This chapter provides a broad overview of tic disorders. Specifically described are different tic disorders and the psychiatric and psychosocial difficulties commonly experienced by children with tic disorders. Common conceptual models of tic disorders and neurobiological and behavioral research that support these models are reviewed. Recommended practices for assessing children with tic disorders are discussed, commonly used measures for assessing tic-related severity and impairment are detailed, and both evidence-based and experimental treatments for tic disorders are described. A case example is provided to demonstrate how clinicians may implement the recommended assessment practices and one of the evidence-based treatments discussed in the chapter (Comprehensive Behavioral Intervention for Tics). The chapter concludes by considering theoretical and practical challenges and implications specific to tic disorders.

Many psychiatric disorders of childhood have a chronic course. As such, they impact multiple developmental epochs and negatively influence developmental trajectories. While early identification and intervention may minimize, or even prevent, symptoms being carried into adulthood, the availability of evidence-based treatments is sparse in children and adolescents compared to adult populations. Establishing effective interventions for psychiatric symptoms presenting in childhood is critical given the chronic course of most psychiatric disorders. This chapter describes psychopharmacological and psychosocial interventions used for the treatment of childhood psychiatric conditions, with an emphasis on empirically supported treatments. Both symptom- and diagnosis-specific approaches are described as well as the use of combined interventions for the following childhood psychiatric conditions: autism spectrum disorder (ASD), intellectual disability (ID), attention-deficit/hyperactivity disorder (ADHD), anxiety, depression, obsessive compulsive disorder (OCD), chronic tic disorders, eating disorders, and conduct problems.

This chapter briefly describes tic disorders and trichotillomania (TTM) and reviews the pharmacotherapy and psychosocial treatment outcome literature for each of these conditions. In contrast to anxiety or depression, distorted or maladaptive cognitions do not appear to play a central role in the etiology or maintenance of tic disorders and TTM, and therefore cognitive therapy is not emphasized in the psychosocial treatments studied to date. Treatment protocols are best characterized as “behavioral,” although some include ancillary cognitive interventions. Behavioral treatments that include habit reversal training (HRT) appear to hold the greatest promise for each of these conditions, and these are described in some detail. Future directions in treatment research are suggested.

The therapy manuals included in this volume—the Unified Protocols for Transdiagnostic Treatment of Emotional Disorders in Children (UP-C) and Adolescents (UP-A)—include evidence-based treatment strategies to assist child and adolescent clients to function better in their lives. The manuals include specific guidelines for treatment delivery, and they also contain information about how to introduce parent-directed strategies to help promote long-term uptake of youth-directed therapy skills. The evidence-based treatment skills presented may be applied by therapists to children and adolescents with a wide variety of emotional disorders. This treatment guide takes a transdiagnostic approach to the treatment of emotional disorders. Some of the disorders that may be targeted include anxiety disorders and depressive disorders. This treatment is flexible enough for use with some trauma and stress-related disorders (including adjustment disorders), somatic symptom disorders, tic disorders and obsessive-compulsive disorders. The transdiagnostic presentation of evidence-based intervention techniques within these treatments may be particularly useful for children and adolescents presenting with multiple emotional disorders or mixed/subclinical symptoms of several emotional disorders.

The Unified Protocols for Transdiagnostic Treatment of Emotional Disorders in Children and Adolescents (UP-C and UP-A) are evidence-based, transdiagnostic intervention programs that target core emotion regulation processes that may be shared across varying presentations of internalizing distress or disorders in youth. Given their popular transdiagnostic and modular structures, the UP-C and UP-A have quickly been disseminated and implemented with a variety of populations and in differing treatment settings. This volume aims to aid UP-C and UP-A therapists in understanding varied applications and modifications of these approaches and assist them in applying such in their own practice. To that end, chapters are offered on not only standard UP-C and UP-A research and practice but also applications for youth with obsessive-compulsive symptoms, tic-related concerns, substance use, serious mental illness, and eating disorders. Structural modifications to the UP-C and UP-A using a stepped care model and when delivering care in pediatric or community practice settings and in differing cultures or languages are also discussed. To aid in their use, each chapter includes a brief, user-friendly description of the modifications or adaptations of the UP-C and/or UP-A that are described therein.

Implanted venous ports in children are commonly placed by pediatric interventional radiologists, primarily through the internal jugular vein to allow long-term delivery of intravenous access. The ideal location of the catheter tip is the junction of the superior vena cava and right atrium. High catheter tip positions may result in failure of the port catheter due to venous thrombosis, catheter occlusion, or fibrin sheath formation, whereas low catheter tip positions may cause catheter occlusion, cardiac arrhythmias, or perforation. This chapter describes indications, contraindications, equipment, techniques, and complications of the endovascular snaring technique of central venous port tip revision in children.

Unified Protocol for Transdiagnostic Treatment of Emotional Disorders in Adolescents: Workbook (UP-A) provides evidence-based treatment strategies to assist adolescent clients to function better in their lives. This treatment is designed for adolescents who are experiencing feelings of sadness, anxiety, worry, anger, or other emotions that get in the way of their ability to enjoy their lives and feel successful. The workbook is written for adolescents and guides them through each week of the program with education, activities, and examples that will help them to understand the role that emotions play in their behaviors every day. Adolescents are taught helpful strategies for dealing with uncomfortable emotions and will receive support in making choices that will move them closer to their long-term goals. The evidence-based treatment skills presented in the accompanying Therapist Guide may be applied by the therapist to adolescents with a wide variety of emotional disorders. The UP-A takes a transdiagnostic approach to the treatment of the emotional disorders. Some of the disorders that may be targeted include anxiety disorders (e.g., generalized anxiety disorder, social anxiety disorder, separation anxiety disorder, specific phobias, panic disorder, illness anxiety disorder, agoraphobia) and depressive disorders (e.g., persistent depressive disorder, major depressive disorder). This treatment is flexible enough for use with some trauma and stress-related disorders (including adjustment disorders), somatic symptom disorders, tic disorders, and obsessive-compulsive disorders. Reverse curve catheters facilitate catheterization of vessels angled oppositely from the point of access. Most commonly, they are used to access caudally angulated vessels from a femoral approach. An array of these catheters exists, and their utilization greatly expands the options for intervention from any single access. Once formed, the catheters are gently advanced forward until their tip engages the vessel of interest, typically confirmed with an injection of contrast under live fluoroscopy. Engagement of the vessel is then performed by advancing a guidewire into the vessel of interest and retracting the catheter to seat the tip more distally within the vessel, providing a stable access. This chapter reviews four techniques for forming large reverse curve catheters.

Bacteria of the genus Rickettsia belong to the family Rickettsiaceae in the order Rickettsiales and have for long been described simply as short, Gram-negative, strict intracellular rods that retain basic fuchsin when stained by the method of Gimenez (Raoult and Roux 1997). These bacteria are associated with ticks, lice, fleas or mites. To date the Rickettsia genus contains 24 recognized species classified into three groups based on their antigenic, morphological, and ecologic patterns: 1) the typhus group, 2) the spotted fever group and 3) Rickettsia bellii (Fournier and Raoult 2007). Most spotted fever group (SFG) rickettsiae are closely associated with ticks belonging to the family Ixodidae (also called “hard” ticks) (Parola et al. 2005). Ticks can act as vectors, reservoirs, and/or amplifiers of SFG rickettsiae and require optimal environmental conditions which determine the geographic distribution of the vectors and consequently the risk areas for rickettsioses. Many Rickettsia species are strictly associated with one genus of ticks and the transmittion to people is made through the tick bite, which generally implies that the Rickettsia can localize to their salivary glands. Therefore, since larvae, nymphs, and adults may all be infective for susceptible vertebrate hosts, the ticks must be regarded as the main reservoir host of rickettsiae. Humans are not considered as good reservoirs for Rickettsiae, as they are seldom infested with ticks for long periods and rickettsiaemia has normally short duration, especially with antibiotic intervention.

Central venous catheters are frequently placed to facilitate the care of patients with multiple conditions, often via jugular approach. Occasionally, the catheter tip can migrate after placement and become positioned within the azygos arch. This abnormal catheter positioning can lead to an increased number of complications, including catheter malfunction, thrombosis, or even rupture of the azygos vein requiring surgical intervention. Although invasive repositioning of the catheter is always an option, minimally invasive options can be attempted to repair azygos catheter malposition. Fortunately, noninvasive maneuvers, including manipulation of the port on the skin and patient breathing instructions, can sometimes repair the malpositioned catheter.

Safe and successful ultrasound-guided interventional procedures depend on the ability to visualize both anatomical structures of interest and the advancing needle. This chapter describes various strategies for optimizing needle visualization and tip localization during freehand guidance of a needle to a tissue target using ultrasound. Challenges to needle visualization include the poor echogenicity of standard needles at insertion angles steeper than 30–45 degrees to the horizontal, the difficulty of aligning the needle with the ultrasound beam, and manipulation in three dimensions based on two-dimensional visual information. Attention to ergonomics improves probe and needle control and facilitates alignment. Needle echogenicity can be improved by using a shallower angle, where possible, and echogenic needle technology. Hand movements of probe and needle should be kept small and controlled. Indirect cues of needle tip location are also extremely useful and should be utilized routinely.

In 1963, the “shuttle diplomacy” efforts of Ellsworth Bunker and Ralph Bunche between Riyadh, Cairo, and Sana’a led to an agreement for the withdrawal of Egyptian and Saudi intervention in the Yemen Civil War. The UN Yemen Observer Mission, which ran from 1963 to 1964, was given the responsibility to oversee this withdrawal. Contemporary and historic perceptions of UNYOM have been tainted by a clash of personalities between the mission leader, Carl von Horn, who embodied the old European leadership of the UN, and Secretary General U Thant, who represented the new Asia-Africa bloc in the UN. UNYOM has been portrayed as the first failure in a new era of “tin-cup peacekeeping” that could scarcely feed and supply UN personnel. The reality, gleaned from interviews in addition to newly available UN and Canadian archives, is starkly different. The mission was in fact a success, limited only by the global conflict that overshadowed UNYOM.

José Revueltas’s essay represents the kind of anti-essentialist critique that had been leveled against the philosophy of lo mexicano in the previous decades. In his own Marxist-inspired intervention, Revueltas accomplishes two things. First, he provides a history of the concept of Mexican national identity, an idea that, given the complex economic and political history of Mexico, has been impossible to realize. Tracing the Aztec Empire’s failures to unify its various tributaries, which ultimately led to the empire’s downfall at the hands of Cotés, Revueltas suggests that the idea of a common national identity has always been a dream of those seeking political and economic power. The second accomplishment is that he offers a brutal critique of the idea of lo mexicano itself, which, in his interpretation, states that there is or can be a unified national identity that depends on finding those essential commonalities that tie all Mexican people together.

Retrograde access of the common femoral artery for ipsilateral iliac and up-and-over contralateral iliac-to-tibial interventions has been the standard of care for lower extremity procedures. However, ipsilateral antegrade access has gained popularity for infrainguinal occlusive disease. Proximity of the access site to the point of occlusion confers a higher technical success rate. Interestingly, there are times where conversion of a single femoral access from retrograde to antegrade or antegrade to retrograde may be desired. Three techniques are reviewed in this chapter: the first technique involves using a reverse curve catheter in conjunction with a Glidewire. The second technique is a “rebound” method whereby a Fogarty catheter is inflated just beyond the tip of a retrograde sheath to deflect a side-by-side Glidewire in the opposite direction. The third technique describes converting an antegrade sheath back to retrograde using a “buddy wire.”

Adverse events are an unfortunate reality of caring for patients in our current healthcare system. Preventing and mitigating these events are an important part of quality improvement. First, an understanding of what events occur and how often they are occurring is critical to planning improvements. Incident reporting systems are one way of gathering this information. Then, events should be categorized and analyzed for improvement. The failure mode and effects analysis (FMEA) and bow-tie diagram are two tools for this purpose. Once an event has occurred, consideration should be given to the caregivers as well as the patient when managing and resolving adverse events. Prevention requires strong analysis of events and recognition of both latent (system) and human causes. Interventions have different degrees of effectiveness, ranging from highly effective forcing functions, to marginally effective encouraging statements. There are four steps to event management: mitigation, immediate management, refractory management, and follow-up.

Percutaneous transluminal coronary angioplasty (PTCA) transformed the cardiac catheter from a diagnostic tool into a treatment tool. The technology involved a special catheter fitted with a balloon near its tip that could be blown up to expand a narrowed coronary artery segment. For patients with angina, the procedure was an attractive alternative to coronary bypass surgery. Mayo cardiologists were among the first to adopt angioplasty and to call for controlled clinical trials to compare it to bypass surgery. Initially, cardiologists (who already performed coronary angiography) learned to perform PTCA informally. After attending one or more live demonstration courses, many began to perform angioplasty in their local hospitals. The philosophy in many contexts was “see one, do one.” By the mid-1980s, however, more rigorous training expectations were elaborated. Heart specialists who performed PTCA were described as “interventional cardiologists,” a phrase that acknowledged that this catheter-based treatment had immediate effects.

In recent years, ideas of conscience and the liberty of conscience have become ever more salient in public discourse. Historically, these concepts have been used to mark out a certain scope of freedom and protection in moral, political and legal conflicts. In our time, individual conscience is frequently used to legitimate objections to, for instance, military service and medical interventions like abortion and vaccination. So too in Sweden – a country widely described as one of the most modern and secularized societies in the world. In this volume, a group of researchers in history, human rights, law, ethics and sociology of religion address some of the most central issues around conscience and the liberty of conscience in Sweden from the middle ages to the present. By situating conscience and liberty in wider intellectual, social and political settings, the essays provide alternative ways of thinking about the most intractable problems surrounding these concepts – the relationship between law and morality, the tension between individual and collective freedom, as well as the role of religion in public affairs. This volume will create new avenues of research for scholars and students interested in challenges related to conscience and liberty: both those in ethics, politics and law seeking a historical perspective, and those in history who want to tie their studies to the present.

Finally, historians turn their attention to those who are usually caught up in events beyond their control or understanding. This volume details the dismal impact war has had on the African people over the past five hundred years, from slavery days, the Zulu War, World Wars I and II, to the horrific civil wars following decolonization and the genocide in Rwanda. Chapters provide a representative range of civilian experiences during wartime in Africa extending from the late eighteenth century to the present, representing every region of Africa except North Africa. Lovejoy's study of the ubiquitous experience of African slavery which has so profoundly affected the development of the continent and the lives of its people. John Laband then examines the rise of the Zulu kingdom in the early nineteenth century and its subsequent conquest by Britain, thus charting the fate of civilians during the formation of an African kingdom and their experiences during colonial conquest. The Anglo-Boer War is situated at a crucial crossroads between colonial and modern warfare, and the concentration camps the British set up for Boer and African civilians pioneered a new form of modern savagery. Bill Nasson examines this war's complex effects on various categories of non-combatants in South Africa. Because it was under colonial rule, Africa was dragged into the two World Wars. Tim Stapleton shows in the fourth chapter that while the African civilian response to the war of 1914-1918 was often contradictory and ranged from collaboration to revolt, the effect of the conflict was only to confirm colonial rule. In the following chapter, David Killingray explains how and why the impact of the Second World War on African civilians was rather different from that of the First in that it undid colonial rule, and paved the way for the future independence of Africa under modernized African leadership. The Portuguese held on to their African empire long after the other colonial empires had relinquished theirs in the 1960s. Angola, the subject of Chapter six, passed seamlessly out of an independence struggle against Portuguese rule into civil war that soon involved Cold War rivalries and interventions. Inge Brinkman describes the dismal sufferings and displacement of Angolan civilians during four decades of interminable fighting. Liberia and Sierra Leone declined from relative stability and prosperity into horrific civil war, and in Chapter seven Lansana Gberie traces the deadly consequences for civilians and the efforts to stabilize society once peace was tentatively restored. The Sudan has suffered decades of ethnic and religious strife between the government and the people of the southern and western periphery, and in Chapter eight Jane Kani Edward and Amir Idris analyze what this has meant, and still means, for the myriad civilian victims. Chapter nine concludes the book with the most horrific single episode of recent African history: the Rwandan genocide. Alhaji Bah explains its genesis and canvasses the subsequent search for reconciliation. The chapter ends with his discussion of African mechanisms that should - and even might - be put in place to ensure effective peacekeeping in Africa, and so save civilians in future from the swarm of war's horrors.
