Повний текст джерелаLe piastrinopenie congenite sono un eterogeneo gruppo di patologie caratterizzato da ridotto numero di piastrine e si presentano con manifestazioni cliniche variabili da forme gravi a forme più lievi di riscontro casuale. Si suddividono in piastrinopenie da difetto della differenziazione e maturazione megacariocitaria, difetto di migrazione dei MK dalla nicchia ossea del midollo a quella vascolare e della elongazione delle pro-piastrine. Vi sono poi le piastrinopenie ereditarie da ridotta sopravvivenza piastrinica e le piastrinopenie a patogenesi ancora sconosciuta. Attualmente si conoscono almeno 20 geni i cui difetti genetici determinano un fenotipo caratterizzato da trombocitopenia ereditaria, ma circa il 50% dei pazienti hanno forme che non coincidono con nessun difetto noto. Negli ultimi anni sono emerse alcune forme di piastrinopenia ereditaria associate allo sviluppo di neoplasie di tipo ematologico e non ematologico che hanno destato un estremo interesse nei ricercatori; in particolare le forme associate a mutazioni di ANKRD26, RUNX1 e ETV6. Scopo Lo scopo di questo lavoro è quello di: 1. Costruire un database completo dei pazienti con piastrinopenia familiare finora pervenuti nel nostro laboratorio, definendo le mutazioni genetiche rinvenute. 2. Condurre uno studio preliminare sulla presenza di mutazioni in 5’UTR di ANKRD26 in pazienti con piastrinopenia sporadica e familiare, che non rientravano in forme note di piastrinopenia ereditaria e che presentavano volume piastrinico nella norma (4μm˂MPV˂8μm), definendone le caratteristiche cliniche e molecolari di questa nuova forma di piastrinopenia ereditaria e studiando le famiglie con mutazioni in questo gene. 3. Ampliare l’indagine ad altri due geni coinvolti in forme di piastrinopenia ereditaria con predisposizione a neoplasie ematologiche: RUNX1 ed ETV6. 4. Raccogliere e analizzare i dati relativi al corso della gravidanza in una ampia coorte di pazienti con forme di IT (inherited thrombocytopenia). Materiali e metodi Presso l’Unità di Clinica Medica 1 dell’Azienda Ospedale-Università di Padova è presente una casistica di circa 70 pedigree familiari di soggetti con piastrinopenia familiare da difetto ignoto. La valutazione comprende storia clinica e lo studio dell’espressione degli antigeni piastrinici di superficie mediante metodica citofluorimetrica utilizzando anticorpi monoclonali. Per valutare la morfologia piastrina è stato adottato il metodo della colorazione con May-Grunwald/Giemsa (MGG) su strisci di sangue periferico (macro, micro e normo trombocitopenia). Il DNA è stato estratto da sangue intero e amplificato per lo studio dei seguenti geni: GpIbα, GpIbβ, GpIX, MYH9, ACTN1. I prodotti delle reazioni di amplificazione sono stati visualizzati con elettroforesi su gel di agarosio marcato e quindi sequenziati con metodo di Sanger. Abbiamo condotto uno studio preliminare sulla presenza di mutazioni in 5’UTR di ANKRD26 in pazienti con piastrinopenia sporadica e familiare, che non rientravano in forme note di piastrinopenia ereditaria e che presentavano volume piastrinico nella norma (4μm˂MPV˂8μm) per poterne definire le caratteristiche cliniche e molecolari. Abbiamo quindi ampliato l’indagine a RUNX1 ed ETV6, due geni coinvolti in forme di piastrinopenia ereditaria con predisposizione a neoplasie ematologiche in pazienti piastrinopenici e non (MDS piastrinopenia +; MDS piastrinopenia -) appartenenti alle diverse classi di sindrome mielodisplastica sec. WHO 2008. Infine, nell’ambito delle piastrinopenie familiari confermate mediante indagine genetica, abbiamo partecipato al Pregnancy In Inherited Platelet Abnormalities (PIPA), studio multicentrico retrospettivo di valutazione di 339 gravidanze in 181 donne con 13 diverse forme di trombocitopenia. Risultati 8 famiglie (12%) presentano mutazioni nella MYH9, MYH9-related disease, 12 famiglie (19%) presentano mutazioni del gene GP1Bα, 4 famiglie (6%) presentano mutazioni del gene GP1Bβ che causano Bernard-Soulier Syndrome (BSS) mono o biallelico. Le rimanenti famiglie (60%) non presentano mutazioni genetiche sulle glicoproteine studiate, e nessuna mutazione sul gene ACTN1. Alla luce dei recenti studi relativi alle forme di piastrinopenie ereditarie caratterizzate, nel nostro laboratorio abbiamo condotto uno studio preliminare sulla presenza di mutazioni in 5’UTR di ANKRD26 in pazienti con piastrinopenia sporadica e familiare, che non rientravano in forme note di piastrinopenia ereditaria e che presentavano volume piastrinico nella norma (4μm˂MPV˂8μm), identificando due diverse mutazioni in eterozigosi, localizzate nella regione del 5’UTR non tradotta del gene. Dato l’interesse nello studio delle piastrinopenie ereditarie associate allo sviluppo di neoplasie di tipo ematologico e non ematologico, in particolare dei geni RUNX1 ed ETV6, responsabili di trombocitopenia non sindromica a trasmissione autosomica dominante, abbiamo preliminarmente raccolto una piccola casistica di soggetti mielodisplastici piastrinopenici (9 soggetti), di soggetti mielodisplastici con conta piastrinica nei limiti di norma (12 soggetti) e di 27 soggetti di controllo con trombocitopenia ereditaria indeterminata; questo per rinforzare le evidenze circa una possibile associazione tra mutazioni dei geni citati, piastrinopenia e rischio di evoluzione mielodisplastica/leucemica. Di questi soggetti abbiamo raccolto i dati relativi all’età, sesso, cariotipo, numero di blasti, emoglobina e piastrine. I dati preliminari (Tabella 1) dimostrano che le mutazioni dei geni RUNX1 e ETV6 sono presenti solo nel gruppo dei mielodisplastici piastrinopenici. Mutazioni rinvenute nei geni RUNX1 ed ETV6 Anche se il coinvolgimento di questi geni nella piastrinopoiesi è noto, l’esatto meccanismo responsabile di trombocitopenia non è ancora chiaro. La rarità di queste patologie rende difficile raccogliere dati utili ad una analisi genotipo-fenotipo. Nello studio multicentrico (PIPA) abbiamo classificato 23 gravidanze (13 donne): 5 donne presentano bBSS, 3 mBSS, 4 MYH9-RD, 1 ANKRD26. L’età media alla diagnosi di trombocitopenia è di 30 anni, la conta media piastrinica prima del parto è di circa 60 x 109 piastrine/L, anche se in alcuni casi è stata osservata una drastica riduzione piastrinica. La tendenza al sanguinamento è bassa prima del parto e solo in due gravidanze è stata necessaria la trasfusione di piastrine. Tutte le nascite sono avvenute tra la 34a e 40 a settimana; 13 sono i nati con parto cesareo e 10 con parto naturale. Solo 9 neonati riportano la stessa patologia della mamma, anche se in 6 casi il dato non è disponibile poiché non sono state fatte indagini a riguardo. Abbiamo riscontrato un unico caso di decesso neonatale a causa di emorragia cerebrale, ma nessun caso di sanguinamento nei nuovi nati. Conclusioni Le piastrinopenie rappresentano il più frequente disordine dell’emostasi e costituiscono un gruppo eterogeneo di entità cliniche. La peculiarità delle forme familiari risiede nella difficoltà della loro identificazione clinica poiché spesso le piastrinopenie vengono classificate come immuni ed i pazienti sottoposti a terapie inappropriate. Come riportato nei risultati di studi recenti che coinvolgono un numero di famiglie più elevato, anche nel nostro lavoro le forme più comuni di piastrinopenia familiare risultano essere la BSS, la MYH9-RD seguite dalla ANKRD26. Grazie ad una attenta analisi clinica ed alle indagini genetiche associate a recenti studi molecolari, ad oggi è possibile individuare e classificare correttamente i pazienti con nuove mutazioni genetiche considerando che ancora più del 50% delle famiglie non ha diagnosi. Per valutare possibili meccanismi implicati nello sviluppo di piastrinopenia in corso di sindromi mielodisplastiche sarà indispensabile studiare in modo estensivo queste tre categorie di pazienti: piastrinopenie ereditarie con difetto noto di ANKRD26, RUNX-1 e ETV6, mielodisplasie o leucemie con prevalente piastrinopenia. Aumentare la casistica di tali soggetti ci permetterà di approfondire lo studio funzionale delle mutazioni definendone le caratteristiche cliniche e molecolari, al fine di valutare un possibile link tra mielodiplasia e piastrinopenia e comprendere a che livello della megacariopoiesi e dell’ematopoiesi esse agiscano

Повний текст джерелаRIASSUNTO INTRODUZIONE: la trombopoiesi è un processo multifattoriale dipendente da vari fattori tra cui microambiente midollare, cellule di superficie e fattori di crescita ematopoietici. Il principale fattore che regola la produzione piastrinica è la trombopoietina (TPO), una proteina di 332 aminoacidi, con peso molecolare pari a 95 kDa, contenente due domini: quello amino-terminale con omologia strutturale con l’eritropoietina e quello carbossi-terminale coinvolto nel mantenimento della stabilità della TPO. La TPO è sintetizzata prevalentemente a livello epatico e presenta livelli inversamente correlati al numero di piastrine e alla massa megacariocitaria anche se vi sono delle eccezioni quali i casi di massiva distruzione piastrinica o di trombocitosi reattiva. La TPO aumenta dimensioni e numero dei megacariociti, favorisce l’espressione dei marcatori di superficie per le piastrine (CD41 e CD61) e stimola poliploidia dei megacariociti. Il recettore per la TPO è il cMPL, costituito da un dominio extracellulare e da uno intracellulare, espresso soprattutto a livello di cellule CD34, megacariociti e piastrine. Esistono diverse isoforme del cMPL, K, P e Tr, di cui quella fondamentale per la trasduzione del segnale è la P. Il legame della TPO al suo recettore determina l’omodimerizzazione del recettore con conseguente fosforilazione di JAK2 e attivazione della via di trasduzione del segnale. Lo scopo del nostro studio è stato quello di valutare i livelli di TPO e l’espressione di cMPL in diversi disordini piastrinici, in soggetti splenectomizzati e in quelli in terapia con agonista del TPO-R 6 MATERIALI e METODI: le patologie da noi considerate sono state piastrinopenia immune primaria, piastrinopenia costituzionale, forme di ipoplasia midollare e sindromi mielodisplastiche; come gruppo di controllo sono stati considerati soggetti sani e soggetti con trombocitemia essenziale. La TPO è stata dosata con metodica ELISA, il cMPL è stato valutato sia in citofluorimetria sia con Western Blot. La valutazione statistica è stata effettuata con il software GraphPad Prism. RISULTATI: lo studio del cMPL in citofluorimetria non è stato possibile per l’impossibilità a confrontare i risultati ottenuti data l’ampia variabilità interassay. I soggetti con ipoplasia midollare presentano i più alti livelli di TPO dell’intera casistica; nei soggetti con ITP si hanno livelli di TPO superiori ai soggetti sani, alle ET e alle forme familiari e inferiori ai soggetti con ipoplasia midollare. I soggetti con ITP hanno un’espressione di cMPL superiore ai controlli e alle ET, ma del tutto sovrapponibile alle ipoplasie midollari. Nei soggetti splenectomizzati i livelli di TPO e l’espressione del cMPL si è ridotta mentre non vi sono state modifiche nei soggetti trattati con agonista del TPO-R. DISCUSSIONE: analogamente a quanto descritto in letteratura anche nei nostri soggetti con ITP, la TPO è solo modestamente aumentata. I soggetti con ipoplasia midollare, così come quelli con mielodisplasia hanno un’aumentata espressione di cMPL e un aumento dei livelli di TPO. La correlazione tra TPO e cMPL nelle ITP è risultata lineare mentre nelle MDS vi è una correlazione inversa a sottolineare un possibile ruolo modulatorio da parte delle diverse isoforme del recettore. I dati relativi ai soggetti trattati, chirurgicamente o no, sono sovrapponibili a quelli descritti in letteratura ma sono relativi a un gruppo estremamente piccolo di pazienti

Повний текст джерелаNext Generation Sequencing (NGS) technologies, such the Ion Torrent platform, could allow to simplify the diagnostic process of diseases characterized by an high genetic and phenotypic heterogeneity, because of the possibility to sequence simultaneously more genes and more patients in a single sequencing run. In order to develop a new diagnostic algorithm for rapid molecular diagnosis of these disorders, we have applied the Ion Torrent technology on two different genetically heterogeneous diseases, Fanconi anemia (FA) and inherited thrombocytopenias (IT). Since FA is a disorder better characterized than ITs, we first validated the Ion torrent technology on 30 samples (2 wild type and 28 FA), 25 of which were already analyzed with Sanger sequencing. Because of their low sequencing quality, we have excluded from this type of analysis 2 of the 28 FA samples. Then, comparing Ion Torrent and Sanger sequencing data, we have evaluated the sensitivity (95%) and the specificity (100%) of Ion Torrent technology. Moreover, in order to detect copy number variations (CNVs) in FA genes, we have improved a statistical analysis based on coverage sequencing data, confirming the presence of large intragenic deletions on FANCA in 5 patients. In summary we have characterized 25 of the 26 FA patients analyzed, identifying also 4 mutant alleles in the rare complementation group FANCL and FANCF and 10 mutations in loci different from genes causing the disease. Since we cannot exclude that new genes are involved in FA, the only patient without any mutation identified is suitable for whole exome analysis. Taking advantage from these good sequencing data, we have developed a diagnostic algorithm that combines the identification of both point mutations and CNVs. In order to verify if this new diagnostic process could be applied also to other genetically heterogeneous diseases, we have analyzed 21 IT patients, already characterized by Sanger sequencing. Among the 2225 variants identified by Ion torrent technology, using this new approach, we have select those (N=75, 56 different) potentially pathogenetic because of their frequency (MAF<0.01), or of their presence in IT mutation database o because of bioinformatics analysis. Thirty of these variants were confirmed by Sanger sequencing, 14 (12 different) of which localized in loci different from the gene causing the disease. It would be interesting to carry out functional studies on these additional variants to unravel the molecular basis of ITs. In summary we were able to characterized 17 of the 21 IT patients, including 2 patients with deletions in RBM8A (Thrombocytopenia and Absent Radii syndrome, TAR). The remaining 4 mutant alleles were not detected because of a low sequencing coverage. In conclusion, according to our data, we can consider the Ion Torrent technology and in particular the diagnostic algorithm proposed in our study, as a feasible approaches for the study of diseases characterized by high genetic and phenotypic heterogeneity. RIASSUNTO Le tecnologie di Next Generation Sequencing (NGS) consentono di analizzare più geni e più campioni contemporaneamente. In questo modo potrebbe essere possibile ridurre i tempi e i costi di analisi di tutte quelle patologie caratterizzate da elevata eterogeneità genetica e fenotipica, la cui caratterizzazione risulta essere spesso complessa e dispendiosa. Al fine di elaborare un nuovo algoritmo diagnostico che consenta la rapida elaborazione di una diagnosi molecolare di tali patologie, abbiamo deciso di validare una tra le più innovative tecnologie NGS attualmente in commercio, la metodica Ion Torrent, su due differenti malattie, entrambe caratterizzate da eterogeneità genetica. l’anemia di Fanconi (FA) e le piastrinopenie ereditarie (IT). Siccome la FA è una patologia meglio caratterizzata rispetto alle IT, durante la prima fase di questo lavoro di tesi abbiamo analizzato 30 campioni (25 dei quali già precedentemente analizzati con sequenziamento Sanger), di cui 2 wild type e 28 affetti. In seguito all’esclusione dalla nostra analisi di 2 campioni FA a causa di una bassa qualità di sequenziamento, abbiamo determinato la sensibilità (95%) e la specificità (100%) della nuova metodica confronto i dati di sequenziamento Ion Torrent e quelli Sanger a nostra disposizione. Inoltre, utilizzando i dati di copertura della sequenza, abbiamo messo a punto un’analisi statistica volta all’identificazione delle Copy Number Variation (CNV), confermando le delezioni a carico del gene FANCA presenti in 5 pazienti. Abbiamo quindi caratterizzato 25 dei 26 pazienti analizzati, identificando inoltre 2 casi con mutazioni nei rari gruppi di complementazione FANCF e FANCL e 10 mutazioni in loci differenti dai geni causativi. Poiché non escludiamo la possibilità che un nuovo gene possa essere coinvolto nella patologia, riteniamo che l’unico paziente ancora privo di diagnosi molecolare possa essere un buon candidato per lo studio dell’esoma. Infine, avvalendoci dei buoni risultati ottenuti, abbiamo elaborato un nuovo processo diagnostico con il quale identificare in modo semplice e rapido sia le mutazioni sia le CNV a carico dei 16 geni coinvolti nella FA. Nella seconda parte del nostro studio, abbiamo verificato se l’applicazione di tale algoritmo possa essere estesa anche ad altre patologie ad elevata eterogeneità genetica. Per questo motivo abbiamo analizzato 21 campioni affetti da piastrinopenie ereditarie, già precedentemente analizzati mediante sequenziamento Sanger. Grazie all’algoritmo proposto abbiamo potuto selezionare tra le 2225 varianti identificate le 75 (56 differenti) che sono risultate essere potenzialmente patogenetiche in base alla loro frequenza nella popolazione (MAF<0.01), alla loro presenza nei database di mutazione e all’analisi bioinformatica di patogenicità. Trenta (27 differenti) di queste varianti sono state confermate mediante sequenziamento Sanger, di cui in particolare 14 (12 differenti) presenti in geni diversi da quelli causativi. Alla luce di questo dato si rendono necessari studi funzionali su tali varianti al fine di comprendere i meccanismi molecolari alla base delle piastrinopenie ereditarie. Infine, utilizzando l’algoritmo proposto, è stato possibile confermare la diagnosi molecolare in 17 dei 21 pazienti IT, compresi i 2 affetti da trombocitopenia con assenza del radio (TAR) e portatori di una delezione sul cromosoma 1q21.1. I restanti 4 alleli mutati non sono stati identificati a causa di una bassa copertura di sequenziamento. In conclusione, in base ai dati raccolti sui campioni affetti da FA e IT, possiamo affermare che la tecnologia di sequenziamento Ion Torrent e l’algoritmo diagnostico da noi proposto sono degli strumenti utili per ottenere una diagnosi molecolare completa, veloce ed economica.
