AbstractIntroductionIn preparation for the London 2012 Olympic Games, existing syndromic surveillance systems operating in England were expanded to include daily general practitioner (GP) out-of-hours (OOH) contacts and emergency department (ED) attendances at sentinel sites (the GP OOH and ED syndromic surveillance systems: GPOOHS and EDSSS).Hypothesis/ProblemThe further development of syndromic surveillance systems in time for the London 2012 Olympic Games provided a unique opportunity to investigate the impact of a large mass-gathering event on public health and health services as monitored in near real-time by syndromic surveillance of GP OOH contacts and ED attendances. This can, in turn, aid the planning of future events.MethodsThe EDSSS and GPOOHS data for London and England from July 13 to August 26, 2012, and a similar period in 2013, were divided into three distinct time periods: pre-Olympic period (July 13-26, 2012); Olympic period (July 27 to August 12); and post-Olympic period (August 13-26, 2012). Time series of selected syndromic indicators in 2012 and 2013 were plotted, compared, and risk assessed by members of the Real-time Syndromic Surveillance Team (ReSST) in Public Health England (PHE). Student’s t test was used to test any identified changes in pattern of attendance.ResultsVery few differences were found between years or between the weeks which preceded and followed the Olympics. One significant exception was noted: a statistically significant increase (P value = .0003) in attendances for “chemicals, poisons, and overdoses, including alcohol” and “acute alcohol intoxication” were observed in London EDs coinciding with the timing of the Olympic opening ceremony (9:00 pm July 27, 2012 to 01:00 am July 28, 2012).ConclusionsSyndromic surveillance was able to provide near to real-time monitoring and could identify hourly changes in patterns of presentation during the London 2012 Olympic Games. Reassurance can be provided to planners of future mass-gathering events that there was no discernible impact in overall attendances to sentinel EDs or GP OOH services in the host country. IntroductionFor the London Olympic and Paralympic Games in 2012, a sentinel ED syndromic surveillance system was established to enhance public health surveillance by obtaining data from a selected network of EDs, focusing on London. In 2017, a new national standard Emergency Care Dataset was introduced, which enabled Public Health England (PHE) to initiate the expansion of their sentinel system to national coverage. Prior to this initiative, we estimated the added value, and potential additional resource use, of an expansion of the sentinel surveillance system.MethodsThe detection capabilities of the sentinel and national systems were compared using the aberration detection methods currently used by PHE. Different scenarios were used to measure the impact on health at a local, subnational and national level, including improvements to sensitivity and timeliness, along with changes in specificity.ResultsThe biggest added value was found to be for detecting local impacts, with an increase in sensitivity of over 80%. There were also improvements found at a national level with outbreaks being detected earlier and smaller impacts being detectable. However, the increased number of local sites will also increase the number of false alarms likely to be generated.ConclusionWe have quantified the added value of national ED syndromic surveillance systems, showing how they will enable detection of more localised events. Furthermore, national systems add value in enabling timelier public health interventions. Finally, we have highlighted areas where extra resource may be required to manage improvements in detection coverage.

The present study investigates the representation of King Arthur in the Historia Regum Britanniae of Geoffrey of Monmouth (1343–1400). In doing so, it concentrates on specific historical context – early Anglo-Saxon England – and a specific form of authority – Anglo-Saxon kingship. The intention of the study is to show how Geoffrey of Monmouth used historical chronicles, not only for cataloguing the stories of various rulers of the island, but also for creating and shaping a single leader who can unify the kingdom.The study claims that the ideal kingship constructed around the figure of King Arthur in the Historia involved a re-orientation of some of the more conventional norms of kingship; the heroic qualities of martial prowess, generosity and morality are quite essential in every conception of an ideal king. Geoffrey’s conception of this ideal king was largely influenced by his personal aspirations, some of which have been outlined in the introduction of this article. The remaining parts of this study offer a historical as well as a literary analysis of the text, addressing the main qualities of kingship that were articulated in the text.All translated quotations from Historia Regum Britanniae are taken from Geoffrey of Monmouth, History of Kings of Britain, translated by Lewis Thorpe (London, Penguin Book, 1966). The Latin text consulted was Geoffrey of Monmouth, Historia Regnum Britianniae, Vol. 1, Bern, Burgerbibliothek, Ms 568, ed. Neil Wright (Cambridge; D.S. Brewer, 1984).

RÉSUMÉLes volumes de Kumashiro (1995) et Seltzer (1995) soulèvent beaucoup de poins sur le vieillissement de la population et présentent done de l'information pertinente à la santé et à la politique sociale au Canada. Malgré ce lien, le mode de présentation de chaque volume ne crée pas d'utilité directe quant à l'établissement des politiques. Dans son livre, Kumashiro pose comme principe que les travailleurs âgés forment la base du maintien et de la revitalisation des sociétés et des économies du globe. Chaque texte traite de certains aspects de la participation continue et productive des travailleurs âgés. Les sujets sont variés et se recoupent peu. Cependant, la traduction vers l'anglais laisse souvent à désirer. Dans un certain sens, le lecteur reçoit peu d'information sur la détermination des enjeux à traiter pour conserver et augmenter la population active âgée au Canada. Le livre pourrait intéresser fortement les ergonomistes, les spécialistes en réadaptation professionnelle et les gens qui étudient ou travaillent dans le secteur de la santé et de la sécurité au travail. Chaque auteur du livre de Seltzer répond à la question: “Et si la vie durait 100 ans?” Une telle spéculation provoque la réflexion du lecteur mais ne lui donne pas de direction sur la façon deprocéder ou de mettre en oeuvre les idées présentées. C'est la nature de la tâche qu' on a exigée d'eux qui force les auteurs à laisser aux lecteurs plus de questions que de réponses. Ce sont des enjeux qui pourraient servir de point de départ au débat d'un séminaire de deuxième cycle universitaire mais leur nombre pourrait frustrer le lecteur qui cherche des principes directeurs. C'est un livre de detente qui conviendrait également aux professeurs qui souhaitent trouver un texte pour stimuler les échanges sur les enjeux reliés au vieillissement de la population.

BackgroundNearly 50% of all deaths in the UK occur in hospitals.1 The majority of these patients die in a generalist setting2 where their medical care is predominately provided by junior doctors. There is a growing recognition of a need to embed palliative care into doctors’ training.3Little evidence exists, however, concerning junior doctors’ current experiences of palliative care.AimsTo review the empirical literature between 2000 and 2018 concerning junior doctors experience of and preparation for palliative and end of life care.MethodsSystematic review and narrative synthesis of qualitative and quantitative studies within six databases to find empirical studies on junior doctors experience of adult palliative care in inpatient hospital or hospice settings.ResultsFrom the initial 6308 titles identified, 32 studies met the inclusion criteria with a further five identified from reference searching.Three key themes were identified:‘Significance of death and dying’‘Thrown into the deep end’‘Addressing the gaps’. All the studies provided evidence that junior doctors care for many dying patients very early in their career. Junior doctors do not feel adequately prepared to care for dying patients and feel unsupported when doing so. Junior doctors report emotional distress when caring for their first few dying patients, memories of which continue to affect them throughout their careers. Their attitudes towards end–of–life care varied: some reported it as a privilege while others associated it with a culture of disengagement that stigmatised dying patients.ConclusionsJunior doctors need further support, education and preparation for their exposure to palliative care. Experiential learning, reflective practice and role modelling are described as the most effective ways to learn palliative care and this also teaches them other transferrable skills such as communication, teamwork and professionalism that are vital for their future careers.References.
