Since the landmark Tarasoff decision, courts have expanded third-party liability to psychotherapists for the harm done to others by patients in their care. While this is an issue which affects all psychotherapists, it has particular importance for those treating sexual offenders. Recent advances in assessment techniques provide the psychotherapist with a more accurate profile of the patient and his potential victim(s). While this improvement in diagnostic accuracy affords greater potential for more focused treatment, it may also increase the psychotherapist's risk for incurring liability should the patient recidivate during treatment. This article examines some of the possible legal implications relevant to treating the sexual offender.

This article addresses the thorny issue of the psychologist or psychotherapist's values or ethical orientation. The suggestion is made that certain aspects of Jacques Lacan's psychoanalytical theory provide the resources to overcome the obstacle of arbitrariness or relativism faced by psychotherapists who unavoidably have to take an ethical stance — implicitly if not explicitly — in relation to clients' or analysands' lives and decisions. The dilemma faced by the psychotherapist is recontructed and specific aspects of the poststructuralist psychoanalytical theory of Lacan are addressed. These include the function of the subject's position in the symbolic register (in contrast to the imaginary register of the ego), the role of the unconscious as the ‘discourse of the Other’, of narrative and of repressed signifiers as ethical ‘anchoring points’. Crucially, however, the implications of the register of the ‘real’ for the ethics of the psychoanalyst as psychotherapist are added. These, offer invaluable means of overcoming the dilemma of ethical relativism faced by psychotherapists.

A large number of named psychotherapies provides clinicians with a broad catalogue from which to pick and choose intervention techniques. Today, many psychotherapists claim to work eclectically, referring to the use of varying intervention techniques both within and betweenthe different psychotherapeutic traditions. The availability of many psychotherapeutic techniques can indeed be beneficial, but the question remains as to how the psychotherapist chooses one technique over another depending on the circumstances. In this paper, we willpresent an analytic framework aimed at providing psychotherapists with a navigation tool with which to qualify the choice and coordination of psychotherapeutic techniques. The foundation for this tool is the concept of qualified self-determination, which points to some of the basic competencies necessary for humans to navigate within the technical, social, and personal domains of life. On this basis, we outline how different psychotherapies are differentially aimed at developing these competencies. Finally, we present specific examples onwhere and how this framework can be used, including how it can guide the client case formulation, and how it can facilitate productive collaboration at network meetings.

The purpose of the study was to discern the empathy ability of psychotherapists for childfree female clients with intersectional identities. Each participating psychotherapist was randomly assigned to view one of four mock video sessions with a childfree woman of varying age and socioeconomic status (SES). Psychotherapists rated their ability to empathize for the woman in their assigned mocked video. An analysis of variance revealed that psychotherapists experienced higher empathy ability for a childfree woman of younger age and lower SES than for an older and upper-SES childfree woman. Implications for research and practice are discussed.

Introduction. The full-scale invasion of the Russian Federation on February 24, 2022 has changed the history of Ukraine and the life of every Ukrainian. Millions of people, saving their lives, were forced to move to safer places In Ukraine as well as other countries. Hundreds of thousands of Ukrainians went to defend their country from the invaders. A huge number of people found themselves in the occupation and suffered tortures, captivity, abuse, hunger, and terrible wanderings. It is obvious that the war and its consequences have radically changed the paradigm of psychological assistance and increased the role of a psychologist/psychotherapist, which makes the problem of psychologists/psychotherapists' mental health resilience extremely important. After all, the effectiveness of psychological help depends on psychologists/psychotherapists' psychological and emotional stability. Aim. To formulate the first reflexive thoughts about the change in the paradigm of psychological and psychotherapeutic work in the context of war and the strategy of maintaining and developing psychologists and psychotherapists' resilience. Results. The article analyzes the peculiarities of psychologists/psychotherapists' work in the conditions of the Russia's war against Ukraine, discusses some transformations in the paradigm of mental and psychotherapeutic help in a crisis situation, the most common symptoms and clients' requests over the past six months as well as the dynamics of clients' psychological and emotional states. The author also considers the changes in the therapeutic relationship between the psychologist/psychotherapist and the client as well as the strategy of maintaining and developing psychologists/psychotherapists' resilience and mental health needed to preserve their professional identity. Conclusions. The information presented in the article can be useful in planning, organizing and providing psychological help as well as in developing and applying a psychologists/psychotherapists' resilience promotion psychotechnology.

IntroductionMost experienced psychotherapists use an eclectic approach in their practice, combining techniques of different schools of psychotherapy to treat their patients. However, there are no good teaching models to train the new generation of psychotherapists in the technique of psychotherapy integration. FEP is a form of psychotherapy that combines techniques from four different psychotherapies: psychodynamics, CBT, IPT and supportive therapy. It also incorporates mindfulness, motivational interviewing, exercise and nutrition. A strong therapeutic alliance is crucial to the success of FEP. Techniques are tailored to the patient's current clinical state. Several techniques from different psychotherapies may be used in the same session. The therapist is empathic, flexible adaptive to the patient's needs and assumes an active role in the therapeutic process.ObjectiveTo present a model of psychotherapy integration that can be used by psychotherapists in their clinical practice.AimsWe aim to present a model of psychotherapy integration that can be taught and implemented by psychotherapists in their clinical practice.ResultsWe present ten case examples were FEP was effective clinically and we compare it to the standard of care received by these patients and show the savings to the healthcare system.ConclusionThe results of our study suggest that FEP could be a cost effective treatment that could be added to the psychotherapists toolbox. Insight-oriented psychotherapies, like psychoanalysis, as the most systematic method, and short-term dynamic psychotherapies, as the related compressed techniques, have a shared goal, which includes restoration of insight in patient by the help of a psychoanalyst or other associated psychotherapists. While there are many psychotherapists, who introduce themselves as psychodynamic therapists, a lot of criticizers believe in futility of analytic methods. Though there is no absolute guarantee of the effectiveness of any therapeutic method, whether pharmaceutical or psychological, it seems that based on physiognomies of systematic methods, some positive consequences can be reasonably expectable, which can be quantifiable by available evidence and records, too.

IntroductionIn the context of the importance of psychotherapy in mental health care the aim of our study is to highlight the importance of supervision as an integral part thereof.AimWe analyze the relationship between the assessment of the quality of supervision and the perceived self-efficacy of the psychotherapist as one of the antecedents of decision-making in the psychotherapeutic process. This framework is applied to an empirical study that focuses on psychotherapist's perception of clinical supervision in relationship to professional self-efficacy in the therapeutic process.MethodsThe study was operationalized using the Manchester clinical supervision scale (MCSS), containing a demographic questionnaire, counselor self-efficacy scale–modified version. One hundred and twenty-five psychotherapists with supervisory experience participated in the research, including 67 trainees and 58 graduates of psychotherapy training.ResultsThe results show a positive relationship between effective supervision and self-efficacy (r = 0.363; sig = 0.000; n = 125). The participants who attributed higher ratings to supervision and supervision work alliance also assigned higher scores to perceived self-efficacy–although there is a significant relationship among the participants of psychotherapeutic training (r = 0.444; sig = 0.000; n = 67), this relation is not significant among the actual graduates. Based on modeling, rapport was discovered to be the most important predictor of supervision effectiveness assessment.ConclusionsThe results support the merits of supervision in the education of psychotherapists and the importance of investing resources in its regular implementation.Disclosure of interestThe authors have not supplied their declaration of competing interest.

In recent decades, the study of psychotherapy effectiveness has been one of the pillars of clinical research because of its implication for therapeutic cure. However, although many studies have focused their interest on the patient’s perception, there are no instruments oriented to the study of psychotherapists’ attributions of effectiveness: to what factors psychotherapists attribute responsibility for the cure of the therapies they provide. The present study aimed to develop and validate an instrument for assessing the attribution of the effectiveness of psychotherapy in a population of 69 psychotherapists of different theoretical orientations. After an initial process of inter-judge content validation, 12 items were selected for validation in the targeted population, adequately fulfilling the quality requirements in the validity–reliability tests, and grouped into four factors after principal component analysis. These factors were as follows: (1) therapeutic alliance enhancers; (2) psychotherapist emotional characteristics; (3) therapy-specific variables; and (4) facilitators of patient engagement with therapy. This four-factor structure also showed a good fit for the fit indices checked in confirmatory factor analysis. In summary, we can conclude that the Psychotherapeutic Effectiveness Attribution Questionnaire (PEAQ-12) developed in our research can be helpful if tested on a larger number of individuals. The results can be replicated in other populations of psychotherapists.

This study aimed to perform a survey with psychotherapists, regarding the criteria they consider relevant to measure the progress of children in psychotherapy and whether the criteria indicated by them differ according to the therapist’s theoretical approach. Therapists were contacted electronically and answered a questionnaire composed of items with criteria of positive change present in the literature. Participants were 154 child therapists from different regions of Brazil. The analysis regarding the criteria of positive change that the participants indicated and their theoretical approach suggested that, despite the theoretical differences, in practice psychotherapists seem to adopt similar criteria to interpret progress in child psychotherapies. The criteria mentioned may be useful for a better systematization of the psychotherapy techniques for children. However, due to the lack of elements to evaluate the sample’s representativeness, caution is advised in generalizing the results.

The current study aims at revealing the nature of attitude of psychotherapists towards Internet-based psychotherapy. The study used a tool to measure the attitude of the psychotherapist towards internet-based psychotherapy, prepared by re-searcher, with consideration of the psychometric properties. The sample of the study consisted of (156) psychotherapists from psychiatric hospitals and clinics in Egypt (60 males, 96 females), whose ages ranged between (26 to 53) years. The results of the study indicated that there is a positive attitude towards Internet-based psychotherapy with a relative weight of (59%), and this was in service of the therapist, then service of the case, and finally service of the therapeutic process. It was also found that there is a positive correlation between the number of hours of daily use of Internet-based psycho-therapy and total score of attitude of Internet-based psychotherapy and all its dimensions (service of case, therapist and therapeutic process). There was a negative correlation between age and both the total score of the positive attitude of Internet-based psychotherapy and all its dimensions. Internet based psychotherapists had higher scores compared to tra-ditional psychotherapists (face to face) in the positive attitude towards Internet-based psychotherapy and all its dimen-sions. Whereas, females had high degrees compared to males in the positive attitude towards Internet-based psychotherapy in the service of therapeutic process only.

The article deals with highlighting of the results of psychological problem analysis demanded by the current state of the development of psychotherapy – medical and psychological science and practice of treatment by word; establishing the relationship between psychology, psychiatry and psychotherapy, which has been recently formed in the world practice on providing psychological care; identifying the ideas of scientists and practitioners working in the field of psychotherapy, the strategies and tactics of training future psychotherapists in universities. The article presents debatable, sometimes diametrically opposed, views of domestic and foreign scientists (physicians, practicing psychotherapists and supervisors) on the definition of their own, different from each other, subjects in psychology, psychiatry and psychotherapy - sciences related to a common object of study – the human psyche, as well as which of the specialists who provide psychological assistance has the right to work as a psychotherapist. Considerable attention in the content of this article is paid to the explanation of what points of view exist in the representatives of science, public organizations, medicine and psychology on the delimitation of psychotherapeutic functions and powers of psychotherapists at present; showing how the competencies of psychiatrists, psychologists, psychoanalysts, medical psychotherapists and psychologists-psychotherapists differ, and what are the specific differences between them. The traditions and author’s understanding of training the future psychotherapists in medical and psychological institutions of higher education and retraining of specialists in the field of psychotherapy, as well as the role and importance of psycholinguistic knowledge, discursive skills, abilities and competencies in psychotherapeutic education are described. The results of monitoring the content of training the psychotherapists in medical and psychological institutions of higher education in the specialties: “clinical psychology”, “psychotherapy” are shown, the imperfection characteristic for the current psychotherapeutic education are described, and it is emphasized on existing of the problem of significant terminological disorder not only among the names of specialities acording to which the psychotherapists are trained, but also in the educational and professional qualifications received by future specialists. It is proved that regardless of the medical or psychological sphere in psychotherapy, it cannot exist and develop in a promising direction without applied psycholinguistics, which equips it with scientifically substatiated rational means, in particular speech-language psychotechnics, metamodels of psychological (discursive) influence, samples of psychotherapeutic discourses providing lapidary effective intervention in the subconscious and conscious layers of the psyche of clients / patients.

Estimating demand for psychotherapy is like trying to evaluate the size of a mountain range from the foothills—the higher one climbs, the more potential needs become apparent. The unspoken dream that at some point all these needs will be met by trained psychotherapists seems firmly destined to stay in the realms of fantasy. In reality, specialist psychotherapists will always be thin on the ground. In Surrey, where my own practice is based, I am, and am likely to remain, the only medically trained psychotherapist with four sessions allocated to a population of 200,000. The psychotherapy department at St. George's Hospital, Tooting, to which I am also attached, has a catchment which extends to the whole South-West Thames Region, encompassing places as far afield as Chichester and Haywards Heath.

SummaryIf many patients having multiple difficulties are to receive the integral and efficient psychotherapies they require, they need attention from psychiatrists who have specialised training in psychotherapy. This paper sets out the roles to which existing holders of the Certificate of Completion of Training have already moved to, as they work with patients, families, teams and organisations. The General Medical Council has recognised that the understanding of medical psychotherapists is also vital to the future clinical teaching of all psychiatric trainees. This paper summarises key components of the knowledge and roles of future specialists in medical psychotherapy. It recommends that the term ‘medical psychotherapy’ be used widely to clearly differentiate psychiatrists with this training and these responsibilities from non-medical psychotherapists.

Non-binary people may face specific challenges in psychological settings. Psychotherapists often display a lack of preparation for non-binary gender identities, resulting in overt or subtle forms of prejudice that compromise the therapeutic alliance. The present study aimed to provide data on non-binary clients’ positive and negative experiences within therapeutic relationships. Twenty-five interviews were conducted with non-binary people in the age range of 19–35. Using codebook thematic analysis, the researchers identified three main themes: (1) the self of the psychotherapist, consisting of the impact of the therapist’s personal (i.e., sexual identity) and professional (i.e., competence) characteristics on the therapeutic experience; (2) the practice of the psychotherapist, emerging as affirmative (validation and microaffirmations) or negative (gender identity change efforts, manifest aggressions, and microaggressions) approaches toward non-binary identities; (3) the therapeutic relationship, referring to the alliance, rupture, and reparation based on the therapists’ openness toward non-binary identities. To provide a safer setting for non-binary clients, psychotherapists should incorporate issues related to gender minority identities in their training, acknowledge clinical errors when they occur and adopt an active predisposition to learn through the client’s experience, giving value to their unique contribution.

I have been intrigued by my very different responses to two uses of the word method. The first, commonplace and currently popular, suggests that psychotherapists should have a number of methods at their disposal. This could be called the ballast argument. The second, from a statement by Laplanche, is that what Freud predominantly gave us was a method. I am rather disturbed by the former, but take the latter very seriously in my own practice. My sense of intrigue comes from the belief that both examples contain radically different notions of what psychotherapists are engaged in. The ballast argument enlists a technological approach, the application of knowledge, where one is engaged in a particular kind of behandlung (treatment); a kind of violence. But Freud's method is also a technology, so why should I favour that? Perhaps, because it is a method that opens up and contains particular kinds of spaces. I wish to explore these ideas by sharing some of the ideas that have been helpful to me in feeling and thinking through this issue over the years; in order to wonder about what kind of spaces are being offered by psychotherapists in these differing positions. What are psychotherapist's responsibilities in such spaces?

Psychotherapists around the world are facing an unprecedented situation with the outbreak of the novel coronavirus disease (COVID-19). To combat the rapid spread of the virus, direct contact with others has to be avoided when possible. Therefore, remote psychotherapy provides a valuable option to continue mental health care during the COVID-19 pandemic. The present study investigated the fear of psychotherapists to become infected with COVID-19 during psychotherapy in personal contact and assessed how the provision of psychotherapy changed due to the COVID-19 situation and whether there were differences with regard to country and gender. Psychotherapists from three European countries: Czech Republic (CZ, n = 112), Germany (DE, n = 130) and Slovakia (SK, n = 96), with on average 77.8% female participants, completed an online survey. Participants rated the fear of COVID-19 infection during face-to-face psychotherapy and reported the number of patients treated on average per week (in personal contact, via telephone, via internet) during the COVID-19 situation as well as (retrospectively) in the months before. Fear of COVID-19 infection was highest in SK and lowest in DE (p < 0.001) and was higher in female compared to male psychotherapists (p = 0.021). In all countries, the number of patients treated on average per week in personal contact decreased (p < 0.001) and remote psychotherapies increased (p < 0.001), with more patients being treated via internet than via telephone during the COVID-19 situation (p < 0.001). Furthermore, female psychotherapists treated less patients in personal contact (p = 0.036), while they treated more patients via telephone than their male colleagues (p = 0.015). Overall, the total number of patients treated did not differ during COVID-19 from the months before (p = 0.133) and psychotherapy in personal contact remained the most common treatment modality. Results imply that the supply of mental health care could be maintained during COVID-19 and that changes in the provision of psychotherapy vary among countries and gender.

In 1975, ten Consultant Psychotherapist posts (six whole-time equivalents) were recorded, and this figure increased steadily as more post holders declared themselves as specialist psychotherapists, after initial reluctance on the part of many to relinquish the title of Consultant Physician in Psychological Medicine or Consultant Psychiatrist, since the title of Psychotherapist did not assert medical status. By 1984 there were 91 Consultant Psychotherapist posts (61.2 whole-time equivalents), but there was no record of the number of Consultant Psychiatrist post holders with specified contractual Special Responsibility or Special Interest in psychotherapy. Such details are held only by Regional Health Authorities, and are not available for manpower statistics.

“The idea of a psychotherapeutically informed psychiatry seems such a simple and obvious one and yet the divide between psychotherapy and general psychiatry – between ‘brainlessness’ and ‘mindlessness’ has, until recently, seemed unbridgeable”.The Psychotherapy section of the Royal College of Psychiatrists has the largest membership in the college. Since psychotherapy became recognised as a discipline within psychiatry in 1975 the Royal College of Psychiatrists has recommended one consultant psychotherapist for each 200,000 of the population. In Northern Ireland there are only 1.9 whole time equivalents rather than the eight expected and in the Republic of Ireland there is none. According to the recent document produced by the Royal College of Psychiatrists' Psychotherapy Faculty Executive Committee in December 1998, The development of psychological therapy services: Role of the consultant psychotherapist, there has been no net growth in the last five years in the numbers of psychotherapists in England and Wales. The future looks equally gloomy in Ireland.This is rather surprising given that recent government documents have highlighted both the importance and the effectiveness of psychological therapies. There is a growing evidence base underpinning the use of psychotherapy in the management of a wide variety of conditions including psychoses, eating disorders and severe personality disorders.Psychotherapy has high public acceptability and finds itself in the unusual position of having both government and public demanding the provision of extra psychological therapies, but not receiving the full support of psychiatry and the purchasers of healthcare.

Drawing upon Donald Winnicott’s classic paper, “Delinquency as a Sign of Hope”, the author has underscored the importance of recognising the role of emotional loss in the aetiology and treatment of adolescent forensic patients, and she has elucidated how this particular psychoanalytical lens can assist psychotherapists to work more effectively with deprivational trauma. The author has also described her experience as both a field social worker and as a forensic psychotherapist, and has shared moving clinical material with great clarity and generosity, demonstrating the value of incorporating Winnicott’s insights into the forensic consulting room. In particular, the author has examined how the patient can use the psychotherapist as an understanding object who can help to counteract earlier deprivations.

During the last 20 years, psychological interventions and psychotherapy have acquired a modest but significant place in health care. The lack of a uniform legal definition of these professional activities in the domain of health care hampers quality control of training programs and delivered services and complicates coordination of care. Training requirements are not always made explicit, and often there are no mechanisms for quality control or for monitoring compliance with ethical codes of conduct. In this review, the legal regulation of the professional activity of psychologists in health care and of psychotherapists in 17 European countries is examined. Eleven of these have adopted a legal regulation the title and the professional activities of psychologists in health care. Seven have an additional law regulating the title and the professional activities of psychotherapists. In five countries, professionals other than psychologists and medical doctors can obtain a legally protected title and license to practice as a psychotherapist. Conclusions are drawn concerning the available models of regulation of psychotherapy and their respective consequences.

The work of a cognitive behavioural therapist is associated with a number of factors contributing to the feeling of satisfaction as well as many concerns. Studies show that psychotherapists are vulnerable to the effects of distress which, if left unattended, lead to burnout and serious professional impairments. Even though it has been emphasised that self-care of a psychotherapist is more of an imperative than an option, all too often professionals ignore their needs. Among the most common self-care myths cognitive behavioural therapists believe in, one may find the assumptions that self-care is optional, the knowledge how to look after oneself is equivalent to managing it, and coping is identical to being a therapist. Responsibility for oneself is usually an optional topic during education and supervision processes. Even though articles on that subject are widely available, it is important to analyse the problem of self-care, and to propose ways for creating a healthy work environment for cognitive behavioural therapists. In the article, the core ideas concerning the cognitive behavioural model of therapy are presented, and the cognitive behavioural model of burnout is described. In addition, the role of self-care in professional activity among cognitive behavioural psychotherapists is examined. The data on the role of awareness, balance, flexibility, and health in increasing self-care practices among psychotherapists is presented, and the importance of a proactive approach to self-care is highlighted. Furthermore, the main ideas that should be incorporated in training courses and supervision are given, together with the description of the role of supervision in psychotherapeutic work.

A survey of 209 licensed psychotherapists was conducted to investigate attitudes towards and observations of nonverbal communication in a reference situation. Background factors such as gender, years of psychotherapeutic experience, hours of therapy practiced each month, and theoretical approach were used to examine relationships. The psychotherapeutic greeting situation, i.e., the first time a therapist and patient meet in a waiting room, was chosen as the reference situation. Female psychotherapists believed that nonverbal communication was more important than male psychotherapists. Psychotherapists with 16 years or more experience supported items that constitute the greeting phase and items on active observation significantly more often than psychotherapists with less experience. Hours of psychotherapeutic practice and frames of reference were not significantly related to any item within the questionnaire. Belief in nonverbal communication was not significantly related to items that involve preparing to meet a patient for the first time.

Abstract The aim of this paper is to describe the fictionalisation of psychoanalysis in the literary therapy genre written by psychotherapists. Being a psychotherapist, Irvin Yalom has written and published several literary therapy novels. The Schopenhauer Cure (2006) presents a psychoanalytic encounter with focus on the patient’s interpersonal issues in a group therapy session and draws a parallel line between fictional patients and the philosopher Arthur Schopenhauer. The fictionalisation of patients’ psychological symptoms and the way therapists examine themselves in the therapeutic milieu in The Schopenhauer Cure correspond to the fundamental concerns of isolation, meaninglessness, death and freedom in existential psychotherapy. I explore the literary representation of the psychotherapist and therapist-patient relationship and the therapeutic encounter in The Schopenhauer Cure in the context of how fictional narratives can be read as a form of highlighting the psychoanalytic encounter

Rationale, aims and objectives When choosing a particular psychotherapy, psychotherapists are expected to take into account their clients’ preferences, needs, and values, their own professional experience, and what is currently known regarding the real impact of these therapies. Psychotherapists widely vary the one from the other in the weight they attribute to each of these factors. We examined the way lay people judge of the appropriateness of therapeutic decisions taken by clinical psychologists.Methods 206 participants were presented with 24 vignettes depicting the way a psychotherapist has selected therapy. They were composed according to a three within-subject factor design: (a) the frequency with which the therapy was used in clinical practice in the area, (b) the patients’ usual level of appreciation of this therapy, and (c) the level of scientific evidence supporting the use of this therapy (strong evidence, weak evidence, unknown evidence and no evidence). The participants rated appropriateness of the decision in each case.Results For half the participants, the most important factor was the patients’ appreciation of the therapy in general. Even scientifically supported therapies were considered as only mildly appropriate if patients in general didn’t like them much. For the remaining half of participants, scientific evidence was the most important factor: Therapies that were well appreciated by patients were not viewed as appropriate if they were not scientifically grounded.Conclusions Creating authoritative multilingual web sites synthesizing the main findings regarding the impact of diverse therapies would be a way through which at least some patients could judge of the appropriateness of the technique they have been offered by their psychotherapists. Their (informed) reactions could ease changes in direction of more weight attributed to existing scientific evidence from the part of therapists.
