AbstractThis study determined if robotic-arm assisted total knee arthroplasty (RATKA) allows for more accurate and precise bone cuts and component position to plan compared with manual total knee arthroplasty (MTKA). Specifically, we assessed the following: (1) final bone cuts, (2) final component position, and (3) a potential learning curve for RATKA. On six cadaver specimens (12 knees), a MTKA and RATKA were performed on the left and right knees, respectively. Bone-cut and final-component positioning errors relative to preoperative plans were compared. Median errors and standard deviations (SDs) in the sagittal, coronal, and axial planes were compared. Median values of the absolute deviation from plan defined the accuracy to plan. SDs described the precision to plan. RATKA bone cuts were as or more accurate to plan based on nominal median values in 11 out of 12 measurements. RATKA bone cuts were more precise to plan in 8 out of 12 measurements (p ≤ 0.05). RATKA final component positions were as or more accurate to plan based on median values in five out of five measurements. RATKA final component positions were more precise to plan in four out of five measurements (p ≤ 0.05). Stacked error results from all cuts and implant positions for each specimen in procedural order showed that RATKA error was less than MTKA error. Although this study analyzed a small number of cadaver specimens, there were clear differences that separated these two groups. When compared with MTKA, RATKA demonstrated more accurate and precise bone cuts and implant positioning to plan.

Charging schemes utilizing mobile wireless chargers can be applied to prolong the lifespan of a wireless sensor network. In considering charging schemes with mobile chargers, most current studies focus on charging each sensor from a single position, then optimizing the moving paths of the chargers. However, in reality, a wireless charger may charge the same sensor from several positions in its path. In this paper we consider this fact and seek to minimize both the number of charging locations and the total required charging time. Two charging plans are developed. The first plan considers the charging time required by each sensor and greedily selects the charging service positions. The second one is a two-phase plan, where the number of charging positions is first minimized, then minimum charging times are assigned to every position according to the charging requirements of the nearby sensors. This paper also corrects a problem neglected by some studies in minimizing the number of charging service positions and further provides a corresponding solution. Empirical studies show that compared with other minimal clique partition (MCP)-based methods, the proposed charging plan may save up to 60% in terms of both the number of charging positions and the total required charging time.

<em>The growing number of Defined Contribution Plans available in the U.S. has presented many in the U.S. work force with investment decisions. Decisions many of these workers have not been faced with in the past. An analysis of the Survey of Consumer Finance data over the early portion of the sample period, 1992-2010 has shown that the annual average percentage of liquid retirement plan savings allocated to equity assets increased from 43.02% to 66.42%. In 2010, after the subprime crisis, the allocation to equity declined but remained high at 50.65%, even though the market in 2001 and 2008 changed the allocation for many participants. Researchers attempt to find the reason that retirement savings plan participants have chosen to take a riskier position over the years. After controlling for factors such as age and income classifications, this study finds the educational classification to be a significant factor in explaining the percentage that participants allocate to the relatively riskier assets.</em>

This paper addresses the problem of searching for a located target in the plane by using one searcher starting its motion from the point . The searcher moves along parabolic spiral curve. The position of the target has a known distribution. We show that the distance between the target position and the searcher starting point depends on the number of revolutions, where the complete revolution is done when . Furthermore, we study this technique in the one-dimensional case (i.e., when the searcher moves with linear search technique). It is desired to get the expected value of the time for detecting the target. Illustrative examples are given to demonstrate the applicability of this technique assuming circular normal distributed estimates of the target position.

Purpose: To present a methodology for aortic arch anatomy measurement to plan and size an arch endograft with 3 inner branches. Technique: The arch endograft is custom manufactured with 3 inner side branches. Computed tomography angiography should be used to measure the clock position, the distances between the supra-aortic vessels, and the length and diameter of the proximal and distal landing zones. On the planning sheet, the vertical axis on the grid represents the spiral stabilizing wire at the 12 o’clock position; the horizontal baseline at 0 mm represents the idealized proximal margin of the innominate artery (IA). The first inner branch for the IA would be at 12:30 clock position and −20 mm from the horizontal baseline, while the second inner branch would be at the 11:30 clock position and at 0 mm. The third inner branch would vary among the different potential positions. Conclusion: Preoperative measurements of aortic arch anatomy can be made using a standard methodology to plan the size and position of inner branches. Future studies will show the potential applicability of a standard 3-inner-branch arch endograft using the planning sheet.

Background: Dosimetric parameters of the planning target volume (PTV) and organs at risk (OARs) were compared among 3 different radiotherapy (RT) modalities in left breast cancer patients after breast-conserving surgery (BCS). Methods: Eleven patients with left breast cancer after BCS were enrolled and underwent CT simulation in the free breathing (FB) and deep inspiration breath-hold (DIBH) position. Three-dimensional conformal RT (3DCRT) and volumetric modulated arc therapy (VMAT) plans were generated for each patient in the DIBH positions. A 3DCRT plan was also created in the FB position. A dose-volume histogram (DVH) was used to analyze each evaluation index of PTV and OARs. The principal outcomes were PTV dose, heart dose, right breast dose, left anterior descending coronary artery (LADCA) dose, and left lung dose. Results: For 3DCRT plans, significant dose reductions were demonstrated in all evaluation parameters of the heart, LADCA, and left lung doses in the DIBH position compared with those in the FB position ( P < 0.05). In the DIBH position, significant dose reductions were found in the heart and LADCA in VMAT plans compared to those in 3DCRT plans ( P < 0.05). For the right breast, VMAT reduced Dmean significantly (0.32 Gy vs 0.08 Gy, P < 0.01). There were no significant differences between 3DCRT and VMAT plans for the left lung dose in the DIBH position. The indicators of PTV had no significant difference between the 3 plans. Conclusion: DIBH and VMAT could reduce dosimetric parameters of the OARs in left breast cancer patients after BCS. RT plans for left breast cancer after BCS can be optimized by DIBH and VMAT techniques to minimize radiation-induced toxicity.

Background: In the multimodal breast-conserving curative therapy of some high-risk breast cancer patients, extended external beam radiotherapy (EBRT) not only to the breast but also to the supraclavicular fossa and the internal mammary chain (parasternal region (PSR)) is indicated. We report a dosimetric study on the EBRT of the breast (“B”) and the breast including PSR (“B + PSR”), comparing the supine and the laterally tilted prone patient positions in free breathing. Methods: The planning CT scans of 20 left- and 20 right-sided patients were analyzed. EBRT plans were calculated with 3D conformal EBRT (3D) and with intensity-modulated EBRT (IMRT) for “B” and “B + PSR” in the prone and supine positions. The mean and threshold doses were computed. The quality of EBRT plans was compared with an overall plan assessment factor (OPAF), comprising three subfactors, homogeneity, conformity, and radiogenic exposure of OAR. Results: In the EBRT of “B”, prone positioning significantly reduced the exposure of the OARs “heart” and “ipsilateral lung” and “lymphatic regions”. The OPAF was significantly better in the prone position, regardless of the planning technique or the treated breast side. In the EBRT of “B + PSR”, supine positioning significantly reduced the OAR “heart” exposure but increased the dose to the OARs “ipsilateral lung” and “lymphatic regions”. There were no significant differences for the OPAF, independent of the irradiated breast side. Only the IMRT planning technique increased the chance of a comparatively good EBRT plan. Conclusion: Free breathing prone positioning significantly improves plan quality in the EBRT of the breast but not in the EBRT of the breast + PSR.

Background: The physician workforce in Quebec is regulated by a government-controlled plan. Many specialty trainees expressed concerns about securing a position. Our objective was to analyze physicians’ employment issues in Quebec and their impact on residents’ training in specialty programs.Methods: We distributed a web-based self-administrated survey to all Quebec residents training in specialty programs to capture data about residents’ ability to find employment, career plans and perceptions regarding the workforce policy. Three groups were considered: graduates, non-graduating senior residents, and junior residents.Results: The overall response rate was 41.5% (985/2372). 47.3% of graduates did not have a position two months before finishing their training. Among residents without a position, 27.1% of graduates intend to leave Quebec, and 19.6% to complete a fellowship to postpone their start in practice. Overall, 77.9% of respondents believed there are not enough job opportunities for the number of trainees.Conclusion: Quebec specialty residents experience significant difficulties obtaining a position in the province and perceive that there are not enough job opportunities, which impacts their career plans and could drive them to complete a fellowship or plan to practice outside the province. Trainees' experience in finding employment needs to be considered in planning the physician workforce.

Development in multicellular organisms relies on cell proliferation and specialization. In plants, both these processes critically depend on the spatial organization of cells within a tissue. Owing to an absence of significant cellular migration, the relative position of plant cells is virtually made permanent at the moment of division. Therefore, in numerous plant developmental contexts, the (divergent) developmental trajectories of daughter cells are dependent on division plane positioning in the parental cell. Prior to and throughout division, specific cellular processes inform, establish and execute division plane control. For studying these facets of division plane control, the moss Physcomitrium (Physcomitrella) patens has emerged as a suitable model system. Developmental progression in this organism starts out simple and transitions towards a body plan with a three-dimensional structure. The transition is accompanied by a series of divisions where cell fate transitions and division plane positioning go hand in hand. These divisions are experimentally highly tractable and accessible. In this review, we will highlight recently uncovered mechanisms, including polarity protein complexes and cytoskeletal structures, and transcriptional regulators, that are required for 1D to 3D body plan formation.

Objective: This study analyzed the correlation between the average segment width (ASW) and gamma passing rate according to the multi-leaf collimator (MLC) position error. Method: To evaluate the changes in the gamma passing rate according to the MLC position error, 21 volumetric modulated arc therapy (VMAT) plans were generated using pelvic lymph node metastatic prostate cancer patient's data which is sensitive to MLC position errors as they involve several long, narrow, irregular fields. The ASW for each VMAT plan was calculated using our own code developed using Visual Basic for Applications (VBA). The gamma passing rate of the VMAT plan according to the MLC position error was evaluated using ArcCHECK (Sun Nuclear, Melbourne, FL, USA) while inducing symmetric MLC position errors in 0.25 mm intervals from −1 mm to +1 mm in the infinity medical linear accelerator (Elekta AB, Stockholm, Sweden). Finally, we examined the correlation between the change in the passing rate ([Formula: see text]) due to the MLC position error and the ASW in VMAT through linear regression analysis using the least squares method. Results: The ASW and [Formula: see text] were found to have a linear correlation according to the MLC position error, and the coefficient of determination was 0.88. For a 1 mm position error of MLC in VMAT, the gamma passing rate improved by approximately 11.9% as the ASW increased by 10 mm. Conclusion: These results are expected to be employed as guidelines to minimize the dose uncertainty due to MLC position error in VMAT.

AbstractPurposePatients treated with intensity-modulated radiation therapy (IMRT) for head-and-neck cancer are often positioned supine on a carbon fibre board to which a thermoplastic mask is attached to immobilise the head and shoulders. For patients unable to tolerate a supine position, we developed a tilting board that accommodates a full-scale head-and-shoulder mask.Materials and methodsPhantom measurements were obtained to confirm the dosimetric accuracy of our treatment planning system when using this board. A patient was simulated in the flat and tilted positions on the board. The two corresponding treatment plans were evaluated by comparing the target coverage and doses with organs at risk. The patient’s intra-fraction motion was quantified during his tilted treatments.ResultsPhantom measurements confirmed the accuracy of the dosimetric calculations. The tilted plan met dosimetric standards for clinical acceptability. The intra-fraction motion of the patient in the tilted position was >3 mm in any direction.ConclusionsThe tilting board met clinical requirements for IMRT planning and delivery. Full-scale head-and-shoulder immobilisation was achieved in a more tolerable tilted position.

Diabetes mellitus is the number one cause of limb loss in North America, and is associated with growing, unacceptable rates of morbidity, mortality and economic loss. Approximately 80% of these amputations are preceded by the development of foot ulceration. Various disciplines have studied the prevention and management of foot ulceration in those with diabetes. The present care plan was constructed to incorporate the important contributions from these disciplines into practical therapeutic guidelines. The care plan has been divided into three basic sections: assessment, general management and antibiotic therapy. Each of these sections is described in detail and borrows heavily from previous Canadian position papers. Application of the care plan is illustrated by multiple diabetic foot clinical scenarios, which have been categorized according to the Wagner classification.

Developing a Sustainability Plan at a Large U.S. College of Education Despite growing awareness of its importance, most sustainability education efforts in tertiary institutions do not significantly impact curricula. This paper details some of the activities and processes used to draft a sustainability strategic plan designed to address sustainability at the curricular level rather than merely the operational level within a large college of education at a large U.S. public university. The plan is also presented. Our goal was not to articulate a fixed policy but rather to produce a coherent plan that (1) fosters awareness and encourage people to join the effort and (2) readily accommodates input as more people become involved. The plan consists of three position statements, five broad recommendations and 20 specific actions aligned with the five recommendations. The hope is that our development processes, analyses and plan will be useful to other teacher education colleges and other groups with similar organisational structures interested in developing sustainability plans.

Although widely used in construction and industrial applications, wood is more prone to defects of different kinds than other materials. These defects are unpredictable and differing randomly from plank to plank. This uncertain nature of the defects complicates establishment of manufacturing plans. In this study, a probabilistic model of wood defects was constructed as a function of three variables which were quantity of defects, position of defects and size of defects. The Kolmogorov-Smirnov hypotheses testing on distributional forms of these variables were carried out. Results showed that Poisson, uniform, and log-normal distributions were suitable to represent the variables statistically. Being knowledgeable of how the defects are distributed on the plank will be of benefit in profitability justification of a cutting plan.

Studying the influence of defect on the dynamic behavior of wood in order to detect the local heterogeneities is of great importance in non-destructive testing of wood. The natural heterogeneities in wood are oriented in a volume. However, one-dimensional models are still used in dynamic characterization of wooden beams. The aim of this study was to experimentally investigate the effects of the orientation and position of an artificial defect on the flexural vibration frequencies. Different batches of Fagus orientalis specimens were drilled in the radial direction at five positions along the specimen. Dynamic tests in free flexural vibration were performed on the specimens before and after drilling both in the longitudinal-radial (LR) and longitudinal-tangential (LT) bending plan. The behavior in free flexural vibration was found to be different depending on the position and orientation of heterogeneity. When the drilling axis lies in the bending plane (LR), the weakening of frequency was maximal at the location of an antinode of vibration. On the contrary, the frequency offset was maximal in the place of a vibration node when the drilling axis was orthogonal to the bending plane (LT).

OBJECTIVEThe purpose of this study was to evaluate the effect of position on lumbar lordosis (LL) in adult spinal deformity (ASD) patients.METHODSThe authors evaluated the radiographic data of ASD patients who underwent posterior corrective fusion surgery from the thoracic spine to L5, S1, or the ilium for the treatment of ASD of the lumbar spine. The spinopelvic parameters were measured in the standing position preoperatively. LL was also evaluated in the supine position preoperatively and in the prone position on the surgical frame. Changes in LL were compared between groups.RESULTSEighty-five patients were included. The average LL in standing, supine, and prone positions was 11.8°, 24.3°, and 24.0°, respectively. LL increased significantly from standing to supine or prone position (p < 0.001). In 80 patients (94.1%), the difference between supine LL and prone LL was within 5°. Change in LL from standing to prone position was significantly higher in the severe deformity group.CONCLUSIONSThe lordotic effect of intraoperative prone positioning was remarkable in patients with severe deformities. LL in the supine position was approximately the same as that in the prone position. Therefore, assessing preoperative supine lateral lumbar radiographs enables one to plan corrective spinal surgeries in ASD patients.

ABSTRACT Objective: To evaluate the effectiveness of computer-assisted orthodontic treatment technology to produce the tooth position prescribed by the virtual treatment plan. Materials and Methods: Posttreatment models of 23 patients treated with SureSmile were digitally superimposed on their corresponding virtual treatment plan models utilizing best-fit surface-based registration. Individual tooth-position discrepancies between virtual treatment plan and actual outcome were computed. Discrepancies less than 0.5 mm in mesial-distal, facial-lingual, and vertical dimensions, and less than 2° for crown torque, tip, and rotation were considered clinically ideal. One-sided test of equivalence was performed on each discrepancy measurement, with P &lt; .05 considered statistically significant. Results: Mesial-distal tooth position was clinically ideal for all teeth with the exception of maxillary lateral incisors and second molars. Facial-lingual tooth position was clinically ideal for all teeth except maxillary central incisors, premolars, and molars, and mandibular incisors and second molars. Vertical tooth position was clinically ideal for all teeth except mandibular second molars. For crown torque, tip, and rotation, discrepancy exceeded the limits considered clinically ideal for all teeth except for crown torque on mandibular second premolars and crown tip on mandibular second premolars and first molars. Conclusions: The effectiveness of computer-assisted orthodontic treatment technology to achieve predicted tooth position varies with tooth type and dimension of movement.

The study aimed to establish the perceived importance that academy soccer practitioners placed on technical/tactical, physical, psycho-social player attributes during player selection and explore whether perceptions change according to Elite Player Performance Plan phase. Seventy academy practitioners working within Elite Player Performance Plan programs (Category 1: n = 29; Category 2: n = 13 and Category 3: n = 28) completed an online survey. Psychological factors were rated significantly ( p ≤ 0.01) higher than sociological, technical/tactical, and physical factors, with recruitment staff specifically valuing psychological factors significantly ( p ≤ 0.01) more than medical staff. Youth Development phase practitioners valued sociological factors significantly ( p < 0.05) more than in the Foundation phase, which was also true for physical factors. Practitioners indicated significant positional differences for most physical and technical/tactical attributes. There was no playing position effect for relative age effect or maturity. Between playing position variance of outfield players for most technical and physical attributes increased according to advancing Elite Player Performance Plan phase. Attitudes to holistic talent identification criteria likely change according to practitioner role. Therefore, this study provides evidence to suggest that Elite Player Performance Plan practitioners place less perceived importance on enhanced maturity status and relative age of players but does indicate an enhancing and significant positional preference for physical and technical/tactical attributes. Suggesting that practitioners are less likely to (de)select players based on transient, maturity-related attributes and instead place greater emphasis on specialist physical/technical position-specific attributes as players navigate the Elite Player Performance Plan pathway towards professional status.

Abstract Attaining sustainability in high-rise office buildings necessitates determining the major elements and their associating impacts on the energy performance of this building typology. This study investigates the impact of architectural and engineering features on the energy performance of high-rise office buildings within a warm-summer-cold-winter climate. A rectangular building plan form with a 1:1.44 plan ratio, vertical split core position and central atrium presented the best building performance. The plan form, core position and atrium effect accounted for 59, 30 and 11%, respectively, of an estimated 20.6% building energy savings. Furthermore, exploiting passive strategies founded on the climate and building features as defined by `PassivHaus’ standards further reduced the building energy usage.

This study aimed to investigate the factors that could be associated with the risk of labial cortical bone wall perforation with immediate implant placement (IIP) in the maxillary aesthetic zone, in a cone-beam computed tomography (CBCT) virtual study. CBCT exams from 126 qualified subjects (756 teeth) were included. Implants were virtually positioned in two different positions: in the long axis of the tooth (prosthetically-driven position) and in an ideal position in relation to adjacent anatomical structures (bone-driven position). Two different implant diameters were planned for each tooth position, namely, 3.75 and 4.3 mm for central incisors and canines, and 3.0 and 3.3 mm for lateral incisors. The incidence of perforation was nearly 80% and 5% for prosthetically- and bone-driven position, respectively. Factors associated with a higher risk of cortical bone wall perforation (bone-driven position), according to logistic regression analysis, were women, wider implants, Sagittal Root Position class IV, and decrease of the labial concavity angle. Perforation of the labial cortical bone wall can be greatly minimized when the implant is placed in a bone-driven position compared to a prosthetically-driven position. It is important to preoperatively evaluate the morphological features of the implant site for risk assessment and to individualize the treatment plan.

124 Background: Left-sided breast cancer patients receiving whole breast radiation therapy (WBRT) are potentially at increased risk for cardiac morbidity. We evaluated heart sparing of left-sided breast cancer patients when placed in the rolled vs. the standard supine body position for WBRT. Methods: Ten patients with left-sided breast cancer underwent planning CT scans in 2 different positions: supine and rolled to the right to allow for the heart to drop away from the chest wall. Patients were immobilized using vacuum cradles. The left lung, opposite breast, stomach, heart, left ventricle (LV), and left anterior descending coronary artery (LAD) were defined as organs at risk (OARs). Comparison 3D conformal tangential field plans were generated for each patient. Dose-volume histograms (DVHs) of the planning target volume (PTV) and OARs were examined. Results: All plans met PTV dose coverage and homogeneity criteria of V95 > 95% and maximal dose < 108. Median V105 for PTV was 12.75% (range, 2.5-16%). None of these parameters differed either statistically or clinically between plans done with the patient supine or rolled. The maximal doses to the opposite breast and stomach were also not significantly different between the 2 positions. The right lung V20 was similar for the 2 positions, 11.1% and 12%. Although there were no significant differences in the mean and max doses to the heart or LV, the mean heart dose decreased by 18-70% in 6 patients in the rolled vs. supine position and increased by 2.7-31.5% in 4 patients. The mean dose to the LAD and the LAD V10 and V15 were significantly decreased when the patient was rolled: 17.1 vs. 29.5 Gy, 41.1% vs. 62.6% and 40% vs. 66.7% (p < .05), respectively. Six patients had superior plans when in the rolled position with substantial differences in the mean and/or max doses and/or DVHs for the heart, LV and LAD. The remaining 4 patients had no overall improvement or worsening of the plan when the patient was rolled vs. supine. Conclusions: For patients with left-sided breast cancer receiving WBRT, the rolled position potentially reduces dose to the heart and its substructures without compromising the coverage and homogeneity of the PTV, hence limiting the need for IMRT.

The ideal gingival position will guide the professionals and determine the need to perform surgical procedures or orthodontic movement before placing the final restorations. In this article, the zenith-driven concept is discussed and a challenging case is presented with 4-year follow-up where tooth extraction, immediate implant placement, bone regeneration, and a connective tissue graft were performed.

AbstractPurposeTo evaluate the impact of leg position on the dose distribution during intracavitary brachytherapy for cervical cancer.Patients and methodsThis prospective study was performed on 11 women with cervical cancer who underwent intracavitary brachytherapy. After insertion of the brachytherapy applicator, two sets of computed tomography slices were taken including pelvis, one with straight leg and one with leg flexion position with knee support. The dose (7 Gy) was prescribed to point A. The radiotherapy plan was run on the Plato Planning Software System V14·1 to get the dose distributions. Also, rectum and bladder doses were measured for both leg positions during the treatment. The doses and volumes of organs were compared via the Wilcoxon signed-rank test by using Statistical Package for the Social Sciences 11·5 statistical software.ResultsNo significant difference regarding the dose distributions and volumes of target, sigmoid and bladder due to leg position was observed, either on 3D planning or on in vivo dose measurements. However, there were significant differences for 25 and 50% isodose coverage of rectum in favour of straight leg position (p=0·026). There were no significant differences regarding maximum doses in any critical organ.ConclusionDifference in leg position caused only a small change in rectum dose distribution and did not cause any other change in either dose distributions or in vivo measured doses of both target and critical organs during cervical brachytherapy. Straight leg position appears better with regard to rectum dose.
