Plexiform neurofibromas in major salivary glands are rarely described. In the literature, most reported tumours have been present in the parotid gland region. A three-year-old boy with a family history of neurofibromatosis presented with a rapidly growing left submandibular mass. The clinical diagnosis was that of a neurofibroma rather than a primary salivery gland tumour. Resection of the lesion revealed a plexiform neurofibroma involving the submandibular gland. Although these tumours have a neurogenic rather than a salivary gland origin, they must be considered in the differential diagnosis of a salivary gland lesion in a patient with a history of neurofibromatosis.

Salivary gland homogenates and oil-induced saliva of the mosquito Aedes aegypti dilate the rabbit aortic ring and contract the guinea pig ileum. The vasodilatory activity is endothelium-dependent, heat-stable, sensitive to both trypsin and chymotrypsin treatments, and both smooth muscle activities cross-desensitize to the tachykinin peptide substance P. Both bioactivities co-elute when salivary gland homogenates are fractionated by reversed-phase HPLC. Molecular sieving chromatography indicates a relative molecular mass of 1400. A monoclonal antibody specific to the carboxy terminal region of tachykinins reacts with material in the posterior part of the central lobe of paraformaldehyde-fixed salivary glands. The presence of a vasodilatory peptide of the tachykinin family in the salivary glands of A. aegypti is proposed and its role in blood feeding is discussed.

The congenital absence of major salivary glands is an infrequent disorder, in which several glands are usually involved at the same time. Aplasia of unilateral submandibular (SM) gland is a very rare finding. Clinical presentation is variable. While some patients have no symptoms, others report difficulty in chewing and swallowing due to dry mouth, and even dental caries. Diagnosis of salivary gland aplasia can be made with a variety of imaging techniques, which include sonography, CT or MRI, sialography or isotope scans (technetium T99m-pertechnetate). We report the case of a 30-year-old man presenting with a gradually progressive neck mass, because of unilateral SM gland hypertrophy associated with contralateral SM gland aplasia.

AbstractEpithelial-myoepithelial carcinoma (EMC) accounts for approximately one per cent of salivary gland tumours. This tumour is gaining wider recognition following inclusion into the WHO histological classification of salivary gland tumours in 1990. Salivary gland tumours characteristically present with an enlarging mass. We describe an unusual presentation of a salivary gland tumour with stenosis of the external ear canal in the absence of a palpable mass. EMC usually arises from the salivary glands but isolated cases have been described arising primarily from the paranasal sinuses, trachea and lacrimal gland. The management of this tumour is still evolving with surgical excision being the main-stay of treatment. The efficacy of radiotherapy has not yet been established but high local recurrence rates despite apparently adequate excision and the possibility of a multicentric origin of the tumour may herald an increasing role for radiotherapy in the future. We stress the importance of awareness of adjacent structures when considering the cause of cartilaginous canal stenosis.

The salivary glands are small structures in the head and neck, but can give rise to a wide variety of benign and malignant pathology. When this occurs, patients may present with palpable swelling, although it is quite common that they are asymptomatic and a salivary gland mass was discovered as an incidental finding on imaging performed for another reason. It is, therefore, critical that radiologists pay careful attention to the salivary glands and have working knowledge of the key differentiating features of the most common neoplastic and nonneoplastic etiologies of salivary gland masses. The purpose of this review is to provide a succinct image-rich article illustrating the various causes of salivary gland masses via an extensive review of the primary literature. In Part 2, we discuss neoplasms and tumor-like lesions of the salivary glands with a key emphasis on specific imaging features of the most common pathologic entities.

Background. The aim was to investigate the amounts of saliva and serum asprosin in order to determine whether it is related to obesity and whether salivary glands synthesize asprosin or not.Methods. A total of 116 underweight, normal weight, overweight, and obese (class I, class II, and class III) volunteers participated in the study. Saliva and blood samples were collected simultaneously from the participants. The amounts of asprosin in saliva, salivary gland tissue supernatants, and bloods were determined by ELISA, whereas asprosin synthesis sites of salivary gland tissues were determined immunohistochemically.Results. The amount of asprosin from the lowest to the highest was in the order as follows: underweight, normal weight (control), overweight, and obese classes I and III. The lowest level of asprosin was detected in underweight individuals. It was also found that the interlobular striated ducts and the interlobular ducts of the submandibular and parotid salivary glands produce asprosin. According to these data, the asprosin level is related with obesity as the amount increases in accordance with increasing body mass index (BMI). On the other hand, there is also a relationship between the underweight and asprosin because the amount decreases with BMI decrease.Conclusions. Asprosin, a new adipokine, may be a novel indicator of adipose tissue mass. Therefore, we anticipate that antiasprosin preparations may be an alternative in the treatment of obesity in the future.

Yellow-winged grasshoppers (Gastrimargus musicus) were captured in the field to examine the morphology and amine immunohistochemistry of their salivary glands. Fifty-eight grasshoppers were collected, with only five being males. Eight of 53 female grasshoppers had food in their crop, and the salivary glands of those insects were significantly heavier than those of grasshoppers without food in their crops. The salivary gland of the yellow-winged grasshopper was an acinar-type gland, similar to gland descriptions for other Orthoptera. The primary secretory part of acini of each gland is composed of zymogen and parietal cells. Staining patterns indicated that serotonin and dopamine could act as neurotransmitters and/or neurohormones to stimulate the glands. The pattern of staining of serotonin in the salivary gland suggested that serotonin stimulates both zymogen and parietal cells. Only the parietal cells were positively stained with dopamine. Comparing staining of glands of grasshoppers with food in their crop with the glands of grasshoppers with empty crops suggested a reduction in staining for serotonin in the latter. The differential staining pattern suggests that these amines have different roles in the salivary gland of G. musicus. The lack of difference in structure but increased mass with feeding suggests that all glands were active, but that secretion was actively occurring only in animals with the heavier glands.

Introduction: The myoepithelioma is a rare form of salivary glands tumors. The conservative surgery is the treatment of choice. Case Report: 49 years old male with a submucosa mass of the inferior lip with over 20 years of evolution without progressive growth, bleeding or pain. The patient had no risk factors except history of smoking (34 pack-years). At physical examination the patient presented a consistent submucosa mass of the inferior lip at the right, mobile, painless and without visual signs of abnormal mucosa over the mass. An excisional biopsy was performed and the piece had a reniform shape and rubberish consistency with 30 x16 mm dimension. The histology exam revealed a myoepithelioma of minor salivary gland. Discussion: Tumors with an exclusive constitution of myoepithelial cells are rare. The majority is located at parotid gland. Conclusion: Myoepithelioma of minor salivary glands are rare and the number of reported cases is limited. The differential diagnosis with pleomorphic adenoma is important.

Background: Pleomorphic adenomas are benign salivary gland tumors predominantly arising from the superficial lobe of the parotid gland and rarely from the small salivary glands located at various locations including parotid, lacrimal duct, lip, floor of the mouth etc. Surgical excision of the tumor mass is the treatment of choice with utmost care taken to preserve the facial nerve in cases involving the parotid gland. Case details: This case series highlights three consecutive cases of pleomorphic adenoma arising from three different locations. Conclusion: Pleomorphic adenoma is the commonest salivary gland tumor characterized by diverse histomorphological features and can occur at any site where salivary tissue is present. Careful histopathological analysis should be done in all tumors arising especially in the head and neck region.

Introduction Mesenchymal tumours of submandibular gland are extremely rare. Schwannoma of the salivary glands is a particularly rare form of an extracranial neurogenic tumour. Case Report Here, we present an unusual case of schwannoma of submandibular gland in a 16 year old girl, who underwent total excision of mass with submandibular gland excision with no cranial nerve deficits. The details of the histopathologic features are present . Discussion Schwannoma of the salivary gland is a particularly rare form of an extracranial neurogenic tumour. Our case indicates good prognosis in a case of submandibular gland schwannoma after surgical excision.

Abstract Background Many viruses infect salivary glands. These include mumps, Epstein-Barr, herpes virus 6, parainfluenza, influenza, adeno virus, boca virus and others. Almost all coronavirus disease 2019 (COVID -19) infected patients carry the virus in saliva. Salivary duct epithelium were the early target cells in macaque monkeys infected with severe acute respiratory syndrome corona virus (SARS-COV). Here we present 2 COVID-19 cases with the involvement of salivary glands. Salivary gland involvement has not been reported in COVID-19. Methods We followed the COVID 19 clinical findings in a Pennsylvania long term care facility with 190 residents. Thirty tested polymerase chain reaction (PCR) positive. However, 48 were presumed infected. Eighteen likely cases were not tested due to shortage of swabs. Thirty four employees also tested positive. Two out of 48 patients aged 78 and 88 developed unilateral sialadenitis during the course of the illness. Both were Hispanic females. We studied the Clinical presentations, co-morbidities, lab and imaging results and the outcome. Results Case 1: Two days after the first confirmed case, a 88 year old Hispanic female developed fever and fatigue and tested COVID-19 positive. Fever lasted 5 days. Twenty days later the patient developed a 5x3 cm tender left parotid mass and hypoxia treated with oxygen via nasal cannula. (Table 1) Case 2: A 78 year old Hispanic female developed high fever and cough 7 days after the index case. Six days later she had persistent fever and presented with a tender 8.5x3.5 cm right submandibular mass. The patient was intubated for 3 days to protect the airway due to the size of the mass. Both made an uneventful recovery. (Table 1 and Figure 1) Conclusion New clinical findings of COVID -19 have been gradually added during the course of the pandemic. The virus is almost universally present in the saliva. In experimental Chinese macaques with SARS-COV early target cells were the salivary duct epithelium. Salivary gland inflammation and swelling should be included amongst the clinical features of COVID-19. Disclosures All Authors: No reported disclosures

Recent research efforts in oral biology have resulted in elucidation of the proteomes of major human salivary secretions and whole saliva. One might hypothesize that the proteome of minor gland secretions may show significantly different characteristics when compared with the proteomes of parotid or submandibular/sublingual secretions. To test this hypothesis, we conducted the first exploration into the proteome of minor salivary gland secretion. Minor gland secretion was obtained from healthy volunteers, and its components were subjected to liquid-chromatography-electrospray-ionization-tandem-mass-spectrometry. This led to the identification of 56 proteins, 12 of which had never been identified in any salivary secretion. The unique characteristics of the minor salivary gland secretion proteome are related to the types as well as the numbers of components present. The differences between salivary proteomes may be important with respect to specific oral functions.

An oncocytoma is a rare salivary gland neoplasm, and about 11% of oncocytomas of the salivary gland occur in the submandibular gland. This is a case report of an oncocytoma of the submandibular gland that showed strong homogenous contrast enhancement on a computed tomography (CT) scan. We summarize our case and review the imaging features of prior literature on oncocytomas. The submandibular mass showed relatively high density on the precontrast CT scan [43 Hounsfield unit (HU)]. There was no remarkable calcification. The mass showed homogenously strong contrast enhancement (mean, 162 HU; maximum, 188 HU; and minimum, 130 HU). In the case presented here, the HU levels of the submandibular oncocytoma on a contrast-enhanced CT is remarkably higher than the HU levels of the salivary gland tumors described in prior literature. If a mass of the salivary gland has strong contrast enhancement on a CT scan, the occurrence of an oncocytoma of the salivary gland may be possible, the possibility of the oncocytoma of the salivary gland may be considered.

Ectopic salivary gland tissue (ESGT) is salivary tissue located outside the major, minorand accessory salivary glands. Neoplasms rarely arise from ESGT, let alone carcinoma expleomorphicadenoma (CEPA) which is a malignant tumour that arises from the epithelialcomponent of pleomorphic adenoma. It usually occurs in the parotid gland and in lesserinstances, from submandibular gland or minor salivary glands. Herein, we report a rare caseof CEPA in right supraclavicular ESGT of a 67-year-old lady.She underwent complete surgicalexcision of the mass and is free from any recurrence to date. Brief review of pathogenesis,clinical features and treatment of ectopic CEPA are discussed. A thorough review of the Englishliterature reveals that our patient may be the second case of ectopic CEPA in the neck regionever reported to date. ABSTRACT Pleomorphic adenoma is the most common benign tumor of salivary glands. Eighty percent of pleomorphic adenomas occur in the parotid gland, 10% in the submandibular gland, and 10% in minor salivary glands and sublingual glands. In this case report, we aim to present the unusually giant presentation of submandibular gland pleomorphic adenoma and its management of a 52-year-old female patient who presented to us with a swelling in the right submandibular region for the last 5 years. Fine needle aspiration cytology (FNAC) suggested salivary gland origin. Ultrasonography of the neck revealed a well-defined large lobulated heterogeneous echotexture lesion with few cystic areas within and showing internal vascularity in the right submandibular region. Computed tomography (CT) scan revealed a large lobulated heterogeneously enhancing mass of 15 cm × 13 cm in the right submandibular region. The patient underwent right submandibular gland mass excision under general anesthesia. The patient presented good aesthetic and functional results with no clinical and radiographic evidence of recurrence. Being painless and slowly growing, some patients neglect it to such large sizes especially if surgery was a psychological setback for them which can then lead to facial disfigurement and severe pressure symptoms which may be fatal. Giant pleomorphic adenoma of the submandibular gland is a rare finding and should be diagnosed cautiously. The exposure to high doses of ionizing radiation during radiotherapy results in severe morphological and functional alterations of the salivary glands, such as xerostomia. In the present study we investigated the chronic effect of a single radiation dose of 15 Gray (Gy) limited to head and neck on rat salivary gland function (salivary secretion and gland mass) and histology. Results indicate that norepinephrine (NE)-induced salivary secretion was reduced significantly at 30, 90, 180 and 365 days after the administration of a single dose of 15 Gy of ionizing radiation compared to non-irradiated animals. The maximal secretory response was reduced by 33% at 30 and 90 days post irradiation. Interestingly, a new fall in the salivary response to NE was observed at 180 days and was maintained at 365 days post irradiation, showing a 75% reduction in the maximal response. The functional fall of the salivary secretion observed at 180 days post irradiation was not only associated with a reduction of gland mass but also to an alteration of the epithelial architecture exhibiting a changed proportion of ducts and acini, loss of eosinophilic secretor granular material, and glandular vacuolization and fibrosis. On the basis of the presented results, we conclude that ionizing radiation produces irreversible and progressive alterations of submandibular gland (SMG) function and morphology that leads to a severe salivary hypo-function.

Abstract Basal cell adenocarcinoma is an unusual basaloid tumour that usually affects the parotid and very rarely involves the submandibular or minor salivary glands. It constitutes 1.6% of all salivary gland tumours and occurs as an asymptomatic freely mobile mass with occasional lymph node metastasis. Differentiation from benign salivary gland tumours like basal cell adenoma is accomplished by determining the depth of tumour invasion and patterns of growth or infiltration. Although it is categorised as a low-grade malignancy, it has a high propensity for recurrence. Surgical excision with a wide margin is the preferred course of treatment with radiotherapy being reserved only for tumours of the minor salivary glands with a diffuse pattern of infiltration. We present here a case of basal cell adenocarcinoma with a rare presentation in the minor salivary glands of the tongue which arose from a previously treated basal cell adenoma in the same site.

Abstract Tissue-resident memory T cells (TRM) survive and persist within non-lymphoid tissues throughout the body, and are therefore subjected to an array of tissue-specific environments. Previous work showed that T cells extracted from the salivary gland were markedly dysfunctional for cytokine secretion upon ex vivo stimulation. This finding is important since the salivary gland is a site of shedding for several viruses. Cytomegalovirus (CMV) is a herpesvirus that persists in the salivary gland and is shed in saliva. Using the natural mouse pathogen murine (M)CMV, we previously showed that most MCMV-specific T cells in the salivary gland readily become TRM. Interestingly, CD8+ TRM in the salivary gland, but not T cells in the spleen, expressed both CD39 and CD73 ectoenzymes that break down extracellular adenosine triphosphate (ATP) to the immunosuppressive molecule adenosine. Expression of CD73 was dependent on TGF-β and neither CD39 nor CD73 expression was dependent on MCMV infection. Importantly, blocking the adenosine receptor A2AR during stimulation restored the function of CD8+TRM recovered from the salivary gland, resulting in IFN-γ and TNF-α production by salivary gland TRM that was comparable to T cells from the spleen. These data suggest that extracellular adenosine inhibits the function of salivary gland CD8+ TRM and that any ATP released during infection may be converted to adenosine by the TRM cells themselves, an avenue we are currently pursuing by use of mass spectrometry with and without MCMV infection.

With the increasing age of boars, the possibility of androstenone (5α-androst-16-en-3-one (AND), 5α-androst-16-en-3α-ol (α-AND), and 5α-androst-16-en-3β-ol (β-AND)) occurrence increases as well. The aim of this study was to evaluate concentrations of androstenone compounds in the saliva of boars concerning the age of animals. In total, 72 boars were evaluated (24 boars per replication). The effect of age (three different ages—152, 163, and 172 days) was observed, and (Landrace × Large White) × Pietrain genotypes were used. Chemical analysis of saliva samples was conducted by multidimensional gas chromatography/mass spectrometry (MDGC/MS). Salivary α-AND increased with age (p < 0.05), and positive correlations were found between age and submaxillary salivary gland weight (p < 0.05), age, and salivary AND concentration (p < 0.05), body weight and submaxillary salivary gland weight (p < 0.05), submaxillary salivary gland weight and salivary β-AND concentration (p < 0.05), as well as submaxillary salivary gland weight and total salivary 5α-androstenone (AND total) concentration (p < 0.001). Nowadays, animal welfare is becoming a more and more discussed topic, and pig breeding is not an exception. Specifically, the castration ban is a current issue, so it is very important to know as much as possible about compounds responsible for boar taint. Androstenone’s appearance in boars’ saliva could be one of the most important precursors for future early detection of boar taint.

Clear cell carcinoma (CCC) is a rare minor salivary gland carcinoma, comprise 0.2%–1% of all salivary gland tumors. This particularly occurs in the minor salivary glands of oral cavity and oropharynx, the palate was the most common location. CCCs are most commonly asymptomatic as an indolent, painless, submucosal mass. The prognosis of CCC is generally good, with low locoregional recurrence after primary surgical management and relatively limited metastatic potential. The recommended standard of CCC treatment is complete local resection of tumor with a 0.5 cm to 2 cm considered sufficient and curative in most cases. We report a case of a 38 years old Balinese woman presented with painless lump in the soft palate. The tumor measured around 2 × 2 cm in diameter and diagnosed as CCC of salivary glands which is a rare finding. Local resection of the tumor done with safety margin and no reconstruction post-surgery needed. The patient was followed up regularly with excellent prognosis and with no signs of recurrence. Pendahuluan: Sialolithiasis adalah penyakit umum kelenjar saliva. Gejalanya termasuk pembengkakan kelenjar yang terlibat, terutama selama makan, dan nyeri tekan, yang mungkin mereda tetapi dapat kambuh kembali. Sialolith terjadi terutama di kelenjar submandibula (80-90%) dan pada tingkat yang lebih rendah di kelenjar parotid (5-20%). Sialolith bisa tunggal atau jamak. Multipel sialolith di kelenjar submandibula jarang terjadi. Tujuh puluh dari delapan puluh persen kasus memiliki sialolith tunggal, hanya sekitar 5% pasien yang memiliki tiga atau lebih sialolith. Faktor etiopatogenesis terkait dengan pembentukan sialolith adalah obstruksi, penurunan laju aliran saliva, dehidrasi, infeksi kelenjar saliva, dan terganggunya kelarutan kristaloid. Tujuan penulisan laporan kasus ini untuk menjelaskan etiopatogenesis dan terapi kasus multipel sialolithiasis kelenjar submandibula. Laporan kasus: Seorang wanita 24 tahun datang dengan pembengkakkan dan nyeri pada submandibula kanan. Radiografi panoramik menunjukkan massa radiopak terdefinisi dengan baik dalam submandibula kanan. Interpretasi ultrasonografi menunjukkan massa tak homogen hypoechoic dengan kalsifikasi ganda. Pengangkatan kelenjar submandibula dilakukan dengan pendekatan ekstraoral. Laporan kasus ini menunjukkan Gambaran sebanyak sembilan sialolith di kelenjar submandibula, yang dihilangkan dengan pendekatan ekstraoral. Simpulan: Etiopatogenesis dari pembentukan multipel sialolithiasis pada duktus kelenjar, yaitu faktor mekanis, inflamasi, kimiawi, dan infeksi. Diperkirakan bahwa alkalin serta saliva kental yang mengandung banyak sel mukus, memiliki persentase kalsium fosfat lebih tinggi seperti pada kelenjar saliva submandibula yang mendukung pembentukan sialolith. Pengangkatan kelenjar submandibula beserta sialolith dilakukan sebagai standar baku perawatan dan dapat menghindari kekambuhan. Pasien kontrol kembali satu minggu pasca operasi dengan kondisi baik dan dijadwalkan untuk pemeriksaan radiografis ulang enam bulan kemudian untuk memastikan tidak terjadinya pembentukan sialolith baru di saluran kelenjar saliva.Kata kunci: Multipel, sialolithiasis, kelenjar submandibula. ABSTRACTIntroduction: Sialolithiasis is a common disease of the salivary glands. Symptoms include the glands inflammation, especially during eating, and tenderness, which may subside but may recur. Sialoliths occur mainly in the submandibular glands (80-90%) and to a lesser extent in the parotid glands (5-20%). Sialolith can be singular or plural. Multiple sialoliths in the submandibular gland rarely occur. Seventy out of eighty per cent of cases have a single sialolith. Only about 5% of patients have three or more sialoliths. The etiopathogenetic factors associated with sialolith formation are obstruction, decreasing salivary flow rate, dehydration, salivary gland infection, and impaired crystalloid solubility. The purpose of this case report was to describe the etiopathogenesis and treatment of multiple cases of submandibular gland sialolithiasis. Case report: A 24-year-old woman presented with inflammation and pain in the right submandibular. Panoramic radiograph shows a well-defined radiopaque mass in the right submandibular. Ultrasound interpretation revealed a hypoechoic homogeneous mass with multiple calcifications. Removal of the submandibular gland was carried out with an extraoral approach. This case report showed the appearance of as many as nine sialoliths in the sub-mandibular gland, removed by an extraoral approach. Conclusion: Etiopathogenesis of the formation of multiple sialolithiasis in the glandular duct are mechanical, inflammatory, chemical, and infectious factors. It is thought that alkaline and thick saliva, which contains many mucus cells, has a higher percentage of calcium phosphate than in the submandibular salivary glands, which support the formation of sialoliths. Submandibular gland removal along with the sialoliths is performed as the treatment standard, which can avoid recurrence. The control visit is carried out one week postoperatively in good condition, and the patient is scheduled for another radiographic examination six months after to ensure that no new sialoliths occurred in the salivary gland.Keywords: Multiple, sialolithiasis, submandibular gland.

To date limited studies have been carried out to elucidate salivary proteins ofAn. stephensisalivary glands. The aim of the present study was to provide detailed analytical attributives of functional salivary gland proteins of urban malaria vectorAn. stephensi. A proteomic approach combining one-dimensional electrophoresis (1DE), ion trap liquid chromatography mass spectrometry (LC/MS/MS), and computational bioinformatic analysis was adopted to provide the first direct insight into identification and functional characterization of known salivary proteins and novel salivary proteins ofAn. stephensi. Computational studies by online servers, namely, MASCOT and OMSSA algorithms, identified a total of 36 known salivary proteins and 123 novel proteins analysed by LC/MS/MS. This first report describes a baseline proteomic catalogue of 159 salivary proteins belonging to various categories of signal transduction, regulation of blood coagulation cascade, and various immune and energy pathways ofAn. stephensisialotranscriptome by mass spectrometry. Our results may serve as basis to provide a putative functional role of proteins in concept of blood feeding, biting behavior, and other aspects of vector-parasite host interactions for parasite development in anopheline mosquitoes.

The hematophagous behaviour emerged independently in several instances during arthropod evolution. Survey of salivary gland and saliva composition and its pharmacological activity led to the conclusion that blood-feeding arthropods evolved a distinct salivary mixture that can interfere with host defensive response, thus facilitating blood acquisition and pathogen transmission. The cat flea, Ctenocephalides felis, is the major vector of several pathogens, including Rickettsia typhi, Rickettsia felis and Bartonella spp. and therefore, represents an important insect species from the medical and veterinary perspectives. Previously, a Sanger-based sialome of adult C. felis female salivary glands was published and reported 1,840 expressing sequence tags (ESTs) which were assembled into 896 contigs. Here, we provide a deeper insight into C. felis salivary gland composition using an Illumina-based sequencing approach. In the current dataset, we report 8,892 coding sequences (CDS) classified into 27 functional classes, which were assembled from 42,754,615 reads. Moreover, we paired our RNAseq data with a mass spectrometry analysis using the translated transcripts as a reference, confirming the presence of several putative secreted protein families in the cat flea salivary gland homogenates. Both transcriptomic and proteomic approaches confirmed that FS-H-like proteins and acid phosphatases lacking their putative catalytic residues are the two most abundant salivary proteins families of C. felis and are potentially related to blood acquisition. We also report several novel sequences similar to apyrases, odorant binding proteins, antigen 5, cholinesterases, proteases, and proteases inhibitors, in addition to putative novel sequences that presented low or no sequence identity to previously deposited sequences. Together, the data represents an extended reference for the identification and characterization of the pharmacological activity present in C. felis salivary glands.

Salivary gland proteins of the human malaria vector, Anopheles dirus B were determined and analyzed. The amount of salivary gland proteins in mosquitoes aged between 3 - 10 days was approximately 1.08 ± 0.04 µg/female and 0.1 ± 0.05 µg/male. The salivary glands of both sexes displayed the same morphological organization as that of other anopheline mosquitoes. In females, apyrase accumulated in the distal regions, whereas alpha-glucosidase was found in the proximal region of the lateral lobes. This differential distribution of the analyzed enzymes reflects specialization of different regions for sugar and blood feeding. SDS-PAGE analysis revealed that at least seven major proteins were found in the female salivary glands, of which each morphological region contained different major proteins. Similar electrophoretic protein profiles were detected comparing unfed and blood-fed mosquitoes, suggesting that there is no specific protein induced by blood. Two-dimensional polyacrylamide gel analysis showed the most abundant salivary gland protein, with a molecular mass of approximately 35 kilodaltons and an isoelectric point of approximately 4.0. These results provide basic information that would lead to further study on the role of salivary proteins of An. dirus B in disease transmission and hematophagy.

Salivary gland choristoma (heterotopic salivary gland tissue) is a rare condition that occurs at various locations within the head and neck. We present a 10-year-old boy with a salivary gland choristoma of the middle ear and compare findings with the 15 similar cases published in the English and German languages. Patients typically have a long-standing conductive hearing loss and visible middle ear mass. Operative findings include a lobulated middle ear mass of histologically normal salivary gland tissue attached posteriorly in the region of the oval window, together with absent or malformed ossicles. Frequently the mass is intimately associated with the facial nerve. The constancy of these findings has led to the proposal of an abnormal developmental syndrome. This syndrome will be described and possible explanations for its cause will be discussed.

Malaria has been prevalent for a long time in tropical developing regions causing great morbidity and mortality. Among the malaria vectors, Anopheles vagus has been known as secondary malaria vector in East Java. Salivary glands of mosquitoes perform various functions for survival of the vectors and also conducive for blood feeding, harbouring of malaria parasites, and eventual parasite transmission. The salivary gland proteomes of An. vagus have not been carried out yet. The aim of our study was to identify and characterize the immunogenic proteins of salivary glands proteins of An. vagus. A proteomic approach combining one-dimensional electrophoresis (1DE) followed by western blot analysis using human sera from healthy people living in an endemic area (Kendal); liquid chromatography mass spectrometry (LC-MS/MS) and bioinformatic analysis was adopted to provide the first direct insight into identification and characterization of salivary proteins of An. vagus. Identification of immunogenic proteins using western blot analysis has revealed three immunogenic bands which had molecular weights of 69, 75 and 232 kDa. Among those proteins analysed by LC-MS/MS, there were alpha,1-4 glucan phosphorylase, putative myosin class I heavy chain which have the highest number of total spectrum count peptide. Other proteins like vitellogenin and heat shock protein 82 (Hsp82) were also identified. ABSTRACTSialoliths are the most common diseases of the salivary glands. They may occur in any of the salivary gland ducts but are most common in Wharton’s duct and the submandibular gland. This report presents clinical and radiographical signs of two unusually large sialoliths. There were painless swellings on the floor of the mouth in both cases. Radiographical examination revealed large irregular radioopaque mass superimposed right canine and premolar areas. After the lesions were removed, histopathological examination was performed and lesions were diagnosed as sialoliths. (Eur J Dent 2009;3:135-139)

ABSTRACT Carcinoma ex-pleomorphic adenoma (CaxPA) is a carcinoma arising from a primary or recurrent benign pleomorphic adenoma (PA). It presents approximately 11.6% of all malignant neoplasms of salivary gland and mostly originates from major salivary glands. It is a common histologic subtype of primary parotid malignancy. Location in the minor salivary gland is uncommon and occurs in less than 7% of all cases. The majority of CaxPA develops from epithelial component of PA. Salivary gland tumors are known to have a benign clinical behavior with unrecognized metastatic potential. It often poses a diagnostic challenge to clinicians and pathologists. The entity is difficult to diagnose preoperatively. Pathologic assessment is the gold standard for making the diagnosis. We present a case of a 55-year-old female patient who had a right preauricular mass of 10 years duration, with sudden onset of pain of duration of 1 month. ABSTRACT Background Acinic cell carcinoma (ACC) is a relatively rare and slowly growing tumor of the major salivary glands, usually arising from the parotid and rarely from the submandibular and sublingual salivary glands. ACC accounts for 3 to 4% of parotid tumors, 2 to 6% of all salivary gland tumors and 10 to 17% of all malignant salivary gland tumors. Case report A 40-year-old woman reported a slow growing mass in the oral cavity. The transoral aspiration biopsy was reported as pleomorphic adenoma arising from the minor salivary glands of the junctional zone of base of tongue. Rest of the neck examination was normal and magnetic resonance imaging (MRI) revealed the mass confined to the superficial layers of the tongue without invasion. Intervention The tumor was resected transorally with adequate margins. The tumor histology was typical of ACC with the margins free and wide on all the sides. The patient was kept on monthly follow-up where palpable nodes developed during the course of 6 months. The neck was addressed with supraomohyoid dissection and followed up without irradiation. Conclusion ACC is a low-grade malignant salivary neoplasm rarely diagnosed in minor salivary glands of the oral cavity. The overall prognosis after surgical resection depends on the extent of lesion and the adequacy of the initial resection. Hence, keeping in mind the malignant potential of the disease careful long-term follow-up is advised. Case summary A 16-year-old neutered female British Shorthair cat presented with a 5-year history of swelling lateral to the left mandible that intermittently discharged viscous, clear fluid from a small defect in the skin. CT, ultrasonography, physical characteristics and cytology of the fluid were suggestive of sialocoele. CT showed a large, cavitary, fluid-filled mass lateral to the left mandible. A ventral approach was used to resect the left mandibular, sublingual and molar salivary glands and sialocoele. Histopathology of the molar and mandibular and sublingual glands showed chronic active sialoadenitis with more severe changes in the molar gland. There were no signs of recurrence of the sialocoele 12 months after surgery. Relevance and novel information This is the first report of a cranial cervical sialocoele potentially involving the molar salivary gland in a cat. Resection of the mandibular, sublingual and molar salivary glands should be considered in cats that present with a cranial cervical sialocoele.

Patients commonly experience pain and/or swelling.Methods: A 51-year-old male came to Department of Oral and Maxillofacial Surgery with a swelling and pain at the right submandibular. The panoramic radiograph showed a radiopacity mass with well-defined in right submandibular. The Ultrasonographic interpretation showed hypoechoic inhomogeneous mass with multiple calcification.The submandibular gland removal was performed by extraoral approach.Result:Multiple sialolith in the submandibular gland are rare. A surgical approach for removal of sialolith in the submandibular gland could be intraoral or extraoral. In this case report, we described five stones in the submandibular gland. which were removed by extraoral approach.Conclusion: There are various methods available for the management of salivary stones. In this case report, removal of the submandibular gland with the stone should still be preferred as the gold standard of treatment and could avoid recurrence of this medical condition.

Adenomatous ductal proliferation/hyperplasia (ADP/H) of the salivary gland, a rare asymptomatic nonneoplastic lesion that histopathologically resembles basal cell adenoma, is typically incidentally identified in resected specimens of other salivary diseases such as tumors and chronic sialadenitis. A 70-year-old male was referred to our hospital with a 9-month history of continuous swelling in the left parotid region. A physical examination revealed a soft mass in the left parotid gland, which was identified as a cystic mass by computed tomography. A parotid tumor with cystic components was suspected, and partial parotidectomy was performed under general anesthesia. The histopathological findings were consistent with the diagnosis of ADP/H of the salivary gland. This case report emphasizes the necessity for a proper diagnosis of ADP/H of the salivary gland. Further large case series are required for a modification of the definition of ADP/H for its correct diagnosis.

Hepatocellular carcinoma (HCC), the most common primary hepatic tumor, metastasizes in more than 50% of cases. However, metastasis of HCC to the submandibular glands is very rare. HCC rarely metastasizes to the salivary glands; only six cases of metastasis to the parotid gland have been reported in the English-language literature. Moreover, only one case of metastasis of HCC to the submandibular glands has been reported to date, and the affected patient died of hepatic failure. This clinical report describes a 55-year-old man who had undergone two surgeries for liver cancer and subsequently presented with a 2-week history of a right submandibular mass. We performed dissection of the right submandibular triangle, and metastatic HCC of the submandibular gland was diagnosed. Although HCC metastases to the oral cavity have been reported, this is only the second case of HCC metastasis to the submandibular gland. Fine-needle aspiration biopsy is recognized as a simple, direct approach for morphologic diagnosis of salivary gland lesions; however, histological and immunohistochemical examination of the surgical specimen remains necessary.

A case of Warthin's tumour originating from heterotopic salivary gland tissue of the upper neck is reported. The radioactive uptake of 131I, evidenced in the neck mass in its pre-diagnostic assessment, suggested a diagnosis of cervical node involvement from a primary malignant thyroid neoplasm. A critical review of the literature on heterotopic salivary gland tissue neoplasms of the head and neck is also presented.

BACKGROUND: Secretory carcinoma (SC) of the salivary gland, also known as mammary analog secretory carcinoma, is a rare tumor in the parotid gland. This kind of tumor is characterized by generally indolent clinical behavior and expression of a break in the ETV6 gene. CASE REPORT: We present a unique case of secretory carcinoma and show its favorable prognoses. CONCLUSION: Secretory carcinoma of the salivary gland is a low-grade carcinoma with a favorable prognosis. It has low regional lymph node and distant metastasis potential. Due to the possibility of misdiagnosis, immunohistochemical studies and FISH are suggested. The most effective treatment is complete surgical excision with negative surgical margins.

ABSTRACT Non-Hodgkin's lymphomas constitute 25 to 40% of all lymphomas. The gastrointestinal tract is the most common site followed by the head and neck area. Salivary gland involvement is a very rare entity and is involved in 0.5 to 5% of all cases. Among all the major salivary glands, the parotid gland is commonly involved followed by the submandibular glands. Misdiagnosis often leads to unnecessary diagnostic procedures, which leads to delay in initiation of appropriate treatment. It is often difficult to diagnose the distinction between lymphoma developing primarily in the parotid gland tissue and in the intraparotid lymph nodes. As per the reports, primary tumors of the parotid gland show no characteristic features on diagnostic imaging, reflecting none of their histological findings. We represent a case of a 61-year-old female who presented with painless, gradually increasing mass in the right parotid region. Imaging studies were suggestive of an infiltrative mass lesion involving right the parotid gland. The patient underwent superficial parotidectomy along with the excision of the part of the deep lobe posterior to submandibular gland with facial nerve preservation. Histopathology of the resected specimen shows infiltrates of lymphocytes into the glandular parenchyma destroying its normal lobular organization and disrupting the normal glandular architecture. The World Health Organization (2005) defines an epithelial-myoepithelial carcinoma (EMC) as a malignancy composed of two cell types that typically form duct-like structures.1 We present herein an archival case from the parotid gland. EMC occurs primarily in the major salivary glands particularly in the parotid where it presents as a painless, slow-growing mass.1 Microscopic examination shows bi-layered tubular duct-like structures with pale to clear areas (Figure 1). The inner luminal layer is composed of cuboidal cells that are of epithelial derivation while the outer layer is composed of polygonal cells that are of myoepithelial derivation (Figures 2 and 3). The latter typically have abundant clear cytoplasm.1,2 The epithelial-myoepithelial dualism is confirmed using immunohistochemical stains; the epithelial cells being immunoreactive for low molecular weight keratin and the myoepithelial cells for S-100 protein, muscle specific actin, vimentin and p63.1, 3 EMC is primarily a tumor of adulthood with peak incidence in the sixth and seventh decades. First described by Donath et al. in 1972,3 they are rare salivary gland neoplasms with an incidence of less than 1% arising mainly in the parotid gland4 although they have been documented in the lungs.5 Perineural and vascular invasion are frequent and recurrence occurs in around 40% of cases and metastasis in 14%.1 Although thought to be of low-grade malignancy, fatal courses have been described4 and “analysis of the various series have demonstrated that tumors with a solid growing pattern, nuclear atypia, DNA aneuploidy and high proliferative activity, generally have a more aggressive behavior and a higher frequency of local recurrences and metastases.”3

Salivary gland homogenates of the sand fly Phlebotomus papatasi contain large amounts of adenosine and 5′-AMP, of the order of 1 nmol per pair of glands, as demonstrated by liquid chromatography, ultraviolet spectrometry, mass spectrometry and bioassays. These purines, 75–80 % of which are secreted from the glands following a blood meal, have vasodilatory and anti-platelet activities and probably help the fly to obtain a blood meal. Salivary 5′-AMP is also responsible for the previously reported protein phosphatase inhibitor in the salivary glands of P. papatasi, which is shown to be artifactual in nature as a result of allosteric modification by AMP of the phosphatase substrate used (phosphorylase a).

Salivary gland homogenates from adult female Anopheles albimanus mosquitoes relaxed aortic rings preconstricted with noradrenaline (NA). This relaxation is slow and is due to destruction of NA. Incubation of NA with the homogenate yielded a product with a spectrum consistent with the corresponding adrenochrome. Oxidation of NA was enhanced by a superoxide generation system and inhibited by the combined action of superoxide dismutase and catalase. Additionally, peroxidase activity on both synthetic (o-dianisidine) and biologically active (serotonin) substrates was also present in the salivary gland homogenates, this latter activity requiring hydrogen peroxide. Noradrenaline oxidation, serotonin and o-dianisidine peroxidation and vasodilation all co-elute with a heme protein of relative molecular mass 50,000, as determined by molecular sieving chromatography. Peroxidase activity was localized in the posterior (female-specific) lobes of salivary glands and was also detected in nitrocellulose membranes probed by hungry mosquitoes. Protein and peroxidase activities were significantly lower in salivary glands of mosquitoes after probing and feeding on blood. It is suggested that adult female Anopheles albimanus mosquitoes contain a salivary heme peroxidase that functions during blood finding and blood feeding by destroying hemostatically active biogenic amines released by the vertebrate host during tissue destruction.

Salivary gland choristoma is an extremely rare middle ear pathology. We present the case of a 10-year-old girl with unilateral conductive hearing loss. Tympanotomy showed a nonspecific middle ear mass, absence of stapes, anomaly of incus, and displaced facial nerve. It was not possible to remove the mass completely. Histology confirmed salivary gland choristoma. The hearing in this case can be improved with a bone-anchored hearing aid.

Background: Mucous Retention Cyst (MRC) are non-neoplastic, developmental salivary gland lesion. Although more common in the minor salivary gland, few cases have been reported in the major salivary gland. Objective: To draw the attention of clinicians to a rare occurrence of MRC in the submandibular gland bilaterally. Case Report: The index case presents as a slowly progressive painless lesion of 10 years duration associated with aesthetics concerns. The lesion appeared as a double jaw with extension into the neck. Though lobulated and fluctuant, aspirate yielded yellow coloured, non-viscous and non-foul smelling fluid. Ultrasonography revealed a multilobulated mass with mobile heterogeneous fluid-like content in the submental and submandibular regions bilaterally. The massive size of the lesion limited the ultrasonic visualization of the submandibular salivary glands. The patient could not afford advanced imaging. During surgery, the lesion had involved the submandibular gland bilaterally with attachment to the hyoid bone and in close relation to the recurrent laryngeal nerve. Although the nerve was preserved, the patient experienced temporary hoarseness, which resolved with medication. Histopathologic examination of the excised lesion revealed the lesion as Mucous Retention Cyst. Conclusion: The massive size of the lesion, delayed presentation and financial constraints in obtaining advanced imaging could have contributed to the diagnostic dilemma. This rare case will help the clinicians include MRC as a differential diagnosis of lesions involving submandibular salivary gland bilaterally. It also draws the attention of the maxillofacial surgeons to the unexpected injury to the recurrent laryngeal nerve and its management in a low-income setting environment.

Pleomorphic adenoma is a benign salivary gland neoplasm most commonly seen in major salivary glands. It’s composed of several epithelial and myoepithelial tissue elements. Pleomorphic adenoma of the lacrimal gland is a rare entity and accounts for approximately 12–25% of all lacrimal tumours. Of these 90% are seen in the orbital part and only 10% are present in the palpebral part of the lacrimal gland. We present a case report of a 33 years old female with a complaint of painless swelling in the right upper eyelid which was gradually increasing in size. A provisional diagnosis of a lacrimal gland neoplasm was made. The lacrimal mass was resected with enbloc enucleation via supratarsal fold approach. On histopathaological evaluation it showed typical morphological features of pleomorphic adenoma. After 12 months of enucleation, patient recovery was satisfactory without any signs of recurrence. This case is presented for its rarity and it highlights the need for proper diagnosis and treatment plan in the cases of benign lacrimal gland tumours.

The low-molecular-mass human salivary mucin has at least two isoforms, MG2a and MG2b, that differ primarily in their sialic acid and fucose content. In this study, we characterize further these isoforms, particularly their peptide moieties. Trypsin digests of MG2a and MG2b yielded high- and low-molecular-mass glycopeptides following gel filtration on Sephacryl S-300. The larger glycopeptides from MG2a and MG2b had similar amino acid compositions and identical N-terminal sequences, suggesting common structural features between their peptides. An oligonucleotide probe generated from the amino acid sequence of the smaller glycopeptide from MG2a was employed in Northern-blot analysis. This probe specifically hybridized to two mRNA species from human submandibular and sublingual glands. A cDNA clone selected from a human submandibular gland cDNA expression library with antibody generated against deglycosylated MG2a also hybridized to these two mRNA species. In both cases, the larger mRNA was polydisperse, and the hybridization signal was more intense in the sublingual gland. In addition, the N-terminal amino acid sequence of the larger glycopeptide was found to be part of one of the selected MG2 cDNA clones.

Marine biodiversity has been yielding promising novel bioproducts from venomous animals. Despite the auspices of conotoxins, which originated the paradigmatic painkiller Prialt, the biotechnological potential of gastropod venoms remains to be explored. Marine bioprospecting is expanding towards temperate species like the dogwhelk Nucella lapillus, which is suspected to secrete immobilizing agents through its salivary glands with a relaxing effect on the musculature of its preferential prey, Mytilus sp. This work focused on detecting, localizing, and testing the bioreactivity of cysteine-rich proteins and peptides, whose presence is a signature of animal venoms and poisons. The highest content of thiols was found in crude protein extracts from the digestive gland, which is associated with digestion, followed by the peribuccal mass, where the salivary glands are located. Conversely, the foot and siphon (which the gastropod uses for feeding) are not the main organs involved in toxin secretion. Ex vivo bioassays with Mytilus gill tissue disclosed the differential bioreactivity of crude protein extracts. Secretions from the digestive gland and peribuccal mass caused the most significant molecular damage, with evidence for the induction of apoptosis. These early findings indicate that salivary glands are a promising target for the extraction and characterization of bioactive cysteine-rich proteinaceous toxins from the species.

Heterotopia is defined as the presence of mature, histologically normal, tissue in unusual anatomic sites. When this heterotopic tissue forms a mass, it is called a choristoma. This case series describes 3 cases of gastroesophageal junction (GEJ) salivary heterotopias. While heterotopias are usually incidental findings, choristomas can clinically and endoscopically mimic carcinomas and might lead to unnecessary procedures for the patients. Clinicians should therefore be aware of this entity. Literature search, however, failed to show any reports of salivary gland heterotopias in the GEJ. In fact, literature review revealed only 6 reported cases of salivary gland choristoma in the gastrointestinal tract, none at the GEJ. In this case series, we report 2 cases of salivary gland heterotopia and one case of salivary gland choristoma arising at the GE junction. To our knowledge, this is the first series of its kind in the literature.

The diagnostic work-up of primary Sjögren’s syndrome (pSS) includes quantifying saliva and tear production, evaluation of autoantibodies in serum and histopathological analysis of minor salivary glands. Thus, the potential for further utilizing these fluids and tissues in the quest to find better diagnostic and therapeutic tools should be fully explored. Ten samples of saliva and tears from female patients diagnosed with pSS and ten samples of saliva and tears from healthy females were included for lipidomic analysis of tears and whole saliva using high-performance liquid chromatography coupled to time-of-flight mass spectrometry. In addition, lipidomic analysis was performed on minor salivary gland biopsies from three pSS and three non-SS females. We found significant differences in the lipidomic profiles of saliva and tears in pSS patients compared to healthy controls. Moreover, there were differences in individual lipid species in stimulated saliva that were comparable to those of glandular biopsies, representing an intriguing avenue for further research. We believe a comprehensive elucidation of the changes in lipid composition in saliva, tears and minor salivary glands in pSS patients may be the key to detecting pSS-related dry mouth and dry eyes at an early stage. The identified differences may illuminate the path towards future innovative diagnostic methodologies and treatment modalities for alleviating pSS-related sicca symptoms.

Chronic sclerosing sialadenitis, also known as Küttner’s tumour, is a chronic inflammatory condition affecting the salivary glands. We present the case of an arteriovenous vascular malformation (AVM) of the right submandibular gland mimicking a Küttner’s tumour on ultrasound scan in a 63-year-old woman. Examination revealed a 2 cm firm, mobile, non-pulsatile, right-sided submandibular mass with no overlying skin changes but with mild tenderness to palpation. Histological appearances revealed an AVM associated with localised fatty metaplasia and glandular atrophy. Although rare, AVMs should be considered a differential diagnosis for a submandibular gland mass that gives unclear or unusual radiological appearances.
