Useless and noise information occupies large amount of big data, which increases our difficulty to extract worthy information. Therefore outlier detection attracts much attention recently, but if two points are far from other points but are relatively close to each other, they are less likely to be detected as outliers because of their adjacency to each other. In this situation, outliers are hidden by each other. In this paper, we propose a new perspective of hidden outlier. Experimental results show that it is more accurate than existing distance-based definitions of outliers. Accordingly, we exploit a candidate set based hidden outlier detection (HOD) algorithm. HOD algorithm achieves higher accuracy with comparable running time. Further, we develop an index based HOD (iHOD) algorithm to get higher detection speed.

The relationship between egg consumption and ischemic heart disease (IHD) remains controversial as there is still no clear answer regarding the relationship, with research limited to a few geographical regions. In the current study, we conducted a longitudinal analysis of the association between egg intake and IHD incidence (IHDi) and mortality (IHDd) using 28 years of international data from 1990 to 2018. Egg intake (g/day/capita) by country was obtained from the Global Dietary Database. Age-standard IHDi and IHDd rates per 100,000 subjects in each country were obtained from the 2019 Global Burden of Disease database. The analysis included a total of 142 countries with populations of at least one million, for which all data were available from 1990 to 2018. Eggs are consumed worldwide, and regional differences in consumption are also shown. Utilizing IHDi and IHDd as objective variables and egg intake as an explanatory variable, the analysis was conducted using linear mixed models, which controlled for inter- and intra-country variation from year to year. The results showed a significant negative association between egg intake, and IHDi (−0.253 ± 0.117, p < 0.05) and IHDd (−0.359 ± 0.137, p < 0.05). The analysis was carried out using R 4.0.5. The results suggest that adequate egg intake might suppress IHDi and IHDd on a global scale.

The intestinal mucosa forms the first line of defense against infections mediated by enteric pathogens such as salmonellae. Here we exploited intestinal “organoids” (iHOs) generated from human induced pluripotent stem cells (hIPSCs) to explore the interaction ofSalmonella entericaserovar Typhimurium with iHOs. Imaging and RNA sequencing were used to analyze these interactions, and clear changes in transcriptional signatures were detected, including altered patterns of cytokine expression after the exposure of iHOs to bacteria.S. Typhimurium microinjected into the lumen of iHOs was able to invade the epithelial barrier, with many bacteria residing withinSalmonella-containing vacuoles. AnS. TyphimuriuminvAmutant defective in theSalmonellapathogenicity island 1 invasion apparatus was less capable of invading the iHO epithelium. Hence, we provide evidence that hIPSC-derived organoids are a promising model of the intestinal epithelium for assessing interactions with enteric pathogens.

CONCLUSIÓN: El nivel del Índice de Higiene Oral Simplificado comunitario en la parroquia es el 29% nivel excelente, 65% nivel bueno y un 6% nivel regular, mientras que el nivel de IHOS de acuerdo al sexo no existe diferencia significativa presentándose un mayor porcentaje el nivel bueno con el 65% en sexo masculino y 64% en el sexo masculino; el promedio de Placa Blanda en la parroquia es de 0,45; mientras el promedio de Placa Calcifica da en la parroquia es del 0,02

The paper examines the literary technique of Ihor Kostetskyi and the innovative forms and content of his prose from the late 1940s. It focuses on the author’s attempts to update the genre nature of a short story. The analysis covers the principles of creating a figurative system, the structure of a prose narrative, the use of cinematic constructions, the ‘stream of consciousness’ technique, the situation of a lacuna, ‘text within a text’, etc. The research involved the works included in the collection “Stories about the Winners”, as well as the short stories “You Own the Whole World”, “Before the Upcoming Day”, “Divine Lies”, and samples of their critical reception (in particular, the studies by Victor Petrov, Volodymyr Derzhavyn, Yurii Sherekh, and others). Kostetskyi’s collection of short prose “The Story of the Winners” is regarded as a kind of laboratory in which the writer explored the possibilities of language and literary techniques, experimenting with the concepts of genre and image. It is argued that the writer sought to create a technical picture of the world that has nothing to do with the physical world, and the images he created are signs and symbols that need to be deciphered. In the short story “You Own the Whole World”, which is perceived as a “telegraphic mini-version of a psychological novel” or a “mini-novel with a mini-prologue”, one can find the ‘stream of consciousness’ technique and the situation of a gap, that is, a gap between the story of traumatic event and reality when the reader does not receive an unambiguous answer to the question of where the event occurs and whether it occurs at all. The compositional technique of ‘text within a text’, tested by Kostetskyi in the short story “Before the Upcoming Day”, performs the function of a hidden ‘message’ and allows inserting ‘internal’ actions and dialogues into ‘external’ ones. In general, Kostetskyi’s works appeal to the subconscious, enhanced associativity and imagination, and the play factor is important in them as one of the elements of the author’s poetics.

There has been significant growth of interest in both international human resource development (IHRD) and ecosystems research. Both literatures highlight important characteristics of each concept; however, to date, they have not yet been linked. We propose an ecosystem perspective as an important framework to understand IHRD. Ecosystems emphasize interdependencies, actor centrality, bargaining power, and relationships between actors as important in shaping IHRD. We utilize a meta-synthesis of the IHRD literature to identify content and process dimensions of an IHRD ecosystem. We conclude with a discussion of implications for IHRD research.

Objetivo: Determinar la prevalencia de enfermedades bucodentales en niños de 6, 12 y 15 años en Islay, Arequipa. Materiales y métodos: Estudio observacional, descriptivo y transversal. La muestra estuvo conformada por 360 estudiantes de 6, 12 y 15 años, de ambos sexos, que cumplieron los criterios de inclusión. El nivel de confianza se determinó en un 95% y el margen de error en un 5%. Para evaluar las patologías orales, se utilizó los índices de IHO-S, IHOS-M, IPC-M, CPOD, ceod, el de la OMS y el de Dean. Los estudiantes fueron examinados visual y clínicamente. Resultados: Con referencia al IHO-S: buena higiene,72,5% en deciduos, 63,3% en permanentes; regular higiene, 27,5% en deciduos, 35,0% en permanentes; mala higiene, 1,7% en permanentes, existe diferencia significativa en cuanto a la edad. De acuerdo con el IPC-M, el 81,4% padece de gingivitis; el 57,5%, periodontitis; el 4,2%, periodontitis severa, y hay diferencias significativas según la edad. Las maloclusiones: oclusión normal, 38,9%; maloclusión leve, 38,6%, y moderada o severa, 22,5%; el 61,1% padece de maloclusiones. Respecto de la fluorosis, el 34,7% no presentó lesión; fluorosis cuestionable, 15%; fluorosis muy leve, 30,3%; fluorosis leve, 11,7%; fluorosis moderada, 1,1%; la prevalencia fue del 58,1%. Conclusiones: La prevalencia de enfermedades bucodentales es elevada y el riesgo de caries dental es alto. Se observa una regular y mala higiene en la dentición permanente. El porcentaje de gingivitis es alto. Las maloclusiones están en el rango de leve a moderado. La fluorosis dental está en un rango de muy leve.

Background: The International Hip Outcome Tool (iHOT-33), which is a self-administered questionnaire that contains 33 questions, was developed to evaluate young to middle-aged physically active adults with hip pathology. Purpose: To translate and cross-culturally adapt the iHOT-33 to create a Thai version of the iHOT-33 (TH-iHOT-33) and to determine the validity and reliability of the TH-iHOT-33. Study Design: Cohort study (diagnosis); Level of evidence, 3. Methods: Translation and cross-cultural adaptation of the iHOT-33 were performed according to international guidelines. Patients aged 18 to 60 years with hip disorders who had a score ≥4 on a modified version of the Tegner activity scale completed the TH-iHOT-33 twice, 2 weeks apart. The Thai versions of the HOOS (Hip disability and Osteoarthritis Outcome Score) and EQ-5D-5L (European Quality of Life–5 Dimensions–5 Levels) were also completed at baseline. At 2 weeks, the Global Perceived Effect scale was completed in addition to the TH-iHOT-33. The statistical testing for validity included construct validity and hypotheses testing. Internal consistency, test-retest reliability, and measurement error were analyzed to determine reliability. Results: The iHOT-33 was successfully translated and cross-culturally adapted to create the TH-iHOT-33. Seventy-two patients with various hip disorders participated in this study. Of the predefined hypotheses used for construct validity, 86% were confirmed. A Cronbach alpha of 0.98 (95% CI, 0.97-0.99) demonstrated excellent internal consistency. Test-retest reliability was high, with an intraclass correlation coefficient of 0.83 (95% CI, 0.73-0.90). The standard error of measurement was 5.1 points. The smallest detectable change was 14.2 points at the individual level and 1.9 points at the group level. The minimal important change was 10.9 points. The TH-iHOT-33 had no observable floor or ceiling effects. Conclusion: The TH-iHOT-33 is a valid and reliable instrument for assessing the functional status and health-related quality of life of young to middle-aged physically active Thai adults with hip disorders.

Background: The International Hip Outcome Tool–33 (iHOT-33) was developed to evaluate patients seeking surgery for hip and/or groin (hip/groin) pain and may not be appropriate for those seeking nonsurgical treatment. Purpose: To evaluate the psychometric properties of the iHOT-33 total (iHOT-Total) score and all subscale scores in adults with hip/groin pain who were not seeking surgery. Study Design: Cohort study (diagnosis); Level of evidence, 3. Methods: Patients with hip/groin pain who were not seeking surgery were recruited from 2 ongoing studies in Australia. Semistructured one-on-one interviews assessed content validity. Construct validity was assessed by testing hypothesized correlations between iHOT-33 and Copenhagen Hip and Groin Outcome Score (HAGOS) subscale scores. Test-retest reliability was assessed in patients not undertaking treatment and who reported “no change” in their Global Rating of Change (GROC) score at 6-month follow-up. Scores were reliable at group and individual levels if intraclass correlation coefficients (ICCs) were ≥0.80 and ≥0.90, respectively. Scores were responsive if Spearman rank correlations (ρ) between the change in the iHOT-33 score and the GROC score were ≥0.40. Results: In total, 278 patients with hip/groin pain (93 women; mean age, 31 years) and 55 pain-free control participants (14 women; mean age, 29 years) were recruited. The iHOT-33 demonstrated acceptable content validity. Construct validity was acceptable, with all hypothesized strong positive correlations between iHOT-33 and HAGOS subscale scores confirmed ( r range, 0.60-0.76; P < .001), except for one correlation between the iHOT-Sport and HAGOS-Sport ( r = .058; P < .001). All scores were reliable at the group level, except for the iHOT-33 job subscale (iHOT-Job) (ICC range, 0.78-0.88 [95% CI, 0.60-0.93]). None of the subscales met the criteria for adequate reliability for use at the individual level (all ICCs <0.90). Minimal detectable change values (group level) ranged from 2.3 to 3.7 (95% CI, 1.7-5.0). All iHOT-33 subscale scores were responsive (ρ range, 0.40-0.58; P≤ .001), except for the iHOT-Job in patients not undertaking treatment (ρ = 0.27; P = .001). Conclusion: All iHOT-33 subscale scores were valid for use in patients with hip/groin pain who were not seeking surgery. Acceptable test-retest reliability was found for all subscale scores at the group level, except the iHOT-Job. All subscale scores, excluding the iHOT-Job, were responsive, regardless of undertaking physical therapist–led treatment or no treatment.

The purpose of this paper. The purpose of the study involves a thorough study on the genre system of choral heritage I. Shamo, as understanding the genre essence of choral work is an essential component of performing interpretation. The methodology. Methodological principles are the principle of historicism, historical-contextual method, genre-style analysis and acmeological method in musicology, used to understand the top achievements of I. Shamo in the context of the evolution of artistic thinking of the artist. The results. The genre system of I. Shamo’s choral heritage consists of the genres of choral song, suite, choral cycle, cantata, cantata-poem, cantata-oratorio, choral opera a cappella, oratorio (concert-performance). The directions of the composer’s creative search in the genre of choral song are connected with the usage of different choral compositions, appeal to the genres of lyrical song, stylization of ritual songs, romanticization of patriotic song. It is determined that the semantics of the first cycle of choirs a cappella by I. Shamo in the poem by I. Franko reveals the theme of the cycle of the seasons, drawing a parallel with the stages of human life. For the choral cycle “Flying Cranes”, the name of which is a symbol of unattainable happiness and, at the same time hope, the composer improves the techniques of sound recording, creates expansive melodic lines based on a synthesis of folk and romantic intonations. The third cycle of I. Shamo “10 choirs on the poems of Ukrainian poets” demonstrates the latest achievements of the artist in choral writing, timbre drama, the implementation of the principles of working with folk poetic sources, characteristic of the “second wave of folklore”. It is substantiated that the unity of the “Carpathian Suite” of the composer is promoted by: tonal plan, contrasting tempo drama and artistic sound images, intonational unity based on the theme of Ukrainian folk models. Based on the author’s genre definitions of the “Yatran Games”, it is determined that in I. Shamo’s choral opera a cappella there is a synthesis of genres: suite, choral scenes originated from folk life and ceremonial action under the auspices of the opera genre. It was found that among the genres of orchestral and choral compositions of the artist — cantata, cantata-poem, cantata-oratorio, which is dominated by civic themes. Innovative is the work of I. Shamo “Skomoroshyny”, which synthesizes the genres of oratorio and concert, with theatricalization of these genres. The choral opuses showed such features of the artist’s style as the synthesis of genres, the tendency to cyclicality, reliance on national roots, the creation of author’s melodies, where deep folklore manifestations, sprechstimme, sonorism, aleatorics. The scientific creativity lies in the analysis of the components of the genre system of I. Shamo’s choral heritage, in the awareness of the genre phenomenon of each choral work and in the identification of the evolution of the artist’s style. The practical significance of the obtained results is due to the need to determine the genre of choral work to create a performing interpretation.

BackgroundErosive HOA (EHOA), defined by an erosive radiographic joint and inflammatory HOA (IHOA) defined by the presence of clinical synovitis are frequent in HOA. The association with specific risk factors and a more severe disease, remains still debated.ObjectivesTo identify demographic, disease-related and clinical factors as risk factors for EHOA or IHAO.MethodsClinical and radiographic data were extracted from baseline data of the 6-years prospective longitudinal HOA French cohort DIGICOD [1].Demographic and hand OA clinical and radiological data were compared at baseline between EHOA (≥1 erosive joint in E- or R-phase according to Verbruggen scoring) versus non EHOA, and IHOA (≥ 1 joint with clinical synovitis) vs non IHOA groups.A model of logistic regression analysis was performed for each demographic and clinical HOA-related studied parameter to identify factors associated with EHOA and/or IHOA. Analyses were adjusted for age, sex and body mass index. Results are expressed as Odd Ratios and their 95%Confidence Interval (OR (95%CI)).Results425 patients were analyzed, 195 EHOA (46%), 185 IHOA (43%), with 113 both EHOA and IHOA (27%).The risk of EHOA was reduced by a half in case of current/past professional occupation involving hands (OR 0.6; 95%CI: 0,4; 0.9) and by at least 0.6 (0.3;1.0) if grip strength ≥ 19kg. It was increased by at least 2.0 (1.1; 3.5) if OA duration was ≥6 years, by 2.6 (1.7; 3.9) if aesthetic damage scored ≥40 on a 0-100 mm VAS, by 2.2 (1.4; 3.5) if ≥2 painful joints on pressure, by 2 (1.3; 3,1) if the AUSCAN stiffness subscale scored ≥20 (0-100 VAS), by 2.3 (1,5; 3,5) if the Functional Index for HOA (FIHOA) scored ≥20 (score normalized to 0-100).The risk of IHOA was increased by 2.4 (1.4; 4.0) if OA duration was ≥11 years versus < 6 years, by 1.8 (1.1; 3.0) if pain at rest VAS was ≥40 and by 2.1 (1.4; 3.2) if pain on activity was ≥40, by 2.5 (1.6; 3.8) if aesthetic damage scored ≥40, by 2.5 (1.5; 4.1) if ≥2 painful joints on pressure and by 1.9 (1.2; 2.9) if ≥2 spontaneous painful joints, by 1.9 (1.2; 3.0) if AUSCAN stiffness subscale ≥40, by 1.8 (1.2; 2.7) if the FIHOA was ≥20.Structural damage was higher in both EHOA and IHOA than in those without EHOA and IHOA, respectively: the mean ±standard deviation number of joints at a Kellgren-Lawrence grade ≥2 was 18±5 in erosive vs 13±6 in non-erosive patients and 18±6 vs 13±6 in IHOA vs non IHOA.ConclusionThere was a high prevalence of EHOA and IHOA in our cohort, partly due to a tertiary-care recruitment. Structural damage was more severe in both phenotypes than in those without erosive or inflammatory signs.There was an inverse correlation between hand use and EHOA, confirming that EHOA is rather linked to systemic factors.Risk factors for both EHOA and IHOA were a longer disease duration, a higher aesthetic damage (notably in IHOA), number of painful joints on pressure, stiffness, and functional impairment (FIHOA). BackgroundIntegrated health service delivery (IHSD) is a promising approach to improve health system resilience. However, there is a lack of evidence specific to the low/lower-middle-income country (L-LMIC) health systems on how IHSD is used during disease outbreaks. This scoping review aimed to synthesise the emerging evidence on IHSD approaches adopted in L-LMIC during the COVID-19 pandemic and systematically collate their operational features.MethodsA systematic scoping review of peer-reviewed literature, published in English between 1 December 2019 and 12 June 2020, from seven electronic databases was conducted to explore the evidence of IHSD implemented in L-LMICs during the COVID-19 pandemic. Data were systematically charted, and key features of IHSD systems were presented according to the postulated research questions of the review.ResultsThe literature search retrieved 1487 published articles from which 18 articles met the inclusion criteria and included in this review. Service delivery, health workforce, medicine and technologies were the three most frequently integrated health system building blocks during the COVID-19 pandemic. While responding to COVID-19, the L-LMICs principally implemented the IHSD system via systematic horizontal integration, led by specific policy measures. The government’s stewardship, along with the decentralised decision-making capacity of local institutions and multisectoral collaboration, was the critical facilitator for IHSD. Simultaneously, fragmented service delivery structures, fragile supply chain, inadequate diagnostic capacity and insufficient workforce were key barriers towards integration.ConclusionA wide array of context-specific IHSD approaches were operationalised in L-LMICs during the early phase of the COVID-19 pandemic. Emerging recommendations emphasise the importance of coordination and integration across building blocks and levels of the health system, supported by a responsive governance structure and stakeholder engagement strategies. Future reviews can revisit this emerging evidence base at subsequent phases of COVID-19 response and recovery in L-LMICs to understand how the approaches highlighted here evolve.
