The optical interface between a light source and a detector is important. In most practical realizations the aim should be to collect the maximum light possible. Lens systems seldom do this efficiently, especially where the light source is diffuse. Underlying any attempt at concentrating or guiding light is subject to a fundamental limitation referred to as étendue (phase space cannot be squeezed). Light collection from small volume scintillators of high refractive index may approach 50 %, while collection from large-area scintillators is typically less than a few per cent. Incorporation of wavelength-shifting light guides and fibres leads to enhanced performance. Efficiency measurements by the author in terms of photoelectrons per keV are presented for selected configurations. Optical recycling derived from total internal reflection provides enhancement in effective quantum efficiency by a factor of up to 10. Concepts such as escape cones, adiabatic light guides, and trapped light are covered in detail.

Performance enhancement in sport has a long and controversial history. Although several organizations enacted prohibitions on the subject of doping prior to the Second World War, public scrutiny on the issue remained relatively light until the second half of the twentieth century. Beginning in the 1960s, officials passed a number of regulatory measures with the twin goals of protecting the health of athletes and ensuring the fairness of competitions. Due partially to the effects of Cold War political rivalries, the use of drugs by athletes nevertheless remained widespread in the world of sport. This policy situation changed dramatically with the end of the superpower conflict in 1991, however. The following decade was marked by increasingly vociferous calls for reform from outside the international governance structure for sport. In February of 1999, regulatory powers over the subject were centralized in a new organization called the World Anti-Doping Agency (WADA).

This article discusses a variety of laser applications in nanotechnology. The laser has proven to be one of many mature and reliable manufacturing tools, with applications in modern industries, from surface cleaning to thin-film deposition. Laser nanoengineering has several advantages over electron-beam and focused ion beam processing. For example, it is a low-cost, high-speed process in air, vacuum or chemical environments and also has the capability to fulfill flexible integration control. This article considers laser nanotechnology in the following areas: pulsed laser ablation for nanomaterials synthesis; laser nanoprocessing to make nanobumps for disk media nanotribology and anneal ultrashort PN junctions; surface nanopatterning with near-field, and light-enhancement effects; and large-area parallel laser nanopatterning by laser interference lithography and laser irradiation through a microlens array. Based on these applications, the article argues that the laser will continue to be one of the highly potential nanoengineering means in next-generation manufacturing.

Whereas the concepts and tools described in this book are intuitive, simple, and easy to understand, their implementation and the process of ongoing improvement are not trivial and require a great deal of management attention. The chapter describes the change-management process. It starts with a full business/functional diagnosis of the organization and goes through an implementation plan draft, training and knowledge transfer, and establishing value enhancement teams. Then, the organization should establish tools for monitoring and control and ensure sustainability through a process of ongoing improvement. The chapter also presents the 3–1–1 model for implementing changes in hospitals. As hospitals are highly complex organizations with multitudes of entities with many interactions among them, the 3–1–1 model guides managers where to focus their efforts. The process of ongoing improvement can use the “traffic lights” system for sustainability and continuous value creation.
