Research activities that have focused on the development and understanding of self-healing concrete have proposed various technologies intended to enhance self-healing capacity. The self-healing performance cannot be identified sufficiently with either a single test or a specific parameter because there are a number of factors that influence the performance of self-healing. Thus, it has become necessary to provide standardized test methods that make it possible to verify and compare the performance of self-healing materials. In this paper, self-healing mortars based on inorganic admixtures, which are developed for sealing 0.3 mm cracks with a healing index of 90%, are produced and used to validate the water permeability test and to propose protocols for the evaluation of self-healing performance. The healing performances of three self-healing mortars and a plain mortar as a reference are evaluated with a comparative study. The equivalent crack width, which can be estimated from the water flow rate, is suggested as a rational evaluation index. Finally, a self-healing performance chart is proposed to comprehensively show the healing performance of cement-based materials.

Abstract Introduction The scalp is an appealing donor site for split-thickness skin grafting (STSG) of burns due to its concealment upon hair regrowth and healing potential; however, it is often reserved for massive burns with limited donor sites. This study aims to understand scalp donor site (SDS) outcomes and complications to elucidate viability of the SDS as a first-line option for coverage of burns. Methods A systematic review across five databases was conducted. Original research reporting cohort outcomes of STSG coverage of burns using the SDS was included. Donor site healing time, graft thickness, graft take, and complications were analyzed. Thereafter, a retrospective chart review of patients who had SDS-STSGs for burns from 2010–2018 was done. Outcomes and complications were compared to non-SDS-STSGs (NSDS-STSGs) in criteria-matched controls. Results Systematic Review: 1489 articles were reviewed. 15 cohort studies met inclusion criteria, yielding 1761 patients who has SDS-STSG (mean age=17 yrs, mean TBSA=37%). Alopecia and folliculitis occurred in 5% and 3% of patients, respectively; folliculitis was associated with certain hair types. Other complications (eg. hypertrophic scarring, hair transfer) were seen in less than 1.5% of patients. Rapid donor site healing times (mean=8 days) and capacity for re-harvest (mean=2 harvests/patient) were noted. Retrospective Review: 30 patients underwent grafting with SDS-STSGs, 23 of whom also received NSDS-STSGs and were thus self-controls; criteria-matched controls were found for the remaining 7. Graft thickness and graft take did not differ between SDS and NSDS groups, while donor site healing time was faster for SDSs vs NSDSs (9 vs 11 days, p=0.02). SDS complications included: alopecia (n=2), delayed healing (3), and folliculitis (1). NSDS complications included: delayed healing (n=1), dyspigmentation (2), and hypertrophic scarring (3). These complications could not be attributed to age, ethnicity, graft thickness, number of graft harvests, comorbidity burden, TBSA, or inhalational injury status. Conclusions The scalp donor site may represent a viable first-line option for split-thickness skin grafting of burns. Our cohort suggests that scalp donor sites pose comparable risks to non-scalp donor sites. Further studies elucidating the role of hair type in SDS-STSG complications are required to optimize outcomes with patient-specific donor site selection. Applicability of Research to Practice In select patients, the scalp may be considered as a first-line donor site for split-thickness skin grafting of burns, with potentially faster donor site healing time and comparable risks to non-scalp donor sites. Some scalp-specific complications may be linked to differences in hair type.

Abstract Introduction Potential complications of autografting for burn wound coverage of the hand include edema, hematoma formation, and bleeding; all of which can lead to graft failure. Self-adherent elastic wraps are commonly used by burn rehabilitation clinicians to minimize complications by providing graft protection and decreasing edema post-operatively; however, there is a lack of evidence on its impact on graft healing. The purpose of this study was to determine if the application of self-adherent elastic wraps to the hand in the operating room after autografting improves the percentage of graft viability. Methods A retrospective chart review was performed for 37 patients with burned hands who underwent autografting from January 2017 to July 2021. Grafted hands were categorized into 2 groups: post-operative dressings with and without self-adherent elastic wraps. Post-operative day 4 pictures for both groups were collected from the medical record and a blinded digital photograph analysis of graft viability was performed by 5 expert raters including 3 Burn Surgery Fellows,1 Burn Attending Surgeon and 1 Hand Attending Surgeon. A rating system was developed based on percentage of graft take as seen in Table 1 and presence of hematomas were assessed. Results Patients who received self-adherent elastic wraps suffered burns with significantly larger TBSA (p=0.007) and were admitted for a longer duration (p=0.009) than patients who did not. Patients with elastic wrap had a higher percentage of grafts with >95% take (64.0% vs 41.7%, p=0.227) and a lower rate of hematoma formation (24.0% vs. 41.7%, p=0.443). Intra-class correlation coefficient across raters was 0.90 for graft take and 0.87 for determining presence of hematomas, indicating excellent interrater reliability. Conclusions Despite suffering larger burns requiring longer hospitalizations, patients who received elastic wrap had a higher rate of >95% graft take than those without. This study is limited by a relatively small sample size, however these findings warrant continued research in the use of self-adherent elastic wrap to maximize graft take in hand burns.

Objective. The patterns and risks of perioperative use of anti-tumor necrosis factor (anti-TNF) medication in patients with rheumatoid arthritis (RA) are not well studied. We examined the patterns of perioperative anti-TNF use and risk of postoperative adverse events (AE) in patients undergoing total knee replacement (TKR).Method.Retrospective cohort study with followup. RA cases within a TKR registry were identified by ICD-9 code (714.0) or self-report. Mailed questionnaires queried anti-TNF use and duration of RA. AE were determined by chart review and patient self-report, and included surgical site infection, pulmonary embolus, deep venous thrombosis, pneumonia, and any infection or re-operation within 6 months.Results.There were 268 TKR cases with RA. The stop time for anti-TNF preoperatively correlated with dosing schedule; restart time was after wound healing. There were 7 surgical site infections (3%), one (0.4%) of which was a deep joint infection in bilateral TKA requiring explant. The anti-TNF group had 3.26% (3/92) local site infection versus 2.10% (3/143) in the group without anti-TNF and this difference was not statistically significant (Fisher exact test, p = 0.68). The one deep joint infection was in the anti-TNF group. Six-month AE rate was 7.61% in the anti-TNF group versus 6.99% in the group without anti-TNF (Fisher exact test, p = 1.0).Conclusion.There was a low risk of infection and perioperative adverse events in patients with RA receiving anti-TNF therapy who were undergoing TKR. This raises the question whether it is necessary to stop anti-TNF for a long period prior to surgery. Given the possible risks associated with stopping anti-TNF, including worsening of disease, further study is needed to determine optimal perioperative use of anti-TNF among patients with RA undergoing TKR.

Background. Bilateral reduction mammoplasty is a common plastic surgery procedure that can be complicated by unfavorable scar formation along incision sites. Surgical adhesives can be utilized as an alternative or as an adjunct to conventional suture closures to help achieve good wound tension and provide an adequate barrier with excellent cosmesis. The recently introduced DERMABOND PRINEO Skin Closure System Skin Closure System combines the skin adhesive 2-octyl cyanoacrylate with a self-adhering polyester-based mesh. Proposed benefits of wound closure with DERMABOND PRINEO Skin Closure System, used with or without sutures, include its watertight seal, easy removal, microbial barrier, even distribution of tension, and reduction in wound closure time. Although allergic reactions to 2-octyl cyanoacrylate have been reported, few allergic reactions to DERMABOND PRINEO Skin Closure System have been noted in the literature. This case series describes three patients who experienced an allergic reaction to DERMABOND PRINEO Skin Closure System after undergoing elective bilateral reduction mammoplasties at our institution to further explore this topic.Methods. Retrospective chart review of bilateral reduction mammoplasty patients who received DERMABOND PRINEO Skin Closure System dressing at our institution was performed.Results. Three patients were identified as having a rash in reaction to DERMABOND PRINEO Skin Closure System after bilateral reduction mammoplasty. All three patients required systemic steroid treatment to resolve the rash. One patient was identified as having a prior adhesive reaction.Conclusions. DERMABOND PRINEO Skin Closure System has demonstrated its efficacy in optimizing scar healing and appearance. However, as we demonstrate these three allergic reactions to DERMABOND PRINEO Skin Closure System, caution must be utilized in its usage, namely, in patients with a prior adhesive allergy and in sites where moisture or friction may be apparent.

Concrete and cement-based materials inherently possess an autogenous self-healing capacity. Despite the huge amount of literature on the topic, self-healing concepts still fail to consistently enter design strategies able to effectively quantify their benefits on structural performance. This study aims to develop quantitative relationships through statistical models and artificial neural network (ANN) by establishing a correlation between the mix proportions, exposure type and time, and width of the initial crack against suitably defined self-healing indices (SHI), quantifying the recovery of material performance. Furthermore, it is intended to pave the way towards consistent incorporation of self-healing concepts into durability-based design approaches for reinforced concrete structures, aimed at quantifying, with reliable confidence, the benefits in terms of slower degradation of the structural performance and extension of the service lifespan. It has been observed that the exposure type, crack width and presence of healing stimulators such as crystalline admixtures has the most significant effect on enhancing SHI and hence self-healing efficiency. However, other parameters, such as the amount of fibers and Supplementary Cementitious Materials have less impact on the autogenous self-healing. The study proposes, through suitably built design charts and ANN analysis, a straightforward input–output model to quickly predict and evaluate, and hence “design”, the self-healing efficiency of cement-based materials.

This study aims to produce mortar through the addition of oil palm shells (OPS)-activated charcoal and oil palm empty fruit bunch (OPEFB) hydrochar, which has high mechanical properties, self-healing crack capabilities, and pollutant adsorption abilities. The cracking of mortar and other cementitious materials is essential in anticipating and reducing building damages and ages due to various reasons, such as chemical reactions, foundation movements, climatic changes, and environmental stresses. This leads to the creation of self-healing mortar, which is produced by adding reductive crack size materials to form calcium carbonate (CaCO3) and silicate hydrate (3CaO.2SiO2.2H2O, CSH). One of these materials is known as activated charcoal, which is obtained from oil palm shells (OPS) and oil palm empty fruit bunches (OPEFB) fibres. This is because the OPS-activated charcoal minimizes crack sizes and functions as a gaseous pollutant absorber. In this study, activated charcoal was used as fine aggregate to substitute a part of the utilized sand. This indicated that the utilized content varied between 1–3 wt.% cement. Also, the mortar samples were tested after 28 days of cure, including the mechanical properties and gaseous pollutant adsorption abilities. Based on this study, the crack recovery test was also performed at specific forces and wet/dry cycles, respectively, indicating that the mortar with the addition of 3% activated charcoal showed the best characteristics. Using 3% of the cement weight, OPEFB hydrochar subsequently varied at 1, 2, and 3% of the mortar volume, respectively. Therefore, the mortar with 3 and 1% of OPS-activated charcoal and OPEFB hydrochar had the best properties, based on mechanical, self-healing, and pollutant adsorption abilities.

This article analyzes the epistemology of healing developed by kiai-traditional healer. In the Indonesian-Islamic tradition of healing, the kiai-traditional healer develops his own epistemological construct of healing, based on: (1) textual basis (<em>bayânî</em>) referring to classical Islamic references, such as <em>al-Awfâq</em> and <em>Shams al-Ma‘ârif</em>; (2) intuitive power (‘<em>irfânî</em>) in the form of mystical practices, such as spiritual endeavor (<em>mujâhadah</em>), <em>tawajjuh</em>, self-cleansing (<em>tazkîyat al-nafs</em>), <em>wara</em>‘, consistency (<em>istiqâmah</em>), and forbearance (<em><em>ṣ</em>abr</em>); and (3) pragmatic basis of the truth, particularly its utility value for the society. Normatively, the kiai-traditional healer uses different media of healing in terms of its easiness, belief or charm. Accordingly, it results in an impression that there are many modes of healing, i.e., medical, magical, mystical, and cosmic. The meaning of healing by the kiai-traditional healer constitutes a placebo effect of well-being, that is, an attempt of the kiai to implant evocation to recover the condition of human being.

ABSTRACTThis artwork thesis depicts a multimedia performance, which combines several media such as photos, videos, sound recordings, installation, diagrams, charts, graphic design, and scientific journals, as well as digital cameras in performance-lecture. The creation practice borrowing the mode of a lecture combined with performance refers to "jek nebenne!" as the director's memory-recollection to start his work practice. The term "jek nebenne!" is not a standard language which means "don't mess with me!", it is used as the director's foothold in connecting a trauma in a family as a result of domestic violence which is reflected in the same experience of his mother, just like the experience of a female masseuse, brick miner, and farmworker for over thirty years in Bangkalan over the behavior of their respective husbands, as well as a shift of function from the feminine to the masculine. "Jek nebenne!" becomes some sort of creation reference which was often said by his mother, haunting him to this day and suspected to be the source of failure. Including the failure to integrate with the two performers who also experienced trauma (sexual and gender) until the show goes on. His position was leaked as a therapist to evacuate from the situation by sharing his sensitive emotion to the public; leaking of conflict concoction, exploration of body and ability, as well as performer's skill through medias mentioned above. The practice of performance-lecture focuses on psychodrama with a shift of dramaturgy from trauma to sensitive and intervening performer and the production team to show that domestic violence creates a prolonged scar. The director fears that the cruising range of his biography trauma will cause him to re-experience the vulnerability and leaving the audience with intricacy. The practice of such shift as a process of evacuation from acute trauma, as a continuation of "jek nebenne!" self-healing method for positive growth of the soul. Keywords: performance-lecture, "jek nebenne!", self-healing method

This chapter begins with human and animal sacrifices to appease the gods during pestilence, but shows that such acts were extremely rare and, when they occurred, quickly disappeared or changed form often to animal sacrifice. It investigates the scapegoat in ancient epidemics, showing the concept as far removed from present-day notions. The ancient one was often a volunteer, exemplary of self-sacrifice for the greater good of the community. Instead of being outcasts, foreigners, or despised minorities (for whom we reserve the term today), in antiquity, almost without exception, they were the elites. More emphatically, from literary and historical descriptions of the fifth century BCE to the sixth CE Justinianic Plague, the chapter charts societal reactions to epidemics, finding that they spawned acts of altruism, public holidays, and self-sacrifice. Instead of blaming or inflicting violence on ‘others’, epidemics were forces for unity, healing rifts between classes, factions, and regions at war.
