Acute kidney injury (AKI), regardless of its aetiology, can elicit persistent or permanent kidney tissue changes that are associated with progression to end-stage renal disease and a greater risk of chronic kidney disease (CKD). In other cases, AKI may result in complete repair and restoration of normal kidney function. The pathophysiological mechanisms of renal injury and repair include vascular, tubular, and inflammatory factors. The initial injury phase is characterized by rarefaction of peritubular vessels and engagement of the immune response via Toll-like receptor binding, activation of macrophages, dendritic cells, natural killer cells, and T and B lymphocytes. During the recovery phase, cell adhesion molecules as well as cytokines and chemokines may be instrumental by directing the migration, differentiation, and proliferation of renal epithelial cells; recent data also suggest a critical role of M2 macrophage and regulatory T cell in the recovery period. Other processes contributing to renal regeneration include renal stem cells and the expression of growth hormones and trophic factors. Subtle deviations in the normal repair process can lead to maladaptive fibrotic kidney disease. Further elucidation of these mechanisms will help discover new therapeutic interventions aimed at limiting the extent of AKI and halting its progression to CKD or ESRD.

Axillary nerve injury has been associated with sports injuries, especially those involving anterior shoulder dislocation. The nerve injury leads to weakness of the deltoid and teres minor muscles, which impairs abduction and external rotation of the arm at the shoulder. Electrodiagnostic studies are helpful for determining extent of reinnervation and recovery after injury. In the absence of clinical or electrodiagnostic signs of recovery 3 to 6 months after injury, it is appropriate to offer surgical exploration. The options for surgical repair include direct nerve repair, nerve grafting, and nerve transfer. In appropriately selected individuals, outcomes are favorable.

Injuries to peripheral nerves must be assessed and treated in a thorough and timely manner to achieve optimal outcomes. Physical examination is the cornerstone in diagnosing acute nerve injuries and includes careful inspection as well as precise motor and sensory testing. Nerve conduction studies and electromyography are often more useful in the setting of delayed presentation. Microsurgical repair techniques differ for clean versus ragged lacerations, and resultant nerve gaps will require a conduit or graft to achieve the necessary tension-free closure. The surgeon and patient should be prepared for a lengthy postoperative course and possible complications as the nerve regenerates and function returns.

This chapter focuses on the clinical and surgical management of facial nerve palsy that occurs as a consequence of injury during resection of a vestibular schwannoma. If the facial nerve is damaged during cerebellopontine angle (CPA) tumor resection, a first attempt to repair it at the skull base should be made. Because this is commonly infeasible, a nerve transfer—scheduled as an elective procedure after the patient has completely recovered from the resection procedure—is mandatory. Hemihypoglossal, masseter, and cross-facial nerve transfers are the techniques most widely used. The authors’ preferred technique is hemihypoglossal nerve transfer, and the surgical technique is described. By contrast, when the facial nerve is preserved during surgery, but complete facial palsy develops afterward, postoperative rehabilitation should be started and continued for up to 1 year. If, however, facial palsy persists beyond 1 year, then the patient should be offered the option of a nerve transfer.

Penetrating eye injury in the pediatric patient is an emergency that requires careful management by the anesthesiologist to prevent further eye damage by the potential extrusion of vitreous fluid. Additionally, since these injuries are due to trauma, the patients typically are not adequately fasted for surgery. Thus prevention of aspiration of gastric contents must also be considered during the perioperative management of the child with an open globe injury. Depending on the risk of aspiration, the anesthesiologist must determine whether to use succinylcholine, a nondepoloarizing muscle relaxant, or no muscle relaxant at all, prior to laryngoscopy. A delicate balance must exist to achieve a smooth induction of anesthesia, blunted response to airway stimulation during laryngoscopy and intubation, and prevention of aspiration. During the perioperative period, the pediatric anesthesiologist can employ a variety of methods to limit the potential for increased intraocular pressure in the patient with penetrating eye trauma.
