Анотація:

Traditional staffing models rely on the productivity metric of hours per patient day, lacking the ability to adequately capture the nursing workload. Acuity-based staffing considers the patient population's acuity for appropriate nursing workload. Using process improvement methodology, a pediatric ICU transitioned to an acuity-based staffing model resulting in an 11.3% (P < 0.05) reduction in the acuity per nursing assignment and a decrease in reportable safety events by 61.3% (P < 0.05).

ABSTRACTObjectives: to analyze the connections of professional practices with the nursing and biomedical models and identify the factors involved in the professional dynamics. Method: this is a descriptive study made with nurses of a hospital and nurses of the Health Center that collaborate in the formation of the nursing students, with a sample of 227 nurses who had answered a structuralized questionnaire. Results: the data disclose that 69,9% of the nurses reported that they used, like professional practice, 27,1% stated that they don’t have a model as reference the nursing model as a reference, and only 3,4% which say they are guided by the biomedical model. 80.5% assume the need of a reference model in the professional practice. The factors that more influence practical as the models of nursing are, the complete development of the process of cares, the formation and the relation using nurse/sick person/equipe and according to biomedical model, the norms of the organization and the medical diagnosis. Conclusion: the hospital nurses see the nursing, essentially, as reparative function, whose main target is the disease and the only purpose is to heal. Nurses from the health center are more distant from these functions and more linked to issues related to health promotion. Descriptors: nursing; nursing care; theoretical models; nursing models.RESUMOObjetivos: analisar as ligações de práticas profissionais a modelos de enfermagem e ao modelo biomédico e identificar fatores que intervêm nas dinâmicas profissionais. Método: trata-se de um estudo descrito realizado com os enfermeiros de um hospital e centros de saúde que colaboram na formação dos estudantes de enfermagem, com uma amostra de 227 enfermeiros que responderam um questionário estruturado. Resultados: os dados revelam que 69,6% dos enfermeiros inquiridos referiram que na sua prática profissional utilizavam um modelo de enfermagem como referência, 27,1% não tem qualquer modelo como referência e apenas 3,4 % afirmam que se guiam pelo modelo biomédico. 80,5% consideram ser necessário um modelo de referência na prática profissional. Os fatores que mais influenciam as práticas segundo modelos de enfermagem são, o desenvolvimento completo do processo de cuidados, a formação e a relação enfermeiro/usuário/equipe e, segundo o modelo biomédico, as normas da organização e o diagnóstico médico. Conclusão: os enfermeiros do hospital perspectivam a enfermagem na função reparadora, cujo alvo principal é a doença e a finalidade é curar e os dos centros de saúde estão mais ligados a aspectos relacionados com a promoção da saúde. RESUMENObjetivos: analizar las conexiones de prácticas profesionales a los modelos de enfermería y al modelo biomédico, y identificar los factores que intervienen en las dinámicas profesionales. Método: esto es un estudio descriptivo hecho con las enfermeras de un hospital y las enfermeras del centro de salud que colaboran en la formación de los estudiantes de enfermería, con una muestra de 227 enfermeras que respondieron a un cuestionario estructurado. Resultados: los datos revelan que el 69,6% de las enfermeras contestaran que utilizan como referencia un modelo de enfermería en su prática profesional, 27,1% declaró que no tiene un modelo de referencia y sólo el 3,4% dicen utilizar como referencia el modelo biomédico. 80,5% siente la necesidad de tener un modelo de referencia en su práctica profesional. Los fatores más que influyen las práticas de acuerdo alos modelos de enfermería son, todo el proceso de atención, la formación y la relación enfermera/usuario/equipe y de acuerdo con el modelo biomédico, las normas de la organización y el diagnóstico médico. Conclusión: eos enfermeros del Hospital, consideren la enfermería esencialmente como una función reparadora, cuyo objetivo principal es la enfermedad y la finalidad es curar e los enfermeros del centro de salud están más alejados de estas funciones y más vinculados a las cuestiones relacionadas con la promoción de la salud. This is the first of two essays about five models of nursing practice delivery—total patient care, functional nursing, team nursing, primary nursing, and the attending nurse. Total patient care, functional nursing, and team nursing are discussed in this essay. The other two models will be discussed in another essay published in a future issue of Nursing Science Quarterly. Each model is described, and its connection with nursing discipline-specific knowledge is discussed. The extent to which each model ascribes accountability for practice also is discussed.
