Повний текст джерелаNudges are subtle modifications to the decision-making environment designed to influence behaviour without restricting individual choice. Used in various fields, including primary care and vaccination, these behavioural interventions rely on cognitive biases to guide decisions, without resorting to direct persuasion or sanctions. The scientific and applied enthusiasm for these gentle incentives is based primarily their effectiveness and social acceptability. However, these two major benefits are being called into question. In this context, ethical issues are being raised, particularly in relation to the potentially manipulative nature of nudges and the threat they may pose to people's autonomy. The aim of this thesis is therefore to examine the relationship between perceived autonomy, the social acceptability of nudges and the effectiveness of nudges in primary care. The aim is to assess whether the effectiveness of a nudge depends on its social acceptability and whether this social acceptability is based on the perceived autonomy of the nudged individuals. The overall model predicts that the way in which the nudge is presented and the fact of having been exposed to it beforehand determine the perceived autonomy when faced with the nudge. This autonomy will then be a determining factor in the social acceptability of the nudge, in the same way or even more so than the type of nudge and its operationalization. This model was tested in several stages: 1. testing the impact of exposure to a nudge, the type of nudge, its transparency and the way in which the nudge is presented on each of the variables in the triptych perceived autonomy - social acceptability - effectiveness of the nudge, 2. testing the model according to which the social acceptability of the nudge acts as a mediator of the positive effect of perceived autonomy on the effectiveness of the nudge. Based on a literature review and eight experimental studies, these concepts are analyzed in the context of primary care. The results show that prior exposure to a nudge and the way in which the nudge is presented as respecting or threatening autonomy may be decisive for its social acceptability, which is correlated with the adoption of the target behaviour. However, the results do not allow us to conclude that there is a causal relationship in the triptych of perceived autonomy - social acceptability - effectiveness. This set of studies highlights both the practical and ethical issues surrounding nudges in the field of public health. It also points the way to interventions that comply with WHO recommendations for primary care

Повний текст джерелаIt's ethically unthinkable to study morphological and functional developments of the human stomach in utero. In contrast, the method developed by Pr DUPREZ and Dr ANGIOI, allows such studies. Human foetal stomachs 6-10 week-old from requested legal abortion were microsurgically engrafted in recipient nude and SCID mice. All grafts grew normally, and their size never exceed 3 x 4 x 4 cm whatever the graft delay. Peptic type gastric fistulae uses to develop after 90 graft-days. The development of stomach grafts was studied by using histological, histophysiological, ultrastructural and biochimieal methods and showed that by 60 graft-days glandular and muscular layers were mature as an adult type gastric mucosa. Host cells in stomach xenografts were restricted to some host fibromyoblasts in the lamina propria and to vascular endothelial cells which augmented with the graft duration. ,Parietal cells (H+ /K+ -A TPase and intrinsic factor positives), chief cells (pepsinogen positive), ECL, D and G cells were all fully differentiated. Stomach grafts secreted an acidic gastric juice, containing mucous, Na+, K+ and crions, intrinsic factor, pepsins and gastrin. Acid secretion was increased by pentagastrin and lowered by omeprazole, with modifications of subcellular structures of parietal cells, showed by transmission electron microscopy analysis. Transplants could be infected pennanently by Helicobacter pylori during the three-months of observation. All this results demonstrate that in vivo development of the human foetal stomach xenografts was normal but early than in utero. The acceleration of stomach differentiation but without changing the chronology is probably induced by host epigenetic factors

Повний текст джерелаThis doctoral thesis is an interdisciplinary study on the question of the representation of nudity in French literature, medicine and artistic discourse in the second half of the eighteenth century. Considering that the naked body is inseparable from the sense of sight, we reveal the idea that the elaboration of a gaze determine a plurality of representations of the undressed body. By the study of texts of different types, this thesis presents a process demonstrating the link between the looking and the nudities in the Enlightenment. From technical words revealing the anatomical detail to descriptions in veiled terms, the writing and the reading of the undressed body are such as a textual and visual interaction (Chapter I). The second chapter is devoted to the effects attributed to the nude in the eighteenth century and presents, through the study of anatomical plates, how the gaze on the bare body can be constructed. The following chapters examine successively the nudities according to different ways of seeing. Voyeurism (Chapter III) and observation (Chapter IV) promote the representation of anatomical detail, the creation of topical figures and the staging of the naked body. The following chapters show that the body can also be mentally undressed. Clothes and draperies, proportions and colors guide a trajectory allowing to see, to touch, to feel and to imagine the nudity (Chapter V). From “costume” to skin, from muscles to bones, the body appears as a set of superimposed layers offering the possibility of a penetrating eye. Clothes and integument once removed, nakedness becomes the image of a physical and moral interiority explored by doctors and writers (Chapter VI). The seventh and final chapter shows that nudity is a way of thinking through decentered or focused gazes. The naked body reflects to the dressed man of the Enlightenment his own image. The fashions, the exhibition of "savages" and the auscultation are also situations in which the physician's figure is drawn in relation to the naked female body.The representations of the undressed body in the eighteenth century are created between sight and touch, between the whole body and its parts, between the clothed and the naked, between the surface and the depth. They proceed from the fragmentation, the magnification, the stripping and the reification of the body and its parts. They involve textual processes, generic and disciplinary contaminations, creations of figures such as bathers, athletes, men of letters, Venus of all kinds, allegories and anatomies. Based on the construction of the gaze and the interactions between arts, literature and medicine, this thesis invites us to discover the nudities of the Enlightenment
