The objective of the study was to describe and explain variations in rates of hospital admissions for long-term complications of diabetes mellitus in rural and urban Victoria as an indicator of the adequacy of ambulatory care services. The Victorian Inpatient Minimum Database (VIMD), Health Insurance Commission data for 1998, Medical Labour Force Annual Survey 1998, Socioeconomic Indexes for Areas 1996 (SEIFA) and Accessibility/Remoteness Index of Australia (ARIA) were merged to determine the extent to which hospitalisation for complications of diabetes can be predicted from accessibility and utilisation of general practitioner services. The rural and urban differentials for long-term diabetic complications and their strong relationship with GP services, the degree of remoteness, lack of insurance, and Aboriginality reflect issues related to equity and access, patient and GP education, and inclination to seek care, all of which have implications for planning of primary health services in rural areas. This study describes a model for the analysis of ambulatory care sensitive conditions, and illustrates the important use of routine databases combined with other sources of information in quantifying the impact of factors related to primary care services.

AbstractIntroduced into Lake Victoria in the 1950s, the Nile perch has gained fame for prompting rapid regional economic growth and for driving scores of endemic fish species into extinction. This study uses oral and archival data to trace the historical development of the Nile perch fishery on Lake Victoria. Particular emphasis is placed on local responses and adaptations to (1) the Nile perch itself; (2) the abrupt integration of the Lake Victoria fishery with the global economy; and (3) the ecological changes that the Nile perch has precipitated. I also attempt to situate Lake Victoria's history in the larger debate about environment and African livelihoods. Because so much of Lake Victoria's species diversity has been lost within one generation – biologist E. O. Wilson (1992) has called this process ‘the most catastrophic extinction episode of recent history’ – the lake is an ideal case study with which to examine ‘local’ perceptions of biodiversity. The data suggest that species diversity is important and highly resolved in the worldviews of Lake Victoria's fishermen; yet, although the will for conservation is present, poverty obstructs its realization. These findings are discussed in relation to other work on indigenous environmental knowledge and ecological ethics. I argue that ‘intrinsic’ valuation of species diversity and ecological processes may be more widespread in rural societies than has traditionally been assumed by natural and social scientists, and that the preponderance of social studies highlighting oppositions between Western science and ethno-science, and between conservation concerns and local livelihoods, may have blinded us to synergies between them. More effort is needed to understand fully the nuances in these complex local ecological worldviews, perhaps via ‘social histories of extinction’ that explore the local consequences of species loss.

Background. Statutory registration of Chinese Medicine (CM) practitioners was introduced in Victoria in 2000. The application assessment process for those who were granted registration during the transitional period (2002–04) was resource intensive, as little was known about their age, education, practice and language proficiency. This study offers insights that may be useful for the planning of national registration to commence in 2012. Methods. Data were extracted from registration application forms submitted to the Chinese Medicine Registration Board of Victoria (CMRB) between 2002 and 2004, using pre-defined data collection forms. Results. In 2006, 639 ‘grandparented’ Victorian CM practitioners had been registered, with a median age of 44 years old (range 23–86). There was a higher proportion of younger female, English-speaking, acupuncturists v. a higher proportion of older male, non-English-speaking, Chinese herbalists. There were few CM practitioners in rural areas, particularly herbalists. More than one-third of practitioners had obtained qualifications overseas and almost half of these practitioners provided no evidence of past study in professional issues and medical ethics. Conclusions. Ageing, diversity in qualifications and training, English proficiency, and level of study in professional issues and medical ethics represent major challenges for the implementation of CM national registration in 2012. What is known about the topic? Statutory registration of Chinese Medicine (CM) practitioners was introduced in the state of Victoria in 2000. The process of registering practitioners during the transitional period was resource intensive, because of the diverse background of the workforce. In May 2009, Health Ministers of all States and Territories and the Commonwealth agreed to include the CM profession, from 1 July 2012, in the National Registration and Accreditation Scheme for the health professions. What does this paper add? This paper, based on data from the registration application forms submitted to the Chinese Medicine Registration Board of Victoria (CMRB) between 2002 and 2004, provides a demographic and geographic profile of the 639 Victorian CM practitioners grandparented under the transitional arrangements of the Chinese Medicine Registration Act 2000. This study offers insights that may be useful for the planning of national registration for the Chinese Medicine profession. What are the implications for practitioners? With the introduction of national registration for the CM profession, this study provides critical data for developing effective strategies to implement the grandparenting process in all states and territories in Australia. Particularly, data collected in this study will help to deal with assessing knowledge in ethics and the healthcare system, biomedical sciences and language proficiency as part of the assessment process for a substantial number of applicants during the national registration of CM practitioners.

Objective In 2015, the Victorian Department of Health and Human Services commissioned the Victorian Allied Health Workforce Research Program to provide data on allied health professions in the Victorian public, private and not-for-profit sectors. Herein we present a snapshot of the demographic profiles and distribution of these professions in Victoria and discuss the workforce implications. Methods The program commenced with an environmental scan of 27 allied health professions in Victoria. This substantial scoping exercise identified existing data, resources and contexts for each profession to guide future data collection and research. Each environmental scan reviewed existing data relating to the 27 professions, augmented by an online questionnaire sent to the professional bodies representing each discipline. Results Workforce data were patchy but, based on the evidence available, the allied health professions in Victoria vary greatly in size (ranging from just 17 child life therapists to 6288 psychologists), are predominantly female (83% of professions are more than 50% female) and half the professions report that 30% of their workforce is aged under 30 years. New training programs have increased workforce inflows to many professions, but there is little understanding of attrition rates. Professions reported a lack of senior positions in the public sector and a concomitant lack of senior specialised staff available to support more junior staff. Increasing numbers of allied health graduates are being employed directly in private practice because of a lack of growth in new positions in the public sector and changing funding models. Smaller professions reported that their members are more likely to be professionally isolated within an allied health team or larger organisations. Uneven rural–urban workforce distribution was evident across most professions. Conclusions Workforce planning for allied health is extremely complex because of the lack of data, fragmented funding and regulatory frameworks and diverse employment contexts. What is known about this topic? There is a lack of good-quality workforce data on the allied health professions generally. The allied health workforce is highly feminised and unevenly distributed geographically, but there is little analysis of these issues across professions. What does this paper add? The juxtaposition of the health workforce demographics and distribution of 27 allied health professions in Victoria illustrates some clear trends and identifies several common themes across professions. What are the implications for practitioners? There are opportunities for the allied health professions to collectively address several of the common issues to achieve economies of scale, given the large number of professions and small size of many.

Increased temperatures provide optimal conditions for pathogen survival, virulence and replication as well as increased opportunities for human–pathogen interaction. This paper examined the relationship between notifications of cryptosporidiosis and temperature in metropolitan and rural areas of Victoria, Australia between 2001 and 2009. A negative binomial regression model was used to analyse monthly average maximum and minimum temperatures, rainfall and the monthly count of cryptosporidiosis notifications. In the metropolitan area, a 1 °C increase in monthly average minimum temperature of the current month was associated with a 22% increase in cryptosporidiosis notifications (incident rate ratio (IRR) 1.22; 95% confidence interval (CI) 1.13–1.31). In the rural area, a 1 °C increase in monthly average minimum temperature, lagged by 3 months, was associated with a 9% decrease in cryptosporidiosis notifications (IRR 0.91; 95% CI 0.86–0.97). Rainfall was not associated with notifications in either area. These relationships should be considered when planning public health response to ecological risks as well as when developing policies involving climate change. Rising ambient temperature may be an early warning signal for intensifying prevention efforts, including appropriate education for pool users about cryptosporidiosis infection and management, which might become more important as temperatures are projected to increase as a result of climate change.

Most women use family planning services during their reproductive lifetime, but many lack ready access to such services, particularly in a rural area. The aim of this study was to document and thus develop an understanding of the facilitators and barriers to accessing three types of family planning services (emergency contraception, termination of pregnancy, and options counselling) within a particular rural area of Victoria, Australia, and how these might affect women’s psychosocial health and their ability to make timely decisions about continuation of a pregnancy.

The systems approach to planning has been used to provide a context for examining the past and emerging policy base, both explicit and implicit, for the introduction and administration of planning controls in the Shire. The impact of land use controls on agriculture and rural land use were examined in physical, economic and social terms using a range of information including Council records, Australian Bureau of Statistics (ABS) census data and the results of a survey of farmers and small lot owners which was undertaken as part of this research.

In recent times, with regards to rural water supplies in Victoria and most other parts of Australia, emphasis is placed more on efficient use of existing water resources than on new resource developments. This is especially true for irrigation systems. This emphasis is mainly due to limited funds available for construction works, lack of suitable hydrologic sites, and the spirited and justifiable lobbying of enviromnental groups against construction of new projects. Hence to meet the increased water demands, efficient operation of existing irrigation systems is required. Recently with the high computational power of personal computers, irrigation scheduling based on an accounting of soil water budget components has become popular around the world. This research project focuses on such irrigation scheduling techniques, with the aim of improving the efficiency of water use, thus leading to conservation of water resources.
