This chapter is concerned with a single consideration: the degree of proportionality between a light signal and its resulting electrical output. This is formally referred to as linearity, which depends on the suitability of the chosen PMT and the mode of operation (pulsed or analogue). Applications fall into two groups: analogue operation (DC) and transient applications. Linearity in a pulsed mode of operation concerns both pulse height (charge) and the rate of events. Generally, in the DC mode, only the mean anode current is relevant. Methods for determining both forms of non-linearity are presented, based on actual measurements. Test methods using multiple light sources, bootstrapping, single step (piggyback), and shot noise are investigated. A method for uncovering non-linearity in high-Z scintillators by using coincident gamma emissions (60Co) is demonstrated. An analytical means for correcting results at the 1 % level is provided.

Doppler examination is an integral part of the echocardiogram. Current systems are equipped with spectral Doppler in continuous wave mode (offering measurements of high velocities with limited spatial specificity due to integration of signal along the scan line), pulsed wave mode (high spatial specificity with maximal recordable velocity reduced by the Nyquist limit), and colour Doppler flow mapping (allowing rapid identification of flow pattern within a cross-sectional B-mode sector). Tissue Doppler echocardiography emerged as a basic tool for sampling regional myocardial velocities, in pulsed wave or colour velocity mapping mode. Finally, three-dimensional systems improve spatial presentation of flow phenomena by integrating Doppler-derived flow patterns in three-dimensional datasets.

Diastole is the part of the cardiac cycle starting at aortic valve closure and ending at mitral valve closure. Evaluation of diastolic function by echocardiography is useful to diagnose heart failure with preserved ejection fraction, and regardless of ejection fraction, echocardiography can be used to estimate left ventricular filling pressure. Assessment of diastolic function includes analysis of left ventricular relaxation and compliance, left atrial and left ventricular filling pressures. This chapter describes the phases of diastole and covers the integrated approach of LV diastolic function through M-Mode and 2D/3D echocardiography, pulsed-wave Doppler echocardiography, and pulsed-wave tissue Doppler echocardiography.

This article provides a conceptual framework, which contributes to the understanding of the mechanisms involved in brain stimulation. The relationship between the induced topographic electric field and specific sites and regions of nerve activation in the brain are still not precisely known. Nevertheless, there is much more specific information available concerning peripheral nerve and nerve root stimulation. In vitro studies provide insight into the different properties of monophasic versus polyphasic pulses. Studies of brain stimulation agree with the in vitro conclusion that the polyphasic pulse is more powerful than the monophasic pulse. The shape of the induced pulse may be a relevant clinical response factor in repetitive TMS.

This chapter addresses a rich variety of effects in spin dynamics arising under the conditions of pump-probe experiments. Here we consider the case where the electron spin is injected by a periodic train of circularly polarized pump pulses and precesses between the pulses in an external magnetic field. Nontrivial effects such as resonant spin amplification and spin coherence mode-locking take place due to commensurability of the repetition period of pump pulses and the charge carrier spin precession period. Theoretical approaches to describing the electron and nuclear spin coherence and experimental manifestations of these unusual regimes of spin dynamics are discussed in detail.

Two statistical processes affect performance: one concerns photon detection at the photocathode (binomial); and the other, gain at each dynode (Poisson). The combined statistical processes dictate resolution, both timing and pulse height. They are best examined using generating functions that are both elegant and capable of providing answers more efficiently than traditional approaches. The requirement for steady and pulsed light sources is an important one for testing and setting up procedures. The use of moments to test the quality of performance is illustrated for a steady DC light source. Amplification provided by a dynode stack is a cascade process, leading to dispersion in gain, and is also ideally handled with generating functions. Theory is developed for essentially continuous pulse height distributions, such as those produced by a multichannel analyser. Arrival time statistics for scintillators are investigated analytically and by Monte Carlo simulation. Treatment is given for dead time and scaling.

DBS implanted pulse generators (IPGs) have greatly increased their capabilities, providing opportunities for different combinations of DBS pulses and stimulation trains. Yet therein lies a dilemma. An increase in technical capabilities could lead to a bewildering and potentially befuddling number of possibilities to consider. Understanding how the nervous system responses to DBS could provide a more rational and hence, more effective approach to DBS programming. Unfortunately, the increasing catalogue of known responses has not translated into an understanding of how DBS works but it is a start and is review in this chapter.

Modern assessment of left ventricular (LV) diastolic function should be based on the estimation of degree of LV filling pressure (LVFP), which is the true determinant of symptoms/signs and prognosis in heart failure.In order to achieve this goal, standard Doppler assessment of mitral inflow pattern (E/A ratio, deceleration time, isovolumic relaxation time) should be combined with additional manoeuvres and/or ultrasound tools such as: ◆ Valsalva manoeuvre applied to mitral inflow pattern. ◆ Pulmonary venous flow pattern. ◆ Velocity flow propagation by colour M-mode. ◆ Pulsed wave tissue Doppler of mitral annuls (average of septal and lateral E′ velocity).In intermediate doubtful situations, the two-dimensional determination of left atrial (LA) volume can be diagnostic, since LA enlargement is associated with a chronic increase of LVFP in the absence of mitral valve disease and atrial fibrillation.Some new echocardiographic technologies, such as the speckle tracking-derived LV longitudinal strain and LV torsion, LA strain, and even the three-dimensional determination of LA volumes can be potentially useful to add further information. In particular, the reduction of LV longitudinal strain in patients with LV diastolic dysfunction and normal ejection fraction demonstrates that a subclinical impairment of LV systolic function already exists under these circumstances.

Fossil fuels and their high yield of available energy regulate the global economy and structure its hierarchy of nations. When a “pulse” of energy—over months, years, decades, or centuries—enters the global industrial system, overshoot dynamics are often observed. The system enters a new mode of production, with new technical combinations. Once it does, it is extremely difficult to return to the old infrastructure, even though the energy resource that provided the pulse likely will yield less over the years (the US and its highway system provide one example of an infrastructural system conceived in a higher-yielding environment, the US oil boom of the early twentieth century). As the energy surplus, or marginal resource return, begins to diminish, output declines, slowing the rise of powerful nations, and transferring growth elsewhere. The effects of declining returns often show up in the monetary system.

Echocardiography is a diagnostic imaging technique by which ultrasound is used to display anatomic and physiologic characteristics of the cardiovascular system. Echocardiography consists of several different imaging modalities that require appropriate settings. In this chapter the most important system settings are discussed in the context of the basic physics of ultrasound image formation. Setting-up the echo machine to optimize patient examination is discussed in detail. All controls are covered. Continuous-wave, pulsed-wave, and colour flow Doppler are explained, as well as more advanced techniques including myocardial velocity imaging and speckle tracking and 3D imaging. Understanding these basic principles will allow optimizing image quality for each individual patient.

Meralgia paresthetica is characterized by anterolateral thigh pain, paresthesia, or dysesthesia without motor weakness. This is due to idiopathic or iatrogenic injury to the lateral femoral cutaneous nerve (LFCN, dorsal rami of L2-L3). Risk factors include obesity, diabetes, and external compression near the inguinal ligament’s attachment to the anterior superior iliac spine. Diagnosis is based on clinical presentation and electrodiagnostic studies. Initial management includes behavioral modification, physical therapy, and pharmacotherapy. More invasive treatment modalities include LFCN infiltration, pulsed radiofrequency, direct nerve stimulation, and spinal cord stimulation. Ultrasound-guided neurectomy is also an effective way to localize the nerve structure and ensure complete nerve transection.

The rapid increase in the use of lasers in healthcare has introduced new challenges. The hazards depend on the characteristics of the laser and the different modes of interaction with tissue. If the laser beam were to strike the unprotected eye, serious damage could occur, the site of damage depending on the wavelength of the laser radiation. The risk is reduced in the case of extended sources but, potentially, may be increased when a repetitively pulsed laser is used. These concepts are all discussed in addition to summarizing current legislation and standards. The laser protection adviser (LPA) is someone who is knowledgeable in the evaluation of risk associated with the use of lasers and has responsibility for provision of expert advice to the user. This chapter contains much useful information for the LPA and others interested in understanding the principles of laser safety.

To gain serious traction in the wider world of medicine, the terminal care protagonists needed to move from a loose web of activism to a concerted body of knowledge and practice. This chapter shows how a focus on cancer pain was a key line in this development. Hospice studies explored and pulled apart some of the prevailing orthodoxies about pain relief. A new confidence emerged in the use of morphine and other drugs. The concept of ‘total pain’ was coined by Saunders as a key perspective on the multi-faceted nature of suffering at the end of life. Clinical studies raised awareness of the hospice movement’s ambitions and began to demonstrate its efficacy. This in turn fuelled further investment and a rapid growth in hospice services.

Анотація:

This chapter presents the details of the optical manipulation of electron spin states. It also addresses manifestations of the electron and nuclear spin dynamics in optical response of semiconductor nanostructures via spin-Faraday and -Kerr effects. Coupling of spins with light provides the most efficient method of nonmagnetic spin manipulation. The main aim of this chapter is to provide the theoretical grounds for optical spin injection, ultrafast spin control, and readout of spin states by means of circularly and linearly polarized light pulses. The Faraday and Kerr effects induced by the electron and nuclear spin polarization are analyzed both by means of a macroscopic, semi-phenomenological approach and by using the microscopic quantum mechanical model. Theoretical analysis is supported by experimental data.

This article reviews recent progress in superconducting quantum bits, including major improvements in design and coherence times. It first provides an overview of the basics of modern superconducting qubit devices and their architectures before turning to single-qubit Hamiltonians and reference frames. It then examines how decoherence originates with noise and shows how to characterize and mitigate this noise using magnetic-resonance-type pulse sequences. It also describes the first-generation superconducting qubits and the now-dominant circuit-quantum electrodynamics architecture in which qubits are coupled to microwave resonators. Finally, it considers several improved designs of superconducting qubits in which coherence times have been significantly improved by minimizing the sensitivity to fluctuating impurities and the coupling to external modes.

This volume is timely in that it explores key issues which are currently at the forefront of the EU’s relations with its eastern neighbours. It considers the impact of a more assertive Russia, the significance of Turkey, the limitations of the Eastern Partnership with Belarus and Moldova, the position of a Ukraine in crisis and pulled between Russia and the EU, security and democracy in the South Caucasus. It looks at the contested nature of European identity in areas such as the Balkans. In addition it looks at ways in which the EU’s interests and values can be tested in sectors such as trade and migration. The interplay between values, identity and interests and their effect on the interpretation of europeanisation between the EU and its neighbours is a core theme of the volume.

This core of this chapter describes and explains a paradox in the religious life of modern Europe: without doubt, Europe is more secular than it used to be, but in terms of public debate, religion is rising rather than falling in significance. The factors that lie behind this seeming contradiction are explored both singly and together. They include deeply embedded cultural factors, the shifts in the historic churches, new forms of religious life, new arrivals, and secular reactions. In each case, the comparison with the American case is carefully considered. The initial sections of the chapter set this comparison in a global context, noting key dates in the reconfiguration of the modern world order and the place of religion in these. The chapter concludes with a brief consideration of the British case—pulled structurally towards Europe and culturally towards the United States.
