This article explores how visible constructions and perceptions of sovereignty in the motion pictures of the United States Information Agency (USIA) factored into the dynamics of US Cold War foreign policy amidst the rise of the Non-Aligned Movement. Specifically, it focuses on agency films about and circulating within the Southwest Asian and North African (SWANA) region – such as the locally produced Iraq al-Youm newsreels (c.1956–58). By mapping the different policy contexts of the Eisenhower and Kennedy administrations onto the USIA films’ aesthetics and themes, the article illustrates continuities in the United States’s attempts to expressively leverage images and evocations of sovereignty to sell and consolidate its policy interests in the region.

Arterial blood pressure (BP) measurements, which include invasive direct methods and noninvasive indirect methods, provide a picture of the hemodynamic status of the patient. Invasive BP methods measure pressure pulse wave amplitude; noninvasive methods rely on blood flow or arterial wall motion as a basis for the determination of BP values. To obtain the most accurate BP value, the clinician must identify which measurement variables in a specific clinical situation are most contributory to error and, if possible, use a method of measurement for which the sources of error are not parallel. Blood pressure values obtained by different methods cannot be compared without a thorough understanding of the user-related and instrumentation-related limitations associated with each BP measurement technique

AbstractDeviations from the Hubble flow directly probe of the underlying total mass distribution, assuming the gravitational instability picture. We discuss the origin of motion of the Local Group with respect to the Cosmic Background Radiation and review the peculiar velocity field deduced from distances to hundreds of elliptical and spiral galaxies, including new results for the Shapley Supercluster. Bulk-flow solutions which are free of Malmquistbias are presented, indicating coherence length larger than that expected from the optical and IRAS dipoles or from Cold Dark Matter models.

Abstract For 8 years, EDTA-dependent pseudothrombocytopenia was observed in a 55-year-old woman with a history of rheumatoid arthritis who had undergone surgery for lymphoepithelial thymoma 11 years earlier. The clinical picture was characterized by the presence of platelet clumps and antiplatelet antibodies of the IgM class. With the recent appearance of a solitary extramedullary plasmocytoma in the right retrobulbar region and the detection of an IgGλ monoclonal gammopathy, blood examination also revealed erythrocyte agglutinates alongside the platelet clumps and the presence of a cold IgG antibody with anti-l specificity. Both phenomena were observed in vitro when the sample temperature declined to 20°C to 25°C, but not at 37°C. While the EDTA-dependent antiplatelet antibodies did not appear to be chronologically correlated with the patient’s diseases, the cold antierythrocyte autoantibodies were strictly related to the plasmocytoma and the IgGλ monoclonal component in serum. To our knowledge, this is the first description of an association between EDTA-dependent platelet and erythrocyte agglutinates, with a clinical picture of pseudothrombocytopenia and pseudoerythrocytopenia due to cold agglutinins.

Abstract During the Cold War, international film festivals proliferated on both sides of the Iron Curtain. The United States and the Soviet Union recognized these festivals as important venues for “cinematic diplomacy” and the pursuit of broader foreign policy goals. This article explores how the U.S. government, together with the U.S. motion picture industry, made use of its participation in the Moscow and Karlovy Vary International Film Festivals in the 1950s and 1960s. It confirms many of the findings of earlier studies of Cold War cultural diplomacy but also expands our historical understanding of this phenomenon. Specifically, it reveals the extent of cooperation and conflict—as well as an interchangeability of roles—among public officials in Washington and private citizens in Hollywood, with implications for both the formulation at home and reception abroad of U.S. cinematic diplomacy.

Background: The optimal cardioplegic solution is still debated. The objective of this study was to compare the effect of cold crystalloid versus warm blood cardioplegia on the myocardial injury during coronary artery bypass grafting. Methods: The study included 34 consecutive patients who underwent elective primary on-pump isolated coronary artery bypass grafting from 2016 to 2019. We randomly assigned the patients into two groups. Group (ICCC) (n= 17) received intermittent antegrade cold crystalloid cardioplegia and Group (IWBC) (n= 17) received intermittent antegrade warm blood cardioplegia. Results: There was no difference in the preoperative and operative variables between groups. The time taken by the heart to regain normal sinus rhythm was significantly longer in the cold crystalloid group (7.06 ± 1.8 vs. 2.17 ± 0.8 minutes, p<0.001) with a higher rate of reperfusion ventricular arrhythmia (35% versus 6%; p=0.03) compared to the warm blood cardioplegia group. Both coronary sinus acid production and lactate level were significantly higher in the warm blood group than in the cold crystalloid group (p< 0.001 and 0.043, respectively). The ischemic ECG changes and the severity of new segmental wall motion abnormalities were non-significantly different between both groups (p= 0.68 and 0.67, respectively). Postoperative CK-MB and cTnI levels in all-time points were not significantly different between groups (p= 0.46 and 0.37, respectively). ICU (2.29 ± 0.77 vs. 2.41 ± 0.87 days, p= 0.68) and hospital stay (9.28 ± 0.76 vs. 9.42 ± 0.88 days, p= 0.62) were non-significantly different between both groups. Conclusion: Intermittent antegrade cold crystalloid cardioplegia was associated with attenuated myocardial metabolism. However, it was associated with a longer time to regain normal sinus rhythm and more reperfusion ventricular arrhythmias. We did not find differences in the clinical and echocardiographic outcomes and cardiac enzymes between cold crystalloid and warm blood cardioplegia.

(1) Background: After motion sickness occurs in the ride process, this can easily cause passengers to have a poor mental state, cold sweats, nausea, and even vomiting symptoms. This study proposes to establish an association model between motion sickness level (MSL) and cerebral blood oxygen signals during a ride. (2) Methods: A riding simulation platform and the functional near-infrared spectroscopy (fNIRS) technology are utilized to monitor the cerebral blood oxygen signals of subjects in a riding simulation experiment. The subjects’ scores on the Fast Motion sickness Scale (FMS) are determined every minute during the experiment as the dependent variable to manifest the change in MSL. The Bayesian ridge regression (BRR) algorithm is applied to construct an assessment model of MSL during riding. The score of the Graybiel scale is adopted to preliminarily verify the effectiveness of the MSL evaluation model. Finally, a real vehicle test is developed, and two driving modes are selected in random road conditions to carry out a control test. (3) Results: The predicted MSL in the comfortable mode is significantly less than the MSL value in the normal mode, which is in line with expectations. (4) Conclusions: Changes in cerebral blood oxygen signals have a huge correlation with MSL. The MSL evaluation model proposed in this study has a guiding significance for the early warning and prevention of motion sickness.

The cold pressor test (CTP) as a diagnostic method of the circulatory system reactivity may be a basis for the qualification for thermal stimulation therapy. The aim of the work was a thermovisual assessment of the reaction to the Hines and Brown cold pressor test. A group of 30 healthy men in the age of 23.5 ± 0.8 years were examined. The average weight of the examinees was 78.4 ± 9.2 kg, their height 180.7 ± 5.9 cms, and BMI 23.9 ± 2.2 kg/m2. A thermovisual picture of a tested and not tested hand of all the subjects was taken before and after the cold pressor test. Under the influence of cold water the surface temperature of a tested hand has decreased in a statistically significant way by 8.3°C on average, which is 29% of the temperature before the test, whilst the temperature of an untested hand dropped by 0.67°C. The decreases of temperature were not even and there was a statistically significant difference between the dorsal and palmar side of the hand. The correlation between the changes of systolic blood pressure and the hand surface temperature before and after CTP was observed.

In the 1950s, the top American Jewish organizations chose a single man, John Stone, to represent their collective interests in Hollywood. Over the course of the decade, Stone's Motion Picture Project sought to prevent antisemitism on film and to inspire the creation of positive Jewish characters. Negotiating the cultural politics of the era, however, resulted in an increasing tendency to favor depictions of biblical Jews over contemporary American ones. In a strange twist, Stone endorsed no film with as much zeal asBen-Hur, a New Testament celebration of Jesus. By following Stone's tortuous attempts to navigate Cold War controversies, and by casting new light on the phenomenal success of biblical epics in the 1950s, this essay suggests that at the heart of postwar popular culture was a shift toward a particular discourse of liberal humanism.

Acute coronary syndrome occurring during the course of a type I hypersensitivity reaction constitutes Kounis syndrome. We report a case of a 64-year-old man who presented with a non-ST elevation myocardial infarction and peripheral blood eosinophilia. He had rhinitis and constitutional symptoms for several days prior to presentation. Blood investigations revealed moderate eosinophilia and elevated IgE levels. A cardiac MRI showed generalised oedema with a subtle wall motion abnormality in basal inferior/inferolateral wall, and subendocardial high signal on late gadolinium enhancement suggesting a localised myocardial infarction. A coronary angiogram then revealed triple vessel disease. A diagnosis of Kounis syndrome was made. Within days of starting appropriate treatment, the patient’s eosinophil count returned to normal with improvement of clinical picture.

The aim of this retrospective study was to assess objective and subjective aspects of the long-term results of finger replantations and revascularisations. Forty patients with 59 successfully replanted or revascularised fingers, who had a mean age of 38 years, were followed-up over an average of 3.5 years. The assessment included: the profile of the blood flow in the digital arteries of replanted fingers with Doppler ultrasound, active range of motion, total grip strength, pinch grip, static two-point discrimination test and Semmes–Weinstein monofilament testing. The dexterity of the hand was evaluated subjectively with the Carlsson’s questionnaire and cold intolerance with a modified McCabe’s questionnaire. Statistical analysis was performed and a statistically significant correlation was found between the Carlsson’s functional score, active range of motion and total grip strength. In 28 fingers (74%), blood flow in the digital arteries showed an undisturbed profile, while ten fingers showed mild stenosis or impaired microcirculation.

Total knee arthroplasty (TKA) is a widely accepted and successful procedure for end-stage arthritis. Nevertheless, fast-track may be compromised by many factors, such as pain, edema, and blood loss. Cryotherapy has been advocated as a safe and effective strategy to improve the postoperative results, acting on pain, edema, and blood loss. This study is a prospective randomized controlled study, involving 50 patients after primary TKA. A power analysis was performed preoperatively. Twenty-four patients were addressed to a postoperative treatment with a continuous cold flow device (Hilotherm, Hilotherm GmbH, Germany). Twenty-six patients represented the control group, treated with crushed ice packs. All the patients shared the same analgesic strategy and the same rehabilitation protocol. Pain, analgesic consumption, active knee range of motion, drain output, transfusion requirement, and total blood loss were evaluated at different follow-ups (postoperative first, third, and seventh days). The two groups were homogenous for preoperative and intraoperative features. The groups showed no statistically significant differences in all the evaluated parameters. A modest reduction of knee volume was evident after 7 days from surgery (trend). No differences in blood loss were noticed. Continuous cold flow device in the acute postoperative setting after TKA did not show superiority in reducing edema, pain, and blood loss, compared with traditional icing regimen. Thus, due to the costs, it should be reserved to selected cases.

Abstract We consider a situation in which a brightest cluster galaxy (BCG) moves in ambient hot gas in the central region of a cool core cluster of galaxies, following the study by Inoue (2014, PASJ, 66, 60). In the rest frame of the BCG, the hot gas is supposed to flow toward the BCG in parallel from a sufficiently large distance. Then, it is expected that only the gas flowing with an impact parameter less than a critical value is trapped by the gravitation field of the BCG because of the efficient radiative cooling, getting a cooling flow, and that the remaining outer gas can get over the potential well. In such a circumstance, we can draw the following picture: A boundary layer between the out-flowing gas and the trapped gas arises around the stagnation point at the back side of the BCG. Since the boundary temperature is so low as to be X-ray dim, the boundary could be observed as the cold front in X-rays. The trapped gas stagnates on the inner side of the boundary and starts in-falling toward the BCG. Since the wandering motion of the BCG is likely to have a rotational component, the Coriolis force induces a rotational motion in the in-falling flow from the stagnation place to the BCG, forming a spiral structure around the BCG. The spiraling flow turns the BCG on the up stream side of the main flow from the far outside, and another boundary layer arises that has contact discontinuity with the main hot gas flow. These pictures reproduce the observed features such as cool cores, cold fronts, and spiral structures well. It can also be resolved how the cooling flow is suppressed from what the cooling flow hypothesis predicts, without any heating mechanism.

In the diagnosis of diseases of the blood system, an important place is given to the patient's complaints, a carefully collected anamnesis, an objective examination, and, along with the use of special laboratory and instrumental studies, routine blood and urine tests. It is necessary to pay attention to the change in the color of urine, which is observed during hemolysis, cold, marching, paroxysmal nocturnal hemoglobinuria, autoimmune hemolytic anemia, porphyria and other conditions. Hematuria is characteristic of disorders of coagulation and platelet hemostasis of hereditary and acquired origin, von Willebrand disease of mild and moderate severity, overdose of anticoagulants and inhibitors of platelet hemostasis. Proteinuria is a characteristic syndrome in paraproteinemic hemoblastoses, AL-amyloidosis, Immerslund-Gresbeck disease, etc. Competent interpretation of the urine test in combination with the clinical picture is the key to a successful diagnosis.

Artificial blood pump assistance of the failing human heart can allow it to recover. Analysis of blood pump fluid flow is a useful tool for design development and thrombosis minimization. The aim of this study was to investigate fluid flow, particularly ventricular clearance rate and stagnation areas, in four different blood pump geometries and to determine the best design. The blood pumps consisted of a polyurethane ventricle, and combinations of inlet/outlet pipe angles and compression plate shapes. A video camera recorded the motion of fluid labelled with an optical tracer (Methyl Blue histological dye). A novel processing method was developed to produce colour maps of tracer concentration, experimentally calibrated. An overall picture of fluid flow in each pump geometry was generated by considering clearance curves, tracer concentration maps and inflow jet animations. Overall and local mixing coefficients are calculated for each pump. The best geometry featured straight inlet/outlet pipes and a domed compression plate. This optical tracer technique has proven convenient, economical, sensitive to low concentrations of tracer and provides instantaneous pictures of tracer distribution in a ventricle.

A key component of acute stress is a surge in vigilance that enables a prioritized processing of highly salient information to promote the organism's survival. In this study, we investigated the neural effects of acute stress on emotional picture processing. ERPs were measured during a deep encoding task, in which 40 male participants categorized 50 unpleasant and 50 neutral pictures according to arousal and valence. Before picture encoding, participants were subjected either to the Socially Evaluated Cold Pressor Test (SECPT) or to a warm water control procedure. The exposure to the SECPT resulted in increased subjective and autonomic (heart rate and blood pressure) stress responses relative to the control condition. Viewing of unpleasant relative to neutral pictures evoked enhanced late positive potentials (LPPs) over centro-parietal scalp sites around 400 msec after picture onset. Prior exposure to acute stress selectively increased the LPPs for unpleasant pictures. Moreover, the LPP magnitude for unpleasant pictures following the SECPT was positively associated with incidental free recall performance 24 hr later. The present results suggest that acute stress sensitizes the brain for increased processing of cues in the environment, particularly priming the processing of unpleasant cues. This increased processing is related to later long-term memory performance.

The aim of this study was to determine the influence of liver milk spots on hematological, carcass and meat quality parameters in slaughter pigs. A total of 120 pigs with a live weight of approximately 115 kg and six months old were examined. Any signs of liver milk spots were recorded as present or absent according to Welfare Quality? protocol (2009). A complete blood picture was investigated. The following carcass quality parameters were measured: live, hot and cold carcass weights, dressing percentage, backfat thickness and meatiness. pH and temperature measurements were performed 45 minutes postmortem. Pork quality classes (PSE - pale, soft and exudative, normal, DFD - dark, firm and dry meat) were determined according to Adzitey and Nurul (2011) using pH45 value. Pigs with liver milk spots had significantly higher middle-sized cell count (monocytes, eosinophils, and basophils) and neutrophils count, but significantly lower red blood cell count, hemoglobin concentration, hematocrit and MCV than unaffected pigs. The same group of pigs had significantly lower live weight, hot carcass weight, cold carcass weight, dressing percentage and meatiness compared to the pigs free of milk spot lesions. Pigs showing liver milk spots had significantly higher pH45 value and incidence of DFD meat than pigs without pathological lesions in the livers. In conclusion, assessment of liver milk spots at slaughter line has potential to serve not only as an indirect measure of pig health and welfare, but also for the carcass and pork quality.

<p><b>Background:</b> Cold ischemia associated with cold static storage is an independent risk factor for primary allograft failure and survival of patients after orthotopic heart transplantation. The effects of normothermic ex vivo allograft blood perfusion on outcomes after orthotopic heart transplantation compared to cold static storage have been studied.</p><p><b>Methods:</b> In this prospective, nonrandomized, single-institutional clinical study, normothermic ex vivo allograft blood perfusion has been performed using an organ care system (OCS) (TransMedics, Andover, MA, USA). Included were consecutive adult transplantation patients who received an orthotopic heart transplantation (oHTx) without a history of any organ transplantation, in the absence of a congenital heart disorder as an underlying disease and not being in need of a combined heart-lung transplantation. Furthermore, patients with fixed pulmonary hypertension, ventilator dependency, chronic renal failure, or panel reactive antibodies >20% and positive T-cell cross-matching were excluded. Inclusion criteria for donor hearts was age of <55 years, systolic blood pressure >85 mmHg at the time of final heart assessment under moderate inotropic support, heart rate of <120 bpm at the time of explantation, and left ventricular ejection fraction >40% assessed by an transcutaneous echo/Doppler study with the absence of gross wall motion abnormalities, absence of left ventricular hypertrophy, and absence of valve abnormalities. Donor hearts which were conventionally cold stored with histidine-tryptophan-ketoglutarate solution (Custodiol; Koehler Chemie, Ansbach, Germany) constituted the control group. The primary end point was the recipients' survival at 30 days and 1 and 2 years after their heart transplantation. Secondary end points were primary and chronic allograft failure, noncardiac complications, and length of hospital stay.</p><p><b>Results:</b> Over a 2-year period (January 2006 to July 2008), 159 adult cardiac allografts were transplanted. Twenty-nine were assigned for normothermic ex vivo allograft blood perfusion and 130 for cold static storage with HTK solution. Cumulative survival rates at 30 days and 1 and 2 years were 96%, 89%, and 89%, respectively, whereas in the cold static storage group survival after oHTx was 95%, 81%, and 79%. Primary graft failure was less frequent in the recipients of an oHTx who received a donor heart which had been preserved with normothermic ex vivo allograft blood perfusion using an OCS (6.89% versus 15.3%; <i>P</i> = .20). Episodes of severe acute rejection (23% versus 17.2%; <i>P</i> = .73), as well as, cases of acute renal failure requiring haemodialysis (25.3% versus 10%; <i>P</i> = .05) were more frequent diagnosed among recipients of a donor heart which had been preserved using the cold static storage. The length of hospital stay did not differ (26 days versus 28 days; <i>P</i> = .80) in both groups.</p><p><b>Conclusions:</b> Normothermic ex vivo allograft blood perfusion in adult clinical orthotopic heart transplantation contributes to better outcomes after transplantation in regard to recipient survival, incidence of primary graft dysfunction, and incidence of acute rejection.</p>

The purpose of this publication – to familiarize the readers with the newest achievements in the field of vision machine technologies with regard to the occlusal plethysmography. The principle of plethysmograph work is similar to a principle of mechanical occlusal plethysmograph work, with that only a difference that working measuring environment (gas or liquid) is replaced with laser labels, and fixation of change of volume is made by machine vision with the subsequent computer processing. Application of laser technologies of machine vision in future considerably simplified process of diagnostics, will allow to carry out diagnostics repeatedly and under various conditions of, for example, case of external action on the examined finiteness of the of heat, cold, physical exercise, and so, is representative of more and accurate picture of the state of blood vessels in the dynamics.

This study aimed to compare vascular involvement, hand functionality, and upper limb disability between Raynaud’s phenomenon participants and controls. Also, we analyzed the relationships between vascular impairment, mobility, and strength with disability in this Raynaud population. We conducted a case–control study with fifty-seven participants. We registered sociodemographic and clinical data; vascular variables (temperature, cold test, blood flow, and oxygen saturation); functional variables (pinch strength, range of motion), and disability (Shortened Disabilities of the Arm, Shoulder and Hand Questionnaire) (Q-DASH). Raynaud participants present more disability in all Q-DASH subscales, lower hands’ temperature pre and post cold test, decreased blood flow on radial artery, decreased ranges of motions at passive extension of index finger, and active flexion and extension of thumb than the healthy controls. The multivariate regression analysis showed that extension of the index finger, lateral pinch strength, and oxygen saturation were significantly associated with disability in RP, almost the 55% of the total variance on the upper limb, 27% at sports/arts, and 42% at work. Our findings suggest that RP has a disabling effect on the upper extremities and a practice of activities in people who suffer it. Also, disability in Raynaud seems to be more related with hand mobility and strength impairment than vascular injury.

Abstract Introduction/Objective Warm autoantibodies are usually IgG-class and/or IgA-class immunoglobulins. may be classified as agglutinins or hemolysins, which may be incomplete or complete, depending on in vitro serology; they almost always bind complement. Autoimmune hemolytic anemia with only warm IgM autoantibodies is extremely rare. Methods A 91-year-old Caucasian male with hypertension presented with non-productive cough for two weeks, associated with shortness of breath and fatigue. He has a paste history of medical history of high degree AV block with sick sinus syndrome and bradycardia. Results He had blood pressure of 111/53 and heart rate of 50. Laboratory investigations showed leukocytes count of 7, Hemoglobin level of 5.6 g/dl, hematocrit level of 15.5%, platelets count of 267, total bilirubin of 5.6 mg/dL, direct bilirubin of 0.7 mg/dL, and lactate dehydrogenase (LDH) of 372 U/L and IgM titer for Mycoplasma pneumoniae was 1493 units/mL (Ref &lt; 770). He was transfused four units of packed red blood cells as emergency release due to a positive antibody screen. His hemoglobin level increased from 5.6 g/dL to 7.2 g/dL then it decreased from 8.2 g/dL to 5.6 g/dL the next two days. Direct antiglobulin test was 3+ positive for C3d and Negative for IgG. He finished a 7-day course of antibiotic treatment. The results the Cold-agglutinin titer and thermal amplitude test were suggestive of an IgM warm autoantibody and a cold agglutinin of a moderate titer. The results exclude a diagnosis of cold agglutinin syndrome. Conclusion Mycoplasma pneumonia is mainly associated with cold agglutinin syndrome but it is very rare to be associated with WAIHA. WAIHA with a DAT positive for C3 only is relatively rare with an incidence ranging between 6% and 13% and can have a clinical picture ranging from mild to severe anemia. Severe symptomatic hemolysis can be treated with Rituximab, cytotoxic agents or plasmapheresis.

The subject of this article is nation branding in the culture market, namely the role cinematography plays in creating a nation brand. Sustainability and sustainable development in the film industry is conditioned by variety in cultural promotion channels. The aim of the authors is to prove that appropriately organised cinematography promotion abroad can positively influence the image of a given country. The first section deals with the relationship between media and sustainable development, as well as with theoretical definitions of branding and the nation brand. Subsequently, cinematography in the context of branding is discussed. The core of this work is a case study of film promotion abroad from the institutional perspective of the Polish Film Institute. The promotional strategy for “Cold War” (2018) directed by Paweł Pawlikowski has been analysed, as this motion picture is considered one of the biggest successes of post-1989 Polish cinematography. Factors positively influencing its popularity were analysed alongside their influence on the general perception of Polish cinema abroad. Based on the results of empirical studies, the authors present their discussion of the functional state and the role of Polish cinematography on the global circuit, as well as attempt to verify its importance in relation to sustainability.

Background and Objectives: To assess the effect of continuous cold flow (CCF) therapy on pain reduction, opioid consumption, fast recovery, less perioperative bleeding and patient satisfaction in patients undergoing a total knee arthroplasty. Materials and Methods: Patients affected by knee osteoarthritis between September 2020 and February 2022 were enrolled in this case-control study. Patients were randomly divided into two groups (n = 50, each): the study group received postoperative CCF therapy while the control group was treated by cold pack (gel ice). The CCF device is a computer-assisted therapy with continuous cold fluid, allowing a selective distribution, constant and uniform, of cold or hot on the areas to be treated. In both groups, pre- and postoperative evaluations at 6, 24, 72 h and at the fifth day were conducted using Visual Analogic Scale (VAS), opioid consumption, passive range of motion, preoperative hematocrit, total blood loss by Gross formula, transfusion requirement and patient satisfaction questionnaire. Results: One hundred patients, 52 women (52%), were included in the study. Reduction of pain, opioid consumption and increase in passive range of movement were statistically significantly demonstrated in the study group on the first and third days. Patients were satisfied with adequate postoperative pain management due to CCF therapy (p = 0.01) and they would recommend this treatment to others (p = 0.01). Conclusions: A continuous cold flow device in the acute postoperative setting after total knee arthroplasty is associated with pain reduction and improving early movement. Patients were almost satisfied with the procedure. The management of perioperative pain control could improve participation in the early rehabilitation program as demonstrated by the increase in ROM, psychological satisfaction and reduction in opioid use.

Introduction The pre-hospital environment provides significant challenges to clinicians who wish to rapidly administer warmed blood products and fluids to patients with haemorrhagic shock. Large-bore circulatory access is required with the use of devices that will successfully warm cold blood with minimal impact on flow rates. Until now, no information has been available that defines UK Helicopter Emergency Medical Services’ (HEMS) use of circulatory access and fluid warming devices, nor the recent adoption of pre-hospital blood product transfusion. Methods A survey was sent to all 22 UK HEMS asking which circulatory access devices crews have available, whether blood products are being transfused and if fluid warming devices are used as part of their resuscitations. Results All services responded. All UK HEMS use peripheral intravenous cannulae and intraosseous access. In addition, seven use central venous catheters and three use large-bore peripheral access (the Arrow Rapid Infusion Catheter®). Three services use landmark technique alone to gain central venous access, whereas four use a combination of landmark and ultrasound-guided techniques. Different sites for central venous access are used: subclavian (seven services), internal jugular (four) and femoral (four). Fourteen services carry pre-hospital blood products of which six transfuse packed red blood cells; four transfuse packed red blood cells and fresh frozen plasma; four transfuse packed red blood cells and lyophilised plasma. Eight services carry no pre-hospital blood products. Seventeen HEMS use fluid warmers; 13 use the Belmont® buddy lite™ and four use the QinFlow Warrior. Conclusion The use of a variety of policies and range of equipment has evolved across UK HEMS, demonstrating a lack of consensus on best practice. This is the first study to record a complete picture of current UK HEMS practice with regard to the use of circulatory access devices, fluid warmers and blood product administration.

Abstract Bladder cancer is likely to recur after resection. For this reason, bladder cancer survivors often undergo follow-up cystoscopy for years after treatment to look for bladder cancer recurrence. 3D modeling of the bladder could provide more reliable cystoscopic documentation by giving an overall picture of the organ and tumor positions. However, 3D reconstruction of the urinary bladder based on endoscopic images is challenging. This is due to the small field of view of the endoscope, considerable image distortion, and occlusion by urea, blood or particles. In this paper, we will demonstrate a method for the conversion of uncalibrated, monocular, endoscopic videos of the bladder into a 3D model using structure-from-motion (SfM). First of all, frames are extracted from video sequences. Distortions are then corrected in a calibration procedure. Finally, the 3D reconstruction algorithm generates a sparse surface approximation of the bladder lining based on the corrected frames. This method was tested using an endoscopic video of a phantom that mimics the rich structure of the bladder. The reconstructed 3D model covered a large part of the object, with an average reprojection error of 1.15 pixels and a relative accuracy of 99.4%.

Relevance. Neurological disorders are one the most frequent cause for developing of chronic foot inflammation. Analysis of pathological picture of chronic osteomyelitis in patients with neurogenic foot deformities, including cases of spina bifida, is not adequately addressed.Purpose of the study — to examine morpho-functional aspects of chronic osteomyelitis in patients with multicomponent foot deformities and with spina bifida.Materials and Methods. The present paper is based on the study of 30 patients with multicomponent neurological foot deformities complicated by chronic osteomyelitis who had medical history of spina bifida (myelomeningocele type). Histology was used to examine resected fragments of affected bone tissue, bone sequestration and skin adjacent to osteomyelitis area. Laser doppler flowmetry was used to study capillary cutaneous blood flow on plantar foot surface. Thermal and pain sensitivity was assessed by electric sensimeter in L4, L5, S1 dermatomas on the right and on the left sides.Results. Biopsy skin specimens harvested at osteomyelitis area demonstrated signs of parakeratosis, absence of stratum lucidum, epidermis acanthosis with 25% thickness increase (р = 0,04), 2,2 times increase of density for microvessels of the dermis (р = 0,73Е-4) and increased rate of capillary blood flow at 81,6±14,2% (р = 0,0004), fibrosis and dermis thinning at 19,1% (р = 0,03), 1,37 times increase in bulk density of perspiratory glands (р= 0,04), loss of adipose tissue and degeneration of nerve fibers in the majority of nerve stems of the dermis. Above factors were accompanied by disorders in thermal and pain sensitivity in 100% of cases and in 29% of those sensitivity was missing. Morphological picture of bone tissue in osteomyelitic area was manifested by multiple destruction cavities with pyogenic membrane, granular tissue of varying maturity, combined chronic and acute stages of the process, and by poor restorative bone formation.Conclusion. Disorders or lack of thermal and pain sensitivity in dermatomas L4, L5, S1, of safety sense and motion control, resulting chronic load on atypical foot segments, as well as patho-histological skin alterations contribute to ulcer formation and osteomyelitis in patients with spina bifida and multicomponent foot deformities. Morphological picture of foot bony tissue at osteomyelitic site indicates typical patho-morphological signs of chronic inflammation with poor restorative bone formation.

Cold atmospheric plasma (CAP) is a novel technology, which has been widely applied in biomedicine, especially in wound healing, dermatological treatment, hemostasis, and cancer treatment. In most cases, CAP treatment will interact with innumerable blood capillaries. Therefore, it is important and necessary to understand the effects of CAP treatment on endothelial cell metabolism. In this study, the metabolite profiling of plasma treatment on endothelial cells was measured by gas chromatography tandem time-of-flight mass spectrometry (GC-TOF-MS). We found that 695 signals (metabolites) were detected by GC-TOF-MS and then evaluated using orthogonal projections to latent structures discriminant analysis (OPLS-DA). All the differential metabolites were listed, and proline and xanthosine were the two of the most downregulated metabolites by plasma treatment. By comprehensive metabolic pathway analysis with the KEGG pathway, we showed that alanine, aspartate, glutamate, and purine metabolism pathways were the most significantly suppressed after gas plasma treatment in human endothelial cells. Our finding gives an overall picture of the metabolic pathways affected by plasma treatment in endothelial cells.

From a large distance tabanid flies may find their host animal by means of its shape, size, motion, odour, radiance and degree of polarization of host-reflected light. After alighting on the host, tabanids may use their mechano-, thermo-, hygro- and chemoreceptors to sense the substrate characteristics. Female tabanids prefer to attack sunlit against shady dark host animals, or dark against bright hosts for a blood meal, the exact reasons for which are unknown. Since sunlit darker surfaces are warmer than shady ones or sunlit/shady brighter surfaces, the differences in surface temperatures of dark and bright as well as sunlit and shady hosts may partly explain their different attractiveness to tabanids. We tested this observed warmth preference in field experiments, where we compared the attractiveness to tabanids ( Tabanus tergestinus ) of a warm and a cold shiny black barrel imitating dark hosts with the same optical characteristics. Using imaging polarimetry, thermography and Schlieren imaging, we measured the optical and thermal characteristics of both barrels and their small-scale models. We recorded the number of landings on these targets and measured the time periods spent on them. Our study revealed that T. tergestinus tabanid flies prefer sunlit warm shiny black targets against sunlit or shady cold ones with the same optical characteristics. These results support our new hypothesis that a blood-seeking female tabanid prefers elevated temperatures, partly because her wing muscles are more rapid and her nervous system functions better (due to faster conduction velocities and synaptic transmission of signals) in a warmer microclimate, and thus, she can avoid the parasite-repelling reactions of host animals by a prompt take-off.

Abstract The dynamics of a hole motion in a quantum antiferromagnet has been studied in the past three decades because of its relationship to models related to superconductivity in cuprates. The same problem has received significant attention because of its connection to very recent experiments of the dynamics of ultra-cold atoms in optical lattices where models of strongly correlated electrons can be simulated. In this paper we apply the diagrammatic Monte Carlo method to calculate the single-hole Green’s function in the t–J model, where the J term is linearized, in a wide range of imaginary-time with the aim to examine the polaron formation and in particular the details of the contribution of the so-called string excitations found in such recent experiments. We calculate the single-hole spectral function by analytic continuation from imaginary to real time and study the various aspects that constitute the string picture, such as, the energy–momentum dependence of the main quasiparticle peak and its residue, the internal excitations of the string which appear as multiple peaks in the spectral function as well as their momentum dependence. We find that the earlier analysis of the spectral function based on a mobile-hole connected with a string of overturn spins and the contribution of the internal string excitations as obtained from the non-crossing approximation is accurate.

Whole body cryotherapy (WBC) involves exposing individuals to extremely cold dry air (under -100 °C) for a period ranging from 1.5 to 3 minutes. The treatment is painless and is intended to produce and use physiological reaction, which is medically beneficial and effective in restoring normal body functions. Whole body cryotherapy provides various benefits such as: analgesic and anti-inflammatory effects, decreases muscle soreness, improves muscle strength and joint function, boosts blood circulation. It also causes a hormonal reaction, growth of ACTH, cortisol, beta-endorphin and testosterone. Cryotherapy is not only used as a way of treatment but also as a part of wellness and spa. WBC is used prophylactically to improve physical comfort and mental health. It is necessary to use kinesis immediately after cryotherapy to achieve its desirable affects. Kinesitherapy is a 20 minutes set of exercises performed on machines under the guidance of a physiotherapist. The purpose of kinesitherapy is the restoration of normal motion stereotypes, which will prevent overloads.

Caraway seeds are widely used as spice for flavoring and seasoning foods, like bread and salads, because of their pungent and anise like flavor and aroma. Carum carvi was utilized in folk medicine for the management of many diseases. It is useful in hypothyroidism, liver and gall bladder problems, common cold, fever, bronchitis, diarrhea, and eczema. It also relives GIT spasms, fullness feelings and relieves baby's flatulent colic. The extracts of caraway have diverse compounds, including carvone and limonene, linalool, γ-terpinene and α-pinene. One or combination of these compounds may participate in the pharmacologic effects of caraway. Aim of this study was to assess toxicity of caraway extract on female rats. Carium carvi extract submitted to chemical analysis (Phytochemical screening and Gas chromatography-mass spectrometry (GC-MS) analysis). Acute toxicity study has been performed using 24 female rats divided randomly in 4 groups (n=6 for each group) that received different doses of hydroalcoholic seed extract of caraway 1000, 3000 and ,5000 mg/kg for 14 days. On day 15, the rats were euthanized and whole blood collected to examine complete blood picture. The liver, kidney and the heart have been harvested for histopathologic study and relative organ weight changes. Caraway extract considered relatively safe on blood profile and immune system within the studied doses, as it shown a non-significant change on complete blood counts (CBCs) in a dose-dependent manner (Hb and RBCs increased slightly, while WBCs and PLTs decreased slightly) and without any change in body weight and relative weight percentage of different organs. As conclusion, hydroalcoholic extract of caraway seeds was relatively safe in rats at a dose up to 5000 mg/kg

Objective: To evaluate the efficacy and safety of perioperative cryotherapy combined with intra-articular injection of tranexamic acid (TXA) in total knee arthroplasty (TKA) and explore a new strategy of enhanced recovery after TKA. Methods: We randomly divided 200 patients into 4 groups: normal saline (10 mL) by drainage (Group A, placebo); intra-articular injection of TXA (1 g, 10 mL, Group B); normal saline (10 mL) and continuous cryotherapy postoperatively (Group C) and intra-articular injection of TXA (1 g, 10 mL) and continuous cryotherapy postoperatively (Group D). Primary outcomes were blood loss volume, postoperative pain and circumference variation. We also recorded consumption of analgesics, postoperative length of stay (p-LOS), range of motion (ROM), function score (Hospital for Special Surgery) and severe complications. Results: There were statistically significant differences in postoperative drainage volume, total blood loss, hidden blood loss, and visual analogue scale at rest and walking on postoperative day 1 (POD1), POD2, POD3, ROM (POD3, 7, discharge, postoperative month), circumference variation (POD3, 7), p-LOS, Hospital for Special Surgery score (discharge) and drop of hemoglobin on POD3 (P < .05) among 4 groups, but there were no significant differences in intraoperative blood loss, postoperative prothrombin, activated partial thromboplastin time, overall number of patients or total consumption of oxycodone and perioperative complications (e.g., incidence of surgical site infection, deep venous thrombosis, and cold injury) among them (P > .05). Conclusion: Continuous cryotherapy combined with intra-articular injection of TXA provides short-term advantages in reducing blood loss, pain, postoperative swelling, p-LOS and increasing ROM and joint function in the early postoperative period after TKA without increasing any severe complications.

Based on the continuous increase of donor risk, with a majority of organs classified as marginal, quality assessment and prediction of liver function is of utmost importance. This is also caused by the notoriously lack of effective replacement of a failing liver by a device or intensive care treatment. While various parameters of liver function and injury are well-known from clinical practice, the majority of specific tests require prolonged diagnostic time and are more difficult to assess ex situ. In addition, viability assessment of procured organs needs time, because the development of the full picture of cellular injury and the initiation of repair processes depends on metabolic active tissue and reoxygenation with full blood over several hours or days. Measuring injury during cold storage preservation is therefore unlikely to predict the viability after transplantation. In contrast, dynamic organ preservation strategies offer a great opportunity to assess organs before implantation through analysis of recirculating perfusates, bile and perfused liver tissue. Accordingly, several parameters targeting hepatocyte or cholangiocyte function or metabolism have been recently suggested as potential viability tests before organ transplantation. We summarize here a current status of respective machine perfusion tests, and report their clinical relevance.

Abstract Background: Adhesion proteins such as soluble intercellular adhesion molecule-1 (sICAM-1) may be important markers for early atherosclerosis and some other diseases. To devise optimum specimen collection procedures, we investigated the effects on serum sICAM-1 of several preanalytical factors (factors that occur before a specimen is analyzed) such as the time of the day, food ingestion, and physical and psychological stress. Methods: Three sets of experiments were conducted: (a) 30 subjects were investigated during the morning, after an overnight fast, and then after the usual breakfast and at 1200; (b) 20 subjects were studied before and after exposure to thermal stress (sauna + swimming in ice-cold water); and (c) 15 volunteers were investigated after their driving license examination and during a (stress-free) control session. Conventional methods and kits were used to determine the blood picture and serum sICAM-1. Results: All of these preanalytical factors induced a significant increase (∼10%) in the concentration of sICAM-1. Conclusion: It is advisable to consider timing, food intake, and stress when collecting specimens and analyzing data on the concentration of sICAM-1 in serum.

A ground-based model [24 h of bed rest (BR) with head-down tilt (HDT)] was used to investigate the cardiovascular deconditioning responsible for orthostatic intolerance, frequently observed after weightlessness flights. This experimental deconditioning is shown to be distinguished by an increase of mean blood pressure (P < 0.05), with increased total peripheral resistances (TPRs). Systolic tangential tension of the carotid arterial wall, cardiac output and frequency (spectral analysis), and plasma norepinephrine and epinephrine were not significantly altered, while plasma dopamine was increased (P < 0.05). Cardiovascular homeostasis was challenged before and after 24 h of BR with HDT through -40 mmHg lower body negative pressure (LBNP). Systolic tangential tension of the carotid wall was decreased, with a decrease of systolic pressure and cardiac output; increased heart rate was likely due to an increase of sympathetic drive with a decrease of vagal braking. The overall picture was not changed after 24 h of BR with HDT, except for a lack of increase of TPRs: their increase (+13.7%, P < 0.05) before was no longer observed after (-2.6%) 24 h of BR with HDT. This apparent deficiency cannot be explained. However, a heterogeneity in the response of TPR should be considered because the magnitude of the increase of blood pressure to cold pressor test was the same after 24 h of BR with HDT as it was before.(ABSTRACT TRUNCATED AT 250 WORDS)

A study was made of the nature of the movement of air mass in the space between the device that warms the room and the confining outer wall of the room. The hydrodynamic motion of the air mass was simulated on the basis of the two-dimensional in space Navier-Stokes equations and the convective heat conduction equation to find out a detailed picture of the physical processes that occur. Also for some particular cases, analytical solutions were obtained for the conjugate problem, where the movement of air is caused by its thermal expansion and the action of gravity in areas with different densities (Archimedes' forces). The simulation of more complex types of air movement with the formation of vortex flows using numerical methods and the corresponding program codes written in C++. It was found that the movement of air mass in the space between the device and the fence strongly depends not only on the temperature of the device, the wall, and the outside air. It was revealed that hydrodynamics and heat transfer are significantly influenced by two geometrical parameters: the distance between the device and the wall, and the distance from the flooring to the bottom of the device. In particular, a thin layer of the downward flow of cold air along the fence and above the floor is possible. The condition for the occurrence of such a downward flow has been found, it is determined by the ratio of the temperature of the wall, the device, and the average temperature in the room.

Introduction: In acute phase after Total Knee Arthroplasty (TKA), physiotherapy, aims at reducing pain, local edema and muscle weakness. Among treatment techniques, cryotherapy increases pain tolerance, helping in uninhibited motor recruitment of quadriceps. Intermittent muscle fiber contraction with Neuromuscular Electrical Stimulation (NMES) improves blood flow, reduces pain and decreases quadriceps arthrogenic muscle inhibition. Exercise therapy causes muscle strengthening. Effects of perioperative cold therapy have conflicting evidences. Early NMES use, post TKA for pain and swelling reduction has shown paucity of literature. Hence, this study aims to compare the effectiveness of these techniques in improving function. Method: Interventional study was conducted with 30 subjects randomly allocated into two groups (Group A- cryotherapy and Group B- NMES). Both received standardized exercise therapy. On 2nd postoperative day- pain (using Visual Analog Scale), Range Of Motion (using universal goniometer) and gait speed (using 4 metre walk test) was measured. Treatment was given for 5 days according to allocated group. Post-treatment outcome measures were taken. Results: Out of 30 participants, both groups showed statistically significant (p<0.05) improvement in all outcomes. Comparing effectiveness between the groups, NMES showed statistically significant improvement for knee flexion ROM. Other outcome measures showed no statistically significant difference. Conclusion: Rehabilitation in acute phase is essential for improving patient’s strength in long term. Along with exercise therapy, both cryotherapy and NMES showed improved function. On comparison, NMES showed better results for improvement in knee flexion ROM. Raynaud’s phenomenon (RP) is a disease that mainly affects human fingertips during cold weather. It is difficult to treat this disease using medicine, apart from keeping the body in a warm environment. In this research, conductive knitted fabrics were fabricated to help relax the vessels of the patient’s fingertips by providing proper heat, and also serving as a sensor to detect finger motion after relaxation of the blood vessels of patients. Four different structures, termed plain, purl, interlock, and rib were produced using conductive silver/PE (polyethylene) yarn and wool yarn, with a computerized flat knitting machine. The effect of knitted structure on the electro-thermal behavior, sensitivity, and stability of resistance change (∆R/R) under different tensile forces was investigated. By comprehensive comparison, the purl structure was identified as the preferred structure for the heating glove for RP patients, owing to superior electro-thermal behavior. Additionally, the purl structure had a greater capacity to detect different motions with stable resistance change. This potential electro-thermal glove could be used for functional, as well as aesthetic (fashion) purposes, and could be worn at any time and occasion with complete comfort.

Ray and proximal phalanx amputations present valid surgical options for the management of severe traumatic finger injuries. However, among these procedures, the superior one for optimal functionality and quality of life for patients still remains unknown. This retrospective cohort study compares the postoperative effects of each amputation type to provide objective evidence and to create a paradigm for clinical decision-making. A total of forty patients who had received either ray or proximal phalanx-level amputations reported on their functional outcomes using a combination of questionnaires and clinical testing. We found a decreased overall DASH score following ray amputation. Particularly, Part A and Part C of the DASH questionnaire were consistently lower compared with amputation at the proximal phalanx. Pain measurements in the affected hand were also significantly decreased during work and at rest in ray amputation patients, and they reported decreased cold sensitivity. Range of motion and grip strength were lower in ray amputations, which is an important preoperative consideration. We found no significant differences in reported health condition, evaluated according to the EQ-5D-5L, and blood circulation in the affected hand. We present an algorithm for clinical decision-making based on patients’ preferences to personalize treatment.

Summary: A 50-year-old man was admitted to the hospital with a chief complaint of long-standing cold-induced numbness in the fingers for more than a year, accompanied by persistent hand pain and fingertip ulceration for 3 months. On physical examination, radial and ulnar artery pulsation was absent in both limbs, and pale skin color, low skin temperature, and limited finger range of motion were noted. Gangrene was detected in the fingertips of the left index and middle fingers, and the right middle and ring fingers. The patient was diagnosed with bilateral arterial occlusion and vibration white finger. Upon admission, the patient underwent an intervention surgery on the right side. Subsequently, transplantation of the descending branches of the left and right lateral femoral circumflex arteries was performed to restore blood flow in the bilateral radial arteries. Additionally, debridement was conducted without shortening after the surgical procedure, the patient received anticoagulation, anti-inflammatory, and symptomatic treatment. The bilateral finger skin temperature increased by 3 °C postoperatively, and the accompanying pain and numbness were alleviated. The wound healed 1 month after surgery, and no recurrence of pain or ulcer was reported during the 1-year follow-up period. Eventually, his hand function recovered, without any impact on the vascular donor site.

Background: disorders of both endothelium-dependent and endothelium-independent vasodilation play a role in the pathogenesis of microvascular angina (MVA), the contribution of which may be different. Aim: to study the processes of endothelium-dependent vasodilation (EDV) in patients with primary MVA. Patients and Methods: an open prospective study included 60 patients (mean age 57.3±6.4 years) with MVA, confirmed clinical picture (chest pain), coronary angiography data (the absence of coronary artery stenosis), positive stress test (pain syndrome and/or ST segment depression by ≥2 mm by treadmill exercise stress test), positron emission tomography (PET) myocardial perfusion imaging (MPI) with tests (cold pressor test and with adenosine). Patients with diseases that can lead to secondary microvascular dysfunction were not included in the study. To evaluate the processes of EDV, all patients underwent the following evaluations: peripheral arterial tonometry (PAT), assessment of circulating endothelial cells (CEC), high-sensitivity C-reactive protein (hsCRP), endothelin-1 (ET-1), total antioxidant status. Adding that, the results of a cold pressor test during PET were analyzed. Results: the majority of the examined patients were women — 81.7%. Most of the female patients included in the study (93.9%) were postmenopausal. In all patients with MVA, according to the data of PET MPI with cold pressor test, signs of EDV disorder were detected as a decrease in coronary blood flow through the three coronary arteries. During PET, the index of reactive hyperemia was reduced (<1,67) in all examined patients and amounted to 1.43±0.15. According to the results of laboratory studies, there was an increase in the level of ET-1 and CEC — 3,335 [1,545; 3,952] fmol/L and 14±8 cells/3×105 leukocytes, respectively. The CRP level was 4,44 [1.02; 4.45] mg/L. The total antioxidant capacity was reduced in 53 (88%) patients, the average values of the indicator were 289,03±52,14 μmol/L. Conclusion: according to laboratory and instrumental studies, signs of endothelial dysfunction, namely disorders of EDV, were revealed in all examined patients with primary MVA. Abstract Wellens‘ syndrome describes an electrocardiographic pattern characterized by T–wave changes in the antero–septal leads that make this finding highly suggestive of a lesion of the proximal segment of the anterior interventricular artery. Case report. A 58–year–old patient from China with a silent medical history presented to the emergency department with angor, dizziness, asthenia, and blurred vision. The electrocardiogram shows a Wellens type A pattern, and blood chemistry tests collected in the emergency department show liver, kidney function and blood count within normal limits. High–sensitivity troponin T values stand at 34 ng/mL. The echocardiogram shows preserved biventricular systolic function in the absence of major regional wall motion abnormality. Given the symptoms, troponin values, and electrocardiographic pattern, the patient underwent coronary angiopgraphy, which showed lesion–free epicardial coronary arteries but with a muscular bridge in the mid–proximal section of the anterior interventricular artery, with no obvious significant systolic–diastolic caliber changes at subsequent coronary CT analysis. To better assess possible myocardial ischemic damage, the patient underwent MRI (magnetic resonance), which showed an entirely unexpected result. In fact, T1 sequences revealed biventricular adipose infiltration at the anterolateral and inferolateral walls of the left ventricle, with homosedial enhancement to nonischemic pattern on late post–contrast images: this picture was considered suggestive of biventricular arrhythmogenic cardiomyopathy. In the absence of history of syncope and documented arrhythmias on prolonged rhythm monitoring during hospitalization, no indication was made for further diagnostic and therapeutic investigations from the arrhythmological point of view. Thus, beta blocker and ace inhibitor therapy was set, and the patient was started on an outpatient cardiology follow–up course and genetic counseling for him and first–degree family members was scheduled. Selecting the correct imaging method becomes crucial for a correct diagnosis, especially in cases in which symptomatology is inconclusive.

AbstractAimsParenteral oxycodone is increasingly used worldwide to manage perioperative pain. Oxycodone doses required for adequate analgesia vary significantly between individuals. Our study investigated whether an analgesic plasma concentration could be determined for oxycodone and which factors affect it.Methods1000 women undergoing breast cancer surgery were recruited to the study. Demographic data were collected and their cold and heat pain sensitivity and anxiety scores were measured preoperatively. After surgery, rest and motion pain intensities were measured. Intravenous oxycodone was administered until the patients reported satisfactory pain relief (NRS <4/10). At this point, plasma concentrations of oxycodone and its metabolites were determined. A second plasma sample for oxycodone deter-mination was taken when the patient requested a new dose of oxycodone. Genomic DNA was extracted from whole blood samples and the patients were genotyped for CYP2D6, CYP3A4 and CYP3A5 variants.ResultsThe two oxycodone concentrations showed a strong correlation (r =0.84). The pain intensity measured during motion before oxycodone dosing correlated significantly with the plasma oxycodone concentration (geometric mean 35.3 ng/ml and CV % 66.4) required to achieve satisfactory analgesia (r = 0.38, p = 1.5 x 10-33). The most important factors associating with postoperative pain intensity were type of surgery (breast conserving or mastectomy with or without axillary clearance) and the age of the patient. Older patients reported lower pain scores and required smaller oxycodone concentrations for satisfactory analgesia. CYP2D6, CYP3A5 or CYP3A4 genotypes did not significantly affect the oxycodone concentrations, but CYP2D6 genotype significantly affected the formation of the metabolites oxymorphone and noroxymorphone. CYP3A4 and CYP3A5 genotypes did not affect the metabolite formation.ConclusionsOur results indicate that the more pain the patient experiences postoperatively the greater her minimum plasma oxycodone concentration must be to achieve satisfactory analgesia. Type of surgery and age significantly affect postoperative pain intensity.

Abstract The evaluation of thrombin generation during blood coagulation has become the preferred clinically relevant approach to assess the global integrity of this complex process. In order to give the full picture of the physiology of this multifactorial reaction, all cellular and soluble plasma participants to this reaction have to be present in their natural whole blood environment. Whole blood thrombin generation tests are best suited for this purpose but are somewhat cumbersome for use in real time clinical practice. Thrombelastography has long been recognized as a clinically useful technology for assessment of whole blood coagulation. The purpose of this study was to evaluate the possibility of linking the tracing generated by clotting of whole blood in a Thrombelastograph® (TEG®) Hemostasis System with the generation of thrombin as assessed by thrombin/antithrombin complex (TAT). 320μL of citrated whole blood (3.2% buffered citrate and 1000μg/ml of corn trypsin inhibitor; 1 part to 9 parts of blood) from volunteers was clotted in the presence of 20μL CaCl2 (200mM) and 20μL of Innovin (1/100 diluted in PBS/albumin 4%, pH 7.4). Clotting was monitored with 8 channels of the TEG® system. At different time points (4 to 40 min) the whole blood TEG® reaction cups were put in 2640μL of cold quenching solution (EDTA 50mM, L-benzamidine 10mM, NaCl 150mM, HEPES 2mM, pH 7.4, and 10μL of 10mM D-Phe-Pro-Arg CH2Cl chloromethylketone diluted in 0.01M HCl), vortexed, and centrifuged, and the supernatants were kept at −80°C until assayed for TAT by ELISA. Total Thrombus Generation (TTG) was calculated from the first derivative of the TEG® waveform and was compared to the thrombin generation as measured by TAT as described above. The two vectors of values - the TAT thrombin generation data and the corresponding TEG® TTG - were analyzed using Pearson correlation coefficients and linear, non-linear, and natural log (Ln) transformation of TAT values for least-squares goodness-of-fit curves. The best least-squares fit is the exponential curve (Fig. 1) and its corresponding natural log transformation of TAT value as shown by Fig. 2, with R2 = 0.8787 (P &lt; 0.0001). The prediction equation for TAT thrombin generation is y = 3122.7e0.0005x, where x is the TEG® TTG variable (P &lt; 0.0001). Linearizing using the natural log of the TAT thrombin generation variable (Fig. 2), produces the same R2 as the exponential curve. The prediction equation is y = 8.0465 + 0.0005x, significant at P &lt; 0.0001, where y is the TAT thrombin generation in the Ln transformation variable and x is the TEG® TTG variable. The high magnitude of R2 and the high significance of the prediction equation demonstrate the high efficacy of the prediction of TAT thrombin generation using the TEG® TTG.

AbstractMars has surely been scrutinised since the dawn of humankind. Its appearance every couple of years like a drop of blood in the sky led to warlike attributes in the ancient world. In the 16th century Tycho Brahe made accurate observations of the position of Mars that enabled Johannes Kepler to obtain his first two laws of planetary motion. These in turn were explained by Newton's laws of motion and gravity. In the 17th century the first telescope observations were made, but Mars is small and very little surface detail could be discerned.Throughout the 18th and 19th centuries telescopes improved, revealing many dark areas on the red tinted surface. During the close opposition of 1877 sufficient detail could be seen that enabled Giovanni Schiaparelli to announce that he could see about 40 canali on Mars. This led to the saga of the canals of Mars, finally laid to rest in 1971 when Mariner 9 made observations from Martian orbit showing that the canali/canals do not exist.Belief that there was life on Mars was widespread in the 19th century. However, the majority of astronomers never believed in Martian intelligence. Least controversial was the view that the dark areas were some form of plant life. This view persisted until Mariner 4 flew past Mars in 1965 and discovered a far thinner atmosphere than previously thought. This was a low point, with impact craters dominating the images. It was Mariner 9 that revealed much more promising landscapes, including volcanic features, and others indicating that water had flowed across the surface, particularly when Mars was young. Thus, the contemporary era of Mars exploration began.Our picture of Mars today is not only much more complete than that before Mariner 4, in several ways it is quite different. The belief, however, that there might be life on Mars persists – subsurface life cannot be ruled out and, failing that, there might be ancient fossils on Mars.

Аннотация: В статье впервые анализируются цветообозначения в мифологическом дискурсе алтайцев. На материале фольклорных текстов — мифов, легенд, сказаний, шаманских текстов — выявлены характерные для мифов базовые традиционные цветообозначения. Цветообозначения рассматриваются как образные признаки мифологических концептов, в семантике которых лежат древние архетипы, возникшие в языке в результате ассоциативно-метафорического переноса. The paper first considers the specifics of color designations in the Altai mythological discourse. The factual material of the research was published and unpublished folklore texts, in particular, myths, legends, tales, shamanistic campaigns in the Altai language. The color code is important in the conceptualization of the mythological picture of Altaians. The national-cultural specificity of color designations in the mythological discourse is due to their compatibility with concepts-nominations, designating various objects and phenomena of the invisible world. Color designations can be the first component of naming the various names of supernatural beings, deities, spirits, or their clothes. The basic colors revealed in the traditional mythological picture of Altaians — ak “white”, kara “black”, kyzyl “red”, sary “yellow”, kÿreң “brown”, kök “blue” and jazhyl “green” — in the conceptualization of mythological, mythical images, objects of the unreal (invisible) world on the basis of their deepest archetypal signs that arose as a result of associative-metaphorical transfer. In combination with mythological concepts related to the Upper World, white is a sign of divine purity, holiness, blue is a sign of heaven, green is a sign of greenery and lighted sky, red and yellow are a sign of the shining sun. In combination with the mythological images of the Lower World, black as the antipode of white symbolizes darkness, death, red symbolizes blood, hellish hell, yellow means withering, disease, evil, blue means cold, ice, water, green — dim light in darkness. When mythological archetypes were refracted into the system of folk ethics, the transformation of the signs “light — dark” into the signs “good — evil” occurred. In addition to the color code, the zoomorphic code of the Altai culture is used in the conceptualization and structuring of the mythological other world, in the modeling of its geographical landscape and the conceptualization of the world.

Abstract Study Design This is a retrospective cohort study. Objective The authors report surgical outcome in a series of very young children who underwent finger replantation after traumatic amputation. Methods During a 10-year period, 65 children were treated with replantation for finger amputation in two institutions. This study focused on replantation of 15 fingers in 13 young patients under 6 years of age (mean age: 2.9 years; range: 1.1–5.7 years). Early postoperative complications were categorized into major or minor. At the time of assessment, the authors evaluated everyday life activities, pain and cold tolerance, total active range of motion (TAM) in patients with successful replantation, and growth disturbance. Results The overall success rate for children younger than 6 years was 47% (7 out of 15), and the authors had 67% of major complications, mainly in patients with crush injuries. There was venous ischemia in 13 (86%) fingers treated with controlled bleeding. The hemoglobin level decreased more than 2 g/dL in six patients, and blood transfusion was necessary in two patients. At the last follow-up, patients with successful replantation had a mean TAM of 72%. Conclusion Despite numerous complications mainly in relation with venous congestion, the functional outcome is satisfactory after successful replantation in young children, which should always be attempted.
