Повний текст джерела[ES] El pimiento es una hortaliza de extraordinaria importancia económica y social en nuestro país. Lamentablemente, la persistente explotación del suelo, el monocultivo y la intensificación de los procesos de producción, conducen al desarrollo de enfermedades del suelo. Esto unido a los estreses abióticos, principalmente la salinidad de las aguas y del suelo, temperaturas subóptimas y estrés hídrico, puede inducir la aparición de fisiopatias en el pimiento como el Blossom-end rot (BER) y cracking o rajado, inducir senescencia vegetal y disminuir no solo la producción, sino también la calidad del producto. La salinidad y la escasez de agua son unos los mayores problemas medio ambientales a los que tienen que hacer frente los cultivos en el área Mediterránea. Un modo de sortear los estreses bajo el prisma de un manejo integrado o ecológico del cultivo, es la utilización de plantas injertadas como estrategia de adaptación. Aunque se ha producido un notable avance en esta técnica (principalmente en tomate, melón, sandía), en el cultivo del pimiento su utilización es poco frecuente aun. En esta Tesis Doctoral se han seleccionado mediante parámetros fisiológicos diferentes genotipos de pimiento tolerantes al estrés salino e hídrico. Los genotipos seleccionados fueron validados como patrones tolerantes a condiciones de estrés hídrico y salino injertados sobre una variedad comercial mediante el estudio de las respuestas fisiológicas, agronómicas y de la interacción patrón/variedad en ambas condiciones de estrés. De los resultados obtenidos se concluye que los genotipos seleccionados y utilizados como patrones mejoraron la tolerancia de las variedades comerciales a la salinidad, en términos de rendimiento (producción comercial) de frutos comparando con otros patrones comerciales y la variedad sin injertar. Diferentes mecanismos fisiológicos explican la tolerancia al estrés, como la capacidad de mantener el potencial hídrico mediante un ajuste osmótico, estimulación del sistema antioxidante, exclusión o retención de los iones tóxicos salinos (Na+ y Cl-) en las raíces y el mantenimiento de la fotosíntesis que permite mantener las funciones metabólicas de las plantas injertadas y la producción.

Açò unit als estressos abiòtics, principalment la salinitat de les aigües i del sòl, temperatures subòptimes i estrés hídric, pot induir l'aparició de fisiopaties en el pimentó com el Blossom-end rot (BER) i cracking, induir senescència vegetal i disminuir no sols la producció, sinó també la qualitat del producte. La salinitat i l'escassetat d'aigua són uns els majors problemes mitjà ambientals als que han de fer front els cultius en l'àrea Mediterrània. Una manera de sortejar els estressos davall el prisma d'un maneig integrat o ecològic del cultiu, és la utilització de plantes empeltades com a estratègia d'adaptació. Encara que s'ha produït un notable avanç en esta técnica (principalment en tomaca, meló, meló d'alger), en el cultiu del pimentó la seua utilització és poc freqüent. En esta Tesi Doctoral s'han seleccionat per mitjà de paràmetres fisiològics diferents genotips de pimentó tolerants a l'estrés salí i hídric. Els genotips seleccionats van ser validats com a patrons tolerants a condicions d'estrés hídric i salí empeltats sobre una varietat comercial per mitjà de l'estudi de les respostes fisiològiques, agronòmiques i de la interacció patrón/variedad en ambdós condicions d'estrés. Dels resultats obtinguts es conclou que els genotips seleccionats i utilitzats com a patrons van millorar la tolerància de les varietats comercials a la salinitat, tant en termes de rendiment (producció comercial) de fruits comparant amb altres patrons comercials i la varietat sense empeltar. Diferents mecanismes fisiològics expliquen la tolerància a l'estrés, com la capacitat de mantindre el potencial hídric per mitjà d'un ajust osmòtic, estimulació del sistema antioxidant, exclusió o retenció dels ions tòxics salins (Na+ i Cl-) en les arrels i el manteniment de la fotosíntesi que permet mantindre les funcions metabòliques de les plantes empeltades i la producció.

Esta familia incluye importantes agentes patógenos que afectan a humanos y a animales. Los Picornaviridae son virus pequeños, icosaédricos, de ARN de cadena sencilla de sentido positivo y causan una gran variedad de enfermedades, tales como encefalitis y poliomielitis. Se dispone de vacunas para el poliovirus, el virus de la hepatitis A y el virus de la fiebre aftosa, pero no se ha implementado ninguna profilaxis efectiva para otros picornavirus. Hasta ahora, la investigación antiviral se ha centrado en la cápside, mientras que los inhibidores dirigidos a proteínas no estructurales (como las proteasas, las helicasas y las polimerasas) están todavía por explorar. Este proyecto se centró en la caracterización estructural y bioquímica de la proteasa 3C de enterovirus B93 (EV-B93) y de los complejos de esta proteasa con varios inhibidores covalentes. El segundo objetivo fue conseguir inhibidores no covalentes de la proteasa EV-B93 3C y realizar una caracterización bioquímica, antiviral y estructural de los complejos de EV-B93 3C con estos inhibidores.

Повний текст джерелаVirtual screening plays a central role in the world of drug discovery today. In silico testing allows to screen millions of small molecules and to choose only the most promising ones for experimental testing. To find potential drug candidates, it is crucial to bring together individual and complementary computational tools. In this thesis, I describe an automated virtual screening procedure that combines pharmacophore modeling and searches, high-throughput molecular docking, consensus scoring and binding free energy estimation with the linear interaction energy (LIE) method through molecular dynamics simulations. One goal of this thesis was to build an evolving and versatile virtual screening methodology, which enables integration of different tools at different steps. The procedure that started as a combination of a simple size filter, molecular docking and consensus scoring, advanced into an elaborate and automated computational workflow with the addition of pharmacophore searches and binding free energy estimation with LIE. This integrated method intends to compensate for weaknesses of individual structure-based techniques and allows the evaluation and comparison of the performance and accuracy of these techniques. Another important goal was to apply the computational workflow to target proteins and find hits that could be drug candidates. Experimental testing performed for human acid β-Glucosidase and bleomycin hydrolase indicate that several small molecules selected by the computational workflow display micromolar inhibitory activity. The standard LIE method used in this work was applied to more than ten thousand ligand-protein complexes for three different targets, which is, to our knowledge, the first time application of LIE at such large scale.

Повний текст джерелаPritchard, Kurt Schneider y Cleckley, entre otros, han ido definiendo a lo largo de los últimos dos siglos el constructo de psicopatía, hasta que en las últimas décadas del siglo XX Hare lo operativiza en un inventario de 20 ítems denominado Psychopathy Checklist-Revised (PCL-R; 1991, 2003), que agrupa en dos Factores y después dividiría en cuatro Facetas: "Interpersonal", "Afectiva", "de Estilo de vida" y "Antisocial". Posteriormente y como versión de cribado (del PCL-R) se diseñó el Psychopathy Checklist: Screening Version (PCL: SV; Hart, Cox y Hare, 1995) con 12 ítems, un tiempo de administración mucho menor y apto para entornos forenses y clínicos. El objetivo general de esta Tesis fue el estudio de las propiedades psicométricas (fiabilidad y validez) de la adaptación española del PCL: SV (Cuquerella y Torrubia, 1996) en muestras forenses (judicial, psiquiátrica, penitenciaria y de agresores sexuales) de nuestro medio cultural. Como objetivos específicos se estudió: a) la fiabilidad de consistencia interna; b) la fiabilidad entre evaluadores; c) el error estándar de medida; d) la validez estructural, mediante análisis factoriales exploratorios y confirmatorios; e) la validez concurrente, con el estudio de la asociación con el PCL-R; f) la validez convergente, con instrumentos como las Karolinska Scales of Personality, Escalas de Sensibilidad al Castigo y de Sensibilidad a la Recompensa, el Cuestionario de Psicopatía experimental y con variables sociodemográficas, penitenciarias y delictivas; y g) la validez discriminante, con medidas de inteligencia (Test de Matrices Progresivas de Raven) y algunas variables sociodemográficas. La muestra fue de 394 participantes voluntarios, agrupados en las cuatro submuestras mencionadas. Los resultados más importantes fueron: En cuanto a la fiabilidad, la consistencia interna fue bastante aceptable y la fiabilidad Inter-evaluadores superior, incluso, a la media de resultados del Manual original (Hart et al, 1995). En relación con la validez y por lo que respecta específicamente a la validez estructural (estructura factorial), la solución que mejor explicó los resultados obtenidos en nuestra Muestra fue la tetrafactorial (análisis factoriales confirmatorios), si bien en el Manual se describía la solución bifactorial como la más parsimoniosa y con mejor bondad de ajuste (Hart et al, 1995). Los datos de validez convergente, aportaron correlaciones en el sentido esperable, destacando, entre otros: a) que escalas como Evitación de la monotonía y Agresión verbal se mostraron asociadas a mayor Puntuación Total PCL: SV); b) correlaciones del Factor 2 negativas con la escala Socialización y positivas con Sensibilidad a la Recompensa; y c) correlaciones positivas del Factor 2 y la Puntuación Total con el Cuestionario de Psicopatía experimental. En cuanto a correlatos sociodemográficos, delictivos y penitenciarios (submuestra Penitenciaria) destacaríamos la asociación entre la Puntuación total (PCL: SV) y tipologías delictivas relacionadas con "quebrantamiento de condena" y "lesiones", o con variables criminológicas como Precocidad en la primera detención, Edad del primer ingreso en prisión, Edad del primer juicio, Número de regresiones (de grado), Número de sanciones graves o Número de quebrantamientos de condena. Ratificamos pues, la similitud de resultados del presente trabajo con los del Manual, los cuales confirman una fiabilidad y una validez aceptables de la adaptación española del PCL: SV, permitiendo, pues, la aplicación en el ámbito forense y de investigación de este instrumento como herramienta de cribado de la psicopatía.
Pritchard, Kurt Schneider and Cleckley, among others, have defined over the last two centuries the psychopathy construct. More recently, in the two last decades of the 20th century, Hare operationalized it into a 20-item checklist called the Psychopathy Checklist-Revised (PCL-R; 1991, 2003), which groups them into two Factors and four Facets: "Interpersonal", "Affective", "Lifestyle" and "Antisocial." Subsequently, a screening version of the PCL-R, the Psychopathy Checklist: Screening Version (PCL: SV; Hart, Cox and Hare, 1995), was developed, including 12 items, with a much lower administration time and that is useful for forensic and clinical purposes. The general aim of this Thesis was the study of the psychometric properties (reliability and validity) of the Spanish adaptation of the PCL:SV (Cuquerella and Torrubia, 1996) in forensic samples (judicial, psychiatric, penitentiary and sexual aggressors). As specific objectives, we studied: a) internal consistency reliability; b) interrater reliability; c) the standard measurement error; d) structural validity, through exploratory and confirmatory factorial analyses; e) concurrent validity, through the study of the association with the PCL-R; f) convergent validity, with instruments such as Karolinska Scales of Personality, Scales of Sensitivity to Punishment and Sensitivity to Reward, the Experimental Psychopathy Questionnaire and sociodemographic, penitentiary and criminal variables; and g) discriminant validity with intelligence measures (Raven Progressive Matrices Test) and some sociodemographic variables. The sample was composed by 394 voluntary participants, grouped in the four above mentioned sub-samples. The most important results were: As far as reliability is concerned, internal consistency was quite acceptable and the interrater reliability superior, even the average results of the original Manual (Hart et al., 1995). Regarding the validity, and with regard to the structural validity (factor structure), the solution that best explained the results obtained in our sample was the four-factor one (confirmatory factor analysis), although the Manual described the two-factor solution as the most parsimonious and with the best goodness of fit (Hart et al., 1995). Data about convergent validity showed correlations in the expected direction, emphasizing, among others: a) that scales such as Monotony Avoidance and Verbal Aggression were shown to be associated with a higher Total Score(PCL:SV); b) negative Factor 2 correlations with the Socialization scale and positive ones with Sensitivity to Reward; and c) positive correlations of Factor 2 and Total Score (PCL:SV) with the Experimental Psychopathy Questionnaire. As regards sociodemographic, criminal and penitentiary correlates, we would highlight the association between Total Score (PCL: SV) and criminal typologies related to "conviction breaks" and "injuries", or with criminological variables such as Precocity in the First arrest, Age of first admission to prison, Age of first trial, Number of regressions (of degree), Number of serious penalties or Number of sentences breaks. It is noticeable the similarity of the results of the present study with those from the Manual, which confirm an acceptable reliability and validity of the Spanish adaptation of the PCL: SV, allowing, therefore, the application of this instrument as a screening tool for psychopathy both in forensic contexts as well as for research purposes.

Повний текст джерелаThis research aims to understand the processes of Cine Theatro Carlos Gomes, as well as to verify the role played by the genres programmed between 1971 and 2002 in the changes occured in the movie theatre. In order to draw a realistic picture of the practices and tranformations of this theatre, different research techniques were used. The first one was quantitative and consisted on gathering data on income, ticket prices, screening method, filme distribution company and movies programmed in each week of the years 1975, 1983, 1990 and 1998. This information was combined and confronted with statements collected by means of open interviews with five professionals who had relationship with the theatre: three former associates, one film critic and one representative of a film distribution company. The adopted theoretical framework was based on João Guilherme Barone Reis e Silva’s perspective of the audiovisual industry, organized in three interrelated triads; on Luiz Gonzaga Assis de Luca’s idea of film economy; and on film genre from the exhibitor’s point of view, built from the definition by Jacques Aumont and Michel Marie. In this study, it was observed that the programmed genre had influence over the entire process of the screening business, and was directly related to the main changes ocurred at Carlos Gomes. However, it is not possible to assert that there is a causal link with such transformations.

Повний текст джерелаHIV-infected patients can now enjoy a longer life expectancy with almost no AIDS defining episodes if diagnosed promptly, as is the case for most vertically infected children in our setting. The question is whether this longer life expectancy is free of comorbidities or whether other medical conditions will appear. Liver disease is a good example of this epidemiological change. A worse outcome of liver disease is expected through both the direct action of the virus and the effect of the antiretroviral drugs. Moreover, HIV itself is considered a risk factor that contributes to increased MTCT of the hepatitis virus. Furthermore, the infection rates of HBV and HCV are considerably higher in patients living with HIV, as they share most of the transmission routes. HIV-infected patients prone to uncommon variations of hepatotropic coinfections which are serologically silent, namely, occult hepatitis B infection and seronegative HCV infection. These entities bring an increased risk of disease reactivation, progression and evolution to chronic and severe liver damage, especially in children who acquire the virus early in life. Current regular screening methods may miss the diagnosis of these coinfections in immunocompromised patients. The inclusion of screening for OBI and seronegative hepatitis C in routine HIV patient monitoring would be a useful tool to identify those patients at risk of hepatic decompensation. In HIV-infected children who face the prospect of a lifetime coinfection, this possibility would be of particular interest, and highly beneficial, as it would become possible to anticipate secondary clinical conditions and future complications. This is a cross-sectional study among the Spanish cohort of HIV-infected children (CoRISPe) in order to assess the prevalence of HBV and HCV infection and occult hepatitis as well. Secondarily noninvasive liver fibrosis assessment through biochemical serum markers was made. A total of 254 patients were included in this study. Three (1.2%) and 13 (5%) patients respectively had serological markers compatible with overt cHBV and cHCV infection. Two patients showed spontaneous clearance of HCV infection. Six children were considered probable OBI cases, 2 presenting with anti-HBc alone and 4 with an anti-HBc+/anti-HBs+ serological pattern, although blood HBV DNA testing was negative in all cases. Two patients (0.8%) had seronegative HCV infection. Noninvasive assessment of liver fibrosis was pathological in all 3 with overt cHBV and in 7 out of 13 with overt cHCV, warning of a possible underlying fibrosis. One HCV-infected patient showed simultaneous alteration of the three scores included in the study. Five out of 6 probable OBI cases appeared to have altered ALT/AST ratio but APRI index and FIB-4 scores were normal in all patients with occult hepatitis. Both cases of seronegative HCV infection showed abnormal scores values. Among HIV-monoinfected children, 77% showed raised ALT/AST ratios, while 5.7% presented APRI abnormal values. Just one patient showed FIB-4 score abnormality. The HBV and HCV prevalence coinfection rates in this cohort appear to be in keeping with previously published data, in the absence of wider prevalence coinfection data for the HIV-infected pediatric population. The global rate of occult hepatitis coinfection cases detected in this cohort was low (3.1%). However, molecular diagnosis assays are indicated to increase the sensitivity of detection and minimize the risk of under-diagnosis and its clinical implications in adulthood. Further research is needed to assess the suitability of using biochemical scores in these populations in order to approach good levels of sensitivity and specificity, and to correlate this method to liver fibrosis assessed by imaging and/or biopsy.

Повний текст джерелаLa motivación principal de esta tesis ha sido validar, mejorar y desarrollar nuevos métodos con relación a los disponibles hoy en día en el área del desarrollo de fármacos, para en un futuro poder estudiar dianas que actualmente están fuera de nuestro alcance. Debido a que la productividad de la industria farmacéutica está disminuyendo durante los últimos años, una mejora en los métodos disponibles sería un gran paso adelante. Esta tesis se ha centrado en diferentes métodos computacionales, como el docking o la dinámica molecular. En la primera de las partes, trabajé en el cribado virtual (Virtual Screening) basado en docking. Concretamente, participé en la validación del programa de docking rDock mediante la comparación con dos programas muy usados hoy en día de su capacidad de predecir correctamente el modo de unión de un ligando con su proteína diana y de sus resultados en el cribado virtual de posibles fármacos. En la segunda parte de la tesis, participé en el desarrollo de un método computacional novedoso en el diseño de fármacos que complementase y mejorase los métodos actualmente disponibles. Éste método, bautizado en inglés como “Dynamic Undocking”, consiste en una implementación específica de dinámica molecular mediante la cual somos capaces de detectar si un ligando puede ser activo o inactivo de manera rápida y eficiente. Se validó el método de manera retrospectiva y posteriormente se aplicó en otro proyecto con el objetivo de encontrar nuevos posibles fármacos para una proteína relacionada con cáncer. Gracias a una colaboración con una empresa del Reino Unido, encontramos nuevos ligandos de manera que aumentamos la tasa de éxito con relación a un método estándar en casi 10 veces. Por último, participé en el “D3R Grand Challenge 2015”, un experimento a escala mundial donde los participantes aplicaron diferentes métodos y compararon sus resultados respecto a dos métricas distintas: la predicción del modo de unión y la capacidad de ordenar los ligandos proporcionados por la organización por su afinidad respecto a la proteína diana. En nuestro caso, aplicamos una combinación de docking y “Dynamic Undocking” con unos resultados excelentes.

The etiology of the above mentioned pathology is very varied but, in outline, 40 % is of feminine origin, 40 % is of masculine origin and 20 % is of mixed reason. Of between the reasons of feminine origin, the fallopian tubes pathology constitutes about 40 % of these. The hysterosalpingography is the complementary exploration more widely used nowadays for the diagnosis of the tubal patency that presents a sensibility of 93 % and a specificity of 90 % with regard to the laparoscopic findings. The transvaginal ultrasound is the test most extended to value any pathology of uterine origin and / or anexial origin. The hysterosalpingosonography is an ultrasound exploration that allows to value the uterine cavity, the tubal patency and, in addition, any pathology of ovarian origin and provided that the above mentioned test is realized by the gynaecologist, actually, it realizes two tests in one since it values the uterine cavity, the tubal patency and the ovaries saving time the patient and obtaining more information on the part of the professional. The tests have been realized radiological, hysterosalpingography, and ultrasound, hysterosalpingosonography, in a total of 105 patients and the results they have demonstrated one not inferiority of the hysterosalpingosonography respect the hysterosalpingography in the study of the internal feminine genitalia obtaining more information with the ultrasound test.

Повний текст джерелаEl aneurisma de aorta abdominal (AAA) es una patología multifactorial con una prevalencia entre el 2% y el 5,4% según la literatura, con variabilidad geográfica y con mayor incidencia en edades avanzadas o concomitancia de factores de riesgo, así como con un componente genético. Las guías clínicas indican su intervención cuando alcanzan cierto tamaño, pero son habitualmente asintomáticos hasta el momento de la rotura. Por lo tanto, se detectan de forma incidental en pruebas de imagen o en formas de cribado. Se dispone de tratamiento quirúrgico para el AAA, abierto o endovascular. Dichas intervenciones están ampliamente validadas y se practican rutinariamente en hospitales de referencia de nuestro entorno con mortalidades bajas de forma electiva, pero en caso de rotura la mortalidad aumenta drásticamente. El diagnóstico es fácil y accesible, tanto por pruebas radiológicas como por ecografía Doppler, siendo ésta última de elección en cribado y seguimiento de AAA por sus características. Por todo esto se convierte en una patología adecuada para cribado poblacional. Hay actualmente en el mundo algunos países con programa de cribado poblacional del AAA. En España, y en Catalunya, nunca se ha establecido un cribado poblacional, y siempre se ha partido del diagnóstico casual de la patología y del cribado oportunista. En la literatura queda demostrada la efectividad del cribado poblacional del AAA en la reducción de la mortalidad asociada, pero en España siempre se ha discutido su viabilidad económica y logística. Nuestra hipótesis de trabajo es que implementar un programa nacional de cribado del AAA sería viable logística y económicamente. En primer lugar, se ha realizado una revisión sistemática de los análisis económicos recientes disponibles sobre coste-efectividad de los cribados poblacionales de AAA, y se ha aplicado un análisis metodológico de la calidad de los trabajos a través de dos cuestionarios de evaluación de estudios económicos. Se ha encontrado suficiente evidencia disponible de calidad metodológica moderada-alta para poder afirmar que el cribado poblacional del AAA es coste-efectivo actualmente y lo sería en el tiempo. Sin embargo, no se dispone de datos de prevalencia fiables del AAA en España ni de estudios de cribado poblacional al respecto. Posteriormente, se ha estructurado la primera prueba piloto de cribado poblacional sistemático de AAA del país, estudiando una zona sanitaria de Barcelona. Se ha llevado a cabo en colaboración con Atención Primaria, con el programa de cribado del sistema británico de salud como referente. Se ha realizado por invitación de todos los pacientes censados que cumplían 65 años durante el año de aplicación del programa en esa zona, tanto hombres como mujeres. Se han registrado antecedentes y factores de riesgo relacionados, una ecografía Doppler abdominal de cribaje para medir la aorta infrarrenal, y se han cumplimentado cuestionarios de percepción de salud y estado anímico para valorar el impacto emocional de la prueba de cribado en los pacientes. El análisis descriptivo de los resultados ha mostrado prevalencias de los factores de riesgo similares a las descritas en la literatura de nuestro entorno, y se ha demostrado la viabilidad logística de un cribado poblacional del AAA en colaboración con Atención Primaria. Sin embargo, la prevalencia obtenida ha sido muy inferior a la esperada en hombres, y nula en mujeres. Se ha aplicado un modelo económico teórico para predecir la eficiencia del cribado poblacional en nuestro entorno en el tiempo, y éste sólo resultaría coste-efectivo en varones a 30 años vista. Estos resultados hacen poco factible la inversión inicial para comenzar un programa de cribado nacional, y nos llevan a sugerir pruebas poblacionales más amplias para corroborar estos resultados y a recomendar el cribado de subgrupos de riesgo como opción potencialmente coste-efectiva en nuestro medio.
Abdominal aortic aneurysm (AAA) is a multifactorial disease with a prevalence between 2% and 5,4% according to literature, with geographical variability and an increasing incidence with age, external risk factors and genetical factors involved. Clinical guidelines indicate intervention at a certain diameter, but AAA are usually asymptomatic until rupture happens, so diagnosis is commonly incidental in image tests or screening programmes. There is surgical AAA repair available, open or endovascular. These interventions are widely validated and are routinely performed in reference hospitals in our area with low mortality rates in elective cases, but in ruptured cases mortality rises dramatically. AAA diagnosis is easy and feasible by radiological tests and duplex ultrasound, being duplex ultrasound first-choice technique for screening and follow-up of AAA because of its features. All these factors make AAA a suitable disease for population screening. There are currently some countries around the world with a population AAA screening programme. In Spain and Catalonia there has never been a population screening programme, with AAA diagnoses coming from incidental findings and opportunistic screenings. Population screening effectiveness in reducing AAA-related mortality is widely proved in literature but in Spain there is a debate on screening economic and logistic feasibility. Our main working hypothesis is that establishing a national AAA screening programme would be economically and logistically feasible. First of all, a systematic review on recent economic analysis on cost-effectiveness of AAA population screening available was performed, and a methodological quality analysis of studies was applied through two questionnaires used to assess economic studies. We found there is enough moderate-to-good quality evidence to state that AAA population screening is cost-effective both currently and over time, but there are neither reliable AAA prevalence data nor population screening pilot programmes available from Spain. Secondly, we performed the first systematic population AAA screening programme in our country, focused on a public health area in Barcelona. It was performed in collaboration with Primary Care, with the British national health system screening programme as a reference model. All 65-years-old inhabitants in this area were invited for screening, both men and women. AAA risk factors and medical history were registered, a duplex ultrasound to measure infrarenal aorta was performed and two physical and mental health perception questionnaires were given to evaluate the screening test's emotional impact in patients. A descriptive analysis retrieved similar risk factors prevalence than those raised by current literature, and results showed an AAA population screening in collaboration with Primary Care is logistically feasible. Nevertheless, AAA prevalence found was lower than expected in men and null in women. We used an economical theoretical model to predict efficiency of AAA population screening in our area over time, and screening turned cost-effective only in men at a 30-years' time horizon. These results make initial budget investment for a national screening programme unlikely to happen, so we suggest similar pilot programmes on greater populations to confirm these findings and we recommend risk subgroup screening as a potentially cost-effective option in our area.

Повний текст джерелаThe aim of this Thesis was to explore new feed strategies to fight digestive pathogens in piglets and contribute to reduce the use of antibiotics at weaning. Firstly we explore in vitro the potential of some common macro-ingredients (wheat, corn, oats, barley, rye, soybean meal 44%, extruded full-fat soybean meal, rapeseed, fish meal, sweet whey powder and brans from wheat, spelt, kamut, rye, oat and rice) to prevent the intestinal adhesion of ETEC K88 to intestinal epithelial cells (IPEC-J2). Secondly we also explore the potential of microbial exopolysaccharides (EPS) isolated from the industrial fermentation of table green olives, to attach specifically ETEC K88 and also to block its adhesion to IPEC-J2 cells. The chemical composition and structure of this by-product is also analyzed in this Thesis and new strategies to improve the functionality of these microbial EPSs are explored testing different mates of Lactobacillus pentosus and Candida boidinii strains as starters in the fermentation of olives. In order to achieve these main objectives a set of three trails were designed. First trial showed that wheat, corn, oats, barley, rye, soybean meal 44%, extruded full-fat soybean meal, as well as sweet whey powder, all demonstrated a clear affinity to attach ETEC K88. Regarding rye, oats and also wheat, all of them could also reduce ETEC K88 adhesion to IPEC-J2 cells. Among differently tested cereal brans, all demonstrated an ability to attach ETEC K88 and also all of them, except rice bran, were able to reduce the pathogen attachment to IPEC-J2 cells. In the second trial the chemical composition of EPS is analyzed by GLC revealing that the main components are glucose and galactose followed by rhamnose and arabinose. The 1H NMR spectrum showed the presence of an α-pyranose in the form of α-D-Glcp-(1) and two different α-furanoses attributed to α-D-Glcf and α-D-Galf. Results from miniaturized tests demonstrated the ability of EPS samples to attach specifically to ETEC K88 and to effectively remove the pathogen once attached to the cells. The third trial showed how L. pentosus 119WT and specially L. pentosus 119-14MT were the most effective starters improving antiadhesive properties of EPSs. In summary results from this Thesis showed that wheat, rye, oat and their brans could be regarded as potential ingredients to prevent diarrhea in weaning piglets and that microbial EPS, obtained as by-products from the industrial production of table green olives, could be regarded as a potential in-feed additive to be used in the prevention of post-weaning diarrhea.

Повний текст джерелаAccording to the Permanent Observatory of Immigration from the year 2000 there has been an increase of immigrant population from Latin-America to different countries of the European Community and, specially, Spain, thus producing the increment of the number of cases of Chagas disease. The control of the different forms of transmission is a constant worry inside the sphere of Public International Health. In Catalonia the control of the congenital transmission of the disease has been promoted from the year 2010 through a program coordinated by the Public Health Agency of Catalonia (known as ASPCAT as per its initials in Spanish) and destined to be part of routine activities of Primary Health Care , as a strategy to approach the problem. The present study consists of two parts. The first one is an observational descriptive study consisting of an evaluation of a before-after, non experimental or reflexive model, where the same indicators before and after the intervention can be observed that is sought to evaluate. The second part of the study consists of a qualitative study of the performance of the Screening Program, obtained by means of a perception survey to professionals and to pregnant Latin-American women. It has been observed that the number of pregnant Latin-American women in control decreases by 10 % along the analyzed period. The 100 % of the units ASSIR applies the screening protocol of Chagas disease, 67 % of these units there were realized formation workshops and 83 % of the polled professionals he said to have received information and formation about the new program. The coverage of Screening Program in the set of the units ASSIRs in the year 2012 was 84 %, and 3 % in the year before the implementation. The professionals (n=106) demonstrate a good perception it brings over of the new program and they consider it to be necessary; they demand of major feedback of the information of the program. The Latin-American polled women (n=95) showed interest in the new program and they indicate positively to the aspects of “empathy” and the “capacity of response of the health care system; those women who realized complete follow-up of the pregnancy they mention a better valuation of the program (p=0,026). The surveillance and control of the vertical transmission of Chagas's disease in Catalonia, he is now in a strategic and operative way inside of the primary health care; constituting the point of item to the actions of the Screening Program, in the population reveille and the origin for the rest of actions foreseen by the program that culminate with the treatment of the sick child and his brothers if it was necessary. This new strategy of Public Health structured as program for the Primary Health Care, is the distinctive stamp of the program in Catalonia and constitutes the key to reach the universal, sustainable coverage and of high impact.

Повний текст джерелаEl cáncer de cérvix es una de las neoplasias ginecológicas más frecuentes a nivel mundial. Su origen se encuentra en la infección por el Virus del Papiloma Humano de tipo oncogénico, que se transmite principalmente por vía sexual. La implementación de programas de cribado ha comportado una drástica disminución de su incidencia y mortalidad, especialmente en los países desarrollados. En España, donde se sigue un modelo de cribado oportunista, se estima que solo el 70% de las mujeres cumplen estos programas de forma correcta. La ausencia de cribado previo, junto con una mayor práctica de conductas de riesgo, comporta que la prevalencia de cáncer de cérvix sea mayor en determinados grupos sociales, como consumidoras de drogas o trabajadoras sexuales. Una posible intervención para incrementar la participación en los programas de cribado sería el uso de métodos de auto-toma para la detección de VPH. Entre estas opciones destaca la identificación del virus en orina. La hipótesis de esta tesis planteaba que la detección de VPH en orina podría ser una alternativa a la citología para el cribado de cáncer de cérvix en mujeres con conductas de riesgo para desarrollar patología cervical y controles ginecológicos insuficientes. En este trabajo se incluyeron 95 mujeres consumidoras de tóxicos o trabajadoras del sexo comercial, sin control citológico los tres años previos. Se determinó la presencia de VPH de alto riesgo en orina y se obtuvo una muestra cervical para la realización de una citología. Las mujeres que presentaron al menos una de los dos pruebas alteradas fueron sometidas a la práctica de una colposcopia, con toma de biopsia si estaba indicada. De forma adicional se analizó la detección de VPH cervical en 59 mujeres. Finalmente, se les ofreció responder a una encuesta para valorar los conocimientos sobre VPH i la aceptabilidad del test de orina respecto a la citología. En nuestro estudio, el 30,5% de las muestras de orina resultaron positivas para VPH de alto riesgo. Los virus más detectados fueron los de alto riesgo no-16/18 (58,6%), seguidos del VPH-16 (38%). La prevalencia de SIL en la citología fue del 15,8% (9,5% L-SIL y 6,3% H-SIL). A nivel histológico se diagnosticó CIN en el 15,8% de participantes (10,5% CIN-1 y 5,3% CIN-2/CIN-3). Entre los factores epidemiológicos estudiados, la edad, la paridad, el número de parejas sexuales y el número de cigarrillos/día, se asociaron de forma estadísticamente significativa con la detección de VPH en orina. Solo el número de parejas sexuales se pudo vincular con la detección de CIN. La sensibilidad del test de VPH en orina para la detección de CIN (100%) resultó mayor que la de la citología cervical (53%). No se observaron diferencias en la especificidad de los dos métodos (82% y 91%, respectivamente). En el diagnóstico de CIN-2/CIN-3, no se detectaron divergencias entre la sensibilidad del test en orina (100%) i la de la citología (80%), pero sí en la especificidad de ambas pruebas (73% y 97%, respectivamente). Los resultados en la detección de VPH en orina y cérvix (positivo o negativo) mostraron acuerdo en el 87% de las mujeres estudiadas, con una buena correlación entre los genotipos de VPH detectados en las dos muestras (κ=0,71). El 71,4% de las mujeres encuestadas mostraron predilección por el test de orina. En estudiar el dolor, incomodidad, sensación de pérdida de control o sensación de vulnerabilidad, generada por ambas pruebas, la escala de EVA fue significativamente superior en la citología que en la recogida de orina en todas las categorías. La puntuación media obtenida en el test para valorar los conocimientos sobre VPH resultó de 9,8/21.
Cervical cancer is one of the most frequent gynaecological malignancies worldwide. Its origin is found in Human Papillomavirus (oncogenic types) infection, which is transmitted mainly through sexual activity. The implementation of screening programs has resulted in a drastic decrease in its incidence and mortality, especially in developed countries. In Spain, where opportunistic screening model is followed, it is estimated that only 70% of women conduct these programs correctly. The absence of prior screening, in conjunction with a greater practice of risk behaviours, leads to higher prevalence rates of cervical cancer in certain social groups, such as drug users or sex workers. One possible intervention to increase screening programs participation would be the use of self-taking methods for the detection of HPV. Among these options, the identification of the virus in urine stands out. The hypothesis of this thesis suggested that the detection of HPV in urine could be an alternative to cytology for cervical cancer screening in women with risk behaviours to develop cervical pathology and inappropriate gynaecological controls. In this work, 95 women drug users or commercial sex workers, without cytological control during the previous three years, were included. The presence of high-risk HPV in urine was determined and a cervical sample was obtained for cytology. Women who presented abnormal results in at least one of those two tests, were subjected to a colposcopy, with biopsy if indicated. Additionally, cervical HPV detection was analysed in 59 women. Finally, they were offered to respond to a survey to assess the knowledge about HPV and the acceptability of the urine test regarding the cytology. In our study, 30,5% of the urine samples were positive for high-risk HPV. The most frequently detected viruses were high-risk non-16/18 (58,6%), followed by HPV-16 (38%). The prevalence of SIL in cytology was 15,8% (9,5% L-SIL and 6,3% H-SIL). Histologically, CIN was diagnosed in 15,8% of participants (10,5% CIN-1 and 5,3% CIN-2 / CIN-3). Among the epidemiological factors studied, age, parity, number of sexual partners and number of cigarettes/day, were statistically significantly associated with the detection of HPV in urine. Only the number of sexual partners could be linked to the detection of CIN. The sensitivity of the urine HPV test to detect CIN (100%) was higher than that of cervical cytology (53%). No differences in the specificity of the two methods were observed (82% and 91%, respectively). In the diagnosis of CIN-2/CIN-3, no discrepancies were detected between the sensitivity of the urine test (100%) and that of cytology (80%), but it was observed in the specificity of both tests (73% and 97%, respectively). Results in the detection of HPV in urine and cervix (positive or negative) showed agreement in 87% of the women studied, with a good correlation between the HPV genotypes detected in the two samples (κ= 0.71). 71.4% of the women surveyed showed a predilection for the urine test. When asking about pain, discomfort, feeling of loss of control or vulnerability generated by both tests, the EVA scale was significantly higher in cytology than in urine collection in all categories. The mean score obtained in the test to assess knowledge about HPV was 9.8 / 21.

Повний текст джерелаINTRODUCCIÓN: El desarrollo de biomarcadores para el consumo de alcohol es un proceso largo y laborioso, donde múltiples etapas se suceden en el tiempo. Desde su descubrimiento en el laboratorio hasta su total implementación clínica, cada etapa superada incrementa la confianza y la relevancia de dicho marcador. En los últimos años se han descubierto marcadores que cuentan con una notable sensibilidad y especificidad en cuanto a la detección de consumo reciente se refiere. Sin embargo, las implicaciones clínicas, terapéuticas y económicas de dichos marcadores todavía no han sido esclarecidas con total claridad. Con la presente tesis se pretende investigar las implicaciones clínicas de dichos nuevos marcadores, específicamente del etilglucurónido (EtG), con el objetivo de determinar su verdadera aportación al campo de los trastornos por uso de alcohol. Esta tesis está compuesta de tres artículos. El primero (Estudio 1) compara el rendimiento en el cribado de consumo de alcohol del etilglucurónido frente al etanol, el juicio clínico y el autoinforme, bajo condiciones rutinarias y reales en pacientes dependientes del alcohol ambulatorios. El segundo artículo (Estudio 2) investiga la evolución clínica diferencial entre los pacientes que obtuvieron un resultado positivo y uno negativo en el Estudio 1, durante los siguientes 12 meses. Finalmente, el tercer artículo (Estudio 3) evalúa los conocimientos y la actitud que los pacientes presentan ante el cribado rutinario de alcohol. MÉTODOS: El Estudio 1 consistió en una comparación transversal cuya finalidad fue la de evaluar el uso de EtG bajo condiciones clínicas rutinarias de elevada validez externa. Para ello 613 muestras de orina consecutivas, proporcionadas por 188 pacientes ambulatorios con dependencia al alcohol, fueron analizadas para etanol y etilglucurónido. El Estudio 2 evaluó retrospectivamente la evolución clínica de los participantes del Estudio 1. Se llevó a cabo un análisis de supervivencia con el fin de comparar la tasa de recaída entre pacientes con un resultado positivo y negativo a etilglucurónido. Se realizaron análisis de regresión para comparar entre grupos el número medio de días hospitalizados, el riesgo de abandonar tratamiento y los costes medios del tratamiento. En el Estudio 3, en consonancia con los principios de la medicina centrada en el paciente, se realizó una encuesta a pacientes con dependencia del alcohol ambulatorios con la finalidad de evaluar sus actitudes, creencias y conocimientos en relación al cribado rutinario de alcohol en orina. Para la evaluación de sus actitudes se adaptó la escala Drug Attitude Inventory (DAI-10), analizándose su consistencia interna, su fiabilidad test-retest y su validez concurrente. RESULTADOS: El Estudio 1 mostró una clara superioridad del EtG sobre el etanol, el juicio clínico y el autoinforme, detectando un mayor número de positivos en condiciones reales de elevada validez externa. El Estudio 2 demostró una clara y diferente evolución clínica entre los pacientes que fueron EtG positivo y EtG negativo durante los siguientes 12 meses, presentando los pacientes EtG positivo un mayor riesgo de recaída, de hospitalización así como mayores costes de tratamiento. El Estudio 3 sugirió que el cribado rutinario de alcohol en orina es percibido por los pacientes dependientes al alcohol como un elemento valioso de su tratamiento. Se observó también que, además de cumplir una función de prevención de recaídas, otras funciones relacionadas con el vínculo terapéutico, la conveniencia social y la gestión de impresiones juegan también un papel clave. CONCLUSIONES: El cribado rutinario de alcohol mediante EtG parece tener un impacto en el manejo de los pacientes dependientes al alcohol ambulatorios, ofreciendo una mejor detección del consumo reciente de alcohol así como la posibilidad de una mejor prevención de recaídas.

Повний текст джерелаIntroducción: Se ha demostrado que el cribado mamográfico reduce la mortalidad por cáncer de mama. Siguiendo las recomendaciones de la Comisión Europea, los países europeos han establecido programas poblacionales de cribado que ofrecen mamografías bienales a mujeres de entre 50 y 69 años de edad. Sin embargo, el cribado de cáncer de mama no está libre de controversia ya que existe un debate en cuanto al equilibrio entre la reducción de la mortalidad y los efectos adversos. Para mejorar este equilibrio, la evidencia científica actual apoya el cribado personalizado. Los estudios de modelización han demostrado que modificar el intervalo de cribado, la prueba de cribado o el rango de edad de la población objetivo en función del riesgo individual de las mujeres produce un mayor beneficio que las estrategias convencionales. Por lo tanto, es necesario ampliar la información actual sobre los factores de riesgo de esta enfermedad y crear modelos de predicción del riesgo individual mediante el análisis de grandes bases de datos poblacionales. Objetivo: El objetivo general de esta tesis es profundizar en el análisis del cribado poblacional del cáncer de mama. En concreto, esta tesis pretende evaluar diferentes factores de riesgo de cáncer de mama para desarrollar y validar un modelo de predicción de riesgo individual de esta enfermedad. Se analizó cómo la densidad mamaria afecta a los distintos indicadores de cribado en el contexto de la mamografía digital. A continuación, se evaluaron las diferencias en el riesgo de cáncer de mama en función de si una lesión benigna de mama se diagnosticó en un cribado prevalente o un cribado incidente. También se analizó la interacción entre la densidad mamaria y las lesiones benignas en el riesgo de cáncer de mama. Posteriormente, se realizó una revisión sistemática para actualizar la evidencia existente, llevar a cabo una valoración crítica y una evaluación del riesgo de sesgo y resumir los resultados de los modelos de riesgo individualizados que se utilizan para estimar el riesgo de cáncer de mama en las mujeres de la población general. Por último, se diseñó un modelo de predicción individual del riesgo de cáncer de mama y se validó internamente, basado en información fácilmente accesible en un episodio de cribado. Conclusiones: i) Los distintos indicadores de cribado se ven afectados negativamente por la densidad mamaria, disminuyendo la sensibilidad y el valor predictivo positivo de la prueba a medida que aumenta la densidad mamaria. ii) El riesgo de cáncer de mama conferido por una lesión benigna difiere según el tipo de cribado (prevalente o incidente). Hasta donde sabemos, este es el primer estudio que analiza el impacto del tipo de cribado en el pronóstico de la lesión benigna. iii) El riesgo de cáncer de mama aumenta de forma independiente con la presencia de una lesión benigna y con una mayor densidad mamaria y se mantiene elevado durante más de 15 años. iv) Los modelos de predicción son herramientas prometedoras para implementar políticas de cribado basadas en el riesgo individualizado. Sin embargo, es un reto recomendar cualquiera de ellos para la personalización del cribado ya que necesitan mejorar su calidad y capacidad discriminatoria. v) Diseñamos y validamos internamente un modelo de predicción de riesgo capaz de estimar el riesgo de cáncer de mama a corto y largo plazo utilizando la información recogida de forma rutinaria en el cribado mamográfico. El modelo incluye edad, antecedentes familiares de cáncer de mama, antecedentes de lesión benigna y patrones mamográficos previos, que resultaron estar relacionados con un aumento del riesgo de cáncer de mama. El modelo debe ser validado externamente y actualizado con nuevas variables.
Background: Mammographic screening has been shown to reduce mortality from breast cancer. Following the recommendations of the European Council, European countries have started population-based screening programs that offer biennial mammograms to women aged between 50 and 69 years. The results of the effectiveness of population-based screening are controversial in terms of the balance between mortality reduction and adverse effects. To improve this balance, current evidence supports personalized screening. Modeling studies have shown that modifying the screening interval, screening modality, or age range of the target population based on women's individual risk yields a greater benefit than conventional standard strategies. Several risk models have been designed to estimate women's individual breast cancer risk based on their personal characteristics. However, most of these models have not been specifically developed to estimate the risk of women targeted for breast cancer screening. There is therefore a need to broaden current information on risk factors for breast cancer and the estimation of individual risk prediction models through the analysis of large population-based databases. Aims: The general objective of the thesis is to deepen the analysis of population-based breast cancer screening. Specifically, the aim of this thesis is to assess different breast cancer risk factors in order to develop and validate an individualized breast cancer risk prediction model. We evaluated how breast density affects screening performance indicators in a digital mammography context. Then, we assessed differences in breast cancer risk across benign breast disease diagnosed at prevalent or incident screens. To our knowledge, this is the first time that such an approach has been used. We also evaluated the interaction between breast density and benign breast disease. Subsequently, we performed a systematic review to update the existing evidence, conduct a critical appraisal and risk of bias assessment and summarize the results of the individualized risk models that are used to estimate the risk of breast cancer in women in the general population. Finally, a breast cancer risk prediction model was designed and internally validated, based on information easily accessible at screening. Conclusions: i) Performance screening measures are negatively affected by breast density, with sensitivity and positive predictive value decreasing as breast density increases. ii) The risk of breast cancer conferred by benign breast disease differed according to type of screen (prevalent or incident). To our knowledge, this is the first study to analyze the impact of screening type on the prognosis of benign breast disease. iii) The risk of breast cancer independently increased with the presence of benign breast disease and with greater breast density and remained elevated for over 15 years. iv) Individualized risk prediction models are promising tools for implementing risk-based screening policies. However, it is a challenge to recommend any of them since they need further improvement in their quality and discriminatory capacity. v) We designed and internally validated a risk prediction model able to estimate short- and long-term breast cancer risk using information routinely reported at screening participation. The model included age, family history of breast cancer, benign breast disease and previous mammographic findings, which were found to be related to an increase in breast cancer risk. The model should be externally validated and updated with new variables.

Повний текст джерелаBackground: Breast cancer-related lymphedema (BCRL) is a complication following breast cancer treatment, it is probably underdiagnosed and the prevalence varies in previous studies. Early signs can be subtle sensations of heaviness and/or tightness in limbs. BCRL can have several negative impact, this motivates the importance of early detection because early intervention improves the prognosis. Aim: Investigate the prevalence of early signs/symtoms of BCRL and investigate the association between patient reported outcomes and the physician assessment of BCRL. Method: Women who underwent curative treatment of breast cancer was included. Data was collected at the follow-up 3 months after finished treatment in purpose for a clinical project. A questionnaire was used for the patient (regarding signs/symtoms from the arm) and physician rescpectively. Results: 61 participants were included. 22 participants reported signs from the arm on the affected side. 70% of those who reported symptoms, reported a presence of ≥1 symtoms. The physicians estimated no signs in 76% of 55 participants. There was an association between the frequency of patient reported signs from the arm and the physicians assessment of armswelling (p=0,005), hand edema (p=0,003) and between patient reported symtoms and the physicians assessment of armswelling (p=0,008). When the patient reported signs/symptoms, there was a significant proportion where the physician estimated no edema/swelling. In cases when the patient reported no signs/symptoms, the physician also assessed absence of edema/swelling. Conclusion: Patients often experience signs/symptoms from the arm on the affected side, and patients reports symptoms more frequent than physicians assess presence of early signs of BCRL.

Повний текст джерелаEl segon estudi permet apreciar com prèviament al programa les mamografies no s'adequaven a les directrius europees. Un 59% de les dones entrevistades dels barris El Clot, Camp de l'Arpa i La Verneda, s'han fet una mamografia de cribratge en els últims quatre anys. Existeix una major utilització en dones més joves, de major nivell educatiu i que tenen historial clínic. La periodicitat era anyal en un 35%, biennal en un 38% i cada 2-4 anys en el 27% restant. El 58% de les mamografies s'han realitzat en la sanitat pública, que mostra el major percentatge (42%) de periodicitat biennal. En les dones participants, la visita al ginecòleg i la pràctica de citologia uterina han presentat la major associació (probabilitats 4 vegades superiors) amb la mamografia prèvia de cribratge. També s'han associat a una major utilització els antecedents personals de patologia mamària i familiars de càncer de mama, l'autoexploració mamària i la visita mèdica recent, mentre que l'autopercepció de salut ha presentat una relació inversa.
Finalment, el tercer estudi és un assaig controlat aleatoritzat on s'han comparat tres diferents estratègies utilitzades per a invitar dones d'entre 50 a 64 anys a participar en el programa. L'estratègia del contacte directe incrementa de forma significativa fins un 22% la probabilitat de resposta després de la primera citació en les dones del barri de Raval Nord invitades. En el grup de contacte directe aquesta resposta era del 63%, comparat amb un 55% quan la carta era enviada pels professionals sanitaris del CAP, i un 52% quan era remesa pels responsables del programa. Aquest increment en el grup de contacte directe s'observa encara que sols s'ha aconseguit contactar en un 45% dels casos directament amb la mateixa dona. Quan aquest fet s'assoleix la resposta és del 72%. La utilització de professionals no sanitaris per a aconsellar les dones sembla ser una estratègia efectiva, particularment entre les dones de baix nivell educatiu.
The beginning of the breast cancer screening program in the Ciutat Vella and Sant Martí districts of Barcelona was the origin of the first study in the Barceloneta quarter, with the aim of analyzing different factors related to participation. The response after the first citation was 65%. Having a previous clinical history, increased in nearly 4 times the response odds. Response after the first citation was influenced by who the recipient of the citation letter was, ranging from 75% when this was the same person concerned to 47% when the letter was left in the letter-box, and to 32% when the address was uncertain. The response after subsequent citations was 26%. Having previous clinical history increased the response odds in almost 8 times. The response after successive citations, ranged from 50% when it was for circumstantial reasons, to 11 % when it was due to lack of interest. Coverage rate (79%) decreased with age, and increased with education level.
The second study shows that, previously to the program, mammography screening did not comply with European guidelines. A screening mammography had been performed on 59 % of women interviewed in the El Clot, Camp de l'Arpa and La Verneda quarters. The utilization of mammography was higher among younger women, women with a higher education level, or who had previous visits to a physician. Periodicity was once a year in 35% of cases, once every two years in 38%, and every 2-4 years in the remaining 27 %. Mammographies were performed at the Public Heath Service in 58 % of cases. The Public Health Service shows the highest percentage (42 %) of biennial periodicity. The variables more strongly associated with the use of screening mammography (probabilities 4 times higher) were: visit to a gynecologist and performance of a pap smear. Also related to the utilization of screening mammography were: personal history of breast pathology, family history of breast cancer, breast self-examination, and recent visit to a general practitioner. Self-perceived health presented an inverse relation with screening mammography use.
Finally, the third study is a randomized controlled trial where three different strategies of inviting 50 to 64 years old women to participate in the program were compared. The strategy of direct contact increased in a significant way, up to 22%, the possibility of response after the first citation among women invited in the Raval Nord quarter. The response rate in the direct contact group was 62%, compared to 55% when the letter was sent by Primary Health Care Team, and 52% when it was sent by those responsible of the program. This increase in the direct contact group is noticeable even though only in 45 % of cases it was possible to contact the subject herself. When this was achieved, response raised to 72%. Use of non-health professionals to advise women seems to be an effective strategy, especially among women with a lower educational level.

Existe controversia respecto al protocolo de cribaje y tratamiento con un aumento de las resistencias a los antimicrobianos en enterobacterias, así como cada vez más, reacciones alérgicas a los antibióticos. Se cuestionan los estudios en los que se basan las guías actuales seguidas en nuestro país y la dudosa efectividad del cribaje. Pese a todas las medidas preventivas y de tratamiento, continúan registrándose infecciones neonatales por este microorganismo. Hipótesis: Existe variabilidad en resultado del cultivo para SGB entre la semana 35-37 de gestación y el día de parto. Objetivo: Investigar la variabilidad del resultado de los cultivos para SGB vaginal y rectal entre el tercer trimestre de gestación (35-37w) y el parto en gestantes del Hospital Universitari Germans Trias i Pujol de Badalona. Metodología: Estudio observacional, descriptivo, transversal de serie de casos en 304 gestantes. Técnica de muestreo no probabilística accidental y un análisis descriptivo e inferencial. Resultados: La variabilidad de resultados se dió en 22 muestras (7,2%). Un 4% (9) cambió de positivo a negativo y un 26% (14) de negativo a positivo. La edad y el peso influyeron en el cambio de resultado. Un antecedente de positivo en anteriores gestaciones (p:0,021) o una infección de transmisión sexual (riesgo x3,8) se relacionó con un cambio de resultado. Ni la paridad, la etnia, la raza, la antigüedad de los cultivos o diabetes gestacional/pregestacional influyeron en un cambio de resultado. La prevalencia de SGB fue de entre el 15,1% y el 15,6 %. El porcentaje de cambios de resultado entre los positivos del primer cultivo fue 7 veces superior que entre los negativos y el porcentaje de cambios de resultado entre los positivos en el segundo cultivo cuadruplicó el porcentaje de cambios entre los negativos. Uno de cada cuatro cultivos llegó al parto con una antigüedad superior a las 5 semanas, dato que no influyó en la modificación de su resultado. En referencia a la asociación entre SGB y las variables estudiadas, se halló una correlación con la paridad (a mayor paridad mayor número de SGB positivos). La diabetes (más del doble entre los resultados positivos que entre los negativos). El sobrepeso (riesgo x 2,7 veces). Generalmente, las mujeres con resultado positivo fueron más obesas. El antecedente de positividad en una gestación anterior (riesgo y odds ratio x4). o de infección de transmisión sexual (riesgo y odds ratio x3,8)., también resultaron factores de aumento de riesgo para la positividad. Sin embargo, el riesgo de ser positiva se redujo un 67% en las mujeres de raza blanca con respecto a mujeres de otras razas (indo-pakistaní, negra y oriental). Conclusiones: La prevalencia de gestantes con Streptococcus agalactiae vaginal y/o rectal se halló entre el 15,1-15,6%. En un 7% de las muestras, existió una variabilidad entre el resultado del cultivo para Streptococcus agalactiae vaginal y rectal del tercer trimestre al parto. Este hecho debe considerarse un dato clínicamente relevante para la práctica asistencial. Se crearon dos modelos predictivos de regresión logística (LOGIT), modelos probabilísticos en los que se detectarán los factores de riesgo entre los parámetros contemplados, y con el que se podrá calcular la probabilidad de ser positivo y de modificar el cultivo, pero no se podrá predecir ni clasificar. Este estudio observó que, el cribaje en el tercer trimestre de gestación para SGB, se podría revalorar y/o individualizar en función del beneficio aportado a la gestante, dada la naturaleza cambiante y prácticamente impredecible de esta bacteria, atendiendo a la controversia de los datos obtenidos y aceptando el potencial riesgo de efectos adversos versus el potencial beneficio para el binomio madre-neonato.
Background: Streptococcus agalactiae (GBS) is the leading cause of vertically transmitted perinatal bacterial infection in the western world. There is controversy regarding the screening and treatment protocol with an increase in antimicrobial resistance in Enterobacteriaceae, as well as, increasingly, allergic reactions to antibiotics. The studies on which the current guidelines followed in our country are based and the doubtful effectiveness of the screening are questioned. Despite all preventive and treatment measures, neonatal infections with this microorganism continue to be recorded. Hypothesis: There is variability in the result of the culture for GBS between the 35-37 week of gestation and the day of delivery. Objective: To investigate the variability of the result of the cultures for vaginal and rectal GBS between the third trimester of gestation (35-37w) and delivery in pregnant women at the Germans Trias i Pujol University Hospital in Badalona. Methodology: Observational, descriptive, cross-sectional study of a series of cases in 304 pregnant women. Accidental non-probability sampling technique and a descriptive and inferential analysis. Results: The variability of results occurred in 22 samples (7.2%). 4% (9) changed from positive to negative and 26% (14) from negative to positive. Age and weight influenced the change in outcome. A history of positive in previous pregnancies (p: 0.021) or a sexually transmitted infection (risk x3.8) was related to a change in outcome. Neither parity, ethnicity, race, age of crops, or gestational / pregestational diabetes influenced a change in outcome. The prevalence of GBS was between 15.1% and 15.6%. The percentage of result changes among the positives of the first culture was 7 times higher than that of the negatives and the percentage of result changes among the positives in the second culture quadrupled the percentage of changes among the negatives. One out of every four cultures arrived at delivery older than 5 weeks, a fact that did not influence the modification of their result. Regarding the association between GBS and the variables studied, a correlation with parity was found (the higher the parity, the greater the number of positive GBS). Diabetes (more than double the positive results than the negative ones). Being overweight (risk x 2.7 times). Generally, women with a positive result were more obese. A history of positivity in a previous pregnancy and a sexually transmitted infection also resulted in increased risk factors for positivity. However, the risk of being positive was reduced 67% in white women compared to women of other races (Indo-Pakistani, Black and Oriental). Conclusions: The prevalence of pregnant women with positive vaginal and / or rectal Streptococcus agalactiae was found between 15.1-15.6%. In 7% of the samples, there was a variability between the culture results for vaginal and rectal Streptococcus agalactiae from the third trimester to delivery. This fact should be considered clinically relevant data for healthcare practice. Two predictive logistic regression models (LOGIT) were created, probabilistic models in which the risk factors will be detected among the parameters contemplated, and with which the probability of being positive and of modifying the culture can be calculated, but it will not be possible to predict or classify. This study observed that screening in the third trimester of pregnancy for GBS could be reassessed and / or individualized based on the benefit provided to the pregnant woman, given the changing and practically unpredictable nature of this bacterium, taking into account the controversy of the data. obtained and accepting the potential risk of adverse effects versus the potential benefit for the mother-neonate binomial.

Повний текст джерелаThe objective of this doctoral thesis is to develop the knowledge on optimization in colon neoplasias detection strategies. The findings of the studies offer an important contribution to the knowledge in the field of the colorectal cancer screening. The thesis consists of two separate projects. The first of them is focused on the fecal immunochemical test (FIT) and its objective is to evaluate its performance at different positivity cut-off values in a population-based colorectal cancer screening program, stratified by age and sex. This study shows that FIT cut-off values could be individualized by age and gender to improve the performance of the test in CRC screening programs. The study gives useful information to optimize resources and to plan a screening program. The second project is focused on colonoscopy, and it analyses the prevalence of serrated polyps and its relationship with synchronous advanced neoplasia. This study shows that large serrated polyps have low prevalence in average-risk individuals and, independently of their proximal or distal location, they are associated with the presence of synchronous advanced neoplasia. Advanced neoplasm associated risk is of the same magnitude to the risk of having three or more small tubular adenomas. This reinforces the recommendation of surveillance after three years. Furthermore, proximal hyperplastic polyps are related to the presence of synchronous advanced neoplasia, although further studies are needed to determine definitely the risk of patients with proximal hyperplastic polyps. These results emphasize the importance of endoscopic detection of these lesions with high quality screening examination. Both projects are based on the COLONPREV study, a population-based, multicenter, nationwide, randomized controlled trial designed to compare the efficacy of one-time colonoscopy and biennial FIT in reducing CRC-related mortality. The strength of both projects is that they are based on a recent, large, nationwide, controlled randomized and multicenter trial, which ensures the reliability of the data. On the other hand, the projects have some limitations as well. In the first project, results were analysed based on one FIT screening round, which does not reflect real world practice, in which people urdergo repeated testing. Prospective studies conducted over consecutive rounds are needed to determine if results are altered. Another limitation is that the study was performed in one country only, and that other factors such as smoking status, or body mass index were not evaluated and could have an influence on the results. Finally, test sensitivity and specificity could not be determined because only FIT positive individuals underwent colonoscopy. The second project also has some limitations, the most important being that pathologic criteria for the diagnosis of serrated polyps were not centrally reviewed. Interobserver variation among pathologists could have therefore influenced the results. Other limitations are that the relationship between each type of non-hyperplastic serrated polyp could not be determined, and that the relationship between large serrated polyps and CRC could not be established due to the few cases of diagnosed CRC. Both projects have been published in indexed journals with a total impact factor of 8.332, and this thesis is presented as compendium of both publications. Finally, this thesis opens the door to future research projects, both in the area of average-risk individuals that undergo colorectal cancer screening and in the surveillance of CRC screening diagnosed lesions.

Identification of novel autoantibodies in these conditions might provide new biomarkers with relevant clinical implications in the diagnosis, prognosis and treatment of these patients. CIDP is a heterogeneous condition featuring a lack of clinically-useful biomarkers, in which the description of specific autoantibodies would benefit patients’ diagnosis and prognosis, as has been previously demonstrated with the discovery of antibodies against paranodal proteins. In this study, a group of 65 CIDP patients were screened for IgG and IgM reactivity against previously described antigens as well as for new reactivities towards dorsal root ganglia neurons, motor neurons and Schwann cells. A subset of seronegative CIDP patients reacted against neural structures and gangliosides. However, immunoprecipitation experiments in patients reacting against neural cells did not reveal novel antigens, and frequencies of reactivity against neural cells did not differ from normal controls. Conclusion: Although immunoprecipitation experiments failed to identify novel antigens, we provide the most comprehensive autoantibody screening study performed to date in CIDP and demonstrate a heterogeneous autoantibody profile in CIDP patients, reinforcing the hypothesis of a significant pathophysiological heterogeneity in this disease. Paraneoplastic neuropathies are rare immune-mediated neurological conditions in which description of novel specific autoantibodies might benefit patients’ diagnosis and prognosis. Two of our CIDP patients with antibodies against Contactin 1 (CNTN1) developed cancer in addition to their neuropathy. Since CNTN1, as well as other cell adhesion molecules have been described to contribute to cancer pathogenesis, we assessed their possible involvement as antigens in patients with paraneoplastic neuropathies. In this study, a group of 34 patients with paraneoplastic neuropathies were screened for reactivity against neural cell adhesion molecules as well as for new IgG and IgM reactivities towards relevant neural structures. Overall, nine (26.5%) patients showed significant reactivity against dorsal root ganglia neurons, motor neurons or Schwann cells. Compared with control sera, serum samples from patients with paraneoplastic sensory motor neuropathies had a higher frequency of IgM antibodies against Schwann cells. Conclusion: Although our experiments failed to identify novel antigens in paraneoplastic neuropathies, we provide a comprehensive autoantibody screening study that not only demonstrates the heterogeneous autoantibody profile in these patients but also proves that a remarkable proportion of them target dorsal root ganglia neurons, motor neurons or Schwann cells.

Повний текст джерелаL’objectiu general d’aquesta tesi és estudiar el cribratge de càncer de mama i en concret les desigualtats socials per classe social i país d’origen, així com la seva relació amb la disminució de la mortalitat. En conseqüència, s’han realitzat quatre estudis diferents: tres de metodologia quantitativa i un de qualitativa. Segons l’estudi quasi-experimental, la mortalitat per càncer de mama a Barcelona disminueix des d’abans de la introducció del programa poblacional de cribatge, però aquesta reducció és més accentuada desprès de la seva introducció. En base a l’Enquesta Nacional de Salut de l’Estat Espanyol de l’any 2006, existeixen desigualtats en la realització de mamografies periòdiques segons país d’origen i classe social. Segons els dos estudis realitzats a Barcelona, les dones immigrades procedents de països de renda baixa coneixen i realitzen menys les pràctiques de detecció precoç, ja que tenen altres prioritats i perceben més barreres i tabús. Les dones xineses són les que presenten més diferències amb les dones autòctones, seguides de les magribines i les filipines. El lloc d’origen, la classe social i el procés migratori són factors claus en les practiques preventives. En conclusió, és necessari afavorir l’accés a les pràctiques preventives a totes les dones i també realitzar accions específiques dirigides als grups més vulnerables sense deixar de tenir en compte els factors socioculturals que influeixen en les pràctiques preventives de les dones.

La gran expansión económica de las últimas décadas en los países desarrollados a contribuido al incremento del consumo de alimentos poco saludables y al uso excesivo de tecnologías de ahorro de energía. Estos cambios han generado estilos de vida poco saludables y el consiguiente aumento de la prevalencia de obesidad. Así pues, la obesidad surge como una respuesta natural a un entorno antinatural. Con el aumento continuo de la población obesa en cada generación, la prevalencia de trastornos asociados a la obesidad como la diabetes tipo II y la artrosis también aumenta, y la perspectiva de desarrollar una terapia médica específica para cada paciente va ganando interés. En este sentido, las dianas proteína tirosina fosfatasa 1B (PTP1B) y la metaloproteasa de la matriz 13 (MMP-13) están implicadas tanto en la obesidad como, respectivamente, la diabetes mellitus de tipo II y la artrosis. La presente tesis doctoral se centra en el desarrollo de estrategias de cribado virtual para identificar compuestos que modulen la actividad de estas dos dianas y puedan influir positivamente en la obesidad y los trastornos asociados a la obesidad.
Obesity is one of the major public health problems in the 21st century. The great economic expansion of the last decades in developed countries has contributed to the increased consumption of unhealthy foods and the excessive usage of energy-saving technologies. These have in turn led to the development of unhealthy lifestyles and the consequent increase of obesity prevalence. Thus, obesity has emerged as a natural response to an unnatural environment. With the continuous increase in obese population in each generation, the prevalence of obesity-associated disorders such as type II diabetes and osteoarthritis is also on the rise, and the prospect of developing a medical therapy specific for each patient earns increasing interest. In this regard, the targets protein tyrosine phosphatase 1B (PTP1B) and matrix metalloproteinase 13 (MMP-13) are involved in both obesity and, respectively, type II diabetes mellitus and osteoarthritis. The present doctoral thesis focuses on developing virtual screening strategies to identify compounds that modulate the activity of these two targets which may have a positive influence on both obesity and its associated disorders.

Повний текст джерелаA new strategy based on a biomechanical screening has been developed and it was applied to the study of idiopathic pulmonary fibrosis (IPF) to identify mechanosensitive genes that might be potential therapeutic targets for the disease. The transcriptional profile of lung fibroblasts (control and fibrotic of different types) cultured on substrates of different stiffness spanning the normal to fibrotic (stiffened) range was obtained. The mechanotranscriptome of the cells was established, which consisted of 63 mechanosensitive genes, 57% of which were upregulated by tissue stiffening. It was built an interactome, showing the regulatory networks associated with the mechanotranscriptome, revealing three different subnetworks: 1) elements of the cytoskeleton and stress fibers; 2) MAPK quinases and their phosphatases; 3) proteins associated with insulin-like growth factor and the fibrinolysis pathway. After validating expression at the mRNA and protein levels for different selected candidates from the mechanotranscriptome, different biological aspects of insulin-like growth factor binding protein 3 (IGFBP-3) were analyzed. Concentration of IGFBP-3 secreted by fibroblasts was distinctively higher in IPF fibroblasts. With substrate stiffening, a mechanosensitivity factor of 3X was detected in all classes of lung fibroblasts, while transforming growth factor- β (TGF- β) induced protein levels by 150X. The effect of tissue stiffening on IGFBP-3 accumulation was independent of signaling through the TGF-β receptor and dependent of the collagen receptor integrin β1. These results show that IGFBP-3 is an important protein for lung fibrogenesis mediated through stiffness and that it could lead to an accumulation of extracellular matrix and contribute to the destruction of the lung architecture.

Повний текст джерелаRegular screening for cervical cancer has proven to reduce the incidence and the mortality from this disease. The objective of this study was to improve the participation and the coverage of the screening for cervical cancer in women from 30 to 70 years, to evaluate the impact in the coverage and to know the characteristics (place and periodicity) of it, after the women that in our database had no screening or were poorly screened received one personalized invitation, of three different kinds. Methods The CRICERVA study is a cluster clinical trial targeted to women from 30 to 70 years of age that were registered, in December 2010, in the Primary Health Care Service (SAP) Cerdanyola area in Barcelona. A total of 32,858 women were registered and had an assigned doctor in one of the ABS (Public Health Care) within the study area (checked from the Register on December 2010). After this, the screening registers were extracted from the clinical history of each woman, and all the women with no record of cervical cytology during the previous 3.5 years were identified, making a total of 15,965 women. These women were randomly assigned to one of four different clusters. The first one (GI1) consisted of a personalized invitation letter to participate in the screening program. The women in the second cluster (GI2) received the invitation plus an informative leaflet. The third cluster (GI3) also received a personalized phone call. Finally, the cluster with no intervention (GNI) was used as a control group, and was based on spontaneous demand. From the 15,965 women, 13,886 were assigned to one of the Intervention Groups (GI) and 2,079 to the No Intervention Group (GNI). Out of these 13,886, it was not possible to contact with 4,263 women, and 4,263 didn’t need screening. The remaining 5,341 were rescued with an insufficient screening. The protocol about the cervical cancer screening of the Departament de Salut de la Generalitat de Catalunya was applied to these final women, and it was complemented with an HPV test if indicated. An analysis of costs of intervention versus increase of coverage was made. A personalized interview offered to all women that participated in the Intervention Groups during the visit, and also to women in the No Intervention Group, but after the study was complete. The last screening visit was performed in December 2013. Statistical Analysis The coverage in every group was estimated as the proportion of women that were correctly screened at the end over all the women susceptible of being screened. It is considered as final coverage the one reached in the total period of 4 years. The qualitative variables are described with absolute frequency and percent value. The quantitative variables, however, are expressed with averages and standard deviations. The Chi Squared test was used in the comparison of qualitative variables, and the Analysis of Variance for the analysis of quantitative variables. The comparison between groups was performed by Chi Squared Distributions, and it was considered that a difference was statistically significant when the p-Value was lower than 0.05. The Confidence Intervals are of the 95%. The analyses were performed with the software Statistical SPSS for Windows, v.20.0. Results From all women initially registered with a poor screening, a 20.6% went to Private Health Care regularly, and a 10.2% did not need active following. The cervical screening coverage for the Cerdanyola SAP after the CRICERVA study in the intervention groups was estimated to be at 84%, which represents an increase of the 31% from the baseline coverage. The highest coverage was registered within women with less than 40 years (95.2%) and the lowest in women older than 60 years (64.3%). The strategy that obtained the highest coverage was the GI3 group for all ages. In the analysis by age it was also observed that the coverage before and after the study were higher with women younger than 40 years. The coverage of the control group (GNI) increased in a 9% during the study. The analyses of the personalized interviews show that approximately a 30% of women with screening records went to Private Health Care. The women with the highest educative and economic level are those who mostly go to Private Health Care. The lack of information about the importance of the detection was the most important factor for not following the screening recommendation. The study identified in 2,500 women with criteria of insufficient screening: 18 CIN2, 10 CIN3 and 4 cases of invasive cervical cancer. The economic evaluation of the interventions estimated that the implementation of sending a letter would cost an average of 490€ per thousand women. Conclusions * The CRICERVA study has estimated the population coverage of cervical cancer screening in the Cerdanyola SAP with a mixt approach, which includes the analysis of the electronic clinical history, personalized interviews and a gynecological visit in the frame of a randomized test of intervention. * The analysis of the clinical history and the personalized interviews show that a 34% of the women of the Cerdanyola SAP had a poor screening compared to the Departament de Salut recommendations. * The intervention strategy that obtained the highest coverage included a personalized invitation letter, an informative leaflet and a phone call three days before the visit. * In a population like Catalonia’s, with approximately 40% of women between 30 and 70 years with poor screening, the implementation of the first intervention strategy (sending a letter) would cost 490€ per thousand women. * The CRICERVA Study results are informative; to ease the implementation of efficient strategies that allow a transition from opportunistic screening to planned population strategies. * The clinical history analysis performed in the CRICERVA Study has allowed eliminating a considerable proportion of errors in the register, which would point to the necessity to regularly check the validaty of the clinical electronic medical records to improve the quality of these registering systems.

El principal objetivo global de las dos partes del estudio fue definir y crear una ruta precisa para la detección, el diagnóstico y el tratamiento de la FA y, por lo tanto, evitar los efectos adversos obvios derivados de dicha patología. Como resultado principal de este proyecto, hemos establecido que un programa sanitario de búsqueda de casos, es una herramienta que nos ayuda aumentar a prevalencia e incidencia de FA en nuestra población y a la vez, hemos podido identificar barreras en la búsqueda de pacientes con FA. En otra parte del estudio, se ha efectuado una investigación sobre diferentes registros clínicos, biológicos y electrocardiográficos para así identificar más casos de FA. Hemos identificado que el marcador NT-proBNP podría ayudarnos en el cribado de FA. Con todos estos datos se ha establecido una ruta más precisa para el diagnóstico de la FA
The study, divided into two parts, has the common objective of establishing a concise route for atrial fibrillation (AF). The main overall objective of the two parts of the study was to define and create a precise route for the detection, diagnosis, and treatment of AF and, therefore, to avoid the obvious adverse effects derived from this pathology. As the main result of this project, we have established that a health case search program is a tool that helps us increase the prevalence and incidence of AF in our population and, at the same time, we have been able to identify barriers in the search for patients with AF. In another part of the study, research has been carried out on different clinical, biological and electrocardiographic records in order to identify more cases of AF. We have identified that the NT-proBNP marker could help us in screening for AF. With all these data, a more accurate route has been established for the diagnosis of AF

Повний текст джерелаFibromyalgia is a common syndrome characterized by widespread chronic pain along with other accompanying symptoms such as fatigue, emotional and sleep disorders, cognitive dysfunction and a remarkable increase of disability and interference in psychosocial domains. In spite of the significant advances in the scientific knowledge of this syndrome attained in recent years, both patients and the diagnosis of fibromyalgia itself endure a discrediting that may worsen patients' suffering and hamper their relations with the health system and social services. In our opinion, this discrediting may be related, among others, to doubts about the legitimacy of the nosologic status of fibromyalgia, suspicions that symptoms are exaggerated or simulated, and explanations of the onset and maintenance of the syndrome based on presumed specific characteristics of personality that foster an abnormal impact of contextual demands and a lower coping ability. Therefore, the studies of this thesis are aimed at assessing: a) if the clinical presentation of the fibromyalgia syndrome is different enough from that of other chronic pain disorders, thus suggesting that fibromyalgia is better understood as a discrete clinical entity; b) if the psychopathological profile observed in fibromyalgia patients allows differentiating them from cases of simulation; and c) if fibromyalgia patients are characterized by a specific personality profile, if personality profiles allow defining subgroups of patients, and if every subgroup shows a different therapeutic response to multidisciplinary treatment. Our results suggest that both in Spanish and Latin American samples, fibromyalgia is distinguishable from other chronic pain disorders on the basis of somatic, depressive and anxiety symptoms, catastrophizing and functional disability. Likewise, fibromyalgia is reliably distinguishable from simulation both if patients have initiated a legal process aimed at obtaining compensation for disability or not. Finally, fibromyalgia patients did not show a specific personality profile once controlled the effect of confusion variables such as psychopathology. The personality assessment allowed, however, the definition of two subgroups of patients showing different therapeutic responses.

Повний текст джерелаINTRODUCTION: Colorectal cancer (CRC) is an important health problem all over the world. In industrialized countries, CRC is the second most common malignancy in men and the third most common in women. Screening for CRC can identify the disease at an earlier stage or at a premalignant phase. The latest generation of fecal immunochemical tests (FIT) allows for quantitation of hemoglobin in feces (f-Hb), allowing for selection of optimal cut-off concentrations. Furthermore the utility of fecal immunochemical tests (FIT) in assessment of symptomatic patients with lower gastrointestinal symptoms has not been well explored. AIMS: We investigated, in a screening setting, whether individuals with positive results from quantitative FITs, in combination with other factors, could be identified as being at greatest risk for advanced colorectal neoplasia (ACRN). We also evaluated the diagnostic yield for ACRN in symptomatic patients using the first of two FIT samples (FIT/1) and the higher concentration of two FIT samples (FIT/max). RESULTS AND CONCLUSIONS: Median fecal hemoglobin concentrations were significantly higher in participants with ACRN compared with participants with non ACRN. Positive predictive values for ACRN, determined using arbitrary fecal hemoglobin concentrations, differed with sex and age. Multivariate logistic regression analysis identified male gender, older age (60–69 y) and fecal hemoglobin concentration as independent predictive factors for ACRN. Combining these factors, we identified 16 risk categories associated with different probabilities of identifying ACRN. Regarding to the symptomatic cohort, colonoscopy detected ACRN in 29 patients. In these patients, FIT/1 and FIT/max were significantly higher than in patients with low-risk adenoma. We observed differences between gender, obtaining the highest sensitivity and the highest f-Hb concentration in men when compared with women. Moreover, the diagnostic yield of collecting two samples for FIT can be achieved with one sample, albeit using a lower f-Hb cut-off.

Повний текст джерелаOur research group collaborated in HIDES I study [1,2] and in 2012 we conducted a retrospective study comparing the rate of HIV test requests guided by HIV Indicator conditions (IC) according to their standard of care (2008) and after formative sessions. In 2008 the number of HIV tests in patients with one of the selected IC was 3.9%, and rose to 11.8% in 2012 (P<.0001) [3]. Subsequently, we conducted an interventional study [4] in 2015 to assess the impact on the proportion of HIV tests requested after the introduction of an electronic prompt instructing primary healthcare physicians to request an HIV test when diagnosing predefined IC. Patients aged 18-65 years without HIV infection and with a new diagnosis of the evaluated ICs were included. The results were compared with pre-prompt (2012) and post-prompt data (2016). During the period of the prompt, 832 patients presented one of the studied IC. HIV tests were requested in 296 individuals and blood tests made in 238. The number of HIV tests requested based on evaluated IC increased from 12.6% in 2012 to 35.6% in 2015 (p<0.001) and fell to 17.9% after removal of the prompt in 2016 (p<0.001). The most important predictor for HIV test request was the electronic prompt (OR 3.36, 95%CI 2.70-4.18). For the first time, an electronic prompt was used to systematize the screening of HIV- infected patients using indicator conditions in primary healthcare (PHC) centers. Implementation of the electronic prompt led to a significant increase in requests for HIV tests compared with the pre-prompt period and there was a significant decrease after the prompt was removed. Our results suggest that this strategy could facilitate HIV screening to general practitioners. These results should encourage future research on ensuring the accuracy and effectiveness of electronic clinical decision (CDS) system alerts that maximize IC-guided HIV testing in PHC centers. 1. Повний текст джерелаIntroducción: El envejecimiento está asociado a cambios cognitivos globales, que en ocasiones progresan al deterioro cognitivo leve o a la demencia, siendo la enfermedad de Alzheimer la más prevalente. Es fundamental hacer una detección precoz de los cambios cognitivos asociados al envejecimiento para discriminar alteraciones que puedan hacer sospechar el inicio de una demencia con el fin de ralentizar el proceso neurodegenerativo. Las pruebas de screening cognitivo son herramientas útiles y rápidas con este fin, pero tienen que ser más sensibles y específicas para orientar correctamente el diagnóstico. Progresivamente hay que ir haciendo el seguimiento del rendimiento cognitivo para identificar posibles involuciones. El efecto de la práctica y el aprendizaje de las pruebas neuropsicológicas utilizadas habitualmente dificulta poder hacer un seguimiento adecuado del deterioro cognitivo. Objetivo: Estudiar la contribución de diferentes pruebas neuropsicológicas en la detección de los cambios involutivos del proceso de envejecimiento, así como en el estudio de la evolución a nivel clínico del deterioro cognitivo leve y la enfermedad de Alzheimer. Método: Se han diseñado tres estudios que se fundamentan en la detección precoz y el seguimiento evolutivo del deterioro cognitivo. En el primer estudio se han comparado tres pruebas de screening (MMSE, MoCA y PD-CRS). Los participantes han sido 126 personas de entre 42 y 89 años de diferentes Servicios de Neurología que se han dividido en cuatro grupos diagnósticos. En el segundo estudio han participado 250 personas cognitivamente sanas de entre 40 y 89 años. Se ha analizado la evolución de la atención y la velocidad de procesamiento a lo largo del envejecimiento y se ha estandarizado la versión más actual del MoCA. En el tercer estudio se han creado, validado y normalizado nuevas pruebas de evaluación neuropsicológica contando con la participación de 250 personas cognitivamente sanas de edades comprendidas entre 40 y 89 años. Resultados: Del primer estudio se extrae que la PD-CRS es la herramienta con mayor sensibilidad y especificidad para detectar el DCL y la enfermedad de Alzheimer. En el segundo estudio se ha observado que la reserva cognitiva tiene un claro efecto sobre el rendimiento en la capacidad atencional y la velocidad de procesamiento. Así mismo, se ha obtenido una buena fiabilidad en la muestra en el MoCA 8.1 con una interferencia de la edad y del nivel educativo sobre los resultados. Las nuevas pruebas de evaluación cognitiva creadas, validadas y normalizadas en el tercer estudio ofrecen resultados consistentes como alternativas clínicas al uso de los instrumentos de evaluación neuropsicológica más habituales en el diagnóstico y seguimiento del deterioro cognitivo. Conclusión: Para hacer una detección precoz del deterioro cognitivo es necesario disponer de mayor diversidad de pruebas de evaluación neuropsicológica con una alta sensibilidad y especificidad. Estas deben contar con validez tanto psicométrica como clínica para contribuir a un mejor seguimiento evolutivo del proceso de envejecimiento y del deterioro cognitivo.
Aging is associated with global cognitive changes, which sometimes progress to mild cognitive impairment or dementia, with Alzheimer’s disease being the most prevalent. It is essential to make early detection of the cognitive changes associated with aging to discriminate alterations that may suggest the onset of dementia to slow down the neurodegenerative process. Cognitive screening tests are useful and quick tools for this purpose but they need to be more sensitive and specific to guide properly the diagnosis. Gradually, cognitive performance needs to be monitored to identify possible involutions. The effect of the practice and learning of commonly used neuropsychological tests makes it difficult to follow up cognitive decline properly. Objective: To study the contribution of different neuropsychological tests in the detection of the involutive changes of the aging process, as well as in the study of the clinical evolution of mild cognitive impairment and Alzheimer's disease. Method: Three studies have been designed based on the early detection and follow up of cognitive impairment. In the first study, three screening tests (MMSE, MoCA, and PD-CRS) were compared. The participants were 126 people between 42 and 89 years old from different Neurology Services who have been divided into four diagnostic groups. The second study involved 250 cognitively intact people between 40 and 89 years old. The evolution of attention and processing speed throughout aging has been analyzed and the most current version of the MoCA has been standardized. In the third study, new neuropsychological assessment tests were created, validated, and standardized with the participation of 250 cognitively intact people between 40 and 89 years old. Results: From the first study, we conclude that the PD-CRS is the tool with greater sensitivity and specificity to detect MCI and Alzheimer’s disease. In the second study it has been observed that cognitive reserve has a clear effect on performance on attentional capacity and processing speed. Likewise, good reliability was obtained in the sample in MoCA 8.1 with an impact on age and educational level on the results. The new cognitive tests created, validated and standardized in the third study offer consistent results as clinical alternatives to the use of the most common neuropsychological assessment instruments in the diagnosis and follow-up of cognitive impairment. Conclusion: In order to make an early detection of cognitive impairment, it is necessary to have a greater diversity of neuropsychological evaluation tests with a high sensitivity and specificity. These must-have both psychometric and clinical validity to contribute to a better follow-up of the aging process and cognitive impairment.

Повний текст джерелаIt was designed a retrospective study with two objectives: First one to confirm the increase of events and the second one trying to figure out whether the cause could be due to side effects of the eye drops. For the first objective, the number of events (apneas, bradycardias, desaturations and gastric retentions) performed by the same infant between the 24-hour control period and the post-exploration period of another 24 hours was compared and for the second objective it was decided to compare two Patterns of dilatation with different amount of drug administered. A pattern A with 3 drops of 0,5% cilopentolate and 3 drops of 2.5% phenylephrine and a pattern B with a drop of cyclopentolate 0.2% and a drop of 1% phenylephrine. A total of 208 patients (104 for pattern A and 104 for pattern B) who provided a total of 575 explorations (273 for pattern A and 302 for pattern B), performed a first study showing the first contact with the medication in their First exploration and a second study including all the explorations performed. Other causes were taken into account that could cause those events, in order to establish the homogeneity between the two treatment regimens for their analysis if the groups were not homogeneous, statistical adjustment was made. Negative binomial regression models were used with paired data for the analysis. The result for the first objective showed an increase in the events on the day of the exploration in both patterns with significant differences for the first exploration, such as desaturations with an Incidence Rate Ratio or IRR of 1.02 (95% CI 1.01-1 , 02) in pattern A, an IRR 1.02 (95% CI 1.02 -1.03) in the B pattern or apneas at the time immediately after the IRR scan of 2.02 (95% CI 1, 16 - 3.52) in pattern A, an IRR 1.87 (IC95% 1.27 -2.73) in pattern B. For the rest of the explorations also confirmed that increase of events the day of the exploration; such as apneas with an IRR of 1.16 (95% CI 1.09 - 1.24) in pattern A, an IRR 1.22 (95% CI 1.15 -1.31). The result for the second objective showed a tendency to reduction with pattern B of the events in all variables on the day of the exploration when comparing both such as desaturations with an IRR 0.98 (95% CI 0.87 - 1.09) and only obtaining only a statistically significant result in the gastric retention variable in nocturnal interval with an IRR of 0.49 (95% CI 0.28-0.84). In conclusion, it is confirmed that exploration is bothersome for preterm newborns and it is necessary to apply comfort improvements and the smallest possible medication that is effective to minimize such side effects.

Abnormal ductus venosus flow at 11-13 weeks has been associated to fetal chromosomal abnormalities and cardiac defects.
The hypothesis of the studies is that flow through the ductus venosus can be assessed routinely at 11-13 weeks of gestation and that abnormal flow at this scan can help identify fetal chromosomal and structural defects as well as adverse pregnancy outcome.
STUDIES:
In the first study, ten sonographers received practical training in accurate assessment of the ductus venosus and performed 300 examinations each. The sonographers required an average of 80 examinations before they could successfully examine the ductus venosus flow.
In the second study ductus venosus flow was assessed immediately before chorion villous sampling (CVS) in fetuses with nuchal translucency (NT) thickness of 3.5 mm or more. A fetal echocardiography was performed in euploid fetuses at 11-13 weeks and/or 18-22 weeks. Reverse or absent flow during atrial contraction was observed in 68.8% of the fetuses with cardiac defects and in 22.9% with no cardiac defects.
In the third study screening by the combined test was performed in singleton pregnancies, including 19,614 with euploid fetuses, 122 with trisomy 21, 36 with trisomy 18, 20 with trisomy 13 and 8 with Turner syndrome. We examined the performance of two screening strategies: firstly, assessment of the a-wave in all patients and secondly, first-stage screening using the combined test in all patients followed by second-stage assessment of the a-wave only in those with an intermediate risk of 1 in 51 to 1 in 1,000 after the first-stage. Reversed a-wave was observed in 3.2% of the euploid fetuses and in 66.4%, 58.3%, 55.0% and 75.0% of fetuses with trisomies 21, 18 and 13 and Turner syndrome, respectively. Inclusion of ductus venosus flow in all pregnancies would detect 96%, 92%, 100% and 100% of trisomies 21, 18 and 13 and Turner syndrome, respectively, at a false positive rate of 3%. The same detection rates were achieved with the two-stage strategy at a false positive rate of 2.6%.
In the fourth study the patients were subdivided into five groups: normal outcome (n=10,120), miscarriage or fetal death (n=185), abnormal karyotype (n=95), major cardiac (n=20) or non-cardiac defect (n=70). The prevalence of reversed a-wave was significantly higher in the groups with miscarriage or fetal death (10.8%), abnormal karyotype (62.1%) and fetal cardiac defect (25.0%) but not non-cardiac defect (4.3%) than in the normal outcome group (3.7%).
The fifth study was a prospective study in 516 dichorionic and 179 monochorionic twin pregnancies. The prevalence of reversed a-wave in the fetal ductus venosus was compared between monochorionic and dichorionic pregnancies and between those with and without pregnancy complications. The prevalence of reversed a-wave in at least one of the fetuses was significantly higher in monochorionic than in dichorionic pregnancies (18.4% vs. 8.3%, p<0.001) and in pregnancies complicated by miscarriage (28.6%, p=0.005), fetal aneuploidy (70.0%, p<0.001) and twin-to-twin-transfusion syndrome (TTTS) (38.5%, p<0.001) compared to the pregnancies with two healthy live births (7.7%). Pregnancy outcome was normal in 76.7% dichorionic and in 42.4% monochorionic twins with reversed a-wave in at least one of the fetuses.
CONCLUSIONS:
After an extensive supervised training, ductus venosus flow assessment can be incorporated into the first trimester scan, where it improves the performance of screening for chromosomal defects and cardiac defects, and it helps to identify the fetuses with a higher risk of death. Similarly, in twin pregnancies ductus venosus assessment identifies the pregnancies with a higher risk of having a fetus with an aneuploidy, those with a higher risk of miscarriage, and those that will subsequently develop TTTS.

Повний текст джерелаINTRODUCCIÓN: Casi 38 millones de personas vivían con VIH a finales del 2020, de los cuales se estima que el 73% estaban recibiendo terapia antirretroviral (TAR). El panorama del VIH ha cambiado enormemente desde que se introdujeron los primeros, altamente efectivos regímenes de TAR en 1996 y las personas con VIH que inician TAR en una etapa suficientemente temprana de la enfermedad tienen una esperanza de vida casi normal. Sin embargo, sus vidas son notablemente diferentes de alguna manera en comparación con las personas sin VIH. La prevalencia de multimorbilidad es mayor en personas con VIH que en la población general, con una amplia gama de enfermedades infecciosas y no infecciosas que contribuyen a la carga de multimorbilidad. Las personas con VIH reportan que se preocupan por la incertidumbre sobre el futuro y tienen altos niveles de depresión y ansiedad. Este trabajo es oportuno debido a la necesidad apremiante de repensar el abordaje convencional de la atención del VIH en España y muchos otros países. A partir de mediados de la década de los 90, los proveedores de atención médica y expertos en salud pública han enfatizado la importancia de usar el TAR para reducir la viremia en personas con VIH, prestando menos atención a otros temas relacionados con la salud en esta población. Sin embargo, debido a la conciencia creciente sobre las altas cargas y los efectos de salud de estas otras cuestiones, ha quedado claro que los criterios convencionales para evaluar la efectividad de la atención del VIH en términos únicamente en resultados de carga viral y mortalidad relacionada con el VIH son insuficientes. Hay llamadas crecientes a reconocer la calidad de vida de las personas con VIH como una preocupación importante de atención médica por parte de investigadores, personas viviendo con VIH y otras partes interesadas en la comunidad del VIH. Este paradigma cambiante se refleja en la Estrategia Mundial sobre el Sida 2021-2026 del Programa Conjunto de las Naciones Unidas sobre el VIH/Sida (ONUSIDA), que aborda "optimizar la calidad de vida y el bienestar a lo largo del curso de la vida". La integración de estos principios y estándares de atención médica centrados en las personas en la práctica clínica es una estrategia esencial para mejorar la CVRS de personas con VIH que están recibiendo cuidado. Un abordaje centrado en la persona conlleva prestar atención a las cuestiones relacionadas con la salud que son importantes para una persona viviendo con VIH en lugar de simplemente centrarse en cómo detener la progresión de la enfermedad del VIH. Debe haber comunicación efectiva entre pacientes y proveedores de atención médica para que las necesidades y preferencias de los pacientes informen de manera significativa sus interacciones con los sistemas de salud. Varios hallazgos de investigación cuestionan si los proveedores de atención del VIH comprenden suficientemente las preocupaciones de sus pacientes. Varios estudios han documentado la falta de conocimiento de los proveedores de los síntomas clínicamente importantes en las poblaciones de pacientes con VIH y personas con VIH han reportado barreras de comunicación con sus proveedores. En la actualidad existe una necesidad insatisfecha de PROMs cortas y ampliamente enfocadas que puedan dar a los proveedores de servicios de salud un medio conveniente para identificar los diferentes desafíos relacionados con la salud que comúnmente experimentan las personas que viven con VIH. El objetivo general del cuerpo de investigación presentado en esta tesis es diseñar y pilotar un instrumento de PROM que se pueda usar en la práctica clínica rutinaria para identificar problemas que socavan la CVRS de personas con VIH en España, lo que ayudará a los sistemas de salud al responder más efectivamente a las necesidades a largo plazo de esta población, más allá del TAR. MÉTODOS: La investigación presentada en esta tesis se basa en tres estudios que se presentan en cuatro artículos. El primer estudio fue una encuesta de políticas que recopiló datos de un experto en cada uno de los seis países europeos para investigar la capacidad del sistema de salud para monitorear los aspectos de la atención médica, como la salud mental y los servicios psicosociales. Los hallazgos están reportados en el Artículo 1. El segundo estudio utilizó un diseño de estudio ex post facto transversal observacional para validar la versión española del “WHOQoL-HIV-BREF”, un PROM de CVRS específico para el VIH ampliamente utilizado, y para explorar la CVRS de personas con VIH de España. Los hallazgos están reportados en el Artículo 2. El tercer estudio se centró en el desarrollo del PROM “CST-VIH” de acuerdo con los procedimientos de desarrollo de instrumentos estándar. Los hallazgos de las dos primeras fases del proceso de estudio, una revisión de la literatura y una investigación cualitativa de las percepciones de personas con VIH y proveedores de atención médica sobre problemas que socavan el bienestar de personas con VIH en España, están reportados en el Artículo 3. Los hallazgos de las últimas fases, incluyendo un estudio piloto español que evalúa las propiedades psicométricas del nuevo instrumento, están reportados en el Artículo 4. RESULTADOS: El primer artículo se trata de una encuesta de políticas que investigó el monitoreo nacional de los problemas de atención del VIH en seis países europeos: Estonia, Italia, Holanda, Eslovenia, Suecia y Turquía. La encuesta se administró de abril a junio del 2018, con un experto seleccionado a propósito en cada país que proporciono información sobre la capacidad de monitoreo del sistema de salud en relación con temas como las comorbilidades, la CVRS, los servicios psicosociales y la discriminación relacionada con el VIH en entornos de atención médica. Según los encuestados, solo dos países (Eslovenia y Suecia) tenían la capacidad de monitorear los indicadores que abordan el cribado, diagnóstico y tratamiento de la ansiedad y depresión. Ninguno de los encuestados report el uso de indicadores a nivel nacional para monitorear la provisión de servicios psicosociales. Los encuestados de tres países (Holanda, Eslovenia y Suecia) indicaron que sus sistemas de monitoreo nacional tenían la capacidad para reportar sobre la CVRS de personas con VIH. El segundo artículo informa sobre la validación del WHOQoL-HIV-BREF en una muestra representativa a nivel nacional de personas con VIH en España (n = 1,462) y la CVRS de esta población. Los datos se recopilaron entre octubre del 2016 y abril del 2017. Pruebas psicométricas demostraron que la versión española del instrumento tiene una construcción adecuada y una validez convergente y concurrente. Las mujeres que participaron en el estudio y los participantes heterosexuales tuvieron una peor CVRS en comparación con sus contrapartes, al igual que las personas de estrato socioeconómico bajo, las personas que habían estado viviendo con VIH durante un período de tiempo más largo y las personas mayores de 50 años. El tercer artículo trata sobre todo el proceso de desarrollo de instrumentos CST-VIH, incluida la definición de dominios, el desarrollo de ítems potenciales, la evaluación experta de los ítems, los interrogatorios cognitivos con ocho personas con VIH y la validación del instrumento en una cohorte de 226 personas con VIH en España. Los ocho dominios elegidos para el instrumento piloto inicial fueron el estigma anticipado, la angustia emocional, la sexualidad, el apoyo social, la privación material, el sueño/fatiga, los problemas cognitivos y los síntomas físicos. Los hallazgos del piloto confirmaron la idoneidad de estos dominios, y llevaron a la selección de tres ítems por dominio entre los 40 elementos utilizados en el instrumento piloto. El instrumento resultante de 24 elementos cumplió con los estándares para el contenido, la cara, la construcción y la validez convergente y concurrente. El cuarto artículo informa sobre los hallazgos de la revisión de la literatura y los hallazgos de las discusiones de los grupos de enfoque que informaron el desarrollo temprano del CST- VIH. La revisión de la literatura documentó la alta prevalencia de síntomas, como los problemas relacionados con el sueño, la fatiga y el dolor en las poblaciones de personas con VIH, y también identifico estudios que demostraron que la carga de síntomas y otros asuntos, como el apoyo social, la salud emocional y la seguridad material se asocian con la CVRS en personas con VIH. Un análisis de los datos de las discusiones de los grupos de enfoque, dos con proveedores de servicios de VIH (n = 8 por grupo) y dos con personas con VIH (n = 8 y n = 7), identificó una serie de categorías amplias de problemas que afectan la CVRS de personas con VIH. Las cuestiones más destacadas por ambas categorías de participantes de los grupos de enfoque fueron problemas sociales, incluido el estigma y la discriminación relacionados con el VIH, los problemas relacionados con el sueño, la fatiga, el dolor y la angustia emocional. DISCUSIÓN Y CONCLUSIONES: Los resultados de esta tesis contribuyen a los esfuerzos en curso para reorientar la atención del VIH con el fin abordar la amplia gama de problemas físicos, emocionales y sociales que desafían a las personas que viven con VIH a largo plazo. El PROM desarrollado a través de esta investigación, el CST-VIH, demuestra una validez preliminar adecuada y actualmente está siendo validada más allá para fortalecer la evidencia que respalde su uso en la atención clínica rutinaria. Hasta donde sabemos, este es uno de los tres instrumentos concisos y validados, desarrollados específicamente para ayudar a los proveedores de atención del VIH en la identificación de una amplia gama de problemas relacionados con la salud que afectan al bienestar de las personas con VIH y el único instrumento de este tipo desarrollado en España. El CST-VIH se desarrolló a través de un proceso metodológicamente riguroso que siguió de cerca las mejores prácticas para el desarrollo del instrumento. Este demostró buenas propiedades psicométricas en las pruebas piloto, así como validez convergente y concurrente. Su brevedad y simplicidad permiten la finalización rápida por pacientes en las clínicas y una fácil evaluación de los datos por parte de los trabajadores de la salud. A la luz de estas consideraciones, el equipo de investigación anticipa que el CST-VIH tiene el potencial de informar significativamente la atención del VIH en España y quizás también en otros países con contextos similares, epidemiológicos y de salud. Se necesitan estudios metodológicamente rigurosos para evaluar cómo el uso del CST-VIH afecta las dimensiones específicas de la experiencia clínica, como la comunicación entre el proveedor y el paciente, la toma de decisiones clínicas y los resultados de salud y CVRS. Se pueden observar similitudes y diferencias cuando el CST-VIH se compara con otros dos PROMs concisos y ampliamente enfocados desarrollados para respaldar la atención clínica de personas con VIH: el PROM “Positive Outcomes” (“Resultados Positivos”), desarrollado en Inglaterra e Irlanda, y el “Short-Form HIV Disability Questionnaire” (“Cuestionario Corto de Discapacidad del VIH”), desarrollado en Canadá. Aún no se sabe cómo las diferencias entre las personas con VIH y los proveedores de atención médica en España y estos otros entornos pueden reflejarse en las diferencias en el CST-VIH y estos otros dos instrumentos. Por lo tanto, una pregunta abierta que debe informar a futuras investigaciones y prácticas en esta área es si un instrumento u otro podría encajar mejor con las necesidades autodefinidas de las personas con VIH y los proveedores de atención médica en otros países. En resumen, el CST-VIH es una nueva herramienta prometedora para mejorar la atención médica para las personas con VIH en España. Se debe explorar su relevancia dentro de la atención del VIH en otros países. Los investigadores deben interactuar con los responsables de la formulación de políticas y las comunidades afectadas para maximizar el potencial de los PROMs de este tipo, para contribuir a promover la salud multidimensional y CVRS de las personas con VIH, lo cual es consistente con la visión de la salud de la OMS como “un estado de completo bienestar físico, mental y social, y no solamente la ausencia de afecciones o enfermedades”.

Повний текст джерелаSudden death in sports has a relatively low incidence, estimated at 1-2 per 100,000 athletes per year; however, it has a great impact; generating an erroneous idea contrary to the paradigm that sport is beneficial. HYPOTHESIS: Pre-participation screening attempts to reduce the incidence of sudden death in sports, identifying the majority of individuals susceptible to having it. OBJECTIVES: 1) To analyze the usefulness of echocardiography in pre-participation screening; 2) To evaluate the utility of each one of the studies used in the pre-participation screening and the cost effectiveness analysis; 3) To evaluate the usefulness of ECG and Doppler echocardiography in the differential diagnosis of hypertrophic cardiomyopathy; 4) Study the prognosis of an exaggerated hypertensive response during exercise. METHODOLOGY: A cohort of 3000 competitive athletes, between 12 and 35 years old, from three high performance centers in which a pre-competitive assessment was carried out by means of; a) family / personal history and physical examination, b) electrocardiogram, c) echocardiogram, and d) stress test. RESULTS: The ECG and the echocardiogram were the most useful studies to detect athletes susceptible to sudden death in the adolescent or young adult age. The echocardiogram provides greater sensitivity, detecting some cardiac structural alterations that can go unnoticed with the physical examination and the ECG; Our results suggest the need for the introduction of echocardiography at least in the first pre-participation screening of competitive athletes. The stress test was more useful for the diagnosis of arrhythmias with specific treatment. The AHA questionnaire has a weak diagnostic utility, its use being limited as the only screening test. In our setting, pre-participation screening was cost effective to detect athletes susceptible to sudden death in sports. For the differential diagnosis between hypertrophic cardiomyopathy and athlete's heart, relative septal thickness> 0.54 may be useful. Elevation of diastolic blood pressure> 95 mmHg in maximal exercise or systolic blood pressure> 180 mmHg in moderate exercise were the best predictors of hypertension in the next 20 years.

Повний текст джерелаINTRODUCCIÓN: La prueba de detección de sangre oculta en heces es la prueba de cribado de cáncer colorrectal (CCR) más implementada en Europa. Aunque ésta ha demostrado reducir la mortalidad por CCR, dista de ser una prueba perfecta dado su riesgo de resultados falsos positivos y falsos negativos. Los efectos adversos del cribado podrían reducirse si conociéramos los determinantes asociados a resultados falsos positivos y negativos. Además, la identificación de polimorfismos genéticos, en combinación con otros biomarcadores moleculares y factores ambientales podría permitir personalizar el cribado poblacional. OBJETIVOS: 1) ldentificar los factores asociados a un resultado falso positivo de la prueba de detección de sangre oculta en heces inmunológica (PDSOHi). 2) Determinar la proporción de resultados falsos negativos de la PDSOHi y explorar sus determinantes. 3) Elaborar un modelo de predicción de riesgo de CCR basado en población española que combine la predisposición genética junto con factores ambientales y antecedentes familiares para estratificar la población en grupos de riesgo de CCR. METODOLOGIA: Para responder a los primeros dos objetivos se utilizaron datos del programa poblacional de cribado de CCR que coordina el lnstituto Catalán de Oncología. Se realizaron dos estudios transversales con participantes en el cribado que obtuvieron un resultado positivo (n = 427) o negativo (n = 218) en la PDSOHi y a quienes posteriormente se les realizó una colonoscopia y un cuestionario. Finalmente, para responder al tercer objetivo se aprovecharon los datos del estudio MCC-Spain, que es un estudio de casos y controles multicentrico que recogió 1.336 casos de CCR y 2.744 controles. De todos ellos disponíamos de un cuestionario epidemiológico y un array de exoma que contenía 21 polimorfismos de nucleótido simple asociados a riesgo de CCR. Con estos datos pudimos desarrollar un modelo de predicción de riesgo para estratificar la población en grupos de riesgo de CCR. RESULTADOS: Estudio 1: Mediante el estudio que evaluó el riesgo de resultados falsos positivos en la PDSOHi, se observó que las variables que resultaron ser determinantes de falso positivo fueron: ser mujer, haber participado previamente en el cribado, la presencia de hemorroides y/o fisura anal, así como tomar fármacos inhibidores de la bomba de protones. Estudio 2: Al evaluar el riesgo de resultados falsos negativos en la PDSOHi, la proporción de falsos negativos observada fue del 15,6%. La hemoglobina fecal fue indetectable en un 45,5% de los resultados y fue inferior a 4 microgr. Hb/g en un 94,0% de individuos con un resultado falso negativo. Estudio 3: Se observó que las variables (ajustadas por el modelo basal) que se asociaron de forma independiente a CCR fueron: el numero de alelos de riesgo, antecedentes familiares de CCR, la obesidad, la actividad física, la ingesta de carne roja y vegetales, y el no consumo de antiinflamatorios no esteroideos. Los individuos con más de 25 alelos de riesgo tenían un 82% más de riesgo que los individuos con menos de 19 alelos de riesgo. CONCLUSIONES: 1. El uso de inhibidores de la bomba de protones durante la realización de la PDSOHi podría incrementar la probabilidad de un resultado falso positivo. 2. Disminuir el umbral de positividad de la PDSOHi no detectaría mas neoplasia avanzada y aumentaría los costes y los riesgos innecesarios para los pacientes. 3. El modelo de predicción del riesgo de CCR basado en población española que combina la predisposición genetica con factores ambientales y antecedentes familiares podría ser útil para estratificar individuos de riesgo.
INTRODUCTION: ln Europe, the most commonly implemented test to screen colorectal cancer (CCR) is the faecal occult blood test. Although it has shown to reduce CRC mortality, it is far from perfect because of its false positive and false negative results. The adverse effects of screening could be reduced if we knew the determinants associated with false positive and negative results. ln addition, the identification of genetic polymorphisms in combination with other molecular biomarkers and environmental factors could allow personalizing population screening. OBJECTIVES: 1) To identify the factors associated with a false positive result of the faecal immunochemical test (FlT). 2) To determine the false negative proportion in and to explore its determinants. 3) To develop a risk stratification model that combined environmental factors with family history and genetic susceptibility to stratify the population in risk groups of CRC. METHODOLOGY: ln order to respond to the first two objectives, the population of the CRC screening program coordinated by the Catalan lnstitute of Oncology was used. Two cross-sectional studies were performed with screening participants who had a positive (n = 427) or negative (n = 218) FlT result and who subsequently underwent a colonoscopy and a questionnaire. Finally, in order to respond to the third objective, data from the MCC-Spain study was used. This multicentre case-control study recollected an epidemiological questionnaire and an exoma array containing 21 simple nucleotide polymorphisms associated with CRC risk of 1,336 cases of CRC and 2,744 controls. With these data we developed a risk stratification model to stratify the population in groups at risk of CRC. RESULTS: Study 1: While evaluating factors associated with false-positive results in a CRC screening, the variables associated with a false-positive result were: women, successive screening, haemorrhoids and/or anal fissure and the use of proton pump inhibitors. Study 2: When analysing the risk of false negative results in the FlT, the false negative rate observed was 15.6%. Faecal haemoglobin was undetected in 45.5% and it was below 4 micrograms Hb/g in 94.0% of the individuals with a FN result. Study 3: The variables (adjusted by the baseline model) that were independently associated with CRC were: the number of risk alleles, family history of CRC, alcohol consumption, obesity, physical activity, red meat and vegetable intake, and nonsteroidal anti- inflammatory drug use. The risk of subjects with more than 25 risk alleles was 82% higher than subjects with less than 19 alleles. CONCLUSIONS: 1. Concurrent use of proton pump inhibitors at the time of FlT might increase the likelihood of a false-positive result. 2. Decreasing the positivity threshold of FlT does not increase the detection rate of advanced neoplasia, but may increase the costs and potential adverse effects. 3. A Spanish population-based CRC risk prediction model that combines genetic predisposition with environmental factors and family history may be useful for stratifying at-risk individuals.

Los falsos positivos son el efecto adverso más frecuente del cribado mamográfico y reducen su efectividad. Objetivos: Estimar el riesgo acumulado de falsos positivos a lo largo de la participación secuencial de la mujer en el cribado mamográfico, y valorar su impacto sobre las mujeres cribadas y los factores de riesgo asociados. Evaluar la tendencia temporal en las tasas de detección de cáncer de mama en el cribado poblacional. Métodos: Se analizaron 4.739.498 mamografías de cribado de 1.565.364 mujeres de 45 a 69 años de edad participantes en los programas de cribado de 8 Comunidades Autónomas del territorio español entre 1991 y 2006. Las estimaciones se obtuvieron mediante modelos lineales generalizados de regresión con efectos aleatorios. Resultados: El riesgo acumulado de falso positivo fue del 20.4% (IC95%: 20.0-20.8) y de un 1.8% (IC95%: 1.6-1.9) para falsos positivos con pruebas invasivas. La adherencia a la segunda invitación al cribado en las mujeres con y sin falsos positivos previos fue 79.3 vs. 85.3%, respectivamente. El riesgo de detección de cáncer fue mayor en mujeres con falsos positivos previos (OR= 1.81; 95%CI: 1.70-1.94), y fue aún mayor en mujeres con falsos positivos con pruebas invasivas (OR= 2.69; 95%CI: 2.28-3.16). iv) Las tasas de detección de carcinoma ductal in situ aumentaron un 2.5% anual (95%CI: 1.3; 3.8). Las tasas de cáncer invasivo se mantuvieron estables. El riesgo acumulado de falso positivo en el programa de Noruega fue del 20.0% (95%CI= 19.7%-20.4%), y 4.1% (95%CI=3.9%-4.3%) para falsos positivos con pruebas invasivas. Conclusiones: Los resultados de estos estudios son relevantes para poder reducir los efectos adversos del cribado mamográfico y poder mejorar su efectividad. Además, permite mejorar la información proporcionada sobre los efectos adversos del cribado a las mujeres participantes.
Background: Mammographic breast cancer screening reduces mortality from this disease. False positive results are the most common adeverse effect of mamographic screening and they reduce the effectiveness of this practice. Aims: To estimate the cumulative risk of false positive results over 10 biennial participations in mammography screening age 50 to 69 years. To assess the factors associated with false positive results, and their impact on women screened. To assess the trends in rates of screen detected breast cancer in population-based screening. Methods: We analyzed 4.739.498 screeneing tests from 1.565.364 women 45-69 years old participating in mammographic screening in 8 different Regions of Spain, in the period 1991-2006. The estimates were obtained using generalized linear regression models with random effects. Results: The cumulative risk of false positive results was 20.4% (95%CI: 20.0-20.8) and 1.8% (95%CI: 1.6-1.9) for false positives with invasive procedures. At the second screening invitation re-attendance among women with and without a false-positive mammogram was 79.3 vs. 85.3%, respectively. The risk of cancer detection was greater in women with previous false positive results (OR= 1.81; 95% CI: 1.70-1.94), and was even higher in women with false positives with invasive procedures (OR = 2.69; 95% CI: 2.28 -3.16). iv) The detection rates of ductal carcinoma in situ increased by 2.5% per year (95% CI: 1.3; 3.8). Invasive cancer rates remained stable. The cumulative risk of false positive results in the Norwegian Breast Cancer Screening Program was 20.0% (95% CI = 19.7% -20.4%) and 4.1% (95% CI = 3.9% -4.3%) for false positives with invasive procedures. Conclusions: The results found are specially relevant to reduce the adverse effects of mammography screening and to improve its effectiveness. It also allows to improve the information given to screened women.

Повний текст джерелаLa retinopatía diabética (RD) es una de las complicaciones más comunes asociadas a la diabetes mellitus (DM). Su adecuado control requiere la implementación de programas de cribado. En este trabajo se presenta un estudio prospectivo de un total de 15 396 pacientes caucásicos con DM, que representan el 86,53% del total de pacientes con DM en nuestras áreas básicas de salud, con un período de seguimiento de 8 años (2007-2014). Todos los pacientes fueron examinados con una media de 3,18 ± 1,11 visitas a lo largo del estudio. En nuestros resultados se observa un aumento en la incidencia de RD y edema macular diabético (EMD), especialmente en los pacientes jóvenes entre 31 y 70 años, relacionado con un mal control metabólico. Además de un aumento de costes en la RD a raíz de la introducción de los nuevos tratamientos con anti-VEGF para el EMD. La periodicidad de los controles de cribado recomendados es de 1 año para pacientes con DM2, y a los 5 años del diagnóstico en pacientes con DM1, según las recomendaciones de diferentes sociedades científicas. Sin embargo, con la periodicidad recomendada solo se criban un 30% de pacientes al año. En nuestros resultados, la detección de la retinopatía diabética cada 2,5 años es coste-efectiva, pero debe ajustarse a los factores de riesgo de cada paciente. Por ello, hemos diseñado un soporte de ayuda a la toma de decisiones clínicas que pueda determinar con una sensibilidad y especificidad del 80%, la probabilidad de desarrollar retinopatía diabética en cada paciente. El uso de este modelo informático pretende ayudar a los médicos a determinar la mejor periodicidad de cribado para cada persona, de manera que se proporcione una atención adecuada y se empleen los recursos humanos, materiales y económicos de una manera más eficiente.
Diabetic retinopathy (DR) is one of the most usual morbidities associated to diabetes mellitus (DM). Its appropriate control requires the implementation of expensive screening programs. This paper reports a prospective population-based study of a total of 15 396 Caucasian patients with DM, who represent 86.53% of the total patients with DM in our health care areas, were studied over an 8-year follow-up period (2007-2014). All patients were screened with a mean follow-up of 3.18 ±1.11 times for each patient over the 8 years. An increase in the incidence of DR and diabetic macular oedema (DMO) was observed, especially in the younger patients aged between 31 and 70 years. This is linked to bad metabolic control of DM. Our results suggest a greater number of ocular complications in the near future, if these current findings are not addressed. Furthermore, our results reveal an increased costs in DR since the new treatments with anti-VEGF for DMO. The periodicity of screening controls recommended in patients with DM is 1 year in DM2, and after five years of diagnosis in DM1 patients, according to the recommendations of different scientific societies. However, with the recommended periodicity, only a screening rate estimated at 30% per year. Screening for diabetic retinopathy every 2.5 years is cost-effective in our results, but should be adjusted to a patient's personal risk factors. Thus, we have build a classifier which may determine, with sensitivity and especifity levels over 80%, whether a diabetic person is likely to develop retinopathy. The use of this model in a decision support tool may help doctors to determine the best screening periodicity for each person, so that an appropriate care is provided and human, material and economic resources are more efficiently employed.

Повний текст джерелаBACKGROUND: Participation rates in colorectal cancer screening are below recommended European targets. Reminders on electronic medical records may contribute to increase uptake in such programmes. AIM: To evaluate the effectiveness of an alert in primary care electronic medical records to increase individuals’ participation in an organized, population-based, faecal immunochemical test-based colorectal cancer screening programme compared to usual care. DESIGN AND SETTING: Cluster randomised controlled trial in primary-care centres of Barcelona. METHODS: Participants were all men and women aged 50-69 invited to the first round of a faecal immunochemical test-based screening programme (n=41,042), and all their primary care professionals. The randomisation unit was the physician-cluster (n=130). Patients were blinded to the study group. The control group followed usual care contemplated in the colorectal cancer screening programme. In the intervention group, plus usual care, an alert to health professionals (cluster level) to promote screening, was introduced in the individual’s primary-care electronic medical record during one year. The main outcome was colorectal cancer screening participation at individual participant level. RESULTS: 67 physicians and their 21,619 individuals in the intervention group, and 63 physicians and their 19,423 individuals in the control group were randomised and analysed respectively by intention-to-treat approach. Screening participation was 44.1% and 42.2%, respectively (OR, 1.08; 95% CI, 0.97 to 1.20; p=0.146). No significant improvements were found after adjusting for clustering and other confounding variables. However, in the per-protocol analysis screening uptake in the intervention group showed a statistically significant increase, after adjusting for potential confounders (OR, 1.11; 95% CI, 1.02 to 1.22; P = 0.018). CONCLUSIONS: The use of an alert in individual’s primary care electronic medical record is not associated with a statistically significant increased uptake of an organized, faecal immunochemical test-based colorectal-cancer screening programme with a moderate baseline participation rate. Nevertheless, a statistically significant increased uptake in patients attending primary care centres is observed.

Повний текст джерелаMuch effort is being made on Fanconi anemia therapeutics to treat bone marrow failure and cancer, the most life threatening signs of the disease. There are also studies searching for prevention, with chronic treatment proposals. As mortality rate by bone marrow failure has been dramatically reduced in the last years, thanks to improved hematopoietic stem cell transplantation protocols, and with the promise of new approaches such as gene therapy, the next step in the horizon of Fanconi anemia treatment may be focused on malignancies. Patients have very low survival rate after solid tumors appear, and no effective therapy currently exists beyond surgical resection. That is the reason why we developed two drug screening assays to measure different Fanconi biomarkers with the purpose of finding new drugs that ameliorate or cure the Fanconi anemia disorder and could be investigated in preclinical studies as a new therapy. We adapted the flow cytometric in vitro micronucleus test to screen drugs for their ability to reduce chromosome fragility and cell cycle arrest in FANCA-deficient cells. Screening of 10 candidate drugs based on literature search and a redox library of 84 antioxidants/antiinflammatory compounds identified several drugs that consistently restored chromosome fragility and G2/M cell cycle arrest in lymphoblast cell lines from 3 independent FANCA-deficient patients. We selected two drugs, CRD3 and CRD8, widely used in clinics, to test in Fanca-deficient mice in vivo. One year later 60 % of placebo-treated mice died. Only 20 % mice of CRD3 treated group were sacrificed due to excess weight loss while all CRD8 treated mice were alive and healthy at the end of the experiment. In a second assay, we set up a cell system based on FANCD2-Ub foci detection in FANCA-deficient cells and we screened 3802 drugs, which included all FDA-approved, kinase inhibitors and chemical diversity compounds in search of a drug to cure Fanconi anemia by re-activating the FA/BRCA pathway. We selected 7 hits and none of them could be validated on phenotypic assays. Taking advantage of this screening, we searched for drugs that killed cells by a non-genotoxic mode of action. Selected drugs were tested in Fanconi anemia patient-derived head and neck squamous cell carcinomas (HNSCC) cell lines and primary fibroblasts. Two potential drugs, CAT2 and CAT29, were especially relevant as they are highly selective for cancer cells. They are already approved for cancer treatment and their mechanism of action does not involve DNA damage. Neither CAT2 nor CAT29 induced chromosome fragility in FANCA-deficient lymphoblastoid cell line or activate FANCD2 ubiquitination suggesting that the Fanconi anemia pathway is not involved in the cellular response to these drugs. In vivo experiments of tumor xenografts in mice showed that CAT2 and CAT29 inhibited the growth of HNSCC Fanconi VU1131-T2.8 in a highly significant manner. Given the results presented in this PhD thesis, we propose to continue the preclinical studies in order to finally repurpose CRD3 and CRD8 to treat Fanconi anemia and CAT2 and CAT29 to treat Fanconi HNSCC.

Повний текст джерелаBackground In breast cancer screening programmes, double reading of digital mammograms is used since it can detect more lesions. Some of these lesions are not aggressive and may be considered false positives or overdiagnosis. Therefore, it is relevant to evaluate the effectiveness and cost-effectiveness of double reading and to characterize benign lesions. Objectives To evaluate the effectiveness and cost-effectiveness of double reading of digital mammograms and to characterize the benign breast lesions detected in a breast cancer screening programme. Methods Four studies with different methodological designs were performed. To determine the effectiveness and cost-effectiveness of double reading of digital mammograms, we performed: i) a cost-consequence study; ii) a cost-effectiveness study; and iii) a systematic review of observational studies and economic evaluations. To determine whether the expression of estrogen (ER), progesterone (PR) and Ki-67 receptors in benign lesions increases the risk of cancer, iv) a case-control study was conducted. Results The cost-consequence study included 57,157 digital mammograms. The average cost of the screening process with double reading with consensus and arbitration was 15% higher (44.9 €) than with single reading (39.1 €). Double reading had 0.3% (N = 181) more false-positive results than single reading (P < 0.01). Both reading strategies had similar cancer detection rates (4.6/1000 and 4.2/1000 mammograms, respectively; P = 0.28) and similar positive predictive values (9.3% and 9.1%, respectively, P = 0.81). In the cost-effectiveness study, 28,636 digital mammograms were analysed. The number of cancers detected was 148 (5.2/1000) with double reading, 138 (4.8/1000) with double reading in prevalent screening and single reading in incident screening, and 137 (4.8/1000) with single reading (P = 0.77). The average cost of detecting one cancer was 8,912 €, 8,614 € and 8,287 €, respectively. The incremental cost-effectiveness ratio (ICER) of double reading in prevalent screening and single reading in incident screening was 53,312 € per detected cancer. This strategy was dominated by extension so it was eliminated from the analysis. The ICER of double versus single reading was 16,684 €. In the systematic review four studies with low risk of bias were included. Three of them were made in Europe and one in Japan. The pooled rate of cancer detection with double reading was 6.0/1000 mammograms and it was 5.7/1000 with single reading (P = 0.76). The proportion of false positives with double reading was 4.7% and 4.1% with single reading (P = 0.12). Single reading plus a diagnosis-aid system was cost-effective in Japan. No studies evaluating mortality, quality of life, or overdiagnosis were identified. The case-control study included 258 benign lesions, 86 were cases and 172 were controls. Compared with benign lesions with low ER (≤ 70%) expression, moderate (71% to 90%) and high (> 90%) expression showed an increased risk of breast cancer (OR = 1.98 and OR = 2.63, respectively). Lesions with high PR (> 80%) showed an increased risk of breast cancer (OR = 2.22) compared to those with low expression (≤ 40%). Proliferative lesions with ≥ 1% Ki67 expression did not show an increased risk of cancer (OR = 1.16). Conclusion In the era of digital mammography, double reading significantly increases the frequency of false positives without an important increase in the cancer detection rate. Double reading is not cost-effective when applied only in the prevalent screening and it also appears not to be cost-effective in the incident screening. Benign lesions with high ER or RP expression have a higher risk of developing breast cancer.

A pesar de ser altamente prevenible mediante su cribado y el tratamiento de pólipos precancerosos, la incidencia y mortalidad por este cáncer sigue creciendo en la Isla, mientras que las ratios de cribado permanecen bajas. El objetivo principal de esta tesis doctoral es describir el proceso de diseño y creación de componentes educativos para un programa de promoción para la salud que pretende incrementar el cribado del CCR en pacientes de 50 años o más pertenecientes a los Centros Federales de Salud Primaria (Federally Qualified Health Centers; FQHC) de PR. Métodos Para ello se utilizó Intervention Mapping (IM), un marco de planificación sistemática basado en la teoría y en la evidencia que nos guio en la planificación y diseño de un programa de promoción para la salud cuya finalidad es incrementar el cribado de CCR entre los puertorriqueños de 50 años o más que sean pacientes de los FQHC. IM consta de seis pasos con tareas específicas. En esta tesis se presentan los cuatro primeros (valoración de necesidades, matriz de objetivos de cambio, selección de métodos y estrategias y creación del programa). Los pasos 5 y 6 (implementación y evaluación del programa) quedan planteados como futuras fases de la investigación. Resultados En un primer paso, determinamos la incidencia y las tasas de mortalidad de CCR usando información el Registro de Cáncer de PR. Condujimos una revisión de la literatura para entender mejor los determinantes personales y factores del entorno relacionados con el CCR y su cribado en puertorriqueños y poblaciones latinas de Estados Unidos y, finalmente, recolectamos nuevos datos mediante grupos focales. Condujimos 7 grupos focales (N=56) para identificar las necesidades y recursos de la población, sub comportamientos específicos (objetivos de acción) y determinantes relacionados con el cribado del CCR. En un segundo paso, creamos las matrices de objetivos de cambio que guiaron el contenido. En un tercer paso, seleccionamos los métodos generales entretenimiento educativo y periodismo conductual, y desarrollamos aplicaciones prácticas, materiales y mensajes que, a su vez, contenían otros métodos como modelaje, persuasión, información y tailoring, y se determinó el tipo de formato para cada componente educativo. En el paso 4, desarrollamos una intervención multimedia interactiva adaptada (tailored interactive multimedia intervention; TIMI), una revista, diversas hojas informativas, infográficos, y un plan de acción. Del mismo modo, creamos un recordatorio para proveedores de salud, mediado por los propios pacientes, con la finalidad de incrementar la recomendación de los médicos acerca del cribado del CCR y, a su vez, mejorar la comunicación paciente-proveedor; y diseñamos un protocolo de llamadas de recordatorio de las pruebas para pacientes. Finalmente, se llevaron a cabo grupos focales (N=19) para valorar la usabilidad, atractivo, sensibilidad cultural, y motivación generada por los componentes para realizarse las pruebas de cribado. Las sugerencias de los participantes fueron valoradas por el equipo de investigación e incorporadas a la versión final de los componentes educativos.
Introduction Colorectal cancer (CRC) is a leading cause of cancer- related mortality in Puerto Rico (PR). Despite being highly preventable through screening and treatment of precancerous polyps, the incidence and mortality from CRC in PR continues to grow while screening rates remain low. The objective of this dissertation is to describe the process of designing and creating educational components for a health promotion program that aims to increase CRC screening in patients aged 50 years or older belonging to the Federally Qualified Health Centers (FQHC) in PR. Methods We used Intervention Mapping (IM), a systematic framework using theory and evidence to plan and design a health promotion program that aims to increase CRC screening among Puerto Ricans 50 years or more who are patients of the FQHC. IM consists of six steps with specific tasks. This dissertation presents the first four (needs assessment, matrix of change objectives, selection of methods and strategies and creation of the Program). Steps 5 and 6 (program implementation and evaluation) are considered as future phases of the research. Results To inform the development of a logic model of the problem during the needs assessment phase, we determined the CRC incidence and mortality rates in Puerto Rico using recent data from the PR Cancer Registry, conducted a literature review to better understand behavioral and environmental factors influencing CRC among Hispanics in general and in Puerto Ricans, and collected new data. We conducted 7 focus groups (n=56) to identify community needs and resources, specific sub behaviors related to CRCS (performance objectives) and the determinants of CRCS. We then developed matrices of change objectives that would guide the content, behavioral change method selection and the practical applications that would be included in the program. We selected two overarching methods: entertainment education and behavioral journalism and developed practical applications, materials and messages containing several other methods including modeling, persuasion, information, and tailoring. We developed and pre-tested a tailored interactive multimedia intervention, newsletter, fact sheets, infographics, and an action plan. We also developed a patient mediated provider prompt to increase provider recommendation and improve patient provider communication and designed a protocol for support calls for patients. Finally, we conducted focus groups (N = 19) to assess the usability, attractiveness, cultural sensitivity, and motivation generated by the components to perform the screening tests. The suggestions of the participants have been evaluated by the research team and are being incorporated into the final version of the educational components.

Повний текст джерелаOBJECTIVES: Define the role of the nurse in the cancer screening programs. Identify the indicators of the activity. Evaluate the nursing care of the cancer screening program Institut Català d'Oncologia. METHODOLOGY: The first objective was achieved through a review of the literature. Delphi methodology was used to contextualize these activities in population-based programs. And finally, a descriptive study to identify the interventions of the Nursing Interventions Classification. The second objective was solved through a literature review and an expert group consensus. The third objective and was done through a cross-sectional survey of primary care professionals. It was based on a survey of knowledge about colorectal cancer screening procedures, risk factors, post-diagnostic follow-up recommendations, and referral strategies. Subsequently, a cluster-randomized controlled trial evaluated an information intervention. RESULTS: The panel of experts identified 25 activities. The taxonomy experts correlated these activities with 15 interventions of the Nursing Interventions Classification. The expert group selected 7 indicators (adequacy and waiting time for referral of participants, delivery and availability of the process report, understanding of professionals involved in the process, satisfaction and understanding of participants). No statistically significant differences were found between the intervention group and the control group. However, in nine questions the percentage of correct answers in the intervention group was increased, mainly related to the follow-up after the diagnostic exploration. CONCLUSIONS: The activities of the cancer screening nurse are act as case manager and provide information, which favors continuity and coordination during the process. However, ongoing evaluation of cancer screening programs does not include these indicators. Therefore, indicators of adequacy and waiting time for referral, delivery and availability of the process report, understanding of the professionals, satisfaction and understanding of participants are proposed. The results obtained from the measurement of the indicator reveal that, although they are aware of the screening process, there are some aspects that could improve. To improve them, an educational intervention was designed and allowed to improve certain areas of knowledge, but not in a significant way. Therefore, it is necessary to design and implement strategies aimed at increasing the knowledge of primary care professionals.

Esto permite a los investigadores ahorrarse el coste de probar experimentalmente cientos o miles de compuestos candidatos, reduciéndolos hasta cantidades manejables. Para la validación de los métodos de cribado virtual hacen falta bibliotecas de moléculas señuelo. El software DecoyFinder fue desarrollado como aplicación gráfica de fácil uso para la construcción de bibliotecas de moléculas señuelo, y fue posteriormente ampliado con los hallazgos de investigación posterior sobre la construcción i rendimiento de bibliotecas de moléculas señuelo. El Protein Data Bank (PDB) es muy útil porque proporciona estructuras tridimensionales para complejos proteina-ligando, y por tanto, información sobre como interactúan. Para los métodos de cribado virtual que dependen de ellas, es extremadamente importante su fiabilidad. VHELIBS fue desarrollado como herramienta para inspeccionar e identificar, fácil e intuitivamente, las estructuras fiables del PDB, basándose en como de bueno es su encaje con sus correspondientes mapas de densidad electrónica. Mientras que el cribado virtual intenta encontrar nuevas moléculas bioactivas para determinadas dianas, el enfoque inverso también se utiliza: a partir de una molécula, buscar dianas donde presente actividad biológica no documentada. Este cribado inverso es conocido en inglés como “in silico target fishing”, o pesca de dianas “in silico”, y es especialmente útil en el ámbito de la reutilización de fármacos. Al comenzar esta tesis, no había ninguna plataforma de “target fishing” de libre acceso, y aunque durante los años se han desarrollado algunas, en todos los casos su predicción de bioactividad es cualitativa. Por eso se desarrolló una plataforma propia de “target fishing” de libre acceso, con la implementación de un nuevo método que proporciona la primera predicción cuantitativa de bioactividad para este tipo de plataforma.
Virtual screening is a cheminformatics method that consists of screening large small-molecule databases for bioactive molecules. This enables the researcher to avoid the cost of experimentally testing hundreds or thousands of compounds by reducing the number of candidate molecules to be tested to manageable numbers. For their validation, virtual screening approaches need decoy molecule libraries. DecoyFinder was developed as an easy to use graphical application for decoy library building, and later updated after some research into decoy library building and their performance when used for 2D similarity approaches. The Protein Data Bank (PDB) is very useful because it provides 3D structures for protein-ligand complexes and, therefore, information on how certain ligands bind and interact with their targets. For virtual screening apporaches relying on these structures, it is of the utmost importance that the data available on the PDB for the ligand and its binding site are reliable. VHELIBS was developed as a tool to easily and intuitively inspect and identify reliable PDB structures based on the goodness of fitting between ligands and binding sites and their corresponding electron density map. While virtual screening aims to find new bioactive molecules for certain targets, the opposite approach is also used: starting from a given molecule, to search for a biological target for which it presents previously undocumented bioactivity. This reverse screening is known as in silico or computational target fishing or reverse pharmacognosy, and it is specially useful for drug repurposing or repositioning. When this thesis was started, there were no freely available target fishing platforms, but some have been developed during the years. However, they are qualitative in the nature of their activity prediction, and thus we set out to develop a freely accessible target fishing web service implementing a novel method which provides the first quantitative activity prediction: Anglerfish.

Повний текст джерелаObjetivo: Analizar y comparar las escalas MNA-SF, DETERMINE y MUST para poder identificar la más adecuada como herramienta de cribado para detectar riesgo de desnutrición en la población de 75 años o más que vive en su domicilio.Metodología: Estudio descriptivo, longitudinal y prospectivo, obtenido a partir de los datos de la Encuesta FRALLE. La muestra fue de 640 individuos. Las variables utilizadas en la fase transversal fueron: el riesgo de desnutrición medido con las escalas MNA-SF, DETERMINE y MUST y los factores asociados con riesgo de desnutrición (variables sociodemográficas, de estado de salud y parámetros antropométricos). En la fase longitudinal se recogieron los efectos adversos de la desnutrición (mortalidad, ingresos hospitalarios, caídas y aparición de discapacidad). La fiabilidad de las escalas se midió mediante el alfa de Cronbach. Para analizar la validez concurrente, se seleccionaron las escalas CES-D, Lawton&Brody y MNA para usarlas como criterios realizando, posteriormente, los cálculos de sensibilidad y especificidad, valores predictivos y curvas ROC. Para la evaluación de la validez predictiva, se llevaron a cabo análisis de regresión logística para ver qué escalas de cribado nutricional se asociaban de manera independiente a los efectos adversos de desnutrición, después de ajustarlas por las variables sociodemográficas, de estado de salud y parámetros antropométricos. Finalmente, se utilizaron curvas de supervivencia y se construyeron modelos de riesgos proporcionales de Cox para observar la capacidad predictiva del riesgo de mortalidad de las tres escalas a los dos años. Para el análisis de los datos se empleó el programa SPSS. El nivel de significación aceptado para todos los análisis fue de p <0,05.Resultados: Las prevalencias de riesgo de desnutrición según el MNA-SF, DETERMINE y MUST fueron del 21,7%, 36,1% y del 12,2% respectivamente. La consistencia interna de las escalas fue de 0,51 en el MNA-SF, 0,50 en el DETERMINE y 0,05 en el MUST. La validez concurrente mostró valores elevados de especificidad y valores predictivos negativos. En cambio, las sensibilidades y los valores predictivos positivos fueron bajos, excepto cuando se empleó como criterio la escala MNA, la cual obtuvo mejores resultados. La capacidad predictiva de los ingresos hospitalarios, las caídas y la aparición de discapacidad no se manifestó en ninguna de las tres escalas. Sin embargo, el MNA-SF fue la única escala capaz de predecir mortalidad, mostrando que: los individuos que presentaban riesgo de desnutrición obtenían una media de supervivencia inferior a los que tenían un buen estado nutricional y el riesgo de desnutrición medido mediante la escala MNA-SF fue un factor pronóstico de mortalidad a los dos años.Conclusiones: En la comparación de las tres escalas evaluadas, la que obtuvo mejores resultados fue el MNA-SF, tanto por su validez concurrente como por su capacidad predictiva de la mortalidad. Por lo tanto, según los resultados de este estudio, se pudo constatar que el MNA-SF fue la escala más adecuada para el cribado nutricional de las personas mayores en el ámbito comunitario.
Aim: To analyze and compare the MNA-SF, DETERMINE and MUST scales to identify the most suitable screening tool to detect nutritional risk in the population aged 75 and over living at home.Methods: A descriptive, longitudinal and prospective study, based on data from the Survey FRALLE, was conducted, using a sample of 640 individuals. The variables used in the transverse phase were nutritional risk, measured by the MNA-SF, DETERMINE and MUST, and the factors associated with nutritional risk (sociodemographic variables, health status and anthropometric parameters). In the longitudinal phase the data associated with the adverse effects of malnutrition were collected (mortality, hospital admissions, falls and disability). The reliability of the scales was measured by Cronbach's alpha. To analyze the concurrent validity the CES-D, Lawton & Brody and MNA scales were selected and used as criteria, later to make calculations of sensitivity and specificity, predictive values and ROC curves. To evaluate the predictive validity logistic regression analyzes were conducted to see what nutritional screening tools were independently associated with the adverse effects of malnutrition, after adjustment for the sociodemographic variables, health status and anthropometric parameters were made. Finally, we used survival curves and constructed Cox proportional hazards models to observe the predictive ability of the nutritional tools of mortality risk for two years. For the analysis of the data SPSS was used with the accepted level of significance for all analyzes of p <0.05.Results: The prevalence of risk of malnutrition according to the MNA-SF, DETERMINE and MUST was 21,7%, 36,1% and 12,2% respectively. Internal consistency of the MNA-SF was 0,51, DETERMINE was 0,50 and MUST was 0,05. The concurrent validity showed high values of specificity and negative predictive values, however, the sensitivity and positive predictive values were low, except when MNA was used as a criterion, which had better results. The predictive ability of hospital admissions, falls and disability was not evident in any of the three scales. However, the MNA-SF was the only scale that could predict mortality, showing that: individuals at risk of malnutrition obtained a lower median survival than those who had good nutritional status. Nutritional risk, which was measured by the MNA-SF scale, was a prognostic factor for mortality in two years.Conclusions: In the comparison of the three scales evaluated, the MNA-SF obtained the best results, for its concurrent validity and its predictive capacity of mortality. Therefore, given the results of this study, the MNA-SF was the most appropriate nutritional screening tool of older people in the community.

Повний текст джерелаEl síndrome de apnea-hipopnea del sueño (SAHS) es una entidad con elevada y creciente prevalencia e importantes consecuencias sobre la salud y calidad de vida del paciente. Se caracteriza por la presentación de episodios repetidos de colapso de la vía aérea superior durante el sueño que provocan descensos cíclicos de la saturación arterial y despertares corticales transitorios, lo que puede dar lugar a un sueño no reparador, somnolencia diurna, alteración de las funciones cognitivas, deterioro de la calidad de vida e incremento de la accidentabilidad laboral y de tráfico. Por otra parte, la hipoxia intermitente y el aumento de la actividad simpática pueden activar diferentes vías patogénicas que explicarían el mayor riesgo de desarrollar enfermedad cardiovascular y metabólica y un incremento de la mortalidad. El tratamiento con presión positiva continua en la vía aérea (CPAP) durante el sueño corrige los eventos respiratorios obstructivos, mejora los síntomas y la calidad de vida de los pacientes con SAHS, reduce de forma significativa la tensión arterial y se ha asociado con un descenso de la mortalidad de cualquier causa cardiovascular. En la actualidad hay consenso de que una enfermedad con un impacto en la salud tan importante debe implicar a todos los niveles asistenciales, incluida como parte fundamental, la Atención Primaria (AP). En la presente tesis se recogen la metodología y los resultados fundamentales del proyecto PASHOS (Plataforma Avanzada para el manejo del Síndrome de Apnea Hipopnea del Sueño). Se trata de un estudio prospectivo y multicéntrico, cuyo objetivo general ha sido crear una red de trabajo coordinada entre la AP y la Unidad de Sueño (US) para el manejo del SAHS, así como desarrollar y validar un modelo predictivo basado en variables clínicas y en un estudio simplificado de monitorización ambulatoria (EMA). Para su desarrollo se contó con la experiencia de cuatro US, que atienden a una población de más de un millón de habitantes, y con seis equipos de AP con diferentes estructuras administrativas y de funcionamiento. En total fueron evaluados 1036 pacientes seleccionados de manera aleatoria en la consulta de AP, de los que 466 (44,9%) fueron elegibles y aceptaron participar en el estudio. A 249 pacientes (53,4%) se les realizó un EMA en AP y en 194 (41,6%) se completó todo el estudio en la US, estableciéndose así el diagnóstico y la decisión terapéutica definitiva. Esta tesis doctoral pone de manifiesto que, con un programa formativo previo, breve y sistematizado, los profesionales referentes de AP pueden llegar a alcanzar un nivel de conocimiento del SAHS que les permita clasificar a los pacientes con un aceptable acuerdo respecto a los especialistas de sueño. Globalmente, el acuerdo es moderado en cuanto a la clasificación diagnóstica, pero alcanza un buen grado de concordancia cuando se establece la decisión terapéutica de iniciar tratamiento con CPAP. Además, se presenta el diseño de un modelo de cribaje del SAHS, de fácil aplicación en AP, con elevada sensibilidad y elevado valor predictivo negativo, que permite mejorar la identificación de los pacientes graves que requieren derivación a la US. El modelo, en dos etapas, incluye un breve cuestionario clínico (perímetro de cuello, IMC y pausas respiratorias objetivadas) que, de ser positivo, se complementa con un EMA. Con la aplicación de este modelo se podría evitar la derivación a la US del 55,1% de pacientes que presentan una alteración leve-moderada y que podrían seguir siendo atendidos en la propia AP.
Obstructive sleep apnea (OSA) is an entity with a high and increasing prevalence with important consequences patients' health and quality of life. It is characterized by repeated upper-airway collapse during sleep, resulting in cyclical drops in arterial saturation and transient cortical arousals, which can lead to non-restorative sleep, daytime sleepiness, cognitive impairment, a deterioration in quality of life and an increase in work and traffic accidents. In addition, intermittent hypoxia and major sympathetic nervous system activity can activate different pathogenic pathways that would explain the increased risk of developing cardiovascular and metabolic disease and mortality in OSA patients. Treatment with continuous positive airway pressure (CPAP) during sleep corrects obstructive respiratory events, improves symptoms and quality of life in patients with OSA, significantly reduces blood pressure, and has been associated with a decrease in mortality from any cardiovascular cause. There is currently a consensus that a disease with such a significant health impact must involve all levels of care, including primary care (PC) as a fundamental part. This thesis collects the methodology and fundamental results of the PASHOS Project (Advanced Platform for the management of Obstructive Sleep Apnea). This is a prospective, multicenter study. Its general objective has been to create a network that coordinates the PC level and the Sleep Unit (SU) for the management of OSA, and to develop and validate a predictive model based on clinical variables and a home sleep apnea test (HSAT). Its development drew on the experience of four SUs, covering a population of more than one million inhabitants, and six PC teams with different administrative and operational structures. In total, 1036 randomly patients were evaluated in the PC consultation, 466 of whom (44.9%) were eligible and agreed to participate in the study. 249 patients (53.4%) underwent a HSAT in PC and the entire study was completed in the SU in 194 (41.6%), thus establishing the diagnosis and the definitive therapeutic decision. This thesis sets out the protocol and methodology of the project. It shows that, with a prior, brief, systematized training program, PC referral professionals can reach a level of knowledge of OSA that allows them to classify patients with an acceptable level of agreement with sleep specialists. Overall, this agreement is moderate in terms of diagnostic classification, but reaches a good degree of agreement when the CPAP therapeutic decision is established. Besides, the study presents the design of the OSA screening model, which is easy to apply in PC and has a high sensitivity and negative predictive value, and which improves the identification of severe patients requiring a referral to a SU. This two-stage model includes a brief clinical questionnaire (neck circumference, BMI and observed apnea) which, if it results positive, is complemented with a HSAT. With the application of this model, referrals to the SU could be avoided in 55.1% of the patients who present with a mild-moderate form of the disease and who could be managed at the PC level.
