In the framework of the Terrafirma project, Persistent Scatterers Interferometry (PSI) has be used for mapping land subsidence at basin scale in Gioia Tauro plain (Italy). The investigated area is built over unconsolidated fine-grained sediments, where the increasing groundwater demands for irrigation have caused the natural sediment consolidation to progressively accelerate. Both historical (1992-2001; ERS1/2 images) and recent (2002-2006; ENVISAT images) scenarios are analyzed to solve the spatial variability and temporal evolution of ground displacements affecting the plain. The results show deformation rates as high as 10-12 mm/yr in 1992-2007, with highest velocities occurred between 1992 and 2000 within the central part of the basin, in the area of Rizziconi (5 km ESE of Gioia Tauro). The outcomes of this PSI study could support the future improvement of groundwater management and the implementation of best strategies for land use planning and sustainable use of groundwater resources.

Calcium pyrophosphate (CPP) dihydrate crystals form extracellularly. Their formation requires sufficient extracellular inorganic pyrophosphate (ePPi), calcium, and pro-nucleating factors. As inorganic pyrophosphate (PPi) cannot cross cell membranes passively due to its large size, ePPi results either from hydrolysis of extracellular ATP by the enzyme ectonucleotide pyrophosphatase/phosphodiesterase 1 (also known as plasma cell membrane glycoprotein 1) or from the transcellular transport of PPi by ANKH. ePPi is hydrolyzed to phosphate (Pi) by tissue non-specific alkaline phosphatase. The level of extracellular PPi and Pi is tightly regulated by several interlinked feedback mechanisms and growth factors. The relative concentration of Pi and PPi determines whether CPP or hydroxyapatite crystal is formed, with low Pi/PPi ratio resulting in CPP crystal formation, while a high Pi/PPi ratio promotes basic calcium phosphate crystal formation. CPP crystals are deposited in the cartilage matrix (preferentially in the middle layer) or in areas of chondroid metaplasia. Hypertrophic chondrocytes and specific cartilage matrix changes (e.g. high levels of dermatan sulfate and S-100 protein) are related to CPP crystal deposition and growth. CPP crystals cause inflammation by engaging with the NALP3 inflammasome, and with other components of the innate immune system, and is marked with a prolonged neutrophilic inflitrate. The pathogenesis of resolution of CPP crystal-induced inflammation is not well understood.

When a moving stimulus is followed by certain transient events, an illusion of very fast motion is perceived, in which a random texture undergoes a slow rotation, but every second most observers perceive a very fast jump in the direction opposite to the preceding or “inducing” rotation. These jumps are illusory: during the perceived jump, every frame is a new, random texture, uncorrelated with the previous textures; the last of these random textures is set to turning once again, and the sequence repeats. Thus there is there is no specific correspondence between the textures before, during, and after the jumps and no motion energy corresponding to the jumps. These illusory jumps are called “high phi.” If the transient is immediately “undone,” a different kind of illusory motion is perceived, called “ghost phi.” This example shows a study of the effects of motion adaptation.

This chapter reviews advances in pathogenesis; European League Against Rheumatism/American College of Rheumatology (EULAR/ACR) classification criteria with clinical, laboratory, and ultrasound criteria for classification as polymyalgia rheumatica (PMR); the heterogeneity and overlap between PMR, inflammatory arthritis, and large-vessel vasculitis as illustrated by representative cases; recent guidelines on early and correct recognition, investigations, and management of PMR; the scope of disease-modifying agents; socio-economic impact, outcomes, and patient experience in PMR. It also discusses areas for future research including clinical trials with biological agents and newer steroid formulations, standardized outcome assessments, and the search for better biomarkers in PMR. PMR is one of the common inflammatory rheumatic diseases of older people and represents a frequent indication for long-term glucocorticoid (GC) therapy. It is characterized by abrupt-onset pain and stiffness of the shoulder and pelvic girdle muscles. Its management is subject to wide variations of clinical practice and it is managed in primary or secondary care by general practitioners (GPs), rheumatologists, and non-rheumatologists. The evaluation of PMR can be challenging, as many clinical and laboratory features may also be present in other conditions, including other rheumatological diseases, infection, and neoplasia. PMR is usually diagnosed in the primary care setting, but standard clinical investigations and referral pathways for suspected PMR are unclear. The response to standardized therapy is heterogeneous, and a significant proportion of patients do not respond completely. There is also an overlap with inflammatory arthritis and large-vessel vasculitis for which adjuvant disease-modifying medications are often used. Prolonged corticosteroid therapy is associated with a variety of side effects, especially when high-dose glucocorticoid therapy is employed. Giant cell arteritis (GCA) is also often linked to PMR. It is a vasculitis of large- and medium-sized vessels causing critical ischaemia. GCA is a medical emergency because of the high incidence of neuro-ophthalmic complications. Both conditions are associated with a systemic inflammatory response and constitutional symptoms. The pathogenesis is unclear. The initiating step may be the recognition of an infectious agent by aberrantly activated dendritic cells. The key cell types involved are CD4+ T cells and macrophages giving rise to key cytokines such as interferon-γ (implicated in granuloma formation), PDGF (intimal hyperplasia), and interleukin (IL)-6 (key to the systemic response). The pathogenesis of PMR may be similar to that of GCA, although PMR exhibits less clinical vascular involvement. The mainstay of therapy is corticosteroids, and disease-modifying therapy is currently indicated in relapsing disease.

A second-order reversed-phi stimulus is composed of moving features (areas filled with texture) whose overall amount of texture-contrast is reversed between successive frames. In peripheral vision, the stimulus is perceived as moving in the reversed direction (opposite to the feature displacement). In central vision, it is perceived in the forward direction at low temporal frequencies but in the reversed direction at high temporal frequencies. Moving the observer away from the displays has the same effect as changing from central to periphery vision: reversed motion becomes more dominant. The illusion demonstrates the different properties of the second- and third-order motion systems.

Progressive multifocal leukoencephalopathy (PML) is a demyelinating disease of the brain that results from a ubiquitous virus in the polyoma family of viruses. PML occurs almost exclusively in individuals with altered immunity. Until the inception of the AIDS pandemic, PML was an extremely rare disorder. Its more recent association with natalizumab and other immunomodulatory therapies has heightened interest in the disorder. This brief review discusses the pathogenesis, clinical and radiographic features, pathology, and therapy of the disorder. Additionally discussed are future directions with respect to therapy and research into this unusual infectious disease of the brain. Effective treatment requires long-term antibiotics.

Up to 90% of women of childbearing-age experience at least some degree of a regular recurrence of various physical and mental symptoms during the days prior to menstruation, which usually subside following menstruation. The cluster of symptoms can alter behaviour and well-being and affect family, friends, and relationships at work. Despite its prevalence, however, research has not yet demystified this enigmatic condition, commonly known as premenstrual syndrome (PMS) and premenstrual dysphoric disorder (PMDD)—severe PMS. Chapter 7 presents an exhaustive review that discusses the definition, diagnosis, prevalence, symptomatology, aetiopathogenesis, and therapeutic modalities of PMS/PMDD. It deliberates on the complex web of associated biopsychosocial factors. The discussion is further enhanced by presenting a real-life scenario of a sufferer with PMS who, in her dissatisfaction with the management provided, selected the option of surgical removal of her reproductive organs.

Inhaled beta-agonists and anticholinergic agents, as well as systemically administered methylxanthines, are frequently employed to achieve bronchodilation in critically-ill patients. Inhaled agents are given by pressurized metered dose inhaler (pMDI), nebulizer, or dry powder inhaler. In ventilator-supported patients, aerosolized agents are generally only administered by pMDI or nebulizer. The ventilator circuit, artificial airway, and circuit humidity complicate the delivery of aerosolized agents, and there is a wide variability in drug delivery efficiency with various bench models of mechanical ventilation. Aerosolized drug by pMDI is affected by the use of spacer devices, synchronization of pMDI actuation and ventilator breath delivery, and appropriate priming of the pMDI device. The efficiency of aerosolized drug delivery by jet nebulization is also affected by device placement in the circuit, as well as by a number of other factors. Several investigators have demonstrated comparable efficiency of aerosol delivery with mechanically-ventilated and ambulatory patients when careful attention is given to the technique of administration. Appropriate administration of aerosolized bronchodilators in patients receiving invasive or non-invasive positive pressure ventilation produces significant therapeutic effects.

An overview of permanent magnet synchronous (PMS) motors and the related control system are presented in this chapter as introductory materials for the rest of the book. The interconnections of the control system to the power electronic inverter and the motor are emphasized. In addition, the major parts of the system are overviewed. Pulse width-modulated voltage source inverter, as the most commonly used power converter in PMS motor drives, is briefly discussed. PMS motors configurations and operating principles are also presented after considering characteristics of permanent magnet materials. Major PMS motor control methods including vector control, direct torque control, predictive control, deadbeat control, and combined vector and direct torque control are briefly reviewed. Finally, several rotor position and speed estimation schemes, and offline and online parameter estimation methods are overviewed.

The chapter begins with a description of the scalar control of PMS motors. The fundamentals of PMS motor vector control (VC) are then presented with an eye on the analogy with DC motor operating principles. Current and voltage operating limits are incorporated into the control systems. This chapter presents dynamic and steady-state modeling of permanent magnet synchronous (PMS) machines with the help of reference frames. The modeling starts with a machine model in terms of phase variables. An equivalent two-axis model in a stationary reference is then obtained by a reference frame transformation. A further transformation to a two-axis rotor reference frame, with its direct axis aligned with the axis of a permanent magnet rotor pole, is derived. Another transformation to a two-axis stator flux linkage reference frame is also presented. Finally, a motor model in polar coordinates, based on space vector theory, is developed. In this chapter, PMS motor equivalent circuits are drawn, based on the mathematical models where appropriate. Iron losses and iron saturation are also taken into the models. The chapter ends with a brief presentation of the dynamic equation of PMS machines mechanical parts.

As one of the reforms supported by the New Public Management movement, Performance Management Systems (PMSs) have been implemented worldwide, across various policy areas and different levels of government. PMSs require public organizations to establish clear goals, measure indicators of these purposes, report this information, and, ultimately, link this information with strategic decisions aimed at improving agencies’ performances. Therefore, the components of any PMS include: (1) strategic planning; (2) data collection and analysis (performance measurement); and (3) data utilization for decision-making (performance management). However, the degree of adoption and implementation of PMS components varies across both countries and levels of government. Therefore, in understanding the role of PMSs in public administration, it is important to recognize that the drivers explaining the adoption of PMS components may differ from those explaining their implementation. Although the goal of any PMS is to boost government performance, the existent empirical evidence assessing PMS impact on organizational performance reports mixed results, and suggests that the implementation of PMSs may generate some unintended consequences. Moreover, while worldwide there is a steady increase in the adoption of performance metrics, the same cannot be said about the use of these metrics in decision-making or performance management. Research on the drivers of adoption and implementation of PMSs in developing countries is still lacking.

Abstract The goal of this volume is to contribute to the collective effort of building cumulative knowledge in the field of political communication. It does so by adapting, refining, and extending the Politics-Media-Politics (PMP) principle. PMP is an intentionally broad conceptual map that is designed to be adopted by researchers working in a variety of fields. The book’s chapters illustrate this concept map’s basic components, demonstrate how to apply it, and showcase its usefulness. The PMP principle can be best understood in two central propositions. The “politics first” proposition makes the following claim: The role of all forms of media in politics can best be understood as one in which variations in political ecosystems are the most important factors leading to variations in communication systems, values, practices, and resources. The “media selection and transformation” proposition is formulated thusly: All forms of media do not merely reflect the nature of the political ecosystem, they can also have an independent effect on political processes by selecting and transforming political events and issues into stories. When these two claims are combined, detailed, and developed, they provide a powerful conceptual structure for integrating research from a variety of fields in political communication. Chapters address the role of the media in election campaigns, historical change, violent conflicts and peace processes, comparative political communication, and how to use the PMP approach to assess media performance in democratic and autocratic regimes.

The third chapter offers a review of the literature on the PRI before the 1980s: how the party’s predecessor was formed, how it integrated millions of Mexicans into the party, and how its leaders were able to manipulate electoral rules to win elections at all levels of government for decades. The chapter also provides a new interpretation of the relation between federal bureaucracy and the party’s national organization by examining the relation among the successive presidents of Mexico, their Secretaries of Governance, and the president of the National Executive Committee (CEN) of the PRI. This is important because when the party lost the presidency, it also lost its third-party enforcer who had been responsible for maintaining party unity.

Cardiac catheterization is a procedure by which information is obtained from the heart by passing fine plastic tubes (occasionally other instruments) either near to or within the heart, to introduce contrast to cardiac structures to understand their anatomy and function better, to measure pressures, and/or to measure oxygen saturations in different cardiac chambers. It is an extraordinarily useful diagnostic procedure. Percutaneous coronary intervention (PCI) is the modern term for an intervention on a coronary artery that relieves narrowing. It includes balloon angioplasty and stent insertion. PCI is a therapeutic procedure.

It seems very plausible that actions are done as a result of the agent’s desire for something. At the same time we sometimes act on the basis of our practical deliberation. But it is hard to fit these thoughts together. Reasoning on the basis of the thought “I want phi” can take place whether or not this premise is true. It would be the same even if this thought were false, in which case the explanation of the resulting action would be the same even if the agent did not actually want phi. At the same time, if one acts on the basis of one’s deliberation it seems to follow that one wanted whatever it was one was aiming at. So apparently in this case the thought “I want phi” must be true, even self-verifying. This paper proposes a solution to these puzzles.

This chapter applies the conclusions of the previous chapters to two cases of controversial belief: atheistic belief and the denial of psychic phenomena (psi). In both cases, in line with so-called “phenomenal conservatism,” the felt obviousness—to the denier—of the non-existence of what they’re denying figures large in their knowledge, if they have it. In both cases, the deniers’ knowledge runs up counterarguments—for example, the fine tuning argument or meta-analyses of a number of controlled studies that purport to demonstrate the existence of psychic phenomena. But these arguments need not destroy knowledge, even if the atheist or psi-denier can’t figure out what’s wrong with those arguments. Knowledge can survive precisely because some atheists and psi-deniers lack sufficient methodological expertise, though it’s crucial that there is no consensus of trustworthy experts offering testimony that the counterarguments are sound.

This chapter explores how congressional campaigning changed for PRI candidates from the hegemonic to democratic era. Under hegemonic conditions, PRI candidates had huge resource advantages, and enjoyed the benefits of weak opposition parties and strong party identification. Under these circumstances, one should expect that the candidates for the hegemonic party would campaign as little as possible: that is, they would shirk. However, the agents sent down from the national party headquarters monitored their campaigning performance and obligated them to remake and strengthen the ties between local leaders and the national party. Once competition drove up uncertainty, PRI congressional candidates worked hard to win their election even though they could not hope for a second term. Despite their resource restraints, they campaign actively, while the national party headquarters has taken over national media appeals and strategies on programmatic promises. Governors were and continue to be important components in congressional campaigns, as they are able to support their co-partisan candidates.

Psychoneuroimmunology (PNI) examines the biological and behavioral pathways whereby psychosocial factors may influence the course of chronic medical conditions, including HIV/AIDS. This chapter summarizes PNI research conducted examining the possible role of negative life events (including bereavement), stress reactivity, personality factors, cognitive appraisals, and affective states (depression) in HIV illness progression. Because much of this research was conducted in the era prior to the advent of effective antiretroviral therapy, important questions remain regarding whether there the associations of psychosocial factors with HIV illness progression are independent of medication adherence and persistence. There is also increasing recognition that chronic viral infections such as HIV have neuropsychiatric effects, and more recent PNI research has focused on studying the bidirectional communication between the immune system and central nervous system in HIV. Future research should focus on obtaining definitive answers to these questions to inform the development of novel approaches for reducing psychiatric symptoms and optimizing health outcomes among persons with HIV.

Inorganic phosphate ions (H2PO4−/ HPO42−) (abbreviated as Pi) are involved in formation of bone and generation of high-energy bonds (e.g. ATP), metabolic pathways, and regulation of cellular functions. In addition, Pi is a component of biological membranes and nucleic acids. Only about 1% of total body Pi content is present in extracellular fluids, at a plasma concentration in adults within the range 0.8–1.4 mMol/L (at pH 7.4 mostly as HPO42−), with diurnal variations of approximately 0.2 mM. A small amount of plasma Pi is bound to proteins or forms complexes with calcium. Under normal, balanced conditions, absorption of dietary Pi along the small intestine equals the output of Pi via kidney and faeces. Renal excretion of Pi represents the key determinant for the adjustment of normal Pi plasma concentrations. Renal reabsorption of Pi occurs along the proximal tubules by sodium-dependent Pi cotransporters that are strictly localized at the apical brush border membrane. Parathyroid hormone (PTH) and FGF23 are key regulators amongst a myriad of factors controlling excretion of Pi in urine, mostly by changes of the apical abundance of Na/Pi cotransporters. Hypophosphataemia may result in osteomalacia, rickets, muscle weakness, and haemolysis. Hyperphosphataemia can lead to hyperparathyroidism and severe calcifications in different tissues.

Chapter 6 discusses trends in mass electoral behavior in Mexico from the 1980s to 2012. Mexico’s authoritarian regime held elections for municipal, state, and federal races from 1930 onward without interruption and the PRI’s candidates won almost every one of these elections through the end of the 1980s. From 1988, with the upswing in competition, voters opted for other options and their choices were respected. Once the PRI lost the 2000 election, however, its support did not collapse because the PRI’s brand name continued to offer Mexican voters the assurance of a certain type of pragmatic governing style. This proved a strong impetus toward party unity. In the decade after the transition, the PRI continued to be Mexico’s only “national” party that is able to win elections in all regions of the country: it is usually in first or second place in voters’ preferences from Baja California to Yucatán.

The fundamental principles of direct torque control (DTC) of permanent magnet synchronous (PMS) motors are presented in this chapter. The basic DTC system is then described. The operating limits of PMS machines under DTC are presented in terms of current limit, voltage limit, and flux linkage limit. Also, flux linkage control, including maximum torque per ampere (MTPA), unity power factor, and flux weakening at high speed, is derived. Then, alternative DTC schemes, including different SVM-DTC schemes, are presented. In line with the increasing energy-saving tendency in industrial applications, major emphasis is placed on the loss minimization of DTC. Finally, a comprehensive comparison was made between the basic DTC and vector control, emphasizing the pros and cons of DTC with respect to vector control.

Plato’s Moral Psychology is concerned with Plato’s account of the soul insofar as it bears on our living well or badly, virtuously or viciously. The core of Plato’s moral psychology is his account of human motivation, and PMP argues that throughout the dialogues Plato maintains that human beings have a natural desire for our own good, and that actions and conditions contrary to this desire are involuntary (from which follows the ‘Socratic paradox’ that wrongdoing is involuntary). Our natural desire for our own good may be manifested in different ways: by our pursuit of what we calculate is best, but also by our pursuit of pleasant or fine things—pursuits which Plato assigns to distinct parts of the soul, sometimes treating these soul-parts as homuncular sub-agents to facilitate psychic management, and other times providing a natural teleological account for them. Thus PMP develops a very different interpretation of Plato’s moral psychology from the mainstream interpretation, according to which Plato first proposes that human beings only do what we believe to be the best of the things we can do (‘Socratic intellectualism’) and then in the middle dialogues rejects this in favour of the view that the soul is divided into parts with good-dependent and good-independent motivations (‘the divided soul’). PMP arrives at its different interpretation through the methodology of reading dialogues with a close eye to the dialectical dependence of what the main speaker says on the precise intellectual problem set up between himself and his interlocutors.

The chapter motivates why and explains how users of such systems describe intended musical results with constraints. This approach to algorithmic composition is similar to the way declarative and modular compositional rules have successfully been used in music theory for centuries as a device to describe composition techniques. This systematic overview highlights the respective strengths of different approaches and systems from a composer’s point of view, complementing other more technical surveys of this field. This text describes the music constraint systems PMC, Score-PMC, PWMC (and its successor Cluster Engine), Strasheela, and Orchidée—most are libraries of the composition systems PWGL or OpenMusic. In this chapter, three control methods recently developed for or applied to electric motors in general and to permanent magnet synchronous (PMS) motors, in particular, are presented. The methods include model predictive control (MPC), deadbeat control (DBC), and combined vector and direct torque control (CC). The fundamental principles of the methods are explained, the machine models appropriate to the methods are derived, and the control systems are explained. The PMS motor performances under the control systems are also investigated. It is elaborated that MPC is capable of controlling the motor under an optimal performance according to a defined objective function. DBC, on the other hand, provides a very fast response in a single operating cycle. Finally, combined control produces motor dynamics faster than one under VC, with a smoother performance than the one under DTC.

Mexico’s Party of the Institutional Revolution (PRI) held executive power continuously from 1929 to 2000, when its candidate suffered a shocking defeat in the presidential elections. This study, which covers the years 1980–2012, uses an institutional focus to understand why the PRI survived its defeat and loss of the resources of the executive bureaucracy to return victoriously after two six-year terms out of office. The book offers a model of the difficulties authoritarian parties must face after they are ousted from the executive through fair and free elections: the danger of dramatic fractures that could destroy the party and the possibility of mass voter rejection. The institutional context of Mexico allowed the party’s factions to continue to cooperate and win elections. Mexico is a federal, presidential regime with a two-tiered electoral system, with no consecutive reelection and generous public party funding. The PRI changed dramatically in organizational terms as its directly elected state governors became power brokers within the party (though governors cannot be reelected). Yet, because of the nation’s electoral rules, the national party office remained a central player, both in party and national politics. The national party headquarters continued to mount an important response to the new government’s executive and coordinated the party’s legislators in Congress. The institutional context played a crucial role in creating spaces for both factions (the governors and the national party) and allowing them to cooperate. The former hegemonic party did not, however, develop a consistent ideology or try to purge itself of its clientelist or corrupt practices, because the governors had no authority strong enough to force them the change their conduct.

This chapter situates public health informatics (PHI) within an informatics perspective and the two broad trends relevant to its understanding; the first concerns medical and health informatics, which focuses on the application of information and communications technologies for the better understanding of the human body. This includes clinical, nursing, radiology, drug-related informatics, among others. This domain has been driven largely by clinicians, and informed by the disciplines of medicine, informatics, and cognitive and behavioural psychology. This field of study and practice has largely been focused on in North America and Europe, while low and middle-income countries (LMICs) have broadly been excluded. The second trend is of PHI systems, which have focused on the population, and practised the collection and use of aggregate statistics. This focus has largely been promoted by statisticians and demographers, and practised primarily in LMICs. These two trends have remained isolated from each other, yet evolved simultaneously.

This chapter explores how the Expanded PHI approach can inform the diagnosis of the fragmentation problem, and solutions to address it. Fragmentation is undoubtedly a ‘wicked’ problem, since trying to address it in one place may throw up new and unexpected issues in others. Integration is positioned as a modern solution, such as the architecture of open health information exchange to address the fragmentation problem, and significant efforts and money are being put in by ministries, donors, software vendors, and others in this area. Various global declarations have emphasized integration as a primary effort in health sector reforms in LMICs. Approaches have also often emphasized ‘big bang solutions’ that have typically not given adequate time for the solutions to take root. The Expanded PHI approach argues for incremental and bottom-up approaches informed by broader top-down visions.
