This study aims to study the effect of intradialysis exercise on patients undergoing hemodialysis. The research method used is a literature search. The research results were published in English and Indonesian journals from 2018 to 2022. The literature search used PICO references in several databases, such as ScienceDirect, ProQuest, Scopus, and PubMed. The results of the study showed that there was a significant effect of intradialysis exercise on improving physical, cognitive, and sleep quality and quality of life. They could help reduce depression levels in patients undergoing hemodialysis. In conclusion, a structured and adequately monitored intradialytic exercise program is the key to the success of an intradialytic exercise program.

People with kidney failure should have hemodialysis therapy to survive. The effectiveness of hemodialysis therapy can be measured by calculating the value of dialysis adequacy: Kt/V and URR values. Intradialytic exercise is an exercise that can improve of dialysis adequacy. The purpose of this review is to report the results of related research on the effect of intradialytic exercise on dialysis adequacy. Relevant articles were search in PubMed, Science Direct and Google Scholar using this keywords intradialytic exercise and dialysis adequacy. Results: 12 articles ware included in the review and found various intradialytic exercises, intradialytic training time and length of intradialytic training. Exercises that can improve the adequacy of dialysis are static bike exercises, airogym and aerobics. While exercise resistance decreases the value of dialysis adequacy. We also found that the initial adequacy varied. Discussion: mild exercise can increase dialysis adequacy, in the first two hours of dialysis and minimum exercise was conducted two month. Currently no literature that recommend type and length of intradialytic exercise. Conclusion: intradialytic exercise can increase the value of dialysis adequacy, although it has increased, the mean value of adequacy before exercise is above standard. To confirm these findings, further research is needed to conducted exercise ware the value of adequacy under standard values.

This study aims to describe the effectiveness of intradialytic exercises and HD temperature profiling in improving the quality of HD services. The literature search method is based on the compilation of PICO through online database searches on Clinicalkey, Clinicalkey Nursing, ScienceDirect, EBSCO, ProQuest, and Scopus. Journal search results obtained 20,106 articles; after the selection stage, nine reports were obtained for analysis, consisting of 5 RCT articles, one systematic review article, two quasi-experimental articles, and one meta-analysis article. The results showed that HD patients who were routinely given intradialytic exercises would show an increase in HD adequacy achievements, suppress inflammatory reactions, increase albumin and hemoglobin values, reduce the erythropoietin resistance index, increase phosphate clearance, improve the quality of life of HD patients and minimize the risk of both acute and chronic complications. Chronic disease caused by HD. Meanwhile, temperature profiling can reduce the incidence of intradialytic hypotension, pruritus, and fatigue and reduce the severity of Restless Leg Syndrome (RLS) without interfering with the achievement of HD adequacy. This study concludes that intradialytic training and temperature profiling effectively improve the quality of HD services.

Hemodialysis therapy that is carried out for a long time can cause physical stress in the form of fatigue and have an impact on the patient’s quality of life. Giving intradialytic exercise intervention is believed to reduce the level of fatigue. The purpose of this study was to determine the effect of intradialytic exercise on the level of fatigue in chronic kidney failure patients undergoing hemodialysis. This study used a systematic literature review method based on data from Google Schoolar, Garuda, Science Direct, and the Directory of Open Access Journal (DOAJ), by including key words are fatigue, chronic kidney failure, hemodialysisi, and intradialytic exercise. Article were selected through screening according to inclusion criteria, including publications within the last 10 years (2011 – 2022), original articles, Indonesian and English, descriptive analytic study types, and quasi experiments. The results show that Intradialytic exercises applied include breathing exercise, Range of Motion (ROM), slow deep breathing, Progressive Muscle Relaxation (PMR), Pursed Lip Breathing and Physical Exercise. Based on the 9 articles found that intradialytic exercise has an effect on the level of fatigue. The average p-value <0.05, the average post-test fatigue score is smaller than the pre=test, the smaller the average value fatigue score then the level of fatigue decreases. In conclusion, Intradialytic exercise affects the fatigue level of chronic kidney failure patients undergoing hemodialysis. Suggestions are given to HD nurses to be able to implement an intradialytic exercise intervention in patients undergoing hemodialysis to reduce fatigue.

Managemen fatigue sangat penting untuk meningkatkan hasil klinis dan kualitas hidup pasien yang menjalani hemodialisis. Intradialytic exercise merupakan latihan yang dilakukan pada saat menjalani hemodialisis. Intradialytic exercise penting untuk mempertahankan dan meningkatkan kesehatan secara keseluruhan. Penelitian ini bertujuan untuk mengetahui pengaruh intradialytic exercise selama hemodialisis terhadap fatigue. Penelitian ini menggunakan metode quasi eksperiment dengan pendekatan pre-post test design with control. Penelitian dilakukan di RSUP Dr.Soeradji Tirtonegoro Klaten. Sampel diambil dengan purposive sampling berjumlah 32 responden yang memenuhi kriteria inklusi. Responden terbagi menjadi kelompok intervensi dan kelompok kontrol. Kelompok intervensi mendapatkan intradialytic exercise. Pengambilan data dilakukan setelah lolos uji etik. Hasil penelitian menunjukkan ada pengaruh intradialytic exercise terhadap fatigue pada kelompok intervensi p value 0,000. Kesimpulan penelitian ini intradialytic exercise menurunkan fatigue pada pasien hemodialisis.Kata kunci : fatigue, intradialytic exercise, hemodialisis

This study is the first, to our knowledge, to directly compare cardiopulmonary and metabolic physiology during hemodialysis, intradialytic exercise, and interdialytic exercise. Hemodialysis was associated with increased respiratory exchange ratio, blunted minute ventilation, and impaired O2 uptake and extraction. We also identified a reduced ventilatory response during intradialytic exercise compared with interdialytic exercise. Impaired arterial-venous O2 difference during hemodialysis was partly restored by intradialytic exercise. Despite dysregulated cardiopulmonary and metabolic physiology during hemodialysis, intradialytic exercise was well tolerated.

Background: Physical inactivity is prevalent and linked with a variety of unfavorable clinical outcomes in hemodialysis patients. To increase physical activity (PA) and improve quality of life in this population, intradialytic and out-of-clinic exercise interventions have been implemented in many studies. However, there is still a lack of consensus in the literature on which type of exercise intervention is more feasible and effective. Summary: This review provides a brief overview of intradialytic and out-of-clinic exercise protocols utilized in previous studies. We also examine data related to the feasibility of each approach, and their efficacy for improving cardiovascular health, muscle mass, strength, and physical function. Key Messages: The benefits from most intradialytic and out-of-center exercise training interventions published to date have been modest or inconsistent. Furthermore, neither appears to provide a significant advantage over the other in terms of benefits for cardiovascular health, muscle mass, strength, and physical function. A significant concern is that most intradialytic and out-of-center exercise interventions are mandated exercise prescriptions that include either endurance or resistance training exercises, performed at low-moderate intensities, for a total of 60–135 min of exercise/week. This volume, intensity, and variety of exercise are far less than what is recommended in most PA guidelines. This type of structured activity is also boring for most patients. To enhance the effectiveness of exercise interventions, we suggest using the intradialytic period to provide patients guidance on how they can best incorporate more activity into their lives, based on their individual needs and barriers.

The low quality of life of hemodialysis (HD) patients at the Riau Province referral hospital, Arifin Achmad Hospital, is due to dialysis adequacy and low activity levels of hemodialysis (HD) patients. Intradialytic exercise has been shown to improve dialysis adequacy and, as a result, patient quality of life. The aim of this community service is to improve HD nurses' intradialytic exercise knowledge and skills. Training HD nurses on intradialytic exercise is the method of performing community service activities, with implementation approaches including sharing material and skill simulation. The result is that HD nurses have more knowledge and abilities concerning intradialytic exercise, with an average score of 94.3 percent on the post-test. Furthermore, nurses are able to share intradialytic exercise knowledge and skills with HD patients, therefore it is believed that HD patients will be able to use intradialytic exercise to improve their quality of life.

Background: Intradialytic exercise is an effective intervention to reduce morbidity and mortality and increase quality of life among patients with chronic kidney disease undergoing dialysis. However, implementing and sustaining it in clinical practice has proved challenging. To identify how to best design an effective and sustainable intervention in clinical practice, we aimed to explore hemodialysis patients’ and nurses’ attitudes towards intradialytic exercise, including their motivation, anticipated barriers, and suggestions for the design of a proposed exercise program. Methods: Data were collected through qualitative semistructured interviews with patients and focus group interviews with nurses and analyzed inductively with content analysis. Results: Overall, patients’ and nurses’ attitudes towards intradialytic exercise were positive. Patients were motivated by their expectations about perceived benefits, such as improved quality of life and reduced musculoskeletal pain. Their main concern was triggering dialysis machine alarms and disturbing nurses. Nurses were more skeptical of intradialytic exercise and concerned about patient safety. Patients and nurses had several ideas on how to design a safe and motivating intradialytic exercise intervention. Conclusion: The analysis of patients’ and nurses’ experiences and attitudes generated recommendations for an intradialytic exercise program. Recommendations include individually tailored programs that are safe and that patients can do independently, continuous collaboration between patients, nurses, physicians, and physiotherapists, and educating nurses about the benefits and safety of intradialytic exercise.

Physical activity levels are typically undesirably low in chronic kidney disease patients, especially in those undergoing haemodialysis, and particularly on dialysis days. Intradialytic exercise programmes could be a solution to this issue and have been reported to be safe and relatively easily implemented in dialysis clinics. Nevertheless, such implementation has been failing in part due to barriers such as the lack of funding, qualified personnel, equipment, and patient motivation. Intradialytic aerobic exercise has been the most used type of intervention in dialysis clinics. However, resistance exercise may be superior in eliciting potential benefits on indicators of muscle strength and mass. Yet, few intradialytic exercise programmes have focused on this type of intervention, and the ones which have report inconsistent benefits, diverging on prescribed exercise intensity, absent or subjective load progression, equipment availability, or exercise supervision. Commonly, intradialytic resistance exercise interventions use free weights, ankle cuffs, or elastic bands which hinder load progression and exercise intensity monitoring. Here, we introduce a recently developed intradialytic resistance exercise device and propose an accompanying innovative resistance exercise training protocol which aims to improve the quality of resistance exercise interventions within dialysis treatment sessions.

ABSTRACT
Background: Hemodialysis defines as a process of cleaning the blood from waste substances through a filtering process outside the body. Patients with chronic renal failure undergoing hemodialysis often experience complications including hypotension. Several references state an alternative intervention to prevent complications of hypotension in hemodialysis patients with an Intradialytic exercise. Intradialytic exercise is a planned and gradual form of exercise that includes various stages of flexibility exercise, strengthening exercise and cardiovascular exercise performed during hemodialysis. Intradialytic exercise is aimed to improve the work of the heart, respiration and improve hemodialysis regulation for the better.
Objective: This study was aimed to determine the difference in blood pressure before and after intradialytic exercise in patients undergoing hemodialysis.
Methods: This research design used a quasi-experimental design with pre-test and post-test with control design. The samples were 38 respondents who taken by total sampling at the Hemodialysis Unit of Panti Rahayu Hospital. The samples were divided into the intervention group who undertook intradialytic exercise for 4 weeks, each respondent experiences twice a week, while the control group who did routine hemodialysis and independent exercise.
Results: The results showed that the distribution of the characteristics of the respondents was 51-54 years old (18.41%) the sex was mostly male (63.2%) Most of them (55.3%) underwent hemodialysis for less than 2 years. Statistically, it was known that there was no significant difference in blood pressure before and after intradialytic exercise inside patient’s body of the control and intervention groups. There was no significant difference in blood pressure between the intervention group compared to the control group, but there was a dynamic difference in blood pressure in patients who did intradialytic exercise.
Conclusion: There was dynamics of differences in blood pressure in patients undergoing intradialytic exercise, it is recommended that hemodialysis nurses at Panti Rahayu Hospital take care patients during hemodialysis so that these interventions are routinely carried out.

Regular exercise can reduce depression. However, the uptake of exercise is limited in patients with end-stage renal disease undergoing hemodialysis. To address the gap, we designed a gamified non-weight-bearing intradialytic exercise program (exergame). The intradialytic exergame is virtually supervised based on its interactive feedback via wearable sensors attached on lower extremities. We examined the effectiveness of this program to reduce depression symptoms compared to nurse-supervised intradialytic exercise in 73 hemodialysis patients (age = 64.5 ± 8.7years, BMI = 31.6 ± 7.6kg/m2). Participants were randomized into an exergame group (EG) or a supervised exercise group (SG). Both groups received similar exercise tasks for 4 weeks, with three 30 min sessions per week, during hemodialysis treatment. Depression symptoms were assessed at baseline and the fourth week using the Center for Epidemiologic Studies Depression Scale. Both groups showed a significant reduction in depression score (37%, p < 0.001, Cohen’s effect size d = 0.69 in EG vs. 41%, p < 0.001, d = 0.65 in SG) with no between-group difference for the observed effect (p > 0.050). The EG expressed a positive intradialytic exercise experience including fun, safety, and helpfulness of sensor feedback. Together, results suggested that the virtually supervised low-intensity intradialytic exergame is feasible during routine hemodialysis treatment. It also appears to be as effective as nurse-supervised intradialytic exercise to reduce depression symptoms, while reducing the burden of administrating exercise on dialysis clinics.

This study aims to systematically review several works of literature that discuss the impact of training or physical activity of Chronic Kidney Disease (CKD) patients undergoing hemodialysis on all factors that can improve the quality of life of CKD patients. The method used is a systematic review by browsing the literature on indexed electronic databases (Scopus, ScienceDirect, CINAHL, Pubmed, Oxford and SAGE). The results showed that the selection of appropriate and safe intradialytic exercises for patients was able to increase physical activity and sleep quality, leg muscle mass, stimulate bone alkaline phosphatase, which can prevent bone loss, lower blood pressure, improve respiratory function and functional capacity, improve physical function, reduce inflammation, improve nutrition, increase bone mineral density, improve exercise tolerance, increase Kt/V and increase the excretion of serum creatinine, blood urea and serum potassium phosphorus. In conclusion, intradialytic exercise during HD will improve the quality of life, decreasing morbidity and mortality of CKD patients undergoing HD.

Abstract
BACKGROUND: Chronic renal failure (CRF) is a reduction in chronic renal function that leads to non-reversible and progressive kidney tissue damage. Blood pressure is a major risk factor that can increase the mortality rate by up to 20 times in patients with chronic renal failure who are undergoing hemodialysis therapy. Intradialytic exercise is an intervention that can be used to control systolic blood pressure.
AIM: This study was to analyze the effect of intradialytic exercise on changes in blood pressure in chronic renal failure patients undergoing hemodialysis therapy at Indriati Solo Baru Hospital.
METHODS: The research method is quasi-experimental design pre-post with control group design. The study population was 97 dialysis patients. The sampling technique used a purposive sampling of 30 people. Data analysis using Paired t-Test.
RESULTS: The research results showed that the sex characteristics of most respondents were male as many as 17 people (56.7%), the average age of the respondents was 51.03 years, the pre-intervention in the control group obtained an average systolic blood pressure of 168.93 mmHg and 106 diastole, 120 mmHg, while the blood pressure in the control group post was 173.13 mmHg systole and 107 mmHg diastoles. In the treatment group, the average blood pressure in the pre-intradialytic exercise showed 162.20 mmHg systole and 104.27 mmHg diastole, while the post-intradialytic exercise system blood pressure was 153.13 mmHg and diastole 94.33 mmHg.
CONCLUSION: There is an effect of intradialytic exercise on changes in blood pressure in chronic renal failure patients undergoing hemodialysis therapy at Indriati Solo Baru Hospital with a p-value of 0.025. ObjectiveTo assess the efficacy and safety of intradialytic exercise for haemodialysis patients.DesignSystematic review and meta-analysis.Data sourcesDatabases, including PubMed, Embase, the Cochrane Library, China Biology Medicine and China National Knowledge Infrastructure, were screened from inception to March 2017.Eligibility criteriaRandomised controlled trials (RCTs) aimed at comparing the efficacy and safety of intradialytic exercise versus no exercise in adult patients on haemodialysis for at least 3 months. A minimum exercise programme period of 8 weeks.Data extractionStudy characteristics and study quality domains were reviewed. Studies were selected, and data extracted by two reviewers.Data analysisThe pooled risk ratios and mean differences (MDs) with 95% CIs for dichotomous data and continuous data were calculated, respectively.ResultsA total of 27 RCTs involving 1215 subjects were analysed. Compared with no exercise, intradialytic exercise increased dialysis adequacy (Kt/V) (MD 0.07, 95% CI 0.01 to 0.12, p=0.02) and maximum volume of oxygen that the body can use during physical exertion peak oxygen consumption (MD 4.11, 95% CI 2.94 to 5.27, p<0.0001), alleviated depression standardised mean difference (−1.16, 95% CI −1.86 to –0.45, p=0.001) and improved physical component summary-short form-36 (SF-36) level (MD 7.72, 95% CI 1.93 to 13.51, p=0.009). Also, intradialytic exercise could significantly reduce systolic blood pressure (MD −4.87, 95% CI −9.20 to –0.55, p=0.03) as well as diastolic blood pressure (MD −4.11, 95% CI −6.50 to –1.72, p=0.0007). However, intradialytic exercise could not improve mental component summary-SF-36 level (MD 3.05, 95% CI −1.47 to 7.57, p=0.19). There was no difference in the incidence of adverse events between the intradialytic exercise and control groups.ConclusionsIntradialytic exercise resulted in benefits in terms of improving haemodialysis adequacy, exercise capacity, depression and quality of life for haemodialysis.

Patients with end-stage kidney disease (ESRD) undergoing hemodialysis (HD) tend to have multiple comorbidities, especially cardiovascular disease, mineral and bone disorders which are important factors and contribute to the morbidity and mortality of HD patients. So it is important to know the types of intra-dialysis physical exercises that are safe and effective for HD patients. The type of research analyzed in this journal writing is a randomized controlled trial (RCT) that focuses on inter-dialysis physical exercise interventions both at home and HD center. With a literature search using PubMed, ScienceDirect, Willey, Proquest and Google Scholar databases. And those who meet the relevant requirements and reviews are: 8. Based on several reviews of intradialytic exercise studies in HD patients, intra-dialysis low intensity strength training is found safe and effective in adults and older. Research shows that doing 30 minutes of intradialytic exercise per session 3 times a week for at least ≥8 weeks is beneficial for HD patients. Regular exercise should be considered as an important therapeutic modality in the care of patients undergoing HD. Strength training improves physical performance, nutritional status and physical activity. But this study needs further research, so that it can contribute to the provision of evidence-based information in applying safe intradialytic exercises and become part of routine dialysis treatment units

Previous reports have shown the benefits of intradialytic exercise to patients undergoing hemodialysis. However, most of those studies assessed the effects of exercise in middle-aged patients and little is known about advanced-age patients undergoing hemodialysis. Therefore, the present randomized controlled trial was performed to determine the effectiveness of exercise therapy in advanced-age patients undergoing hemodialysis. This non-blinded, randomized controlled parallel trial enrolled a total of 101 patients who were randomly assigned to intradialytic exercise (n = 51) or usual care (n = 50) groups. The training program included both resistance and aerobic exercises and was performed three times per week for 6 months. The aerobic exercise intensity was adjusted to a target Borg score of 13 for 20 minutes. Four types of resistance exercises were performed using elastic tubing, with three sets of 10 exercises performed at moderate intensity (13/20 on the Borg scale). The usual care group received standard care. Lower extremity muscle strength, Short Physical Performance Battery score, and 10-m walking speed were the outcomes and were evaluated before the hemodialysis session and after 6 months of training. There were statistically significant improvements in Short Physical Performance Battery score (effect size, 0.57; 95% confidence interval, 0.15‒1.95) in the exercise group relative to the control group. There were no statistically significant differences in lower extremity muscle strength or in the 10-m walking speed between the two groups. These findings suggest that 6 months of intradialytic training could improve physical function in older patients undergoing hemodialysis.

The demographic questionnaire was completed at the beginning of the study followed by pre-test for both the groups. Experimental group received intradialytic exercise during initial 2 hours of hemodialysis for 3months besides control group received the routine care. Post test was conducted at the end of 3rd month after the dialysis session. Data were analyzed using Sigma Plot 12 (Systat, USA). After 3 months of intradialytic exercise, a significant reduction was observed in serum creatinine, blood urea, serum potassium, serum phosphate and increase in serum calcium and hemoglobin at the level of p<0.001 in the experimental group. The study findings concluded that intradialytic exercise programme is safe, complementary intervention and do not cost the patient extra time and this effective care can be incorporated in the regular care of patients on hemodialysis.

BACKGROUND: Fatigue is one of the most common problems experienced by chronic kidney disease (CKD) patients undergoing hemodialysis. The application of intradialytic exercises can be the best solutions to overcome fatigue related to CKD.
CASE PRESENTATION: A 50-year-old male patient with CKD, who complained of severe fatigue, was selected in case study. The FACIT Fatigue Scale questionnaire and clinical indicators were measured before and after intervention. The intradialytic exercise was administered at 3rd hour of hemodialysis for 4 sessions in 2 weeks. Each session required 30 minutes, containing warming up, main exercise and cooling down. After two weeks intervention, there was an improvement of fatigue score (16 to 29) and reduction of weakness, tiredness, washout feeling, sleepy, and tiredness on eating. Moreover, the clinical indicators showed positive improvement after the intervention.
CONCLUSION: Thus, intradialytic exercise is practically promising to minimize fatigue related to CKD among patients undergoing hemodialysis. Hemodialysis nurses or other health care providers could utilize this evidence as a program to reduce the fatigue related to CKD.

A Quasi-experimental study was conducted to assess the effectiveness of intradialytic exercise on haemodialytic patients in selected areas of Gandhinagar. The study was conducted among the haemodialytic patients group in selected areas of Gandhinagar. The main aim of our study is to evaluate the effectiveness of intradialytic exercise on haemodialytic patients in selected areas of Gandhinagar. The general system model was used as conceptual framework. A quantitative approach with Quasi experimental study design was used to achieve the objective of the study. The study samples considered of 50 haemodialytic patients in selected hospitals of Gandhinagar. The observational checklist was used to assist the effectiveness of intradialytic on haemodialytic patients in selected areas of Gandhi The result shows that intradialytic exercise given to the haemodialytic patients in experimental group, the muscle strengthening improved which was assessed by observational checklist. In the experimental group, the pretest mean is 8.08 and posttest is 10.6, the mean difference was 2.52, standard deviation of pre-test was 1.87 and of posttest was 3.05 which showing that level of muscle strengthening was improved. In control group, the pre-test mean is 7.4 and the post-test is 7.12, the mean difference is 0.28, standard deviation of pre-test is 1.83 and of post-test is 1.94. Calculated value is 3.83 which is more than table value at level of 0.05 level of significance which showing effectiveness of intradialytic exercise. This study therefore, offers an encouraging solution towards improvement of muscle weakness and level of muscle strengthening score after administration of intradialytic exercise.nagar. The study intends to assess the effectiveness of intradialytic exercise on haemodialytic patients in selected areas of Gandhinagar. The study reveals that the post-testscore of haemodialytic patients on muscle strengthening is higher in the experimental group than the control group. It shows that Intradialytic exercise is more effective in experimental group so Null hypotheses is rejected and research hypotheses is accepted.

Pendahuluan: Chronic Kidney Disease (CKD) yang terjadi > 3 bulan dan ditunjukan oleh nilai laju filtrasi glomerulus (GFR) < 15 ml/menit/1,73m2 pada tahap stadium akhir (End Stage Renal Disease (ESRD)) diperlukan terapi pengganti ginjal, yaitu salah satunya hemodialisis. Salah satu permasalahan yang sering dikeluhkan pasien hemodialisis rutin adalah muscle cramp, yang biasanya dirasakan satu jam sebelum hemodialisis selesai dilakukan. Untuk mengatasi gejala tersebut dilakukan leg exercise intradialytic. Tujuan penelitian untuk mengidentifikasi pengaruh terapi leg exercise intradialytic terhadap penurunan muscle cramp pada pasien CKD yang menjalani hemodialisis. Metode: Metode penelitian: penelitian bersifat kuantitatif dengan desain quasi eksperimental one group pre-post test design. Sampel ditentukan dengan metode purposive sampling. Hasil: Hasil penelitian menunjukkan bahwa terjadi perubahan yang signifikan muscle cramp sebelum dan sesudah dilakukan terapi leg exercise intradialytic dengan p value = 0,000. Kesimpulan: Terapi leg exercise intradialytic selama hemodialisis dapat menurunkan muscle cramp pada pasien CKD yang menjalani hemodialisis.

Hemodialysis patients typically have extremely low physical activity levels, which contributes to poor physical function and quality of life (QOL). Numerous studies show that exercise, intradialytic cycling in particular, may improve physical and cardiovascular function and QOL. But there are also significant inconsistencies in the literature, and the benefits in many studies are modest. This may be due in part to methodological limitations in intradialytic cycling trials, including small sample sizes, short interventions, low volume, and intensity of exercise prescriptions, and/or low retention and protocol adherence rates. The goal of this review is twofold. First, we summarize the current literature on intradialytic cycling in HD patients, highlighting benefits and concerns noted in several recently published trials that were among the most robust and clinically relevant trials conducted to date. Second, we will consider strategies for moving forward with exercise and physical activity interventions in HD, including promoting intradialytic cycling as a core component of a more patient-centric and comprehensive strategy that helps progress patients towards standard physical activity guidelines. We urge researchers and exercise professionals to consider intradialytic cycling as a valuable component of a comprehensive patient-centered, lifestyle intervention, as opposed to a stand-alone exercise prescription.

Background: Exercise training has positive effects on physical functions and could reduce a sedentary lifestyle for hemodialysis (HD) patients. Given that low-level physical activity increases morbidity and mortality, here, we aimed to determine the effects of an intradialytic exercise program delivered at different frequencies on HD patients in Taiwan. Methods: This study is a prospective, randomized control trial. An intradialytic exercise program will be arranged for patients after receiving their informed consent. Patients will be segregated at random into three groups as follows: (a) three times/week of intradialytic exercise training plus standard care maintenance of HD, (b) two times/week of intradialytic exercise training plus standard care maintenance of HD, or (c) standard care maintenance of HD. Subjects will be followed for 24 weeks. At three time points, 0, 12, and 24 weeks, the primary outcome, the Short-Form 36 score, will be measured. Additional secondary outcomes to be measured are the Beck depression inventory, 6 min walking test, sit-to-stand test, and anthropometric measures such as the body mass index, thigh circumference, and the proportion of fat in the body composition. Conclusions: There is emerging evidence in support of intradialytic exercise improving health-related quality of life for patients on HD. However, the difference in the therapeutic effects between three times per week and twice per week has never been determined. With this study, we anticipate to fill the knowledge gap in the exercise prescription among HD patients.

Objective: To evaluate the effect of intradialytic cycling exercise on physical functional performance with gain in muscle strength and endurance in end-stage renal disease patients with haemodialysis. Design: Randomized controlled trial, with repeated measurements at baseline and after 4, 8, and 12 weeks of intradialytic cycling exercise. Setting: A 50-bed haemodialysis centre in a regional hospital in Taiwan. Subjects: Seventy-six regular haemodialysis patients, recruited and equally and randomly assigned to exercise and control groups. Intervention: The intradialytic cycling exercise was performed for 12 weeks and comprised warm-up, main, and cool-down exercise phases. A stationary cycling equipment was used, which involved aerobic and resistance modalities. The intensity was maintained at somewhat hard exertion. Each intradialytic cycling exercise was implemented for 30 minutes, starting at the second hour of treatment. Main measure: Measured outcomes were 6-minute walk distance, time taken to complete 10 sit-to-stand-to-sit cycles and number of sit-to-stand-to-sit cycles in 60 seconds. Results: Average (standard deviation) participant age was 55.47 (13.00) years. Therefore, the 6-minute walk distance was significantly different at weeks 8 ( P = 0.01) and 12 ( P < 0.001) in the exercise group compared with that in the control group at baseline. Notably, sit-to-stand-to-sit outcomes ( P = 0.01) significantly influenced the 6-minute walk distance. Sit-to-stand-to-sit outcomes significantly improved in the exercise group ( P < 0.05). Conclusion: Twelve-week intradialytic exercise for patients on haemodialysis can improve physical functional performance with gain muscle strength and endurance. This is a safe and effective method for improving health.

Background: Exercise improves the physical function of people suffering from chronic kidney disease on hemodialysis (HD). Virtual reality is a new type of intradialysis exercise that has a positive impact on physical function. Intradialysis exercise is recommended during the first 2 h, but its safety in the last part of the dialysis session is unknown. Methods: This was a pilot sub-study of a clinical trial. Several hemodynamic control variables were recorded, including blood pressure, heart rate, and intradialytic hypotensive events. These variables were recorded during three different HD sessions, one HD session at rest, another HD session with exercise during the first two hours, and one HD session with exercise during the last 30 min of dialysis. The intradialysis virtual reality exercise was performed for a maximum of 30 min. Results: During exercise sessions, there was a significant increase in heart rate (6.65 (4.92, 8.39) bpm; p < 0.001) and systolic blood pressure (6.25 (0.04,12.47) mmHg; p < 0.05). There was no difference in hemodynamic control between the sessions with exercise during the first two hours and the sessions with exercise during the last 30 min. There was no association between intra-dialytic hypotensive events at rest (five events) or exercise at any point (two vs. one event(s), respectively). Conclusion: performing exercise with virtual reality at the end of a hemodialysis session is not associated with hemodynamic instability.

Hemodialysis patients were mostly physically inactive and their functional capacity had been reduced, even though exercise could improve physical fitness, aerobic capacity, quality of life, decrease depressive symptoms and include dialysis adequacy (measured as Kt/V). A lower Kt/V value means that the patient had inadequate HD, can lead to increased hospitalization time and costs, increased morbidity and mortality and should therefore be avoided. This study aims to determine the application of intradialytic exercise to hemodialysis adequacy. The method used in this study was literature review with electronic searches on 4 online databases: Wiley Online, Sciencedirect, clinical key and EBSCOhost (Medline and CINAHL plus). The keywords used were "Intradialytic Exercise", "Intradialytic Training", "Hemodialysis Adequacy", "Urea reduction" and "Kt / V". The criteria for the articles chosen in this paper: full text, published in 2015-2020 and in English. Selected articles were assessed for quality using an evaluation table. From the search results and assessed for quality, there were 7 articles. The results showed that the application of intradialytic exercise improves hemodialysis adequacy, quality of life, physical fitness, lowers blood pressure, and had an effect on the mental of hemodialysis patients. In its application, it needs support from staff and hospital organizations so that it could be carried out optimally without neglecting other important tasks. The type of exercise most widely used was aerobic exercise. This exercise did not require special tools like other types of exercises. Easy to implement and did not cause side effects, so it is recommended as a type of exercise that can be applied.

Pasien hemodialisis sebagian besar tidak aktif secara fisik dan kapasitas fungsionalnya telah berkurang padahal olahraga/latihan dapat meningkatan kebugaran fisik, kapasitas aerobik, kualitas hidup, berkurangnya gejala depresi dan termasuk adekuasi dialisis (diukur sebagai Kt/V). Nilai Kt/V yang lebih rendah berarti HD yang dijalani pasien tidak memadai, dapat menyebabkan peningkatan waktu rawat inap dan biaya, peningkatan morbiditas serta mortalitas dan karena itu harus dihindari. Penelitian ini bertujuan untuk mengetahui penerapan latihan intradialitik terhadap adekuasi hemodialisis. Metode yang dilakukan dalam penelitian ini adalah literatur review dengan pencarian elektronik pada 4 database online yaitu: Wiley Online, Sciencedirect, clinical key dan EBSCOhost (Medline dan CINAHL plus). Kata kunci yang digunakan “Intradialytic Exercise”, “Intradialytic Training”, “Hemodialysis Adequacy”, “Urea reduction” dan “Kt/V”. Kriteria artikel yang dipilih dalam penulisan ini adalah full text, dipublikasi tahun 2015-2020 dan berbahasa Inggris. Artikel terpilih dilakukan penilaian kualitas menggunakan tabel evaluasi. Dari hasil pencarian dan penilaian kualitas didapatkan sebanyak 7 artikel. Didapatkan hasil bahwa penerapan latihan intradialitik meningkatkan adekuasi hemodialisis, kualitas hidup, kebugaran fisik, menurunkan tekanan darah, dan berefek pada mental pasien hemodialisis. Pada penerapannya perlu dukungan dari staf dan organisasi rumah sakit sehingga dapat dilaksanakan secara maksimal tanpa mengabaikan tugas penting lainnya. Jenis latihan yang paling banyak digunakan yaitu latihan aerobik. Latihan ini tidak membutuhkan alat khusus seperti jenis latihan lain. Mudah dilaksanakan dan tidak menimbulkan efek samping, sehingga direkomendasikan sebagai jenis latihan yang dapat diterapkan.

Hemodialysis (HD) patients have high hospitalization rates. Benefits of intradialytic exercise have been proven in numerous studies yet exercise programs are still rarely used in the treatment of end-stage kidney disease (ESKD). Our objective was to determine if there was an association between a 6-month intradialytic bicycling program and hospitalization rates and length of stay (LOS) in ESKD patients. This was a retrospective cohort study that took place 6 months prior to and 6 months during an intradialtyic exercise program at an outpatient HD unit in Calgary, Alberta, Canada. Participants comprised 102 patients who had commenced HD <6 months (incident) or >6 months (prevalent) prior to starting exercise. The intervention comprised a 6-month intradialytic bicycling program. Main outcome measures were hospitalization rate, cause of hospitalization, and LOS. Patients were predominantly male (67.6%) aged 65.6 ± 13.5 years and median HD vintage 1 year (range: 0–12). Comorbidities included diabetes mellitus (50%) and cardiac disease (38.2%). The hospitalization incidence rate ratio (IRR) was 0.48 (0.23–0.98; P = 0.04) in incident and 0.89 (0.56–1.42; P = 0.64) in prevalent patients. The LOS decreased from 7.8 (95% confidence interval (CI): 7.3–8.4) to 3.1 (95% CI: 2.8–3.4) days and LOS IRR was 0.39 (0.35–0.45; P < 0.001). The main predictors of hospitalization were lower albumin levels (P = 0.007) and lack of intradialytic exercise program participation (P < 0.001). In conclusion, 6 months of intradialytic exercise was associated with decreased LOS in both incident and prevalent HD patients.

The aim of this study was to conduct a systematic review in order to evaluate the main effects of intradialytic physical exercise on different parameters of functional capacity in dialytic patients. The clinical question that guided the objective of this study was developed using the PICO method (Population, Intervention, Comparation and Outcome)." Where i) population: patients on hemodialysis; ii) intervention: physical exercise; iii) comparison: active vs sedentary patients; and iv) outcome: functional capacity. The combinations of the following keywords were used: CKD, physical exercise, exercise, physical training and hemodialysis. The selection of studies was performed using the PubMed database and only studies dating from 2011 to 2021 were selected. The search results led to 53 studies. The following steps were carefully analyzed, such as the title, abstract and the full paper description to evaluate whether they met the following inclusion criteria: i) target audience of the studies should be patients on hemodialysis; ii) outcomes that analyze different parameters of functional capacity; iii) Intervention using physical exercise; and iii) intradialytic exercise. The final results indicate that intradialytic physical exercise can cause significant changes in the evaluated outcomes of functional capacity, such as increased strength, improvement of cardiorespiratory function, and improvement of locomotor activity. It was concluded that intradialytic training protocols should be encouraged in clinical practices because they are responsible for causing beneficial changes in the functional capacity of hemodialytic patients.

Exercise is fundamentally important in managing chronic diseases and improving health-related quality of life (HRQL). However, whether intradialytic exercise is safe through assessment of changes in dialytic parameters and has a positive impact on HRQL and depression status of hemodialysis patients requires further research with diverse racial and cultural populations to identify. This study aimed to evaluate the effects of intradialytic exercise on dialytic parameters, HRQL, and depression status in hemodialysis patients. A randomized controlled trial was conducted at a medical center in Northern Taiwan. Sixty-four hemodialysis patients were recruited using stratified random sampling. Participants were randomized into an experimental group (EG, n = 32) or a control group (CG, n = 32). The EG received a 12-week intradialytic exercise program while the CG maintained their usual lifestyles. Dialytic parameters, HRQL, and depression status were collected at baseline and at 12 weeks. The results indicated no differences in the dialytic parameters from the baseline between both groups. However, the EG had increased HRQL (ß = 22.6, p < 0.001) and reduced depression status (ß = −7.5, p = 0.02) at 12 weeks compared to the CG. Therefore, a 12-week intradialytic exercise regime is safe and effective in improving HRQL and reducing depression status for hemodialysis patients.

Acute haemodynamic instability is a natural consequence of disordered cardiovascular physiology during haemodialysis (HD). Prevalence of intradialytic hypotension (IDH) can be as high as 20–30%, contributing to subclinical, transient myocardial ischemia. In the long term, this results in progressive, maladaptive cardiac remodeling and impairment of left ventricular function. This is thought to be a major contributor to increased cardiovascular mortality in end stage renal disease (ESRD). Medical strategies to acutely attenuate haemodynamic instability during HD are suboptimal. Whilst a programme of intradialytic exercise training appears to facilitate numerous chronic adaptations, little is known of the acute physiological response to this type of exercise. In particular, the potential for intradialytic exercise to acutely stabilise cardiovascular hemodynamics, thus preventing IDH and myocardial ischemia, has not been explored. This narrative review aims to summarise the characteristics and causes of acute haemodynamic instability during HD, with an overview of current medical therapies to treat IDH. Moreover, we discuss the acute physiological response to intradialytic exercise with a view to determining the potential for this nonmedical intervention to stabilise cardiovascular haemodynamics during HD, improve coronary perfusion, and reduce cardiovascular morbidity and mortality in ESRD.

Objective: To determine the effects of different intradialytic exercise training modalities on physical functioning and health-related quality of life of maintenance hemodialysis patients. Methods: We searched MEDLINE, Cochrane Trials Register and CINAHL for controlled trials that evaluated the effects of intradialytic exercise training for maintenance hemodialysis patients and published from the earliest available date to December 2017. Weighted mean difference and 95% confidence interval (CI) were calculated, and heterogeneity was assessed using the I2 test. Results: Fifty-six studies met the study criteria, comprising a total of 2586 patients. Compared with no exercise, combined aerobic and resistance exercise resulted in significant improvement in peak VO2 weighted mean difference (5.1 mL kg−1 min−1; 95% CI: 3.4, 6.8 mL kg−1 min−1), depression symptoms (−7.32; 95% CI −9.31, −5.33) and both physical function (10.67 points; 95% CI 1.08, 20.25 points) and vitality (10.01 points; 95% CI 4.30, 15.72 points) domains of health-related quality of life. Resistance exercise alone was significantly associated with improvement in the 6-minute walk test distance (30.2 m; 95% CI 24.6, 35.9 m), knee extensor strength (0.6 N; 95% CI 0.1, 1.0 N) and Physical Component Score of health-related quality of life (9.53 points; 95% CI −3.09, 22.15 points) when compared with control group. Aerobic exercise alone was not significantly associated with aerobic capacity and quality of life improvement. Conclusion: The results provide support to interventions that combine intradialytic aerobic and resistance exercises to improve physical functioning and quality of life in end-stage renal disease patients undergoing hemodialysis.

Introduction: Mortality rates are high in end-stage renal disease due to cardiovascular complications. Perfusion of the myocardium declines during and after hemodialysis sessions with the potential for aerobic exercise to mitigate these during hemodialysis. Objectives: The purpose of this study was to investigate acute changes in hemodynamics in subjects with end-stage renal disease (ESRD) during exercise. Patients and Methods: Subjects (n = 10) were monitored for 1.5 hours during hemodialysis treatment during a control (CON) and an exercise (EX) session. Subjects cycled using an ergometer strapped to the reclining dialysis chair at an RPE of 11-13 for 30 minutes during the EX session beginning at 30 min into dialysis and ending at 60 minutes. Data for systolic blood pressure (SBP), diastolic blood pressure (DBP), and mean arterial pressure (MAP) were collected using an automated blood pressure cuff attached to the hemodialysis machine. Data for cardiac output (Q̇ ), cardiac power index (CPI), stroke volume (SV), systemic vascular resistance (SVR), and heart rate (HR) were collected using the NICaS bioelectrical impedance device. Results: During the EX session, CPI, Q̇ , SV, and HR were significantly greater (P<0.05) than the CON session. Additionally, Q̇ was significantly (P< 0.05) greater at 45 minutes and 60 minutes compared to 15 minutes. HR was significantly (P<0.05) greater at 45 minutes compared to 90 minutes. No significant interactions were found for MAP, CPI, Q̇ , HR, SV, SBP, DBP, or SVR. Conclusion: In conclusion, exercise during dialysis may decrease the likelihood of experiencing ischemic or hypotensive events by enhancing myocardial perfusion through increasing CPI and Q̇ .

Aim: This study was aimed to determine the effect of intradialytic aerobic exercise on blood urea in hemodialysis (HD) patients. Introduction: HD is one way to treat advanced kidney failure and can help to carry on an active life despite failing kidneys. Intradialytic exercise (IDE) is the exercise training performed during the HD session to increase the patient’s strength and endurance. Materials and Methods: A quasi-experimental study was carried out among 60 adult patients on HD (30 each in control and experimental groups), selected by consecutive sampling technique, at selected tertiary care centers in Chennai. The baseline characteristics and pretest blood urea were collected from both the groups and the intradialytic aerobic exercise was administered for 8 weeks (3 times/week) among the experimental group of patients. Then Post-test I and II blood urea were assessed in both the groups at end of 4th and 8th week. The data were analyzed in SPSS 22. Results: IDE showed a significant effect in reducing blood urea (mg/dl) among experimental group of participants than control group in post-test I (76.73 ± 17.58, 93.20 ±19.24) and in post-test II (62.23 ± 13.28, 90.20 ±19.60) at P = 0.001. There was no significance difference in baseline data/pre-test between experimental and control group pre-test (103.53 ± 30.03, 103.70 ± 30.46). Conclusion: This study reveals that intra dialytic aerobic exercises is beneficial in terms of reducing blood urea, as a supplementary therapy for HD patients.

Metode pengumpulan data menggunakan purposive sampling yang melibatkan 7 pasien yang memenuhi kriteria inklusi berdasarkan jurnal acuan.Aerobic exercise dilakukan dilakukan selama 10-15 menit / hari, dalam dua kali seminggu selama 1 bulan. Peningkatan adekuasi dinilai dengan melihat Kt/v dan nilai Urea Reduction Rate (URR)) sebelum dan sesudah pemberian intervensi. Hasil: Adekuasi dialisis meningkat pada akhir intervensi dengan rata- rata peningkatan Kt/v sebesar 2.17 dan peningkatan nilai URR sebesar 2.14%

Hemodialisa merupakan terapi yang dilakukan oleh pasien yang mengalami Chronic Kidney Disease (CKD). Kelelahan sering dialami oleh pasien CKD yang menjalani hemodialisa. Studi ini bertujuan untuk menerapkan evidence base practice yaitu terapi intradialytic exercise pada pasien hemodialisa. Desain studi ini menggunakan pre-post design. Studi ini menggunakan kuesioner FAS (Fatigue Assesment Scale) dengan interpretasi tingkat kelelahan normal (skor <22), ringan (skor 23-32), sedang (skor 33-34), berat (skor >35). Sampel penelitian berjumlah 3 responden. Penelitian dilakukan pada bulan September 2022 di unit Hemodialisa RS PKU Aisyiyah Boyolali. Terapi intradialytic exercise dilakukan pada 1 jam terakhir selama sesi hemodialisa dengan durasi waktu 10 menit dalam satu kali pertemuan. Hasil penelitian menunjukkan terdapat penurunan tingkat kelelahan pada responden setelah dilakukan terapi intradialytic exercise. Didapatkan data pada 2 responden saat pretest menunjukkan skor FAS dalam kategori berat yaitu dengan skor 36 dan 1 responden dalam kategori sedang yaitu 33. Saat posttest, ketiga responden mengalami penurunan skor FAS yaitu responden dalam kategori skor FAS berat menurun menjadi kategori sedang, sedangkan responden dengan kategori sedang mengalami penurunan menjadi kategori ringan. Penerapan evidence base practice berupa terapi intradialytic exercise yang dilakukan dapat menurunkan skor FAS yang menandakan adanya penurunan tingkat kelelahan pada pasien yang menjalani hemodialisis di RS PKU Aisyiyah Boyolali.

Cardiovascular disease is the most common cause of death in hemodialysis (HD) patients. Intradialytic aerobic exercise training has a beneficial effect on cardiovascular system function and reduces mortality in HD patients. However, the impact of other forms of exercise on the cardiovascular system, such as hybrid exercise, is not clear. Briefly, hybrid exercise combines aerobic and strength training in the same session. The present study examined whether hybrid intradialytic exercise has long-term benefits on left ventricular function and structure and the autonomous nervous system in HD patients. In this single-group design, efficacy-based intervention, twelve stable HD patients (10M/2F, 56 ± 19 years) participated in a nine-month-long hybrid intradialytic training program. Both echocardiographic assessments of left ventricular function and structure and heart rate variability (HRV) were assessed pre, during and after the end of the HD session at baseline and after the nine-month intervention. Ejection Fraction (EF), both assessed before and at the end of the HD session, appeared to be significantly improved after the intervention period compared to the baseline values (48.7 ± 11.1 vs. 58.8 ± 6.5, p = 0.046 and 50.0 ± 13.4 vs. 56.1 ± 3.4, p = 0.054 respectively). Regarding HRV assessment, hybrid exercise training increased LF and decreased HF (p < 0.05). Both conventional Doppler and tissue Doppler imaging indices of diastolic function did not change after the intervention period (p > 0.05). In conclusion, long-term intradialytic hybrid exercise training was an effective non-pharmacological approach to improving EF and the cardiac autonomous nervous system in HD patients. Such exercise training programs could be incorporated into HD units to improve the patients’ cardiovascular health.

Intradialytic exercise yang dilakukan pada saat hemodialisa dapat meningkatkan aliran darah otot, memperbesar jumlah kapiler serta memperbesar luas dan permukaan kapiler sehingga meningkatkan perpindahan urea dan toksin dari jaringan ke vaskuler yang kemudian dialirkan ke dializer atau mesin. Penelitian ini untuk mengetahui adanya Pengaruh Intradialytic Exercise Terhadap Penurunan Tekanan Darah Pada Pasien Hemodialisa di Rumah Sakit Grandmed Lubuk Pakam. Jenis penelitian ini kuantitatif dengan desain penelitian pra eksperimental dengan penelitian (one-group pre-post test design). Yang dilakukan pada 55 orang pasien gagal ginjal kronik yang menjalani terapi hemodialisa di Rumah Sakit Grandmed Lubuk Pakam yang dilaksanakan pada bulan Maret 2021. Bedasarkan tabel Uji Wilcoxon menunjukkan bahwa nilai Z pada sistol post test – sistol pre test yaitu -5,787 dengan nilai sig ( p = 0,000 0,005) dan nilai Z pada diastol post test – diastol pre test yaitu -6,205 dengan nilai sig ( p = 0,000 0,005) sehingga dapat diasumsikan bahwa ada pengaruh intradilaytic exercise terhadap penurunan tekanan darah pada pasien hemodialisa di Rumah Sakit Grandmed Lubuk Pakam Tahun 2021
