Background: Emotional-behavioural problems in children present a barrier to engagement and participation in school occupations. Applying practice theory is an essential process to explore the impact of clinical problems and to orient clinical thinking to the domain of occupation. Purpose: Occupational therapy practice theory and frameworks are applied to the formulation of School-Based Filial Therapy, a viable treatment response to emotional-behavioural problems in children. Key issues: Bowen family systems theory, the Occupational Therapy Practice Framework IV and the Canadian Model of Occupational Performance and Engagement are applied to intervention formulation. Implications: School-Based Filial Therapy engages the dynamic interaction of the person, the occupations he/she engages in and the environment via therapeutic medium of play. It is consistent with the Occupational Therapy Practice Framework IV and the Canadian Model of Occupational Performance and Engagement and provides new possibilities as an intervention for occupational therapists working in children's mental health.

Success at university may be influenced by concerns with occupation and occupational performance. To understand occupations of concern and occupational performance among a sample ( N = 144) of university students in the Midwest United States, the Canadian Occupational Performance Measure was administered. Socially related ( n = 103), academic-related ( n = 75), and work-related ( n = 64) occupations were the three most frequently reported occupational concerns. Time management ( n = 79) was the most frequent person-level concern. Mean self-perceived competence in occupations was 29.83 ( SD = 7.18) out of 50 and mean performance satisfaction was 26.80 ( SD = 8.01) out of 50. There were no differences in occupational performance across gender, race/ethnicity, class standing, living environment, or work status. However, within participants, there was a significant and clinically relevant difference between performance satisfaction and self-perceived competence in occupation, t(143) = 7.052, p < .0005, d = 0.58. Students have varied occupations that they find important, and future research should explore how occupational performance and performance satisfaction influence university success.

Sexuality and occupation are two constructs studied in different fields of knowledge. Particularly, in occupational therapy and occupational science, their relationship has been sparsely explored from a philosophical perspective. In the following reflection, we present some philosophical approaches to sexuality, occupation, and performativity to inquire about the impact of discourses about sexuality on the practice of occupations. For this analysis, we take an example developed by Foucault in one of his classes at the Collège de France. We believe that this helps us question the established discourses surrounding truth, especially concerning sexuality. Subsequently, we address the concept of occupation, taking into consideration Schliebener’s proposal from existentialist philosophy. Understanding occupation as a dimension of being, we explain how it can be comprehended through Butler’s theory of performativity and, consequently, express a sexual dimension. Finally, we establish an interconnection between the preceding concepts, taking sexual dissidents as an example. We propose that this reflection can broaden the possibilities of understanding occupations as points of resistance against normative discourses about sexuality. This may be relevant for contemplating practice and research in occupational science and occupational therapy, as it expands the understanding of sexuality beyond mere activities and centers it as a form of expression of being through occupation.

In theories of occupational therapy, occupation serves as a conduit for creating meaning and well-being. A crucial component of occupational therapy is developing and maintaining hope of clients during periods of major difficulty. Understanding the relationship between hope and occupation can prove helpful in stressful situations, such as caring for children with chronic illnesses. The aim of the present study was to identify occupations that foster experiences of hope among mothers of adult children with mental illness. A qualitative approach, informed by a constructivist framework and thematic analysis, was used to interpret and explicate relevant occupation-related themes that fostered hope in four mothers. The mother-participants reported engaging in various occupations, yet only occupations that were imbued with spiritual significance fostered experiences of hope. This study brings to the forefront of occupational therapy discourse the issue of spiritual connectedness as a potential link between occupations and hope.

Maintaining relationships and interacting socially are essential aspects of the occupational performance of childhood and adolescence. Social participation occurs during many childhood occupations, such as play and school work. Occupational therapists assess and treat children with difficulties in social participation. Rather than assessing a child's social skills deficits in isolation, the use of occupation-based theoretical models guides clinicians to consider the individual in his or her environments and during occupations. Familiarity with existing models of social competence and available assessment tools provides occupational therapists with the basis for a comprehensive assessment of children. This paper presents an occupational therapy model (the Model of Human Occupation), models of social competence from cognate fields and a range of assessment tools in order to guide occupational therapists in assessing and treating children with social participation difficulties in a more occupation-centred manner. The paper also presents a rationale for the use of multiple methods for a comprehensive assessment of a child's social competence.

This article proposes that the idea of meaningful occupations can be used by occupational therapists and occupational scientists to create organizational development interventions to help build sustainable business organizations. Review of recent organizational development literature indicates a trend toward an increasing lack of concern for individual employee needs. A conceptual model of organizational development that reinstates focus on the well-being of the individual as a basis for building sustainable organizations in which employee and organizational goals match is proposed for use by occupational therapists and occupational scientists who may be interested in contributing to this area of human endeavor. An occupation-based worker evaluation formula based on participation in occupations that are meaningful to an employee is suggested.

In the United Kingdom, a wide range of health care reforms has been introduced to enhance the wellbeing of older people. These reforms should ensure that both the public and the private sectors deliver best practice to older people. The role of the occupational therapist with older people is well established in a variety of health and social care settings but there is a noticeable absence of input in care homes, despite evidence that has demonstrated the importance of occupations for wellbeing. The Canadian Occupational Performance Measure (COPM) was used in a research study to determine the types of occupation that seven older people perceived as important, their perceptions of their performance and their perceived level of satisfaction. It was found that these older people most valued leisure and self-care occupations, although occupations related to productivity were also cited. A perceived high performance rating often transferred to a high satisfaction rating and a perceived low performance rating to a low satisfaction rating. The challenge for occupational therapists is to implement occupation-based therapy that meets all the needs of older people. Furthermore, there is a requirement to develop the evidence base and to look at strategies to promote occupation in order to ensure occupational satisfaction for all.

Occupational therapy theory, practice and research has increasingly emphasized the transactional relationship between person, environment and occupation. Occupational performance results from the dynamic relationship between people, their occupations and roles, and the environments in which they live, work and play. There have, however, been few models of practice in the occupational therapy literature which discuss the theoretical and clinical applications of person-environment interaction. This paper proposes a Person-Environment-Occupation Model of occupational performance which builds on concepts from the Occupational Therapy Guidelines for Client Centered Practice and from environment-behaviour theories. The model describes interactions between person, occupation and environment, outlines major concepts and assumptions, and is applied to a practice situation.

Aim and Background: Occupations are everyday activities that help occupy our time and provide us with a sense of purpose in life. Adolescence is a unique stage of life when individuals experience occupational change that assists with preparation for adulthood. Participation in occupations is linked to health and wellbeing. Existing literature on occupational participation appears to combine adolescents’ and children’s experiences. Due to the unique nature of adolescence and an increased focus on service development and re-design for all age groups in Australia since the implementation of the National Disability Insurance Scheme, this scoping review sought to explore Occupational Therapy interventions that promote adolescents’ participation in everyday occupations. Design and Methods: A systematic search of eight databases for peer reviewed papers published between 2008 and 2018 was conducted. The PRISMA guidelines were used to guide the review processes. Study selection criteria focussed on Occupational Therapy interventions promoting participation in everyday occupations for adolescents (between 10 to 19 years of age). The data was charted and synthesised to identify key features of the interventions. Results: The 26 included studies revealed various intervention types and adolescent groups. The majority of the interventions targeted adolescents with physical disabilities and autism spectrum disorder. Many studies focussed on promoting functional independence in work, personal care, social participation and health management. Features of interventions include collaborative (with the adolescent and family) intervention goal setting and evaluation processes, as well as the use of technology. The majority of the papers reported positive outcomes, however mixed results were also identified. The findings provide guidance for adolescent service development and re-design by identifying the areas of occupation and features of programs, documented in Occupational Therapy-specific literature. Future reviews can extend our understanding further by incorporating by including publications that focus on assessment and other roles of Occupational Therapists. Background. The paper is based on a keynote address delivered at the 2004 CAOT Conference in Charlottetown, Prince Edward Island Occupational therapists are widely associated with a medical model of health care in which recognition of how engagement in occupation contributes to health status is poorly understood. Occupational science as the study of people as occupational beings has the potential to increase such understanding. Purpose. This paper considers some aspects of the relationship between health and the occupations of older people to highlight avenues for change and the research required to support them. Method. The paper is structured around a simple verse of dialogue between a healthy old man and an occupational therapist. Explanation of the dialogue draws upon historical and current literature as well as occupational science research to provide a rationale for future practice based on broader concepts of occupation for health. Results and Practice Suggestions. The dialogue promotes the need for discussion about health and about the health notion of Active Ageing. It highlights professional language as one impediment to change and suggests that research concerning occupation as it relates to population health is a primary requirement for the future of occupational therapy.

Nonlinear dynamics systems theory, also referred to as chaos theory, is infiltrating the biomedical and social sciences, including the practice of occupational therapy. Consequently, the Neuro-occupation model has emerged for the treatment of different illnesses, in which a therapist strives to apply the benefits of this theory. This model provides occupational therapists with a method of anticipating the natural flux between underlying or hidden order and chaos in occupations, which allows individuals to function at their highest level. These occupations typically involve the interaction of numerous variables over time. By considering a nonlinear dynamics approach in illnesses such as alcoholism, occupational therapists may understand how meaningful occupations serve as a catalyst to facilitate change, which may assist in better understanding the effect of the therapeutic relationship with the clients they serve. This paper explains the nonlinear dynamics of the interventions in a single case study on alcoholism using the Neuro-occupation model.

Introduction Occupation-based practice is a core tenet of occupational therapy. This study explores perspectives and experiences with occupation-based practice among Thai occupational therapists. Method Using mixed methods, a focus group was conducted with eight occupational therapists, and the results were interpreted using thematic analysis. A questionnaire survey was also completed concerning practitioner perspectives ( n = 216). Findings Three major themes were identified: (a) perspectives on occupation-based practice were related to practice areas and felt to strengthen the identity of occupational therapy (52% strongly agree), leading to improved services and better outcomes for clients (44% strongly agree); (b) approaches focused on activities of daily living were appropriate for cross-practice areas; and (c) occupation-based practice is consistent with integrated medical sciences and occupations for clients (52% strongly agree). Conclusion Implementing occupation-based practice will be of value to clients, provide benefits as a compass to direct processes, and enhance clinical reasoning and outcomes.

Background. “Choice” is central to occupational therapy’s theoretical tradition, which maintains that individuals can impact their well-being through wisely choosing their occupations. However, the assumption that opportunities to choose are universally available is negated by research evidence. Purpose. To review the ideology of “choice” in occupational therapy theory, and to encourage more critical approaches toward determinants of occupational opportunity and choice. Key Issues. Evidence indicates that within Canada, and throughout the world, opportunities to make occupational choices are inequitably distributed among people of different socioeconomic classes, castes, genders, races, abilities, sexualities, citizenship statuses, and experiences of colonialism. Implications. Because occupation is a determinant of health and well-being, social injustices that create inequitable occupational choices are unfair violations of occupational rights. The occupational therapy profession’s espoused aim of enhancing well-being through occupation demands theories that explicitly recognize the socially structured and inequitable shaping of choice, and consequent impact on people’s occupational rights.

The objective of this study was to increase the evidence regarding the use of occupation-based practice and its effects on social and occupational participation in adults with spinal cord injury. Three individuals who had experienced a spinal cord injury 2 to 5 years previously, lived in the community, and received services from one of the two selected occupation-based occupational therapists participated in the study. Using a qualitative ethnographic design, they participated in one semi-structured, open-ended interview that contained questions designed to elicit experiences of life satisfaction, participation in meaningful occupations, social roles and responsibilities, and participation in occupational therapy. Data were analyzed using thematic analysis, narrative analysis, literature review, and narrative smoothing. Seven occupation-based approaches appeared to directly support the participants' current level of occupational and social participation. This study provides occupational therapists with a more practical understanding of how to apply these approaches in their own practices.

Introduction An occupational repertoire is the array of occupations that an individual has the perceived capacity to perform at a specific point in time. The Occupational Repertoire Development Measure – Parent is a new tool that uses parent report to identify the occupations children can and do perform and their interests in and opportunities for doing them. This study aimed to test the face validity, comprehensiveness, and internal consistency of the Occupational Repertoire Development Measure – Parent as a tool to measure the occupational repertoire of children aged between 2 and 12 years. Method Twenty-nine occupational therapists completed an online questionnaire about the Occupational Repertoire Development Measure – Parent, and 27 parents completed it and then provided feedback via a structured interview. Descriptive statistics, content analysis, and the content validity index guided data analysis. Results Participants view the Occupational Repertoire Development Measure – Parent as practical, simple, and supportive of occupation-centred practice to optimize children’s development of a meaningful repertoire. Overall, self-care occupations were rated highly relevant. Lower-scoring occupations were those perceived as performed only by older children, nonessential for children with disabilities, or culturally unimportant. Conclusion Results indicate value in the further development of the Occupational Repertoire Development Measure – Parent and validate that it asks relevant questions to understand a child’s performance, engagement, and opportunities, leading to optimal repertoire development.

Hoarding is often described as a medical disorder, defined by a persistent difficulty in discarding possessions and associated high levels of emotional distress when forced to part with these. This article will discuss how having a different view of hoarding, seeing hoarding as a daily occupation which provides value, purpose, and meaning and with a relationship to self-identity and life purpose, could offer alternate interventions to support an individual who hoards. The article will consider the components of hoarding activity and how these relate to health and wellbeing and doing, being, belonging, and becoming as understood by occupational therapists. The article will consider what occupational therapy, a profession which considers a person’s daily occupations, the things that occupy their time and which give meaning to their existence, could offer as an alternative to current hoarding interventions. Proposals for occupational therapy interventions will be suggested which would support occupational choice, support engagement in activities which have more positive outcomes on a person’s health, and seek to address barriers which limit engagement and occupational performance in activities within the person’s home environment.

Introduction The home context is considered a good location for children with cerebral palsy to practise meaningful occupations. The aim of this critical review was to gain a deeper understanding of how bimanual therapy and modified constraint induced movement therapy or constraint induced movement therapy methods are used within occupational therapy home programmes from an occupational perspective. Method A literature search was conducted using electronic databases for health research; five studies met the inclusion criteria. These were critically appraised and were analysed according to the relevant supports and barriers of the person–environment–occupation conceptual model of practice. Findings Family collaboration, strategic use of outcome measures, construction of the programme within the home environment and occupation-focused goals and activities emerged as commonly used methods. Enhanced descriptions of intervention context, the child’s voice in defining goals and challenges in optimising occupational balance surfaced as gaps in the programmes. Conclusion The methods used in implementing these approaches within occupational therapy home programmes requires combining motor and non-motor approaches, core occupational therapy skills, working within individual contexts and valuing family preferences. Using child-specific goal-setting instruments would strengthen the child’s voice and promote participation within a wider range of occupations.

Introduction. Practices of occupational therapists, particularly those supporting older persons with physical impairments, remain overly focused on remediating impairments, and implementation of occupation-centered practices remains fraught with difficulties. In Japan, this issue exists across the continuum from acute care to rehabilitation settings and into the community. This is despite the existence of international models and frameworks that place occupation at the core of the profession. Accordingly, there is a need to better understand how occupational therapists respond to the call for occupation-centered practices across the said continuum of care with this population. The aim of this study was at exploring and understanding occupational therapists’ experiences of supporting the resumption of occupations among older persons with physical impairments, in Japan. Methods. Embedded in a constructivist world view, this was a qualitative focus group study. Four focus groups (two in urban areas and one each in rural and semirural areas), consisting of seven or eight occupational therapists with at least three years of relevant practice experience, convened twice to narrate and explore their support of older persons. All were participating voluntarily with confidentiality of their participation being guaranteed by the researchers. They met for a third time to verify emerging analytic results. Data were analysed using a reflective thematic analysis. Results. Identified were three themes, namely, calling forth powers of occupations, imagining client’s future, and cocreating plots, which we synthesized into recurring cocreations from emerging opportunities. Discussion. Supporting the resumption of occupations among older persons with physical impairments hinges on repeated processes of identifying possibilities for occupation, followed by actions to bring these (e.g., images of clients’ future) into reality. Occupations’ healing properties (i.e., occupations’ powers) can be used to assist clients in experiencing health and well-being. The results suggest a reframing of occupational therapy practices as recurring processes of recognizing opportunities for occupation, followed by actions whereby these possibilities are turned into reality. Occupational therapy effectiveness might be enhanced when goals and methods are repeatedly and creatively aligned with the evolving plots cocreated between the client, therapist, and stakeholders.

Анотація:
Occupational deprivation is a relatively new term which describes a state in which people are precluded from opportunities to engage in occupations of meaning due to factors outside their control. As we face the new millennium, it seems likely that, due to widespread social and economic change as well as increasing civil unrest, occupational deprivation will be experienced by increasing numbers of people globally. This article describes the conceptual origins of occupational deprivation, presents definitions of the term and discusses specific populations that may be vulnerable to being occupationally deprived. Global, contextual issues of economic reform and technological advances are addressed with specific reference to these populations. Finally, consideration is given as to how an understanding of occupational deprivation is of relevance to occupational therapy and its concern with social and occupational justice.

Statement of context The Danish Health Authority recommends that patients with brain tumours should have their rehabilitation needs evaluated prior to hospital discharge. Critical reflection on practice To our knowledge, no specific recommendations for specialised occupational therapy intervention in patients with glioma have been published. We rationalise how occupational therapy practices founded on shared decision-making and common goal-setting are implicated to patients with brain tumours and elaborate on how an occupation-centred approach with occupation-focused and based intervention has the potential to impact a patient’s performance ability and satisfaction in performing occupations established by the Canadian Occupational Performance Measure. This practice was embedded in a randomised controlled trial investigating the effectiveness of intensive rehabilitation efforts and involving occupational therapy compared with standard care in patients with glioma (ClinicalTrials.gov Identifier NCT02221986). Implications for practice Occupational therapy makes an important contribution in neurorehabilitation, which may also apply to patients with brain tumours.

Occupational therapists believe that there is a relationship between occupation, health and well-being but there is little evidence in the occupational therapy literature to support this belief. This paper describes the results of a critical review of research examining the relationship between occupation and health and well-being. Twenty-two studies from the health and social sciences literature were reviewed using specific methodological review criteria. The findings of these studies provide moderate to strong evidence that occupation has an important influence on health and well-being. Because most of this research has been completed with persons without disabilities, further research is required to explain the nature of the relationship between occupation and health and well-being for persons who experience a disability which affects their daily occupations

Introduction Individuals with autism spectrum disorder demonstrate difficulty with joint attention, affecting social and occupational performance. Studies of joint attention in children with autism spectrum disorder employ a variety of instrumentation, environments and occupations. From the occupational therapy perspective, current literature lacks a rigorous analysis of the transactions of person, environment and occupation embedded within the procedures of these studies. The goal of this scoping review was to investigate how these components transact to affect occupational performance. Method Using the Person–Environment–Occupation model as an evaluative lens, a scoping review was completed to summarize person, environment and occupation transactions in studies examining joint attention in children aged 6–12 years with autism spectrum disorder. Results Six studies were included. Findings indicated that simplified social environments and demonstrations of joint attention promoted higher joint attention performance in children with autism spectrum disorder, at the cost of ecological validity. Maintaining ecological validity in complex social environments resulted in lower joint attention performance. Conclusion The Person–Environment–Occupation model can be used to develop an occupational therapy perspective on literature from outside the discipline. There is a relationship between the person, environment, occupation transactions and joint attention in children with autism spectrum disorder.

Recent changes in the philosophy and structure of the National Health Service give greater emphasis to the prevention of ill health within locally defined communities. Occupational therapists, by virtue of their unique philosophy, have an opportunity to influence primary care strategy and practice by highlighting the links between environment, occupation and health. The recent changes in the structure of the National Health Service are described and the philosophy of occupational therapy is discussed in relation to these changes. This description provides the basis for recommendations as to how occupational therapists can work to build a recognition of the fundamental importance of adaptive occupation to individual health and, hence, to health at a community and population level. Working at a community and population level will require occupational therapists to strengthen links with health promotion and public health in order to help promote health through meaningful occupations within local settings.

This study is the first to examine the cultural sensitivity of the Canadian Occupational Performance Measure (COPM), as reported by occupational therapists who have used it with people from an ethnic minority background. The research was conducted as part of a Master's degree, held in Boston, USA, and funded by the Elizabeth Casson Trust, UK. Twenty-five practising occupational therapists from North America answered an emailed survey and, from this sample, three were interviewed by telephone. The findings showed that the practitioners perceived the COPM as able to address the cultural occupations of ethnic minorities; however, they felt that including examples of cultural occupations and children's occupations and modifying the rating scales would improve the tool. Two of the three therapists interviewed worked with children and, because of this, there was a focus on children's occupations that was only revealed in the second part of the research. The nature of the tool, its ability to focus on occupation and the therapist's interview skills were identified as the reasons that the COPM is effective in addressing the cultural occupations of ethnic minorities. Additional research is needed using a larger sample to validate these findings.

This opinion piece describes central issues arising from discussions at a recent conference exploring the implications of global poverty for the occupational therapy profession. The connection between poverty, disability and the marginalisation that these problems produce presents an opportunity for occupational therapists to realise their potential for facilitating social change. To do so, however, entails some reconceptualising of the profession. In some areas of intervention, the struggle to obtain a clear definition for occupational therapy has both arisen from and contributed to a marginal status, linked to difficulties in developing capacity for research. The social questions around occupation suggest both challenges and opportunities for the profession.

Background. This descriptive paper offers reflections on power and justice associated with occupational therapy's client-centred practice, now described as the practice of enabling occupation. Framed as a contribution to the sociology of professions, the questions addressed are: How do power and justice work in occupational therapy today? What vision of power and justice guides the profession in implementing the client-centred practice of enabling occupation? Method. The paper opens with an overview of the analytic framework for reflections. Two sources for reflections are highlighted: the development of the Canadian guidelines and, client/consumer and occupational therapy perspectives. To illustrate the discussion of power and justice, two contrasting diagrams are presented, one on late 20th century power relations and the other on potential power relations. Results. Recommendations for research, education, practice, and guidelines development are offered prior to a conclusion that acknowledges the dissonance facing occupational therapists who struggle to focus on occupations in client-centred practice. Practice Implications. The paper offers insights and strategies for addressing power and justice as issues in implementing the client-centred practice of enabling occupation. The analysis may be used in raising awareness and guiding the strategic development of institutional change toward social inclusion and enabling occupation.

Occupational scientists have offered various theoretical propositions regarding the dimensions of meaning derived from engagement in occupations, and empirical studies have explored the personal meaning of engaging in a wide range of occupations. Purpose. The study aimed to distil the dimensions of personal meaning from existing research, with a view to informing theory in this area. Method. A qualitative metasynthesis of research studies was conducted, focusing on research reporting individuals’ accounts of the personal meaning of an occupation to them. Twenty papers met the search criteria and were reviewed, rated for quality, analyzed, and synthesized following a process of meta-aggregation. Findings. There were four common dimensions of personal meaning: a sense of fulfilment; a sense of restoration; social, cultural, and intergenerational connection; and identity shaping. Implications. This review provides occupational therapists with sound evidence of the link between engagement in a meaningful occupation and a sense of personal and social well-being.

Introduction Children with mobility difficulties are thought to share similar participation repertoires due to the impact of their physical impairments. However, with these children, as with all others, individual differences and contexts should mediate occupational repertoires and experiences, resulting in participation differences, at least in part. This study aimed to explore the occupational repertoires of children with mobility difficulties and their view of their occupational participation. Methods Five children with mobility difficulties were assessed using the paediatric activity card sort, to establish their occupational repertoires. Semi-structured interviews were used to understand the children’s occupational participation. Findings Our findings suggest that children with mobility issues are more likely to participate in quiet occupations, but not exclusively. The children defined participation in their own way. Conclusion The knowledge about how children with mobility difficulties understand participation in occupation can help occupational therapists to establish goals with their clients and personalise interventions.

Introduction The number of people surviving bowel cancer is increasing globally, with many of those affected living with long-term psychological and physical sequelae that potentially disrupt occupations. Method A scoping review – guided by Arksey and O’Malley’s methodological framework – was conducted to provide an overview of what is known about the impact of having had bowel cancer on occupations, and to reveal the potential role of occupational therapy. A systematic search of four databases and a grey literature repository generated 244 results. Applying inclusion and exclusion criteria, 33 items addressing bowel cancer and occupations were selected and thematically analysed. Results Researchers have identified six domains of occupation that are impacted by bowel cancer (social activity, physical activity, sexual activity, employment and role functioning, physical functioning, and self-care) but survivors report a distinct lack of health professional support after completion of medical treatment and being left to devise self-management strategies to accommodate the long-term effects of their cancer. Conclusion The sequelae of bowel cancer can have a significant impact on occupational participation. There is scope for increasing and improving occupational therapy input to assist people with cancer-related loss of function to re-engage in valued occupations and improve wellbeing.

Background. Survivors of stroke often experience environmental isolation and decreased occupational performance after discharge from the hospital. Peer groups benefit psychological, social, and cognitive functioning, though few studies have examined their influence on occupational performance of survivors of stroke. Purpose. This study explores the experiences of occupational performance in survivors of stroke attending an outpatient peer support group. Method. An interpretive qualitative study using semi-structured interviews was conducted with seven survivors of stroke attending an outpatient peer support group. Data was thematically analyzed. Findings. Four themes related to the experience of peer support on occupational performance emerged: finding hope to return to meaningful occupation, a place for belonging, problem-solving occupational concerns, and finding purpose beyond oneself. Implications. This research adds to the existing literature that peer support groups help survivors of stroke reengage in meaningful occupations, manage their stroke experience, and move positively through recovery.

Yoga is a complete system of occupational regulation, based on complex theories that address occupational risk factors while relating occupation directly to health and wellbeing. Although aspects of yoga have been used in Western occupational therapy settings, some of its underlying theories may have been overlooked. If validated by research, these theories may enrich occupational therapy philosophy. Yogic techniques of potential clinical use include pulse diagnosis, meditation and breath control. These are not described in detail in this opinion piece, but should be the subject of thorough literature reviews and, perhaps, further research.

Introduction Retirement from work is a complex process, with work influencing this transition. Occupational therapists can facilitate a meaningful occupational transition to retirement for better health and wellbeing in later life. This article explores (1) how work attributes influence and relate to the work-to-retirement transition stages, (2) the relationship of findings to the Model of Human Occupation volitional processes and (3) the potential occupational therapy role. Method Semi-structured interviews were completed with retirees. In this qualitative study, themes were identified in relation to the work-to-retirement transition stages using thematic analysis with findings related to the Model of Human Occupation volitional processes. Findings Eleven Australian participants (five females, six males) were recruited. Themes and sub-themes were identified within each stage of the work-to-retirement transition and collectively encapsulated all volitional processes. Themes were time to move on, taking action, ceasing work, feeling retired, letting go and work-related involvement. Conclusion Work attributes related to personal factors, social factors, meaningful occupational engagement and the nexus of work and retirement influence all work-to-retirement transition stages. Volition assists in explaining the complexity of this transition. Occupational therapists can assist older workers to continue working or identify and implement meaningful occupations to replace work activities in retirement.

AbstractDuring the process of community integration, individuals with acquired brain injury may experience difficulties in all areas of occupational performance including self-care, home management, community access, leisure, social activities and vocational pursuits. Community based rehabilitation services provide opportunities to minimise such difficulties by working with clients as they engage in meaningful real-life context-based occupations. The therapeutic use of occupation is at the core of occupational therapy practice. We reflect on the nature and principles of occupation and highlight particular benefits for facilitating community integration after brain injury. This is illustrated using the example of executive dysfunction. Several challenges for the occupational therapy profession arise from the shift in focus from hospital to community based rehabilitation, and the need for further research on community integration after brain injury from an occupational perspective is recognised.

Introduction Occupational therapists believe that meaningful occupations are linked to an individual’s health and wellbeing, and there is a growing literature exploring the personal meaning of specific occupations. However, few of these studies have focused on the personal meaning of active sporting occupations and their therapeutic benefits. This study aimed to explore the personal meanings attributed to the occupation of cycling, with a view to providing occupational therapists and others with an understanding of its therapeutic potential. Method Following approval by an ethics committee, seven men and women who cycled regularly took part in semi-structured interviews. These interviews were audio-recorded and transcribed. Interpretative phenomenological analysis was used to interpret the data. Findings The findings describe how and why cycling was meaningful. Cycling altered their mind-set and their emotions, contributed towards their identity and connected them to others. These themes concur with the findings of other studies and provide a rationale for their use therapeutically. Conclusion This study contributes to what is known about the value people derive from cycling, and its impact on their health and wellbeing. Occupational therapists can use this information for individual and group therapy as well as for guidance on the health and wellbeing of communities.

Scholarly databases were searched for documents relating to occupational therapy and PTSD. Two reviewers independently applied selection criteria and systematically extracted information. Data were extracted and synthesized in a narrative format. Findings. Fifty sources met inclusion criteria and three major themes were identified: recognizing trauma within specific populations, PTSD’s impact on a range of occupations, and occupational therapy’s response to PTSD. Implications. Occupational therapists are working in multidisciplinary teams to reduce the impact of PTSD on the daily occupations of their clients. More effectiveness studies are required to understand the outcomes of occupational therapy interventions for clients with PTSD.

Анотація:
The article illustrates the sociological theory and method of institutional ethnography by describing a study of occupational therapy's mental health practice. Over almost 6 months, the author collected data through observation, interviews, and review of documents. Data describe the practice of occupational therapy in seven adult mental health day programs with at least one site in each of the four provinces of Atlantic Canada. The question asked was, What are the possibilities and constraints for occupational therapists to enable the empowerment of adults who attend mental health day programs? Although developed as a feminist methodology for displaying the social organization of patriarchy, institutional ethnography is used in this study to show how the organizational context invisibly shapes occupational therapy practice. Through this method, occupational therapy ideas about occupation can be seen to be organizationally subordinated. Occupational therapists' work in enabling empowerment through occupations such as “chores” is relegated to the background to make practice fit prevailing medical and psychologic ideas about health.

Анотація:
At the close of the 20th century, there is a renaissance of occupation in occupational therapy and occupational science. Kielhofner (1992) offers an intraprofessional explanation that the growing interest in occupation recaptures occupational therapy's lost identity. An extraprofessional explanation is that postmodern ideas and social practices have helped to create a societal context in which a renaissance of occupation is welcome. Postmodernism raises questions and awareness of power, diversity, temporality, and situatedness in which normative ideas of occupation as paid work can be challenged. Since occupation is of primary concern to occupational therapy and occupational science, the authors reflect on postmodernism and its influence on a renaissance of occupation in these two fields. These reflections consider what such a renaissance means for occupational therapists and occupational scientists in the 21st century.

A ocupação é composta de ações com propósitos e significados, nas quais as pessoas se engajam no cotidiano e estruturam suas vidas. Estas ações agregam significados pessoais e culturais e são influenciadas pelo histórico de vida de cada indivíduo. Desta forma, este artigo objetivou por meio de uma reflexão teórica compreender as ocupações sob os pressupostos da Ciência Ocupacional e sua relação com a Terapia Ocupacional. Trata-se de um estudo de revisão narrativa como aporte metodológico. Os resultados apontam que uma das linhas pioneiras de investigação desenvolvidas no contexto da à Ciência Ocupacional que focaliza a forma, a função e o significado da ocupação. A Ciência Ocupacional trata de questões universais e singulares sobre a ocupação, com possibilidades de aplicação à Terapia Ocupacional. Assim, esta disciplina pode ser uma fonte rica de evidências para esta profissão.Abstract The occupation is composed of purposeful and meaningful actions in which people engage in daily live. These add personal e cultural meanings that are influenced by the life history of each one. Thus, this article aimed through a theoretical understanding the occupations under the aegis of assumptions of Occupational Science and its relation to occupational therapy. It is a study of narrative revision as method. The results show that a line of research of the Occupational Science that focuses on form, function and meaning of the occupation. Occupational Science issues universal and unique about occupation, with possibilities of application to Occupational Therapy. Resumen La ocupación se compone de acciones con propósito y significado, en el que las personas se dedican a la vida cotidiana y estructurar sus vidas. Estas acciones se suman los significados personales y culturales y son influenciados por la historia de vida de cada individuo. Por lo tanto, este artículo dirigido a través de una reflexión teórica entender las ocupaciones, bajo los auspicios de los supuestos de Ciencia de la Ocupación y su relación con la terapia ocupacional. Se trata de un estudio de revisión narrativa como método. Los resultados apuntan que una de las líneas de investigación desarrolladas en la Ciencia Ocupacional es la que enfoca la forma, la función y el significado de la ocupación. La Ciencia Ocupacional trata de cuestiones universales y singulares sobre la ocupación, con posibilidades de aplicación a la Terapia Ocupacional.
