This study investigated the reliability of the revised Early Expository Comprehension Assessment (EECA), a measure that looked at preschoolers' comprehension of expository text. Thirty-seven preschool children between the ages of four and five were administered two comparable versions of the measure by two examiners. Scoring procedures were created and the protocols were scored and compared for reliability. The data was analyzed using a mixed models Analysis of Variance for repeated measures and a maximum likelihood estimate of variance components. Results from the analysis showed that version and order had no significant effect on three of the response task scores (Purpose of the Text, Problem/Solution Retelling, and Problem/Solution Mapping), indicating these tasks were reliable. Results showed that variation due to controlled administration variables (version and order) was larger as compared to variability among the subjects in two of the response task scores (Graphics and Problem/Solution Questions), indicating these tasks to be unreliable.

The study of transformative learning within collaborative teams was conducted to gain new applicable knowledge used to influence overall school improvement and implementation of professional learning communities. To obtain this new knowledge, the Professional Learning Community Collaborative Team Survey (CTS) was developed and psychometrically tested to identify indicators of transformative learning within collaborative teams. The CTS was sent to a purposive sample of 25 northwest Missouri schools that had undergone at least three years of membership within the Missouri Professional Learning Communities (MPLC) Project. Nineteen of the selected 25 schools chose to administer the CTS to an estimated overall sample of 457 teachers. The Northwest Missouri Region 5 Regional Professional Development Center (RPDC) provided blinded archival data of 234 to 255 responses per CTS item. Conclusions indicated the CTS was a reliable and valid instrument. Principal component analysis yielded two components confirming transformative learning within collaborative teams: (a) Purposeful Work and (b) Productive Relationships. A data reduction process produced two alternate versions of the CTS, which were also determined to be reliable and valid.

Although the implementation of the Common Core State Standards has included more informational texts in early grades to emphasize reading to gain knowledge (Green, 2012; Roskos & Neuman, 2014), the lack of available expository assessments leaves teachers unsure of what students need to know in order to be successful comprehenders of these texts (Hall, Markham, & Culatta, 2005; Harding, 2014). Moreover, there are very few early expository assessments available which makes it difficult for teachers to monitor young children's expository text knowledge and skills and then adjust their instruction to meet children's unique needs. The EECA R-2 is an early expository assessment measure that was created in order to meet these demands. Data from 128 preschoolers between the ages of four and five in seven different Title I classrooms were collected to determine the reliability and validity of the EECA R-2. Children were given the Test of Story Comprehension (TSC) subtest of the Narrative Language Measures (NLM) Preschool Assessment as well as two versions of the EECA R-2. A Many Facets Rasch model was used to determine reliability and to allow for examination of individual test items. In addition, correlations were run between the NLM and the EECA to determine the validity of the EECA. Results indicate the EECA R-2 is a reliable and valid measure. High reliability was obtained for all facets (rater .00, person .97, and form .96). Items 6-13, 15, 18, and 23 were deemed quality items. Items 1, 2, 5, 15, 16, 20, 21, 25, and 26 were deemed problematic items. There was a positive correlation between the TSC subtest of the NLM Preschool Assessment and the EECA R-2 tasks (r=.76, p= ≤.01). Future research could include another rendition of the EECA in order to gain a better understanding of the problematic items. Doing so could support teachers in assessing student abilities and preparing classroom instruction that targets specific areas of focus to move learning to higher levels.

Peer assessment is used by health care professionals as a way to share knowledge and evaluate the performance of colleagues. Peer assessment is used widely in medical education as a preparatory tool for students, but peer assessment research in athletic training education is lacking. Athletic trainers are healthcare providers with a similar skill-set to physicians, thus, athletic training education can benefit from the use of peer assessment. Athletic training educators need to research the use of peer assessment as an evaluation tool in order to better prepare students to practice as healthcare professionals. This study investigated the accuracy and reliability of undergraduate athletic training students in their ability to assess their peers. This quasi-experimental study used between-group and within-group designs to answer the research questions. Junior-level students, senior-level students, and their instructors were enrolled as participants. Each student group’s ratings of clinical skills and professional behaviors were compared to instructor ratings to measure accuracy, and each student group’s ratings were compared for reliability. Cohen’s kappa coefficient measured inter-rater agreement for all statistical analyses. Both groups of students were accurate raters (p < .05) of their peers on clinical skills, but only the senior-level students were accurate in rating professional behaviors. Both groups of students were reliable in rating their peers on about half of the clinical skills. The senior-level students were also reliable in evaluating professional behaviors, but the junior-level students were not. The data for this study showed high levels of observed agreement for most clinical skills, subscales and the professional behaviors, but some items had low Cohen’s kappa values, most likely due to a known paradox that occurs with the kappa statistic. As the first study in athletic training education to use undergraduate students, live data collection, and rating of professional behaviors, the findings were promising for future research. Future research needs to include training in peer assessment, use of repeated measures, and comparison of instructor scores in order to better understand peer assessment in this population. Additionally, there is a need to establish consistent, quality measures in peer assessment research, including those used in athletic training education.

Counselling services as provided by non-professional counsellors have been in place for a number of decades. Counselling traumatised people demands a significant amount of emotional investment from the counsellor. A neglected area as far as non-professional counsellors in South Africa is concerned, is the well-being of the counsellors. Burnout as well as its antithesis, work engagement, are two possible transactional outcomes impacting on the well-being of these counsellors. The measurement of burnout and work engagement requires valid and reliable measuring instruments. The dearth of research studies in the area of burnout and work engagement, together with the unique contribution of non-professional counsellors in organisational settings, has led to the primary focus of this study being the exploration of the experience of this group of counsellors doing trauma counselling in financial institutions in South Africa. A lack of norms for the Maslach Burnout Inventory - Human Services Survey (MBI-HSS), and the Utrecht Work Engagement Scale (UWES) for non-professional counsellors doing trauma counselling makes the identification of burnout and work engagement within this specialist environment difficult. Consequently, investigating the reliability and validity of the MBI-HSS and the UWES would result in the standardisation of these specific measuring instruments, therefore contributing to the identification of burnout and work engagement with non-professional trauma counsellors. Some of the factors that could play a role in the prevalence of burnout and work engagement are secondary traumatic stress, the demands of counselling, lack of resources, personal consequences, social support and sense of coherence. The objectives of this research were to standardise the MBI-HSS and UWES for non-professional counsellors as well as to develop and test a causal model of burnout and work engagement for this specialist group. The research method involved four separate articles, each consisting of a brief literature overview and an empirical study. A cross-sectional design, whereby a sample is drawn from a population at a particular point in time, was used. The data for this study was collected from 168 non-professional counsellors, employed by three of the major banks in South Africa. The MBI-HSS, UWES, Orientation to Life Questionnaire (OLQ) as well as a Self-Report Questionnaire (SRQ) and a biographical questionnaire were administered. Descriptive statistics, analysis of variance, correlations, canonical analysis, and structural equation modelling were used. Structural equation modelling confirmed a three-factor model of burnout (emotional exhaustion, depersonalisation and personal accomplishment). In contrast with research findings confirming the three-factor model of the UWES (vigour, dedication and absorption), a one-factor model for the UWES was confirmed for non-professional counsellors. The internal consistency of the scales for the MBI-HSS and UWES was found to be satisfactory and in line with reported findings in the literature. Structural equation analysis showed that the lack of resources and job demands predicted the core of burnout, namely emotional exhaustion and depersonalisation. The conflicts and pressures that are already associated with the everyday work of non-professional counsellors are likely to be magnified by the counselling role. Non-professional counsellors continually face conflicts created by the fact that they are accountable to large organisations, but professionally, ethically and morally devoted to their clients (the victims of trauma who are being counselled by them). They must balance the competing, and sometimes opposing demands of several parties such as trauma victims, employees, families and communities. To add to these circumstances it is important to remember that counselling is not the main job objective of the non-professional counsellors. Counselling is seen as an "add-on" to their job description and is in most instances not part of their performance measurement/assessment. Work engagement was related to low burnout scores, while personal accomplishment was associated with work engagement. High sense of coherence had a mediating effect on burnout and a positive effect on work engagement. This study seems to emphasise that job demands have a more negative effect on engagement when sense of coherence is low than when sense of coherence is high. Conversely, it is assumed that sense of coherence provides functions such as increased perception of coping capacity or minimised stress appraised, which decreases the effects of stress on an individual. Recommendations for the organisations and future research were made.

Unintended consequences of electronic health records represent undesired effects on individuals or systems, which may contradict initial goals and impact patient care. The purpose of this study was to determine the extent to which a new quantitative measure called the Carrington-Gephart Unintended Consequences of Electronic Health Record Questionnaire (CG-UCE-Q) was valid and reliable. Then, it was used to describe acute care nurses' experience with unintended consequences of electronic health records and relate them to the professional practice environment. Acceptable content validity was achieved for two rounds of surveys with nursing informatics experts (n = 5). Then, acute care nurses (n = 144) were recruited locally and nationally to complete the survey and describe the frequency with which they encounter unintended consequences in daily work. Principal component analysis with oblique rotation was applied to evaluate construct validity. Correlational analysis with measures of the professional practice environment and workarounds was used to evaluate convergent validity. Test-retest reliability was measured in the local sample (N = 68). Explanation for 63% of the variance across six subscales (patient safety, system design, workload issues, workarounds, technology barriers, and sociotechnical impact) supported construct validity. Relationships were significant between subscales for electronic health record-related threats to patient safety and low autonomy/leadership (P < .01), poor communication about patients (P < .01), and low control over practice (P < .01). The most frequent sources of unintended consequences were increased workload, interruptions that shifted tasks from the computer, altered workflow, and the need to duplicate data entry. Convergent validity of the CG-UCE-Q was moderately supported with both the context and processes of workarounds with strong relationships identified for when nurses perceived a block and altered process to work around it to subscales in the CG-UCE-Q for electronic health record system design (P < .01) and technological barriers (P < .01).

Coaches are constantly seeking more ecologically valid and reliable data to improve professional sporting performance. Using unobtrusive, valid and reliable mobile physiological monitoring devices may assist in achieving this aim. For example, there is limited information regarding professional fast bowlers in cricket and understanding this role during competitive in-match scenarios rather than in simulated bowling events could enhance coaching and physical conditioning practices. The BioharnessTM is a mobile monitoring device and assesses 5 variables (Heart rate [HR], Breathing frequency [BF], Accelerometry [ACC], Skin temperature [ST] and Posture [P]) simultaneously. Therefore, the aims of this research were to assess the effectiveness of the BioharnessTM mobile monitoring device during professional sporting performance using fast bowlers in cricket and this was to be achieved in five research studies. Study 1 presented the physiological profile of professional cricketers reporting fitness data with other comparable professional athletes, with a specific interest in fast bowlers who were to be the focus of this work. The 2nd and 3rd study assessed the reliability and validity of the BioharnessTM through controlled laboratory based assessment. For validity, strong relationships (r = .89 to .99, P < .01) were reported for HR, BF, ACC and P. Limits of Agreement reported HR (-3 ± 32 beat.min-1), BF (-3.5 ± 43.7 br.min-1) and P (0.2 ± 2.6o). ST established moderate relationships (-0.61 ± 1.98 oC; r =.76, P <.01). Reliability between subject data reported low Coefficient of Variation (CV) and strong correlations for ACC and P (CV < 7.6%; r = .99, P <.01). HR and BF (CV ~ 19.4%; r ~.70, P <.01) and ST (CV 3.7%; r = .61, P < .01), present more variable data. Intra and inter device data presented strong relationships (r > .89, P < .01, CV < 10.1%) for HR, ACC, P and ST. BF produced weaker data (r < .72, CV < 17.4%). Study 4 assessed reliability and validity of the BioharnessTM in a field based environment using an intermittent protocol. Precision of measurement reported good relationships (r = .61 to .67, P < .01) and large Limits of Agreement for HR (> 79.2 beat.min-1) and BF (> 54.7 br.min-1). ACC presented excellent precision (r = .94, P < .01). Results for HR (r = ~ .91, P < .01: CV <7.6%) and ACC (r > .97, P < .01; CV < 14.7%) suggested these variables are reliable in the field environment. BF presented more variable data (r = .46 - .61, P < .01; CV < 23.7%). In all studies, as velocity of movement increased (> 10 km.h-1) variables became more erroneous. HR and ACC were deemed as valid and reliable to be assessed during in-match sporting performance in study 5. This final study sought to utilise and assess the BioharnessTM device within professional cricket, assessing physiological responses of fast-medium bowlers within a competitive sporting environment, collected over three summer seasons. The BioharnessTM presented different physiological profiles for One Day (OD) and Multi Day (MD) cricket with higher mean HR (142 vs 137 beats.min-1, P < .05) and ACC (Peak acceleration (PkA) 227.6 vs 214.9 ct.episode-1, P < .01) values in the shorter match format. Differences in data for the varying match states of bowling (HR, 142 vs 137 beats.min-1, PkA 234.1 vs 226.6 ct.episode-1), between over (HR, 129 vs 120beats.min-1, PkA 136.4 vs 126.5 ct.episode-1) and fielding (115 vs 106 beats.min-1, PkA 1349.9 vs 356.1 ct.episode-1) were reported across OD and MD cricket. Therefore, this information suggests to the coach that the training regimes for fast bowlers should be specific for the different demands specific to the format of the game employed. Relationships between in-match BioharnessTM data and bowling performance were not clearly established due to the complexities of uncontrollable variables within competitive cricket. In conclusion, the BioharnessTM has demonstrated acceptable validity and reliability in the laboratory and the field setting for all variables (Heart rate, Breathing frequency, Accelerometry, Skin temperature and Posture) but with limitations for heart rate and breathing frequency at the more extreme levels of performance. Furthermore, taking these limitations into account it has successfully been utilised to assess performance and provide further insight into the physiological demands in the professional sport setting. Therefore, this work suggests that coaches and exercise scientists working together should seek to utilise new mobile monitoring technology to access unique insights in to sporting performance which may be unobtainable in the laboratory or a simulated field based event.

Dr. Ronald Cervero (1985) identified learner motivation, the nature of the change, the social system of the organizational climate, and the educational program as factors affecting the application of learning to professional practice. A repeated measures research design was used to measure stability over time of instruments developed to measure variables in Cervero's model. Participants, N=27) graduate students, completed the instruments, "New Ideas and You" which measures learner's motivation to change, "The Nature of Change" which measures the learner's perception of the proposed change, and "Organizational Climate of the Social System" which measures the learner's perception of the social system's affect on the implementation of change. Staff nurses (N=27) completed the instrument "Continuing Education Offering Evaluation" which measures the learner's perception of the educational offering. Participants then completed the instruments again in three weeks. Procedures for the protection of human subjects were followed. The test-retest reliability coefficients were: "New Ideas and You," r=.72 L)-.01; "The Nature of Change," r.84 p=.01; "Organizational Climate of the Social System," r.83 p=.01; "Continuing Education Offering Evaluation," r.91 p=.01. The significance of this study was the initial establishment of stability over time of instruments developed to measure specific factors that affect the application of newly gained knowledge to nursing practice. Establishing reliability coefficients of instruments to measure the variables in Cervero's model is a step forward in the investigation of the larger question, "Does continuing education change practice?"

Abstract This research explored professional singers’ pronunciation of German in Frauenliebe und Leben, a song cycle, composed in 1840 by Robert Schumann (1810–1856). Adopting a two-fold approach, the thesis examined pronunciation from the viewpoints of normative German pronunciation and vocal pedagogy, paying special attention to the relationship between the two sets of norms. Normative German pronunciation is based on ‘reine Hochlautung’ as defined by Theodor Siebs, as well as on his instructions to classical singers. Norms of pronunciation used in vocal pedagogy were derived from 11 German pronunciation manuals for classical singers. At the centre of this study were 11 elements of the German language: the phonemes /s/, /r/, /b/, /d/ and /g/, the suffix -ig, double consonants, connecting consonants, the letter combination ng, liaison and diphthongs in a melismatic setting. Twelve professional singers’ – Anne Sofie von Otter, Barbara Bonney, Brigitte Fassbaender, Elly Ameling, Elisabeth Grümmer, Irmgard Seefried, Janet Baker, Jessye Norman, Kathleen Ferrier, Soile Isokoski, Marjana Lipovšek, Tamara Takács – pronunciation of these elements was studied via their recording of Frauenliebe und Leben between 1950 and 1997. Both descriptive statistical analysis methods and hierarchical cluster analysis were applied to the data. A reliability analysis was conducted using intra-category correlations and relative frequencies. This analysis indicated a reasonable degree of reliability. Descriptive statistical analysis methods yielded the result that the actual pronunciation of the professional singers, as evidenced by their recordings, was considerably more varied than what could be expected on the basis of normative or pedagogical norms. Hierarchical cluster analysis, too, showed that the professional singers shared the same category to a moderate degree. We may conclude that vocal pedagogical theory needs to be further developed, because pedagogical norms for pronunciation proved partially confusing and difficult to grasp. In addition, specific attention must be given to source critique. The actual pronunciation of professional singers challenges singing instructors to reflect on how they teach German pronunciation and what they can do to help their students develop the skills required for a critical listening of recordings
Tiivistelmä Eksperttilaulajien saksan kielen ääntämistä havainnoitiin Robert Schumannin (1810–1856) vuonna 1840 säveltämässä laulusarjassa Frauenliebe und Leben, ja ääntämistä tutkittiin suhteessa sekä saksan kirjakielen ääntämisnormistoon että pedagogiseen ääntämisnormistoon. Tutkimuksessa syvennyttiin myös ääntämisnormistojen välisen suhteen tarkasteluun. Saksan kirjakielen ääntämisnormisto perustui Theodor Siebsin reine Hochlautung -ääntämistapaan sekä Siebsin klassisen laulun ohjeistuksiin. Pedagoginen ääntämisnormisto eli klassisille laulajille osoitetut saksan kielen ääntämisen oppaat oli rajattu yhteentoista teokseen. Tutkittavana oli yksitoista saksan kielen elementtiä: konsonantit s, r, b, d, g, suffiksi -ig, kirjainyhdistelmä ng, kaksoiskonsonantit, konsonanttien yhdistäminen, vokaalien sitominen ja diftongit. Laulusarjan levyttäneitä eksperttilaulajia oli kaksitoista: Anne Sofie von Otter, Barbara Bonney, Brigitte Fassbaender, Elly Ameling, Elisabeth Grümmer, Irmgard Seefried, Janet Baker, Jessye Norman, Kathleen Ferrier, Soile Isokoski, Marjana Lipovšek ja Tamara Takács. Äänitteet olivat vuosien 1950 ja 1997 väliseltä ajalta. Tutkimuksessa käytettiin perinteisiä kuvailevia tilastollisia analyysimenetelmiä ja hierarkkista ryhmittelyanalyysia. Hierarkkinen ryhmittelyanalyysi puolestaan osoitti, että eksperttilaulajien sijoittuminen samaan ryhmään oli maltillista. Tutkimus vahvistaa käsitystä, että laulupedagogista teoriaa tulee tarkastella dynaamisena, alati muuttuvana ilmiönä. Tutkimuksen perusteella myös havaitaan, että eksperttilaulajien ääntäminen noudattaa laulupedagogiikassa käytettyjä ääntämisen ohjeistuksia vain osin. Inter-rater reliability (IRR) of the Structured Assessment of Violence Risk in Youth (SAVRY) amongst mental health professionals was explored. Participants (n=22) rated vignettes varying in violence severity and gender (case characteristics). Professional characteristics including perception of their confidence and objectivity in ratings were measured. Using ICC1 scores, the IRR was low for items when interpreted using the 95% confidence interval but much higher using individual ICC scores. Case characteristics did not conclusively affect ratings. Self-reported confidence in ratings was not associated with increased reliability. This suggests that professionals completing risk assessments are likely to benefit from ongoing training.

Digitization has an impact on the change that the accounting industry is undergoing. The vision of development is to automate and systematize society, which means that the technology mainly implements tasks of society. The study aims to dig deeper which effects the digitization has on the accounting firm. These effects are derived from an analysis model in which the role of the accounting assistants is questioned. Employees with different occupational positions at the accounting firm have been interviewed for empirical data collection. The interviews were conducted in a purposeful manner, since the selection is aimed at employees who are helpful in answering the study's question and purpose. The interviews were used to find links with the theoretical reference frames. With the help of the theoretical frame of reference and the collected empirical data, conclusions have been drawn that digitization causes different effects. The work approach is streamlined by automating the basic tasks of the accounting firm. The streamlining will lead to the accounting firm being able to deliver a broader service during the same time frame, which leads to increased client satisfaction. Quality achieves higher levels by increasing customer satisfaction. When the accounting firm delivers services with higher time gains, profitability will be higher. Profitability is something accounting firms and other businesses strive for, therefore it is likely that digitization will be implemented even more. Finally, the different effects of digitization show that the need for accounting assistants is diminishing, which is why digitization is partly perceived as a threat.

Obiettivo: Lo studio ha l’obiettivo di indagare quali siano le scale di valutazione caratterizzate dalle migliori proprietà psicometriche per la misurazione dei disturbi del tono in seguito a GCA. Metodi: La revisione sistematica ha incluso studi osservazionali cross sectional relativi ad un campione di pazienti adulti dopo GCA di varia eziologia e in cui gli outcome indagati fossero le proprietà psicometriche delle scale di valutazione dell’ipertono muscolare. I database consultati sono PubMed, Cochrane Central Register of Controlled Trials e PEDro. La valutazione del rischio di bias e dell’applicabilità degli studi è stata eseguita tramite l’utilizzo dello strumento QUADAS-2. Risultati: I risultati prodotti dalla ricerca sono 1056, di cui solo 13 inclusi in seguito all’analisi del full text. Il disegno di tutti gli studi reclutati è osservazionale cross sectional e i principali outcome indagati sono l’affidabilità e la validità. Dagli studi inclusi nella revisione sono emerse una serie di scale di valutazione dell’ipertono muscolare: AS/MAS/MMAS, MTS, HAT, BADS, UDRS e BFMMS. La AS e MAS hanno dimostrato un’affidabilità inter-operatore da scarsa a moderata e una bassa validità. La MMAS ha rivelato un’elevata affidabilità inter-operatore nella valutazione dei distretti dell’arto superiore e la MTS un’affidabilità inter-operatore da scarsa a moderata e intra-operatore da moderata ad alta. HAT, BADS, UDRS e BFMMS, studiate su campioni di bambini con PCI, hanno mostrato un’affidabilità inter-operatore da moderata a buona e una buona consistenza interna. Conclusioni: Lo studio ha permesso di individuare una serie di scale di valutazione dell’ipertono muscolare e di analizzarne le relative proprietà psicometriche. I limiti dello studio riguardano l’inclusione di 4 studi relativi a pazienti pediatrici con PCI, l’analisi da parte di un solo revisore indipendente e la mancanza di confronto con il gold standard diagnostico in diversi studi inclusi nella revisione.

Background: Patient empowerment is viewed as a priority by policy makers, patients and practitioners worldwide. Although there are a number of measures available, none have been developed specifically for patients in the UK with long-term conditions. It is the aim of this study to report the development and preliminary validation of an empowerment instrument for patients with long-term conditions in primary care.Methods: The study involved three methods. Firstly, a systematic review was conducted to identify existing empowerment instruments, and to describe, compare and appraise their content and quality. The results supported the need for a new instrument. Item content of existing instruments helped support development of the new instrument. Secondly, empowerment was explored in patients with long-term conditions and primary care practitioners using qualitative methods, to explore its meaning and the factors that support or hinder empowerment. This led to the development of a conceptual model to support instrument development. Thirdly, a new instrument for measuring empowerment in patients with long-term conditions in primary care was developed. A cross-sectional survey of patients was conducted to collect preliminary data on acceptability, reliability and validity, using pre-specified hypotheses based on existing theoretical and empirical work. Results: Nine instruments meeting review inclusion criteria were identified. Only one instrument was developed to measure empowerment in long-term conditions in the context of primary care, and that was judged to be insufficient in terms of content and purpose. Five dimensions (‘identity’, ‘knowledge and understanding’, ‘personal control’, personal decision-making’, and ‘enabling other patients’) of empowerment were identified through published literature and the qualitative work and incorporated into a preliminary version of the new instrument. A postal survey achieved 197 responses (response rate 33%). Almost half of the sample reported circulatory, diabetic or musculoskeletal conditions. Exploratory factor analysis suggested a three factor solution (‘identity’, ‘knowledge and understanding’ and ‘enabling’). Two dimensions of empowerment (‘identity’ and ‘enabling’) and total empowerment showed acceptable levels of internal consistency. The measure showed relationships with external measures (including quality of chronic illness care, self-efficacy and educational qualifications) that were generally supportive of its construct validity.Conclusion: Initial analyses suggest that the new measure meets basic psychometric criteria and has potential for the measurement of patient empowerment in long-term conditions in primary care. The scale may have a role in research on quality of care for long-term conditions, and could function as a patient-reported outcome measure. However, further validation is required before more extensive use of the measure.

Syftet med denna undersökning var att undersöka förekomsten av tillitsbaseratledarskap och i vilken grad tillitsreformen påverkat det tillitsbaserade ledarskapetsförekomst i de högsta kommunala ledningsgrupperna. Det teoretiska antagandet harvarit att ett engagerat ledarskap har stor betydelse för att förbättringsarbeten skalyckas och att den högsta ledningens egna beteende utgör en viktig förebild som kanskapa tillit men även misstro hos de som förväntas utföra det praktiska arbetet.Undersökningen genomfördes genom en webbaserad enkät som riktades tillkommunchefer och kommunfullmäktiges ordföranden i samtliga sverigeskommuner. Frågan som ställdes var hur de kommunala ledarna bedömer sitt egetoch statens tillämpande av tillitsbaserad styrning och ledning och hur tillitsreformenupplevs ha påverkat detta tillämpande. Undersökningens svarsfrekvens har intevarit tillräcklig för att säkerställa statistisk generaliserbarhet för landet som helhet,de resultat som uppnåtts antyder dock att de kommunala ledarna skattar varandrastillämpning av tillitsbaserad styrning och ledning högre än vad de gör statens. Denförändring i ledarskapet som skett till följd av tillitsreformen har framförallt sketthos de kommunala chefstjänstemännen. Likaväl tycks det vara de kommunalachefstjänstemännen som överlag är mest kritiska till statens tillämpning av etttillitsbaserat ledarskap. Statens tillitsbaserade ledarskap är också det som anses haförändrats minst av tillitsreformen. Inom kommunerna tillämpar de förtroendevaldaledarna framförallt principen medborgarfokus medan tjänstemännen framförallttillämpar ledningsprinciperna stöd och öppenhet. Ledningsprinciperna kunskap,medborgarfokus och öppenhet är de som staten till störst del tillämpar. Principentillit framstår som den minst tillämpade principen såväl mellan förtroendevalda ochtjänstemän inom kommunernas ledningsgrupper som i statens styrning och ledninggentemot kommunerna men är samtidigt den princip vars tillämpning utvecklatsmest positivt inom de kommunala ledningsgrupperna. Analysen har inte kunnatstyrka att något uppenbart samband föreligger mellan statens och de kommunalaledarnas engagemang i tillitsreformen vad gäller tillämpningen avledningsprinciperna. Den analytiska slutsats som dras är istället att kommunernasrepresentanter uppfattar det statliga engagerade ledarskapet i tillitsreformen somrelativt svagt. Detta kan därför tänkas haft en hämmande effekt för tillitsreformensgenomslag även i kommunernas styrning och ledning.
The purpose of this thesis has been to examine the presence of trust-based controland management (tillitsbaserad styrning och ledning) and whether, and in that caseto what degree, the swedish trust reform (tillitsreformen) has affected the level oftrust-based control and management being practiced in the highest municipalmanagement groups. The theoretical assumption has been that a committedleadership (engagerat ledarskap) is essential to achieve success in qualityimprovement projects and that the actions of the most senior managementrepresentatives serve as an important role-model that can create trust, but just aswell disbelief, among those who are to carry out the work in practice. An onlinesurvey study has been directed to the most senior professional and politicallyappointed officials in all swedish municipalities. The question asked was to whichdegree those leaders rate each other’s practice of trust-based control andmanagement and how they view the practice of trust-based control and managementin the government’s and state authorities’ relations with the municipalities. Thequestion was also what change in that practice the trust reform has brought. Theresponse frequency has not been sufficient to make the findings generalizable on anational level. The results that have been achieved show that the municipalityleaders rate each other’s practice of trust-based control and management to a higherdegree than the government and state authorities’ dito. The results indicate that it isthe trust-based leadership of the professional municipal officials who overall havebenefited the most from the trust-reform, the professional municipality leaders arealso the most criticising of the state’s practice of trust-based control andmanagement. The state is also where the change in leadership practice based on thetrust reform is considered to have been the least prominent. The principle ofcitizen-focus (medborgarfokus) is the most prominent principle among thepolitically appointed leaders in the municipalities whereas the principle openness(öppenhet) is the primary principe among the professional leaders. The principles ofknowledge (kunskap), citizen-focus, and openness are the primary principles in thegovernment’s and state authorities’ practice of trust-based control and management.The principle of trust (tillit) is the principe with the lowest degree of practise both inthe relationship between professional and politically appointed officials withinmunicipalities and between the municipalities and the state but at the same timetrust is the principle that has seen the most positive change in it’s practice as a resultof the trust reform. The analysis has not been able to determine any correlationbetween the practice of trust-based control and management from the governmentand state authorities and the practice of the same among the municipal leaders.Instead the analytical conclusion is made that the committed leadership from thegovernment and the state in regards to the the trust reform so far has been regardedas relatively shallow by the municipal leaders and that could have had a negativeimpact on the implementation of trust-based control and management in themunicipal management groups as well.

Nurses experience stress, fear and anger while they are trying to reconcile their ideals/ goals about health care with its inadequacies and abuses (Jameton, 1984:5), while at the same time trying to stay true to their convictions (Lindh et al., 2010:552). Moral distress is experienced when nurses cannot adhere to these goals (Corley, 2002:637). Conflicting moral principles, stress-provoking and contradicting demands weaken the nurse’s sense of control, power and autonomy (Lützen et al., 2010:213). The current descriptions of moral distress inadequately define the concept, and this might lead to the inconsistent use of the term moral distress. Therefore, conceptual clarity is needed. Current available instruments measure antecedents and situations causing moral distress. Therefore, an instrument measuring the attributes of moral distress is urgently needed. Such an instrument might be used in a variety of clinical departments because it is not based on department-specific situations but on the attributes of moral distress. Moral distress has a great impact on the nurse, patient care and the organization. This research used Benson and Clark’s (1982) method of instrument development as a theoretical framework. It is the aim of this study to develop and validate an instrument to measure moral distress in the clinical health care context of the professional nurse. In order to attain this aim the following objectives were set: To conduct an integrative literature review to identify antecedents, consequences, attributes and empirical indicators of moral distress; to conduct interviews to explore professional nurses’ experience of moral distress; to develop an instrument to measure moral distress in professional nurses; to validate the instrument. A qualitative and quantitative research design with explorative, descriptive and contextual strategies was used. The research process was divided into phases. During Phase One, an integrative literature review was conducted and the population included all available national and international data on moral distress in nurses/ nursing and sampling included all-inclusive sampling. Data analysis was performed through descriptive synthesis.Phase Onealso included semi-structured interviews and the population included professional nurses working in hospitals and clinics in the North-West Province. The sampling method applied was purposive sampling. Tesch’s method was used as data analysis method. During Phase Two, a content validation was conducted and the population included experts in the field of moral distress and instrument validation, and purposive sampling was applied. Data collection was done through the instrument that was developedand data analysis was the content validity index. Phase Twoalso included a qualitative evaluation which was conducted and the population consisted of professional nurses working in hospitals and clinics in the North-West Province and purposive sampling was applied. Data was collected through the developed instrument and a focus group session. Data analysis was conducted through aconsensus discussion. During Phase Three, a pilot study was conducted and the population was professional nurses working ina hospital with different departments and clinics in the Free State Province. All-inclusive sampling was applied and the instrument that was developed was used as data collection. Data analysis included: Descriptive statistics, factor analysis (exploratory, confirmatory and Bartlett’s test of spherity), Cronbach’s alpha coefficient, correlations and ANOVA. According to the results from the face-, content-, exploratory and confirmatory, discriminant- as well as divergent validity, the instrument has been shown to be valid. The Cronbach’s alpha for the Moral Distress Instrument was deemed reliable. Finally, the research was evaluated and limitations were identified. Recommendations for nursing education, -practice, research and policy were formulated.

Women are provided labor support during delivery, can improve outcomes for mothers and babies, increase women’s adaptive capacity, and have positive and satisfying birth experience. In order for nursing to remain as a viable profession, nurses,competence is highly important to promote positive outcomes for mothers and babies.The testing tools of labor support were developed based upon the model of social support in Taiwanese clinical practice, and do not reflect the full range of professional labor support , and lack of evaluation that competence connotation of providing labor support for nurses .In light of this, it is highly important to develop, and easy-to-use testing tool that measures mothers,perceptions of professional labor support provided by the registered nurse during labor.Therefore, the purpose of this study was to employ the Bowers''s PPLST to translate into chinese and to assess the reliability and validity of the chinese version Perceived Professional Labor Support Tool , and apply to Taiwanese women. This study used questionnaires, 240 subjects was selected by convenience sampling in the obstetrics and gynecology ward of a central medical center, testing the chinese version Perceived Professional Labor Support Tool. Applied the SPSS 17.0 statistical software package for data collection and analysis, and used content validity, factor analysis, concurrent validity, internal consistency reliability, and test-retest reliability to test the reliability and validity of the chinese version Perceived Professional Labor Support Tool. This study results display that Its CVI was .95, and factor analysis found five factors, “Nursing Competency”, “concern needs”, “Communication –to pass information”, “physical and psychological comfort”, and“convey caring”, to account for 64.27% of the total variance. Its Cronbach''s αwas.95, and test-retest reliability coefficient was.88 in the one-to-two weeks. The five factors, its Cronbach''s α ranged from .75~.93, and test-retest reliability coefficients ranged from .78 ~. 87. Overall, this study results suggest that the psychometric properties of the the Chinese Version Perceived Professional Labor Support Tool has a good reliability and validity, may be apply to assess the women''s perceptions of the professional labor support in Taiwanese. In the future, this study can help healthcare professional staffs to understand the status of labor support, as a basis of the in-service education of Labor Support for obstetric nurses, and to increase the quality of obstetric care.

Every person who for the first time somehow finds himself in the hall where air traffic controllers work, has the impression of a very calm atmosphere. But it is deceptive. Outside this calm hides a huge internal tension. The tension of responsibility that each of the employees in the control room bears personally and cannot be transferred to someone else. And the point here is neither in clearly described procedures and rules, nor in the desire to prove that they can cope with the situation, but in the unusual nature of these people. Each of them throughout the nervous system feels that the dots that glow on the indicator are hundreds of human hearts, which they have no right to stop because of, sometimes, nonsensical errors. Therefore, no matter how difficult the situation, no matter how strong the tension, the most important task of the dispatcher is the obligation to remember, On average, about 3,000 people die each year in plane crashes worldwide. In comparison with these data we will give the same statistics of road accidents, and we will see that the indicator of victi is much bigger. This type of transport takes more than 30,000 lives, and only in Ukraine. That is, according to statistics, civil aviation is the safest mode of aero port. However, gradually the intensity of air traffic increases, the number of airlines and, accordingly, the transportation of both passengers and cargo. According to Eurocontrol statistics for the next 3 years in the European region is projected to increase air traffic by an average of 3.7% annually. Despite the introduction of the concept of flexible use of airspace, the activities of air traffic controllers are becoming more complex every year. They have to work in conditions of constant increased workload and stress. It is known that at relatively low levels of functional stress, performance is restored fairly quickly, and no harmful effects on the human body do not occur. However, in cases where the functional stress reaches levels at which the restoration of the spent functional potential during the work shift is impossible, conditions are created to reduce the efficiency of the employee. Usually short-term rest and change of activity promote restoration of the spent resources of an organism.

Objective: To compare and analyze different existing compassion fatigue (CF) scales, and to test reliability and validity to find out a more suitable evaluation tool of CF for Chinese front-line medical staff. Method: With a sample of 252 Chinese clinicians (doctors and nurses), this study compared the validation of the two most used CF scales, Compassion Fatigue Short Scale (CF-Short Scale) and Professional Quality of Life Scale (ProQOL) in Chinese healthcare setting. Exploratory factor analysis, correlation analysis, and Cronbach's α were employed to examine the reliability and validity of the Chinese version of the CF scale by front-line clinical nurses and doctors. Lastly, One-way ANOVA test was conducted to examine and compare the CF scores of the medical staff with different characteristics. Finding: The Compassion Fatigue Short Scale included two factors, explaining totally 64.273%of the total variance, and Cronbach's α of C-CF Short Scale = 0.918, Job Burnout (JB) = 0.892, and Secondary Trauma (ST) = 0.909. The ProQOL (C-ProQOL) Scale also had a good internal consistency, with Cronbach's α of Compassion Satisfaction (CS) = 0.925, Secondary Trauma (ST) = 0.925, and Burnout (BO)= 0.705. However, the construct validity of C-ProQOL Scale was unsatisfactory with some problematic items. The CF scores among medical staff was at a medium level and differed significantly by the number of hours worked, the number of night shifts, and other characteristics of the medical staff. Conclusion: The C-CF Short Scale has better applicability that can be used as a reliable CF measurement for Chinese medical staff.
Objectivo: Comparar e analisar as diferentes escalas de fadiga de compaixão (CF) existentes, e testar a fiabilidade e validade para descobrir uma ferramenta de avaliação mais adequada da CF para o pessoal médico chinês da linha de frente. Metodologia: Com uma amostra de 252 clínicos chineses (médicos e enfermeiros), este estudo comparou a validação das duas escalas CF mais utilizadas, a CF-Short Scale e a ProQOL Scale no contexto dos cuidados de saúde chineses. Análise exploratória dos factores, análise de correlação, e Cronbach's α were utilizado para examinar a fiabilidade e validade da versão chinesa da escala CF por enfermeiros clínicos e médicos da linha da frente. Por último, foi realizado o teste ANOVA unidireccional para examinar e comparar os resultados da CF do pessoal médico com características diferentes. Encontrar: A Escala Curta de Fadiga de Compaixão incluiu dois factores, explicando totalmente 64.273%of a variação total, e Cronbach's α da Escala Curta de C-CF = 0,918, Job Burnout (JB) = 0,892, e Secondary Trauma (ST) = 0,909. A Balança C-ProQOL também tinha uma boa consistência interna, com Cronbach's α de Compassion Satisfaction (CS) = 0,925, ST = 0,925, e Burnout(BO)= 0,705. No entanto, a validade de construção da Escala C-ProQOL foi insatisfatória com alguns itens problemáticos. A pontuação da CF entre o pessoal médico era de nível médio e diferia significativamente pelo número de horas trabalhadas, o número de turnos nocturnos, e outras características do pessoal médico. Conclusão: A C-CF Short Scale tem melhor aplicabilidade que pode ser usada como uma medição CF fiável para o pessoal médico chinês.

Hintergrund: Für eine sicherheitsorientierte Versorgung in der Zentralen Notaufnahme (ZNA) bedarf es einer Sicherheitskultur, die aufbauend auf einem transparenten Umgang mit Fehlern das Lernen fördert. Insbesondere in Hochzuverlässigkeitsorganisationen (HRO) wird das Lernen als Chance für die stetige Verbesserung der Sicherheit gesehen. Ein Einflussfaktor für eine sicherheitsorientierte Patientenversorgung ist das Individuum. Aufbauend auf den individuellen Kompetenzen können die Kompetenzen auf allen Ebenen weiterentwickelt werden, welches in einer Lernenden Organisation angestrebt wird. Die Gestaltung einer Sicherheitskultur in einer ZNA im Sinne der Hochzuverlässigkeit durch Kompetenzen von Individuen und Kompetenzentwicklungsmaßnahmen erfolgt durch die Verknüpfung von den Fachdisziplinen der Organisationstheorie und der Bildungswissenschaft. Methodik: Für die Identifikation von Kompetenzen und Kompetenzentwicklungsmaßnahmen wurde ein Multi-Methoden-Ansatz gewählt. Zunächst wurden mögliche Risikofelder in der ZNA durch eine Analyse von 230 Critical Incident Reporting System (CRIS)-Fällen aus der CIRSmedical Datenbank identifiziert. Daraufhin wurde ein Kompetenzkatalog: Patientensicherheit in der Zentralen Notaufnahme aufbauend auf dem Kompetenzkatalog Europäisches Curriculum für Notfallmedizin sowie 34 weiteren Quellen entwickelt. Neben dem Kompetenzkatalog wurde der Basiskompetenzkatalog: Patientensicherheit hochzuverlässig gestalten für die Identifikation von Kompetenzen zugrunde gelegt. Weitere Informationen hinsichtlich der Identifikation und Entwicklung von Kompetenzen wurden durch Interviews mit zehn Experten und sieben Führungskräfte eines Kooperationskrankenhauses generiert. Ergebnisse aus dem Multi-Methoden Ansatz: Die Sicherheitskultur wird durch die Führungskräfte, die Institutionalisierung von Risikomanagementinstrumenten und durch das Individuum gestaltet. Für Professionals in der ZNA bedarf es fachlicher, methodischer, personeller und hochzuverlässiger Kompetenzen, um interprofessionelle, interdisziplinäre und situationsadäquate Entscheidungen zu treffen. Zudem sind die situative Sensibilität und Resilienz erforderlich. Von den Interviewpartnern wurde der kontinuierliche Lernprozess als ein entscheidender Einflussfaktor für die Sicherheitskultur in einer ZNA im Sinne der Hochzuverlässigkeit bezeichnet. Die Kompetenzentwicklung kann neben Personalentwicklungsmaßnahmen auch durch Risikomanagementinstrumente erfolgen. Dennoch gibt es hinsichtlich der Kompetenzentwicklung von Professionals in ZNA auch Herausforderungen. Schlussfolgerung: Die Sicherheitskultur in einer ZNA im Sinne der Hochzuverlässigkeit kann durch ein Kompetenzset aus fachlichen, methodischen, personellen und hochzuverlässigen Kompetenzen sowie der kontinuierlichen Kompetenzentwicklung gestaltet werden. Da bisher ein differenziertes Grundverständnis für die Hochzuverlässigkeit in der ZNA vorliegt und lernfördernde Rahmenbedingungen zu schaffen sind, sollte die ZNA im Hinblick auf den Zusammenhang mit der Hochzuverlässigkeit als high reliability seeking organization (HR-S-O) bezeichnet werden.

part 1 Background: Stroke is the second leading cause of death and a major cause of adult disability in Taiwan. This research established correlations between pre-admission and discharge data in stroke patients to promote education of the general public, prevention, treatment and high standards of chronic care. Methods: A total of 790 stroke patients at Chung Shan Medical University Hospital from 2007 to 2009 contained in the Taiwan Stroke Registry were included in this study. The patients were classified into two major categories: ischemic and hemorrhagic. Thirteen variables, including time series variables: (1) Gender (2) Age (3) Education (4) Mode of arrival (5) Time from symptom onset to ED arrival (6) Time from ED arrival to ED physician assessment (7) Time from ED arrival to neurologist assessment (8) Thrombolytic therapy (9) Discharge status from ED (10) Reason for leaving ED (11) Reason for transfer to another facility (12) In-hospital main caregiver (13) Length of stay, were explored. Results: Our results showed that age, education, hours from symptom onset to the emergency department (ED) arrival, and length of stay in hospital were significantly associated with stroke incidence. Conclusions: These findings show the present situation and medical quality of medical care for stroke patients in Taiwan. The factors association model may assist in developing a set of improvement plans for the coming year. part 2 Background: This paper described the validation of a critical thinking disposition assessment (CTDA) to assess medical care professionals’ or students’ level of critical thinking disposition and to assess the psychometric properties of this instrument. Methods: To undertake and determine the reliability and validity of the CTDA. Assessments of psychometric equivalencies and similarities in reliability and validity across the CTDA and CTDS were conducted. Using Pearson product-moment correlations to determine the concurrent validity between the scores of the CTDA and CTDS. Statistics were calculated using the SPSS (version 14.0), set at a significance level of 0.05. Conclusions: In light of these findings, CTDA was found to be a valid and reliable instrument to measure critical thinking disposition for medical care professionals and students.
