Vigabatrin is a second-generation antiepileptic drug characterized by a few antiepileptic indications, with a very good interaction profile in polytherapy. Clinically relevant adverse effects (especially visual field defects) have reduced its use as antiepileptic drug in routine clinical practice considerably. Vigabatrin has an acceptable behavioural tolerability profile, although patients with epilepsy treated with this medication can report depression, psychosis, and irritability. Risk factors for developing psychiatric adverse effects include high starting and maintenance doses, past psychiatric history, and epilepsy severity. Vigabatrin has no approved indications or clinical uses in psychiatry, despite weak evidence for its usefulness in the treatment of anxiety disorders and addictions.

Psychological factors related to sport concussion have been overshadowed by interests in neurocognitive recovery. This chapter begins by examining psychological factors relevant to research and management of sport concussion in the context of a culture where normalizing pain and injury is routine. Among the key components of this chapter is a discussion of emotional disturbance following concussion characterized as the “concussion crevice,” which is represented by high fatigue, low vigor, elevated depression and confusion scores, and high overall emotional distress. This differs from pre-injury “iceberg” profiles of high energy, and low depression, fatigue, and confusion, which is characteristic of mentally healthy athletes. Acute emotional response to concussion is distinguished from response to musculoskeletal injury, mirrors neurocognitive recovery, and appears to correspond with the dynamic neurometabolic restoration pattern described in the literature. Directions for future research are recommended.

Liver failure is common and carries high morbidity and mortality. Liver transplantation (LT) is the only definitive treatment available performed as an emergency in acute liver failure and electively for chronic liver disease. In the last 50 years, a number of extracorporeal liver support devices and modifications have emerged , some of them purely mechanical in nature aimed at detoxification, while others are cell based systems possessing bio-transformational capability. Mechanical devices are mainly based on albumin dialysis, albumin being a key transporter protein that is severely deficient and irreversibly destroyed in liver diseases. Despite a sound scientific rationale and good safety profile, none of the currently available devices have shown enough promise to be incorporated in routine clinical practice, their use being limited to specific clinical situations. This chapter describes currently available devices, their operational characteristics, current evidence of their utility and limitation, and the future developments in the field of extracorporeal liver support.

Trafficking is a form of transnational crime that involves the illicit movement of goods and people around the world. Such global criminal markets take a variety of forms, and this book reviews six of them: trafficking in drugs, humans, wildlife, diamonds, arms, and antiquities. While there is a healthy literature on many of these types of trafficking, there is relatively little written that systematically compares and contrasts them. In doing that, this book allows us to lift the viewpoint above the details of each individual type of trafficking, to think theoretically about what they have in common. The book therefore serves two purposes. First, it is a primer and review of the main points of what we currently know about how each trafficking market works: who the traffickers are, what routines and structures are involved, what harm is caused, and the main types of regulation and control that attempt to constrain trafficking. Second, the text sets out a social theory of transnational markets, constituted and illustrated throughout by the empirical data reviewed. That theory ties the criminal practices of traffickers into the wider social promotion of a business-like mindset. This allows individuals and groups to compartmentalise the emotional and moral implications of illegal entrepreneurial profit generation, so that harmful action is seen as ‘just business’. As such, trafficking is rationalised by participants as comparable to the perceived amoral economic calculations of conventional business.

During the past decade, traumatic brain injury (TBI) has become a frequent topic in the media. It has been a decade of expanding awareness, increased research, and growing concern about TBI of all severity levels. Consistent with this increased attention, researchers and policymakers have made strides toward greater understanding of the risks of TBI, the scope and complexity of the symptom profiles seen after TBI, and the types of treatments that optimize recovery. Recent studies have confirmed a 50 to 60% prevalence of TBI among prisoners. Most have experienced multiple injuries and experienced their first TBI in their mid-teens. Routine screening for TBI is rarely done in these settings in spite of there being a number of tested instruments available. The cognitive deficits associated with mild to moderate TBI are often indistinguishable from those associated with many mental illnesses and substance abuse. Etiology is difficult to establish; nevertheless, the common symptom patterns often make adjustment to jail or prison difficult. Educational interventions designed to improve staff knowledge of the prevalence of TBI and frequent symptom patterns are important first steps. Training staff how to modify their behavior and facilitate communication with inmates expressing these symptoms may reduce episodes of misunderstanding and potential aggression. Similarly, current programming may be modified to accommodate the cognitive deficits suffered by inmates with TBI as well as other disorders. This chapter reviews the prevalence of TBI in correctional settings, its impact on co-occurring mental illness and substance use, and opportunities to recognize, intervene, and treat patients with TBI.

It is important for rheumatologists to have an understanding of biochemical tests including an awareness of their limitations. The biological variability of an analyte both within and between individuals, the limitations of the measurement technology, the sensitivity of laboratory internal quality control and external quality assurance procedures, as well as interlaboratory variations in practices including sample collection procedures, may all impact on the interpretation of a result. Biochemical tests are often requested to monitor organ-specific dysfunction arising as an adverse consequence of pharmacotherapy or as a component of a systemic rheumatic disease, although dysfunction may also reflect infection or coincidental pathology. Patients with rheumatic diseases are at high risk of renal and hepatic disease. Serum creatinine and its derivative estimated glomerular filtration rate (eGFR) are the most readily available surrogate markers of GFR and are used to assess renal impairment and monitor its course. However, the use of creatinine alone lacks sensitivity and a substantial loss of function must occur before creatinine levels are increased. Additional biochemical screening for kidney damage can be performed by assessment of glomerular integrity, including proteinuria or albuminuria and haematuria. A wide spectrum of rheumatic diseases can affect the liver with various degrees of involvement and hepatic pathology. These often present with cholestatic or hepatitic biochemical profiles. The medical management of rheumatic diseases also involves medications that are hepatotoxic, and routine monitoring of liver function is recommended. This approach is not problem-free and may be improved by quantitative determinations of non-invasive markers of liver fibrosis in the future. Together with imaging techniques, biochemical tests play an important role in the assessment and differential diagnosis of metabolic bone disease.

From their limited use in China during World War II, for example, to their often clandestine use in Vietnam ferrying supplies before the war escalated in 1964 and 1965 when their role became more prominent-and public-private military contractors (PMCs) have played made essential contributions to the success and failures of the military and United States. Today, with an emphasis on force restructuring mandated by the Pentagon, the role of PMCs, and their impact on policy-making decisions is at an all time peak. This work analyzes that impact, focusing specifically on PMCs in Iraq since the fall of Saddam Hussein in 2003. Isenberg dissects their responsibilities, the friction that exists between contractors and military commanders, problems of protocol and accountability, as well as the problems of regulation and control that PMC companies create for domestic politics. Isenberg organizes his work thematically, addressing all facets of PMCs in the current conflict from identifying who the most influential companies are and how they got to that point, to the issues that the government, military, and contractors themselves face when they take the field. He also analyzes the problem of command, control, and accountability. It is no secret that PMCs have been the source of consternation and grief to American military commanders in the field. As they work to establish more routine protocols in the field, however, questions are also being raised about the role of the contractors here at home. The domestic political arena is perhaps the most crucial battleground on which the contractors must have success. After all, they make their corporate living off of taxpayer dollars, and as such, calls for regulation have resonated throughout Washington, D.C., growing louder as the profile of PMCs increases during the current conflict.

Antenatal and intrapartum fetal monitoring aim to identify the beginning of the process of fetal hypoxia before irreversible fetal damage has taken place. Fetal movement counting by the mother has not been reported to be of any benefit. The biophysical profile score, incorporating ultrasound and fetal heart rate monitoring, has not been proven to reduce perinatal mortality in randomized trials. Doppler ultrasound allows the exploration of the perfusion of different fetal organ systems and provides data on possible hypoxia and fetal anaemia. Maternal uterine artery Doppler can be used to select women with a high risk for intrauterine growth restriction and pre-eclampsia but does not directly provide information on fetal status. Umbilical artery Doppler has been shown to reduce perinatal mortality significantly in high-risk pregnancies (but not in low-risk women). Adding middle cerebral artery Doppler to umbilical artery Doppler does not increase accuracy for detecting adverse perinatal outcome. Ductus venosus Doppler demonstrates moderate value in diagnosing fetal compromise; it is not known whether its use adds any value to umbilical artery Doppler alone. Cardiotocography (CTG) reflects the interaction between the fetal brain and peripheral cardiovascular system. Prelabour routine use of CTG in low-risk pregnancies has not been proven to improve outcome; computerized CTG significantly reduces perinatal mortality in high-risk pregnancies. Monitoring the fetus during labour with intermittent auscultation has not been compared to no monitoring at all; when compared with CTG no difference in perinatal mortality or cerebral palsy has been noted. CTG does lower neonatal seizures and is accompanied by a statistically non-significant rise in caesarean delivery. Fetal blood sampling to detect fetal pH and base deficit lowers caesarean delivery rate and neonatal convulsions when used in adjunct to CTG. Determination of fetal scalp lactate has not been shown to have an effect on neonatal outcome or on the rate of instrumental deliveries but is less often hampered by technical failure than fetal scalp pH. Analysis of the ST segment of the fetal ECG (STAN®) in combination with CTG during labour results in fewer vaginal operative deliveries, less need for neonatal intensive care, and less use of fetal blood sampling during labour, without a change in fetal metabolic acidosis when compared to CTG alone.
