Повний текст джерелаBreast radiotherapy, performed after breast-conserving surgery, is a critical step in the treatment ofbreast cancer. Although radiotherapy significantly reduces the risk of cancer recurrence, clinicalexperience reveals a wide variability in visible side effects during treatment. To gain a deeperunderstanding of these side effects, a clinical study was conducted by the "Institut de CancérologieStrasbourg Europe" (ICANS), collecting surface data to analyze the evolution of the breast duringradiotherapy. This thesis introduces a novel approach to model the evolution of the treated breastshape during radiotherapy, exploiting the valuable yet complex ICANS clinical trial dataset. Themethodologies and findings presented in this thesis demonstrate the potential of munctional mapbased correspondence approaches to assist clinicians in modeling geometric changes in the bustand breast during radiotherapy. Additionally, an estimation of the dosimetric impact of thesechanges has been studied, with a particular focus on the skin region, offering motivation for futureresearch in this field

Повний текст джерелаThis works aims at characterizing radioresistant cells within human breast cancer development that is responsible for treatment failure and cancer recurrences. Although the literature is flourishing with papers tightly linking the presence of "Cancer Stem Cells" to cancer treatment resistance, the intrinsic radioresistance of those "CSC" and the mechanism involved have yet to be established.Dr. Weinberg and his team have developed an in vitro cell model that produces mammary tumor cells noted « CD24-CD44+ » after an epithelial-to-mesenchymal transition (EMT) induced by TGF-β1. This model is based on healthy human mammary cells that have been immortalized and transformed.Within this context, my Ph.D. project has focused on using this new model in order to compare the radiosensibility of two cell lines: the « CD24-CD44+ » cells and the « CD24+CD44- » one. Underlying this choice is the fact this model allows for a comparison of two cellular populations at distinct stage of the tumor’s development.This work has shed light on apoptotic and detoxification mechanism involved in the radio resistant behavior of the « CD24-CD44+ » cells. After a brief introduction of key concepts required to the understanding of this work, this manuscript will begin by presenting the characterization of the chosen model, then a study of the radiation response that enabled a first description of the « CD24-CD44+ » cell radioresistant phenotype through a mild stop at the G2/M stage of the cell cycle, the presence of polypoid cells and a high progeny generation ability after exposure to radiation.Furthermore, this works shows implications of apoptotic mechanism of « CD24-CD44+ » cells with a radioresistance phenotype. Hence, we were able to show that reduced cell death observed for the « CD24-CD44+ » cells is linked to a lower activation of apoptotic pathways.Finally, the last part will present detoxification mechanism involved in « CD24-CD44+ » radioresistance phenotype, showing an altered transcriptional signature of two detoxication genes SOD2 and HMOX1 after exposure to radiation

Повний текст джерелаWith around 25,000 cases in 2015, breast cancer is the most common cancer among Canadian women. In term of mortality, breast cancer is second (13.8%), only behind lung cancer (26.5%). As part of the therapeutic approaches, around 70% and 30% of women with advanced and early breast cancer (respectively) will chose to have a total mastectomy. Following a mastectomy, few options of breast reconstruction are offered to the patients. More than 70% of breast reconstructions are implant-based. Among the complications following this surgery, capsular contracture (CC) is one of the most common. CC is defined as the abnormal, and sometimes painful, contraction of the fibrous capsule that normally grows aroung breast implants, resulting in a aesthetic failure and associated with a higher surgical revision rate. Many studies have demonstrated that the incidence of CC was higher among women who had undergone radiotherapy prior to their implant-based breast breast reconstruction procedure. Most of published studies focus on the influence of radiotherapy on capsular tissue at the immune-histological level but no study has described its influence on capsular architecture. As our team recently demonstrated that capsular ultrastructure is influenced by many factors (prosthetic texture, expansion timing). The main purpose was to describe the impact of radiotherapy on periprosthetic capsule ultrastructural features by comparing irradiated and non-irradiated capsules using scanning electron microscopy (SEM). We observed a significant loss of tridimensional texture at the implant-capsule interface in irradiated capsular samples. This feature was also found to be associated with CC. Finally, our results tend to show that radiotherapy impaires the stability of capsules by preventing them to grow withing their surrounding implant texure. In conclusion, the present research project offers the first description of the capsular response following exposiong to radiations. Even though our results do not allow us to conlude to a clear relation between loss of capsular architecture and a higher risk of CC, we think that the impairement of capsular stability might be part of the answer
