Despite the increased use of high-fidelity simulation in the education of paramedics, little research has been done into its impact on paramedic student performance. The objective of this quasi-experimental quantitative analysis was to investigate differences in the performance of students in the paramedic field internship between those trained using low-fidelity and those trained using high-fidelity simulation practices. Data were derived from the student field internship records from a community college-based paramedic programme in the United States, with students in two groups: a low-fidelity simulation group (2010–2013) and a high-fidelity simulation group (2014–2017). The students in the high-fidelity simulation group required fewer patient contacts to complete the internship than the students in the low-fidelity simulation group, demonstrating a significant improvement in student performance. While further studies are required to more fully investigate the use of high-fidelity simulation in paramedic education, this study provides necessary insight into the impact of high-fidelity simulation in the training of paramedics.

AbstractIntroductionTraditionally, Emergency Medical Services (EMS) educators have divided the pediatric population into age groups to assist in targeting their clinical and didactic curriculum. Currently, the accrediting body for paramedic training programs requires student exposure to pediatric patients based entirely on age without specifying exposure to specific pathologies within each age stratification. Identifying which pathologies are most common within the different pediatric age groups would allow educators to design curriculum targeting the most prevalent pathologies in each age group and incorporating the physiologic and psychological developmental milestones commonly seen at that age.HypothesisIt was hypothesized that there are unique clusterings of pathologies, represented by paramedic student primary impressions, that are found in different age groups which can be used to target provider education.MethodsThis is a retrospective review of prospectively collected data documented by paramedic students in the Fisdap (Field Internship Student Data Acquisition Project; Saint Paul, Minnesota USA) database over a one-year period. For the purposes of this study, pediatric patients were defined arbitrarily as those between the ages of 0-16 years. All paramedic student primary impressions recorded in Fisdap for patients aged 0-16 years were abstracted. Primary impression by age was calculated and graphed. The frequency of primary impression was then assessed for significance of trend by age with an alpha ≤.05 considered significant.ResultsThe following primary impressions showed clinically and statistically significant variability in prevalence among different pediatric age groups: respiratory distress, medical-other, abdominal pain, seizure, overdose/poisoning, behavioral, and cardiac. In patients less than 13 years old, respiratory and other-medical were the most common two primary impressions and both decreased with age. In patients 5-16 years old, the prevalence of abdominal pain and behavioral/psych increased. Bimodal distributions for overdose were seen with one spike in the toddler and another in the adolescent population. Seizures were most common in the age group associated with febrile seizure. Sepsis was seen most often in the youngest patients and its prevalence decreased with age.ConclusionThere are statistically significant variations in the frequency of paramedic student primary impressions as a function of age in the pediatric population. Background: Cardiovascular disease remains the most prominent cause of death in England. Healthcare professionals have been encouraged to identify cardiovascular risk factors (CVRFs). The aim of this study was to explore how paramedics contribute to the identification of CVRFs in the pre-hospital setting, through their role, behaviours and practice.Methods: The study took place within the North East Ambulance Service NHS Foundation Trust supported by a National Institute for Health Research clinical research internship. A qualitative approach was used, and a single focus group was conducted. The study recruited participants through advertising for volunteers and purposive sampling. The themes that arose from the focus group allowed the initial exploration of the views of paramedics in relation to role, behaviour and practice in identifying CVRFs.Results: A single focus group with five paramedics was conducted in June 2021. Two central themes emerged: education/health promotion and fear/anxiety. Participants agreed that their role in this area centred around patient education. Participants’ behaviours and practice were adversely affected through fear of complaints, fear of hypocrisy and feeling a lack of support from the ambulance service. Participants felt that further training and subsequent indemnity from complaints would improve the likelihood of more direct patient education. Support from the ambulance service to improve employees’ own health and well-being was also a key topic of discussion.Conclusion: The study explored the views of a small sample of paramedics on this topic. Patient education was felt to be part of a paramedic’s role; however, barriers were identified that prevent paramedics from carrying out this role. Further research is needed to explore these barriers further.

Objective This study compares the results of an objective structured clinical examination (OSCE) between 2 groups of students before an internship and after 6 months of clinical practice in an internship. Methods Seventy-two students participated, with 36 students in each cohort. The OSCEs were performed in the simulation laboratory before the participants' clinical practice internship and after 6 months of the internship. Students were tested in 9 stations for clinical skills and knowledge. The same procedures were repeated for both cohorts. The t test was used for unpaired parametric samples and Fisher's exact test was used for comparison of proportions. Results There was no difference in the mean final score between the 2 groups (p = .34 for test 1; p = .08 for test 2). The performance of the students in group 1 was not significantly different when performed before and after 6 months of clinical practice, but in group 2 there was a significant decrease in the average score after 6 months of clinical practice. Conclusions There was no difference in the cumulative average score for the 2 groups before and after 6 months of clinical practice in the internship. There were differences within the cohorts, however, with a significant decrease in the average score in group 2. Issues pertaining to test standardization and student motivation for test 2 may have influenced the scores.

Paramedical training field allows the integration of practical and theoretical knowledge, the mobilization of knowledge offers the possibility to test the skills necessary for the exercise of the profession. Nursing education is organized based on the principle of alternating between theoretical learning in classrooms, and practical learning in the form of simulation sessions in laboratories and internships in healthcare institutions. Indeed, through our article we study the relationship between these two teaching/learning practical methods applied in nursing and technical health professions students’ education. Our study is of the descriptive correlational type, we carried it out through a survey that we conducted with a sample of 289 students, using a questionnaire developed via the Google Forms platform; the data we collected were analyzed using the descriptive statistics method, in frequencies and percentages, presented in the form of figures and simple and bi-variate (Contingency) tables. The main results of this study showed that (a) 59.17% were moderately satisfied with the alternation between simulation sessions and internships; (b) 48% stated that simulation as a method of learning sometimes reflects the reality of internships; (c) 52% announced that it is often possible to transfer the learning acquired through simulation into practical gestures during clinical internships, and (d) 90.66% stated that simulation learning could not replace internships. As a result, this study has concluded that nursing training is based on practical teaching / learning which materializes in simulation activities and clinical internships which are strongly correlated.

Abstract Introduction Paramedics are essential health workers, and are regularly exposed to shift work, which negatively impacts sleep and fatigue. Paramedics also experience adverse mental health outcomes and sleep disorders at higher rates than the general population. Consequently, retention and occupational burnout issues are common. However, the relationship between shift work, sleep disorders, fatigue, and mental wellbeing in paramedics is poorly understood, limiting our ability to design and implement effective interventions. Method This study uses a qualitative action research design involving one-to-one interviews, with an intended sample of 30 paramedics (completion Aug 2022). The Scenario Invention Task Technique is used, requiring workers to explore how they engage and interact with their work. Participants generate challenging scenarios specific to their work when feeling rested, and then consider the same scenario when feeling extremely fatigued. Data are analysed using an inductive thematic structure. Results Analysis of preliminary data (n=10) indicates sleep loss and fatigue negatively impact workers’ ability to manage challenging scenarios and the self-regulation of emotions. While participants know sleep problems impact their wellbeing, limited knowledge on identification and management of sleep disorders is apparent. Early findings suggest an optimal window for better sleep education would be during internship, and paramedics suggest the information needs to be provided by an expert, and trusted, voice. Discussion Study findings will help inform sleep education, screening and support toolkits about sleep, fatigue and shift work tailored to paramedics in order to support wellbeing and sustainable careers.

<p class="abstract"><strong><span lang="EN-US">Background: </span></strong>In India internship training is considered to be a period of learning entrusted with clinical responsibilities. This study helps to know the self-reported preparedness of fresh graduates for internship as well as trained interns to be a practicising primary care doctor.</p><p class="abstract"><strong><span lang="EN-US">Methods: </span></strong>Cross sectional study was done in a tertiary care teaching hospital of South India. Thirty five interns who completed training and thirty five fresh graduates entering internship were randomly recruited for the study after getting ethics committee approval. Data was collected in a structured questionnaire which assessed the confidence levels of the participants in performing various skilled activities. The feedback regarding training and suggestions were also received. The data was analysed by SPSS 16. Both the groups were considered to be equally competent and hence non parametric test for 2 independent samples Mann Whitney U, Chi square test and Fisher’s exact test was done. </p><p class="abstract"><strong><span lang="EN-US">Results:</span></strong><span lang="EN-US"> Apart from delivering health talks in local bodies and prescribing oral contraceptives there was significant difference (p&lt;0.05) in the confidence levels of interns who completed training. Total scores of trained interns were greater than that of fresh graduates (p&lt;0.001). 97.14 % participants wanted cardiology as a compulsory area of training for internship. Suggestions for improvement include orientation classes, exact procedure demonstrations, more seminars and active involvement of interns in management of the patients.</span></p><p class="abstract"><strong><span lang="EN-US">Conclusions: </span></strong>Internship training is effective in this tertiary care hospital and trained interns make better doctors than fresh doctors as the theoretical knowledge gained is put into practice.</p>

This study aimed to identify the factors that influence Debre Markos University bachelor degree graduates’ transition duration from higher education to employment. A retrospective study design and a total of 1448 undergraduates were considered in this study. Kaplan-Meier, Cox proportional hazard, and parametric accelerated failure time survival analysis were used to identify the factors that influence the duration of the job search of graduates. The best parametric model that could describe the duration of the job search to obtain the first job was chosen using the Akaike information criterion. As per the study, 50% of graduates were able to get their first employment 18 months after graduation. The Weibull parametric accelerated failure time model performed better in predicting the time it takes for graduates to find their first job. College, cumulative grade point average, father education, graduate region, public advertisement, starting own business, and doing an internship during the study were found to be statistically significant indicators of the transition duration of the graduates to obtain the first job. Debre Markos University 2019 graduates had a longer period of unemployment. The institution should have a fully functioning career service office that provides job search training and connects students with employers.

Innovative work behavior shown by employee activities that generate ideas, promote ideas, and implement ideas is very useful for organizational sustainability. This research seeks to improve innovative work behavior by analyzing the contribution of self-efficacy and organizational climate to ionovative work behavior. The research was conducted on non-permanent instructors at PT PLN (Persero) Pusdiklat with samples 149. Hypothesis testing uses parametric statistical analysis and RCA (Root Cause Analysis) method. The findings of this research: (1) If self-efficacy increases, innovative work behavior will increase; (2) if the organizational climate increases, innovative work behavior will increase; (3) If self-efficacy and organizational climate increase together, innovative work behavior will increase. Based on the RCA (Root Cause Analysis) method, several priority indicators were found to improve innovative work behavior, namely: forming an innovation team, assigning internships / benchmarks, budgeting for awards, K3L training, creating Knowledge Capturing books, recruiting K3L HR, and book review.

In this study, it was aimed to examine the relationship between physical education and sports teacher candidates’ academic life satisfaction and their beliefs in the teaching profession. The population of the research consisted of 1482 physical education and sports teacher candidates studying in the Mediterranean region. The sample group consisted of 318 teacher candidates who participated in the study in this universe. Volunteers were asked to fill in the personal information form, the academic life satisfaction and professional belief scales. It was decided to apply non-parametric tests in the statistical analysis of the data obtained. Spearman correlation analysis was applied to reveal the relationships between the scores obtained from the scales. As a result; it was determined that the academic life satisfaction levels of the teacher candidates were above the average, and the average of the scores they got from the total professional belief scale and the sub-headings of the scale were found to be high. It was found that there is a moderate relationship between academic life satisfaction and professional beliefs. It is thought that the results of the present study are due to the fact that physical education and sports teacher candidates learn the sportive knowledge and skills that may be necessary in their future profession, theoretically and practically, at the highest efficiency level in the undergraduate programs they study, and that they can actively apply this knowledge and skills in the field with different internship applications.

In March 2020 I was in the Ulm University Clinic for 2 weeks. My internship course took place on the basis of the department of general and visceral surgery. The work in the clinic is structured in such a way that not only surgeons but also doctors of the adjoining specialties, trainees and students participate in the discussion of patients’ treatment. And at the morning conferences, a resuscitator, a radiologist and an endoscopist must always be present. The operating unit has all necessary equipment in sufficient quantity. Preoperative preparation is carried out by the anesthetic team in the preoperative room. A special role is given to the patient’s thermal isolation using special blankets, protection of the patient’s eyes with a patch, and perioperative antibiotic prophylaxis. In addition to the operating surgeon and two main assistants, the operating team obligatorily includes a student. Continuous training by senior surgeons of junior ones is practiced. Basic surgical instruments are represented with everything you need. In the postoperative period all drugs are charged into infusion machines at a daily dosage, which simplifies the work of paramedical personnel and also reduces the risk of catheter infection. When entering a medical university the competition is initially very high (more than 20 people per place). All doctors work in a unified team, there is no strict division into university chairs and clinic departments. Two weeks spent in the Ulm University Clinic have shaped my understanding of medical care and medical education in Germany, acquainted me with the specificity of the surgical service and the technical features of performing surgical interventions.

IntroductionBurn-out is quite common in hospitals especially among young doctors. It results from a mismatch between expectation and professional reality.ObjectivesTo determine the prevalence of sever burnout and to identify its associated socio-demographic and psychological factors among young residents.MethodsAnalytical and descriptive cross-sectional study conducted among residents and interns working at the Hedi Chaker and Hbib Bourguiba University hospital in Sfax, Tunisia, during the month of July 2019. The characteristics of the participants were collected using a questionnaire. Burnout was evaluated using the Maslach Burnout Inventory(MBI) differentiating 3 components: emotional exhaustion, depersonalization and luck of personal achievement.ResultsOut of 85 questionnaires disturbed,60 were selected corresponding to a response rate of 72.94%. The sex ratio (M/F) was 0.87. The middle age was 28.22. Forty three percent of the participants were married. More than half consumed tobacco and 45% of them consumed alcohol. The majority of doctors were residents(81.7%). The average working time was 55 hours per week. Burn-out was severe in 30% of our population. Furthermore, doctors who suffered from physical aggression(p=0.001) were more likely to develop severe burn-out. The dissatisfaction with the internship (p=0.01) and the feeling of do not satisfy seniors(p=0.02) were statistically associated with severe burnout. Severe burn-out was associated with anxiety (p=0.0073), conflictual partnership (p=0.0001),conflicts with colleagues(p=0.001) and the paramedical framework (p=0.0001)ConclusionsThe risk of burn-out is quite high among young doctors. Some factors seem to be associated with this phenomenon. Introduction. The paper presents the results of a survey of 42 experienced workers of a chemical enterprise exposed to vinyl chloride (VC). The purpose of the study is to establish the characteristics of changes in some biochemical indices in workers exposed to vinyl chloride, depending on the exposure load. Material and methods. A neurological, general therapeutic, ultrasound examination, biochemical studies with the determination of lipid profile, lipid peroxidation, and antioxidant protection, neurotransmitters was executed. The toxic exposure load (TEL) for the entire period of internship has been determined. Non-parametric methods of statistical analysis with the determination of the Mann-Whitney U-test were used. Results. Clinical manifestations of employees working in contact with VC included asthenic disorder with cognitive impairment and autonomic dysfunction syndrome, arterial hypertension, and gastrointestinal diseases. The prevalence of cognitive impairments and hypertension has been established to increases with an extremely high level of exposure load. In individuals with an extremely high level of exposure, higher concentrations of LDL cholesterol, atherogenic index (AI) and serotonin levels, and a decrease in the level of ceruloplasmin were noted. An ultrasound examination of the liver revealed an increase in the anteroposterior size of the right lobe in the group of experienced workers who were in contact with VC, compared with the control (p = 0.03). Discussion. Among the indices of lipid metabolism, pro-atherogenic cholesterol fractions are a more sensitive marker, reflecting the degree of exposure to VC. A decrease in the concentration of ceruloplasmin is probably associated with a weakening of the antioxidant defense mechanisms with an increase in the level of TEL. Increased serotonin levels in individuals with extremely high levels of TEL, confirms the prevalence of cognitive impairment in this group. In the group of individuals with extremely high levels of TEL, a statistically significant increase in LDL cholesterol, IA, a decrease in ceruloplasmin concentration, an increase in serotonin serum levels due to impaired metabolic and protective functions of the liver as a result of prolonged contact with VC were noted.

IntroductionInternationally, emergency ambulance paramedic education has evolved from ‘on the job’ training by ambulance services to tertiary-level qualifications. However, across many countries and jurisdictions, clinical practicum requirements still differ substantially. For example, Australian paramedic students spend fewer hours on clinical practicum than their United Kingdom (UK) counterparts.MethodsFifteen semi-structured face-to-face interviews were conducted with Australian and UK paramedics and managers to identify their perceptions regarding the preparedness of graduate paramedics for practice. ResultsInterviewees in Australia and the UK identified that newly graduated paramedics require an internship period before independent practice. Interviewees in both jurisdictions recognised the robust theoretical knowledge and practical skills of graduates. Organisational operations, clinical judgement and decision-making, and care-pathway selection were areas where interviewees identified that an internship period consolidated the graduate’s learning. ConclusionThis research found that increased time spent by paramedic students on ambulances did not increase a new graduate’s ability to practice independently before an internship period. The time spent on ambulance practicums, objectives and supervisory model should be reviewed.

IntroductionIf a paramedic moves interstate in Australia, they are often required to undertake a local state-specific internship in the state or territory they are moving to. The small differences in roles undertaken between the states and territories are not sufficient to explain this requirement. In order to better understand this expectation, the authors explored the national and organisational cultural factors that could explain its purpose. MethodsSemi-structured interviews were conducted with six major stakeholders of an internship, and interpreted using organisational and national-cultural dimensions derived from the work of Hofstede, Waisfisz and Minkov.ResultsSeven core themes were identified, layered according to the stakeholders that have the greatest influence over the theme, and discussed using the study’s interpretive framework (cultural dimensions). These core themes describe the concerns the respondents had about elements of the paramedic internship, including the culture of the internship, attitudes, education, communication, the profession, the organisation, and recruitment and selection. ConclusionThe insights gained into the cultural characteristics of the ambulance organisation help us to better understand the need for the internship. From a cultural perspective, paramedics need to be able to balance risk aversion (policy/procedure) with risk taking (unpredictable work demands); structure (militaristic) with independence (autonomy); a hierarchy-based power relationship and a competency-based power relationship and, finally, indulgence (socialisation for organisational acceptance) with restraint (becoming a professional). This balancing of judgement and actions is extremely complex, delicate and organisation-specific, thus explaining the importance for the individual organisation of having a prolonged observation period during which ‘a feel’ for these ‘balances’ can be obtained to ensure that the paramedic ‘fits’ into the specific organisation to become ‘one of us’.

IntroductionThe purpose of this study was to determine if the opportunity to execute advanced skills, including intravenous (IV) placement, intubations and medication administration was associated with preceptor/intern gender combination.Methods16,466 emergency calls were analysed from the Field Internship Student Data Acquisition Project. The analysis focussed on emergency calls requiring IV placement, intubation and/or medication administration. A chi-square test for independence was used to compare three advanced skills executed based on the preceptor/intern gender combination.ResultsMale interns were more likely to execute IV placement regardless of the preceptor gender when compared to female preceptor and female intern combination (p<0.001). When male preceptors were paired with female interns, the number of IV placements executed was significantly lower only when compared to a male preceptor and male intern combination. Female preceptor and female intern combination resulted in a reduced number of times endotracheal intubation was executed only when compared to female preceptor and male intern combination (p=0.017). There were no differences in any combination when executing medication administration.ConclusionThese findings suggest female interns may experience bias in executing fewer intubation and IV placement skills than their male counterparts on their first ambulance call of their internship.

IntroductionThis study explores the career intentions of undergraduate university paramedic students. Other like studies have been completed before paramedicine registration, and this is the first career intentions study to focus on the Queensland perspective. Methods Second year Queensland University of Technology (QUT) undergraduate paramedic students (n=182) were invited to participate. Ethics approval was gained and consent to participate was implied by the completion of a mixed methods questionnaire developed from the extant literature. Data were coded and analysed using Stata 15.1 and NVIVO 12. ResultsThe response rate was 47.3% (n=86). Findings indicated 82.35% of participants intend to pursue paramedic employment after graduating. Queensland Ambulance Service is the preferred choice of employer (78.05%), however 67.07% intended to apply to multiple services for employment, 62.2% are considering private industry employment and 59.76% are contemplating overseas work. Intended paramedicine career length revealed 39.5% of participants plan to work for 20 years or more, 29% for 6–10 years and 18.4% for 11–15 years. Qualitative findings suggest students are concerned about employment opportunities, dislocation from support networks, and feeling unprepared to work in other paramedic jurisdictions. Furthermore, the employer’s reputation, culture, internship program and career development opportunities impact on employment decisions. ConclusionThe paramedic landscape in Australia appears to be changing. Although the majority of participants intend to work for jurisdictional ambulance services, students are exploring a range of employment opportunities. Understanding the career intentions of undergraduate paramedic students is essential for universities when preparing students for the transition to a globally evolving profession.

PurposeThis paper investigates areas for improvement in internship practices from the perspectives of key stakeholders, such as university department leaders, host company leaders, lecturers, work supervisors, graduates, and final year students. Student choices of internship practices are also reported.Design/methodology/approachAn exploratory sequential mixed methods approach was implementing that included three focus groups, 15 individual in-depth interviews, and 461 responses to a student survey. In the qualitative phase, deductive thematic analysis was employed to explore areas for improvement in internship practices. In the quantitative phase, descriptive statistical analysis, and two non-parametric tests were used: the Mann–Whitney tests and Kruskal–Wallis tests, followed by pairwise comparisons to identify student choices of internship practices.FindingsThe corroboration and triangulation of the qualitative and quantitative data sets revealed three distinct areas for improvement in internship practices in Vietnamese universities. These are internship learning outcomes, internship support, and internship assessment. Findings highlighted the crucial role of industry stakeholders, including work supervisors in the entire process of the internship, as well as the key responsibility of universities in improving student internship experiences.Originality/valueAreas for improvement and student choices of internship practices in Vietnamese universities have not been discussed previously. Findings carry practical, policy and theoretical implications for higher education in Vietnam and other countries striving to enhance student internship experiences. Hence, this study contributes to the Vietnamese and international WIL literature with its findings emerging from a complex mixed-methods design. This methodological approach offers enhanced reliability and validity of findings compared to previous research in the field that relied on a single data set.

Research Question: This study aims to determine students’ satisfaction, experience and attitude toward internship in hospitality higher education. Motivation: Work integrated learning in the system of vocational higher education should enable young people, who have acquired theoretical and practical knowledge, to increase their employability, develop expertise through the process of socialization in the workplace, as well as to develop work habits and have a better understanding of the work culture. According to Hussien and La Lopa (2018), well-organized internship with detailed feedback, flexible working hours, adequate institutional support, and the ability to acquire a diverse range of skills, can increase satisfaction of all stakeholders. Idea: The main idea is to examine students’ experience, attitudes and satisfaction with internship, in order to propose practical implications for internship improvement. Data: The data were collected by means of a questionnaire during the summer term of 2017/18 academic year. The survey involved 189 students of The College of Hotel Management. Tools: Collected data were analysed and interpreted using some descriptive statistics techniques, as well as non-parametric tests such as Mann-Whitney U Test, Kruskal-Wallis H Test and Spearman's Rho. Findings: A vast majority of students were given employment opportunities during internship, which has increased their satisfaction. Gastronomy and hospitality students most often worked in the profession, while junior and senior students find the curriculum to a lesser extent consistent with the internship program. Students were most satisfied with the mentor’s objectivity in the assessment, and least satisfied with the school support during internship. Student satisfaction is also affected by better alignment of theory and practice, adequately rewarding overtime and doing the job for which they have been trained. Contribution: This study contributes to the identification and understanding of factors affecting satisfaction of hospitality students with internship and proposes measures to better internship organization.

Current system of medical education in Ukraine needs improvement and reforms in order to enhance the proficiency of doctors and paramedics. Training of practical/technical skills, communication, as well as teamwork skills is considerably important.The use of simulation techniques and methods in medical education is called simulation training in medicine. Medical skills are acquired through cognitive (knowledge) and psychomotor (practice) skills. The first medical simulation centers appeared in Ukraine in 2006 according to the order of the Ministry of Health Care of Ukraine.On June 20, 2013, a new simulation training center was opened in Ivano-Frankivsk on the base of Regional Perinatal Center. Similar medical simulation centers were opened in the second half of 2013 in Volyn and Vinnytsia regions under the Ukrainian-Swiss Mother and Child Health Programme, which started in the area of perinatology. Their goal is to improve the teamwork of all specialists involved in the process of delivery and neonatal intensive care,as well as to engage internship doctors and senior medical students in clinical skills training programmes.The use of simulation techniques and training programs offers a powerful platform to study and practice clinical reasoning behaviors and patterns.

Background and Objectives: Learning styles can show changes in how a learner receives and interacts with the environment. Since teaching and improving the current situation for students to learn in clinic settings is one of the tasks of medical education centers, the present study aims to determine the learning styles in clinical education among students of Qom University of Medical Sciences (QUMS). Methods: This descriptive cross-sectional study was conducted in 2020 on 308 students of QUMS from faculties of Medicine, Nursing, Midwifery and Paramedical Sciences who were selected by a stratified random sampling method in two groups of apprenticeship and internship. The data collection tool was the Kolb Learning Styles Inventory. Data were analyzed using descriptive statistics and chi-square test. Results: The mean age of students was 23.7±4.3 years; 54.9% were female and 45.1% were male; 64% were undergraduate students and 36% were doctoral students. It was found that 48.1% of students had an assimilating learning style, 24.7% had a diverging learning style, 17.5% had a converging learning style and 9.7% had an accommodating learning style. There was a significant difference between learning styles in terms of the field of study (P=0.03), but not in terms of gender, educational level, marital status and academic year (first-year and final-year students). Conclusion: Most of students in QUMS have assimilating and diverging learning styles; therefore, providing learning environments appropriate to these styles is important for their better learning and gaining the necessary capabilities in future educational and professional environments.

In the present study, carried out during the internship in Surgical Pathology of Santo Spirito Hospital in Casale Monferrato, a score based on the quantification of mismatch repair proteins (MMR) in patients with colorectal adenocarcinoma was devised, to determine whether a different expression in these proteins could correlate with some morphological parameters, and therefore whether it was possible to define a prognostic stratification for these patients. Objective During this period, 73 cases of colorectal adenocarcinoma referred from 01/01/2020 to 31/03/2021 were analyzed. The cases were investigated by immunohistochemical examination to assess the expression of mismatch repair proteins. Subsequently, the immunohistochemical preparations were observed by light microscopy, evaluating a total percentage of expression in the tumor tissue, in order to perform a correlation study between the expression of mismatch repair proteins and some morphological characteristics of the tumor at diagnosis. The total expression of MLH1 protein, as representative of the MLH1/PMS2 heterodimer, and of MSH6 protein, as representative of the MSH2/MSH6 heterodimer, was quantified. Methodology Evaluation of microsatellite instability was obtained by immunohistochemical investigation using ROCHE's automated VENTANA Bench-Mark Ultra platform. Correlation study, between tumor morphological data at diagnosis and expression of mismatch repair proteins, was performed by use of parametric and nonparametric tests.Results Total expression of MSH6 protein was higher than MLH1 expression in 60 of 73 cases. MSH6 protein is more frequently associated with high expression (from 75% to 100%) than MLH1, which is characterized by a lower expression range. The correlation test of MLH1 expression per- centage with perineural invasion gave a statistically significant results, with a p-value below the limit of significance (p=0.007191). In the case of MSH6, statistically significant p-values were shown for tumor budding (p=0.02577), perineural invasion (p=0.007498) and growth margin (p=0.04664). Conclusions Quantifying MMR protein expression in MSS tumors could be a valuable aid in therapeutic choice, as well as in prognostic stratification of MSS patients between subjects with high expression, and thus less aggressive tumors, and subjects with low expression lesions of MLH1 and/or MSH6, and thus more biologically aggressive tumors. If a different expression in mismatch repair proteins correlates with morphological characteristics of greater or lesser malignancy, it could be hypothesized that patients with MSS colorectal cancer, treated with the same therapy, may have a greater or lesser response, depending on the percentage of expression of mismatch repair proteins, estimating a more effective adjuvant therapeutic response in subjects with high expression.
