, and after the addition of PSS oscillation control equipment can be muted. So that the overshoot and settling time of each generator can be reduced and the generator can quickly go to steady state condition.

OBJECTIVE: We sought to assess the clinical status of pediatric sinusitis patients after a minimum of 2 years follow-up and to evaluate relative treatment effectiveness. STUDY DESIGN: We conducted a medical record review of 308 patients, with a 2-year follow-up questionnaire. RESULTS: Overall, 55% of children were “much improved.” A 4-stage prognostic system was developed and used to compare results in 133 patients who had not responded to medical treatment and who had positive sinus computed tomography scans. Endoscopic sinus surgery (ESS) had no apparent advantages in stages I and IV, but in stages II and III, rates of “much improved” for ESS versus medical therapy were 79% versus 54% and 68% versus 42%, respectively. CONCLUSION: These results suggest that ESS may be particularly effective for patients from intermediate prognostic stages. SIGNIFICANCE: This type of prognostic staging and evaluation of treatment effectiveness is necessary and important in chronic sinusitis, where randomized trials are unlikely to be performed.

This paper is concerned with spatial policy in Ireland. It adopts an historical lens to help explain why Ireland currently finds itself at the bottom of the European league table with regard to local governance. After categorising the Irish political and planning system as highly centralised, bureaucratic and linear, the paper uses a case study of the Moycullen village plan to show an alternate path towards place development in Ireland. This case study sets out to contrast the desire of a people to collaborate in the authorship of their place with the top down nature of spatial planning in Ireland. By making clear the methods and results of the project, this paper highlights the latent demand that exists in a community that is subject to national planning system that reduces their ability to affect change. Through the use of some innovative approaches, this project has sought to fire the geographic imaginary of a people with respect to their place.

Internal security is today an intensively explored area of national security. This is hardly surprising. It is this area that is mainly concerned with emerging threats. Hence, new scientifically justified solutions are being sought to counteract them and minimise the effects of their occurrence. Given the above, the research has been conducted, and its results are presented in this paper. It addresses the main problems relating to internal security through the prism of identification criteria, essence, and systemic approach. That has made it possible to present the assumptions of the project in question. The research aimed to diagnose the functioning of the state’s internal security system, and the participation of academic centres dealing with security issues. One of such centres is the Police Academy, which is implementing a project entitled “Vacuum chamber for revealing fingerprints with organic compounds in the gas phase’, No DOB-BIO9/03/01/2018. It was financed by the National Centre for Research and Development under call for proposals No 9/2018 for the execution and financing of projects in the field of scientific research or development work for the benefit of national defence and security. After they were implemented, it was stated that the constant changes in the internal security environment, and the limited potential of state institutions designated to care for this security entail the need to continually search for practical solutions to ensure the undisturbed existence and development opportunities of each entity. These searches are conducted by scientific institutions, which with their knowledge and experience serve to improve the tools used in the practice of institutions responsible for internal security.

Aim. To study the hemostasis system state in rats during hypothermic and post-hypothermic periods. Methods. Male Wistar rats (53 individuals) were used in the study. The animals from the experimental group underwent single immersion cooling in water at a temperature of 5 °C until profound hypothermia was reached, the control group of the animals was placed in water at a temperature of 30 °C. From the animals of the first group, blood was taken immediately after reaching profound hypothermia and from the second group - 24 hours after cooling was stopped. Results. Comparative analysis of the results showed that immediately after the end of a single cold exposure, significant increase in platelet aggregation activity occured, as well as appearance of thrombinemia markers in the bloodstream and inhibition of fibrinolytic system activity. 24 hours after the experimental exposure, these parameters returned to the initial values. When assessing the activity of external and internal ways of coagulation immediately after the termination of cooling, development of hypocoagulation was established, both with routine tests and from thromboelastography. After 24-hour period, hypocoagulation, recorded immediately after reaching the sought rectal temperature, persisted. Thus, after the end of a 24-hour period after cold exposure termination, most of the parameters of hemostatic system that had deviated immediately after the end of the experiment, returned to the normal level. The delayed effect of hypothermia in such cold exposure regimen manifested only by hypocoagulative shift at the initial stages of coagulation. Conclusion. Signs of abnormal hemostasiological blood properties, recorded immediately after the cooling termination, disappear within 24 hours, and only hypocoagulation persists in the blood.
