There is growing concern that suffering multiple sport-related concussions may increase an athlete’s risk of cumulative neurocognitive and neurobehavioral impairment. Many concerns have not been well-validated, however, owing to limited samples of repeatedly concussed players. In this article, we review the theoretical risks and current evidence regarding the extent to which repeat concussions impact players’ experience of and recovery following successive injuries. Concussion effects are considered at multiple levels (e.g., self-reported physical and psychiatric symptoms, neuropsychological performance, and neurophysiological measures) across both the acute and chronic phases of recovery. Recommendations for applying findings to injury management decisions are provided. Although repeat concussions appear to have the potential for cumulative neurophysiological burden, a number of factors (e.g., individual risk for experiencing or responding poorly to injury, recovery time between injuries) appear important to explain discrepant findings among studies and to translate general scientific principles into clinical decisions for individual players. Future work that accumulates larger, prospective samples will allow for clearer delineation of the factors that appear important for predicting how recurrent concussions impact individual athletes.

This chapter focuses on the punishment of offenders sentenced for multiple offenses that have not been separated by independent prosecution and sentencing. Most scholars believe that the striking discrepancy between sentences imposed on multiple and repeat offenders cannot be justified in terms of retributivism. The existing solutions to the overpunishment of offenders convicted of multiple crimes invoke concurrent sentencing or the exercise of mercy by a sentencing court. Both approaches mitigate excessive punishment, but also create a number of problems. This chapter first considers the nature of the problem and the deficiencies of current approaches to multiple-offense sentencing before explaining how much of the gap between the repeat and the multiple offenders’ sentences can be accommodated within a retributive framework. It also describes culpability and harm as independent elements of deservedness and argues in favor of reduced sentences when the offenses are related to one another.

A member of the expanded polyglutamine (polyQ) repeat family of neurodegenerative disorders, Huntington disease (HD) is a rare, autosomal, dominantly inherited neuropsychiatric disorder. Characterized by midlife onset, HD exhibits progressive motor, behavioral, and cognitive changes. There is no effective treatment and death usually ensues 15 to 20 years after diagnosis. The expanded polyglutamine repeat causes multiple cellular dysfunctions to induce neurodegeneration. Many brain regions are affected in HD though striatal degeneration is particularly prominent. Widespread availability of specific genetic testing facilitates diagnosis. Management is largely supportive care.

Myotonic dystrophy type 1 affects multiple organ systems and is associated with cardiac, endocrine, and ophthalmological disorders. Executive dysfunction can lead to missed appointments and an apathetic attitude, causing patients to underestimate symptoms. The findings of both clinical and electrical myotonia should suggest myotonia congenita, DM1, or DM2. Myotonic dystrophy demonstrates anticipation, where the disease has an earlier onset and more severe course with subsequent generations due to increasing trinucleotide repeats. Therefore, the diagnosis can be heralded in an adult by his/her affected child. Management of the disorder is supportive, addressing issues relating to loss of strength, cardiac conduction defects, endocrinopathies (regular monitoring with glycosylated hemoglobin and thyroid-stimulating hormone). Diagnosis and management of this disorder is discussed.

This article investigates multi-vortex states in mesoscopic amorphous superconductors with different geometries by means of scanning SQUID microscopy. It first describes the setup of the scanning SQUID microscope used in magnetic imaging of superconducting vortices before discussing the physical properties of amorphous superconducting thin films. It then presents the results of experiments showing the formation of multi-vortex states in mesoscopic dots of weak pinning, amorphous MoGe thin films, along with the formation of vortex polygons and concentric vortex rings in mesoscopic disks. It also considers the concept of multiple vortex shells and its applicability to vortex patterns observed in mesoscopic circle and square dots. The article highlights some of the key features of mesoscopic superconducting dots, including commensurability effect, multiple shell structures, repeated packing sequences, inclusion structural hierarchy, and pinning effect.

Routine radiographs are not cost effective in the intensive care unit (ICU) setting. Most published guidelines agree that radiographs are worthwhile after insertion of tubes or catheters, and in patients receiving mechanical ventilation. Otherwise, they are required only for change in the patient’s clinical status. Picture archiving and communication systems utilize digital imaging technology. They provide superior quality images, rapid image availability at multiple sites, and fewer repeat examinations, reducing both cost and patient radiation. Disadvantages of picture archiving and communication systems include expensive equipment and personnel required to keep them functioning. The majority of chest X-ray abnormalities in the ICU are best understood by paying careful attention to the initial appearance of the X-ray in relation to the patient’s onset of symptoms and the progression of abnormalities over the next few days.

Each chapter emphasizes interaction with specific biblical texts, interpreting them in the context of ancient Israel’s national storyline, and encourages readers to approach them dialogically. Multiple psychosocial treatments for substance-use disorders have been studied for efficacy. A recent meta-analysis indicates that psychosocial interventions are effective across multiple types of substances used. In the case of opiates, psychosocial interventions combined with medication appear to be the most effective. Many studies further agree that psychosocial interventions are an integral and necessary part of treating substance-use disorders. Although theoretical orientations may differ across psychosocial treatments, they have several principles and practices in common. All involve talk therapy or talk in communities as a way to clarify triggers, build commitment, and improve accountability. Many also target addiction behaviors and work to develop alternative contingencies to reduce or eliminate use. Finally, targeting repeated performance (or building “chains of committed behavior”) decreases the likelihood of relapse. This chapter discusses the most frequently studied and employed psychosocial treatments for substance use including CBT, motivational interviewing, contingency management, mindfulness, and community-based programs.

This chapter reviews the basic theory of reliability, and examines the relation between reliability and measurement error. It derives the standard form of reliability, the intraclass correlation or ICC, from repeated measures ANOVA. The chapter explores issues in the application of the reliability coefficient, including absolute versus relative reliability, the reliability of multiple observations, and the standard error of measurement. It examines several other measures of reliability—Cohen’s kappa, Pearson r, and the method of Altman and Bland—and derives the relation between them and the ICC. The chapter determines the variance of a reliability estimate. It also calculates sample size estimates for reliability studies, and methods to combine reliability estimates in systematic reviews.

Olivier Berthod, Michael Grothe-Hammer, and Jörg Sydow report an unconventional research design using multi-site ethnography. The aim is to study inter-organizational relationships, which are not well understood, and are not addressed by single-site ethnographic methods. Ethnography is a popular and established methodology in organization studies. However, organizing is a process that crosses boundaries, and the traditional approach that involves immersing the ethnographer in one defined social or organizational setting means that inter-organizational phenomena are overlooked. The challenge is to conduct fieldwork at multiple sites, across which inter-organizational relations may be conducted with varying degrees of formality, and be more or less visible. Inter-organizational ethnography thus builds on the combination of several techniques. Four techniques are explored: following boundary objects, capturing network enactments, using several investigators, and repeat interviews. The methodology is illustrated with a study of the network of eighty organizations that deal with large-scale crises and emergencies in Düsseldorf.

The psychoanalytic psychotherapies, which include brief psychodynamic psychotherapy, psychoanalysis, long-term psychoanalytic psychotherapy, transference focused psychotherapy, mentalization based treatment, and panic focused psychodynamic psychotherapy, are based on the underlying theory that symptoms stem from unconscious traumatic memories or conflicts about sexual and aggressive wishes as well as maladaptive or self-destructive behavior patterns that are unconsciously repeated. The cognitive-behavioral psychotherapies, which include cognitive-behavioral therapy and dialectical behavior therapy, are based on the assumption that symptoms arise from maladaptive patterns of cognition and behavior that are learned via behavioral conditioning. Interpersonal psychotherapy, family therapy, and group therapy can be regarded as multiple-person therapies that view symptoms as arising from problems in relations between and among people. Crisis intervention and other supportive psychotherapies provide patients with advice and education to enhance coping skills and ego functions.

The lung is a relatively new site for the application of ultrasound. Beyond the more established assessment of pleural effusion, this organ has been traditionally considered off limits for sonographic investigation, since air is a well-known foe of the ultrasound beam. However, in recent years it has been shown that this apparent physical limitation can be overcome when the air content decreases, as happens in a diseased pulmonary parenchyma. The most useful lung ultrasound sign for cardiologists is the presence of B-lines, the sonographic hallmark of pulmonary interstitial syndrome, including interstitial pulmonary oedema. Bilateral multiple B-lines are present in patients with pulmonary congestion and may help assess and semiquantify the extent of extravascular lung water in patients with heart failure. This sign is low cost, easy to perform, can be repeated at bedside, and does not employ ionizing radiation. Lung ultrasound is also useful for detecting other pulmonary conditions such as pneumothorax, and lung consolidations such as pneumonia or pulmonary infarction.

Gaining insight into the mechanisms by which neural transmission governs behavior remains a central goal of behavioral neuroscience. Multiple applications exist for monitoring neurotransmission during behavior, including fast-scan cyclic voltammetry (FSCV). This technique is an electrochemical detection method that can be used to monitor subsecond changes in concentrations of electroactive molecules such as neurotransmitters. In this technique, a triangular waveform voltage is applied to a carbon fiber electrode implanted into a selected brain region. During each waveform application, specific molecules in the vicinity of the electrode will undergo electrolysis and produce a current, which can be detected by the electrode. In order to monitor subsecond changes in neurotransmitter release, waveform application is repeated every 100 ms, yielding a 10 Hz sampling rate. This chapter describes the fundamental principles behind FSCV and the basic instrumentation required, using as an example system the detection of in vivo phasic dopamine changes in freely-moving animals over the course of long-term experiments. We explain step-by-step, how to construct and surgically implant a carbon fiber electrode that can readily detect phasic neurotransmitter fluctuations and that remains sensitive over multiple recordings across months. Also included are the basic steps for recording FSCV during behavioral experiments and how to process voltammetric data in which signaling is time-locked to behavioral events of interest. Together, information in this chapter provides a foundation of FSCV theory and practice that can be applied to the assembly of an FSCV system and execution of in vivo experiments.

Epilepsy is a common disease of the brain, occurring in roughly 1% of all people, and although repeated epileptic seizures are its clinical hallmark, epilepsy is not just a medical phenomenon, but a social construct, with cultural, political, and financial consequences. People with epilepsy are exposed to stigma and burdened with disadvantages which can be far reaching. There are indeed many remedies, but no cure. This book provides a biography of modern epilepsy in the form of a brief and selective narrative of some of the important developments in medical and social epilepsy research, with its many ups and downs, over the period since 1860. Its anatomy of modern epilepsy in eight chapters is, inevitably in this short book, selective, and intentionally provocative. The book’s main objective is to provide both a survey of the evolution of epilepsy and its treatment in the post-Jacksonian era, and also a critical look at where we are today and how we got there. This book tries to make an effort to separate the wheat from the chaff in the development of better epilepsy care. Good and bad events and concepts of historic consequence are discussed. It is acknowledged that, although the end of epilepsy is in reach of some, there is at present no prescribed scientific path to the end of epilepsy for others. Regardless of the severity of epilepsy, patients, with the support of their physicians and modern medicine, must create their own solutions to the multiple issues they face.

Based on hundreds of interviews with party leaders, activists and voters and three decades of election results across Europe, The New Party Challenge introduces new tools for mapping and measuring party systems and develops an integrated conceptual framework for analysing the dynamics of party politics, particularly the birth and death of parties. The book charts and explains the patterns of politics in Central Europe since 1989, and then shows how similar processes are at play on a far wider geographical canvas. The repeated breakthroughs of new parties poses multiple challenges: existing parties that must staunch the outflow of disillusioned voters to fresh alternatives, new parties must figure out how to hold on to those new voters in the face of even newer alternatives, and society as a whole must find a way to pursue long-term policies in a political environment where the roster of political actors is constantly changing.
